ABSTRACT

Se presenta un caso femenino de dengue clásico (DC) en el marco de la epidemia 2023-2024 en la provincia de Misiones, con predominio de síntomas dermatológicos de exantemas máculo papulosos, habonosos y eritrodérmicos sobre los síntomas sindrómicos cardinales. Las lesiones presentan componente humoral y de extravasación, sin diátesis ni componentes purpúricos apreciables, presentando una rápida y efectiva evolución al eritema y la normalización con tratamiento antihistamínico y corticoide parenteral. De la misma manera se evalúan alteraciones analíticas hematológicas y hepáticas de gran magnitud, con escasa repercusión clínica, que se mensuran en función del riesgo relativo al dengue hemorrágico (DH) y el pronóstico de la paciente. A female case of classic dengue (DC) is presented in the context of the 2023-2024 epidemic in the province of Misiones, with a predominance of dermatologic symptoms of maculopapular, hives, and erythrodermic rashes overlapping the cardinal syndromic symptoms. The lesions have a humoral and extravasation component, without any significant diathesis or purpuric components, showing rapid and effective progression to erythema and normalization with antihistamine and parenteral corticosteroid treatment. Similarly, hematologic and hepatic analytical alterations of great magnitude are evaluated, with little clinical impact, measured in terms of relative risk for hemorrhagic dengue (HD) and the prognosis of the patient. ABSTRACT

Los tumores cutáneos presentan una alta prevalencia en dermatología en el mundo, siendo los benignos más frecuentes que los malignos; sin embargo, estos últimos son más estudiados debido a su morbimortalidad. El objetivo principal de este estudio fue conocer los principales diagnósticos y técnicas quirúrgicas correspondientes desarrolladas en el Centro de Tratamiento de Enfermedades de la Piel (CETEP) entre 1996 y 2019, evaluando aspectos clínicos y demográficos. Se realizó un estudio retrospectivo y observacional que incluyó todo paciente con lesión cutánea y posterior resolución quirúrgica de la misma. En la muestra analizada (N 6.659) hubo un predominio del sexo femenino (68%) y la media de edad fue 53 ± 21 años. Los pacientes residían mayoritariamente en Montevideo (58%). Los tumores benignos fueron los más frecuentes (41%), seguidos de los malignos (28%), dentro de éstos: carcinoma basocelular (CBC) 66%, carcinoma espinocelular (CEC) 21% y melanoma (MM) 5%. Las técnicas quirúrgicas realizadas fueron cirugías convencionales (57%), principalmente losange (93%), seguidas de procedimientos de cirugía dermatológica (42%), predominando biopsias (52%) y afeitado con electrocoagulación (23%). Se destaca que el CETEP resolvió un número mayor de pacientes de centros externos que del propio Centro Hospitalario Pereira Rossell (CHPR): 59% no CHPR vs 41% CHPR. En conclusión, este trabajo proporcionó información nacional sobre la epidemiología de distintos tumores cutáneos, así como las técnicas quirúrgicas más utilizadas en su resolución. Además, estableció la importancia de la cirugía dermatológica y la capacidad del CETEP en dar respuesta a pacientes propios tanto como referenciados desde otros centros del sistema público.


Cutaneous tumors have a high prevalence in dermatology worldwide, with benign tumors being more common than malignant ones. Nevertheless, the latter are more extensively studied due to their associated morbidity and mortality. The main objective of this study was to identify the primary diagnoses and corresponding surgical techniques developed at the Center for the Treatment of Skin Diseases (CETEP) between 1996 and 2019, while assessing clinical and demographic aspects. A retrospective, observational study was conducted, including all patients with cutaneous lesions and subsequent surgical resolution of the same. In the analyzed sample (N 6659), there was a predominance of females (68%), and the mean age was 53 ± 21 years. The majority of patients resided in Montevideo (58%). Benign tumors were the most prevalent (41%), followed by malignant tumors (28%), with the latter comprising basal cell carcinoma (BCC) at 66%, squamous cell carcinoma (SCC) at 21%, and melanoma (MM) at 5%. The performed surgical techniques included conventional surgeries (57%), primarily using the lozenge method (93%), followed by dermatologic surgery procedures (42%), with a predominance of biopsies (52%) and shave excision with electrocoagulation (23%). It is noteworthy that CETEP resolved a greater number of patients from external centers than from its own hospital, Pereira Rossell Hospital Center (CHPR). 59% non-CHPR vs. 41% CHPR. In conclusion, this study provided national information on the epidemiology of various cutaneous tumors, as well as the most commonly employed surgical techniques in their resolution Furthermore, it emphasized the importance of dermatologic surgery and highlighted the capacity of CETEP to respond to both its own patients and those referred from other centers within the public healthcare system.


Os tumores da pele apresentam alta prevalência na dermatologia em todo mundo, sendo os tumores benignos mais frequentes que os malignos, porém estes últimos são mais estudados devido à sua morbidade e mortalidade. O objetivo principal deste estudo foi conhecer os principais diagnósticos e correspondentes técnicas cirúrgicas desenvolvidas no Centro de Tratamento de Doenças da Pele (CETEP) no período 1996-2019, avaliando aspectos clínicos e demográficos. Foi realizado um estudo retrospectivo e observacional, que incluiu todos os pacientes com lesões cutâneas e com posterior tratamento cirúrgico. Foram estudados 659 pacientes com predomínio do sexo feminino (68%) e média de idade de 53 ± 21 anos. A maioria dos pacientes residiam em Montevidéu (58%). Os tumores benignos foram os mais frequentes (41%), seguidos dos tumores malignos (28%), entre estes: carcinoma basocelular (CBC) 66%, carcinoma espinocelular (CEC) 21% e melanoma (MM) 5%. As técnicas cirúrgicas realizadas foram cirurgias convencionais (57%), principalmente em forma de cunha (93%), seguidas de procedimentos cirúrgicos dermatológicos (42%), predominando biópsias (52%) e shaving com eletrocoagulação (23%). Destaca-se que o CETEP atendeu um número maior de pacientes de centros externos do que do próprio Centro Hospitalar Pereira Rossell (CHPR): 59% não-CHPR vs. 41% CHPR. Concluindo, este trabalho forneceu informações sobre a epidemiologia dos diferentes tumores de pele no país, bem como as técnicas cirúrgicas mais utilizadas no seu tratamento. Além disso, estabeleceu a importância da cirurgia dermatológica e a capacidade do CETEP de atender os pacientes do hospital e também os que foram encaminhados de outros centros da rede pública.

ABSTRACT

Vaccines against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the causative agent of the 2019 coronavirus disease (COVID-19) pandemic, have been rapidly developed and authorized. However, re-cent studies showed that COVID-19 vaccination is associated with a wide range of dermatological reactions. The most prevalent adverse dermatological reaction observed in observational studies was a delayed large lo-cal reaction (DLRs), which is characterized by the appearance of an erythematous and edematous patch at the injection site four days or more after vaccination. Most of these reactions are common in females and resolve spontaneously within a few days to a weak. The second dose of the vaccine was associated with a higher inci-dence of cutaneous reactions compared to the first dose but milder in intensity. It seems that the Moderna vaccine is associated with a higher incidence of these adverse events compared to the Pfizer vaccine. Fur-thermore, mRNA vaccines had a higher incidence than vector-based and inactivated vaccines. There is a lack of evidence regarding the side events of the Johnson & Janssen vaccine. Further long-term, multicenter studies are required to compare these vaccines and highlight the best practice in managing these reactions.

Introduction: The management of medical images has been gaining followers based on the advantages it offers for the diagnosis of diseases, which, like COVID-19, present with clinical manifestations that can be captured in the form of images. Objective: Take advantage of the quasi-periodicity of the principal components (PCs) in the decomposition into PCs of medical images, which represent dermatological manifestations in paucisymptomatic patients of COVID-19. Methods: Here, a set of photos was taken of one of the most frequent patterns in COVID-19, the maculopapular pattern, characterized by an erythmatopapular rash, and compression of one of the medical images was performed. Said compression was carried out in different ways: (1) using two PCs, (2) using both a periodic PC and a non-periodic PC, (3) using two periodic PCs, (4) using a single PC, and (5) using a single periodic PC. Result: The results of this research proved that it is possible to work with acceptable reconstructions of compressed images in the field of dermatology, without losing the quality and characteristics that allow to reach a correct diagnosis. In addition, this achievement permits to correctly classify many diseases without fear of being wrong. Conclusion: With the method presented, the use of a robust medical image compression technique that could be very useful in the field of health is proposed. The images allow the diagnosis of diseases such as COVID-19 in paucisymptomatic patients, understanding them allows minimizing their weight without losing quality, which facilitates their use and storage.


Introducción: El empleo de imágenes médicas en el diagnóstico de enfermedades ha ido ganando adeptos. Un ejemplo es la COVID-19 que cursa con manifestaciones clínicas dermatológicas. Objetivo: Aprovechar la cuasi-periodicidad de los componentes principales de la descomposición en imágenes médicas, que representan manifestaciones dermatológicas en pacientes paucisintomáticos de COVID-19. Métodos: Se tomó un conjunto de fotografías de uno de los patrones más frecuentes en la COVID-19 (el patrón maculopapular), caracterizado por un exantema eritematopapular, y se realizó la compresión de una de las imágenes médicas. Dicha compresión se realizó de diferentes formas: (1) usando dos componentes principales, (2) usando tanto un componente principal periódico como no periódico, (3) dos componentes principales periódicos, (4) un único componente principal, y (5) un solo componente principal periódico. Resultados: Es posible trabajar con reconstrucciones aceptables de imágenes comprimidas en el campo de la dermatología, sin perder la calidad y características que permitan llegar a un diagnóstico correcto. Además, este logro permite clasificar correctamente muchas enfermedades sin miedo a equivocarse. Conclusiones: Con el método presentado se propone el uso de una técnica robusta de compresión de imágenes médicas que podría ser de gran utilidad en el campo de la salud. Las imágenes permiten el diagnóstico de enfermedades como la COVID-19 en pacientes paucisintomáticos, con cuya compresión se minimiza su peso sin perder la calidad, lo que facilita su uso y almacenamiento.

ABSTRACT

Las enfermedades cutáneas son una amplia gama de afecciones que afectan la piel, de origen bacteriano, viral, fúngica, reacción alérgica, cáncer, parásito, hereditarias, ideopáticas; por sus sintomatologías pueden generar estigma. Por tal motivo, la Organización Mundial para la Salud, alienta a los Estados miembros a que se involucren en actividades de promoción para aumentar la concienciación con el fin de combatir la discriminación que perciben los pacientes con enfermedades dermatológicas estigmatizantes (EDE), tanto por la comunidad y en los establecimientos de salud, pudiendo repercutir negativamente en la adherencia del tratamiento. El objetivo de este estudio fue valorar el uso de endomarketing para la adherencia al tratamiento de EDE. Se realizó una investigación cuantitativa. Se les aplicó encuesta de endomarketing a 279 participantes; además con 147 pacientes con EDE se indago la percepción de actitudes estigmatizantes. Los resultados mostraron un alcance sobre el endomarketing de 0,62 en personal de atención directa y 0,45 para atención indirecta; los pacientes con mayor percepción de actitudes estigmatizantes fueron los que presentaban afecciones en piel tipo infecciosas, mayormente proveniente de la atención indirecta. Se infiere que el conocimiento sobre las EDE disminuye las expresiones de rechazos. Como alternativas para la formación del personal con una mayor satisfacción en el trabajo, competencia para gestionar y elevado sentido de espiritualidad corporativa, que garantice atención humanitaria a estos pacientes, se sugiere el uso de endomarketing, para conciliar los objetivos e intereses del recurso humano interno a las necesidades y expectativas de los pacientes con EDE(AU)


Skin diseases are a wide range of conditions that affect the skin, of bacterial, viral, fungal origin, allergic reaction, cancer, parasite, hereditary, idiopathic; due to their symptoms they can generate stigma. For this reason, the World Health Organization encourages Member States to engage in promotional activities to raise awareness in order to combat the discrimination perceived by patients with stigmatizing dermatological diseases (SDD), both for the community and in health establishments, which may have a negative impact on adherence to treatment. The objective of this study was to assess the use of endomarketing for adherence to DTS treatment. A quantitative investigation was carried out. An endomarketing survey was applied to 279 participants; In addition, with 147 patients with EDE, the perception of stigmatizing attitudes was investigated. The results showed a reach on endomarketing of 0.62 in direct attention personnel and 0.45 for indirect attention; The patients with the highest perception of stigmatizing attitudes were those who presented infectious-type skin conditions, mostly from indirect care. It is inferred that knowledge about DTS decreases the expressions of rejections. As alternatives for the training of personnel with greater job satisfaction, competence to manage and a high sense of corporate spirituality, which guarantees humanitarian attention to these patients, the use of endomarketing is suggested, to reconcile the objectives and interests of internal human resources. to the needs and expectations of patients with DTS(AU)

ABSTRACT

Immunotherapy represented by PD-1/PD-L1 inhibitors has become the main treatment of malignant tumors. However, the adverse events caused by immunotherapy can not be ignored. Among them, dermatological immune-related adverse events (irAEs) occur with the highest incidence. Most dermatological irAEs belong to grade Ⅰ-Ⅱ, which does not affect the application of PD-1/PD-L1 inhibitors. The pathogenesis of dermatological irAEs is not fully understood. The most common types of dermatological irAEs are rash, pruritus and vitiligo. The domestic PD-1 inhibitor camrelizumab has unique adverse reactions of reactive cutaneous capillary endothelia proliferation (RCCEP) . It is found that dermatological irAEs can predict the clinical efficacy of PD-1/PD-L1 inhibitors in patients with malignant melanoma and non-small cell lung cancer (NSCLC) , especially RCCEP can be used as a potential biomarker of the efficacy of camrelizumab in the treatment of NSCLC, hepatocarcinoma, and esophageal cancer.

Introduction@#In Mongolia, there is opportunity to replace importing products by using raw materials based on the animal and minerals. Those products can be used for medical purposes.</br> In traditional medicine, yellow marrow has been widely used to treat burns and mechanical wounds, as well as frozen and open wounds. The purpose of this study was to determine the pharmacological action of “CIBO” ointment. We created a model of mechanical wound to evaluate the healing of wounds and its index after applying the ointment contains the above mentioned raw materials. @*Material and methods@#Four different groups of “CIBO” ointment was prepared in the technology sector of the Drug Research Institute. Mebo wound ointment was used for comparison as control. The study was con- ducted in the Pharmacology Sector of the Drug Research Institute on white rats of the WISTAR breed under one feeding regime and one condition (12h / 12h). Mechanical wound injury and exposure to open wounds in experimental rats and its size were measured on the 3<sup>rd</sup>, 7<sup>th</sup>, 14<sup>th</sup>, 21<sup>st</sup>, and 28<sup>th</sup> days of treatment. Wound healing, morphology, structure, size, and its index was quan- tified based on the defects. The ointment were applied once a day during the treatment period of 28 days to cover the wound.@*Result@#Mechanical wound showed that study treatment group healed 0-6.2% on the 3<sup>rd</sup> day, 21.4-35.7% on the 7<sup>th</sup> day, 50-60% on the 14<sup>th</sup> day, and 33.3-50% on the 21<sup>st</sup> day separately compared with the untreated group. On the 28<sup>th</sup> day, all the groups were decreased by 66.6%. The wound index was between from 0.002 to 0.0005 in the all experimental groups. Among experimented groups, the group consists of yellow marrow and Chinese white cinder was showed better performance than other groups.@*Conclusion@#The study group 4 (Yellow marrow + Chinese white cinder) has been showed to have a pharmacological action of rapid healing and regeneration of burn wounds caused by experimental rats. It has also been shown to have pharmacological action to regenerate skin tissue and suppress caused by mechanicals. Inclusion of Chinese white cinder in “CIBO” ointment promotes regener ation and healing of “CIBO” ointment.

Resumen La pythiosis se relaciona comúnmente con ambientes tropicales húmedos y pantanosos. Esta es considerada una enfermedad piogranulomatosa, la cual afecta principalmente en equinos la piel y el tejido subcutáneo. Además, ocurre con mayor frecuencia en los miembros, el rostro y la región abdominal. Se presenta a la clínica veterinaria de la Corporación Universitaria Remington un equino de sexo hembra, pelaje pinto, con una edad de once años; el motivo de consulta de la paciente es una lesión a nivel ventral abdominal, la cual es de tijo granulomatoso ulcerativo y aparentemente necrótico. El animal presenta secreción mucopurulenta activa y continua con varios tractos fistulosos. Asimismo, al lavado de la herida se evidencia la presencia de estructuras compatibles con kunkers. El plan diagnóstico incluye hemograma y química sanguínea básica. Se envía un hisopado de la herida con el fin de evaluar el crecimiento bacteriano en el cultivo y el antibiograma. A nivel terapéutico, se inicia VeterflucinaR (compuesto a base de penicilina G, estreptomicina, flumetasona) y metronidazol a nivel local. Se realizan infiltraciones en la lesión con KenacortR (acetato de triamcinolona); la respuesta al tratamiento instaurado es parcialmente efectiva, con lo cual se decide hacer la remoción quirúrgica de la herida, retirando una estructura con una longitud de 15 cm de largo, 7 cm de ancho y un peso de 300 gramos. El animal responde satisfactoriamente a la cirugía y a la fecha no ha mostrado reincidencia de la patología.


Abstract Pythiosis is commonly associated with humid and swampy tropical environments. It is considered a pyogranulomatous disease, which in equines mainly affects the skin and subcutaneous tissue. It occurs most frequently in the limbs, face and abdominal region. To the veterinary clinic of the Remington University Corporation is presented an equine of female sex, pinto coat, with an age of eleven years. The patient arrives on the occasion of consultation of a lesion present at the ventral abdominal level, which is granulomatous, ulcerative and apparently necrotic. It presents a lot of purulent discharge active and continuous with several fistulous tracts. To the washing of the wound the presence of structures compatible with Kunkers is evidenced. The diagnostic plan includes blood count and basic blood chemistry. A swab of the wound is sent in order to evaluate the bacterial growth in the culture and the antibiogram. At the therapeutic level, treatment starts with Veterflucina® (composed of penicillin G, streptomycin, flumetasone) and metronidazole locally. Infiltrations are performed in the lesion with Kenacort® (triamcinolone acetate). The response to the treatment established is partially effective, which is why it is decided to perform the surgical removal of the wound by removing a structure with a length of 15 cm long, 7 cm wide and weighing 300 grams. The animal responds satisfactorily to surgery and to date it has not shown recurrence of the pathology.

Abstract Plasmacytoid dendritic cells are part of the dendritic cells family and are a relevant link between innate and adaptive immunity. They are the most potent producers of type 1 interferon, generating antiviral response, stimulating macrophages and dendritic cells and inducing activation and migration of natural killer cells. Plasmacytoid dendritic cells also exert a role as antigen-presenting cells, promote T-lymphocyte responses, immunoregulation, plasma cells differentiation and antibody secretion. Even though plasmacytoid dendritic cells are not usually present in normal skin, their presence is detected in healing processes, viral infections, and inflammatory, autoimmune, and neoplastic diseases. In recent years, the presence of plasmacytoid dendritic cells in several dermatological diseases has been described, enhancing their potential role in the pathogenesis of such conditions. Future studies on the role of plasmacytoid dendritic cells in dermatology may lead to new therapeutic targets.

Background@#Atopic dermatitis (AD) is a chronic, recurrent, pruritic inflammatory skin disease that causes significant burden to affected children. Staphylococcus aureus plays a vital role in AD, and its resistance to current topical antibiotics is worrying. This study aims to determine the frequency of Staphylococcus aureus colonisation and its resistance pattern. It further assesses the association between Staphylococcus aureus colonisation and disease severity; as well as its impact on quality of life.@*Methods@#A cross-sectional study was conducted among 153 children with AD. Skin and nasal swabs were collected. Antibiotic sensitivity to penicillin, cefoxitin, erythromycin, methicillin, clindamycin, gentamicin, trimethoprim/sulfamethoxazole, tetracycline, rifampicin, fusidic acid and linezolid were tested. Clinical evaluation was performed using the SCORing Atopic Dermatitis index (SCORAD). Quality of life was assessed with the Dermatological Life Quality Index (DLQI).@*Results@#Twenty-nine patients had positive skin swab results. One patient had methicillin-resistant Staphylococcus aureus isolated from nasal swab. Skin colonisation with Staphylococcus aureus (p=0.03) and DLQI (p<0.01) were significantly associated with disease severity. The resistant rate is highest in penicillin, followed by fusidic acid, tetracycline, and erythromycin.@*Conclusion@#Skin colonisation with Staphylococcus aureus is an indicator of disease severity in children with AD. Patients with severe disease have lower quality of life. Clinicians need to be aware of high resistance rates towards penicillin and fusidic acid and be prudent in the choice of antibiotics. Antiseptic wash can be considered in patients with Staphylococcus aureus colonisation.

ABSTRACT

Las manifestaciones clínicas del SARS-Cov-2 en niños difieren a la de los adultos, con afección respiratoria, gastrointestinal, dermatológica y/o cardiovascular. La mayoría de los niños son asintomáticos o presentan síntomas leves de la infección por COVID-19. Sin embargo, en los últimos meses se ha identificado un pequeño número de niños que desarrollan respuesta inflamatoria sistémica significativa. A continuación, realizamos una revisión sobre las manifestaciones extrapulmonares del SARS-Cov-2


The clinical manifestations of SARS-Cov-2 in children differ from that of adults, with respiratory, gastrointestinal, dermatological and / or cardiovascular conditions. In the meantime it has been shown that the conventional therapy of different diseases by making use of antibiotics and similar antibacterial treatments may disturb the harmony of the Skin Microbiome, resulting in dysbiosis. There are efforts of using “live” or “tyndallized (lysed)” probiotics in order to treat different diseases of the skin. It is also known that amino acids are one of the important key elements of the skin. In this paper, a hypothesis for the utilization of yogurt as an excipient for various topical dermatological products will be proposed. Yogurt contains significant amounts of; Probiotics (starter cultures), Amino Acids, Vitamins, Minerals and various Fatty Acids (saturated, monounsaturated and polyunsaturated). Besides, it has been shown that Antimicrobial Peptides (Bacteriocins) are also present in yogurt. Yogurt could eventually be used as an excipient for the production of various topical dermatological products in order to deliver some of the above-mentioned constituents to the Stratum Corneum (Skin) locally.

ABSTRACT

A pálpebra inferior é uma das regiões mais acometidas pelo carcinoma basocelular (CBC). Apresentamos uma opção de retalho da pele do canto interno com enxerto de mucosa oral para reconstrução palpebral após exérese de CBC acometendo aproximadamente 60% da pálpebra inferior e com comprometimento completo pelo tumor do canalículo lacrimal inferior. A utilização do retalho de transposição de pele do canto interno e enxertia de mucosa oral é segura. Apresentamos uma técnica reproduzível e com boa evolução no pós-operatório. O posicionamento e a movimentação palpebral foram restabelecidos, permitindo a drenagem da lágrima pelo canalículo superior com correção da epífora.


The lower eyelid is one of the sites most affected by basal cell carcinoma (BCC). We present an option of inner corner skin flap with oral mucosa graft for eyelid reconstruction after BCC excision. This tumor affects approximately 60% of the lower eyelid and shows complete lower lacrimal canaliculus involvement. The use of internal corner skin transposition flap and oral mucosa graft is safe. The technique is reproducible and with good postoperative evolution. Positioning and eyelid movement were restored, allowing tear drainage through the upper canaliculus with complete correction of the tears' overflow (epiphora).

ABSTRACT

Background: Acne vulgaris is one of the most common dermatological disorder affecting the pilo sebaceous glands resulting in their blockage or inflammation. Symptoms of acne may include local erythema and tenderness and pain. Medication include benzoyl peroxide, retinoids, salicylic acid, alpha hydroxy acid, nicotinamide, azelaic acid, anti-seborrheic medications, hormonal treatment, anti-androgen medications and keratolytic soaps.Methods: About 166 patients who were treated for acne vulgaris were included into the study after taking the informed consent from the patients. This study was approved by the Institutional Ethics Committee. The demographic details of the patients with regards to their, age sex, weight, BMI were taken. The acne was medically diagnosed, and the grade was assessed.Results: Out of the 166 patients included into the study, 92 (55%) were females while 74 (45%) were males and the mean age was 25.4±1.6 of the patients belonged to the middle class. Of the patients, most of them were adolescents or less than 25 years of age (54.2%). 64 of the patients were between 26-45 years while only 12 were >45 years of age. A total of 521 drugs were prescribed. Of them, 37.6% were through the oral route and 62.4% were topical the most common prescribed drugs were antimicrobials, both topical and oral.Conclusions: The multidrug prescription in most of the cases can be rationalized if a drug monitoring system can be effectively put into place. This would not only reduce the number of drugs per prescription but also the cost of treatment.

Resumen Objetivo: describir la frecuencia de las lesiones dermatológicas en pacientes de la Unidad de Cuidados Intensivos (UCI) de adultos del Hospital de San José y del Hospital Infantil Universitario de San José entre enero y abril de 2012. Pacientes y métodos: se realizó un estudio descriptivo de corte transversal. Se incluyeron pacientes mayores de 18 años con estancia mínima de 72 horas en UCI, ingresados por patología dermatológica o valorados por dermatología por aparición de lesiones en piel durante su estancia. Resultados: se evaluaron 595 pacientes, encontrando lesiones dermatológicas en 30% (182 pacientes). La edad promedio fue de 63.5 años ± 19.1 sin diferencias de género. La admisión a la UCI se debió principalmente a motivos no quirúrgicos con un porcentaje de 57.3% (105 pacientes). Las lesiones cutáneas más frecuentes fueron las lesiones terciarias observada en 91.8% (168 pacientes), seguida por lesiones secundarias, con predominio infeccioso 6% (11 pacientes) y las últimas fueron las primarias con sólo 1.6% (3 pacientes). Conclusiones: las lesiones dermatológicas en los pacientes de UCI son más frecuentes de lo esperado y en algunos casos pueden ser la causa directa del ingreso a la UCI poniendo en riesgo la vida de los pacientes, por lo tanto, consideramos importante un examen exhaustivo de piel en estos pacientes y el trabajo interdisciplinario con dermatología que permita disminuir el riesgo de mortalidad a causa de complicaciones de origen dermatológico. Abstract Objective: to describe the frequency of dermatological lesions in patients of the Intensive Care Unit (ICU) of adults of the Hospital de San José and the Hospital Infantil Universitario de San José between January and April 2012. Patients and Methods: a descriptive cross-sectional study was carried out. Patients older than 18 years with a minimum stay of 72 hours in the ICU admitted for dermatological pathology or assessed by dermatology due to the appearance of skin lesions during their stay, were included. Results: 595 patients were evaluated, finding dermatological lesions in 30% (182 patients). The average age was 63.5 years ± 19.1 without gender differences. Admission to the ICU was mainly due to non-surgical reasons with a percentage of 57.3% (105 patients). The most frequent skin lesions were the tertiary lesions observed in 91.8% (168 patients), followed by secondary lesions, with an infectious predominance of 6% (11 patients) and the last ones were the primary ones with only 1.6% (3 patients). Conclusions: dermatological lesions in ICU patients are more frequent than expected and in some cases may be the direct cause of admission to the ICU putting patients' lives at risk; therefore, we consider important a thorough skin examination in these patients and interdisciplinary work with dermatology to reduce the risk of mortality due to dermatological complications. Background: Skin disorders form 2% of total Out Patient Department consultations worldwide. But no such data is available from India. Studying prescription pattern is a component of medical audit that help prescribers to provide rational and cost-effective medical care. This study is about the prescription pattern of antifungals in this institution and will help in developing local policies for appropriate use of antifungal drugs. The objective was to study the prescription pattern of antifungals in dermatological diseases.Methods: This is a retrospective study conducted from 1st June 2017 to 30th November 2017 in Dermatology Department. Prescriptions included all newly diagnosed patients with cutaneous fungal infection of both sex who attended Dermatology OPD. Factors considered were sociodemographic parameters, number of patients with combination and monotherapy, number of single topical agents and Fixed Dose Combination (FDC) details of prescribing indicators.Results: 1500 prescriptions were analysed; 500 prescriptions were of fungal infection. 40.2% of the patients were in 18-35 years age group. There were a greater number of males (59.6%) than females (40.4%).Conclusions: The most common oral antifungal used was fluconazole. Terbinafine and clotrimazole were the most commonly used topical agents.

ABSTRACT

Background: Dermatological adverse drug reactions (ADRs) are easily detected by patients and that precludes further usage of drugs. So, decided to study the pattern, causative drugs, severity of adverse drug reactions and their causality in tertiary care hospital.Methods: It was prospective non inventional cross sectional study. Patients attending OPD or admitted to IPD of all age group and both gender with suspected dermatological ADRs following drug intake were included and the ADRS were recorded on CDSCO抯 Pharmacovigilance form. Collected data was analyzed for assessment of causality using WHO-UMC scale, for severity by using Modified Hartwig and Siegel. Morphological pattern, drug groups, gender and age distribution was analyzed.Results: 231 dermatological ADRs were recorded and analyzed. Maximum cases were found in 21-30 years age group (74 cases). Dermatological ADRs were found in 143 females and in 88 males. Three major classes of drugs found responsible for causing dermatological ADRs were -oral Antimicrobials-41 (17.75%) and Injectable Antimicrobials-40 (17.32%), NSAID's-40 (17.32%.) and Topical Betnovate-36 (15.58%.). Regarding the type, 95 cases were of maculopapular rashes (41.12%), steroid damaged face in 42 (18.18%) andacute urticaria in 20 (8.65%). In terms of Severity assessment, authors found 23 cases (9.95%) as Mild, 176 cases (76.19%) of moderate severity and 32 cases (13.85%) of Severe category. In terms of causality assessment: 3 cases as Certain, 68 cases as Probable and 160 cases as Possible.Conclusions: From this study, it was found maximum Dermatological ADRs of moderate severity and few cases of causality category as 揅ertain�.

ABSTRACT

BACKGROUND: Drug eruptions are common in hospitalized patients. Rapid and accurate diagnosis is essential but often difficult. OBJECTIVE: This study defined the clinical features and causative drugs among inpatients presenting with drug eruptions. METHODS: We retrospectively analyzed the clinical and laboratory data of inpatients who sought consultations with the Dermatology Department for a diagnosis of drug eruptions. RESULTS: A total of 228 patients were diagnosed with drug eruptions, and this study included 139 patients. The highest incidence of drug eruptions was observed in patients in their 50s (22.3%). The most common latent period was up to 1 week (57.6%). The most common drug eruptions were exanthematous eruptions (59.7%), acneiform eruptions (10.8%), and urticaria (9.3%). The most common causative drugs were antibiotics (53.2%), followed by anticancer drugs (19.4%), and contrast media (6.5%). Laboratory abnormalities included eosinophilia (15.8%), abnormal liver function tests (7.9%), leukopenia (4.3%), an elevated serum creatinine level (2.2%), and leukocytosis (0.7%). CONCLUSION: In descending order, the most frequent drug eruptions were exanthematous eruptions, acneiform eruptions, and urticaria, and the most common causative drugs were antibiotics, anticancer agents, and contrast media. Prompt diagnosis and discontinuation of the causative drug are important in this context. Clinicians should be aware of cutaneous adverse drug reactions.

ABSTRACT

Background@#Complementary and alternative medicine (CAM) is a popular treatment option among patients. There is a need to assess the content of published local literature on complementary and alternative medicine in dermatology in the Journal of the Philippine Dermatological Society (JPDS) to provide evidence for its integration into clinical practice and to guide future research.@*Objective@#To analyze the content of published literature on complementary and alternative medicine published in the JPDS@*Method@#We screened the titles and abstracts of all published articles in the JPDS from 1992-2017 and included articles on any aspect of complementary and alternative medicine. We extracted relevant characteristics such as study design, disease, type and details of CAM therapies, from the included articles. We summarized data using descriptive statistics for continuous data, and frequency distribution for ordinal data.@*Results@#CAM articles accounted for 4% of all published articles in the JPDS over the past 25 years of its existence. Most were RCTs (22%) (mean size=69), with the greatest number being on herbal medicine (66%), and for the treatment of infections, infestations and bites (46%). Majority of articles were primarily authored by resident trainees (87%) and the most common institutional affiliation was UP-PGH (24%). Among the herbal articles, the most common herb family was Fabaceae (9%), while coconut had the highest number of articles. Leaves were the most common plant part used (40%), and creams were the predominant type of formulation (29%). Other CAM interventions included were animal products, dietary supplements, and lifestyle modification.@*Conclusion@#The poor representation of CAM in the JPDS may reflect the lack of integration of this field in the local practice of dermatology. There is a need to increase awareness and gather more evidence on the application of CAM in the maintenance of healthy skin as well as prevention and treatment of skin diseases.
