ABSTRACT

Analisar as concepções sobre competição esportiva em publicações acadêmicas brasileiras é o foco deste trabalho. Utilizando-se da revisão sistemática, do tipo estado da arte, foram analisados 175 artigos no recorte temporal 2010-2020 (11 anos) em 14 revistas da área da Educação Física. Há parcimônia dos/as autores/as quanto a complexidade da competição e foram salutares indicativas do rememorar a essência do tema; independente da área de conhecimento específico, foram recorrentes indicativos de um processo respeitoso e pedagógico da competição aos praticantes. Ademais, confirmam que amadores e profissionais; homens e mulheres; idosos e jovens, todos podem (e devem) ter a opção da experiência esportiva no âmbito competitivo (AU).


Analyzing the concepts about sports competition in Brazilian academic publication is the objective of this paper. Using the state-of-art systematic review, 175 articles were considered from 14 Physical Education Journals from 2010 to 2020 (11 years). There is parsimony by the authors regarding the complexity of the competition and greetings indicative of remembering the essence of the theme; regardless of the area of specific knowledge, there were recurrent indicators of a respectful and pedagogical process of the competition for practitioners. Also, it was confirmed that amateurs and professionals; men and women; elderly and young; everyone can (and should) have the possibility of sporting experience in the competitive environment (AU).

Hay parcimônia a la complejidad de la competición y fueron saludos indicativos de recordar la esencia del tema; independientemente del área de conocimiento específico, fueron recurrentes indicativos para un proceso respetuoso y pedagógico de la competencia por practicantes. También confirman que los atletas aficionados y profesionales; hombres y mujeres; ancianos y jóvenes, todos pueden (y deben) tener la opción de la experiencia deportiva en el entorno competitivo

ABSTRACT

Esta tese visa identificar e analisar fatores associados à insegurança alimentar (IA) e à mortalidade na infância, apontando tendências e distribuições espaciais para estratos de menores níveis de agregação possíveis que em conjunto tenham abrangência em todo o território brasileiro. As análises investigaram a hipótese de piora do desfecho de IA grave, medido pela Escala Brasileira de Insegurança Alimentar (EBIA) e desfecho de morte antes de completar 5 anos, como efeitos da crise e/ou da política de austeridade. O trabalho envolve estudos com desenhos transversais para análises seccionais e ecológico misto de tendências espaçotemporais, a partir de dados de 4 inquéritos nacionais do IBGE, dos sistemas de informações em saúde do Ministério da Saúde para os desfechos e dados das estatísticas do registro civil do IBGE, de indicadores do Programa Bolsa Família (PBF), da Estratégia Saúde da Família (ESF) e de outros indicadores socioeconômicos de variadas fontes, todos como exposições representantes proxies da crise e da austeridade. Para os estudos seccionais adotou-se procedimento de calibração dos pesos segundo distribuição populacional por sexo e faixa etária, assim como a aplicação de métodos de estimação e modelagem que incorporam os efeitos do desenho amostral. A regressão de Poisson com estimação robusta de variância foi empregada para estimar em nível etiológico razões de prevalências de IA grave. Para o nível ecológico, empregou-se duas abordagens de modelagem multinível para 4 ou 14 medidas repetidas de estratos ao longo de dois artigos diferentes: regressão múltipla do tipo log-log para associações e modelagem de splines cúbicas para estimação de tendências. Os achados apontam impactos da austeridade em vigor, com inflexões de tendências no PBF e ESF, com reflexos sobre o aumento da IA grave e da incidência de mortalidade na infância. Projeta-se o aumento da IA e afastamento do alcance da meta 2.1 do objetivo nº 2 dos ODS em 2030 pelo Brasil, a despeito do sucesso já alcançado em 2014 para o primeiro dos ODM. Aponta-se também que, no prosseguindo na rota austera estabelecida pela emenda do teto dos gastos, o Brasil poderá continuar caminhando em sentido oposto ao estabelecido pela meta 3.2 dos ODS, podendo não ter êxito no seu alcance em 2030.


This thesis aims to identify and analyze factors associated with Food Insecurity (FI) and childhood mortality, pointing out trends and spatial distributions for strata with the lowest possible levels of aggregation that together cover the entire Brazilian territory. The analyzes investigated the hypothesis of worsening of the severe FI outcome, measured by the Brazilian Food Insecurity Scale (BFIS) and death outcome before completing 5 years, as effects of the crisis and/or austerity policy. The work involves studies with cross-sectional designs for sectional and mixed ecological analysis of spatio-temporal trends, based on data from 4 national surveys by the IBGE, from the health information systems of the Ministry of Health for outcomes, and data from statistics from the civil registry from the IBGE, from indicators from the Bolsa Família Program (BFP), from the Family Health Strategy (FHS) and from other socioeconomic indicators from various sources such as exposures that are proxies representatives of the crisis and austerity. For the sectional studies, a procedure was adopted to calibrate the weights according to population distribution by sex and age group, as well as the application of estimation and modeling methods that incorporate the effects of the sample design. Poisson regression with robust variance estimation was used to estimate at the etiological level the prevalence ratios of severe FI. For the ecological level, two multilevel modeling approaches were used for 4 or 14 repeated measures of strata over two different articles: log-log multiple regression for associations; and, modeling cubic splines for trend estimation. The findings point to the impacts of the austerity in force, with inflections of trends in the BFP and FHS, with reflections on the increase in severe FI and the incidence of mortality in infancy. It is projected that FI will increase and move away from the achievement of target 2.1 of goal 2 of the SDGs in 2030 by Brazil, despite the success already achieved in 2014 for the first of the MDGs. It is also pointed out that, by continuing on the austere route established by the spending ceiling amendment, Brazil may continue to move in the opposite direction to that established by target 3.2 of the SDGs, and may not be successful in reaching it in 2030.

ABSTRACT BACKGROUND: Receptors of living donor liver transplantation (LDLT) have higher rate of postoperative biliary and vascular complications that may reduce posttransplant quality of life (QOL) due to the need of invasive and repetitive treatments. OBJECTIVE: The purpose of our study is to assess the various aspects of QOL of receptors undergoing LDLT after 10 years of transplantation and to identify potential factors that might be associated with impaired QOL. METHODS: Data of all patients with more than 10 years of LDLT were retrospectively evaluated. Patients were interviewed through a quality of life questionnaire (SF-36). RESULTS: From a total of 440 LT performed in 17 years (from September 1991 through December 2008), 78 patients underwent LDLT, of which 27 were alive and 25 answered completely the questionnaire. There were 17 (68%) men and 8 (32%) women, with a mean age of 38.6±18.5 years at the time of transplantation and mean follow up time of 15.1±1.9 years. The average MELD was 16.4±4.9 and the main indication for LT was hepatic cirrhosis caused by hepatitis B virus (32%). When compared to the general po­pulation, LDLT patients had lower mental health score (66.4 vs 74.5, P=0.0093) and higher vitality score (87.8 vs 71.9, P<0.001), functional aspects (94.6 vs 75.5, P=0.002), social aspects (93 vs 83.9, P=0.005), physical aspects (92 vs 77.5, P=0.006), and emotional aspects (97.33 vs 81.7, P<0.001). General health status (73.28 vs 70.2, P=0.074) and pain (78.72 vs 76.7, P=0.672) scores were similar in both groups. CONCLUSION: It is concluded that the various aspects LDLT recipients' QOF are similar to those of the general population more than a decade after the transplant, except for the mental health domain which is lower.


RESUMO CONTEXTO: Receptores de transplante hepático inter-vivo (THIV) apresentam elevada taxa de complicações biliares e vasculares pós-operatórias que podem reduzir a qualidade de vida (QV) devido à necessidade de tratamentos invasivos e repetitivos. OBJETIVO: O objetivo deste estudo é avaliar os vários aspectos da qualidade de vida dos pacientes submetidos a THIV após 10 anos de transplante e identificar possíveis fatores que possam estar associados à diminuição da QV. MÉTODOS: Os dados de todos os pacientes com mais de 10 anos de THIV foram avaliados retrospectivamente. Os pacientes foram entrevistados por meio de um questionário de qualidade de vida (SF-36). RESULTADOS: Do total de 440 transplantes hepáticos realizados em 17 anos (setembro de 1991 a dezembro de 2008), 78 pacientes foram submetidos a THIV, dos quais 27 estavam vivos e 25 responderam completamente ao questionário. Destes, 17 (68%) homens e 8 (32%) mulheres, com idade média de 38,6±18,5 anos no momento do transplante e tempo médio de acompanhamento de 15,1±1,9 anos. O MELD médio foi de 16,4±4,9 e a principal indicação para o transplante hepático foi cirrose hepática causada pelo vírus da hepatite B, 32%. Quando comparado com a população geral, os pacientes submetidos a THIV apresentaram menor escore de saúde mental (66,4 vs 74,5; P=0,0093) e escores mais altos de vitalidade (87,8 vs 71,9; P<0,001), aspectos funcionais (94,6 vs 75,5; P=0,002), aspectos sociais (93 vs 83,9; P=0,005), aspectos físicos (92 vs 77,5; P=0,006), e aspectos emocionais (97,33 vs 81,7; P<0,001). Os escores do estado geral de saúde (73,28 vs 70,2; P=0,074) e de dor (78,72 vs 76,7; P=0,672) eram similares nos dois grupos. CONCLUSÃO: Conclui-se que os vários aspectos da QV dos receptores de transplante hepático inter-vivo são semelhantes aos da população geral mais de uma década após o transplante, exceto o domínio da saúde mental que é menor.

ABSTRACT

OBJECTIVE:To study the effect and potential mechanism of the total flavonoids from Marchantia convoluta on anti-hepatic fibrosis in the mice. METHODS :Seventy-two mice were randomly divided into blank group ,model group ,positive control group (colchicine 0.2 mg/kg)and M. convoluta total flavonoids high-dose ,medium-dose and low-dose groups (300,150, 75 mg/kg),with 12 mice in eac group. Except for blank group ,other groups were subcutaneously given 25% CCl4-peanut oil solution on the back to induce liver fibrosis model. At the same time ,blank group and model group were given water intragastrically,while other groups were given relevant medicine intragastrically 20 mL/kg,once a day ,for consecutive 10 weeks. After last administration ,the serum levels of ALT and AST were detected . Histopathological changes of liver tissue in mice was observed. The levels of COL- Ⅰ,COL-Ⅲ and TGF-β1 in liver tissue were detected . The protein expression levels of α-SMA and TGF-β1,Smad2,Smad4 and Smad 7 in liver tissue were detected . The expression levels of TGF-β1,Smad2,Smad4 and Smad 7 mRNA in liver tissue were detected . RESULTS :Compared with blank group ,the serum levels of ALT and AST in model group,the levels of COL- Ⅰ,COL-Ⅲ and TGF-β1 in liver tissue,protein expression levels of α-SMA,TGF-β1,Smad2 and Smad 4,mRNA expression levels of TGF-β1,Smad2 and Smad4 were increased significantly (P<0.05 or P<0.01).The mRNA and protein expression levels of Smad 7 in liver tis sue were decreased significantly (P<0.05). The degree of liver tissue injury and collagen fiber hyperplasia were serious. Compared with model group ,above indexes of mice were reversed significantly in positive control group and M. convoluta total flavonoids high-dose group (P<0.05 or P<0.01). Serum level of ALT ,the levels of COL- Ⅰ,mRNA and protein expression of TGF-β1,Smad2 and Smad 4 in liver tissue were decreased significantly in M. convoluta total flavonoids medium-dose group (P<0.05 or P<0.01). Protein expression of Smad 2 and Smad 4 in liver tissue were decreased significantly in M. convoluta total flavonoids low-dose group (P<0.05). The liver injury and fibrosis of mice were relieved in administration groups. CONCLUSIONS :M. convoluta total flavonoids possess the effect of anti-hepatic fibrosis ,the mechanism of which is related to the regulation of mRNA and protein expression of TGF-β1,Smad2,Smad4 and Smad 7 in the signaling pathway of TGF-β/Smad.

ABSTRACT

Objective: To investigate the effect of lipopolysaccharide (LPS) on the expressions of epithelial-mesenchymal transition (EMT) markers and fi-catenin in the breast cancer MDA-MB-231 cells, and to clarify its possible mechanism. Methods: The breast cancer MDA-MB-231 cells were divided into control group and different concentrations (5, 10, 20, and 40 mg • L _ 1 ) of LPS groups. Inverted microscope was used to observe the morphology of MDA-MB-231 cells in various groups. Immunofluorescence test was used to detect the β-catenin expression and location in the MDA-MB-231 cells in various groups. Real-time quantitative PCR (RT-qPCR) and Western blotting methods were used to detect the expression levels of the E M T markers E-cadherin, Vimentin and β-catenin mRNA and proteins in the MDA-MB-231 cells in various groups. Results: The morphology of MDA-MB-231 cells in control group was epithelial phenotype, and the morphology of MDA-MB-231 cells in different concentrations of LPS groups were the phenotype of mesenchymal cells. The results of immunofluorescence staining showed that the expression of β-catenin was mainly located in the nucleus. Compared with control group, the expression levels of Vimentin and β-catenin mRNA and proteins in the MDA-MB-231 cells in different concentrations of LPS groups were increased (P < 0. 05 or P < 0. 01), especially in 20 mg • L _ 1 LPS group. Compared with control group, the expression levels of E-cadherin mRNA and proteins in the MDA-MB-231 cells in different concentrations of LPS groups were decreased (P < 0. 05 or P < 0. 01), especially in 20 mg • L _ 1 LPS group. Conclusion: LPS could promote the E M T, invasion and metastasis of the breast cancer MDA-MB-231 cells by down-regulating the E-cadherin expression and up-regulating the Vimentin expression, and its mechanism may be related to Wnt/ fj-catenin signaling pathway.

ABSTRACT Purpose: To evaluate three superior oblique surgical weakening procedures for correcting A-pattern strabismus: tenectomy, superior oblique hang-back recession, and that involving the use of superior oblique suture spacers. Methods: The inclusion criteria were A-pattern strabismus ≥10Δ and horizontal deviation ≥10Δ, with no other ocular abnormality and a follow-up period of ≥6 months. The 24 patients (mean age, 16.3 ± 8.1 years; mean postoperative follow-up, 9.63 ± 3.11 months) were randomly divided into three groups of 8 patients each. Sigmascan® Pro 5.0 software was used to measure the degree of torsion pre- and postoperatively. Results: Preoperatively, the mean angles of A-pattern deviation were 19.33Δ° ± 3.53Δ° (tenectomy group), 15.71Δ° ± 1.11Δ° (hang-back recession group), and 14.62Δ° ± 1.18Δ° (suture spacers group); these values did not differ significantly. At the final follow-up examination, the mean angles of A-pattern deviation were 4.67Δ° ± 0.67Δ° (tenectomy group), 6.29Δ° ± 1.48Δ° (hang-back recession group), and 4.38Δ° ± 1.03Δ° (suture spacers group), with no statistically significant difference in the correction in A-pattern strabismus among the three groups. Preoperatively, the mean torsional angles were +5.4° ± 3.9° (tenectomy group), +5.6° ± 4.9° (hang-back recession group), and +6.0° ± 3.3° (suture spacers group); these values did not differ significantly. At the final follow-up examination, the mean torsional angles were + 0.3° ± 5.6° (tenectomy group), +0.5° ± 4.6° (hang-back recession group), and +0.2° ± 5.2° (suture spacers group), with no statistically significant difference in the intorsion correction among the three groups. Conclusion: All three superior oblique weakening procedures were effective for correcting A-pattern strabismus and fundus intorsion.


RESUMO Objetivo: Avaliar três procedimentos de debilitamento dos músculos oblíquos superiores para a correção de estrabismo com padrão em A: tenectomia, sutura em rédea (hang-back recession) e o uso de espaçadores de sutura oblíqua superior. Métodos: Os critérios de inclusão foram estrabismo padrão em A ≥10Δ e desvio horizontal ≥10Δ, sem outras anormalidades oculares e tempo de acompanhamento ≥6 meses. Os 24 pacientes (média de idade de 16,3 ± 8,1 anos; média de seguimento pós-operatório de 9,63 ± 3,11 meses) foram divididos aleatoriamente em três grupos de 8 pacientes cada. O programa Sigmascan® Pro 5.0 foi utilizado para medir o grau de torção no pré e pós-operatório. Resultados: No pré-operatório, a média e o desvio padrão dos ângulos de padrão em A foram de 19,33Δ° ± 3,53Δ° (grupo da tenectomia), 15,71Δ° ± 1,11Δ° (grupo da sutura em rédea), 14,62Δ° ± 1,18Δ° (grupo de espaçadores de sutura); esses valores não diferiram significativamente. No exame pós-operatório, a média e o desvio padrão dos ângulos de desvio do padrão em A foram de 4,67Δ° ± 0,67Δ° (grupo da tenectomia), 6,29Δ° ± 1,48Δ° (grupo da sutura em rédea), 4,38Δ° ± 1,03Δ° (grupo de espaçadores de sutura), sem diferença estatisticamente significativa na correção do estrabismo padrão em A entre os três grupos. No pré-operatório, os ângulos médios de torção foram de +5,4o ± 3,9° (grupo de tenectomia), +5,6° ± 4,9° (grupo da su­tura em rédea), e +6,0° ± 3,3° (grupo de espaçadores de sutura), esses valores não diferiram significativamente. No pós-operatório, os ângulos médios de torção foram de +0,3° ± 5,6° (grupo da tenectomia), +0,5° ± 4,6° (grupo da sutura em rédea), e +0,2° ± 5,2° (grupo de espaçadores de sutura), sem diferença estatisticamente significativa na correção da intorção entre os três grupos. Conclusão: Os três procedimentos de debili­tamento dos músculos oblíquos superiores foram efetivos para a correção do estrabismo com padrão em A e da intorção ocular observada na fundoscopia.

ABSTRACT

With the development of Wi-Fi technology and widespread exposure to electromagnetic radiation (EMR), people are increasingly concerned about the health hazards caused by radiofrequency electromagnetic fields as from cellphones and Wi-Fi, particularly about the current decline in sperm concentration and increase in male infertility. Long-term exposure to EMR not only damages male reproductive organs, but also affects the number, morphology, motility and oocyte-binding ability of sperm, and indirectly increases the risk of infertility. However, EMR is not unavoidable. Low-intensity short-term or intermittent exposure to EMR has little adverse effect on reproductive organs and sperm. And many antioxidant and anti-free radical agents, such as vitamin E and melatonin, can protect some special populations from EMR. This review presents an overview of the impacts of EMR from cellphones and Wi-Fi on sperm, some countermeasures, and prospects of EMR protection.

ABSTRACT

Objective To investigate the effect of Taurine on the differentiation of neural stem cells(NSCs)in fetal rats with intrauterine growth restriction( IUGR),and to explore the neuroprotective mechanism of Taurine. Methods IUGR fetal rats models were established with low protein diet. NSCs from subventricular zone( SVZ)were isolated and cultured in υitro. The NSCs were divided into 3 groups:normal control group,IUGR group,and IUGR+Taurine group. The cells were examined by adopting immunofluorescence for counting βⅢ-tubulin protein,glial fibril-lary acidic protein(GFAP)and myelin basic protein(MBP)-positive cells. Protein levels of βⅢ-tubulin and GFAP were detected by using Western blot. Results (1)The number of βⅢ-tubulin protein positive cells in normal control group,IUGR group and IUGR+Taurine group were(18. 50 ± 0. 64)%,(15. 61 ± 0. 76)% and(18. 42 ± 0. 82)%, respectively;the number of GFAP positive cells in normal control group,IUGR group and IUGR+Taurine group were (72. 19 ± 0. 82)%,(74. 87 ± 0. 67)% and(71. 68 ± 0. 92)%,respectively;and the differences were statistically sig-nificant(F=49. 103,44. 643,all P<0. 01). Compared with the normal control group,βⅢ-tubulin protein positive cells in IUGR group decreased significantly(P<0. 01),but GFAP positive cells in IUGR group increased significantly (P<0. 01). Compared with IUGR Group,βⅢ-tubulin protein positive cells in IUGR+Taurine group increased sig-nificantly(P<0. 01),but GFAP positive cells in IUGR+Taurine group decreased significantly(P<0. 01). There was no significant difference between groupⅠand groupⅢ(all P>0. 05).(2)The levels of βⅢ-tubulin protein in nor- mal control group,IUGR group and IUGR+Taurine group were 0. 44 ± 0. 02,0. 33 ± 0. 03 and 0. 42 ± 0. 02,respective-ly;and the levels of GFAP protein in normal control group,IUGR group and IUGR+Taurine group were 1. 13 ± 0. 02, 1. 50 ± 0. 04,1. 21 ± 0. 01,respectively;and the differences were statistically significant(F=45. 191,234. 525,all P<0. 01). Compared with normal control group,the levels of βⅢ-tubulin protein in IUGR group decreased significantly (P<0. 01),but the levels of GFAP in IUGR group increased significantly(P<0. 01). Compared with IUGR group, the levels of βⅢ-tubulin protein in IUGR+Taurine group increased significantly(P<0. 01),but the levels of GFAP in IUGR+Taurine group decreased significantly(P<0. 01). There was no significant difference between normal control group and IUGR+Taurine group(all P>0. 05). Conclusions Taurine can promote the differentiation of NSCs toward neurons in IUGR fetal rats,and maintain the normal proportion of all differentiated cells.

ABSTRACT

Transforming growth factor-β1(TGF-β1)is an important factor in tissue fibrosis. TGF-β1 directly ac- tivates Smad signaling which triggers the overexpression of profibrotic genes. A large number of studies have shown that the dysregulation of TGF-β1/Smad pathway is an important pathogenic mechanism in tissue fibrosis. Smad2 and Smad3 are the two major downstream regulators that promote TGF-β1-mediated tissue fibrosis,whereas Smad7 acts as a nega- tive feedback regulator of the TGF-β1/Smad pathway,blocking TGF-β1-mediated fibrosis. In addition,the recent re- searches have shown that microRNA(miRNA)can regulate the TGF-β1/Smad signaling pathway,and affect the process of tissue fibrosis. This article reviews the role of TGF-β1/Smad signaling pathway in renal,hepatic,pulmonary and myo- cardial fibrosis,as well as the regulation of TGF-β1/Smad signaling pathway by miRNA.

Aim To observe the effectof paeoniflorin (PA) on diabetic wound healing and its mechanisms. Methods A full-thickness skin excision splint model in diabetic mice was used to study the effects of PA on diabetic wound healing and the underlying mechanism. HE staining and Masson staining were used to observe the changes of granulation tissue and collagen in wound tissues. Immunofluorescence technique was employed to detect angiogenesis in wound tissues of diabetic mice. To detect the effects of PA, human umbilical vein endothelial cells ( HUVECs) and mouse fibroblasts were cultured in vitro. MTS, BrdU and scratch assays were used to detect the effects of PA on proliferation and migration. Tube formation assays were used to detect the effects of PA on the tube formation of HUVECs. qPCR was applied to assess the expression of collagen HI, fibronectin and ot-SMA gene. Immunoflu orescence was used to detect the expression of a-SMA. Results Compared with db/db model group, the formation rate of granulation tissues and collagen and the capillary density around the wound of db/db model group treated with PA was significantly higher than that of db/db model group(P <0. 01 ). PA had no significant effect on endothelial cell proliferation, but it could markedly promote endothelial cell migration and tube formation. PA could significantly up-regulate the migration , proliferation, secretion and differentiation of fibroblasts. Conclusions PA can significantly promote diabetic wound healing, which may be related to accelerating the generation of extracellular matrix and promoting angiogenesis.

ABSTRACT

Objective: To evaluate the efficacy of rotary Kedo-S and hand K-files in shaping ability in primary canines using cone beam computed tomography. Material and Methods: Thirty extracted primary maxillary and mandibular canines were selected. Using cone beam computed tomography the teeth were scanned before instrumentation. In Group I, the teeth were prepared using stainless steel K file up to the size of 40. In the Group II teeth, U1 size Kedo-S rotary file was used in crown down technique. The instrumented teeth were scanned again with cone beam computed tomography and the images were compared. Results: The canal taper was more conical for rotary Kedo-S files as compared to K-files, which was statistically significant. Conclusion: Canal preparation with Rotary files resulted in more conical canals when compared to manual instrumentation in primary teeth that contributes to more uniform obturation. (AU)


Objetivo: Avaliar a eficácia da instrumentação rotatória com limas Kedo-S e instrumentação manual com limas Kerr na modelagem de caninos decíduos utilizando tomografia computadorizada de feixe cônico (TCFC). Material e Métodos: Foram selecionados 30 caninos decíduos superiores e inferiores extraídos. Usando a TCFC, os dentes foram escaneados previamente a instrumentação. No Grupo I, os dentes foram confeccionados com limas de aço inoxidável até o tamanho 40. Nos dentes do Grupo II, utilizou-se a lima rotatória Kedo-S tamanho U1 na técnica coroaápice. Os dentes instrumentados foram novamente escaneados por TCFC e as imagens foram comparadas. Resultados: A conicidade do canal foi maior no grupo com instrumentação rotatória em comparação com a instrumentação manual, com diferença estatística significante entre eles. Conclusão: O preparo do canal com o Rotary resulta em canais mais cônicos em comparação com a instrumentação manual em dentes decíduos, o que contribui para uma obturação mais uniforme.(AU)

ABSTRACT

Anesthetic agent propofol needs to be administered at an appropriate rate to prevent hypotension and postoperative adverse reactions. To comprehend more suitable anesthetic drug rate during surgery is a crucial aspect. The main objective of this proposal is to design robust automated control system that work effi ciently in most of the patients with smooth BIS and minimum variations of propofol during surgery to avoid adverse post reactions and instability of anesthetic parameters. And also, to design advanced computer control system that improves the health of patient with short recovery time and less clinical expenditures. Unlike existing research work, this system administrates propofol as a hypnotic drug to regulate BIS, with fast bolus infusion in induction phase and slow continuous infusion in maintenance phase of anesthesia. The novelty of the paper lies in possibility to simplify the drug sensitivity-based adaption with infusion delay approach to achieve closedloop control of hypnosis during surgery. Proposed work uses a brain concentration as a feedback signal in place of the BIS signal. Regression model based estimated sensitivity parameters are used for adaption to avoid BIS signal based frequent adaption procedure and large off set error. Adaptive smith predictor with lead–lag fi lter approach is applied on 22 diff erent patients' model identifi ed by actual clinical data. The actual BIS and propofol infusion signals recorded during clinical trials were used to estimate patient's sensitivity parameters EC50 and λ. Simulation results indicate that patient's drug sensitivity parameters based adaptive strategy facilitates optimal controller performance in most of the patients. Results are obtained with proposed scheme having less settling time, BIS oscillations and small off set error leads to adequate depth of anesthesia. A comparison with manual control mode and previously reported system shows that proposed system achieves reduction in the total variations of the propofol dose. Proposed adaptive scheme provides better performance with less oscillation in spite of computation delay, surgical stimulations and patient variability. Proposed scheme also provides improvement in robustness and may be suitable for clinical practices.

ABSTRACT

Nutrient foramen (NF) conducts blood vessels which are chief nutritional source to bones. The anatomicalknowledge of nutrient foramina plays vital role in certain operative procedures to preserve the circulation.Details of NF in metacarpals and metatarsals is essential in reconstruction surgeries. This study designed toassess the architecture of nutrient foramina over metacarpal and metatarsal shafts. For this, a total 400metacarpal and 400 metatarsal bones were collected. All bones are examined for details of number, position,location and direction of nutrient foramina on shaft. Foraminal index was calculated by using Hughes formula.Majority bones are having single nutrient foramen (79.7% metacarpals, 79.5% metatarsals), double nutrientforamina was situated in very few metacarpals and metatarsals. Nutrient foramina are situated over medialsurface of 1st & 2nd metacarpals and 4th &5th metatarsal bones, whereas other bones have nutrient foramina overlateral surface of shaft. The anatomical and morphological details of nutrient foramina is beneficial for orthopedicand plastic surgeries during vascularized bone microsurgery and in bone grafts.

Resumen Introducción: la enfermedad de hígado graso no alcohólico (NAFLD) constituye un problema de salud pública asociado con el síndrome metabólico; su patogénesis implica el inicio de una cascada de señalización bioquímica compleja y su estimulación continua podría consolidar un proceso de fibrogénesis en el tejido. El objetivo del estudio fue analizar expresión de genes implicados en daño hepático, en los procesos iniciales de la lesión en pacientes con NAFLD o con factores de riesgo relacionados a esta patología, en búsqueda de biomarcadores moleculares útiles a la práctica clínica tales como TGF-pi, COL1A2 y MMP20. Metodología: estudio analítico de corte transversal. Se estudiaron características epidemiológicas, bioquímicas, y expresión génica de TGF-pU, COL1A2 y MMP20 en tejido hepático, en individuos con factores de riesgo para NAFLD. Resultados: se incluyeron 83 participantes con factores de riesgo asociados a NAFLD, 22 individuos (26.5%) fueron diagnosticados con NAFLD mediante ultrasonografía. Los factores de riesgo hallados fueron hipertensión arterial (50.6%), obesidad (49.4%), diabetes mellitus (34.9%) y dislipidemia (21.7%). La dislipidemia fue significativamente asociada con el riesgo de desarrollar NAFLD (OR=4; p=0.011). Se encontraron diferencias significativas para colesterol total (p<0.05); y una expresión génica de TGF--31 (con NAFLD p<0.0001 y sin NAFLD p<0.0001 frente al control) y COL1A2 (con NAFLD p=0.002 y sin NAFLD p=0.955 frente al control) con un patrón de expresión creciente a mayor grado de lesión hepática. Conclusión: para concluir, sugerimos activación de las vías de señalización que conducen a fibrogénesis en individuos con factores de riesgo para NAFLD, y mucho más acentuada en pacientes con NAFLD.


Abstract Introduction: nonalcoholic fatty liver disease (NAFLD) is a public health problem associated with the metabolic syndrome; its pathogenesis implies the start of a complex biochemical signaling cascade and its continuous stimulation could consolidate a fibrogenesis process in the tissue. The aim of the study was to analyze expression of genes involved in liver damage in the initial processes of the lesion in patients with NAFLD or with risk factors related to this pathology, in search of molecular biomarkers useful to clinical practice such as TGF--31, COL1A2 and MMP20. Methodology: cross-sectional analytical study. Epidemiological, biochemical, and gene expression characteristics of TGF--31, COL1A2 and MMP20 in liver tissue in individuals with risk factors for NAFLD were studied. Results: 83 participants with risk factors associated to NAFLD were included; 22 individuals (26.5%) were diagnosed with NAFLD by ultrasonography. The risk factors found were hypertension (50.6%), obesity (49.4%), diabetes mellitus (34.9%) and dyslipidemia (21.7%). Dyslipidemia was significantly associated with the risk of developing NAFLD (OR = 4; p = 0.011). Significant differences were found for total cholesterol (p <0.05); and a gene expression of TGF-P1 (with NAFLD p <0.0001 and without NAFLD p <0.0001 versus control) and COL1A2 (with NAFLD p = 0.002 and without NAFLD p = 0.955 versus control) with a pattern of increasing expression at higher degree of liver injury. Conclusion: to conclude, we suggest activation of the signaling pathways that lead to fibrogenesis in individuals with risk factors for NAFLD, and much more accentuated in patients with NAFLD.

ABSTRACT

Four new triterpene saponins, mandshunosides F-I (1-4), together with five known compounds (5-9), were isolated from the roots and rhizomes of Clematis mandshurica. Their structures were elucidated on the basis of spectroscopic evidences and hydrolysis products. Bisdesmosidic saponin (3-9) showed modest suppression of NO production with the inhibition ratios in the range of 51.3%- 64.6% at 50 μmol·L, whereas monodesmosidic saponins with a free carboxyl group at C-28 (1 and 2) showed potent inhibitory activities with IC values being 12.7 and 8.3 μmol·L, respectively.

ABSTRACT

Introduction@#Postprandial lipemia represent an important risk factor for lifetime development of cardiovascular disease in patients with type 2 diabetes mellitus. Daily administration alone or combined statin and fi brate therapy has been shown to be an effective therapeutic approach but brings about serious logistics problem in our local setting. To address this concern, we report this observation where alternate day statin and fi brate treatment given alone or in combination in type 2 diabetes mellitus and similar effectiveness in lowering postprandial dyslipidemia has been obtained.@*Methodology@#This is a retrospective case study in an endocrine clinic involving 53 patients seen from April 2014 to October 2015. The patients were on statin and fi brate combination (atorvastatin 20- 40mg and gemfi brozil 300-600 mg or fenofi brate 145-160mg), statin alone (atorvastatin 20-40mg) and fi brate alone (gemfi brozil 300-600mg/fenofi - brate 145-160mg) given on alternate days. Percent reductions of cholesterol, triglycerides, LDL for combined statin and fi brate; cholesterol and LDL for atorvastatin alone; and triglyceride for fi brate alone were determined.@*Results@#In this preliminary report, 26 patients have available data. Follow-up period range was 4 to 48 weeks (mean 22.76+ 11.8 weeks). Alternate statin and fi brate (gemfi brozil) treatment yielded percent reductions from baseline as follows: cholesterol 7%, triglycerides 15%, and LDL 37% (P values= 0.02, 0.10 and 0.019, respectively). On the other hand, alternate statin and fi brate (fenofi brate) yielded percent reduction from baseline as follows: cholesterol 41% and LDL 20.4% (P=0.15 and 0.13, respectively). The population is small, the decrease did not yield signifi cant difference from baseline, however there is a tendency for triglyceride to decrease (P=0.09) with the combined statin and fenofi brate. With statin alone the percent reduction from baselinewere as follows: cholesterol 39% and LDL 62% (P= 0.29 and 0.11, respectively). No percent reduction of triglyceride is seen with fi brate given on alternate day with P= 0.19 The monthly cost reduction with combined alternate statin and fi brate treatment is at 34-48% while alternate day administration of the statin reduced cost by 60%.@*Conclusion@#This study showed lowering of postprandial total cholesterol, triglyceride and LDL with alternate statin and fi brate treatment, and total cholesterol and LDL with alternate day statin. The cost of treatment was also signifi cantly lowered with the alternate regimen. However, a follow through study with adequate sample size is recommended to support these observations.

ABSTRACT

Objective To analyze the etiological or associated factors and the treatment of patients with atrial fi brillation in high altitude areas in Tibet. Methods The clinical data of atrial fi brillation patients hospitalized in our hospital during January 2012 to Jane 2016 were analyzed retrospectively. Results (1) A total of 442 patients (male:female,1.2:1) were included in the study with ages of 30-96(65.9±12.3) years. The percentages of paroxysmal, persistent and permanent atrial fi brillation were 14.9%, 69.2%, and 15.8% respectively. The associated factors of atrial fi brillation included hypertension (53.4%), rheumatic heart disease (7.5%),chronic mountain sickness (10.6%), coronary heart disease(5.7%), hyperthyroidism (6.1%) and diabetes (9.0%).(2)Only 42 patients (9.5%) had evaluation with CHADS2score during hospitalization and actually 74.6% patients scored≥2. Twenty-one patients were restored to sinus rhythm during hospitalization and no patients had radiofrequency ablation.Conclusions The associated factors of atrial fi brillation in high altitude areas are similar to other areas. Thrombosis risk evaluation and anticoagulation therapy was not sufficient. Rhythm control rate was low and development of radio frequency ablation therapy should be considered.

ABSTRACT

Aim To observe whether paeonol can in-hibit fibronectin (FN) and intercellular cell adhension molecule-1 (ICAM-1) expressions in high glucose (HG)-induced glomerular mesangial cells(GMCs) via up-regulating CKIP-1 and activating the Nrf2 signaling pathway. Methods The effects of paeonol on the ex-pressions of CKIP-1,Nrf2,FN and ICAM-1 were eval-uated in GMCs treated with HG. Small interfering RNA was used to deplete CKIP-1 protein expression, and Western bolt was used to detect the expressions of Nrf2, HO-1 and SOD1. DHE fluorescent probe tech-nique was used to determine intracellular superoxide level. Results The protein levels of CKIP-1 and Nrf2 were elevated by paeonol in HG-treated GMCs. In the meanwhile,the expressions of Nrf2 downstream antiox-idant enzymes, i.e. HO-1 and SOD1, were also up-regulated by paeonol, which was accompanied by re-ductions of superoxide and H2O2levels. Importantly, paeonol reversed the excessive accumulation of FN and ICAM-1 in HG-induced GMCs. si-CKIP-1 decreased the up-regulation of Nrf2,HO-1 and SOD1 expressions during paeonol treatment, which was accompanied by increased superoxide and H2O2levels. Furthermore, si-CKIP-1 reversed the down-regulated levels of FN and ICAM-1 induced by paeonol. Conclusion Pae-onol inhibits the expressions of FN and ICAM-1 in HG-treated GMCs possibly by up-regulating CKIP-1 and activating the Nrf2 signaling pathway.
