ABSTRACT

Abstract Ectopic pregnancy is the leading cause of pregnancy-related death during the first trimester, and it occurs in 1 to 2% of pregnancies. Over 90% of ectopic pregnancies are located in the fallopian tube. Abdominal pregnancy refers to an ectopic pregnancy that has implanted in the peritoneal cavity, external to the uterine cavity and fallopian tubes. The estimated incidence is 1 per 10,000 births and 1.4%of ectopic pregnancies. Lithopedion is a rare type of ectopic pregnancy, and it occurs when the fetus from an unrecognized abdominal pregnancymay die and calcify. The resulting "stone baby" may not be detected for decades andmay cause a variety of complications. Lithopedion is a very rare event that occurs in 0.0054% of all gestations. About 1.5 to 1.8% of the abdominal babies develop into lithopedion. There are only ~ 330 known cases of lithopedion in the world. We describe a lithopedion that complicated as intestinal obstruction in a 71-year-old woman.


Resumo A gravidez ectópica é a principal causa de morte materna no primeiro trimestre, e ocorre em 1 a 2% das gestações. Mais de 90% ocorrem nas tubas uterinas. Gravidez abdominal refere-se à gravidez ectópica implantada na cavidade peritoneal, externamente ao útero e às tubas uterinas.Aincidência estimada éde 1 por 10mil nascimentos e 1,4%das gravidezes ectópicas. A litopedia é um tipo raro de gravidez ectópica, e ocorre quando o feto de uma gravidez abdominal não reconhecida morre e se calcifica. O "bebê de pedra" resultante pode não ser detectado por décadas, e pode causar complicações futuras. A litopedia é um evento muito raro que ocorre em 0,0054% de todas as gestações. Cerca de 1,5 a 1,8% dos bebês abdominais se tornam litopédios. Existem somente cerca de 330 casos conhecidos de litopedia no mundo. Descrevemos uma litopedia que se agravou, tornando-se uma obstrução intestinal, em uma idosa de 71 anos.

ABSTRACT

Primary ovarian pregnancy is a rare form of ectopic pregnancy due to the implantation of the gestational sac in the ovary. It is reported to occur in 1 in 25000-40000 pregnancies following natural conceptions and accounts for 0.3- 3% of all ectopic pregnancies. The preoperative diagnosis is difficult and is mostly diagnosed following surgery or histo pathological examination. Here we present a case of ovarian pregnancy which was managed surgically and its review of the literature is discussed.

ABSTRACT

Emergence of a new species of non-albicans Candida species in Immunocompromised patient is a very alarming and life threatening trend. Accurate identification and prompt treatment in such patients is necessary to curtail mortality. Candida haemulonii as the name suggests is mostly isolated from blood culture. Susceptibility pattern of such isolates is important as most of them are not susceptible to the commonly used azole group of drugs rather they are susceptible to newer drugs like echinocandins. In such patient automated blood culture and susceptibility should be preferred over conventional blood culture so that early diagnosis and treatment can to some extent decrease the percentage of mortality.

ABSTRACT

Primary broad ligament pregnancy is a rare form of extrauterine pregnancy with a high maternal and perinatal mortality. Early diagnosis is important to prevent life threatening hemorrhage but can be missed on antenatal ultrasonography. The diagnosis is usually established during laparotomy. We present a case of 22 year old female with ectopic gestation in broad ligament which was missed on ultrasonography and diagnosed during surgery.

ABSTRACT

Objective To improve the accuracy rate of diagnosis by discussing the ultrasonographic features of extrauterine pregnancy by the transvagiual ultrasonography. Methods To analyze the characteristic of 156 cases which were examined by transvaginal ultrasonography,the features as well as distinguish main point,throuth the ret- rospective method. Results The accuracy rate is 94.3 % in this group. Conclusion There are many styles of ultra- sonographic .features,which examined by transvaginal ultrasonography in the extrauterine pregnancy. And different from one to another. Itis in evidence to improve the accuracy rate of diagnosis by using transvaginal ultrasono- graphy. The differentiate diagnosis are related to the history of sickness and the mutuality test and check.

ABSTRACT

Objective To observe the curative effect of Modified Gongwaiyun Ⅱ on extrauterine pregnancy.Methods 69 patients of extranterine pregnancy were recruited into a treatment group and a control group randomly. 34 patients in the treatment group were treated with modified Gongwaiyun Ⅱ and 35 patients in the control group were treated with Gongwaiyun Ⅱ, with therapeutic course being one week. Values of β-HCG of serum at the beginning of the treatment,age, days of menelipsis, effective rate, average value of serum β-HCG decreasing and percentage at every course of the treatment, and the number of therapeutic courses were observed. Results There are significant statistic differences between the treatment group and the control group at the number of therapeutic courses, and average value of serum β-HCG decreasing and percentage at every course of the treatment, with P<0.05. Conclusion The curative effect of modified Gongwaiyun Ⅱ had superiority to traditional Gongwaiyun Ⅱ.

Se presenta un caso de una paciente que consulta por dolor abdominal agudo que es intervenida quirúrgicamente con extirpación de una masa que compromete el anexo derecho. El estudio anatomopatóligo confirma embarazo ovárico.


A case of a patients is presented that consults for sharp abdominal pain that is intervened surgically with extirpation of a mass that commits the right annex. Histological preparations showed confirms ovarian pregnancy.

El embarazo ectópico se ha convertido en un problema de salud a nivel mundial, debido a que afecta a un gran número de mujeres en edad reproductiva; su frecuencia ha aumentado de manera espectacular en las dos últimas décadas, por lo cual es necesario establecer una conducta adecuada en base al diagnóstico clínico que puede ser realizado por el médico general hasta el gineco-obstetra.El embarazo ectópico complicado es aún la causa más común de muerte materna en la primera mitad de la gestación y la segunda más común en total. Sin embargo, debido a los adelantos en los métodos diagnósticos y terapéuticos como los que ya se están realizando por medio de técnicas microquirúrgicas y farmacológicas, se ha logrado disminuir la mortalidad materna y conservar la fertilidad.Las enfermedades de transmisión sexual son un factor de riesgo importante para el desarrollo de esta patología, ya sea por la frecuencia de sus secuelas o sus efectos en la permeabilidad tubaria.Actualmente muchos casos de embarazo ectópico se diagnostican en fases tempranas cuando los síntomas son escasos o nulos y la trompa aún está íntegra y si el trofoblasto está en regresión; este puede tratarse con Metrotexate o experimentar una regresión espontánea y ser candidato a vigilancia médica.


Ectopic pregnancy has turned into a world wide health issue, considering that a great deal of women in the reproductive age are affected; its frequency has increased in the last 2 decades, reason why it is necessary to establish an adequate conduct in regards to the clinical diagnosis which can be performed by the general physician or the gynecologist.Complicated ectopic pregnancy is still a leading cause of maternal death during the first half of the pregnancy and second most common when all stages of pregnancy are considered. However, due to advances in diagnostic and therapeutical methods as those performed through microsurgical and pharmacological techniques, maternal mortality has decreased and fertility conserved. Sexually transmitted diseases are important risk factors for the development of this entity, Esther due to the frequency of its complications as due to tubal permeability.Currently many cases of ectopic pregnancy are diagnosed at early stages when symptoms are mild or null and the Fallopian tube is still intact and if the trophoblast is in a regressive stage; this can be treated with methrotexate or experience spontaneous regression or be a candidate for medical surveillance.
