ABSTRACT

Objective: To assess the prevalence of hypertension in children with infrequently relapsing nephrotic syndrome (IRNS) and its association with dyslipidemia, and end organ damage including left ventricular hypertrophy (LVH), at relapse and after steroid induced remission. Methods: Prospective observational study conducted in 83 children aged 1-12 years with IRNS, presenting in relapse. Blood pressure, fundus examination, blood and urine investigations were done at relapse and then at 4 weeks of therapy. Echocardiography at 4 weeks was performed for assessment of LVH and relative wall thickness (RWT) for concentric geo-metry (CG). Results: 27 patients (32.5%) developed hypertension, out of which 21 patients (25.3%) had stage I hypertension. Hypertension in first episode (63.0%, P<0.01) and in previous relapses (87.5%, P<0.001) was significantly associated with hypertension in the current episode. 12 patients had a positive family history of hypertension, of which 8 (66.7%) were classified under the hypertensive group (P=0.016). Concentric geometry (CG) was found in 28% of hypertensive and 5.5% of non-hypertensive children (P=0.011). On regres-sion analysis, a lower Up:Uc at the time of relapse was found to have a protective role for development of hypertension. Conclusion: One third children with IRNS had hypertension at relapse and a high proportion of hypertensive patients had CG pattern on echocardiography.

ABSTRACT

Background: Hypertension is considered as the silent killer of human because the hypertensive patient may be not aware that he has hypertension and death can occur at any time without the reason being known. We aim to study the effect of gray zone hypertension [SBP from 120-139] and or [DBP from 80-89] which was classified as prehypertension under JNC-7 on the structure, geometry, and operation of the left ventricle using echocardiography Methods: This study included 100 patients who asymptomatic apparent healthy subjects visited cardiology outpatient department (OPD) for routine checkup. Patients were grouped into two equal groups: Group A whose blood pressure was in the gray zone hypertension [SBP from 120-139] and or [DBP from 80-89], and group B with SBP <120 mmHg & DBP <80 mmHg. Results: The two groups being studied were statistically significant different as regard LV geometrical pattern (p=0.028). They were also statistically significant in LV geometrical pattern between the males and females (p<0.05). The two groups were statistically significant different regarding GLS (p =0.001). Conclusions: Gray zone hypertension affects ventricular diastolic function and LV geometry, although systolic function was normal; GLS showed that subclinical LV dysfunction can occur.

ABSTRACT

Abstract This study evaluated a method to determine the spatial geometry of root canal preparation (RCP) using navigation dynamics and a specific algorithm from a new CBCT software (e-Vol DX). CBCT scans of 168 root canals of mandibular molars were acquired before and after RCP, using nickel-titanium (NiTi) instruments (ProTaper Next, BioRace, Reciproc Blue, and WaveOne Gold). The spatial geometry of the root canals and the operative risk of disproportional wear of dentinal walls after RCP were evaluated using a new CBCT software. A 3-point scoring system was used after the measuring of cementum/dentin thickness before and after RCP in all root thirds. The root thirds were distributed into three parts of similar sizes, and the scores were categorized at three levels: 1. mild risk (1/3), 2. moderate risk (2/3), 3. severe risk (3/3). These levels were proposed according to the risk of creating disproportionate shapes, thin walls, or perforations. The data were analyzed statistically by Fischer's exact test (α = 5%). There were no significant differences in operative risk among the NiTi engine-driven systems, for the distal or mesial walls of all the root canal thirds (p>0.05). The spatial geometry method to assess operative risk allows clinical planning for a predictable enlargement of the root canal in all root thirds. Based on using a map-reading strategy on root canals in CBCT scans, NiTi engine-driven instruments did not present an increased operative risk during RCPs.


Resumo Este estudo avaliou um método para determinar a geometria espacial do preparo do canal radicular (PCR) usando uma dinâmica de navegação e um algoritmo específico do software e-Vol DX. Imagens de tomografia computadorizada de feixe cônico (TCFC) de 168 molares inferiores foram adquiridas antes e depois do PCR, usando instrumentos de níquel-titânio (NiTi) (ProTaper Next, BioRace, Reciproc Blue e WaveOne Gold). A geometria espacial dos canais radiculares e o risco operatório de desgaste desproporcional das paredes dentinárias após o preparo foram avaliados usando o software de TCFC e-Vol DX. Um sistema de Score de 3 pontos foi usado após a mensuração da espessura cemento/dentina antes e depois do PCR, em toda extensão dos canais radiculares. Os terços radiculares foram distribuídos em três partes de tamanhos semelhantes, e os Scores foram categorizados em três níveis: 1. risco leve (1/3), 2. risco moderado (2/3), 3. risco severo (3/3). Esses níveis foram propostos de acordo com o risco de criar formas desproporcionais, paredes finas ou perfurações radiculares. Os dados foram analisados estatisticamente pelo teste exato de Fischer ((=5%). Não houve diferenças significativas no risco operatório entre os sistemas de NiTi acionados a motor, para as paredes distal ou mesial em todos os terços do canal radicular (p>0,05). O método de geometria espacial para avaliar o risco operatório permite o planejamento clínico para um alargamento previsível do canal radicular em todos os terços radiculares. Com base no uso de uma estratégia de navegação dinâmica de canais radiculares de molares inferiores analisados em imagens de TCFC, os instrumentos de NiTi acionados estudados não apresentaram um aumento do risco operatório durante o preparo dos canais radiculares.

ABSTRACT

Objective:To investigate the effect of myopia on retinal vascular geometry in young subjects.Methods:A retrospective cross-sectional study. From June 2018 to December 2018, 235 participants (235 eyes) who took part in routine physical examination in Huadong Sanatorium were included. There were 94 males and 141 females; age was (34.89±6.15) years old; equivalent spherical refraction (SE) was (-3.78±3.25) D. 59 (25.11%, 59/235) were divided into high myopia group (SE≤-6.0 D), along with 131 (55.74%, 131/235) low to moderate myopia group (-0.5 D> SE>-6.0 D), and 45 (19.15%, 45/235) emmetropia group (0.5 D≥SE≥-0.5 D). Retinal vascular geometric measurements, including central retinal arteriolar equivalent (CRAE), central retinal venular equivalent (CRVE), fractal dimension arteriole (FDa), fractal dimension venule (FDv), curvature tortuosity arteriole (CTORTa), curvature tortuosity venule, branch angle arteriole (BAa), branch angle venule, branch coefficient arteriole and branching coefficient venule, were extracted by using a validated computer program. One-way analysis of variance and analysis of covariance were performed to compare the measurements across the high myopia, low to moderate myopia, and emmetropia groups. Linear regression analysis was used to explore the relationship between SE and retinal vascular geometric parameters.Results:The differences in CRAE ( F=65.11), CRVE ( F=61.52), FDa (F=14.26), FDv ( F=8.31), CTORTa ( F=5.07) and BAa (F=6.51) among eys of high myopia group, low to moderate myopia group and emmetropia group remained significant ( P<0.05) after adjusting for age, glycosylated hemoglobin, mean arterial pressure, body mass index, and intraocular pressure. CRAE and CRVE were linearly correlated with the SE ( P<0.05). FDa, FDv, cTORTa and BAa decreased with the decrease of SE in high myopia ( P<0.05). Conclusions:Myopia is associated with the change of the retinal vascular geometric characteristics. With the deepening of myopia, the change of retinal vascular geometric characteristics gradually worsens.

ABSTRACT

Objective:To explore the changes of bone turnover markers and geometric parameters of hip bone in overweight postmenopausal women with metabolic syndrome(MS), as well as the influence of MS components. To analyze the association of these factors with the risk of fracture.Methods:A total of 505 overweight postmenopausal female patients who underwent health check-up in Lianhu Community Service Center, Danyang City, Jiangsu Province from January to December 2017 were selected. According to the MS diagnostic criteria of the International Diabetes Federation(2009), the patients were divided into MS group( n=331)and non-MS group( n=174). Blood samples were collected to determine the level of procollagen type 1 N-terminal propeptide(P1NP)and carboxy-terminal cross-linked telopeptide of type 1 collagen(CTX). Bone mineral density and hip bone geometry parameters were tested with dual-energy X-ray absorptiometry and hip structural analysis software. Results:The incidence of osteoporotic fracture and hip fracture in MS group was significantly higher than that in non-MS group(21.1% vs 13.8%, 4.8% vs 1. 1%, P<0.05). However, the bone mineral density of lumbar vertebra 1-4, femoral neck, and total hip in MS group was significantly higher than that in non-MS group, which remained after adjusting for age( P<0.05), but the difference disappeared after further adjustment for body mass index( P>0.05). The P1NP, CTX, femur strength index(FSI), section modulus(SM), and cross-sectional area(CSA)of MS group were significantly lower than those of non-MS group, the buckling ration(BR)was significantly higher than that in non-MS group, and the differences were still statistically significant after adjusting for age and body mass index( P<0.05). There was no significant difference in bone mineral density of lumbar vertebra 1-4, femoral neck, total hip, P1NP, and CTX between fracture group and non-fracture group in patients with MS. But FSI, SM, cross-sectional moment of inertia(CSMI), and CSA were significantly lower, BR was significantly higher( P<0.05) and femur strength decreased in patients with fracture. Regression analysis showed that high BR was an independent risk factor for fracture risk, while high FSI, SM, CSMI, and CSA were protective factors. Multivariate linear regression analysis showed that wasit circumference, diastolic blood pressure, and fasting plasma glucose were the main MS components affecting bone mineral density, bone turnover indexes, and hip bone geometry parameters. Conclusions:Overweight postmenopausal MS patients had decreased bone turnover rate, femoral strength, and relatively poor bone quality. Hip bone geometry parameters can be used as one of the methods to assess fracture risk in MS patients. Waist circumference, diastolic blood pressure, and fasting blood glucose are the important MS components affecting bone mass and bone quality.

ABSTRACT

Objective:To investigate the variations and related influencing factors of the volumes, positions, and morphologies of geometries formed by surgical clips in intensity-modulated radiation therapy (IMRT) for whole breasts after breast-conserving surgery.Methods:A retrospective analysis was conducted on 18 patients treated with IMRT from October 2021 to September 2022 after breast-conserving surgery. Set-up errors were obtained by reading the coordinate information of tumor bed surgical clips according to the cone beam CT (CBCT) images at 0, 10, 20, 30, 40, and 50 Gy sequentially. Geometries were constructed using the convex hull program, and the volumes ( Vct and V0-5) of geometries were then calculated based on the localization CT images and the six CBCT images, respectively. The centroid displacements ( D0-5) and degrees of inclusion ( DI0-5) of these geometries were compared. The t-test or rank-sum test was used to analyze the effects of radiation doses on V0-5, D0-5, and DI0-5. The Pearson correlation analysis was conducted to analyze the correlations between D0-5 and corresponding set-up errors in three-dimensional directions, and the correlations of average V0-5 variation ( AV), average D0-5 ( AD), and average DI0-5 ( ADI) with the body mass index (BMI), the volume of a breast ( VB), the breast axial height ( H), and the time interval from surgery to radiotherapy ( T) each. The influences of quadrants ( Q) where geometries are located and states ( S) of geometries adhering to or away from the chest walls on AV, AD, and ADI were analyzed through one-way ANOVA. Results:The differences between D5 and D0, between D5 and D1, and between D5 and D2 were statistically significant ( t = -3.27, -4.52, -3.38, P < 0.05), respectively. The differences between DI5 and DI0, between DI5 and DI1, between DI5 and DI2 were statistically significant ( t = 2.53, 2.70, 2.64, P < 0.05), respectively. D1 and D4 correlated with the set-up errors in the superior-inferior dimension, and D3 correlated with the set-up errors in the left-right dimension ( r = 0.50, 0.56, 0.53, P < 0.05). The AD was related to BMI, VB, and H ( r = 0.54, 0.48, 0.50, P < 0.05). There were statistically significant differences in AV and ADI of geometries away from ( S3) and near ( S2) chest walls ( F = 2.66, 3.83, P < 0.05). The AD differences of geometries between S3 and the state adhering to chest walls ( S1) and between S3 and S2 were statistically significant ( F = 7.46, P < 0.05), respectively. Conclusions:The geometries are relatively constant volumes during whole-breast radiotherapy. However, their positions and morphologies vary greatly in the late course. The volume variation ratios, centroid displacements, and degrees of inclusion of geometries are all affected by the states of their attachments to the chest walls. The centroid displacements correlate with breast morphologies and BMI. The radiotherapy plan modification can be individualized.

ABSTRACT

Objective:To investigate the correlations between serum E selectin, intercellular adhesion molecule 1 (ICAM-1), vascular cell adhesion molecule 1 (VCAM-1) and left ventricular geometry and function in patients with obstructive sleep apnea syndrome (OSAS) combined with prehypertension (pre-HT).Methods:A total of 462 patients with pre-HT and OSAS diagnosed by polysomnography (PSG) in the sleep monitoring unit of the Department of Respiratory and Critical Care Medicine at the First Hospital of Shanxi Medical University from July 2019 to July 2022 were restrospectively analysed, and 52 patients with pure pre-HT (pre-HT group) and 73 patients with pure OSAS (OSAS group) in the same period were selected as the control group. OSAS and pre-HT patients were divided into four groups according to left ventricular geometry: normal geometry (NG) group, concentric remodeling (CR) group, eccentric hypertrophy (EH) group and concentric hypertrophy (CH) group. The general clinical data, PSG parameters, blood biochemical parameters and left ventricular structure and function parameters were compared among the six groups. Pearson correlation and multivariate Logistic regression were used to analyze the correlation between E-selection, ICAM-1, VCAM-1, general clinical data, PSG parameters, blood biochemical parameters with left ventricular geometry and function.Results:①Serum E selectin, ICAM-1, and VCAM-1 concentrations increased sequentially from the NG, CR, and EH to CH groups, with the most significant increase in CH group (all P<0.05). In addition, there were statistically significant differences in age, body mass index (BMI), OSAS severity, neck circumference, waist circumference, systolic blood pressure (SBP), diastolic blood pressure (DBP), Glu, lowest oxygen saturation (Lowest-SaO 2), mean oxygen saturation (Mean-SaO 2), percentage of time with oxygen saturation below 90% of total sleep time (T90), left ventricular end-diastolic diameter (LVEDd), interventricular septal thickness (IVST), left ventricular posterior wall thickness (LVPWT), left ventricular mass index (LVMI), relative ventricular wall thickness (RWT), left ventricular ejection fraction (LVEF), peak mitral early diastolic flow velocity/peak mitral late diastolic flow velocity (E/A), E wave deceleration time (DT), A wave duration (AD), and isovolumic relaxation time (IVRT), and overall long-axis longitudinal strain (GLS) and so on(all P<0.05). ②Pearson correlation analysis showed that E selectin was negatively correlated with LVEF, E/A, e′, E/e′, IVRT, and GLS ( r=-0.236, -0.131, -0.224, -0.215, -0.285, -0.336; all P<0.05). ICAM-1 was negatively correlated with LVEF, E, E/A, e′, IVRT, and GLS( r=-0.130, -0.129, -0.104, -0.351, -0.252, -0.259; all P<0.05). VCAM-1 was negatively correlated with E, e′, and IVRT ( r=-0.132, -0.312, -0.387; all P<0.001). ③Multifactorial logistic regression analysis showed that E selectin and VCAM-1 were independently correlated with EH (β=1.139, OR=3.124, P=0.030; β=1.288, OR=3.626, P<0.001) and with CH (β=1.178, OR=3.248, P=0.013; β=1.108, OR=3.028, P<0.001). Conclusions:E selection and VCAM-1 were independently correlated with hypertrophic left ventricular geometry, suggesting that E selectin and VCAM-1 may be involved in the process of abnormal left ventricular structure and function in patients with OSAS combined with pre-HT.

ABSTRACT

Reconstructing three-dimensional (3D) models from two-dimensional (2D) images is necessary for preoperative planning and the customization of joint prostheses. However, the traditional statistical modeling reconstruction shows a low accuracy due to limited 3D characteristics and information loss. In this study, we proposed a new method to reconstruct the 3D models of femoral images by combining a statistical shape model with Laplacian surface deformation, which greatly improved the accuracy of the reconstruction. In this method, a Laplace operator was introduced to represent the 3D model derived from the statistical shape model. By coordinate transformations in the Laplacian system, novel skeletal features were established and the model was accurately aligned with its 2D image. Finally, 50 femoral models were utilized to verify the effectiveness of this method. The results indicated that the precision of the method was improved by 16.8%-25.9% compared with the traditional statistical shape model reconstruction. Therefore, the method we proposed allows a more accurate 3D bone reconstruction, which facilitates the development of personalized prosthesis design, precise positioning, and quick biomechanical analysis.

ABSTRACT

The Monte Carlo N-Particle (MCNP) is often used to calculate the radiation dose during computed tomography (CT) scans. However, the physical calculation process of the model is complicated, the input file structure of the program is complex, and the three-dimensional (3D) display of the geometric model is not supported, so that the researchers cannot establish an accurate CT radiation system model, which affects the accuracy of the dose calculation results. Aiming at these two problems, this study designed a software that visualized CT modeling and automatically generated input files. In terms of model calculation, the theoretical basis was based on the integration of CT modeling improvement schemes of major researchers. For 3D model visualization, LabVIEW was used as the new development platform, constructive solid geometry (CSG) was used as the algorithm principle, and the introduction of editing of MCNP input files was used to visualize CT geometry modeling. Compared with a CT model established by a recent study, the root mean square error between the results simulated by this visual CT modeling software and the actual measurement was smaller. In conclusion, the proposed CT visualization modeling software can not only help researchers to obtain an accurate CT radiation system model, but also provide a new research idea for the geometric modeling visualization method of MCNP.

Abstract Objective: To explore the diagnostic utility of 31 electrocardiogram (ECG) criteria for detecting echocardiographic (Echo) left ventricular geometry using accuracy. Methods: This cross-sectional study included consecutive adults (> 18 years) that were classified by Echo left ventricular geometry as normal (NL), concentric remodeling (CR), concentric hypertrophy (CH), and eccentric hypertrophy (EH). Thirty-one state-of-the-art ECG criteria for Echo left ventricular hypertrophy were calculated. AUC 95%CI, accuracy, sensitivity, specificity, and positive and negative predictive value for detecting Echo left ventricular geometries were compared. Multivariable linear regression models were produced using the ECG criteria as the dependent variable. Results: A total of 672 adults were included in the study. From 31 ECG criteria, Cornell (ECG21, SV3 + RaVL) and modified Cornell (ECG 31, RaVL + deepest S in all leads) criteria have the best overall AUC in differentiating NL versus CH (0.666 and 0.646), NL versus EH (0.686 and 0.656), CR versus CH (0.687 and 0.661), and CR versus EH (0.718 and 0.676). In multivariable linear regression models, CH and EH had the strongest effect on the final voltage in Cor- nell (ECG21) and modified Cornell (ECG31). Conclusions: From 31 state-of-the-art criteria, Cornell and modified Cornell criteria have the best AUC and accuracy for predicting most left ventricular geometries. CH and EH had the strongest effect on the voltage of Cornell and modified Cornell criteria compared to body mass index, age, diabetes, hypertension, and chronic heart disease. The ECG criteria poorly differentiate NL from CR and CH from EH.


Resumen Objetivo: Explorar la utilidad diagnóstica de 31 criterios de ECG para detectar la geometría ecocardiográfica del ventrículo izquierdo usando la exactitud, área bajo la curva, sensibilidad, especificidad, y valor predictivo positivo y negativo. Métodos: Este estudio transversal incluyó adultos (> 18 años) que se sometieron a ECG y ecocardiograma transtorácico. Los pacientes fueron clasificados según la geometría del ventrículo izquierdo: normal (NL), remodelado concéntrico (RC), hipertrofia concéntrica (HC) e hipertrofia excéntrica (HE). Se calcularon 31 criterios clásicos de ECG para detectar hipertrofia ventricular izquierda y se comparó el rendimiento diagnóstico en cada geometría. Creamos un modelo de regresión lineal múltiple usando los criterios de ECG como variable dependiente. Resultados: Se incluyeron 672 adultos. Los criterios de Cornell (ECG 21, SV3 + RaVL) y Cornell modificado (ECG31, RaVL + S mas profunda de las 12 derivaciones) tienen el mejor AUC para diferenciar NL versus HC (0.666 y 0.646), NL versus HE (0.686 y 0.656), RC versus HC (0.687 y 0.661) y RC versus HE (0.718 y 0.676). En el análisis multivariado la geometría del ventrículo izquierdo (HC e HE) fue la variable que mas influyó en el resultado final del criterio de Cornell y de Cornell modificado. Conclusión: De los 31 criterios clásicos explorados, los criterios de Cornell y Cornell modificado tienen el mejor AUC y exactitud para predecir la mayoría de las geometrías del ventrículo izquierdo. Los criterios del ECG no diferencian bien la geometría NL del RC ni HC de la HE.

Introducción: Las fístulas perianales complejas siguen siendo un auténtico desafío terapéutico en nuestros días, dada la ausencia de una terapia universal y los resultados a largo plazo, ya que el porcentaje de complicaciones o recidivas no es desdeñable. Entre las opciones terapéuticas encontramos los tapones biológicos, siendo respetuosos con los esfínteres, pero con resultados dispares. Objetivos: Evaluar los resultados a largo plazo tras la colocación de un tapón de colágeno piramidal en dos pacientes con fístula perianal compleja. Materiales y Método: Se presentan 2 pacientes con fístula perianal compleja intervenidos mediante colocación de tapón biológico cumpliendo estrictos criterios de inclusión. Discusión: Dentro del arsenal terapéutico disponible actualmente, los tapones biológicos prometen una solución definitiva para determinadas fístulas perianales, evitando una posible incontinencia. Con resultados dispares en la bibliografía, en nuestros dos casos los pacientes han evolucionado de forma excelente, con resolución completa de la enfermedad fistulosa sin complicaciones ni recidiva a largo plazo. Conclusiones: Dados los excepcionales resultados obtenidos en nuestros dos primeros casos con este tipo de tapón, nos planteamos si la morfología del mismo puede ser determinante en su éxito.


Introduction: Complex perianal fistulas continue to be a real therapeutic challenge today, given the absence of universal therapy and long-term results, since the percentage of complications or recurrences is not negligible. Among the therapeutic options we find biological plugs, being respectful with the sphincters but with disparate results. Aim: To evaluate the long-term results after the placement of a pyramidal collagen plug in two patients with complex perianal fistula. Materials and Method: We present two patients with complex perianal fistula who were operated on by placing a biological plug, meeting strict inclusion criteria. Discussion: Within the therapeutic arsenal currently available, biological plugs promise a definitive solution for certain perianal fistulas, avoiding possible incontinence. With different results in the literature, in our two cases the patients have evolved excellently, with complete resolution of the fistulous disease without complications or long-term recurrence. Conclusions: Considering the exceptional results obtained in our first two cases with this type of plug, we wonder if its morphology can be a determining factor in its success.

ABSTRACT

Nasal spray can treat local diseases such as rhinitis, it also plays an important role in the treatment of systemic diseases including vaccine immunity. As a drug-device combination product, spray pattern is often used as the quality indicator of nasal spray to ensure its quality, plume geometry can not only be combined with the spray pattern to evaluate the performance of the nasal spray, but also can predict the deposition of the nasal spray in the nasal cavity. This article systematically reviews the definition and measurement methods of the spray pattern and plume geometry of nasal spray and their correlation with nasal deposition behavior. The measurement parameters of spray pattern and plume geometry are defined. The influence of formulation, device and trigger parameters on spray pattern and plume geometry is clarified. The correlation between various parameters and nasal deposition is analyzed. The measurement parameters are classified according to the size and shape of the spray. Plume angle is closely related to the deposition of drugs in the nasal cavity. Jet-like plume with a smaller plume angle can increase the navigation ability of the nasal spray in the curved anatomical structure of the nasal cavity, which is conducive to increase drug deposition. This makes it possible to increase deposition of the nasal spray in the nasal cavity via appropriately increasing viscosity and thixotropy of the nasal spray formulation. This review provides the theoretical basis for the high quality nasal spray product development.

ABSTRACT

Objective:To investigate the correlation between serum cystatin C (Cys C) and left ventricular geometry in patients with obstructive sleep apnea syndrome (OSAS) complicated with prehypertension(PH).Methods:A total of 408 patients with PH and OSAS diagnosed by polysonogram monitoring in the sleep monitoring room of Respiratory Department, the First Hospital of Shanxi Medical University from July 2018 to June 2021 were collected. Serum Cys C concentration and echocardiography were performed in all patients. According to the classification of left ventricular geometry, all patients were divided into four groups: normal configuration (NG) group( n=297), concentric remodeling (CR) group( n=49), eccentric hypertrophy (EH) group( n=33), and concentric hypertrophy (CH) group( n=29). General clinical data, sleep parameters, blood biochemical parameters, Cys C concentration and echocardiographic parameters were compared among the four groups, and the correlation between serum Cys C and left ventricular geometry was analyzed. Results:①The serum Cys C concentration increased successively from NG group, CR group, EH group to CH group, and the increase in CH group was the most obvious ( P<0.05). In addition, there were statistically significant differences in body mass index (BMI), waist circumference, systolic blood pressure (SBP), diastolic blood pressure (DBP), severity of OSAS, the percentage of the time that the blood oxygen saturation was less than 90% of the total sleep time (T90), lowest oxygen saturation (Lowest-SaO 2), mean oxygen saturation (Mean-SaO 2), left ventricular end diastolic diameter (LVEDD), inter-ventricular septal diameter (IVST), left ventricular posterior wall thickness diameter (LVPWT), left ventricular mass index (LVMI), relative wall thickness (RWT), left ventricular ejection fraction (LVEF) and E/A among all groups (all P<0.05). ②Multivariate Logistic regression analysis showed that Cys C was independently correlated with CR (β=0.721, OR=2.057, P=0.047), EH(β=0.961, OR=2.614, P=0.017) and CH (β=1.180, OR=3.254, P=0.010). Conclusions:There is a correlation between serum Cys C and left ventricular geometry in OSAS patients with PH, suggesting that serum Cys C might be involved in the change of left ventricular geometry.

ABSTRACT

Transfer learning is provided with potential research value and application prospect in motor imagery electroencephalography (MI-EEG)-based brain-computer interface (BCI) rehabilitation system, and the source domain classification model and transfer strategy are the two important aspects that directly affect the performance and transfer efficiency of the target domain model. Therefore, we propose a parameter transfer learning method based on shallow visual geometry group network (PTL-sVGG). First, Pearson correlation coefficient is used to screen the subjects of the source domain, and the short-time Fourier transform is performed on the MI-EEG data of each selected subject to acquire the time-frequency spectrogram images (TFSI). Then, the architecture of VGG-16 is simplified and the block design is carried out, and the modified sVGG model is pre-trained with TFSI of source domain. Furthermore, a block-based frozen-fine-tuning transfer strategy is designed to quickly find and freeze the block with the greatest contribution to sVGG model, and the remaining blocks are fine-tuned by using TFSI of target subjects to obtain the target domain classification model. Extensive experiments are conducted based on public MI-EEG datasets, the average recognition rate and Kappa value of PTL-sVGG are 94.9% and 0.898, respectively. The results show that the subjects' optimization is beneficial to improve the model performance in source domain, and the block-based transfer strategy can enhance the transfer efficiency, realizing the rapid and effective transfer of model parameters across subjects on the datasets with different number of channels. It is beneficial to reduce the calibration time of BCI system, which promote the application of BCI technology in rehabilitation engineering.

RESUMEN Introducción. En hematología, el estudio de las alteraciones de la morfología eritrocitaria contribuye con el diagnóstico de la normalidad o anormalidad de estas estructuras, sin embargo, el carácter cualitativo de los criterios diagnósticos dificulta su interpretación y alcance. Objetivo. Caracterizar los eritrocitos nucleados de tilapia roja (Oreochromis spp), en el contexto de la geometría fractal y euclidiana. Metodología. Se tomaron 50 eritrocitos nucleados de 20 extendidos de sangre de tilapia roja. Posteriormente todos los contornos del núcleo y el citoplasma de los eritrocitos fueron delineados, para superponer dos rejillas, una con el doble tamaño que la otra, para calcular mediante el método de Box Counting la dimensión fractal de cada eritrocito delineado. Adicionalmente fue calculada la superficie de estas dos partes del eritrocito. Resultados: Los resultados de este estudio revelaron que los valores de la dimensión fractal no permiten hacer comparaciones entre eritrocitos nucleados. Por su parte, la superposición de rejillas de 5x5 y 10x10 píxeles permitió observar que los valores de ocupación del citoplasma y el núcleo permiten hacer comparaciones entre los eritrocitos nucleados, junto con los valores de la superficie de estas dos partes del eritrocito nucleado. Conclusión: Los eritrocitos nucleados de tilapia roja pueden ser caracterizados mediante la medición de los valores espacios ocupados por su citoplasma y el núcleo, junto con los valores de la superficie de cada una de estas dos partes del eritrocito.


ABSTRACT Introduction. In hematology, the study of erythrocyte morphology alterations contributes to the diagnosis of normality or abnormality of these structures. However, the qualitative nature of the diagnostic criteria makes their interpretation and scope difficult. Objective. Characterize the nucleated erythrocytes of red tilapia (Oreochromis spp) in the context of fractal and Euclidean geometry. Methodology. Fifty nucleated erythrocytes were taken from twenty red tilapia blood smears. Subsequently, all the contours of the nucleus and the cytoplasm of the erythrocytes were delineated to superimpose two grids, one twice the size of the other, to calculate the fractal dimension of each delineated erythrocyte using the Box Counting method. Additionally, the surface of these two parts of the erythrocyte was calculated. Results: This study revealed that the fractal dimension values do not allow comparisons between nucleated erythrocytes. The superposition of 5x5 and 10x10 pixel grids allowed us to observe that the occupancy values of the cytoplasm and the nucleus allow comparisons between the nucleated erythrocytes, together with the values of the surface of these two parts of the nucleated erythrocyte. Conclusion: Red tilapia nucleated erythrocytes can be characterized by measuring the values of the spaces occupied by their cytoplasm and nucleus, together with the values of the surface of each of these two parts of the erythrocyte.

Abstract Introduction: Morphometrics analysis is an efficient and low-cost technique used in studies of sexual dimorphism in turtles. Kinosternon scorpioides scorpioides, scorpion mud turtle, has a wide phenotypic variation, depending on the area of its occurrence. Objective: The objective of this work was to identify the anatomical sexual difference of K. s. scorpioides, adults and hatchlings, through morphometric analysis; and relate the weights of adult animals to environmental factors (temperature and rainfall) in Marajó Island, Brazil. Methods: The sample collection was carried out from March to September 2018, covering both the rainy season (January to July) and the dry season (August to December). For the biometric analysis, 95 adults and 21 hatchlings were used, in which the length and width of the carapace and plastron, height of the shell, and weight were measured (adults only). For the geometric morphometry analysis, 21 adults and 13 hatchlings were used, in which 27 coordinates of anatomical landmarks were inserted in each image of the carapace and 11 in the plastron. Hatchlings were sexed by histology which was enabled by the identification of the ovaries and testicles. Results: The results showed the existence of dimorphism in adults. The plastron and height were higher in females, which had a more rounded carapace than males. This characteristic may be related to the species' sexual strategy, where males impose copulation. Histologically, it was possible to identify the ovaries and testicles in the hatchlings, but there was no anatomical sexual difference, despite the tendency to differentiate in the analysis of carapace PCA. Conclusions: Sexual dimorphism in K. s. scorpioides may play an important role on its reproductive behavior, which is synchronized with environmental events. This fact suggests that the reproductive strategies of this species would be severely affected by changes in the ecosystem.


Resumen Introducción: La morfometría es una técnica eficiente y de bajo costo, utilizada en estudios de dimorfismo sexual en tortugas, mediante el marcado de puntos anatómicos. Este grupo incluye Kinosternon scorpioides scorpioides, con una amplia variedad fenotípica, dependiendo del área en la que se encuentra presente. Objetivo: Este estudio tuvo como objetivo identificar la diferencia sexual entre K. s. scorpioides, adultos y juveniles, a través del análisis morfométrico; y verificar la relación entre las medidas de peso para adultos y los factores ambientales (temperatura y precipitación) en la isla de Marajó, Brasil. Métodos: La recolecta se realizó de marzo a septiembre de 2018, cubriendo tanto la temporada de lluvias (enero a julio) como la estación seca (agosto a diciembre). Para el análisis biométrico, se utilizaron 95 animales adultos y 21 crías, en los que se midió la longitud y el ancho del caparazón y el plastrón, la altura del casco y el peso en adultos. Para la morfometría geométrica, se utilizaron 21 adultos y 13 juveniles, en los que se identificaron 27 coordenadas de puntos de referencia anatómicos en el caparazón y 11 en el plastrón. Los individuos recién eclosionados fueron sexados por histología de rutina, que permite identificar ovarios y testículos. Resultados: Los datos analizados mostraron que existe dimorfismo sexual en adultos. El plastrón y la altura fueron mayores en las hembras, que tenían un caparazón más redondeado que los machos. Esta característica puede estar relacionada con la estrategia sexual de la especie, donde los machos imponen la cópula. En los juveniles recién eclosionados fue posible identificar histológicamente los ovarios y los testículos, pero no hubo diferencias sexuales en relación con la morfometría corporal, a pesar de la tendencia a diferenciarse en el análisis de PCA de caparazón. Conclusiones: El dimorfismo sexual en K. s. scorpioides puede desempeñar un papel importante en su comportamiento reproductivo, que ocurre en sincronía con los eventos ambientales. Este hecho sugiere que las estrategias reproductivas de esta especie se verían gravemente afectadas por los cambios en el ecosistema.

ABSTRACT

BACKGROUND: At present, the standard method for clinical treatment of bone defects is autologous bone transplantation. However, the source of autologous bone is limited and it can cause new trauma and defects. Therefore, the development of ergonomic tissue-engineered bone has become the trend of repairing bone defects. OBJECTIVE: To summarize the characteristics and manufacturing techniques of collagen/inorganic materials to construct tissue-engineered bone. METHODS: PubMed, Web of Science (January 2000–January 2020) and CNKI (2010–2020) databases were used online to retrieve the relative articles regarding the application of collagen/inorganic composite scaffold. The key words were “collagen; inorganic materials; tissue-engineered bone; bone repair; geometry of bone scaffold” in English and Chinese, respectively. RESULTS AND CONCLUSION: Applying the principle of bionics, collagen composite inorganic materials were used to construct tissue-engineered bones by simulating the composition, structure, and characteristics of natural bones, which is an important direction in the field of bone graft materials. Collagen provides the extracellular matrix effect of the scaffold material, and promotes the migration, adhesion, and proliferation of cells on the scaffold, and accelerates the degradation of the scaffold in the body, and makes the composite scaffold has good biocompatibility and osteoinductivity. Choosing a suitable manufacturing technology to change the geometry of the collagen/inorganic material scaffold, and achieving the best three-dimensional porous nanoscale structure can promote cell adhesion and proliferation, and enhance the bone regeneration capacity of collagen/inorganic material scaffolds. Simultaneously, the surface of collagen/inorganic scaffolds is modified. Stem cells, biological factors, and polymers with osteogenic or angiogenic potential were compounded onto collagen/inorganic material scaffolds, which will improve the poor mechanical properties, insufficient structural stability, and limited bone regeneration capacity of collagen/inorganic material scaffolds. This provides a good idea for the option of tissue-engineered bone graft materials.

ABSTRACT

This study explored the variation of bursting force of multi-chamber infusion bag with different geometry size, providing guidance for its optimal design. Models of single-chamber infusion bag with different size were established. The finite element based on fluid cavity method was adopted to calculate the fluid-solid coupling deformation process of infusion bag to obtain corresponding critical bursting force. As a result, we proposed an empirical formula predicting the critical bursting force of one chamber infusion bag with specified geometry size. Besides, a theoretical analysis, which determines the force condition of three chamber infusion bag when falling from high altitude, was conducted. The proportion of force loaded on different chamber was gained. The results indicated that critical bursting force is positively related to the length and width of the chamber, and negatively related to the height of the chamber. While the infusion bag falling, the impact force loaded on each chamber is proportional to the total liquid within it. To raise the critical bursting force of in fusion bag, a greater length and width corresponding to reduced height are recommended considering the volume of liquid needed to be filled in.

Introducción: Hace años, Cuba, en su Anuario Estadístico de Salud, viene presentando las enfermedades del corazón como la primera causa de muerte. Asociados a la morbimortalidad cardiovascular se encuentran el sobrepeso y la obesidad, también identificados por la American Heart Association como uno de los mayores e independientes factores de riesgo cardiovascular. Desde entonces, varios estudios han relacionado el aumento de peso con modificación de la morfofunción del corazón. Objetivo: Describir las modificaciones del patrón geométrico del ventrículo izquierdo y de la función cardiaca asociadas a malnutrición por exceso en sujetos de la provincia La Habana. Métodos: Se estudiaron 72 sujetos de ambos sexos, aparentemente sanos, que acudieron al Hospital Calixto García a chequearse. A todos se les realizó anamnesis, examen físico, se midió el porcentaje de grasa corporal utilizando un equipo de bioimpedancia; esto permitió clasificar a los sujetos en tres grupos: grupos N (normal), A (alto) y MA (muy alto) y un ecocardiograma. Se procedió a comparar los grupos entre sí mediante análisis estadístico. Resultados: En 23 sujetos con porcentaje de grasa corporal superior al normal, predominó el remodelado concéntrico del ventrículo izquierdo. A mayor composición de grasa corporal aumentó la frecuencia de relajación prolongada, comprometiendo la función diastólica y decreció ligeramente la fracción de eyección del ventrículo izquierdo. Conclusiones: En asociación con el aumento en el porcentaje de grasa corporal hubo cambios morfológicos del corazón, destacando la remodelación concéntrica y el deterioro de la función diastólica(AU)


Introduction: For years, Cuba, in its Statistical Health Yearbook, has presented heart disease as the leading cause of death. Associated with cardiovascular morbidity and mortality are overweight and obesity, also identified by the American Heart Association as one of the largest and independent cardiovascular risk factors. Since then, several studies have linked weight gain with modification of the morphofunction of the heart. Objective: To describe the modifications of the geometric pattern of the left ventricle and of the cardiac function associated with malnutrition due to excess in subjects from the province of Havana. Methods: Seventy-two apparently healthy subjects of both sexes who came to the Calixto García Hospital for a check-up were studied. All underwent anamnesis, physical examination, the percentage of body fat was measured using a bioimpedance equipment; This allowed the subjects to be classified into three groups: N (normal), A (high) and MA (very high) groups and an echocardiogram. The groups were compared with each other through statistical analysis. Results: In 23 subjects with a percentage of body fat higher than normal, concentric remodeling of the left ventricle predominated. A higher body fat composition increased the frequency of prolonged relaxation, compromising diastolic function, and the left ventricular ejection fraction decreased slightly. Conclusions: In association with the increase in the percentage of body fat, there were morphological changes of the heart, highlighting the concentric remodeling and the deterioration of the diastolic function(AU)

RESUMEN Introducción: En los cambios de la geometría ventricular en hipertensos influyen el control de la presión arterial durante las 24 horas del día, así como su descenso adecuado en el período nocturno. Objetivo: Describir los patrones geométricos del ventrículo izquierdo en pacientes con y sin hipertensión arterial nocturna. Método: Se realizó monitoreo ambulatorio de la presión arterial (MAPA) y ecocardiograma a 54 pacientes con hipertensión arterial del 2016 al 2017. Se calcularon las variables de MAPA: promedio y cargas de presión diurno, nocturno y 24 horas, y el patrón circadiano. En el ecocardiograma se midieron los diámetros, el tabique interventricular y la pared posterior del ventrículo izquierdo, se calculó su masa e índice de masa, y se determinó su patrón geométrico y de función diastólica. Resultados: La edad promedio fue 55,4±14,1 años y predominaron el sexo femenino (57,4%) y el color de la piel blanco (59,3%). La hipertensión arterial al despertar y nocturna se halló en el 38,9% y 51,9%, respectivamente, y el fenómeno dipper alterado en 68,5% de los pacientes, asociado significativamente al incremento de la presión arterial nocturna (p=0,001). El tabique interventricular fue significativamente mayor en pacientes con hipertensión arterial nocturna en comparación con el grupo que no la presentaba (11,1±2,2 vs. 9,6±1,4 mm; p=0,006). Predominó la geometría alterada (53,7%) a expensas de mayor remodelado concéntrico (40,7%), sin asociación con la hipertensión arterial nocturna. Conclusiones: Los patrones geométricos del ventrículo izquierdo se comportaron de forma similar en pacientes con y sin hipertensión arterial nocturna.


ABSTRACT Introduction: Changes in ventricular geometry in hypertensive patients are influenced by 24-hour control of blood pressure, as well as its adequate decrease during nighttime. Objective: To describe the left ventricle geometric patterns in patients with or without nocturnal hypertension. Method: An ambulatory blood pressure monitoring (ABPM) and an echocardiogram were performed on 54 patients with high blood pressure, from 2016 to 2017. The following ABPM variables were calculated: average and pressure loads of daytime, nighttime, and 24-hour, as well as circadian pattern. In the echocardiogram, the diameters, the interventricular septum and the left ventricular posterior wall were measured; its mass and mass index were calculated, as well as determined its geometric and diastolic function patterns. Results: The average age was 57.4%±14.1 years old. Females (57.4%) and white skin color (59.3%) predominated. Waking and nocturnal hypertension were found in 38.9% and 51.9% respectively, and the altered dipper phenomenon was found in 68.5% of the patients, significantly associated with increased nocturnal blood pressure (p=0.001). The interventricular septum was considerably higher in patients with nocturnal hypertension when compared to the group without it (11.1±2.2 vs. 9.6±1.4 mm; p=0.006). Altered geometry predominated (53.7%) at the expense of higher concentric remodeling (40.7%), without any association with nocturnal hypertension. Conclusions: Left ventricle geometric patterns behaved similarly in patients with and without nocturnal hypertension.
