ABSTRACT

ABSTRACT Introduction: The para-Bombay phenotype, or H-deficient secretor, results from different mutations of the FUT1, with or without the FUT2 mutation. Consequently, there is an absent or weak expression of the H antigen on red blood cells (RBCs). Routine ABO blood grouping for two siblings with blood group O showed discrepant results with their parental blood group AB. Fragments encompassing the entire coding region of the FUT1 and FUT2 genes were investigated. Methods: Blood and saliva specimens were collected to verify the correct ABO grouping by cell grouping, serum grouping and the hemagglutination inhibition (HI) test, respectively. The FUT1 and FUT2 genomes were identified using the whole-exome sequencing (WES) in two children's DNA blood specimens and may have caused, or been relative to, their blood group. Genetic variations of the FUT1 and FUT2 genes have been investigated in the other family members using the Sanger sequencing. Results: The serologic reaction results of the proband revealed that A, B and H antigens were absent on RBCs, and that the serum contained anti-H. However, ABH and AH antigens were present in the saliva PB1 and PB2, respectively. The probands PB1 and PB2 were assigned as AB and A blood groups, respectively. Blood genotyping confirmed that heterozygous mutations of the FUT1 gene, c.551_552delAG, were identified. Three family members, PB3, PB, and PB8, also showed normal ABO blood groups, but their genotypes were also the FUT1 mutation c.551_552delAG. Conclusions: The FUT1 mutation c.551_552delAG may result in the reduced or absent H antigen production on RBCs, which characterizes the para-Bombay phenotypes. Blood genotyping is essential if these individuals need a blood transfusion or are planning to donate blood.

ABSTRACT

ABSTRACT

@#Introduction: Previous studies on the degree of leg length discrepancy that causes limb biomechanical problems did not differentiate between adults and children. We conducted this study to determine the effects of simulated leg length discrepancy on vertical ground reaction force in children and adults to enable decision-making for intervention in patients with leg length discrepancy for different age groups or heights. Materials and methods: This cross-sectional study involved male volunteers of children 150cm and adults with 170cm in height. Vertical ground reaction force was measured using a gait analysis study. The first measurement was taken without any leg length discrepancy as a baseline. Subsequently, different amounts of leg length discrepancy were simulated on the left leg with shoe lifts of 2, 3, and 4cm. The measurements were repeated on each volunteer with similar shoe lifts on the right leg. Therefore, 14 volunteers provided simulations of 28 leg length discrepancies for each group. The first and second peaks of vertical ground reaction force were separately analysed. The vertical GRF of a simulated leg length discrepancy was compared with the baseline. Repeated measurement of analysis of variance (ANOVA) within each group was done. Results: In both groups, the second peak of vertical ground reaction force in the longer leg reduced gradually as the shoe lift increased sequentially from 2 to 3cm and then to 4cm. A discrepancy of 3cm and above was statistically significant to cause a reduction in the vertical GRF on the longer limb in both height groups. Conclusion: The degree of leg length discrepancy that caused significant changes in second peak ground reaction force in children with 150 and adults with 170cm height population was similar at 3cm. Therefore, the cut-off point for intervention for both groups are similar with additional consideration of future growth in children.

ABSTRACT

Abstract The scientific concept of High Intellectual Ability (HIA) is expanded attending the question of discrep ancy between the high cognitive potential and the low performance of some people with HIA. The study of the pathways and conditioning factors that lead to low performance is necessary in order to facilitate the development and optimal expression of high potential. The role of executive and metacognitive regulation of high intellectual resources is highlighted, in interaction with other endogenous and exogenous modulators. The aim of this work is to know if there is a relational model linking executive and metacognitive regulation, and perfectionism (as an endogenous modulator of HIA). The executive, metacognitive and perfectionism measures obtained in a sample of n = 140 schoolchildren, diagnosed as HIA, are statistically analyzed using Structural Equation Models. Two relational models are obtained: one indicating that executive function influences metacognition, and the other indicates that perfectionism influences executive functioning. The need for more concrete analyses of the direct, indirect or total influence among these regulation constructs is concluded and discussed, with derivations for the need of effective measures that guide the optimal expression of HIA, preserving personal well-being.

ABSTRACT

Objective To study topological structure of a new type of three-dimensional (3D) printed height increasing insoles for leg length discrepancy (LLD) and its effect on biomechanics of lower limbs. Methods Topological structure for middle and rear part of the insole was optimized by solid isotropic microstructures with penalization (SIMP), the force was loaded and the boundary conditions were set according to force area of the insole, and the height increasing insole with thermoplastic polyurethanes (TPU) materials was printed by selected laser sintering (SLS). The insoles were used in 9 patients with LLD, visual analogue scale (VAS) and Maryland foot function scores were used to compare pain and foot function changes of patients before and after using the insole, and the 3D gait analysis system was used to compare spatiotemporal parameters and vertical ground reaction force (vGRF) of both lower limbs. Result sAfter the patient wore 3D printed insole, VAS scores decreased, Maryland foot function scores increased, vGRF of both lower limbs decreased, and the difference of cadence, stance phase and swing phase in both lower limbs decreased. Conclusions The 3D printed height increasing insole after topology optimization can improve coordination of lower limb movement, reduce ground impact, relieve pain and improve foot function, thus providing an effective personalized orthopedic plan for LLD treatment in clinic.

ABSTRACT

OBJECTIVE@#To investigate the effect of different proximal femoral shapes on leg length discrepancy(LLD) after total hip arthroplasty(THA).@*METHODS@#Total 131 patients with osteoarthritis or osteonecrosis received unilateral biological total hip arthroplasty from June 2013 to June 2019. All patients' age, sex, side and pelvis anteroposterior digital radiography were retraspectively analyzed. There were 69 males and 62 females, 57 cases of left hip and 74 cases of right hip. The age ranges from 25 to 89 with an average age of 62 years. There were 48 cases of osteoarthritis and 83 cases of osteonecrosis. In this study, femoral cortical index (FCI) was used as the classification of proximal femoral shape, and bilateral lower limb length differences were measured by preoperative and postoperative pelvis anteroposterior digital radiography. Grouping according to FCI:> 0.6 was Dorr A group, 0.5 to 0.6 was Dorr B group, 0.6, the postoperative LLD was 6.30 mm (IQR 1.00 to 10.95 mm). When FCI was 0.5 to 0.6, the postoperative LLD was 5.85 mm(IQR-0.55 to 8.90 mm). FCI<0.5, the postoperative LLD was 1.95 mm(IQR -2.50 to 6.68 mm). LLD comparison of different proximal femoral shape was statistically significant (@*CONCLUSION@#High FCI increases the risk of lower extremity prolongation after surgery on the affectedside, while low FCI reduces the risk of lower extremity prolongation after surgery on the affected side. The surgeon can assess the shape of the proximal femur of the patient preoperatively and inform the patient in advance of possible changes in leg length of both lower extremities after total hip replacement.

ABSTRACT

【Objective】 To investigate the effect of cold agglutination on blood group typing. 【Methods】 37℃ water bath, absorption elution test and 2-mercaptoethanol method were used to eliminate the influence of cold agglutination. Forward and reverse blood group typing, cross matching, DAT and IAT experiments were then performed on red blood cells and serum after treatment. 【Results】 Before treatment, obvious discrepancy in forward /reverse typing and nontypable cross matching in 16 blood samples were noticed due to cold agglutination. After corresponding treatments, all samples were consistent or negative in forward/reverse typing, cross matching and antibody screening. No adverse reactions to cross matching blood transfusion occurred in patients, and the increase of hemoglobin was in line with the effective standard of transfusion. 【Conclusion】 37℃ water bath, absorption elution test and 2-mercaptoethanol method can be used to eliminate the interference caused by cold agglutination to obtain correct typing results. The strong reactivity caused by cold agglutination in AIHA patients were different from other cases, which deserved our attention.

ABSTRACT

@#Intermaxillary tooth size discrepancy (TSD) is the disproportion in patients’ tooth size that is usually referred to the Bolton analysis. This size imbalance may also affect the outcome of orthodontic treatment. Therefore, this retrospective study was conducted to determine the prevalence of intermaxillary tooth size discrepancy (TSD) beyond 2 standard deviation (2 SD) from Bolton’s mean in orthodontic patients and to evaluate the effect of TSD beyond 2 SD of Bolton’s mean on orthodontic treatment outcomes. A total of 226 sets of orthodontic study models that met the selection criteria were selected from 3077 orthodontic cases. Bolton's analysis of the anterior and overall ratios was performed on each of the pre-treatment models. The orthodontic treatment outcomes for the cases which were identified of having TSD beyond 2 SD were evaluated and any interventional techniques to improve the outcome of the treatment were recorded. The results showed that 29.20% of cases had TSD beyond 2 SD of Bolton’s mean. 26.54% of cases had anterior ratio of more than 2 SD. Meanwhile, 7.08% of cases had an overall ratio greater than 2 SD. Of all cases with TSD beyond 2 SP, most of these cases (62.12%) had affected the outcome of orthodontic treatment. In conclusion, there is more than one quarter of orthodontic patients present with TSD beyond 2 SD of Bolton’s mean which may influence the outcome of orthodontic treatment. Intervention techniques should be carefully planned to achieve satisfactory treatment outcomes.

ABSTRACT

@#Introduction: Accurate provisional diagnosis in the Emergency Department (ED) is important as it has a significant impact on safety. It also affects the patients’ treatment, length of stay and cost of treatment. The data on the accuracy of making diagnosis made by ED doctors is scarce and most results vary with different countries. This study was conducted to evaluate the accuracy of the diagnosis made by the ED doctors in Hospital Universiti Sains Malaysia (USM) and the factors contributed to the discrepancy. Method: This was a retrospective cross-sectional study conducted in Hospital USM from May 2016 to December 2017. Medical records of the patients who were admitted to the hospital were selected using simple random sampling methods. The folders were reviewed and the association within the categorised diagnosis accuracy was analysed using the ICD-10 classification. The sample size was 180 cases, and cases were divided into two main categories. The factors associated with the unmatched diagnosis from both patients and provider were then measured using multiple logistic regressions. Results: Hospital USM Emergency Department had 15.6 per cent of unmatched diagnosis and 84.4 per cent of matched diagnosis. No difference between age and gender in making accurate diagnosis. The odds of having unmatched diagnosis in patients from the green zone are 4.2 times higher compared to the red zone. Conclusion: ED Hospital USM has a high diagnostic accuracy, especially involving the patients in red zone and yellow zone.

ABSTRACT

Background: Sickle cell anaemia, (SCA) the homozygous and most severe of the sickle cell diseases exerts adverse effects on growth and linear body proportions. In Nigeria, these changes in the lower extremity are scarcely documented. Objectives: To evaluate the differences between the real leg lengths and apparent leg lengths determined by tape measure among children with SCA. Methods:A clinic-based, cross-sectional, comparative study of 140 age and sex matched “SS” and “AA” children. The real leg lengths, anterior superior iliac spine to medial malleolus and to the heel, the apparent leg length, umbilicus to the medial malleolus were measured with a plastic tape and recorded. Leg length discrepancy (LLD), the arithmetric differences between corresponding leg lengths were recorded and classified into nil (0), mild (<2cm), moderate (2-5cm) and severe (>5cm).Results:At the medial malleolus, the subjects had more mild and moderate Real LLD than controls while more controls than subjects had nil Real LLD (p = .033). Both right and left real leg lengths were significantly shorter in the 6-9 year old subjects especially the males, all female subjects, all 10-13 year olds subjects more so the females than their respective counterparts. (All p values < 0.05) Similarly, the right and left leg lengths at the heel were significantly shorter in subjects of the same age and sex groups as above than all their respective counterparts. (P values < 0.05). However, male subjects had only the right real leg length significantly shorter than those of the controls. Real LLD: ASIS-MM was significantly higher in 10-13 year old female subjects, real LLD: ASIS-HEEL in14-18 year old subjects than the controls. The right and left apparent leg length were significantly lower in all 10-13 year old subjects as well as 10-13 year old female subjects than the respective matches. All p values were < 0.05. No significant difference existed in the apparent LLD comparisons

ABSTRACT

Inside the traverse alteration of the maxillary the compressions are described as uni or bilateral where an imbalance is settled down between the dental size and the size of the maxillary causing alterations in the position of the teeth. In the early mixed teething the narrowness of the maxillary can be settled by means of indexes that relate the dental size with the traverse width of the dental arch. Thirty models of the maxillary of children according to Barnett's third development period were analyzed. The metrical analysis of the dental arch form was carried out through Pont's index. The theoretic values were compared with the real ones establishing the deviations of the norm that is to say the narrow nest. Out of the 30 cases analyzed, 40% presented mesial rotation of the 4 incisors; 27% showed a stop of the front teeth; 27% atypical reabsorption; 20% mesial rotation of the central incisors and distal rotation of the lateral incisors and the 10% presented a distal rotation of the central incisors. With regard to the front atypical discrepancy 36.66% of the cases had a discrepancy above 6 mm and the 23.33% below 3 mm (AU)

To evaluate the marginal/internal discrepancy and precision of the prosthesis, scan data were superimposed by the triple-scan protocol and the combinations calculator, respectively. Quantitative and qualitative analysis was performed using root mean square (RMS) value and color difference map in 3D analysis program (Geomagic control X). Statistical analysis was performed using the Kruskal-Wallis test (α=.05), Mann-Whitney U test and Bonferroni correction (α=.05/3=.017).RESULTS: The marginal discrepancy of S3P group was superior to MIL and D3P groups, and MIL and D3P groups were similar. The D3P and S3P groups showed better internal discrepancy than the MIL group, and there was no significant difference between the D3P and S3P groups. The precision was excellent in the order of MIL, S3P, and D3P groups.CONCLUSION: Within the limitation of this study, the 3-unit fixed dental prostheses fabricated by additive manufacturing showed better marginal and internal discrepancy than the those of fabricated by subtractive manufacturing, but the precision was poor.

ABSTRACT

Objective@#To establish the local data on the growth of ear in Hong Kong children and provide a reference for the timing of reconstruction in unilateral microtia.@*Methods@#We reviewed case notes of paediatric patients up to 18 years of age who had attended our Ear, Nose and Throat Out-patient Clinic from March to November 2017. We recorded the pinna length of these patients and that of their parents, compared the patients’ ear length against that of their parents, and investigated any discrepancy of pinna.@*Results@#We recruited 226 local individuals (139 males and 87 females). The patients were divided into different age groups. The means of ear length of patients were compared with the mean length of pinna of their parents. Data of boys and girls are analyzed separately. Boys at 7-8 years old achieved 87.33% and 93.54% of their fathers’ and mothers’ ear length respectively. Girls at 7-8 years old achieved 83.00% and 90.80% of their fathers’ and mothers’ ear length respectively. Moreover, the average ratio of the length of left and right ear ratio is 97.3% in all groups of children.@*Conclusions@#In Hong Kong children, at the age of 7-8 the ear approaches the size of normal adult ear and is the feasible age with less asymmetry after reconstruction of the microtia.

ABSTRACT

Background: Minor salivary gland tumors (MSGTs) are less common than major salivary glands and involve only 15–20% of all salivary gland tumors. Most of the cases originate at intra- and peri-oral region. Minor salivary gland lesion cytology has been studied rarely in India. Aims: This present study was performed to evaluate the role of fine needle aspiration cytology (FNAC) in diagnosis of MSGTs and to explore the cases of cytohistological discrepancies in the study. Materials and Methods: This retrospective study was conducted over a 5-year period on 42 cases of MSGTs. In all the cases, cytology was correlated with histology and cytohistological discrepancies were searched. Sensitivity, specificity, and diagnostic efficacy were calculated using histopathology as gold standard. Results: We diagnosed 27 malignant (64.28%) and 15 benign (35.71%) MSGTs in cytological evaluation. We found two false negative and one false positive case in cytology. Sensitivity, specificity, and diagnostic accuracy of the study were 92.59%, 93.33%, and 92.85%, respectively. Conclusion: FNAC is a minimally invasive and cost-effective procedure with high accuracy (92.85%) in the assessment of MSGTs and helps in the management of benign and malignant tumors.

ABSTRACT

Leg length discrepancy is when the length of one leg is different from the other, and is a common reason for consultation at the pediatrician's and pediatric orthopedist's office. The objective of this study was to develop recommendations for the follow-up, pre-surgical planning, and treatment of children with leg length discrepancy based on expert consensus. Material and methods. The Delphi method was used. A coordinating group selected a panel of experts, designed and analyzed each of the rounds of consultations. Semistructured questionnaires were sent by personalized e-mail. Agreement among experts ≥ 80 % was established as the criterion for consensus. At each round of consultation, non-consensual aspects were reformulated and new aspects suggested in the previous round were included. A measure of stability to conclude the consultation was determined when more than 70 % of experts sustained their opinion in successive rounds. Results. Eight experts in orthopedics and six experts in imaging studies participated. After three rounds of consultations, consensus was reached in terms of 39 recommendations for follow-up, pre-surgical planning, and treatment. These were reorganized into 32 final recommendations. Conclusions. These are the first recommendations for the follow-up of children with leg length discrepancy agreed by expert consensus.

ABSTRACT

ABSTRACT: Precautionary measures are necessary to prevent a potential irregularity from progressing into a more severe malocclusion. The determination of the tooth size-arch length discrepancy in mixed dentition requires an accurate prediction of the mesiodistal widths of the unerupted permanent teeth to prevent future malocclusions. Objective: The purpose of this study is to generate a new regressive equation to predict the diameter of the cuspid and bicuspid by measuring the wide of lower incisor and upper molar teeth in a Chilean population sample. Matherials and methods: the study was conducted in the Araucanía Clinical Center in Temuco city and the sample was composed of historical dental cast from 200 school children (93 boys and 107 girls), of IX region, Chile. Age group of 13-16 years old. Measurements of the mesiodistal widths of mandibular incisors, maxillary and mandibular canines and premolars were made with the help of Vernier gauge calipers calibrated to 0.01 of 1 millimeter. Results: The results show that there is a moderate correlation between the upper molar and upper and lower premolars. In addition, the lower incisors are correlated to a greater extent with the upper and lower canines. There are no significant differences between men and women. Conclusion: A new formula with 75% accuracy was developed to predict the width of canine teeth and premolars in mixed dentition in the Chilean population of the Araucanía region. This formula is better than the current Tanaka method.
