ABSTRACT

Background: Hypothyroidism is a common endocrinal cause of growth retardation in children. Following adequate treatment with thyroxine, growth resumes at an accelerated rate which is known as catch-up growth. There are few observational studies from India on the growth parameters following treatment with thyroxine in children with hypothyroidism. Methods: A retrospective study was done in children aged 2-10 years who were newly diagnosed cases of primary hypothyroidism [Total serum Thyroxine (T4) levels <5 'g/dl and serum Thyroid Stimulating Hormone (TSH) levels ?15 'U/ml] and treated with oral thyroxine to attain euthyroid state. Height measured before starting treatment and at the time of follow up visits was noted, the Height Standard Deviation Scores (HSDS) were calculated. The effect of thyroxine on linear growth was studied. Results: There were 23 children who were diagnosed as having primary hypothyroidism of whom 16(69.6%) were females and 7(30.4%) were males. The mean age of the children studied was 7.3'2.3 years. The mean dose of thyroxine required to maintain euthyroid status was 4.6'2.2 'g/kg/day. Mean duration of follow up was 13.7'2.4 months. The initial HSDS was - 2.31'0.9 which improved to a final value of - 1.7'0.76 (?HSDS0.61, p value <0.0001). Mean height velocity was 8.1 cms/year.Conclusions: Following adequate thyroxine replacement therapy catch-up growth occurs and increased growth velocity leads to partial regain of height deficit in the first couple of years of treatment.

ABSTRACT

The aim of the study was to investigate the relationship between biological maturation, parasympathetic heart rate modulation and intermittent endurance test in young soccer players. Participated of the study 23 young soccer players (15.3 ± 1.1 years, 175 ± 6 cm, 64 ± 7 kg) of the sub15 and sub17 categories of the team from the 1st division championship Paulista. The maturational status was determined by the age of the peak height velocity (PHV), used as relative indicator of somatic maturity and are calculated trhough measures of height, trunk-cephalic height, leg length, body weight, and skinfolds. The Heart rate variability (HRV) measures were obtained in rest during 5 minutes and, posteriorly, analyzed by the variable root mean square of the successive difference (rMSSD). For the intermittent endurance test the subjects performed the Yoyo intermittent recovery test level 2 (YIRT 2), before the HRV measures has been taken. The results of the present study shown which there was not relationship between parasympathetic heart rate modulation (rMSSD = 57,4 ± 26,4 ms) and status biological maturation (0,67 ± 0,81 years) of the young soccer players (r= 0,30; p=0,163), then that the intermittent endurance test (YIRT 2= 658 ± 151 m) has a great correlation with PHV (r= 0,71; p< 0,001). The parasympathetic heart rate modulation has a great correlation with intermittent endurance test of the players (r= 0,54; p= 0,007). In this way, the use of the PHV age and HRV as indicator of performance in YIRT2 in young soccer players is suggested....(AU)

ABSTRACT

PURPOSE: To analyze the effects of clinical and laboratory factors, including insulin-like growth factor (IGF) levels, on the height velocity of normal prepubertal children. METHODS: Ninety-five healthy prepubertal children (33 boys, 62 girls) were enrolled. The mean chronological age was 6.3±1.4 years, with a height standard deviation score (SDS) of -0.88±0.70. IGF-1, IGF binding protein-3 (IGFBP-3), SDS for anthropometric measurements, and changes in SDS for anthropometric measurements were analyzed for 1 year, and their associations with 1-year height velocity were investigated. RESULTS: The group of children with a 1-year height velocity of ≥6 cm were chronologically younger than the group with a 1-year height velocity of < 6 cm (5.9±1.3 years vs. 6.7±1.3 years, P=0.004), with a lesser increase of SDS for body mass index (BMI) over 1 year (-0.18±0.68 vs. 0.13±0.53, P=0.014). There were no differences between the 2 groups in IGF-1 SDS and IGFBP-3 SDS. Multiple linear regression showed that baseline chronological age (r=0.243, P=0.026) and height SDS (r=0.236, P=0.030) were positively associated with IGF-1 SDS. Binomial logistic regression showed that an older chronologic age at referral (odds ratio [OR], 0.68; 95% confidence interval [CI], 0.47–0.99) and an increase of BMI SDS over 1 year (OR, 0.41; 95% CI, 0.18–0.89) were associated with a decreased growth possibility of an above-average height velocity (≥6 cm/yr). CONCLUSIONS: Height velocity of normal prepubertal children is affected by an increase of BMI SDS and chronological age. Prepubertal IGF-1 SDS reflects height SDS at the time of measurement but is not associated with subsequent height velocity.

ABSTRACT

Longitudinal standards for height and height velocity are essential to monitor for appropriate linear growth. We aimed to construct standards in Korean children and adolescents through the population-based longitudinal Kangwha study. Our study was a part of a community-based prospective cohort study from 1986 to 1999 with 800 school children. Height and height velocity were recorded annually from age 6 until final height. Results were compared with cross-sectional data from the 2007 Korean National Growth Charts. Final height was 173.5 cm in boys and 160.5 cm in girls. Although final height was similar between longitudinal and cross-sectional standards, the mean height for age was higher in the longitudinal standard by 1-4 cm from age 6 until the completion of puberty. Using the longitudinal standard, age at peak height velocity (PHV) was 12 in boys and 10 in girls; height velocity at PHV was 8.62 cm/yr in boys and 7.07 cm/yr in girls. The mean height velocity was less than 1 cm/yr at age 17 in boys and 15 in girls. Thus, we have presented the first report of longitudinal standards for height and height velocity in Korean children and adolescents by analyzing longitudinal data from the Kangwha cohort.

ABSTRACT

Purpose: We compared longitudinal height velocity between subjects with Osgood Schlatter Disease (OSD) and those with other chronic lower leg diseases (LD) group involving the knee in order to examine the utility of this velocity as an indicator of bone axis development.Method: Subjects included 92 adult men who have played organized sports. Height values at 9 to 15 years of age were individually approximated by fitting 6-degree polynomial functions. Each polynomial function was differentiated to be led the height velocity curve. Timing of growth spurt and values of height velocity in the OSD and LD group were compared to those in a normal (N) group. Results: Peak height velocity (PHV) was accelerated in the OSD group compared to the LD group. When individual velocity curves were aligned by age of peak height velocity (PHA), the average height velocity in the OSD group was higher than that in the N group at 0.8 years after PHA. Furthermore, when the averaged velocity curve of each group was shifted based on the average PHA, the height velocity in the OSD group was significantly greater than that in the N group at ages 11.6–12.5 years. However the height velocity in the LD group was greater at ages 10.6–10.8 years and 13.3–14.0 years, smaller at ages 11.8–12.3 years than that in the N group. Conclusion: Subjects with OSD may possess higher height velocity than normal individuals before PHA. Conversely, subjects with LD seem to be related to other complicated factors.

ABSTRACT

OBJECTIVE: This study investigated the onset, peak height velocity (PHV) and end of adolescent growth spurt as well as menarcheal timing deduced from surveying accumulative height growth over many years. METHODS: Ninety six students of Samgoe high school between 1st and 3rd grade that were in good health participated in the research. A survey questionnaire was distributed to examine the students' health status and menarche timing. RESULTS: Adolescent growth spurt typically began at the age of 9.9 and reached a PHV at the age of 11.6. The growth spurt ended at the age of 14.1 on average. The average age of menarche was 12.6 years, which was about one year after the PHV of adolescent growth spurt. In most cases, menarche came after PHV. But in 24% of the students, menarche and PHV was nearly coincident or menarche preceded PHV. The growth curves were classified into four types. A typical adolescent growth spurt showed one PHV on graph that drastically drops after the PHV. However, there were cases with two PHVs. CONCLUSIONS: The results indicate that individual growth patterns show large individual differences, however the categorization into the various growth curves may aid in predicting individual growth patterns.

ABSTRACT

The treatment of systemic diseases with glucocorticoids is often associated with decreased height velocity (HV), and can result in shorter final height. Interactions between adrenal and GH-IGF axis have been described and can occur at hypothalamic-pituitary level or at the regulation of IGF system, including the IGF1R signaling. The clinical state of these patients may be considered as an absolute and/or functional IGF-1 deficiency. Interventions aiming to restore the normal function of GH-IGF axis might reduce the glucocorticoids-induced growth suppression in these children. It has been shown that recombinant human GH (hrGH) induces an increase in HV and a decrease in protein loss in patients with juvenile idiopathic arthritis treated with glucocorticoids. Significant increment in HV was also described after hrGH treatment in children under glucocorticoid therapy due to inflammatory bowel disease or renal transplantation. There is a positive correlation between HV and the dose of hrGH. The results support that the IGF-1 deficiency in these children may be counteract by hrGH therapy. The effect of hrGH is observed only during the treatment period and depends on the replacement strategy, nutritional status and disease control.

ABSTRACT

Treatment with oral testosterone undecanoate (40 mg daily) in 9 boys diagnosed as having constitutional delayed growth and puberty (CDGP) was studied. The mean chronological age was 12.1+ 1.7 years and the mean duration of treatment was 0.7+ 0.3 year. The height velocity was increased from 3.9+ 2.1 cm per year before treatment up to 8.1+ 2.3 cm per year treatment (p=0.003). However, the predicted adult height was not different between pre and post treatment. Eighty-nine percent of children reached the pubertal testicular size after treatment. Therefore, oral testosterone undecanoate was proved to be effective to improve height velocity but the predicted adult height in CDGP boys.

ABSTRACT

The auther analysed 29 patients who were treated with prednisolone, including nephrotic syndrome (13 cases), acute leukemia(10 cases), bronchial asthma (2 cases), juvenile rheumatoid arthritis (2 cases) and ulcerative colitis (1 cases) patients in childhood, after remission induction, pro9longed alternate or interrupted treatment was used. The auther observed patterns of height velocity of these patients from 1.6 to 10.3 years. The results were as follow 1) Of the total 29 patients who were treated with prednisolone, most of them showed normal height velocity within the percentile channels, but 4 cases (13.8%) of them showed delay in height velocity. 2) The 4 cases who delayed in height velocity, including 2 cases of bronchial asthma and 2 cases of juvenile rheumatoid arthritis patients, received prednisolone daily about 0.4~1.5mg/kg from 3 to 6 years alternatively or interruptedly. Height velocity was decreased from bet ween 3 and 25 percentile channel at first to below 3 percentile channel at last contiously. 3) In these 4 cases, height velocity was inversely proportion to dosage of prednisolone.
