ABSTRACT

Objetivo: Avaliar as atitudes e crenças de pacientes e médicos ginecologistas-obstetras sobre o rastreamento cervical e o exame pélvico no Hospital Universitário de Brasília (HUB). Métodos: Foram realizadas entrevistas com pacientes que aguardavam por uma consulta previamente agendada no ambulatório de ginecologia e com médicos ginecologistas-obstetras que atuavam no HUB. Cada grupo respondeu a um questionário que enfocava a realização do rastreamento cervical e do exame pélvico (EP). Resultados: No total, 387 pacientes responderam ao questionário. Dessas, apenas 4,13% sabiam que, de acordo com as diretrizes brasileiras, o rastreamento cervical deveria ser iniciado aos 25 anos de idade, 5,17% sabiam que ele deveria ser encerrado aos 64 anos e 97,93% esperavam um intervalo menor do que o trienal recomendado. Após serem informadas sobre as diretrizes, 66,93% acreditavam que o início aos 25 anos é tardio, 61,5%, que o encerramento aos 64 anos é precoce, 88,37%, que o intervalo trienal é muito longo e 94,06% ficaram com receio de que problemas de saúde pudessem aparecer nesse intervalo. Dos 44 médicos que responderam ao questionário, embora a maioria concordasse com as diretrizes, somente 31,82%, 38,64% e 34,1% as seguia com relação à frequência, à idade de início e à idade de encerramento, respectivamente. Quanto ao EP, aproximadamente metade dos participantes de cada grupo considerava que o exame deveria ser realizado nas consultas regulares com o ginecologista. Conclusão: Foi observada uma discrepância entre as expectativas das pacientes e as diretrizes para o rastreamento de câncer cervical. A maior parte das pacientes não as conhecia e, quando informadas, não concordava com elas. Quanto aos médicos ginecologistas- obstetras, a maioria não as seguia, apesar de conhecê-las. Quanto ao EP, grande parte dos médicos e pacientes considerava-o importante e acreditava que ele deveria ser realizado de forma rotineira nas consultas ginecológicas.


Objective: Evaluate the attitudes and beliefs of patients and obstetrician-gynecologists about cervical screening and pelvic examination in the University Hospital of Brasília (HUB). Methods: Face-to-face interviews with patients waiting for a previously scheduled consultation at the gynecology outpatient clinics and attending obstetrician-gynecologists at the HUB. Each group answered a questionnaire addressing cervical screening and pelvic examination (PE). Results: 387 patients answered the questionnaire. Of these, only 4.13% were aware that, according to Brazilian guidelines, cervical screening should begin at age 25, 5.17% that it should stop at age 64 and 97.93% expected a shorter interval than the recommended triennial. After being informed of the guidelines, 66.93% believed that starting at age 25 is late, 61.5% that stopping at 64 is early, 88.37% that the triennial interval is too long, and 94.06% would be afraid that health problems could appear during the interval. Of the 44 participating physicians, although most agreed with the guidelines, only 31.82%, 38.64% and 34.1% followed them regarding frequency, starting and stopping age, respectively. As for EP, approximately half of the participants in each group believed that it should be performed in regular consultations with the gynecologist. Conclusion: There was a discrepancy between patients' expectations and cervical screening guidelines. Most patients didn't know and, when informed, didn't agree with them. As for Ob-Gyn physicians, most did not follow these guidelines, despite knowing them. As for pelvic exam, most physicians and patients considered it important and believed it should be routinely performed during gynecological consultations.

ABSTRACT

Introduction: Bacterial vaginosis and vaginal trichomoniasis are frequent causes of health care demand. Objective: To estimate the prevalence, identify associated factors, and investigate the performance of diagnostic tests for bacterial vaginosis and trichomoniasis. Methods: Cross-sectional study with participants over 18 years old. All of them were submitted to an interview and gynecological examination with evaluation of vaginal secretion, pH verification, collection of material for Pap smear, wet mount test, Whiff test, bacterioscopy, and polymerase chain reaction for trichomoniasis detection. Logistic regression analysis was applied to identify associated factors with bacterial vaginosis. Diagnostic performance for bacterial vaginosis was evaluated following Amsel criteria, the Ison and Hay score, and the Pap smear, considering the Nugent score as the gold standard. As for trichomoniasis, diagnostic performance was evaluated through the Pap smear and the wet mount test, using the polymerase chain reaction as the gold standard. Results: The prevalence of bacterial vaginosis was 33.7%, and for trichomoniasis, 0.5%. The complaint of abnormal vaginal secretion was associated with the diagnosis of bacterial vaginosis (odds ratio 2.2). The diagnostic accuracy by Amsel criteria, the Ison and Hay score, and the Pap smear was 35.6, 97.0, and 84.2%, respectively. The sensitivity for trichomoniasis through wet mount test was 0.0%, and through the Pap smear, 100%. Conclusion: The prevalence of bacterial vaginosis was high, and trichomoniasis was low. The only associated factor with bacterial vaginosis was the report of abnormal vaginal secretion. The methods with the most accurate diagnostic performance for bacterial vaginosis were the Ison and Hay score and the Pap smear and, for trichomoniasis, the Pap smear

Resumen ANTECEDENTES: El teratoma es un tumor casi siempre benigno, con una amplia variabilidad en su localización, extremadamente rara, en el ligamento redondo. CASO CLÍNICO: Paciente de 30 años, con antecedente de un embarazo a término. En la revisión ecográfica se reportó una lesión quística paraovárica izquierda de 46 x 35 mm, sin componente sólido, graso ni calcificaciones. En el examen ginecológico se palpó una masa por encima del útero, orientada hacia el lado izquierdo, no dolorosa. El resto del examen físico trascurrió sin otras alteraciones. La impresión diagnóstica inicial fue de teratoma abdominopélvico sustentada en la ecografía transvaginal en la que los ovarios se observaron normales, con una masa de ecogenicidad mixta de 50 x 40 x 46 mm, con áreas ecolúcidas y ecogénicas de intensidad variable hacia la fosa iliaca izquierda superior, sin contacto con el ovario. El ligamento redondo se seccionó a ambos lados de la masa y se extrajo sin complicaciones; el reporte histopatológico fue de: teratoma quístico maduro. CONCLUSIONES: El ligamento redondo es una localización infrecuente de los quistes dermoides que debe considerarse en los diagnósticos diferenciales de las masas pélvicas; es susceptible de tratamiento quirúrgico por vía laparoscópica.


Abstract BACKGROUND: Teratoma is an almost always benign tumor, with a wide variability in its location; its location in the round ligament is extremely rare. CLINICAL CASE: 30-year-old patient, with a history of a full-term pregnancy. On routine ultrasound examination, a 46 x 35 mm left paraovarian cystic lesion was reported, with no solid, fatty component or calcifications. In the gynecological examination a mass was palpated above the uterus, oriented to the left side, non-painful. The rest of the physical examination passed without other alterations. The initial diagnostic impression was of abdomino-pelvic teratoma supported by transvaginal ultrasound in which the ovaries were normal, with a mass of mixed echogenicity of 50 x 40 x 46 mm, with echolucent and echogenic areas of variable intensity towards the left superior iliac fossa, without contact with the ovary. The round ligament was sectioned on both sides of the mass and removed without complications; the histopathologic report was: mature cystic teratoma. CONCLUSIONS: The round ligament is an infrequent location of dermoid cysts that should be considered in the differential diagnoses of pelvic masses; it is amenable to surgical treatment laparoscopically.

Abstract Introduction: The participation of students in clinical consultations is essential for their medical education. This experience allows for the acquisition of technical skills and the transmission of ethical and professional values. Objective: To evaluate how differences in outpatient conditions (gynecological or prenatal care), appraisal of previous experience, and sociodemographic profiles influence women's willingness to accept student participation in their consultations. Methods: We selected 743 cases (45.1% prenatal) with previous experience in student participation of 893 women attending outpatient gynecological (52.6%) or prenatal (47.4%) clinics at Brasilia University Hospital from 2016 to 2019. Scales were adopted for women's appraisal of student interpersonal communication, willingness to accept, and unwillingness to accept student participation. We used t tests to assess differences, chi-square statistics to compare proportions between outpatient groups, correlations between key variables, and linear regression to estimate variables predicting the willingness outcome. Results: Odds ratios over 1 (p< 0.01) emerged for age older than 35 years, not married, less than higher education, multiparity, discomfort with students, and lower acceptance of gender equality in relation to the Ob-Gyn gender for the gynecological group. Women in the gynecological group offered a better appraisal (from one to five) of interpersonal communication (4.75 vs. 4.43, effect size g= 0.605), showed more willingness (4.58 vs. 4.26, g= 0.625), and conveyed less unwillingness to accept student participation (2.35 vs. 2.47, g= 0.143) than women in the prenatal group. In the linear regression analysis (N= 743), a higher willingness to accept student participation was significantly related (in decreasing impact) to better appraisal of student interpersonal communication (p< 0.001), lower unwillingness (p< 0.001), gynecological group (p< 0.001), tolerance to pelvic examination by a student (p= 0.017), and age older than 35 years (p= 0.016). Conclusions: The experience of supportive interpersonal communication, especially regarding the gynecological group, had a predominant impact on the patient's willingness to accept the participation of students in consultations. Overall, the willingness to accept this participation differs depending on the patient's (reason for consultation, lower unwillingness, age) and student's (communication, gender) factors. Hopefully, the findings can contribute to fostering student-patient partnerships from the perspective of the articulation between service and teaching in medicine.


Resumo Introdução: A participação de estudantes nas consultas clínicas é essencial para a educação médica deles. Tal experiência permite a aquisição de habilidades técnicas e a transmissão de valores ético-profissionais. Objetivo: Este estudo teve como objetivo avaliar como diferenças na condição ambulatorial (atendimento ginecológico ou pré-natal), na apreciação de experiências anteriores e nos perfis sociodemográficos influenciariam a disposição das mulheres em aceitar a participação de estudantes em suas consultas. Método: De um total de 893 mulheres atendidas no ambulatório de ginecologia (52,6%) e de pré-natal (47,4%) do Hospital Universitário de Brasília de 2016 a 2019, foram selecionadas 743 (45,1% pré-natal) que referiam experiência prévia com a participação de estudantes em suas consultas. Foram adotadas escalas para avaliar a apreciação das mulheres sobre a comunicação interpessoal dos estudantes e a disposição e a falta dela em aceitar a participação deles. Foram utilizados o teste t para avaliar as diferenças, as estatísticas qui-quadrado para comparar as proporções entre os grupos, as correlações entre variáveis-chave e a regressão linear para estimar as variáveis associadas à boa vontade das pacientes quanto aos estudantes. Resultado: Razões de chance acima de 1 (p < 0,01) para mulheres do grupo ginecológico emergiram quanto à idade acima de 35 anos, não casadas, escolaridade aquém da educação terciária, multíparas, desconfortáveis com estudantes e menor aceitação de equidade de gênero dos médicos ginecologistas obstetras. Mulheres no grupo ginecológico apresentaram melhor apreciação da comunicação interpessoal, de um a cinco (4,75 versus 4,43, tamanho do efeito g = 0,605), maior disposição (4,58 versus 4,26, g = 0,625) e menor indisposição (2,35 versus 2,47, g = 0,143) em aceitar a participação de estudantes do que mulheres do grupo pré-natal. Na análise de regressão linear (n = 743), maior disposição em aceitar estudantes foi significativamente associada (em impacto decrescente), com melhor apreciação da comunicação interpessoal destes (p < 0,001), menor indisposição (p < 0,001), grupo ginecológico (p < 0,001), tolerância ao exame pélvico realizado por estudante (p = 0,017) e idade maior que 35 anos (p = 0,016). Conclusão: A experiência de comunicação interpessoal de suporte, principalmente no que concerne ao grupo ginecológico, teve impacto predominante na disposição das pacientes em aceitar a participação de estudantes nas consultas. No geral, a vontade de aceitar essa participação difere dependendo dos fatores da paciente (motivo da consulta, menor indisposição, idade) e do estudante (comunicação, gênero). Espera-se que os achados possam contribuir para fomentar parcerias estudante-paciente na perspectiva da articulação entre serviço e ensino em medicina.

ABSTRACT

RESUMO Objetivo compreender a ótica dos enfermeiros sobre as barreiras enfrentadas pelas mulheres com deficiência física no que concerne à realização do exame citopatológico. Métodos estudo qualitativo, realizado com 11 enfermeiras das Unidades Básicas de Saúde, incluindo zona urbana e rural. O processamento e a análise dos dados utilizaram o software IRAMUTEQ - acrônimo de Interface de R pour les Analyses Multidimensionnelles de Textes et de Questionnaires. Resultados as classes 1 e 5 foram aglutinadas na classe "Inadequação da infraestrutura"; a classe 2 foi "Resultados dos exames coletados"; a 3, "Sentimentos na realização do exame"; e a 4, "Capacitação das equipes". Conclusão desvelaram-se sentimentos negativos vivenciados pelas mulheres na realização do exame, falta de capacitação dos profissionais, ausência de estratégias de manejo na unidade, e estrutura física inapta para as necessidades das usuárias. Contribuições para a prática comprovou-se a necessidade de atenção às mulheres com deficiência física e de realização do exame de rotina. Ademais, mostrou-se necessário fomentar estratégias para a adequação e melhoria dos conhecimentos dos profissionais que realizam os atendimentos.


ABSTRACT Objective to understand the perspective of nurses regarding the obstacles faced by physically disabled women in regard to cytopathological examinations. Methods qualitative study with 11 nurses from Basic Health Units, including urban and rural areas. Data was processed and analyzed using the software IRAMUTEQ (R pour les Analyses Multidimensionnelles de Textes et de Questionnaires). Results classes 1 and 5 were grouped into the class "Inadequate infrastructure"; class 2 was "Results of the collected exams"; class 3, "Feelings at the time of examination"; and 4 "Team training". Conclusion we revealed that women presented negative feelings during the examination, professionals lacked further training, there were no strategies to deal with the situation in the unit, and facilities were inadequate to attend the needs of the users. Contributions to practice: we demonstrated that it is essential to provide care to physically disabled women and carry out routine exams in this population. Furthermore, this study can give support to strategies to adapt and improve the knowledge of the workers who provide care.

ABSTRACT

Objective:To analyze the expectations and their influencing factors of gynecological examination among female physical examination population.Methods:Convenience sampling method was used to select women undergoing gynecological examinations in the health management center of the Shandong Provincial Hospital Affiliated to Shandong First Medical University between April to June 2021. A face to face interview was conducted with a 20-item structured questionnaire including general information, the women?s feelings about a gynecological examination, expectations of doctors, preferences for the doctor ?s gender, and the presence of chaperones in male doctor examination scenarios. Of the 1 600 questionnaires distributed, 1 583 were valid, with a recovery rate of 98.93%. To compare the preferences for a doctor ?s gender and chaperones with different demographic characteristics, an χ2 test was used. Logistic regression was used to analyze the influencing factors of preferences for doctors ?gender and for chaperones. Results:For a gynecological examination, women were mainly worried about the possibility of bad results (77.4%), while the main expectation to doctors was communication before the examination (49.2%); most women wanted to be examined by a female doctor (85.5%), mainly because they felt relaxed (76.0%); most women did not want chaperones when examined by a female doctor (56.5%), while most of them preferred to be accompanied by a nurse when examined by a male doctor (50.1%). Women whose family income was lower than expenditure and had gynecological examination experiences with male doctors tended to have no preference for a doctor ?s gender. Additionally, there was a strong inverse association between the education level and likelihood of no preference for a doctor ?s gender, that is, the lower the education level, the more likely they were to have no preference for the doctor ?s gender ( P<0.05), when examined by a male doctor. Furthermore, women older than 55 years and who had a gynecological examination more than twice a year, was less likely to need chaperones, while women who had gynecological examination experience with male doctors tended to have no preference for chaperones. Conclusions:Women preferred gynecological examinations to be performed by female doctors who provide gentle and companion staff support during the examinations. Education level, family income, and examination experience of male doctors are the influencing factors for gender preferences of a doctor. Age, examination frequency, and examination experience of male doctors are the influencing factors affecting the preferences of choosing an accompanying person.

Objetivo: Analisar desigualdades regionais e sociais na realização de mamografia e exame citopatológico. Métodos: Estudo transversal, com dados da Vigilância de Fatores de Risco e Proteção para Doenças Crônicas por Inquérito Telefônico (Vigitel), de 2019. As variáveis de desfecho foram realização de mamografia e exame citopatológico; e as variáveis de exposição, raça/cor da pele, escolaridade e macrorregião nacional de residência. Medidas de desigualdades absolutas foram apresentadas por meio do slope index of inequality (SII) e equiplots. Resultados: Foram incluídas 23.339 mulheres. A realização de mamografia foi 5,2 pontos percentuais maior naquelas com maior escolaridade, e de exame citopatológico, 5,3 pontos percentuais menor nas mulheres de raça/cor da pele preta. A realização de mamografia e exame citopatológico foram 3,9 e 11,2 pontos percentuais maiores na região Sul, respectivamente. Conclusão: Desigualdades sociais e regionais persistem no país e afetam, principalmente, mulheres de raça/cor da pele preta, de baixa escolaridade e residentes no Nordeste brasileiro.

Métodos: Estudio transversal con datos de la Vigilancia de Factores de Riesgo y Protección de Enfermedades crónicas por encuesta telefónica de 2019. Las variables de resultado fueron mamografía y examen citopatológico. Las variables de exposición fueron raza/color de piel, nivel de educación y macrorregión nacional de residencia. Las desigualdades absolutas se presentaron utilizando el slope index of inequality y el equiplots. Resultados: Se estudiaron 23.339 mujeres. La realización de mamografía fue 5,2 puntos porcentuales más alta en las mujeres con educación superior y la citopatología fue 5,3 puntos porcentuales más baja en mujeres de raza /piel negra. La mamografía y la citopatología fueron 3,9 y 11,2 puntos porcentuales más altos, respectivamente, en la Región Sur. Conclusión: Las desigualdades sociales y regionales persisten en Brasil y afectan principalmente a mujeres de piel negra, con baja escolaridad y residentes en la región Nordeste del país.


Objective: To assess regional and social inequalities in mammography and Papanicolaou tests. Methods: This was a cross-sectional study with data from the 2019 Chronic Disease Risk and Protective Factors Surveillance Telephone Survey (Vigitel). The outcome variables were mammography and cytopathology test. The exposure variables were race/skin color, schooling and region of residence in Brazil. Absolute inequality measurements were presented using the slope index of inequality (SII) and equiplots. Results: 23,339 women were included in this study. Having a mammography was 5.2 percentage points higher in women with higher levels of education, while having a cytopathology test was 5.3 percentage points lower in women of Black race/skin color. Having mammography and cytopathology tests was 3.9 and 11.2 percentage points higher, respectively, in the Southern region. Conclusion: Social and regional inequalities persist in Brazil and affect mainly women of Black race/skin color, with low education levels and living in the Northeast region of the country.

Resumen: ANTECEDENTES: El melanoma vaginal es una alteración excepcional, por lo que el diagnóstico se establece en etapas avanzadas de la enfermedad. El pronóstico a corto plazo es malo y no existen factores de riesgo identificados hasta la fecha. CASO CLINICO: Paciente de 77 años, acudió a consulta por sangrado transvaginal fétido, de dos meses de evolución. Durante la colposcopia se observó una tumoración en la cara lateral izquierda de la vagina, que se extendía hasta su tercio inferior; la cara anterior estaba hiperpigmentada y friable. De acuerdo con el reporte de citología y la biopsia se estableció el diagnóstico de melanoma invasor. La concentración de marcadores tumorales fue positiva para HMB-45, Ki-67 (20%), MART-1 (Melan-1) y PS-100. La paciente fue enviada al servicio de Oncología para estadificación y tratamiento de la enfermedad. CONCLUSIONES: La identificación de una tumoración hiperpigmentada en la exploración ginecológica, además de la biopsia dirigida complementada con estudio de inmunohistoquímica, es sugerente de melanoma vaginal. Las pacientes con este tipo de lesión deben atenderse por un equipo médico multidisciplinario.


Abstract: BACKGROUND: Vaginal melanoma is an exceptional alteration, for which the diagnosis is established in advanced stages of the disease. The short-term prognosis is poor and there are no identified risk factors to date. CLINICAL CASE: 77-year-old patient, who came to the clinic for fetid transvaginal bleeding, two months old. During colposcopy, a tumor was observed on the left lateral aspect of the vagina, which extended to its lower third; the anterior face was hyperpigmented and friable. Based on the cytology report and biopsy, the diagnosis of invasive melanoma was established. The concentration of tumor markers was positive for HMB-45, Ki 67 (20%), MART-1 (Melan-1) and PS-100. The patient was sent to the Oncology service for staging and treatment of the disease. CONCLUSIONS: The identification of a hyperpigmented tumor on gynecological examination, in addition to a directed biopsy, complemented by an immunohistochemical study, is suggestive of vaginal melanoma. Patients with this type of injury should be cared for by a multidisciplinary medical team.

ABSTRACT

Objective: To study the impacts of the new medical reform on the utilization equity of gynecological examination services for women in economically underdeveloped regions of western China. Methods: Based on the data of the 4th and 5th National Health Service Survey in Shaanxi Province, we used concentration index and decomposition of concentration index to explore the inequality of women's gynecological examination service utilization and its sources; we also used horizontal inequity index to analyze the horizontal equity of the women's gynecological examination service utilization. Results: The concentration index of gynecological examination rate of the women surveyed in 2008 and 2013 was 0.0882 and 0.0640, respectively. The horizontal inequity index was 0.0894 and 0.0463, respectively. The factors that contributed most to the inequality of the women's gynecological examination service utilization were the economic level and contribution which accounted for 54.49% and 61.86%, respectively. Conclusion: The implementation of basic public health and major public health services in the new medical reform policy has increased women's gynecological examinations rate and plays a positive role in improving the equity in utilization of women's gynecological examination services.

Objetivo Analisar o conhecimento e a adesão ao Papanicolau de mulheres que frequentam Unidades Básicas de Saúde. Métodos Um questionário composto de questões pré-codifi cadas e abertas foi aplicado às usuárias de duas Unidades Básicas de Saúde da cidade de São José do Rio Preto, São Paulo, localizadas em bairros com perfi l socioeconômico distinto. Os dados foram analisados estatisticamente pelo programa Statistical Package for the Social Sciences.Resultados Após a análise de 99 questionários respondidos, ficou evidente um nível de conhecimento melhor sobre o exame de Papanicolau das residentes do bairro com melhores condições socioeconômicas e das mulheres mais jovens. Ficou evidente que muitas fazem esse exame sem saber o devido objetivo desse procedimento. A vergonha e a falta de tempo foram relatadas como motivos relevantes para não realizar o exame. Conclusão A falta de informação sobre o exame de Papanicolau das mulheres com mais de 60 anos e menor nível socioeconômico ficou evidente neste estudo e pode ser considerada um dos aspectos mais relevantes à não adesão à prevenção do câncer do colo de útero. Dessa maneira, ações de saúde alternativas deveriam ser adotadas pelas Unidades Básicas de Saúde para melhor orientar a população e, assim, aumentar a adesão a esse exame.


Objective To analyze the knowledge and adhesion to the Papanicolaou smear test of women attending Basic Health Units. Methods A questionnaire composed of pre-coded and open questions was applied to users of two Basic Health Units of the city of São José do Rio Preto, São Paulo, located in districts with different socioeconomic profiles. The data were statistically analyzed by the Statistical Package for the Social Sciences program. Results After the analysis of 99 questionnaires answered, a better level of knowledge about the Pap smear was found among the residents of the better socioeconomic neighborhood and the younger women. It has become evident that many take this test without knowing the proper purpose of this procedure. Shame and lack of time were reported as relevant reasons for not doing the test. Conclusion The lack of information on the Papanicolaou smear of women over 60 years of age and lower socioeconomic level was evident in this study and may be considered one of the most relevant aspects to non-adherence to cervical cancer prevention. In this way, alternative health actions should be adopted by the Basic Health Units to better guide the population and thus increase adherence to this examination.

ABSTRACT

Introduction: Vaginal discharge is a frequent gynecological complaint, and may represent a disease or not. A vaginal discharge is considered recurrent when it occurs four or more episodes per year. Among the aetiologies, physiological and infectious conditions are mentioned, being the infectious ones, particularly those caused by Candida spp. fungus, the most related to the symptom. Despite the diagnostic and therapeutic resources available, empirical clinical treatments and self-treatments are very frequent and related to ineffective therapeutic results, leading this population to question what the differences regarding women with no symptoms are. Objective: To identify sociodemographic, behavioral and microbiological differences between women with recurrent vaginal discharge and asymptomatic women. Methods: Cross-sectional study involving 126 women with recurrent discharge complaints (study group) and 155 (control group), totaling 281 evaluated women. The group included women in the menacme, sexually active, and those who fit in the criteria of recurrent vaginal discharge, without definite previous diagnosis, compared with asymptomatic women, who attended an annual routine examination. Pregnant, diabetic and immunosuppressed women were excluded. The study was based on the principle of the null hypothesis, when there are no differences between the two studied groups. Results: The average age was 29.95 years, predominantly single and without children. There was no significant difference in the analysis of relationship time with the current partner, numbers of partners throughout life, gender and contraceptive method. There was predominance of normal vaginal flora (type 1) in both groups, with average prevalence of 44.9%. The alkaline vaginal pH was predominant in the study group. Conclusion: The null hypothesis was confirmed. Biological, behavioral and sociodemographic differences in the studied populations were not identified. In women with recurrent discharge group, there were no infectious etiologic factors, suggesting that clinical diagnoses are not sufficient for the most efficient management of these situations, indicating laboratory evaluation for these cases in order to improve diagnostic accuracy


Introdução: O corrimento vaginal é queixa ginecológica frequente, podendo ou não representar doença. Conceitua-se como corrimento vaginal recorrente aquele que ocorre em quatro ou mais episódios ao ano. Entre as etiologias, citam-se condições fisiológicas e infecciosas, sendo as infecciosas, particularmente as causadas por fungo Candida spp., as mais relacionadas ao sintoma. Apesar dos recursos diagnósticos e terapêuticos disponíveis, tratamentos clínicos empíricos e autotratamentos são muito frequentes e associados a resultados terapêuticos pouco efetivos, levando essa população a questionamentos sobre quais diferenças elas teriam em relação a mulheres sem sintomas. Objetivo: Identificar diferenças sociodemográficas, comportamentais e microbiológicas entre mulheres com corrimento vaginal recorrente e mulheres assintomáticas. Métodos: Estudo transversal envolvendo 126 mulheres com queixa de corrimento recorrente (grupo de estudo) mais 155 controles, totalizando 281 mulheres avaliadas. Foram incluídas no grupo de estudo mulheres no menacme, sexualmente ativas e enquadradas nos critérios de corrimento vaginal recorrente, sem diagnóstico prévio definido, comparadas a mulheres assintomáticas, que compareciam a exame de rotina anual. Foram excluídas as gestantes, diabéticas e imunossuprimidas. Partiu-se de princípio da hipótese nula, em que não há diferenças entre os dois grupos estudados. Resultados: A média de idade foi de 29,95 anos, predominando solteiras e sem filhos. Não houve diferença significativa quando analisados: tempo de relacionamento com o atual parceiro, número de parceiros ao longo da vida, sexarca e método anticoncepcional. Houve predomínio da flora vaginal normal (tipo 1) em ambos os grupos, com prevalência média de 44,9%. O pH vaginal alcalino foi predominante no grupo de estudo. Conclusão: Confirmou-se a hipótese nula, não se identificando diferenças biológicas, comportamentais e sociodemográficas nas populações estudadas. Não se observaram, no grupo de mulheres com corrimento recorrente, fatores etiológicos infecciosos, sugerindo que diagnósticos clínicos não são suficientes para o manejo mais eficiente dessas situações, indicando-se avaliação laboratorial para esses casos com o objetivo de melhorar a acurácia diagnóstica.

ABSTRACT

Objective To analyze the ultrasonic features of gynecological emergency and severe cases.Methods To analyze 431 cases in clinical,ultrasonic images and examination data of gynecological emergency and severe patients in Peking University Third Hospital from September 2014 to September 2015,and to study clinical pathological and ultrasonic imaging examination.Results In 431 severe cases of gynecologic emergency,the clinical symptom were shown as acute abdominal pain or and vaginal bleeding.They were divided into seven types by clinical examination,operation or conservative treatment under dynamic observation.There were 137 cases of fracture disease,accounting for 31.8%,with corpus luteum rupture in 67 cases,ectopic pregnancy burst in 59 cases and tumor rupture in 11 cases.There were 114 cases of pelvic inflammatory disease,accounting for 26.5%,with hemorrhagic disease of department of gynaecology in 67 cases (15.5%),dysfunctional uterine bleeding in 39 cases,cervical cancer in 11 cases,submucosal myoma in 7 cases,endometrial carcinoma in 6 cases,carcinosarcoma in 4 cases.There were 58 cases of early pregnancy related diseases,accounting for 13.5%.Among them,32 cases were incomplete abortion,and 21 cases were inevitable abortion and 5 cases were hydatidiform mole.Forty-six cases were torsion of pedicle (10.6%).Five cases were genital tract malformation,accounting for 1.2%,with vaginal septum obliquumevery 4 cases and cervical atresia in one case.There were damages after the operation in 4 cases (0.9%),uterus perforation in 2 cases,abdominal wall hematoma in 1 case after cesarean section,and false aneurysm in 1 case after cesarean section.In the 431 cases,there was emergency surgery oroperation after symptomatic treatment in 329 cases,interventional treatment in one case and non-operative treatment in 101 cases.Conclusions There are corresponding typical ultrasonographic characteristics in different diseases of emergency and severe gynaecologic cases,combined with clinical symptoms and medical history.The right diagnosis can be made.Therefore,there are important clinical values of ultrasound in the treatment of emergency and severe gynecologic cases.

ABSTRACT

This study aimed to determine the effect of the SNAPPS (summarize, narrow, analyze, probe, plan, and select) method versus teacher-centered education on the clinical skills of midwifery students in Iran. In this clinical trial, 36 midwifery students in their 4th year of education in 2015 were enrolled and divided into 6 groups, 3 groups for teacher-centered education and 3 groups for the SNAPPS method, with each group spending 10 days in the outpatient gynecology clinic. A questionnaire and a checklist were used to gather data. An independent t-test and chi-square test were used to analyze the data. Ability to gain the trust of the patient, verbal and nonverbal communication skills, history taking, preparation of the patient for gynecological examination, and diagnosis and treatment of common diseases were significantly better in the SNAPPS group compared to the teacher-centered education group (P<0.05). The SNAPPS education method can significantly improve the clinical skills of midwifery students in gynecology, in particular history taking, differential diagnosis, and treatment of common diseases.

ABSTRACT

Aim: Analyze the acceptability of digital cervicography as a complementary method to Papanicolaou cytology for diagnosing cervical cancer. Method: An exploratory-descriptive study, quantitative approach. The survey had 63 participants and data was collected between June and August 2012. For data evaluation we used the results of cytologic collection and digital cervicography. Result: 100% of acceptance verified within participants. Regarding the Papanicolaou cytology, 96.8% of glandular, 8% of metaplastic, and 100% of squamous epithelium were found. Inflammation was found in 58.6% of the sample and malignant cell alterations in 1.6%. Discussion: The low sensitivity level of Papanicolaou cytology and the good acceptability rates of digital cervicography suggest the complementarity of these for cancer diagnosis. Conclusion: The implementation of digital cervicography as an exam complementary to Papanicolaou cytology is of great importance in order to increase the sensitivity level at cervical cancer detection.


Objetivo: evaluar la aceptación de la Cervicografia Digital como método complementar a la citología de Papanicolaou para diagnóstico del cáncer de cuello uterino. Método: estudio descriptivo exploratorio, enfoque cuantitativo. La investigación conto con 63 participantes, los datos fueron recolectados entre junio y agosto de 2012. Para evaluación de los datos se tuvo el resultado de recolección citológica, y la cervicografia digital. Resultado: Se verifico 100% de aceptabilidad de la cervicografia digital entre las participantes. Cuanto al examen de Papanicolaou, se verifico 100% de epitelio escamoso, 96,8%, glandular e 8%, metaplásico. La inflamación estuvo presente en 58,6% de la muestra y las alteraciones celulares malignas en 1,6%. Discusión: La baja sensibilidad de la citología de Papanicolau y la buena aceptabilidad y eficacia de la cervicografia digital sugiren a complementariedad de estos en el diagnóstico del cáncer. Conclusión: La implementación de la cervicografia digital como examen complementar a citología de Papanicolaou es de grande valia para aumentar la sensibilidad en el rastreo del cáncer de cuello uterino.

ABSTRACT

Objetivo: Relatar o perfil das usuárias participantes de um projeto de extensão universitária. Método: Pesquisa documental, descritiva e quantitativa. Resultados: A ação extensionista contou com a participação de acadêmicos de enfermagem UFF/PURO, e foi realizada em cinco Unidades Básicas de Saúde escolhidas de forma a cobrir a maioria da população do município de Rio das Ostras/RJ. Foram atendidas 462 usuárias, sendo realizados exames citopatológicos, solicitação de mamografias e educação em saúde. Depreende-se que a população atendida encontra-se em algumas situações de vulnerabilidade que as mulheres estão submetidas. Conclusão: Atividade foi de grande importância para a formação profissional dos acadêmicos de enfermagem dando a eles a oportunidade de participar nas ações de saúde, facilitando a sedimentação do conhecimento teórico.


Objective: To report the profile of the users participating of an extension project. Method: This is a quantitative, descriptive and documentary study. Results: The action had as participants the nursing academics of the Federal Fluminense University/UFF/PURO and conducted in five Basic Unit Health chosen to cover most of the population in the city of Rio das Ostras/RJ. Were performed cytopathological exams, mammograms request and health education to 462 users. It appears that the involved population found in some situations of vulnerability in which women subjected. Conclusion: This activity was of great significance for the professional training of nursing students, providing them, opportunities to participate in the health activities, facilitating the sedimentation of the theoretical knowledge.

OBJETIVO: Analisar diferenças socioculturais e percepções sobre a consulta ginecológica por adolescentes. MÉTODOS: Estudo transversal com 418 alunas do ensino médio de três escolas de diferentes perfis, localizadas na cidade do Rio de Janeiro, RJ, em 2010. Aplicou-se questionário estruturado, abordando características sociodemográficas, comportamento sexual e avaliação da consulta ginecológica. Utilizou-se o teste de Qui-quadrado (Yates) e o t de Student, adotando-se p < 0,05. RESULTADOS: Alunas dos colégios privado e público apresentaram perfis semelhantes e diferiram daquelas da rede pública estadual que tiveram nível socioeconômico mais baixo, menor escolaridade dos responsáveis, predominância da raça negra, maior número de parceiros, gestações e histórico de violência sexual. As médias de idades da menarca e sexarca foram semelhantes entre as estudantes e a primeira consulta ginecológica foi significativamente mais tardia nas alunas da rede estadual. A maioria referiu conhecimento sobre anticoncepção e doenças sexualmente transmissíveis, porém pequena parte obteve essas orientações na consulta. As estudantes manifestaram desejo de que o profissional investisse mais tempo, paciência e disponibilidade no atendimento. CONCLUSÕES: O atendimento ginecológico na adolescência é insatisfatório segundo a avaliação das adolescentes estudadas. As usuárias dos serviços privados submetem-se à consulta ginecológica em idade mais precoce do que aquelas que têm acesso apenas à rede pública. É necessário criar mecanismos que facilitem o acesso e a adesão desse grupo etário à rotina preventiva ginecológica.


OBJECTIVE: To analyze sociocultural differences and perceptions of gynecological consultations for high school girls. METHODS: A transversal study with 418 high school girls from three schools of different profiles in the city of Rio de Janeiro, Southeastern Brazil in 2010. A structured questionnaire encompassing socio-demographic characteristics, sexual behavior and evaluation of gynecological consultations was completed. Yates' Chi-square test and the Student's t-test were utilized adopting a value of p < 0.05. RESULTS: The students of private and federal public schools presented similar profiles but both were different from the state school girls. The latter had lower socioeconomic status, and their parents had lower levels of education, the predominance of afro-descendants was observed, as were a larger number of sexual partners, pregnancy and cases of sexual violence. The average age of menarche and sexarche among the students were similar, but the first gynecological consultation was significantly later among the state school students. The majority showed some knowledge of contraception and STDs, although only a minority received guidance from the consultations. Students expressed the desire that the professionals dedicate more time, patience and availability to them during consultations. CONCLUSIONS: The provision of gynecological services for teenagers is not satisfactory, according to the teenagers' evaluations. Users of the private health system have gynecological consultations earlier than those who only have access to the public system. It is necessary to create mechanisms that facilitate access and adhesion to a routine of gynecological prevention for this age group.

RESULTADOS: Alumnas de los colegios privado y público presentaron perfiles semejantes y se diferenciaron de las que estudian en la red pública estatal quienes presentaron nivel socioeconómico más bajo, menor escolaridad de los representantes, predominancia de raza negra, mayor número de parejas, gestaciones e historia de violencia sexual. El promedio de edades de las menarca y sexarca fueron semejantes entre las estudiantes y la primera consulta ginecológica fue significativamente más tardía en las alumnas de la red estatal. La mayoría mencionó tener conocimiento sobre la anticoncepción y enfermedades transmitidas sexualmente, sin embargo, un pequeño número tuvo esas orientaciones en la consulta. Las estudiantes manifestaron interés por disponer de más tiempo, paciencia y atención por parte del profesional. CONCLUSIONES: La atención ginecológica en la adolescencia es insatisfactoria de acuerdo con la evaluación en las adolescentes estudiadas. Las usuarias de los servicios privados acuden a la consulta ginecológica en edad más precoz que las que tienen acceso a la red pública. Es necesario crear mecanismos que faciliten el acceso y la adhesión de ese grupo etario a la rutina preventiva ginecológica.

ABSTRACT

Objective Analyze the influence of applying background music on heart rate,blood pressure and dysphoric state of patients during gynecologial examination.Methods A total of 564 patients were randomly divided into music and control group 282 cases each.In control group,the 282 patients were just actualized the routine psychological care.And the other 282 patients of music group were actualized music therapy based on the routine psychological care.After these patients of music group entered examination room,the doctor broadcasted some good background music for them.After 30 minutes,the doctor recorded their heart rate and blood pressure changes and evaluated the anxiety level by using S-AI.Results After the patients of music group had listened to background music for 30 minutes,their systolic pressure and heart rate reduced evidently and the level of anxiety mitigated obviously.Conclusions Applying background music during gynecological examination can relieve anxiety emotion,stabilize heart rate and blood pressure and improve comfort of gynecological examination.It's an effective,simple and convenient nursing care method.
