ABSTRACT

OBJECTIVE@#To observe the effects of electroacupuncture (EA) pretreatment on cardiac function, sympathetic nerve activity, indexes of myocardial injury and GABAA receptor in fastigial nucleus in rats with myocardial ischemia reperfusion injury (MIRI), and to explore the neuroregulatory mechanism of EA pretreatment in improving MIRI.@*METHODS@#A total of 60 male SD rats were randomly divided into a sham operation group, a model group, an EA group, an agonist group and an agonist+EA group, 12 rats in each group. The MIRI model was established by ligation of the left anterior descending coronary artery. EA was applied at bilateral "Shenmen" (HT 7) and "Tongli" (HT 5) in the EA group and the agonist+EA group, with continuous wave, in frequency of 2 Hz and intensity of 1 mA, 30 min each time, once a day for 7 consecutive days. After intervention, the MIRI model was established. In the agonist group, the muscone (agonist of GABAA receptor, 1 g/L) was injected in fastigial nucleus for 7 consecutive days before modeling, 150 μL each time, once a day. In the agonist+EA group, the muscone was injected in fastigial nucleus 30 min before EA intervention. The data of electrocardiogram was collected by PowerLab standard Ⅱ lead, and ST segment displacement and heart rate variability (HRV) were analyzed; the serum levels of norepinephrine (NE), creatine kinase isoenzyme MB (CK-MB) and cardiac troponin I (cTnI) were detected by ELISA; the myocardial infarction area was measured by TTC staining; the morphology of myocardial tissue was observed by HE staining; the positive expression and mRNA expression of GABAA receptor in fastigial nucleus were detected by immunohistochemistry and real-time PCR.@*RESULTS@#Compared with the sham operation group, in the model group, ST segment displacement and ratio of low frequency to high frequency (LF/HF) of HRV were increased (P<0.01), HRV frequency domain analysis showed enhanced sympathetic nerve excitability, the serum levels of NE, CK-MB and cTnI were increased (P<0.01), the percentage of myocardial infarction area was increased (P<0.01), myocardial fiber was broken and interstitial edema was serious, the positive expression and mRNA expression of GABAA receptor in fastigial nucleus were increased (P<0.01). Compared with the model group, in the EA group, ST segment displacement and LF/HF ratio were decreased (P<0.01), HRV frequency domain analysis showed reduced sympathetic nerve excitability, the serum levels of NE, CK-MB and cTnI were decreased (P<0.01), the percentage of myocardial infarction area was decreased (P<0.01), myocardial fiber breakage and interstitial edema were lightened, the positive expression and mRNA expression of GABAA receptor in fastigial nucleus were decreased (P<0.01). Compared with the EA group, in the agonist group and the agonist+EA group, ST segment displacement and LF/HF ratio were increased (P<0.01), HRV frequency domain analysis showed enhanced sympathetic nerve excitability, the serum levels of NE, CK-MB and cTnI were increased (P<0.01), the percentage of myocardial infarction area was increased (P<0.01), myocardial fiber breakage and interstitial edema were aggravated, the positive expression and mRNA expression of GABAA receptor in fastigial nucleus were increased (P<0.01).@*CONCLUSION@#EA pretreatment can improve the myocardial injury in MIRI rats, and its mechanism may be related to the inhibition of GABAA receptor expression in fastigial nucleus, thereby down-regulating the excitability of sympathetic nerve.

ABSTRACT

OBJECTIVE@#To observe the effect of electroacupuncture (EA) preconditioning at heart meridian acupoints on the contents of dopamine (DA) and 5-hydroxytryptamine (5-HT) in lateral hypothalamus area (LHA) and cerebellar fastigial nucleus (FN) in the rats with acute myocardial ischemia-reperfusion injury (MIRI), and explore the role and mechanism of LHA and FN in the effect of EA at heart meridian acupoints against acute MIRI.@*METHODS@#Sixty SD rats were randomly divided into a sham-operation group, a model group, an EA heart meridian group and an EA lung meridian group, 12 rats in each group, as well as an LHA plus heart meridian group (damage of bilateral LHA) and an FN plus heart meridian group (damage of bilateral FN), 6 rats in each one. Three days after nucleus destruction, EA was applied to "Shenmen" (HT 7) and "Tongli" (HT 5) in the EA heart meridian group, the LHA plus heart meridian group and the FN plus heart meridian group and EA was applied to "Taiyuan" (LU 9) and "Lieque" (LU 7) in the EA lung meridian group, with 1 V in stimulating voltage and 2 Hz in frequency, lasting 20 minutes each time, once a day, for consecutively 7 days before model replication. Except in the sham-operation group, MIRI rat models were duplicated by ligation of the left anterior descending branch of the coronary artery in the rest groups. Using Power lab physiological recorder, ST segment displacement value was recorded before modeling, 30 min after ligation and 120 min after reperfusion separately. The high performance liquid chromatography-electrochemical detection and analysis system was adopted to determine the contents of DA and 5-HT in LHA and FN dialysate after rat modeling in each group.@*RESULTS@#In comparison of ST segment displacement value 30 min after ligation and 120 min after reperfusion among groups, the value in the model group was higher than that in the sham-operation group (@*CONCLUSION@#EA preconditioning at heart meridian acupoints can effectively alleviate myocardial injury in acute MIRI rats, during which, DA and 5-HT in LHA and FN may be the important material basis.

ABSTRACT

Background:Cerebellar fastigial nucleus (FN)is involved in regulation of visceral activities such as cardiovascular, ingestion,respiratory,and acute gastric mucosal injury,yet it is unclear whether it participates in the regulation of visceral hypersensitivity and what is the possible mechanism. Aims:To investigate the effect and possible mechanism of glutamic acid (Glu ) injection into cerebellar FN on chronic visceral hypersensitivity in rats. Methods: Chronic visceral hypersensitivity rat model was established by neonatal colorectal distension (CRD). After 8 weeks,the rats were divided into CRD group,solvent group (0. 2 μL 0. 9% NaCl solution injection into cerebellar FN),high-,medium-,low-dose Glu groups (12,6,3 μg Glu injection into cerebellar FN,respectively),3-MPA +Glu group (12 μg Glu injection after glutamate decarboxylase inhibitor 3-MPA injection into cerebellar FN),Bic + Glu group (12 μg Glu injection into cerebellar FN after GABAAreceptor blocker Bic injection into lateral hypothalamic area). Pain threshold,abdominal withdrawal reflex (AWR)score and abdominal external oblique muscle electromyography (EMG)were used to detect visceral sensitivity,and malondialdehyde (MDA)content and superoxide dismutase (SOD)activity were measured. Results:Chronic visceral hypersensitivity rat model was successfully established. Compared with CRD group,pain threshold was significantly increased (P<0. 05),AWR score,EMG amplitude,MDA content were significantly decreased (P<0. 05 ),and SOD activity was significantly increased in a dose-dependent manner in Glu group (P <0. 05 ). Compared with 12 μg Glu group,pain threshold was significantly decreased (P<0. 05),AWR score,EMG amplitude, MDA content were significantly increased (P <0. 05),and SOD activity was significantly decreased in 3-MPA +Glu group,Bic+Glu group (P<0. 05). Conclusions:Glu injection into cerebellar FN can significantly reduce the visceral sensitivity in rats. The mechanism may be that Glu in cerebellar FN produces GABA via glutamate decarboxylase,and then binding GABAAreceptor in lateral hypothalamic area,resulting in increased intestinal mucosal antioxidant capacity, thereby reducing visceral hypersensitivity.

@#Objective To investigate the effect of electrical stimulation to cerebellar fastigial nucleus on expression of nuclear factor-kappa B (NF-кB) P50, tumor necrosis factor-α (TNF-α) and Bcl-xL mRNA in rats brain after cerebral ischemia-reperfusion. Methods Sprague-Dawley rats were randomly divided into normal control group (NC group), cerebral ischemia-reperfusion group (I/R group), fastigial nucleus stimulation (FNS) group, and fastigial nucleus lesion (FNL) group. A focal cerebral ischemia-reperfusion model was established with middle cerebral artery occlusion (MCAO). 7 and 14 days after operation, the infarct volume was measured, and the protein of NF-кB P50 in rats brain was detected with Western blotting; the expression of TNF-α and Bcl-xL mRNA was detected with RT-PCR. Results Compared with I/R group, the expression of NF-кB P50 protein increased in FNS group (P<0.05), with the decrease of expression of TNF-α mRNA (P<0.01) and increase of Bcl-xL mRNA (P<0.05), while the infarct size decreased (P<0.01). There was no significant difference between FNL group and I/R group for all the measurements (P>0.05). Conclusion FNS could induce the expression of P50 protein and Bcl-xL mRNA, and inhibit the expression of TNF-α mRNA, and reduce infarct size, which may associated with the neuroprotection of central nervous system from injury.

ABSTRACT

Objective To study the effect of electrical stimulation of the fastigial nucleus of the cerebellum (FNS) on learning,memory and the expression of growth-association protein-43 (GAP-43) after cerebral ischemia and reperfusion.Methods Sixty healthy,adult,male Sprague-Dawley rats were randomly divided into a normal group,a sham-operation group (sham group),a model group and an FNS group,with 15 rats in each.Left middle cerebral artery occlusion and reperfusion (MCAO/R) was administered to the rats in the FNS and model groups using the thread embolism method.The rats of the FNS group were given FNS treatment using a pair of needle electrodes inserted into the cerebellar fastigial nucleus 3hrs after the MCAO/R.Needle electrodes were similarly inserted in the model group rats,but no electrical stimulus was applied.Then the rats' learning and memory abilities were tested using a Morris water maze on days 1,3 and 7 after the MCAO/R modeling.The expression of GAP-43 mRNA on the side of the cerebral infarction was detected using a quantitative,real-time polymerase chain reaction.Results The average escape]atencies of the rats in the model and FNS groups were significantly longer than those of the normal and sham groups at all time points,but the FNS group rats demonstrated a significantly shorter average escape latency than the rats of the model group at each time point.The normal and sham groups showed a significantly lower expression of GAP-43 mRNA than the model and FNS group rats at all time points.The FNS group rats had a significantly higher level of GAP-43 mRNA than the rats of the model group.Conclusion FNS improved the rats' learning and memory abilities.This might be associated with the up-regulation of GAP-43 mRNA in neurons on the side of the cerebral infarction which could promote the regeneration and repair of peripheral nerve axons in the area of the infarction.

ABSTRACT

Objective To preliminarily proved that the cerebellar hypothalamic pathways regulate inflammatory cytokines involved in post?stroke depression(post?stroke depression,PSD). Methods 160 male(Sprague?Dawley,SD)rats were randomly divided into sham operation group,the stroke group,PSD group,kainic acid(kainic acid,KA)damage of cerebellar fastigial nucleus group,electrical damaged decussation of superior cer?ebellar peduncle(superior cerebellar peduncle,xSCP)group. The behavioral observation were observed and expression of IL?1βand TNF?αin hip?pocampus by immunohistochemistry. Results Compared with the sham operation group and stroke group,PSD group,TNF?α,IL?1βcontent signif?icantly increased;and the factor of rats with KA group and xSCP group expression consistent with PSD group,there was no significant difference among the three groups. Conclusion The cerebellar fastigial nuclear may through the cerebellar hypothalamic pathways regulate inflammatory cyto?kines and participate in post stroke depression.

ABSTRACT

Saccades are rapid eye movements that shift the line of sight between successive points of fixation. The cerebellum calibrates saccadic amplitude (dorsal vermis and fastigial nucleus) and the saccadic pulse-step match (flocculus) for optimal visuo-ocular motor behavior. Based on electrophysiology and the pharmacological inactivation studies, early activity in one fastigial nucleus could be important for accelerating the eyes at the beginning of a saccade, and the later activity in the other fastigial nucleus could be critical for stopping the eye on target, which is controlled by inhibitory projection from the dorsal vermis. The cerebellum could monitor a corollary discharge of the saccadic command and terminate the eye movement when it is calculated to be on target. The fastigial nucleus and dorsal vermis also participate in the adaptive control of saccadic accuracy.

ABSTRACT

@#Objective To explore oropharyngeal swallowing disorders with videofluoroscopic swallowing study (VFSS). Methods 16 patients with dysphagia accepted VFSS with 10 ml of thin barium meal (50% w/v), thick barium meal (270% w/v), biscuit coated with thick barium meal in single swallow. Their swallowing function was observed on the lateral and anterior/posterior planes, including: symmetry of pyriform sinuses, oral transit time, presence of pharyngeal delay, pharyngeal transit time, oral and pharyngeal residue, and presence of aspiration.Results 5 patients demonstrated oral swallowing disorder. 3 patients demonstrated pharyngeal swallowing disorders, that was pharyngeal delay which caused in aspiration after swallowing. 8 patients demonstrated oropharyngeal swallowing disorders, and 3 of them presented aspiration,2 patients were silent aspirators, 1 was aspiration before and 1 after swallowing. The aspiration time could not be judged from the videofluoroscopy in the other one. For 4 patients with aspiration, 3 were severe, with more than 25% of the bolus aspirated, and 1 aspirated less than 5%. Conclusion VFSS can be helpful to plan individual rehabilitation.

ABSTRACT

Objective To investigate changes in the expression of prepro-orexin and orexin receptor-1 ( OX1R) following permanent middle cerebral artery occlusion ( MCAO ) with or without preconditioning through electrical stimulation of the cerebellar fastigial nucleus (FNS). Methods Wistar rats were subjected to permanent MCAO and randomly divided into 5 groups: a sham-operated control group (PO), an FNS preconditioning + shamoperated control group (FNS-PO) , an ischemia group, an FNS preconditioning + ischemia group (FNS-PI) and a cerebellar fastigial nucleus injury + FNS preconditioning + ischemia group (FNL-FNS-PI). Each group was divided into 5 subgroups according to the time at which the animals were sacrificed after the MCAO ( 1, 3, 6, 12 and 24 h).RT-PCR was used to detect expression of OX1R mRNA, and ELISA to measure the levels of orexin-A in the hypothalamus and plasma. Results The immunoreactivity of prepro-orexin decreased significantly in the PI groups, with further decreases over time. At the 12th h after MCAO, the immunoreactivity of prepro-orexin reached a minimum.There were significant differences between the rats in the PO and FNS-PO groups. On the contrary, the immunoreactivity of OX1R increased significantly in the PI groups, with further increases continuing over time, peaking at 12 h after the MCAO. There were significant differences between the PO and FNS-PO groups. In the rats with FNS preconditioning (PI-FNS) , the decrease in prepro-orexin and the increase in OX1R were significantly inhibited compared to the PI subgroups at the 6th and 12th hour. There was no significant difference between the FNL-PIFNS group and the PI group. The expression of OX1R mRNA increased significantly in the PI group, with further increases continuing over time, peaking at 24 hours. The plasma levels of orexin-A were not significantly different among the groups, but the levels of orexin-A in the hypothalamus decreased significantly in the PI and FNL-PI-FNS groups, with further decreases continuing over time. At the 12th h after the MCAO the levels were significantly different compared with the PO and PO-FNS groups. While in the rats with FNS preconditioning (PI-FNS) , the decrease in orexin-A level was reversed and there was no significant difference compared with PO and PO-FNS groups. Conclusions The orexinergic system is altered following cerebral ischaemia. FNS preconditioning may be able to regulate these changes.

@#ObjectiveTo study the therapeutic effects of cerebellar fastigial nucleus electrical stimulation (FNS) on motor and depression symptoms and monoamine neurotransmitters in the spinal cord fluid of patients with Parkinson's disease (PD).Methods65 patients with idiopathic Parkinson's disease following depression were divided into stimulation group (FNS+Madopar, n=35) and control group (Madopar, n=30). The stimulation group took Modopar, and treated with FNS, 30 miniutes once a day for 30 days. The control group took Modopar only. Madopar dose has no change during the treatment. The patients were evaluated by Webster Parkinson's Disease Evaluation Form, and Hamilton Depression Scale (HAMD) before and after FNS treatment. The loading of monoamine neurotransmitters was measured by high performance liquid chromatography-electrochemical process.ResultsAfter the treatment, the stimulation group improved in clinical feature and depression, scored significantly lower on Webster and HAMD than the control group(P<0.05); the loading of 5-hydroxytryptamine(5-HT) in spinal cord fluid increased; however noradrenalin and dopamine had no different. But there was no significant change in the symptoms and the loading of monoamine neurotransmitters in the control group.ConclusionFNS is efficient to relieve the motor and depression symptoms of PD, which possible mechamism might be central neuroprotection and the release of 5-HT by FNS induction.

Objective To investigate the effect of electrical stimulation of cerebellar fastigial nucleus (FN) on the contents of monoaminergie transmitters in frontal lobes of rats with depression, and to explore the pathway and mechanism of electrical stimulation of cerebellum in treating depression. Methods A total of 30 Wistar rats were used tO establish the animal depression model by exposure to chronic stressing circumstances. Electrical stimulation delivered to the left cerebellar FN or left dentate nucleus (DN) for 1 h was used as the intervention. The contents of serotonin (5-HT), noradrenalin (NE) and dopamine (DA) in frontal lobe were determined with fluorospectropho-tometry. Results Compared with normal control group, the contents of 5-HT and NE in bilateral frontal lobes of depression model rats decreased significantly (P < 0. 001). No significant change of all three monoaminergic trans-mitters in normal rats was observed after stimulation of FN or DN (P > 0. 05), but significant increases of 5-HT and NE in right and left frontal lobes were observed in depression model rats after stimulation of FN (P < 0.05, P < 0.001; P <0.05, P <0.01), with the increase in right lobe more significant than in left lobes (P <0.05). In de-pression model rats underwent DN stimulation, significant increase of contents of all three monoaminergic transmitters in frontal lobes was observed. Conclusion ElectrieM stimulation of cerebellar FN can increase the contents of 5-HT and NE in frontal lobes of rats with depression, and this suggest that electrical stimulation might have therapeutic effect to depression.

@#Objective To observe the of changes of cerebral blood flow and electroencephalography in chronic cerebral circulation insufficiency (CCCI) treated with fastigial nucleus stimulation (FNS) and hyperbaric oxygen (HBO). Methods 144 cases of CCCI were divided into 4 groups: 36 cases were treated with FNS and HBO, 36 cases with FNS, 36 cases with HBO, 36 cases without any treatment as control group. The blood velocity of anterior, middle, posterior cerebral arteries, vertebral artery and basilar artery were measured with transcranial Doppler (TCD) and the brain waves (α, β, δ, θ) were recorded with electroencephalography (EEG) before and after the treatment. Results Compared with the control, the brain blood velocity and α wave increased in all the treatment groups, especially in the HBO+FNS group, while β, δ, θ waves decreased (P<0.05). Conclusion FNS and HBO can increase cerebral blood flow and improve the cerebral function respectively.

@#Objective To observe the effect of fastigial nucleus stimulation(FNS)on heart rate variability(HRV)of surgically induced myocardial infarction rats.Methods 100 Sprague-Dawley rats were randomly allocated in four groups,including sham-operation control group,rats with coronary arteries ligated but fastigial nucleus(FN)sham stimulated(AMI group),rats both coronary arteries ligated and FN stimulated(FNS group),and rats on which FN lesioned 5 d before,then coronary arteries ligated and FN stimulated(FNL group).HRV characteristics were determined 6 h,1 d,7 d and 21 d after the ligation,and mortality rates were observed after 21 d.Results FNS can improve the survival of myocardial infarction rats,and this may be due to the increased vagal tone and decreased sympathetic tone.Conclusion FNS may have cardio-protective effects on surgically induced myocardial infarction rats.

@#Objective To observe the effect of very early intervention with cerebellar fastigial nucleus electrical stimulation and neurodevelopmental treatment on infants of central coordination disturbance(CCD).Methods 86 infants(0~6 months)with central coordination disturbance were divided into intervention group and control group.The intervention group was treated with neurodevelopmental treatment,cerebellar fastigial nucleus(FN)electrical stimulation and family interference.The control group was treated with neurodevelopmental treatment and family interference.The effect of infant was assessment with Gross Motor Function Measure(GMFM)and Gesell Development Test after 3 months treatment.Results The total improved incidence of the intervention group was 95.6%,which was significantly higher than that of the control group(P<0.01).The difference of scores in social,adaptive and motor area improved in the intervention group compared with that of control group(P<0.05).Conclusion Very early intervention with cerebellar fastigial nucleus electrical stimulation can facilitate the development of gross motor,social,adaptive capability of infants with CCD treated with neurodevelopmental therapy.

ABSTRACT

Objective To investigate the effect of preconditioning through stimulation of the fastigial nucleus (FNS)on the expression of matrix metalloproteinase-9 after cerebral ischemia. Methods A total of 312 male Wistar rats were divided randomly into three groups:a control group,a sham-FNS group and a FNS group.All the rats in the sham-FNS and FNS groups were subjected to middle cerebral artery occlusion(MCAO)by using a thread to occlude the right middle cerebral artery,while those in the control group were subject only to a similar operative procedure without real MCAO.Twenty-four hours before the operation,all the rats in the FNS group were given 1 hour of preconditioning FNS,while those in the sham-FNS group was given sham-FNS.Individuals were sacrificed at the 6th,12th,24th,and 72nd hour after MCAO.The brain water content was measured by the dry-wet weight method.The blood-brain barrier(BBB)permeability was measured using Evan's blue.MMP-9 protein expression was measured by Western blotting.Results The water content in the ischemic brain tissue,the BBB permeability and the expression of MMP-9 were all found to be significantly higher in the sham-FNS group than in the control group.Compared with the sham-FNS group,the brain water content and BBB permeability were significantly lower in the FNS group,while M MP-9 protein expression also decreased significantly.Conclusion FNS preconditioning can inhibit MMP-9 expression,thereby relieving BBB permeability and ischemic brain edema.

@#Objective To observe the effects of fastigial nucleus stimulation(FNS)combined with rehabilitation on vascular dementia.Methods63 vascular dementia patients were randomly divided into FNS group(40 patients)and control group(23 patients).Patients in the FNS group were treated with FNS and rehabilitation therapy,while those in the control group were treated with rehabilitation therapy only.Cognitive function were evaluated with Mini-Mental State Examination(MMSE),activities of daily living(ADL)were evaluated with Barthel Index(BI),cerebral blood flow velocity before and after therapy was detected with transcranial Doppler(TCD).ResultsThe scales of MMSE and BI increased significantly;the clinical efficiency was 95%.Cerebral blood flow velocity after therapy improved significantly(P<0.01).ConclusionFNS can improve the cognitive function and ADL in vascular dementia patients,that may be related with the improvement of cerebral blood flow.

@#The research on fastigial nucleus could be traced back to 19th century. However the great achievements on cerebrum overshadowed the importance of cerebellum and its nucleus. In recent 30 years, with the finding of brain protection effect of fastigial nucleus stimulation, much attention has been given to this field. Now its widely accepted that electrical stimulation of fastigial nucleus has brain protection effect and concrete mechanisms can be concluded as: improving the electrical instability around the infarct zone, reducing peri-infarction depolarizing wave and cortical spreading depression; suppressing microvessels inflammatory reaction; inhibiting cell apoptosis; reducing the neurons damage and facilitates neovascularization and so on. These new discoveries was reviewed and further expectation on this field was put forward by the authors.

@#Objective To explore the effect of cerebellar fastigial nucleus stimulation (FNS) on the expression of Nestin in adult Wistar rat brain after focal cerebral ischemia/reperfusion.MethodsThe animal model of focal cerebral ischemia/reperfusion was made by filament occlusion of the right middle cerebral artery. 180 male Wistar rats were randomly divided into five groups: normal control group (NC group), sham operation control group (SC group), ischemia/reperfusion group (I/R group), ischemia/reperfusion treated with sham FNS group (I/RFs group), and ischemia/reperfusion treated with FNS group (I/RF group), each group contain 1 d, 3 d, 7 d, 14 d, 21 d and 28 d six time points (for each point, n=6). Immunohistochemistry method was used to detect the number of Nestin expression positive cells following various time and interference in lateral cerebral ventriculus and hippocampus in adult Wistar rat brain.ResultsAfter focal cerebral ischemia/reperfusion, the number of Nestin positive cells increased at each time point, reached small peak value in 7th d ( P<0.01). After treated with FNS, the number of Nestin positive cells increased more strikingly at each time point ( P<0.05, P<0.01), reached higher peak value in 7th d ( P<0.01), and maintained at higher level in 14th d. Furthermore, the shape of Nestin positive cells changed significantly.ConclusionFNS can increase the number of Nestin positive cells in some brain regions after focal cerebral ischemia/reperfusion.
