ABSTRACT

Congenital entropion, an abnormal condition in which the eyelids roll inward, with the eyelashes losing their normal angulation and tilting toward the eye, especially in the lower eyelids and inner canthus, often occurs in infants and young children. Congenital entropion may lead to corneal epithelial abrasion, inflammation and ulcer, which may affect the function of the eye if not treated in time. Early surgical intervention is helpful to the health of children's eyes. The purpose of surgery is to change the structure of eyelid and weaken the force of entropion, thus improving the symptoms and corneal astigmatism. At present, there are many surgical treatments for congenital entropion. In this paper, the advantages, disadvantages and indications of these treatments are analyzed and summarized, providing a reference for clinical practice.

ABSTRACT

Objetivo: Describir los resultados de la técnica de entrecruzamiento del orbicular, descrita por profesores eméritos del Pando Ferrer, en el entropión senil. Métodos: Se realizó un estudio observacional descriptivo longitudinal retrospectivo con pacientes atendidos en el Instituto Cubano de Oftalmología Ramón Pando Ferrer entre enero del 2021 a junio de 2022. La muestra estuvo constituida por pacientes operados con entropión senil con la técnica de Martínez Rodríguez modificada. Se les realizó examen ocular completo con seguimiento a las 24 h, 7 y 15 días, al mes y a los tres meses posteriores a la cirugía. Las variables utilizadas fueron edad, sexo, lateralidad, tiempo de cirugía y complicaciones. Resultados: La edad media fue de 79,26 años, con predominio del sexo masculino (69,56 %) en 23 pacientes y 26 párpados; siendo 20 casos unilaterales y 3 bilaterales. Se reportó una complicación de hipocorreción (3,84 %) y un tiempo medio de 11,9 minutos. Conclusiones: La técnica descrita es un abordaje efectivo demostrado en la baja frecuencia de complicaciones, sencillo y poco invasivo para entropión senil sin laxitud horizontal grave.


Objective: To describe the results of the orbicularis crosslinking technique, described by emeritus professors from Pando Ferrer institute, in senile entropion. Methods: A retrospective, longitudinal, descriptive and observational study was carried out with patients who received attention at Instituto Cubano de Oftalmología Ramón Pando Ferrer between January 2021 and June 2022. The sample consisted of patients with senile entropion operated on with the modified Martinez-Rodriguez technique. They underwent a complete ocular examination with follow-up at 24 hours, 7 and 15 days, one month and three months after surgery. The used variables were age, sex, laterality, surgery time and complications. Results: The mean age was 79.26 years, with a predominance of the male sex (69.56 %) in 23 patients and 26 eyelids; there were 20 unilateral cases and 3 bilateral ones. One complication of hypocorrection (3.84 %) and a mean time of 11.9 minutes were reported. Conclusions: The described technique is an effective approach for senile entropion, demonstrated according to the low frequency of complications, as well as simple and minimally invasive and without severe horizontal laxity.

ABSTRACT

INTRODUCCIÓN Y OBJETIVO: el entropión involucional no deja de ser una patología de presentación frecuente en la consulta de cirugía plástica. A lo largo del tiempo se describieron diferentes técnicas quirúrgicas para su manejo pero en la actualidad aún no existe un consenso de cuál es la técnica ideal.El entropión es la rotación interna y mantenida del margen palpebral superior o más comúnmente del parpado inferior, de forma unilateral o bilateral.De los cuatro tipos descritos el entropión involutivo es el más común.Se presenta una técnica de fácil ejecución que resuelve la mayoría de los factores fisiopatológicos que desarrollan entropión senil. MATERIAL Y MÉTODO: se operaron 13 párpados en total, con la técnica denominada acortamiento vertical y alargamiento horizontal palpebral. Todas las cirugías cursaron sin intercurerncias.El seguimiento promedio fue de 6 meses. RESULTADOS: en el examen físico post operatorio de todos los pacientes demuestra, mejoría en la posición palpebral y eliminación completa de la exposición escleral. CONCLUSIÓN: la técnica propuesta combina la corrección de la laxitud horizontal y vertical de los tejidos palpe-brales, con resultados efectivos en el tratamiento del entropión involutivo.


INTRODUCTION AND OBJECTIVE: involutional entropion is a common pathology, there are many techniques for its resolution, but so far none has proven to be the preferred oneEntropion is the internal and maintained rotation of the upper or more common palpebral margin of the lower eyelid, unilaterally or bilaterally.Of the four types described, involutive entropion is the most common.An easy-to-execute technique is presented that solves most of the physiological factors that lead to senile entropion. MATERIAL AND METHOD: thirteen eyelids were operated in total, with the technique called vertical shortening and palpe-bral horizontal elongation. All surgeries went without complicationsThe average follow-up was 6 months. RESULTS: postoperative physical examination, in all patients shows an improvement in palpebral position and complete elimination of scleral exposure. CONCLUSION: the proposed technique combines the correction of the horizontal and vertical laxity of the palpe-bral tissues, with effective results in the treatment of involutional entropion

ABSTRACT

Background: Severe cicatricial entropion in Stevens–Johnson syndrome (SJS) patients is difficult to treat and is associated with a higher recurrence rate. Also, entropion in the presence of lid margin mucous membrane graft (MMG) further complicates the surgical anatomy and approach. Purpose: To report a modified surgical technique of repairing severe upper eyelid cicatricial entropion in an SJS patient with history of lid margin MMG. Synopsis: Cicatricial entropion in patients with SJS is entirely different from trachomatous cicatricial entropion. The involvement of the lid margin with keratinization, tarsal scarring, persistent conjunctival inflammation, and unhealthy ocular surface affects the surgical approach and outcomes. Conjunctiva?sparing surgery with reconstruction of the lid margin using MMG, flattening and repositioning the anterior lamella, and covering the bare tarsus with MMG rather than leaving it raw are the necessary modifications in this technique from conventional anterior lamellar recession. The video demonstrates the surgical technique for harvesting and preparation of a labial MMG, the splitting of the anterior and posterior lamella of the lid margin, scar tissue release between the lash line and the tarsus, repositioning of the anterior lamella, and anchoring of the labial MMG. Highlights: Anterior lamellar recession combined with MMG wrapping the lid margin and bare tarsus offers good cicatricial entropion repair outcomes. Removal of fat and submucosa from the mucosal graft should be done for better cosmesis. Adequate separation of the scar tissues from the lash line and the tarsus is essential.

ABSTRACT

Purpose: Different techniques for lateral canthal suspension have been used in the management of various eyelid malpositions. We describe a simplified technique for lateral canthal suspension and review its outcome along with a review of existing variations. Methods: We conducted a retrospective chart review of 28 eyelids in 22 patients who underwent simplified lateral canthal suspension. Demographics, symptoms at presentation, and associated eyelid malposition were noted. We evaluated the palpebral fissure and margin?reflex distance 2 (MRD2) on the preoperative and final postoperative photographs by using MEEI FACE?gram software. We also reviewed existing literature on different surgical management options for comparison. Results: At three?month postoperative follow?up, presenting symptoms resolved in all cases. The average postoperative decease in palpebral fissure was 0.73 mm (P = 0.018) and the average decrease of the MRD2 was 1.02 mm (P = 0.0003). Recurrence occurred by three months in one eyelid (4%) with ectropion due to moderate eyelid laxity, and this case was managed with tarsal strip procedure. One patient (5%) who had bilateral surgery had asymmetric lower eyelid position and one patient (5%) had persistent edema of the operated eyelid for six months. Conclusion: This simplified canthal suspension is a simple and effective technique that tightens the lateral canthal tendon and improves the lower eyelid position. It can be used in various mild?to?moderate eyelid laxities and has favorable operative characteristics compared with many existing techniques

ABSTRACT

AIM: To investigate the efficacy of orbicularis oculi muscle resection combined with orbicularis oculi muscle shortening and lower eyelid retractor reduction in the treatment of elderly lower eyelid entropion.METHODS:A retrospective study was conducted among 97 elderly patients(175 eyes)with lower eyelid entropion who admitted to the hospital from June 2019 to June 2021. According to the treatment method, the patients were divided into control group(47 patients of 82 eyes treated with orbicularis oculi muscle resection)and combination group(50 patients of 93 eyes treated with orbicularis oculi muscle resection combined with orbicularis oculi muscle shortening and lower eyelid retractor reduction). The two groups were compared in terms of short-term curative effect, perioperative indexes, scores of symptoms and signs before and after surgery, width of palpebral fissure before and after surgery, direction of eyelashes, exposure rate of lacrimal caruncle, complications, and patient satisfaction.RESULTS: The total response rate in the combination group was significantly higher than that in the control group(95% vs 80%, P=0.004). The intraoperative blood loss, operation time and hospital stay of the combination group were significantly more/longer than those of the control group(P<0.001). The scores of symptoms and signs such as lacrimation, foreign body sensation, photophobia and irritation in the combination group after the surgery were significantly lower than those in the control group(all P<0.001). After surgery, the width of palpebral fissure, direction of eyelashes and exposure rate of lacrimal caruncle in the combination group were higher than those in the control group(P<0.001). The incidence of postoperative complications in the combination group was lower than that in the control group(8% vs 18%, P=0.032). The patient satisfaction scores of comfort level, trichiasis correction, scar appearance, eyes symmetry and appearance in the combination group were higher than those in the control group(all P<0.001).CONCLUSION: Orbicularis oculi muscle resection combined with orbicularis oculi muscle shortening and lower eyelid retractor reduction is effective and safe in the treatment of elderly lower eyelid entropion, which can meet the requirements of the patients.

@#AIM: To compare the efficacy of two surgical approaches that microscopical Hotz combined with eyelid margin incision and flap translocation and pure Hotz method in correction of severe upper eyelid cicatricial entropion. <p>METHODS: Totally 84 eyes of 60 patients who underwent corrective surgery for upper eyelid trichiasis between July 2017 to July 2019 were retrospectively assessed. The patients in trial group(32 cases, 42 eyes)underwent the surgical procedure of combining Hotz with eyelid margin incision and skin flap transposition under microscope, the other group(28 cases, 42 eyes)were treated with Hotz method. The follow-up time was 12mo. Subjective symptoms, eyelid marginal position, eyelash eversion and patient satisfaction were recorded.<p>RESULTS: The surgery time of one eye in trial group was longer than that in control group(40.8±2.57min <i>vs</i> 28.5±2.64min, <i>P</i><0.01). The cure rates of 1 and 12mo in trial group were 100% and 95%, respectively. And those in control group were 95% and 76%, respectively. The cure rate of 12mo in trial group was higher than that of the control group(<i>P</i>=0.013). There was no significant difference in patient satisfaction between the two groups at 1mo after surgery(<i>Z</i>=1.1825, <i>P</i>=0.2371). 12mo after surgery, patient satisfaction in the trial group was higher than that in the control group(<i>Z</i>=3.7346, <i>P</i><0.01).<p>CONCLUSION: While it spents longer time, the procedure of combining Hotz with eyelid margin incision and skin flap transposition under microscope is reasonably successful and satisfactory in rectifying severe upper eyelid cicatricial entropion. It offers more higher cure rate than Hotz alone at long time after operation.

ABSTRACT Purpose: Inferior eyelid laxity is classically evaluated using "snap-back" and "distraction" tests. This study aimed to assess the reproducibility of the technique used to indirectly quantify the horizontal tension in the lower eyelids using digital image processing. Methods: This longitudinal study was conducted to assess the reproducibility of a new technique that quantifies the horizontal tension in the lower eyelid. The study was conducted at the Hospital das Clínicas of Porto Alegre. The protocol was established by two trained ophthalmologist examiners, allowing intra- and interobserver agreement analyses. Image acquisition was done in two stages: the first image was captured with the eyelid in primary gaze position and the second with the eyelid in traction position. All images and measurements were processed using Image J 1.33m software from the National Institute of Health. The Bland-Altman method, intraclass correlation coefficients, concordance correlation coefficients, and technical measurement error were used to evaluate reproducibility. Results: The study participants comprised healthy individuals with no ophthalmologic pathologies. The measurements obtained in the neutral position showed a slightly higher agreement than those obtained in the traction position. The mean difference between the measurements performed in the traction position was 0.028 ± 0.7 mm and 0.014 ± 0.9 mm in the intra- and interobserver analyses, respectively. The Bland-Altman method demonstrated adequate confidence limits for both measurements. Correlation coefficients for measurements varied between 0.87 [95% confidence interval (CI) 0.68-0.95] and 0.91 (95% CI 0.77-0.97) in the neutral position and between 0.72 (95% CI 0.37-0.89) and 0.76 (95% CI 0.4-0.91) in the traction position. Conclusion: A high intra- and interobserver concordance was observed in the studied method to quantify lower eyelid tension. The proposed method is simple and easily reproducible, and to the best our knowledge, this is the first method that quantifies lower eyelid horizontal tension on the basis of digital image processing. This modified distraction test might be useful in studies quantifying lower eyelid horizontal tension.


RESUMO Objetivo: A frouxidão palpebral inferior é avaliada classicamente por meio de testes de "snap-back" e "distraction test". O objetivo deste estudo foi avaliar a reprodutibilidade da técnica utilizada para quantificar indiretamente a tensão horizontal nas pálpebras inferiores através do processamento digital de imagens. Métodos: Este estudo longitudinal foi realizado para avaliar a reprodutibilidade de uma nova técnica que quantifica a tensão horizontal na pálpebra inferior. O estudo foi realizado no Hospital das Clínicas de Porto Alegre. O protocolo foi estabelecido por dois examinadores oftalmologistas treinados, permitindo análises de concordância intra e interavaliador. A aquisição de imagens foi feita em duas etapas: a primeira imagem foi capturada com a pálpebra na posição primária do olhar e a segunda com pálpebra tracionada. Todas as imagens e medições foram processadas usando o software Image J 1.33m do National Institute of Health. O método de Bland-Altman, os coeficientes de correlação intraclasses, os coeficientes de correlação de concordância e o erro técnico da medida foram utilizados para avaliar a reprodutibilidade. Resultados: Os participantes do estudo foram indivíduos saudáveis e sem patologias oftalmológicas. As medidas obtidas na posição neutra mostraram concordância levemente maior do que as obtidas na posição tracionada. A diferença média entre as medidas realizadas na posição tracionada foi de 0,028 ± 0,7mm e 0,014 ± 0,9mm nas análises intra e interobservadores, respectivamente. O método de Bland-Altman demonstrou limites de confiança adequados para ambas as medidas. Os coeficientes de correlação para as medidas variaram entre 0,87 [intervalo de confiança de 95% (IC) 0,68-0,95) e 0,91 (IC 95% 0,77-0,97) na posição neutra e entre 0,72 (IC 95% 0,37-0,89) e 0,76 (IC 95% 0,46-0,91) na posição tracionada. Conclusão: Observou-se elevada concordância intra e interobservador no método estudado para quantificar a tensão palpebral inferior. O método proposto é simples e facilmente reproduzível, e, do melhor modo possível, este é o primeiro método que quantifica a tensão horizontal da pálpebra inferior com base no processamento digital de imagens. Este teste de distração modificado pode ser útil em estudos que quantifiquem a tensão horizontal da pálpebra inferior.

@#AIM: To evaluate the effects and complications of retractor repair combined with a lateral tarsal strip procedure and retractor repair alone for treating lower eyelid involutional entropion.<p>METHODS: This was a prospective study. Totally 79 cases(91 eyes)with involutional lower eyelid entropion who were hospitalized from January 2015 to February 2018 were divided randomly into observation group(41 cases 46 eyes, underwent combined procedure)and control group(38 cases 45 eyes, underwent retractor repair procedure). The clinical effects and the complications of the two groups were compared.<p>RESULTS: At 3mo postoperatively, there was no significant difference in the clinical effective rate(100% <i>vs</i> 98%, <i>P</i>=0.495)between the two groups. At 24mo postoperatively, the observation group had a higher clinical effective rate(98% <i>vs </i>84%, <i>P</i>=0.030). There were no complications occurred in the observation group, significantly fewer than the control group(11%,<i> P</i>=0.026).<p>CONCLUSION: Compare to retractor repair procedure, the combined procedure has a higher clinical effective rate and a lower complications rate.

@#AIM: To investigate the effect of surgical time on the development of visual refraction in children with congenital entropion and trichiasis.<p>METHODS: This was a prospective interventional study. Fifty nine eyes of 30 patients aged from 2 to 12 years old who suffered congenital lower eyelid entropion with trichiasis were divided into three groups according to the age. Improvement in binocular vision, refractive error, retinography astigmatism and cornea astigmatism by topography were analyzed. All patients were followed up for a period of 6 to 12mo.<p>RESULTS: There were no significant complications of overcorrection, undercorrection or eyelid deformity. The postoperative corneal astigmatism(CC)was significantly improved in three groups(<i>P</i><0.05). retinography astigmatism(RC)and best corrected visual acuity(BCVA)were significantly improved in low-aged and middle-aged groups(<i>P</i><0.05). The proportion of astigmatism with the rule and amblyopia was larger in middle-aged group. The postoperative CC, RC and BCVA was not statistically different in high-aged group(<i>P</i>>0.05).<p>CONCLUSIONS: Children with congenital entropion and trichiasis should be corrected surgically before 6 years old. The incidence of astigmatism with the rule and amblyopia is higher in 4 to 6 aged children and should be corrected earlier.

ABSTRACT

Excessive facial skin folds is observed in several breeds of dogs and the weight exerted on the eyelids accents or promotes entropion, trichiasis and ptosis. Thus, this study reported a case of the 8-months-old male Shar-pei weighting 21.5kg was presented with an obstructed visual axis, eye discharge, fetid odor in facial folds around the jaw and the neck. It was indicated the surgical resection of the folds and correction of the upper and lower entropion. In this case, the association of Hotz-Celsus technique with rhytidectomy shaped in semiarchs, using the anchoring points with the modified walking suture, was effective in correcting the entropion and unblocking the visual axis with minimal scarring and preservation of the breed standard in 12 months follow up after surgery.


O excesso de pregas faciais é observado em várias raças de cães e o peso exercido sobre as pálpebras acentua ou promove entrópio, triquíase e ptose. Assim, este estudo relata o caso de Shar-pei, macho de 8 meses de idade, pesando 21,5 kg que apresentava o eixo visual obstruído, secreção ocular e odor fétido nas dobras faciais em torno da mandíbula e do pescoço. Foi indicada a ressecção cirúrgica das pregas e a correção do entrópio superior e inferior. Neste caso, a associação da técnica de Hotz-Celsus com a ritidectomia em forma de semiarcos, utilizando pontos de ancoragem com a técnica "walking suture" modificada, foi efetiva na correção do entrópio e na liberação do eixo visual, com mínima cicatriz e preservação do padrão da raça 12 meses após a cirurgia.

ABSTRACT

Resumo Paciente de 69 anos evoluiu com entrópio palpebral severo após cirurgia de correção de ptose palpebral pela técnica de reinserção da aponeurose do músculo levantador da pálpebra superior. Realizada reintervenção onde foi diagnosticado uma fixação da aponeurose em uma posição muito inferior e feita uma refixacação no 1/3 superior do tarso, com melhora do quadro funcional e estético com boa satisfação da paciente. Devido às suturas em topografia mais inferior, o tarso adquire forma de U em decorrência do dobramento no centro da placa tarsal e da rotação inferior da sua metade superior resultando no entrópio. Este caso ressalta a importância do cuidado quanto a localização da inserção da aponeurose do MLPS, principalmente nos paciente idosos, como forma de evitar o encurvamento vertical do tarso.


Abstract Sixty-nine (69) year old patient with severe upper eyelid entropion following surgical correction of ptosis through levator muscle aponeurosis advancement and reinsertion. The aponeurosis advancement appeared to be much lower than typically intended, and surgical repair was performed via aponeurosis re-fixation into the superior 1/3 of the tarsal plate, with subsequent improvement in the aesthetic and functional outcome, and a satisfied patient. Due to the inferiorly located tarsal sutures, the tarsal plate acquires a U-shape due to a central fold and an inferior rotation of its upper half, resulting in entropion formation. This case highlights the importance of taking great care when advancing the levator muscle in ptosis due to levator aponeurosis dehiscence, particularly in elderly patients, so as to avoid vertically folding the superior tarsal plate.

ABSTRACT Purpose: This study aimed to share the results of patients who underwent anterior tarsal flap rotation combined with anterior lamellar reposition because of cicatricial upper eyelid entropion, and to determine the effectiveness and reliability of this surgical technique. Methods: Fifteen eyes of 11 patients (2 right eyes; 5 left eyes; and 4 bilateral eyes) on whom we performed anterior tarsal flap rotation surgery combined with anterior lamellar reposition because of cicatricial entropion were included in this study. The medical records of the patients were analyzed retrospectively, and the causes of cicatricial entropion as well as the preoperative and postoperative ophthalmic examination findings were recorded. Normal anatomical appearance and function of eyelid were considered to have been achieved. Results: The mean age was 59.81 ± 18 years. The mean follow-up period was 21.72 ± 14 months (range, 5-43 months). The causes of cicatricial entropion were postoperative cicatrices development due to multiple electrolyzes for trichiasis and/or distichiasis in 7 eyes, trachoma in 6 eyes, and trauma in 2 eyes. Irritation and watering were detected in all patients preoperatively, whereas corneal opacity and erosion were detected in 10 patients and epithelial erosion was detected in one patient. Full anatomical and functional success was achieved for all patients. Conclusion: Anterior tarsal flap rotation combined with anterior lamellar reposition in the repair of cicatricial entropion was found to be an effective and reliable alternative surgical procedure.


RESUMO Objetivo: Compartilhar os resultados dos pacientes submetidos à rotação de retalho tarsal anterior, combinados com a reposição lamelar anterior devido à entrópio cicatricial da pálpebra superior e determinar a eficácia e a confiabilidade desta técnica cirúrgica. Métodos: Foram incluídos neste estudo quinze olhos de 11 pacientes em quem realizamos cirurgia de rotação de retalho tarsal anterior combinada com reposição lamelar anterior devido ao entrópio cicatricial. Os registros médicos dos pacientes foram analisados retrospectivamente e as causas da entrópio cicatricial, bem como os achados do exame oftalmológico pré-operatório e pós-operatório foram registrados. A integridade anatômica e funcional da pálpebra foi considerada como sucesso cirúrgico. Resultados: A idade média foi de 59,81 ± 18 anos. O período médio de seguimento foi de 21,72 ± 14 meses (intervalo 5-43 meses). As causas da entrópio cicatricial foram o desenvolvimento de cicatrizes pós-operatórias devido a eletrólises múltiplas para triquíase e/ou distiquiase em 7 olhos, tracoma em 6 olhos e trauma em 2 olhos. Todos os pacientes foram tiveram irritação e lacrimejamento pré-operatório, enquanto que 10 pacientes apresentavam opacidade e erosão da córnea e 1 paciente apresentava apenas erosão epitelial. O sucesso total anatômico e funcional foi alcançado em todos os casos. Conclusão: A rotação do retalho tarsal anterior combinada com a reposição lamelar anterior no reparo da entrópio cicatricial é um procedimento cirúrgico alternativo efetivo e confiável.

ABSTRACT

Objective@#To observe the clinic effect of autologous orbicularis muscle filling combined with eyelid gray line incisionin treating scar upper eyelid entropion.@*Methods@#The clinic data of 36case (58eyes) were analyzed from March 2015 to October 2016. All adopted the autologous orbicularis muscle filling combined with eyelid gray line incision method, and the efficacy of treatment was observed.@*Results@#All of 36 case (58eyes) had success surgery, and all cases had no complications. The cure rate was 91.67%(33/36) and the total effective rate was 100.00%(36/36).@*Conclusions@#The method of autologous orbicularis muscle filling combined with eyelid gray line incision can get good treatment effect and worth popularizing.

ABSTRACT

AIM: To analyze and compare the clinical effect in treatment of degenerative entropion of lower eyelid with blepharochalasis between the modified orbicularis oculi muscle shortening procedure and orbicularis muscle shortening procedure.?METHODS:From January 2012 to August 2014, ninety-three cases ( 108 eyes ) diagnosed as involutional entropion of lower eyelid with blepharochalasis were enrolled in our study. According to different surgical methods, patients were divided into observation group (52 eyes in 47 cases) and control group (56 eyes in 46 cases ) . The observation group was performed on the modified orbicularis muscle shortening procedure, and the control group was using a simple orbicularis muscle shortening method. The general datum, short - term clinical effect and long-term clinical effect were collected to compared between two groups.?RESULTS:The age, gender and the course of disease were no significance differences between the observation group and the control group (P>0. 05). There was no statistical significance on the short-term effective rate ( P>0. 05 ) between the observation group and the control group ( 98% vs 95%, respectively ) . The long - term recurrence rate of the observation group was 2%, while the control group was 14%. It had a significance statistical difference between two groups (P=0. 033).?CONCLUSION: The clinical curative effect of modified orbicularis oculi muscle shortening procedure, in the treatment of lower eyelid entropion with blepharochalasis, is better than the orbicularis muscle shortening procedure's.

ABSTRACT

Objective To analyze the clinical efficacy of patients with lower eyelid entropion after the treatment of Fox surgery,modified Fox surgery and modified orbicularis oculi muscle fold surgery.Methods 250 patients with lower eyelid entropion were retrospectively analyzed.The modified orbicularis oculi muscle fold surgery (group A),Fox surgery (group B),and modified Fox surgery (group C) were used for lower eyelid entropion.Results Postoperatively,No skin redness and swelling occured in group A,and mild skin redness appeared in 8 and 2 cases in groups B and C,respectively;the difference was statistically significant between the three groups (P<0.05);No eyelid hyperemia was found in groups A and C,but 6 cases presented with eyelid margin hyperemia in group B;the difference was statistically significant between the three groups (P < 0.05).Long term follow-up showed that the satisfaction score of group A was (8.23±1.08),B group was (7.54±1.09),C group was (9.26± 1.94),and the difference between three groups was statistically significant (P<0.05).In addition,the long-term recurrence rate showed that the recurrence rate in group C was the lowest,followed by group A,and the recurrence rate in group B was the highest,and the differences between various groups were statistically significant (P<0.05).Conclusions The improved Fox surgery has better satisfaction and lower recurrence rate in patients with lower eyelid entropion,and especially the long-term effect is more definite.

ABSTRACT

Objetivo: evaluar los resultados terapéuticos de la aplicación de toxina botulínica A en pacientes con alteraciones espásticas palpebrales. Métodos: se realizó un estudio observacional, descriptivo, longitudinal y prospectivo en 18 pacientes con estas afecciones, seleccionados en la consulta de Oculoplastia del Instituto Cubano de Oftalmología Ramón Pando Ferrer en el período comprendido entre enero y diciembre del año 2016. Se describió la muestra según la edad, el sexo, el color de la piel, los antecedentes patológicos personales, las afecciones palpebrales y las limitaciones en tareas de la vida diaria. Se evaluó en el posoperatorio el tiempo de desaparición de los síntomas, la duración del efecto de la inyección de toxina botulínica, las complicaciones inmediatas y las mediatas. Resultados: el 70,6 por ciento estuvo comprendido en el rango de edad mayor de 60 años, y el 52,9 por ciento fueron mujeres. La piel blanca constituyó el 58,8 por ciento de la muestra. La afección que se presentó con mayor frecuencia fue el blefaroespasmo (47,1 por ciento). Todos los pacientes presentaron limitaciones para realizar tareas de la vida diaria. El 100 por ciento presentó dolor en el sitio de la inyección. En el 94,1 por ciento de ellos hubo desaparición de los movimientos involuntarios entre el cuarto y el quinto día posterior a la aplicación de xeomeen y en el 88,2 por ciento demoró la reaparición de los síntomas entre cuatro y seis meses. Conclusiones: la toxina botulínica es efectiva en la corrección de las afecciones palpebrales espásticas con desaparición de los síntomas en un período de tiempo entre cuatro y cinco días, con una rápida incorporación del paciente a la vida laboral y social por cuatro a seis meses(AU)


Objective: to evaluate the therapeutic results of the application of toxin botulínica A in patients with palpebral spastic alterations. Methods: an observational, descriptive, longitudinal and prospective study was performed on 18 patients with these conditions, who were selected at the Oculoplasty Service of Ramón Pando Ferrer Cuban Institute of Ophthalmology in the period from January to December 2016. The analyzed variables were age, sex, race, personal pathological history, eyelid disorders and restricted daily life. In the postoperative period, length of time for symptom relief, duration of the effect of botulinum toxin injection, and immediate and mediate complications were assessed. Results: in the group, 70,6 percent were included in the over 60 y age group and 52,9 percent were women. Caucasians accounted for 58,8 percent of the sample. The most frequent condition was blepharospasm (47,1 percent). All patients had limitations to perform daily life tasks. In 94,1 percent of the patients, involuntary movements disappeared between the 4th and the 5th day after the administration of Xeomeen and in 88,2 percent, the symptoms delayed to appear again from 4 to 6 months. Conclusions: botulinum toxin A is an effective alternative to correct palpebral spastic conditions, with symptoms disappearing in four to five days, and quick reincorporation of the patient to working and social life for 4 to 6 months(AU)

@#AIM:To compare the clinical effect of the lower eyelid retractor muscle transposition and eyelid orbicularis muscle folding shorten combined with lower eyelid retractor muscle transposition in the treatment of senile lower eyelid entropion. <p>METHODS: Sixty-four cases(85 eyes)with senile lower entropion were divided into Group A(31 cases 42 eyes)and Group B(33 cases 43 eyes)according to the different ways of operation from January 2013 to October 2014 in our hospital. Patients in Group A were treated by eyelid retractor muscle transposition while patients in Group B treated by eyelid orbicularis muscle folding shortening combined with lower eyelid retractor muscle transposition. The short-term and long-term therapeutic efficacy and two-year recurrence rates after operation were compared between the two groups. <p>RESULTS: The short-term effective rate of patients in Group B was higher than that of Group A while the difference was not statistically significant(98% <i>vs</i> 95%, <i>P</i>>0.05). The long-term effective rate of patients in Group B was higher than that of Group A and the difference was statistically significant(95% <i>vs </i>83%)and two-year recurrence rate of patients in Group B was lower than that of Group A(5% <i>vs</i> 17%)and the difference was statistically significant(<i>P</i><0.05). <p>CONCLUSION:The clinical curative effect of eyelid orbicularis muscle folding shortening combined with lower eyelid retractor muscle transposition is better than single retractor muscle transposition in the treatment of senile lower eyelid entropion.
