ABSTRACT

El comercio ilegal de armas de fuego representa un problema significativo para mantener el orden y la seguridad del país, pues conlleva la adquisición de armas y municiones sin cumplir la normatividad y las regulaciones proporcionadas por el Estado, desviando la utilización de dichos equipos hacia la delincuencia. La investigación tiene como propósito analizar la dinámica del comercio ilegal de armas de fuego en Lima, Perú, mediante una metodología cualitativa y de diseño fenomenológico hermenéutico. La muestra incluye veinte funcionarios de la Policía Nacional del Perú (PNP) y la Superintendencia Nacional de Control de Servicios de Seguridad, Armas, Municiones y Explosivos de Uso Civil (Sucamec). La información se recolectó por medio de entrevistas semiestructuradas. El estudio concluye que el comercio ilegal de armas de fuego se estructura en tres fases clave: la primera fase determinada por la filtración o desvío de armas del mercado legal al ilegal, debido a la falta de control y fiscalización por las autoridades pertinentes. La segunda fase implica la creación de un mercado ilegal en espacios itinerantes, donde participan diversos actores, como intermediarios y compradores. La tercera y última fase comprende la consumación del delito, incluye la entrega del arma y el pago bajo estrictas medidas de seguridad para asegurar la impunidad de los involucrados.


The illegal trade in firearms represents a significant problem for the maintenance of order and security in the country, as it leads to the acquisition of arms and ammunition without complying with the norms and regulations provided by the State, diverting the use of such equipment towards crime. The purpose of this research is to analyse the dynamics of the illegal firearms trade in Lima, Peru, using a qualitative methodology and a hermeneutic phenomenological design. The sample includes twenty officers from the Peruvian National Police (PNP) and the National Superintendence for the Control of Security Services, firearms, Ammunition and Explosives for Civilian Use (Sucamec). The information was collected through semi-structured interviews. The study concludes that the illegal firearms trade is structured in three key phases: the first phase determined by the leakage or diversion of arms from the legal to the illegal market, due to the lack of control and oversight by the relevant authorities. The second phase involves the creation of an illegal market in itinerant spaces, where various actors, such as intermediaries and buyers, participate. The third and final phase involves the consummation of the crime, including the delivery of the weapon and payment under strict security measures to ensure the impunity of those involved.


O comércio ilegal de armas de fogo representa um problema significativo para a manutenção da ordem e da segurança no país, pois leva à aquisição de armas e munições sem o cumprimento das normas e regulamentos fornecidos pelo Estado, desviando o uso desses equipamentos para o crime. O objetivo desta pesquisa é analisar a dinâmica do comércio ilegal de armas de fogo em Lima, Peru, usando uma metodologia qualitativa e um desenho fenomenológico hermenêutico. A amostra inclui vinte policiais da Polícia Nacional do Peru (PNP) e da Superintendência Nacional de Controle de Serviços de Segurança, Armas, Munições e Explosivos para Uso Civil (Sucamec). As informações foram coletadas por meio de entrevistas semiestruturadas. O estudo conclui que o comércio ilegal de armas de fogo está estruturado em três fases principais: a primeira fase é determinada pelo vazamento ou desvio de armas do mercado legal para o ilegal, devido à falta de controle e fiscalização por parte das autoridades competentes. A segunda fase envolve a criação de um mercado ilegal em espaços itinerantes, onde vários atores, como intermediários e compradores, participam. A terceira e última fase envolve a consumação do crime, incluindo a entrega da arma e o pagamento sob rigorosas medidas de segurança para garantir a impunidade dos envolvidos.

Abstract Objective To verify the association between victims of bullying and weapon possession among school adolescents. Methods This cross-sectional study was carried out with 2,296 high school students aged 14-19 years. An instrument containing questions from the validated Youth Risk Behavior Survey questionnaire and the National School Health Survey was used. To describe the profile of interviewees, absolute and relative frequencies were calculated, and the chi-square test was applied to verify the existence of associations. To test the association of bullying with weapon possession, Poisson logistic regression (univariate and multivariate) was used. The significance level used for all analyses was 5%. Results Among adolescents interviewed, 23.1% reported being victims of bullying. Among victims of bullying, 37.6% (PR = 1.68; 95% CI = 1.30 - 2.17) reported having already carried some type of weapon (knife, revolver, or truncheon) in the last 30 days, while the reported firearm possession was 38% (PR = 1.67; 95% CI = 1.16 - 2.40) and 47.5% of these adolescents carried some type of weapon (knife, revolver or truncheon) in the school environment (PR = 2.10; 95% CI = 1.50 - 2.93). Conclusions It could be observed that adolescents who are victims of bullying are twice as likely of carrying some type of weapon (knife, revolver, or truncheon) to the school environment, and are also more likely of carrying a firearm.

ABSTRACT

Este estudo teve como objetivo identificar o risco de desenvolvimento de transtorno de estresse pós-traumático (TEPT), bem como sua associação com pensamentos ou tentativas suicidas e a saúde mental de policiais militares feridos por arma de fogo, na Região Metropolitana de Belém (RMB), nos anos de 2017 a 2019. A pesquisa contou com a participação de 30 entrevistados, que responderam o Inventário Demográfico e a Lista de verificação de TEPT para o DSM-5 (PCL-5). Para análise dos dados, utilizou-se a técnica estatística Análise Exploratória de Dados e a técnica multivariada Análise de Correspondência. Os resultados revelaram a existência de risco de desenvolvimento do transtorno de forma parcial ou total em uma expressiva parcela da população entrevistada, tendo homens como maioria dos sintomáticos, com média de 38 anos, exercendo atividades operacionais e vitimados em via pública quando estavam de folga do serviço. O ferimento deixou a maioria com sequelas, com destaque para dores crônicas, limitações de locomoção e/ou mobilidade e perda parcial de um membro. E, ainda, policiais sintomáticos apresentaram comportamentos suicidas, relatando já terem pensado ou tentado tirar a própria vida. Desta forma, conclui-se que policiais militares são expostos constantemente a traumas inerentes a sua profissão. Quando há ameaça de vida, como nos casos de ferimentos por arma de fogo, são suscetíveis a sequelas físicas decorrente do ferimento, somadas a sequelas mentais tardias, como o surgimento de sintomatologias de TEPT e ideação suicida.(AU)


This study aimed to identify the risk of developing post-traumatic stress disorder (PTSD) and its associations around suicidal thoughts or attempts and mental health in military police officers injured by firearms, in the Metropolitan Region of Belem (RMB), from 2017 to 2019. The research had the participation of 30 respondents who answered the Demographic Inventory and the PTSD checklist for DSM-5 (PCL-5). For data analysis, we used the statistical technique Exploratory Data Analysis and the multivariate technique Correspondence Analysis. The results revealed the existence of risk of developing partial or total disorder in a significant portion of the interviewed population, with men as most of the symptomatic individuals, with mean age of 38 years, developing operational activities and victimized on public roads when they were off duty. The injuries left most of them with sequelae, especially chronic pain, limited locomotion and/or mobility, and partial loss of a limb. In addition, symptomatic officers showed suicidal behavior, such as reporting they had thought about or tried to take their own lives. Thus, we conclude that military policemen are constantly exposed to traumas inherent to their profession. When their lives are threatened, as in the case of firearm wounds, they are susceptible to physical sequelae resulting from the injury, in addition to late mental sequelae, such as the appearance of PTSD symptoms and suicidal ideation.(AU)


Este estudio tuvo como objetivo identificar el riesgo de desarrollo de trastorno de estrés postraumático (TEPT) y sus asociaciones con pensamientos o tentativas suicidas y la salud mental en policías militares heridos por armamiento de fuego, en la Región Metropolitana de Belém (Brasil), en el período entre 2017 y 2019. En el estudio participaron 30 entrevistados que respondieron el Inventario Demográfico y la Lista de verificación de TEPT para el DSM-5 (PCL-5). Para el análisis de datos se utilizaron la técnica estadística Análisis Exploratoria de Datos y la técnica multivariada Análisis de Correspondencia. Los resultados revelaron que existen riesgos de desarrollo de trastorno de estrés postraumático de forma parcial o total en una expresiva parcela de la población de policías entrevistados, cuya mayoría de sintomáticos eran hombres, de 38 años en media, que ejercen actividades operacionales y fueron victimados en vía pública cuándo estaban de día libre del servicio. La lesión dejó la mayoría con secuelas, especialmente con dolores crónicos, limitaciones de locomoción y/o movilidad y la pierda parcial de un miembro. Aún los policías sintomáticos presentaran comportamiento suicida, tales como relataran qué ya pensaron o tentaron quitar la propia vida. Se concluye que los policías militaran se exponen constantemente a los traumas inherentes a su profesión. Cuando existe amenaza de vida, como en los casos de heridas por armamiento de fuego, son expuestos a secuelas físicas transcurridas de la herida, sumado a secuelas mentales tardías, como el surgimiento de sintomatologías de TEPT y la ideación suicida.(AU)

Introducción: Las fracturas por heridas de arma de fuego son un motivo de consulta habitual en nuestro país. Existe gran variabilidad de conductas respecto a su tratamiento. El objetivo principal de este trabajo es analizar los distintos tratamientos y sus indicaciones. Materiales: Se realizó una revisión sistematizada de la literatura en las bases de datos Pubmed y Scielo. Se incluyeron artículos con fracturas por herida de arma de fuego en miembros superiores e inferiores, excluyendo la mano. Se analizó: tratamiento (ortopédico o quirúrgico), debridamiento, antibioticoterapia y complicaciones. Resultados: Se obtuvieron 19 artículos que cumplían los criterios de inclusión y exclusión. Los artículos tuvieron un Nivel de Evidencia tipo 2b, 3 y 4. Conclusiones: Los artículos analizados tienen un bajo nivel de evidencia. La fijación quirúrgica es variable y depende de la topografía ósea, la lesión de partes blandas y las lesiones asociadas. El debridamiento profundo está relacionado con mayores índices de infección. Las fracturas estables de tratamiento ortopédico no deberían debridarse ya que aumenta los índices de infección. Debería realizarse antibioticoterapia intravenosa inicial en todos los pacientes, la terapia posterior es discutida.


Introduction: Fractures due to gunshot wounds are a common reason for consultation in our country. There is great variability of conduct regarding its treatment. The main objective of this work is to analyze the different treatments and their indications. Materials: A systematic review of the literature was carried out in the Pubmed and Scielo databases. Articles with fractures due to gunshot wounds in the upper and lower limbs (excluding the hand) were included. We analyzed: treatment (orthopedic or surgical), debridement, antibiotic therapy and complications. Results: 19 articles were obtained that met the inclusion and exclusion criteria. The articles had a Level of Evidence type 2b, 3 and 4. Conclusions: The articles analyzed have a low level of evidence. Surgical fixation is variable and depends on bone topography, soft tissue injury, and associated injuries. Deep debridement is associated with higher rates of infection. Stable orthopedically treated fractures should not be debrided as this increases infection rates. Initial intravenous antibiotic therapy should be performed in all patients, subsequent therapy is discussed.


Introdução: As fraturas por ferimentos por arma de fogo são motivo comum de consulta em nosso país. Há grande variabilidade de conduta quanto ao seu tratamento. O objetivo principal deste trabalho é analisar os diferentes tratamentos e suas indicações. Materiais: Foi realizada revisão sistemática da literatura nas bases de dados Pubmed e Scielo. Foram incluídos artigos com fraturas por arma de fogo em membros superiores e inferiores, excluindo a mão. Foram analisados: tratamento (ortopédico ou cirúrgico), desbridamento, antibioticoterapia e complicações. Resultados: foram obtidos 19 artigos que atenderam aos critérios de inclusão e exclusão. Os artigos tinham Nível de Evidência tipo 2b, 3 e 4. Conclusões: Os artigos analisados ​​apresentam baixo nível de evidência. A fixação cirúrgica é variável e depende da topografia óssea, lesão de tecidos moles e lesões associadas. O desbridamento profundo está associado a maiores taxas de infecção. Fraturas estáveis ​​tratadas ortopedicamente não devem ser desbridadas, pois isso aumenta as taxas de infecção. A antibioticoterapia intravenosa inicial deve ser realizada em todos os pacientes, a terapia subsequente é discutida.

@#The incidence of close-range firearm shootings on fishing vessels in Malaysia has been increasing every year. The challenge faced by law enforcement agencies is to demonstrate how these shooting crimes are carried out. To address this problem, a model of shooting angles and ricochets within a confined space needs to be constructed. This study examines the effects of shooting with the CZ 75 SP-01 Shadow semi-automatic pistol, manufactured by the Czech Republic, using 9mm FMJ bullets weighing 7.45 grams on three types of wood commonly used in fishing vessel construction: Resak, Seraya, and Balau. The factors of shooting distance (1, 3, 5, and 10 meters), shooting angles (15°, 30°, 45°, 60°, 75°, and 90°), ricochet angles, and the type of wood material (Resak, Seraya, and Balau) are investigated. The results indicate that only 114 out of 432 shots resulted in ricochets. Shots on Resak wood produced several bullet ricochets at shooting angles of 15° and 30° and a distance of 1 meter. An equation model relating ricochet angles and successful shots was developed. For Seraya wood, only a few ricochets were observed at a shooting angle of 15° and a distance of 1 meter, and statistical tests showed that an equation model could not be constructed. For Balau wood, an equation model relating ricochet angles and shooting distance was successfully established. Validation tests on the Resak and Balau wood models showed that the Resak model achieved an accuracy of 90%. However, the Balau wood model did not yield any reliable results and thus cannot be trusted. In conclusion, the three factors examined are insufficient to explain bullet ricochet occurrences when shooting is performed at close range in an enclosed space.

ABSTRACT

Hip firearm injuries are rare injuries that could lead to serious complications, such as posttraumatic hip arthritis and coloarticular fistula. We report a case of a 25-year-old male who sustained a pelvic injury caused by a single bullet which led to a bilateral acetabular fracture, concomitant with a colon injury treated on an emergency basis by a diverting colostomy; acetabular fractures were treated conservatively by traction. After the patient recovered from the abdominal injury, he was presented with bilateral hip pain and limited motion; plain radiographs showed bilateral hip arthritis with proximal migration of the femoral head and bilateral acetabular defect classified as Paprosky type ⅢA. Reconstruction of the hips was performed using the same technique: impaction bone grafting for acetabular defect reconstruction and a reversed hybrid total hip arthroplasty (THA) 6 months apart. The patient presented with loosening of the left THA acetabular cup 3 years later, which was revised; then he presented with a discharging sinus from the left THA with suspicion of coloarticular fistula, which was confirmed using CT with contrast material. A temporary colostomy and fistula excision were performed, and a cement spacer was applied to the hip. After clearing the infection, a final revision THA for the left hip was performed. Treating post-firearm hip arthritis by THA is challenging, especially in the situation of neglected cases with the presence of an acetabular defect. Concomitant intestinal injury increases the risk of infection with the possibility of coloarticular fistula formation, which could present later. Working with a multidisciplinary team is paramount.

Resumen Las heridas por proyectil de arma de fuego se definen como los efectos que producen sobre el organismo los disparos realizados con armas cargadas de proyectiles o diversos tipos de pólvora u otros explosivos. La valoración médico legal en estos casos, en personas vivas, se realiza en la Sección Clínica Médico Forense del Departamento de Medicina Legal del Organismo de Investigación Judicial, esta incluye la historia médico legal, el examen físico, el análisis de documentos médicos aportados, pericias conexas efectuadas y revisión de bibliografía, esto para tener todos los elementos de juicio que permitan realizar un análisis objetivo y basado en la evidencia científica, y así poder responder a las interrogantes de la Autoridad Judicial. El presente artículo abarca algunos de los aspectos principales a tomar en cuenta ante hallazgos atípicos de los orificios de entrada y salida de los proyectiles, mediante la presentación y abordaje de un caso clínico.


Abstract Firearm projectile injuries are defined as the effects produced on the body by shots fired with weapons loaded with projectiles or various types of gunpowder or other explosives. The legal medical assessment in these cases, in living persons, is carried out in the Forensic Medical Clinical Section of the Department of Legal Medicine of the Organismo de Investigación Judicial, this includes the legal medical history, physical examination, analysis of medical documents provided, expert carried out and bibliography review, this to have all the elements of judgment that allow an objective analysis based on scientific evidence, and thus be able to answer the questions of the Judicial Authority. This article covers some of the main aspects to be considered when faced with atypical findings of the entry and exit holes of the projectiles, through the presentation and approach of a clinical case. ABSTRACT

O Ferimento por Projétil de Arma de Fogo (PAF), também conhecido como trauma balístico é todo e qualquer trauma físico causado por arma de fogo, sendo considerado um problema de saúde pública mundial, devido sua alta prevalência, seu grande impacto psicossocial e o alto custo com tratamento. O objetivo deste estudo é avaliar as diferentes abordagens terapêuticas relacionadas ao ferimento por arma de fogo na área de cirurgia e traumatologia bucomaxilofacial, contribuindo assim para a elaboração de protocolos de atendimento mais eficazes e formulação de políticas públicas mais eficientes. Para isso, foi realizado uma revisão de literatura nas principais bases de dados: Scielo, Medline e Pubmed, no período entre 1998 e 2021, nas línguas inglesa e portuguesa. Observou-se que este tipo de lesão foi mais prevalente em jovens do sexo masculino, os quais geralmente apresentaram fraturas cominutivas em mandíbula, sendo o tratamento cirúrgico o mais indicado. Sendo assim, é de suma importância que todos os profissionais envolvidos no tratamento ao paciente vítima de PAF saibam tratar de forma adequada, para a melhoria da qualidade de vida de todos os envolvidos.


Firearm Projectile Injury (FPI), also known as ballistic trauma, is any physical trauma caused by a firearm considered a worldwide public health problem due to its high prevalence, great psychosocial impact and the high cost of treatment. The aim of this study is to evaluate the different therapeutic approaches related to gunshot wounds in the area of maxillofacial surgery and traumatology, thus contributing to the development of more effective care protocols and the formulation of more efficient public policies. For this, a literature review was carried out in the main databases: Scielo, Medline and Pubmed, in the period between 1998 and 2021, in English and Portuguese. It was observed that this type of injury was more prevalent in young males, who generally presented comminuted fractures in the mandible, with surgical treatment being the most indicated. Therefore, it is of paramount that all professionals involved in the treatment of FAP know how to treat it properly, providing better health outcomes for its victims.

ABSTRACT

Objective:To establish a stable fragment-induced penetrating liver injury model in landrace pigs and evaluate the characteristics of deep tissue injury.Methods:According to the different positioning methods of aiming points, twelve healthy adult landrace pigs were divided into group A (the relative height "h" of the aiming point and the highest point of the body surface on the tracing line was set to 5 cm) and group B ("h" was set to 6 cm). Ultrasonography was used to determine the direction of fragment projection, and an experimental ballistic gun was used to project high-velocity fragments to cause injury to animals. The vital signs of the two groups were monitored, and whole blood cell count, blood gas analysis, and liver and renal function were tested. Damages to the liver and adjacent organs, as well as the amount of bleeding and survival time were analyzed.Results:For the overall analysis of the two groups, the liver hit rate of fragment simulating projectiles was 100% (right anterior lobe and right lateral lobe injury), the hit rate of other organs in the abdominal cavity was 25% (3/12), and the incidence of hemothorax or pneumothorax was 8% (1/12). The wounds were mainly characterized by liver lacerations, with total or partial disconnection of the distal liver lobe. There was no significant difference in wound length and bleeding amount between groups A and B [wound length (cm): 9.8±1.7 vs. 11.2±3.8, bleeding amount (g): 597.0±477.1 vs. 1 032.0±390.3, both P > 0.05]. The depth of liver parenchymal laceration in group B with the aiming point closer to the anterior median line was significantly longer than that in group A (cm: 2.8±0.4 vs. 1.9±0.6, P = 0.015). Mean arterial pressure (MAP), pH value, residual arterial blood base (BE), hemoglobin (Hb) and hematocrit (HCT) levels decreased after the fragment-induced injury, and then reached a trough level [MAP (mmHg, 1 mmHg ≈ 0.133 kPa): 87.0±33.6, pH: 7.26±0.15, BE (mmol/L): -6.65±8.48, Hb (g/L): 9.86±1.10, HCT: 0.309±0.029, all P < 0.05] in the first hour. Blood lactate (Lac), lactate dehydrogenase (LDH) and aspartate aminotransferase (AST) levels increased over time, and reached a peak level [Lac (mmol/L): 10.21±4.40, LDH (U/L): 1 417.0±223.3, AST (U/L): 234.5 (162.5, 357.5), both P < 0.05] at 1 hour after injury. Pearson's correlation analysis showed that the total amount of bleeding was correlated with the depth of liver parenchyma laceration ( r = 0.684, P = 0.014). The Kaplan-Meier survival curve showed that the 3 hours survival rate in group A was higher than that in group B, but the difference was not statistically significant [83.3% (5/6) vs. 33.3% (2/6), P > 0.05]. Conclusions:The high-velocity fragment-induced penetrating liver injury model established by striking landrace pigs closer to the anterior median line with fragment simulating projectiles is reproducible and the degree of damage is controllable, and the model is applicable to further relevant research of hepatic ballistic trauma.

Se describe el caso clínico de un paciente de 26 años de edad, atendido en la consulta de Ortopedia y Traumatología del Centro de Diagnóstico Integral Salvador Allende de Caracas, en la República Bolivariana de Venezuela, quien desde hacía 6 meses había recibido un disparo de escopeta en la cara posterior de ambos muslos. Esta vez acudió a consulta con dolor intenso en el miembro inferior izquierdo, sobre todo por debajo de la rodilla, acompañado de parestesias en la zona de inervación de ambas ramas de bifurcación del nervio ciático. Se pudo comprobar que el joven presentaba múltiples impactos de proyectiles en la cara posterior de ambos muslos y marcha claudicante. Los estudios radiográficos confirmaron la presencia de 8 proyectiles, 2 en el muslo derecho y 6 en el izquierdo. En la vista lateral se observó que uno de ellos se encontraba a una profundidad que se correspondía con la región anatómica del nervio ciático izquierdo. Para la intervención quirúrgica se utilizó anestesia local, sedación y un intensificador de imágenes. Con la cooperación del paciente se pudo extraer el proyectil alojado en el epineuro del ciático sin complicaciones. A los 3 meses habían desaparecido las molestias y pudo reincorporarse a sus actividades habituales.


The case report of a 26 years patient is described. He was assisted in the Orthopedics and Traumatology Service of Salvador Allende Integral Diagnostic Center from Caracas, in the Bolivian Republic of Venezuela who had received a shotgun shot in the posterior face of both thighs 6 months ago. This time he went to visit the doctor with intense pain in the left inferior member, mainly below the knee, accompanied by paresthesias in the innervation area of both bifurcation branches of the sciatic nerve. It could be demonstrated that the young man presented multiple projectile impacts in the posterior face of both thighs and hesitating gait. The radiographic studies confirmed the presence of 8 projectiles, two in the right thigh and 6 in the left one. In the lateral view it was observed that one of them was at a depth that matched with the anatomical region of the left sciatic nerve. Local anesthesia, sedation and an images intensifier were used for the operation. The projectile located in the epineurium of the sciatic nerve could be removed with the patient cooperation without complications. After 3 months the discomfort had disappeared and he could return to his usual activities.

ABSTRACT

Discussions about firearms have become usual in the Brazilian context, implying a wide range of arguments and positions among individuals. In view of this wide array of opinions and the concrete episodes involving firearms, the present study aimed to analyze firearms' social representations. The sample consisted of 100 participants, ranging from 19 to 66 years old (M = 34.31; SD = 12.12). Participants responded to a word evocation questionnaire, an open-ended question on firearm carrying permit, and reported demographic data. The openEvoc and Iramuteq software were used for data analysis. At their core, social representations concerning firearms were characterized by the perception of death, succeeded by the following elements: violence, security, danger, defense, and protection, simultaneously encompassing an ambiguity between risk and protection.


A discussão sobre armas de fogo tem se tornado usual no contexto brasileiro, acarretando uma variabilidade de argumentos e posicionamentos entre as pessoas. Tendo em vista essa multiplicidade de opiniões e os episódios concretos envolvendo arma de fogo, o presente trabalho teve como objetivo analisar as representações sociais sobre armas de fogo. Para isso, a amostra foi composta por 100 participantes, com idades variando entre 19 e 66 anos (M = 34,31; DP = 12,12). Os participantes responderam a um questionário de evocações de palavras, uma questão aberta sobre o porte de armas e informaram dados demográficos. Para a análise dos dados, foram utilizados os programas openEvoc e Iramuteq. Verificou-se que as representações sociais sobre armas de fogo foram caracterizadas em sua centralidade pela percepção da morte, seguida dos elementos: violência, segurança, perigo, defesa e proteção, abrangendo, concomitantemente, uma ambiguidade de risco e proteção.


La discusión acerca de las armas de fuego se ha vuelto habitual en Brasil, trayendo en consecuencia una variabilidad de argumentos y posicionamientos entre las personas. Teniendo en vista esta multiplicidad de opiniones y los episodios concretos envolviendo armas de fuego, el presente trabajo tuvo como objetivo analizar las representaciones sociales acerca de las armas de fuego. La muestra fue compuesta por 100 participantes, con edades que varían entre 19 y 66 años (M = 34,31; DP = 12,12). Los participantes han respondido un cuestionario de evocaciones de palabras, una pregunta abierta acerca del porte de armas y reportaron dados demográficos. Para análisis de los datos, fueron utilizados los programas openEvoc y Iramuteq. Se ha comprobado que las representaciones sociales sobre las armas de fuego fueron caracterizadas en su centralidad por la percepción de la muerte, seguida de los elementos: violencia, seguridad, peligro, defensa, protección; que abarca, concomitantemente, una ambigüedad de riesgo y protección.

Las heridas precordiales son una entidad poco frecuente y desafiante en cirugía de trauma en los servicios de Emergencia. La incidencia elevada de taponamiento y trauma cardiaco, así como su alta mortalidad obliga a tomar decisiones diagnósticas y terapéuticas rápidas y precisas. Material y Métodos: se presentan los casos de dos pacientes masculinos que ingresaron al servicio de Emergencia del Hospital Roosevelt con lesiones precordiales penetrantes con trauma pericárdico y cardiaco, con diferentes manifestaciones clínicas a los que se les realizó ventana pericárdica subxifoidea diagnóstica y seguido una esternotomía media con exposición pericárdica y cardiaca como abordaje y tratamiento quirúrgico definitivo. Discusión: Es de suma importancia reconocer los amplios escenarios de presentaciones clínicas del paciente con heridas en la región precordial y mantener un alto índice de sospecha de trauma y taponamiento cardiacos en todas las lesiones precordiales penetrantes en pacientes estables e inestables para no retrasar el diagnóstico y tratamiento quirúrgico temprano y adecuado. Las lesiones asociadas y complicaciones trans y postoperatorias aumentan la mortalidad de estos pacientes (AU)


Precordial wounds are rare and challenging lesions in Trauma Services. The high incidence of cardiac trauma and the high mortality requires fast and precise diagnostic and therapeutic decisions. Case report: Two male patients who were admitted to the Roosevelt Hospital Emergency Service with penetrating precordial injuries with pericardial and cardiac trauma are presented, with different clinical manifestations, who underwent a diagnostic subxiphoid pericardial window and a median sternotomy with pericardial and cardiac exposure as a definitive surgical approach and treatment. Discussion: Is important to recognize the broad scenarios of patients with wounds in the precordial region and to maintain a high index of suspicion of cardiac trauma and tamponade in all penetrating precordial injuries inclusive in stable patients to not delay diagnosis and treatment and a proper surgical intervention. Associated injuries and perioperative complications increase the mortality of these patients

Introdução: A face é uma região exposta a diversos traumas que são causados por várias etiologias, dentre eles por projétil de arma de fogo que são a segunda maior causa de fraturas perdendo somente para os acidentes automobilísticos. As fraturas acometem com frequência a região mandibular, por ser um osso de destaque na face devido sua proeminência. O objetivo deste trabalho é elucidar uma fratura cominutiva na região de côndilo e colo mandibular causada por ferimento de arma de fogo e seu respectivo tratamento. Relato de Caso: Paciente de 25 anos, compareceu a um serviço de atendimento de cirurgia bucomaxilofacial vítima de um ferimento por arma de fogo. Após a realização dos exames de imagem constatou-se que o projétil estava alojado no pescoço da vítima, causando a fratura cominutiva o ramo e o côndilo mandibular do lado esquerdo. A paciente foi submetida a anestesia geral para realização da osteossíntese da fratura. Considerações finais: O diagnóstico e o tratamento são realizados por uma equipe multidisciplinar para que o tratamento seja o mais correto e efetivo, com finalidade de minimizar morbidades e mortalidades, já que os pacientes sobrevivem com sequelas permanentes quando o diagnóstico e o tratamento não são realizados de forma correta... Introduction: The face is a region exposed to several traumas that are caused by several etiologies, among them a firearm projectile that is the second major cause of fractures losing only to automobile accidents. The fractures frequently affect the mandibular region, as it is a prominent bone in the face due to its prominence. The purpose of this study is to elucidate a comminuted fracture in the condyle and mandibular neck region caused by a firearm injury and its respective treatment. Case Report: A 25-year-old patient who attended a buccomaxillofacial surgery service as a victim of a gunshot wound. After imaging examinations it was found that the projectile was lodged in the victim's neck, causing the comminuted fracture of the branch and mandibular condyle on the left side. The patient underwent general anesthesia to perform the fracture osteosynthesis. Final considerations: Diagnosis and treatment are performed by a multidisciplinary team so that treatment is the most correct and effective, in order to minimize morbidities and mortalities, since patients survive with permanent sequelae when diagnosis and treatment are not performed correctly... RESUMEN Introducción: Las lesiones cervicales por arma de fuego en pediatría tienen baja incidencia, aproximadamente a 5 a 10%, con una mortalidad de 0 a 11%. Objetivo: Describir un caso de politraumatismo por arma de fuego, su manejo multidisciplinario y las consecuencias de la imprudencia a la exposición de niños a las mismas. Caso Clínico: Paciente de sexo masculino, de 13 años de edad, con traumatismo de cara, cuello y tórax por herida accidental por arma de fuego (escopeta) con diagnósticos: Trauma Facial: fractura de maxilar superior e inferior; Trauma de cuello: lesión en cuerpo vertebral C4/C7 con perdigones incrustados; Trauma de tórax: contusión pulmonar derecha; Trauma de partes Blandas: con perdigones incrustados en celular subcutáneo. Ingresa al hospital de Trauma en choque hipovolémico presentando casi inmediatamente paro cardiorespiratorio, respondiendo a reanimación cardiopulmonar avanzada. TAC de cráneo al ingreso normal; al 8vo día se evidencia esquirla en sistema venoso de la base; TAC de cuello: se evidencia desplazamiento de tráquea y aire peri tráquea; AgioTAC: pseudoaneurisma en carótida interna izquierda. Fibrobroncospcopía: contusiones a nivel de tráquea; EDA: esófago y estómago con erosiones cubiertas por fibrina sin evidencia de fístula o perforación de víscera hueca. Tratamiento: en Asistencia Mecánica Respiratoria (ARM) por 10 días; inotrópicos por 72 hs, se realiza reparación de arteria carótida interna; colocación de arco de Erich por cirujanos maxilofaciales. Alta a sala de pediatría con buena evolución.


ABSTRACT Introduction: Pediatric cervical gun injuries have a low incidence, approximately 5 to 10%, with a mortality of 0 to 11%. Clinical Case: a 13-year-old male patient who presented with trauma to the face, neck and thorax due to accidental firearm injury (shotgun) with the following diagnoses: Facial trauma: fracture of the upper and lower jaw; Neck trauma: C4 / C7 vertebral body injury with embedded pellets; Chest trauma: right pulmonary contusion; Soft tissue trauma: embedded pellets in subcutaneous soft tissue. He was admitted to the Trauma hospital in hypovolemic shock, presenting almost immediate cardiorespiratory arrest, which responded to advanced cardiopulmonary resuscitation. CT scan of the skull was normal at admission; on the 8th day it showed evident splintering in the basal venous system; CT of the neck: tracheal displacement and evident peri-tracheal air; Angio CT: pseudoaneurysm in left internal carotid artery. Fibrobronchoscopy: bruises at the tracheal level; EDA: esophagus and stomach with fibrin-covered erosions without evidence of fistula or hollow viscera perforation. Treatment: on Mechanical Respiratory Assistance (MRA) for 10 days; inotropes for 72 hours, internal carotid artery repair was performed, Erich arch placement by maxillofacial surgeons. He was discharged to the pediatric ward with favorable evolution.

Background: Injuries inflicted on head & neck  are of different types. It could be either blunt injury, sharp penetrating injury or gunshot injury. Multiple cranial nerve injury, airway injury, injury to bone and soft tissues are common outcome of such injury Methods: This study was a retrospective undertaken in the Department of ENT, JNMC, AMU over a period of five years from January 2000 to June 2005. In our present study we have exclusively dealt with firearm injuries inflicted on head & neck area and those patients who survived and were treated in our medical college. We studied ten cases of firearm injury. We had discarded all those cases of head & neck firearm assault who were brought dead. Results:  The study was conducted in the Department of ENT, JNMC over a period of five years. The age of presentation ranged from 15yrs-50 yrs. The male & female ratio was of 4:1. Most of the patients belonged either to areas east of Aligarh or Aligarh itself. Conclusion: The firearm injuries are now more common because of the development of sophisticated ammunition, intolerance and hatred. Timely intervention if given saves lifes of many patients. Team management and sound facilities are significant in the management of firearm injuries

Resumen: Antecedentes: La introducción de armas de fuego ha incrementado las muertes, asesinatos y suicidios. Las lesiones por proyectil de arma de fuego (PAF) afectan cada vez más a poblaciones civiles (500,000 lesiones anualmente) y es la segunda causa de muerte en jóvenes. La mitad requiere tratamiento de fracturas (45%), acompañado de reportes de 6% que desarrollaron una infección profunda. Material y métodos: Estudio retrospectivo, observacional y transversal en cuatro hospitales de la Secretaría de Salud de la CDMX de la frecuencia de infección en fracturas expuestas por PAF y si existe algún otro factor que acompañe a la infección. Resultados: Se encontraron en la revisión de los expedientes de los cuatro hospitales a 67 pacientes, hombres, con una incidencia de infecciones de 4.5%, al analizar los factores de riesgo éstos no presentaron asociación significativa. Discusión: Ningún factor de riesgo posee significancia estadística de que se presente algún proceso infeccioso, probablemente por un buen tratamiento inicial.


Abstract: Background: The introduction of firearms has increased deaths, assassinations and suicides. Firearm projectile injuries (PAF) increasingly affect civilian populations (500,000 injuries annually), it is the second cause of death in young people. Half required fracture treatment (45%), accompanied by reports of 6% who developed a deep infection. Material and methods: Retrospective, observational, cross-sectional study in four hospitals of the Secretary of Health of CDMX of the frequency of infection in fractures exposed by PAF and if there is any other factor that is associated with the infection. Results: In the review of the files of the four hospitals, 67 patients were found, men, with an incidence of infections of 4.5%, when analyzing the risk factors these did not present a significant association. Discussion: No risk factor has statistical significance for any infectious process to occur, probably due to a good initial treatment.

ABSTRACT

Resumo Objetivo: sistematizar a produção bibliográfica sobre a ocorrência de ferimentos por arma de fogo (FAF) entre profissionais de segurança pública, incluindo policiais e militares de forças armadas. Métodos: revisão integrativa realizada em bases bibliográficas eletrônicas brasileiras e internacionais (BVS, Scopus, Web of Science e Pubmed), sem estipulação de recorte temporal ou territorial. Resultados: foram encontrados 31 artigos segundo os critérios de inclusão e de exclusão. Notou-se aumento crescente de publicações entre 1976 e 2016, a maioria publicada em inglês, em periódicos da área da saúde. Os FAF foram o principal mecanismo de injúria em serviço entre os policiais, mais frequentes entre profissionais do sexo masculino, na faixa etária de 30 anos. As circunstâncias de maior risco foram: ações de prisão e confrontos com criminosos; respostas a chamados policiais para verificação de perturbação à ordem; e em situações de guerra. Existem poucos estudos que discutam as repercussões físicas dos FAF para o exercício profissional dos trabalhadores de segurança pública.


Abstract Objective: to systematize the bibliographic production on the occurrence of firearm injuries (FAI) among public safety professionals, including police officers and military personnel. Methods: integrative review carried out in Brazilian and international electronic bibliographic databases (Brazilian Virtual Health Library, Scopus, Web of Science and Pubmed), without time or geographic specification. Results: thirty-one articles were found according to the inclusion and exclusion criteria. There was a growing increase of publications between 1976 and 2016, most of them in English and published in health sciences journals. FAI were the mainspring of injury among police officers, most frequent in male professionals, aged range 30. The riskiest circumstances were: arrest actions and clashes with criminals; attendance to police calls to verify order disturbance; and in war situations. There are few studies discussing the physical repercussions of FAI for the public safety workers' professional practice.

Abstract The surgical management of firearm injuries to the zygomaticomaxillary complex is a topic that needs to be addressed with much care. The complications that can arise during treatment, as well as the potential residual problems that can appear, lead to the need of recognizing these injuries as a maxillofacial treatment challenge. The infraorbital zone is the second most common area of injury in the face, accounting for up to 45% of midfacial fractures. The case presented is of a 54 year old man affected by the backward firing of a gun rifle that caused an oblique impaction towards the posterior lateral inferior side of the infraorbital complex. He was rapidly stabilized with an antibiotic and analgesic scheme, yet presented the following: a comminute fracture of the orbit floor with consequent communication towards the maxillary sinus, a non-displacing fracture of the frontomalar suture, a non-displacing fracture of the zygomatic arch and a lesion with loss of osseous tissue in the petrous region of the temporal bone. Rigid fixation followed stabilization, and the area was covered with a titanium mesh. Occasional tinnitus and photopsia were reported by the patient upon return, yet all other evaluated factors were in order. There have been different forms of management described in the literature, and it is recognized that each patient has different needs, therefore each case must be closely evaluated, so it is approached in the most appropriate manner.


Resumen El manejo quirúrgico de las lesiones por armas de fuego al complejo cigomaticomaxilar representa una situación compleja en la práctica maxilofacial que debe ser abordada con extrema cautela, debido a las complicaciones que pueden surgir durante el tratamiento y los potenciales problemas residuales. La zona infraorbitaria es la segunda área más común de lesiones en la cara, representando hasta 45% de las fracturas mediocraneales. El presente caso es de un masculino de 54 años de edad, que sufrió trauma ocasionado por la parte trasera de un rifle de caza que causó un impacto en sentido oblicuo hacia el lado lateral inferior del complejo infraorbitario. El paciente fue estabilizado con esquema antibiótico y analgésico, el diagnóstico fue: fractura cominuta del piso de la órbita, con consecuente comunicación al seno maxilar, fractura no desplazada de la sutura frontomalar, fractura no desplazada del arco cigomático, y lesión con pérdida de tejido óseo en la región petrosa del hueso temporal. El tratamiento quirúrgico consistió en reducción de las fracturas y fijación rígida de las estructuras. Al control post operatorio el paciente refirió acufenos y fotopsia ocasional, no presentando otras alteraciones. Se ha descrito en la literatura diferentes abordajes para el manejo de estos traumatismos, sin embargo cada caso debe ser individualizado y evaluado de forma minuciosa para que éste, sea el adecuado.

@#Death due to firearm is more common in countries with higher gun ownership such as United States of America compared to countries with restrictive gun ownership such as European countries and Malaysia. However, the increasing number of shooting cases in Malaysia recently should not be overlooked. This study was carried out to investigate the incidence of firearm-related deaths and epidemiology in Klang Valley for 11-year period. The demographic data on the incidence of firearm-related deaths from 2006 to 2016 was collected retrospectively from four government hospitals in Klang Valley. 204 cases of firearm-related deaths were reported with irregular trend of the incidence throughout the 11-year period. The results revealed that most victims were male, aged between 30 to 34 years old and Malaysian citizen. Firearms fatalities were higher among Malaysian specifically Indian, as compared to non-Malaysian. Most firearms fatalities were homicides and took place during late night between 12.00 am to 5.59 am. The highest cause of death was head injury due to gunshot wound. It is hoped that this study will raise awareness to Malaysian citizen regarding firearm-related cases and to develop strategies to overcome the issue of gun violence in Malaysia with appropriate authorities.
