ABSTRACT

AIM: To evaluate the binocular visual function in high myopia patients after the implantation of implantable collamer lens(ICL)V4c.METHODS: A total of 35 cases(70 eyes)that received binocular ICL implantation at our hospital from May 2019 to May 2021 were enrolled in this prospective study. Binocular full-range visual acuity, contrast sensitivity, stereopsis, mesopic vision and glare sensitivity, and monocular wavefront and the quality of vision questionnaire were assessed before the surgery and at 1 mo postoperatively.RESULTS: At 1 mo postoperatively, 35 cases(100%)had binocular uncorrected distance visual acuity(UDVA)≤0.00(LogMAR), 16 cases(46%)had binocular UDVA≥preoperative corrected distance visual acuity(CDVA). Binocular UDVA and uncorrected intermediate visual acuity(UIVA,80 cm)were improved compared to preoperative CDVA and distance-corrected intermediate visual acuity(DCIVA,80 cm)(all P<0.05).While there were no differences in the binocular postoperative UIVA(60 cm)and preoperative DCIVA(60 cm),and uncorrected near visual acuity(UNVA,40 cm)and preoperative distance-corrected near visual acuity(DCNVA,40 cm)(all P>0.05). The binocular contrast sensitivity was significantly improved postoperatively(P=0.001), and the postoperative binocular mesopic vision, glare sensitivity(no glare/glare)and binocular stereopsis(5 m/40 cm)had no differences(all P>0.05). The postoperative total higher-order aberration, trefoil aberration, coma and spherical aberration were increased, besides the median of total coma in the right eye with a pupil diameter of 3.0 mm was decreased after surgery. The mean total score of quality of vision questionnaire was significantly increased from 54.87 preoperatively to 80.92 after implantation(P<0.05), with high satisfaction and no obvious visual disturbance in patients.CONCLUSION: Although the monocular high-order aberrations increased in the early stage after ICL V4c binocular implantation in patients with high myopia, the binocular visual function was improved.

ABSTRACT

AIM: To study the early outcomes of anterior segment parameters after implantation of an implantable collamer lens with a central hole(ICL V4c)in patients with high myopia.METHODS:A total of 82 cases(160 eyes)with high myopia, including 42 males(82 eyes)and 40 females(78 eyes), aged 26.0±4.6(21 to 37)years, who underwent ICL V4c implantation at our institution from February 2019 to September 2022 and were followed up for 1 a, were included. The general characteristics of the anterior segment of the eye were measured preoperatively: spherical equivalent, mean horizontal corneal curvature, white-to-white(WTW), and axial length(AL); intraocular pressure(IOP), endothelial cell density(ECD), central anterior chamber depth(CACD), anterior chamber volume(ACV)and anterior chamber angle(ACA)were measured preoperatively and at 1 d, 1 wk, 1, 3 and 6 mo postoperatively. Furthermore, the distance from the centre of the posterior surface of the ICL V4c optical zone to the anterior surface of the lens(vault)was measured at 1 d, 1 wk, 1, 6 mo, and 1 a after surgery.RESULTS: The mean preoperative spherical equivalent of the patients was -7.56±2.55 D, mean horizontal corneal curvature was 42.89±1.47 D, WTW was 11.64±0.37 mm, and AL was 26.64±0.93 mm. The baseline IOP was 15.97±2.13 mmHg, and the differences in IOP at each time point after ICL V4c implantation compared to preoperative were not statistically significant(F=0.875, P=0.504); ECD was 2 989.30±140.78 cells/mm2 at baseline, and ECD at 6 mo after ICL V4c implantation was not statistically significant compared with preoperative ECD(t=1.475, P=0.142); CACD was 3.19±0.21 mm at baseline, and ACV was 210.30±27.7 mm3, and CACD and ACV were significantly lower than preoperative at all postoperative time points(F=111.10, 288.38, all P<0.001). The baseline ACA was 35.44°±11.27°, and the ACA at each time point after ICL V4c implantation was significantly lower than preoperatively(F=21.23, P<0.001). The vault was 665.32±184.03 μm at 1 d postoperatively, and continued to be significantly reduced at 1 wk, 1, 6 mo, and 1 a postoperatively compared with 1 d(F=52.10, P<0.001). However, it remained stable at 6 mo and 1 a postoperatively, and the difference was not statistically significant compared with vault at 1 mo postoperatively(P>0.05).CONCLUSION: ICL V4c has certain safety and efficiency in 1 a postoperative follow-up, and the parameters of the anterior segment of the eye stabilized in the early period.

ABSTRACT

AIM: To investigate the short-term visual quality outcomes after femtosecond laser small incision lenticule extraction(SMILE)and evolution implantable collamer lens(EVO-ICL)implantation for the correction of moderate myopia.METHODS: Prospective control study. A total of 51 cases(51 eyes)with moderate myopia who underwent SMILE or EVO-ICL implantation surgery at Weifang Eye Hospital from April 2021 to February 2022 were selected. They were divided into SMILE group(30 patients, 30 eyes)and EVO-ICL group(21 patients, 21 eyes)according to the surgical methods. The changes of visual acuity [uncorrected distance visual acuity(UDVA), corrected distance visual acuity(CDVA)], diopter [spherical equivalent(SE)] and related parameters of optical quality analysis system(OQAS Ⅱ)were observed before surgery and at 1wk, 1 and 3mo after surgery, and the quality of vision(QoV)questionnaire was completed.RESULTS: At 3mo after surgery, the safety index(postoperative CDVA/preoperative CDVA)of SMILE gruop and EVO-ICL group were 1.20(1.00, 1.20)and 1.20(1.00, 1.38), respectively, the efficacy index(postoperative UDVA/preoperative CDVA)were 1.00(1.00, 1.20)and 1.00(1.00, 1.20), respectively, and the percentage of SE within ±0.50D was 87% and 100%, respectively. In SMILE group, the objective scattering index(OSI)was increased after surgery, while modulation transfer function cutoff frequency(MTF cutoff), contrast visual acuity(VA)100%, and VA20% at 1wk and 1mo after surgery, and Strehl ratio(SR)and VA9% at each time point after surgery were all decreased compared with those before surgery(all P<0.05). The OSI, MTF cutoff, SR and VA of EVO-ICL group showed no difference at each time point after surgery compared with those before surgery(all P>0.05). The most common visual symptoms after SMILE and EVO-ICL implantation were visual haze and halos, respectively.CONCLUSION: Both SMILE and EVO-ICL implantation have good safety, efficacy and predictability in the short term after the correction of moderate myopia. Both groups had visual symptoms after surgery, but the overall satisfaction of patients was high. Furthermore, EVO-ICL implantation has better objective visual quality performance.

ABSTRACT

With the development of posterior chamber phakic intraocular lenses implantation and the constant improvement of the implantable collamer lens(ICL), ICL V4c implantation has become one of the main methods for correcting moderate and high myopia. Vault is an important indicator to evaluate the security of posterior chamber intraocular lens implantation. In recent years, optimizing surgical procedures to obtain the ideal vault in ICL V4c implantation surgery has become a research hotspot. This paper aims to provide help for improving surgical safety by summarizing and analyzing the optimized programs of ICL V4c implantation surgery. The focus will be on preoperative examination, intraoperative surgical design, and postoperative follow-up.

ABSTRACT

Implantable collamer lens(ICL)and toric ICL(TICL)are clinically common types of posterior chamber phakic intraocular lenses, they are mainly used to correct high myopia and low-to-moderate myopia of eyes whose corneal conditions are not suitable for corneal laser surgery. Accurate preoperative measurement, safe intraoperative implantation and appropriate postoperative location of lenses are critical to ensure the outcome of ICL implantable, and the appearance of V4c ICL with a central hole ensures the safety of this surgery to a greater extent. However, the intraocular position of both ICL and TICL is not invariable. Some studies have shown that the vault has a trend of decreasing year by year after ICL surgery, but its reason is unknown. And spontaneous rotation may occur after the TICL surgery, thereby causing visual loss, which is one of the main causes of TICL replacement. And postoperative changes in the position of the central hole also need attention. In addition, the visual quality of the operated eye will change after ICL/TICL implantation, but whether the change of intraocular positions of implanted lenses will affect the postoperative visual quality is also a problem that needs to be paid attention to and explored. Therefore, this paper reviewed the postoperative changes in vault, TICL axis, position of the central hole and their causes, as well as the effects of these changes on postoperative visual quality, offering valuable clinical guidance for accurate preoperative selection of ICL/TICL and surgical design, so as to improve the effectiveness and stability of ICL implantation in correction of myopic refractive error.

ABSTRACT

Purpose: To evaluate the agreement between the biometric measurements used to calculate the size of the implantable collamer lenses (ICL) with different technologies: swept?source optical coherence tomography, spectral domain optical coherence tomography, and Scheimpflug tomography. Methods: This retrospective observational study included subjects undergoing refractive surgery with posterior chamber phakic IOL implantation to correct their myopia. The anterior chamber depth (ACD) and the horizontal white to white (WTW) or the angle to angle (ATA) distance were measured with the following four devices: the IOLMaster 700 biometer (Carl Zeiss Meditec, Jena, Germany), based on swept?source optical coherence tomography; the Cirrus and Visante optical coherence tomographs (Carl Zeiss Meditec) based on low?coherence interferometry; and the Pentacam rotating Scheimpflug camera (Oculus, Wetzlar, Germany). Results: In the horizontal corneal diameter measurements, there were statistically significant differences between Pentacam–IOLMaster 700 pair (P < 0.001) and Pentacam–Visante pair (P < 0.001). WTW from CIRRUS showed the lowest correlation when paired with Pentacam and IOLMaster 700 (R2 = 0.452 and 0.385 for Visante and R2 = 0.494 and 0.426 for Cirrus). Regarding the linear correlation of the ACD measurements, all pairs of devices were statistically significant and all of them showed a very good correlation index. Conclusion: There is a good agreement between the different devices under evaluation for ACD measurements. As for WTW, the values measured with the different devices showed large discrepancies with low correlation levels, especially when comparing the tomographs with the other devices under evaluation

@#High intraocular pressure is the most concerned complication of the implantation surgery of V4c implantable contact lens(ICL). According to the appearance time of intraocular pressure elevation, it can be divided into three periods: the early period(1wk after surgery), the middle period(1-4wk after surgery)and the late period(1-3mo after surgery). During the early period, the intraocular pressure increase is mainly caused by the residue of the intraoperative viscoelastic. In the middle period, the application of the hormone drugs results the elevation of the intraocular pressure. Furthermore, during the late period, persistent increasing of the intraocular pressure may cause the secondary glaucoma. Identifying the mechanism of different periods of intraocular pressure rise correctly and thus formulating a reasonable treatment plan, is of great significance for the prevention of glaucoma and other long-term vision threatening complications. Therefore, this article reviews the appearance time and mechanism of the intraocular pressure rise after surgery.

ABSTRACT

Objective: To isolate, purify, and characterize gossypol from the fruits of Thespesia populnea (L) Sol. ex Correa, test its anti- dermatophytic activity, identify its targets on the dermatophyte, and confirm the binding of gossypol with the fungal target by molecular docking study. Methods: Gossypol from Thespesia populnea was characterized by high performance liquid chromatography, liquid chromatograph- mass spectrometry, Fourier transform infrared spectroscopy, and nuclear magnetic resonance. The anti-dermatophytic activity of gossypol was tested against four different dermatophytes, viz. Trichophyton mentagrophytes, Trichophyton rubrum, Microsporum canis, and Microsporum gypseum. Trichophyton mentagrophytes was selected for further studies. The inhibitory mode of action of gossypol on Trichophyton mentagrophytes was determined by analyzing the modulation of gene expression in various pathways of the dermatophyte. Results: Gossypol inhibited all the dermatophytes. The minimum inhibitory concentrations were 12.5 μg/mL for Trichophyton mentagrophytes and Microsporum canis and 25 μg/mL for Trichophyton rubrum and Microsporum gypseum. The minimum fungicidal concentrations were 50 μg/mL for Trichophyton mentagrophytes, 100 μg/mL for Microsporum canis and Trichophyton rubrum, and 200 μg/mL for Microsporum gypseum. Gossypol inhibited the mRNA expression of metalloprotease (MEP4) and isocitrate lyase (ICL). The binding of gossypol with the enzymes was confirmed by molecular docking studies. The best docking poses were found and the low binding energies were recorded with the two target enzymes. Conclusions: Gossypol is a potential antifungal agent and can be further explored as an anti-dermatophytic drug.

@#AIM:To evaluate the effect of the vault on corneal endothelial cells after implantation of central hole implantable lens(ICL)V4c without viscoelastic microincision.<p>METHODS: Prospective non-randomized controlled study. A total of 70 patients(137 eyes)with myopia who underwent ICL V4c implantation in our hospital from November 2017 to February 2018 were divided into low vault group(100μm<vault≤250μm, 37 eyes), medium vault group(250μm<vault≤750μm, 69 eyes)and high vault group(750μm<vault≤900μm, 31 eyes)according to the size of postoperative vault. After 1a of follow-up, the changes of corneal endothelial cell count and anterior chamber parameters were observed.<p>RESULTS:At 1wk after operation, the anterior chamber depth, anterior chamber volume and anterior chamber angle of the three groups were all decreased compared with those before operation(<i>P</i><0.05). There was no significant difference in corneal endothelial cell count among the three groups at different time points before and after surgery. The loss rate of corneal endothelial cells in the low vault group at 1wk, 3mo and 1a after surgery were about 1.2%, 1.5% and 1.7%, respectively. The loss rate of corneal endothelial cells in the middle vault group were about 0.5%, 0.7% and 1.0%, respectively. The loss rate of corneal endothelial cells in the high vault group were about 1.1%, 1.3% and 1.4%, respectively.<p>CONCLUSION:The vault in early period after ICL V4c implantation without viscoelastic microincision had no significant effect on corneal endothelial cells.

@#AIM: To compare the clinical effects of FS-LASIK and ICL implantation.<p>METHODS: From January 2015 to January 2020, used computer to search the clinical contrast test of FS-LASIK and ICL implantation published in CNKI database, Wanfang database, Chongqing VIP Chinese Sci tech journal full-text database, China Bioligy Medicine disc(CBMdisc), Cochrane Library, PubMed, MEDLINE and other databases,using rev-man5.3 statistical software for Meta-analysis and statistical processing.<p>RESULTS: Totally 11 studies were brought into the analysis, including 673 patients and 1 301 operative eyes. The results of Meta-analysis showed that there was no statistical significance in the safety, objective visual quality, spherical equivalent degree, cylinder, diopter of correction, trefoil aberration and coma aberration of FS-LASIK and ICL implantation after operation, but there was statistical significance in the effectiveness, total higher-order aberration, spherical aberration and contrast sensitivity of FS-LASIK and ICL implantation after operation.<p>CONCLUSION: Compared with ICL implantation, the total high-order aberrations and spherical aberrations of FS-LASIK are larger, and the contrast sensitivity is lower. The ICL implantation has more clinical-effect superioity than FS-LASIK, meanwhile it has reversibility and less complications.

ABSTRACT

@#AIM: To compare the objective optical quality of high myopic patients between SMILE and V4c ICL implantation.<p>METHODS: This was a prospective control study. Thirty-eight patients underwent SMILE and thirty-two patients underwent V4c ICL implantation with high myopia were chosen for this study. Uncorrected visual acuity(UCVA), best corrected visual acuity(BCVA)and refractive situation were evaluated preoperatively, at 1wk, 1mo, 3mo and 6mo postoperatively. The objective scattering index(OSI), strehl rate(SR)and modulation transfer function(MTF)cut off frequency were measured by the double-pass optical quality system at the same time.<p>RESULTS: No significant difference of spherical equivalent(SE), UCVA and BCVA was found between SMILE group and V4c ICL implantation group at all time points. At 1wk and 1mo after operation, the OSI values of two groups were significantly higher than pre-operation(<i>P</i><0.01). OSI values of two groups returned to baseline at 3mo after operation. The MTF cut off frequency of both groups decreased significantly at 1wk after operation(<i>P</i><0.05)and recovered to baseline at 1mo after operation, no significant difference was found between two group at all time points(<i>P</i>>0.05).<p>CONCLUSION: Both SMILE and V4c ICL implantation can obtain good visual acuity and objective optical quality.

ABSTRACT

A 28-year-old male presented to retina clinic with complains of blurring and distortion in right eye for past 1 week. There was history of implantation of phakic intraocular lens (pIOL) bilaterally 4 months back. Ophthalmic examination revealed a full-thickness macular hole in the right eye. Pars plana vitrectomy with inverted internal limiting membrane flap was planned. Post-operatively, patient had a good gain in vision (20/40) with closure of the hole. Macular hole is an unusual complication of pIOL. A detailed pre-operative fundus screening is indispensable. Early presentation and timely intervention can optimize the visual outcome.

ABSTRACT

Purpose: To compare clinical outcomes following implantation of two types of posterior chamber phakic intraocular lenses: Visian™ Implantable Collamer Lens with Centraflow (ICL, V4C Staar Surgical, Nidau, Switzerland) and Implantable Phakic Contact Lens (IPCL, V1, Caregroup Sight Solution, India) for the correction of myopia and myopic astigmatism. Methods: This retrospective case series included eyes which underwent phakic intraocular lens implantation with a minimum follow-up period of 1 year. Visual outcomes including safety, efficacy, refractive predictability, and stability were compared at 1 week and at 1, 6, and 12 months' postoperative visit. Complications and adverse events were analyzed. Results: The study included 119 and 203 eyes in the IPCL and ICL groups, respectively. At 1-year postoperative visit, median corrected distance visual acuity was 0.10 (interquartile range [IQR] 0,0.10) and 0 (IQR 0,0) in the IPCL and ICL cohorts, respectively (P = 0.066). An uncorrected visual acuity of 20/32 or better was achieved in 86.5% and 88.67% of the eyes, respectively (P = 0.574). Ninety and 94% of the eyes achieved a postoperative manifest spherical equivalent within ± 0.5D (P = 0.169, ?2 test). Three eyes (2.52%) in the IPCL group versus one eye (0.49%) in the ICL group developed visually significant cataract requiring surgical intervention (P = 0.113). No vision-threatening complications were noted in either cohort. The mean follow-up period was 94.69 ± 32.45 and 102.67 ± 61.82 weeks, respectively. Conclusion: Both groups demonstrated similar efficacy and safety profile. The IPCL is an effective and economically viable option for the correction of myopia.

@#AIM: To compare the differences of corneal diameter measured with wavelight anterior segment analyzer(Wavelight),IOL-master optical bio-measurement(LS900), anterior segment OCT(AS-OCT)and gauge.<p>METHODS: Totally 89 patients(177 eyes)with myopia who want to accept ICL were examined before operation with more than five kinds of instruments respectively, white to white(WTW)were examined with Wavelight, IOL-master, LS900 and gauge, angle to angle(ATA)with AS-OCT. Then the size of ICL was calculated based on the value of gauge. The vault of ICL was examined with OCT 3mo after operation.<p>RESULTS: WTW was(12.45±0.73, 11.96±0.39, 11.92±0.36, 11.49±0.30)mm measured by Wavelight, IOL-Master, LS900 and gauge respectively, ATA was(11.80±0.44)mm with AS-OCT. The value of gauge was the smallest one among them(all <i>P</i><0.01). Among four instruments, there was no statistical difference between LS900 and AS-OCT(<i>P</i>=0.098), they can instead of each other; there were differences among other instruments(<i>P</i><0.01). The size of ICL based on gauge was(12.77±0.37)mm. The vault of ICL is(537.41±181.22)μm 3mo after implantation obtained by OCT.<p>CONCLUSION: Gauge is ideal tool for measuring corneal diameter, it cannot be instead. Wavelight, IOL Master, LS900 and AS-OCT can be taken as reference.

@#AIM: To observe the efficacy and safety of central hole phakic posterior chamber intraocular lens(ICL V4c)implantation for correction high myopia.<p>METHODS: Fifty-eight high myopia patients(116 eyes)who underwent the ICL V4c implantation in our hospital from January 2016 to June 2017 were observed 1wk, 1mo and 1a after operation respectively. Uncorrected visual acuity(UCVA), best corrected visual acuity(BCVA), equivalent spherical mirrors(SE), intraocular pressure(IOP), corneal endothelial cell count, ICL-vault, adverse reaction and complication were investigated.<p>RESULTS: Postoperative UCVA and BCVA were significantly higher than that in preoperative group(<i>P</i><0.01). Significant differences were found between postoperative groups except between 1mo and 1a(<i>P</i>>0.05). Postoperative SE at all time points were lower than that in preoperative group(<i>P</i><0.01). SE at 1mo and 1a after operation were close to 0 diopter. The safety index in 1a postoperative was 1.33±0.67 and the effective index was 1.32±0.69. No significant differences were found in IOP and endothelial cell density after surgery comparing to that in preoperative group(<i>P</i>>0.05). None of subjects developed cataract, glaucoma and decompensation of corneal endothelium. The degree of postoperative satisfaction was 100%.<p>CONCLUSION: ICL V4c implantation is a safe and effective method for correction of high myopia. The visual acuity stabilized 1mo after operation.

@#AIM: To investigate the stability of ICL V4c for moderate and high myopia and its effect on high order aberrations.<p>METHODS: The differences of uncorrected visual acuity(UCVA), best corrected visual acuity(BCVA), spherical equivalent(SE), cylinder equivalent(CE), intraocular pressure(IOP), corneal endothelial cell count, total high order aberration(RMS), spherical aberration, vertical coma, horizontal coma, coma were compared among high myopia patients who treated with ICL V4c intraocular lens implantation(research group: 17 cases, 34 eyes)and femtosecond small incision matrix lens extraction(SMILE)(control group: 18 cases, 34 eyes)before operation(T0), 1d after operation(T1), 1wk after operation(T2), 1mo after operation(T3), 3mo after operation(T4)respectively.<p>RESULTS: SE and CE in research group were higher than those in control group at T3-T4(<i>P</i><0.05), RMS of higher order aberration and coma at T2-T4 were lower than those in control group(<i>P</i><0.05), spherical aberration and horizontal coma at T4 were lower than those in control group(<i>P</i><0.05).<p>CONCLUSION: ICL V4c implantation is stable in the correction of moderate and high myopia, and has little effect on high order aberration.

@#AIM: To observe the efficacy and safety of central hole phakic posterior chamber intraocular lens(ICL V4c)implantation for correction low to moderate myopia.<p>METHODS: Twenty low to moderate myopia patients(forty eyes)who underwent the ICL V4c implantation in our hospital from January 2016 to June 2017 were observed 1wk, 1mo and 1a after operation respectively. Uncorrected visual acuity(UCVA), best corrected visual acuity(BCVA), equivalent spherical mirrors(SE), intraocular pressure, corneal endothelial cell count, ICL-vault, adverse reaction and complication were investigated.<p>RESULTS: Postoperative UCVA and BCVA were significantly higher than that in preoperative group(<i>P</i><0.01). Significant difference was found in UCVA between postoperative groups except between 1mo and 1a. No significant difference were found in BCVA among postoperative groups(<i>P</i><0.05). Postoperative SE at all time points were lower than that in preoperative group(<i>P</i><0.01). Average SE after 1wk after operation were close to 0 diopter. The safety index in 1a postoperative were 1.05±0.14 and the effective index was 1.07±0.14. No significant difference was found in intraocular pressure and endothelial cell density after surgery comparing to that in preoperative group(<i>P</i>>0.05). None of subjects developed cataract, glaucoma and decompensation of corneal endothelium. The degree of postoperative satisfaction was 100%.<p>CONCLUSION: ICL V4c implantation is a safe and effective method for the correction of low to moderate myopia. The visual acuity stabilized 1mo after operation.

ABSTRACT

@#AIM: To investigate the changes of ganglion cell-inner plexiform layer and retina thickness in myopic eyes after intraocular collamer lens(ICL)implantation using quantitative optical coherence tomography(OCT).<p>METHODS:A prospective study enrolled 41 myopic eyes of 26 patients which underwent ICL implantation(average age: 28.19±6.28 years). Aaxial length(AL), uncorrected visual acuity(UCVA), refractive diopter(RD), best corrected visual acuity(BCVA), intraocular pressure(IOP)and OCT were measured and compared in all patients before and after surgery. Optical coherence tomography(OCT)was used to image central retinal thickness(CRT)and ganglion cell-inner plexiform layer thickness(GCT). The changes between pre-and postoperative were analyzed by repeated measures analysis of variance(ANOVA). Least significant difference test was used to compare the specific time points after operation with those before operation.<p>RESULTS: Compared with the preoperative data, all the patients showed significant improvements in the postoperative UCVA and BCVA(<i>P</i><0.05). However, there was no statistical significance in IOP. CRT were 273.20 ±25.48, 274.07±27.64, 277.85 ±25.49, and 275.99 ±24.68μm before and after surgery, respectively, while GCT were 85.31 ±5.19, 88.95±6.87, 87.73±4.23, and 87.45±4.59μm, respectively, with statistically significant changes(all <i>P</i> <0.05). Among them, CRT increased in one month after operation, with statistical significance(<i>P</i><0.01). GCT increased in 1wk, 1mo and 3mo after operation, with statistical significance(<i>P</i><0.05). The changes of GCT in 1wk after operation was positively correlated with AL(<i>r</i>s=0.529, <i>P</i>=0.001).<p>CONCLUSION:ICL implantation for myopic eyes has good efficacy and safety, but macular area changes will occur after surgery, which need to be paid attention to.

@#AIM: To explore the application of improved skills in central hole ICL V4c implantation and its clinical efficacy. <p>METHODS: This study included 141 eyes of 72 patients with myopia from September 2015 to August 2016. An implantable contact lens with a central hole(ICL V4c)was inserted by improved surgical skills including single 2.8mm clear corneal incision, no anterior chamber viscoelastics before ICL implantation. The intraocular pressure(IOP)was measured at the early stage within 6h after operation. The visual acuity, diopter, intraocular pressure and corneal endothelial cell density were observed after operation and follow up 2a. <p>RESULTS: ICL V4c was implanted successfully in all patients by this improved skills and fine clinical results were achieved. Elevated IOP is possible at early stage post-operation, especially within 2h after operation. 95.7% had normal IOP 6h after operation. The postoperative IOP was stable within 2a follow-up period. The cumulative corneal endothelial loss rates of 6mo, 1a and 2a were 4.70%, 7.59% and 9.63% respectively. There was no complications such as subcapsular cataract during the 2a follow-up period.<p>CONCLUSION: The improved skills used in our study is safe and feasible.

@#AIM: To evaluate the clinical efficacy of the posterior chamber intraocular lens(ICL)implantation for high myopia by the double-pass optical quality analysis system II(OQAS II).<p>METHODS: A total of 26 eyes of 52 patients with high myopia admitted to the First Affiliated Hospital of Anhui Medical University from December 2017 to December 2018 were selected. All patients underwent ICL implantation. Follow-up to obtain the best corrected visual acuity(BCVA)of all patients. And a series of parameters, such as uncorrected visual acuity(UCVA), object scatter index(OSI), modulation transfer function(MTF), Strehl ratio(SR)and Predicted VA of 100%,20% and 9%,were obtained before and 1wk,1 and 3mo after operation.<p>RESULTS: Compared with preoperative UCVA and BCVA, the 1wk, 1 and 3mo UCVA were better than preoperative and gradually improved, and the differences were statistically significant(<i>P</i><0.01). Compared with OSI, MTF, SR and Predicted VA 100%, 20% and 9% before operation, the situation improved at 1wk,1 and 3mo after operation, with statistical significance(<i>P</i><0.01).<p>CONCLUSION: ICL implantation can effectively correct high myopia and improve UCVA. Through OQAS II analysis, OSI was reduced after ICL implantation. The postoperative MTF, SR and Predicted VA of 100%, 20% and 9% higher than those before surgery. The visual quality of patients with high myopia was improved after ICL implantation.
