ABSTRACT

Objetivo: Analisar a produção do cuidado de enfermagem às pessoas em sofrimento psíquico atendidas em um hospital geral na perspectiva da corresponsabilidade e integralidade. Método:Foi realizado estudo qualitativo, exploratório, por meio da análise documental, observação participante e entrevista semiestruturada com 12 técnicos de enfermagem e 15 enfermeiros do Hospital Municipal de Serrinha-Bahia/Brasil. Os dados foram analisados apartir análise de conteúdo. Resultados:O atendimento psiquiátrico no hospital geral é visto pela maioria da equipe como porta de entrada para o primeiro atendimento, porém demonstram sentir receio, insegurança e despreparo para lidar com estes pacientes,sinalizando a necessidade de educação permanente para elaboração e integração de novos saberes. Considerações finais: Para efetivação do novo modelo de assistência à saúde mental, faz-se necessário a promoção de reflexão direcionada a desconstrução de preconceitos e estigmas previamente estabelecidos. Nesse sentido, torna-se imprescindível a compreensão das novas práticas para o enfrentamento do processo de transição de paradigma, que demanda dos profissionais, a disponibilidade para rever suas próprias percepções e práticas diante dos desafios advindos do processo de aproximação da pessoa em sofrimento psíquico no serviço de saúde.


Objective: To analyze the production of nursing care for people in psychological distress treated in a general hospital from the perspective of co-responsibility and comprehensiveness. Method:A qualitative, exploratory study was carried out using document analysis, participant observation and semi-structured interviews with 12 nursing technicians and 15 nurses from the Municipal Hospital of Serrinha-Bahia/Brazil. The data was analyzed using content analysis. Results:Psychiatric care in the general hospital is seen by most of the team as the gateway to first aid, but they feel afraid, insecure and unprepared to deal with these patients, signaling the need for ongoing education to develop and integrate new knowledge.Final considerations: In order to implement the new mental health care model, it is necessary to promote reflection aimed at deconstructing previously established prejudices and stigmas. In this sense, it is essential to understand the new practices in order to cope with the process of paradigm transition, which requires professionals to be willing to review their own perceptions and practices in the face of the challenges arising from the process of approaching people in psychological distress in the health service.

Método:Se realizó un estudio cualitativo y exploratorio mediante análisis de documentos, observación participante y entrevistas semiestructuradas con 12 técnicos de enfermería y 15 enfermeros del Hospital Municipal de Serrinha-Bahia/Brasil. Los datos se analizaron mediante análisis de contenido. Resultados:La atención psiquiátrica en el hospital general es vista por la mayoría del equipo como la puerta de entrada a los primeros auxilios, pero se sienten temerosos, inseguros y poco preparados para tratar con estos pacientes, lo que señala la necesidad de una formación continua para desarrollar e integrar nuevos conocimientos. Consideraciones finales: Para implementar el nuevo modelo de atención en salud mental es necesario promover una reflexión orientada a deconstruir prejuicios y estigmas previamente establecidos. En este sentido, es esencial comprender las nuevas prácticas para hacer frente al cambio de paradigma, lo que requiere que los profesionales estén dispuestos a revisar sus propias percepciones y prácticas frente a los desafíos derivados del proceso de acercamiento a las personas que sufren enfermedades mentales en el servicio de salud.

ABSTRACT

Objective To construct the assessment index system for general nursing goals management based on the performance structure theory. Methods Expert interviews and Delphi expert enquiry were used to complete the construction of index system, and SPSS17.0 was used to do data statistics and reliability analysis. Results The assessment index system for general nursing goals management was constructed which contained 6 level-Ⅰindicators such as nursing quality, nursing safety, nursing service, teaching and training, the contribution of department and the communication skills and 22 level- Ⅱ indicators. The department monthly examination scores calculation method and feedback mechanism were also established. The expert positive coefficients were 97.6%and 98.8%respectively, the authority coefficients were 0.85 and 0.86, the importance and feasibility mean scores of every index were 3.86-4.98 and 3.98-4.88, and coefficients of variation were 0.03-0.27 and 0.05-0.25 in 2 rounds of enquiry. The importance of level-Ⅰor level-Ⅱ indicators and operability coordination coefficients were 0.10-0.36, the significance test showed P<0.05. Conclusions The design of each dimension of the index system gives full consideration to 3 dimensions of performance: task-adaptive-relationship. Representative of key indicators is strong. The Delphi method used in building is scientific and the results are reliable. The maneuverability of calculate Method is strong, easy to quantify. The feedback mechanism has a strong incentive by combining the assessment results closely with the individual interests. The establishment of the index system can provide scientific and objective basis for the full implementation of the general nursing goals management.

ABSTRACT

Objective To investigate the performance of Orem’ sef-care theory application in the patients with a tendency of ovarian hyperstimulation syndrome (OHSS). Methods A total of 307 patients with a tendency of progressing OHSS, who were treated in Reproductive Medicine Center of Changhai Hospital of Second Military Medical University from Oct 2015 to Oct 2016, randomly received routine nursing management in combination with Orem’ sel--care theory (Orem’ sel--care theory group, n=165) or routine nursing management (routine group, n=142). The incidence and severity of OHSS of the patients in the two groups were compared and analyzed. Results Sixty-eight of 307 OHSS tendency patients finally developed OHSS, with 38 in the routine group (16 mild OHSS, 18 moderate and four severe), and 30 in the Orem’ sef-care theory group (20 mild, nine moderate and one severe). There was no significant difference in OHSS incidence between the two groups (P>0. 05); while the reduction of OHSS severity in the Orem’ sef-care theory group was more obvious than that in the routine group, and the difference was statistically significant (P<0. 05). Conclusion Orem’ sef-care theory nursing management has an advantage in reducing the severity of OHSS over routine nursing and is worthy of clinical use.

ABSTRACT

Objective To investigate the effect of virtual simulation teaching method in the comprehensive experimental teaching.Methods In experimental teaching of nursing,simulation medical records were compiled,plans were implemented,experiment guidance materials were preliminarily formed and teaching effect was evaluated.Results The application of virtual simulation teaching in nursing comprehensive experiments reflected the comprehensiveness of teaching content,and the comprehensiveness of knowledge,ability and capacity.It could train students creative ability,exploring spirit,enhance the sense of professional and humane care consciousness,train the communication skills and strain capacity,make active teaching atmosphere,improve the quality of teaching.Conclusions This teaching method can make students achieve mastery through a comprehensive multi-disciplinary knowledge,improve the ability of solving practical problems,help nurses to develop the comprehensive professional quality.

ABSTRACT

Objective To investigate the effect of general nursing intervention on the mental state of the patients with breast cancer undergoing chemotherapy.Methods Forty-eight breast cancer patients undergoing postoperative chemotherapy received nursing general interventions including healthy education,psychological nursing care,pantosomatous,relaxation therapy and rehabilitative exercise. All patients were evaluated by self-rating depression scale (SDS) and depression status inventory (DSI) before and after the general intervention.Result The scores of SDS and DSI after intervention were significantly lower than those before intervention (bothP<0.05).Conclusion The general nursing intervention could significantly improve patients’ psychological state and life quality,promote rehabilitation and improve their quality of life.
