Aseguramiento metrológico para la implementación de un sistema de gestión de la calidad / Metrological assurance for the implementation of a quality management system

RESUMEN Fundamento: la implementación de un sistema de gestión de la calidad basado en la norma NC: ISO 9001 conlleva el establecimiento de la metrología como garantía de calidad de la confiabilidad de sus equipos de medición. Objetivo: demostrar la importancia del aseguramiento metrológico para la implementación de un sistema de gestión de la calidad. Métodos: se realizó un estudio descriptivo de corte transversal en el Centro de Inmunología y Productos Biológicos en la Universidad de Ciencias Médicas de Camagüey, en el período enero de 2014 a mayo de 2018. Resultados: en el primer levantamiento metrológico realizado ninguno de los instrumentos estaba verificado, se realizó un subregistro en el 2014, en el 2017 se incrementó el número de instrumentos registrados en la universidad, durante el estudio se actualizó el plan de verificación y se logró la verificación de la mayoría de los instrumentos de medición que posee el centro. Se constató la necesidad de contar con una asignación en el presupuesto para la adquisición de algunos instrumentos en déficit. Conclusiones: el diagnóstico realizado como parte del aseguramiento metrológico permitió conocer la situación real de los instrumentos de medición en el Centro de Inmunología y Productos Biológicos y lograr la verificación de un número importante de los mismos.

ABSTRACT Background: the implementation of a quality management system based on the NC standard: ISO 9001 entails the establishment of metrology as a guarantee of the reliability quality of its measuring equipments. Objective: to demonstrate the importance of metrology assurance for the implementation of a quality management system. Methods: a descriptive cross-sectional study was carried out at the Center of Immunology and Biological Products of the Camagüey University of Medical Sciences, from January 2014 to May 2018. Results: according to the first metrological survey none of the instruments had been verified, there was an underreporting in 2014, in 2017 the number of registered instruments in the university was increased, during the study the verification plan was updated and most of the measuring instruments were verified. It was found that there is a need of a budget allocation for the acquisition of some of the lacking instruments. Conclusions: the diagnosis made as part of the metrology assurance allowed to know the actual situation of the measuring instruments in the Center of Immunology and Biological Products and to verify an important number of them.

Primeros pasos para implementar un sistema de gestión de la calidad en la universidad médica / First steps in the implementation of a quality management system in the medical university

RESUMEN Fundamento: un sistema de gestión de la calidad brinda la estructura organizativa, los procesos, los procedimientos y las herramientas para implementar las actividades necesarias para alcanzar los objetivos y los requerimientos de la calidad. Objetivo: mostrar las tareas emprendidas en la implementación de un sistema de gestión de la calidad en laboratorios del Centro de Inmunología y Productos Biológicos de la Universidad de Ciencias Médicas de Camagüey. Métodos: se realizó una investigación en sistemas y servicios de salud en los tres laboratorios de las Ciencias Básicas Biomédicas y el de determinaciones clínicas en el Centro de Inmunologìa y Productos Biológicos de la Universidad de Ciencias Médicas de Camagüey, desde abril de 2016 a diciembre de 2018. Resultados: las tareas fueron la capacitación de todo el personal en temas de gestión de la calidad, el diagnóstico de la situación de los laboratorios en lo referente a la calidad, la elaboración del cronograma de trabajo por etapas, la política, los objetivos de la calidad y la proyección estratégica del centro; todas las actividades que se ejecutan en los laboratorios quedaron organizadas en el Diagrama General de los Procesos; se diseñaron indicadores para medir eficacia y eficiencia, y está en proceso de elaboración la documentación de todo el sistema. Conclusiones: se trabaja en el proceso de implementación de un sistema de gestión de la calidad en los laboratorios docentes de las ciencias básicas biomédicas y el de determinaciones clínicas del Centro de Inmunología y Productos Biológicos de la Universidad de Ciencias Médicas de Camagüey, han sido cumplidas las tareas iniciales y en la actualidad está en proceso de elaboración toda la documentación del sistema.

ABSTRACT Background: a quality management system provides the organizational structure, the processes, procedures and tools to implement the necessary activities to achieve the objectives and quality requirements. Objective: to describe the tasks undertaken for implementing a quality management system for laboratories at the Immunology and Biological Products Center of the Camagüey University of Medical Sciences. Methods: a research in health systems and services was carried out in the three teaching laboratories of Basic Biomedical Sciences and the clinical determinations laboratory at the Immunology and Biological Products Center of the Camagüey University of Medical Sciences, from April 2016 to December 2018. Results: the tasks were: training of the whole personnel in quality management issues, diagnosis of the situation of the laboratories in relation to quality, elaboration of the time table by stages, the policy, the objectives of quality and the strategic projection of the center; all the activities carried out in the laboratories were organized in the general diagram of processes; indicators to measure effectiveness and efficiency were designed; and the documentation of the whole system is in process. Conclusions: work is being done in the process of implementing a quality management system in the teaching laboratories of the basic biomedical sciences and in the clinical determinations laboratory of the Immunology and Biological Products Center of the Medical Sciences University of Camagüey, the initial tasks have been fulfilled and presently the elaboration of all the system's documentation is in progress.

The ISO9000 family standards developed by the international organization are the international quality standards which are widely used in various types and sizes of organizations.At present,the graduate student management method introduced by our country plays a certain positive role in guaranteeing the graduate students quality,but it still lacks effective quality control and has not formed the perfect quality management system.Through the analysis of the status quo of the development of graduate education and the problems need to pay attention to in the ISO9000 family standard procedure,document and other factors as well as the construction of the ISO9000 family standard,we put forward goal elaboration,regular staff training and other measures.

Primeros pasos para la implementación de un sistema de gestión de la calidad en los laboratorios clínicos de Camagüey / First steps for the implementation of a quality management system in the clinical laboratories of Camagüey

Fundamento: la necesidad de mejorar la práctica de la salud pública obliga a implementar los Sistemas de Gestión de la Calidad en los laboratorios. Objetivo: describir las acciones llevadas a cabo en la provincia de Camagüey para iniciar la implementación de manera efectiva y sostenida de un Sistema de Gestión de la Calidad en los laboratorios clínicos. Método: se realizó un estudio descriptivo y documental de las acciones realizadas en la provincia de Camagüey para implementar un Sistema de Gestión de la Calidad según Plan de Acción Nacional. Resultados: se capacitaron a veintiún profesionales de diferentes laboratorios de la provincia que recibieron un dar cumplimiento a un plan de acción nacional elaborado al respecto. Se hizo énfasis en la revisión exhaustiva y difusión del proyecto de las Buenas Prácticas de Laboratorio Clínico, según la regulación 3-2008 y en el tema de Ética con el apoyo de un especialista en Bioética Médica, para finalizar con el diagnóstico de la situación actual de las unidades participantes. Los requisitos organizativos, de recursos y relacionados con el producto se cumplen en el 60 por ciento de las unidades de la provincia, sin embargo, los requisitos relacionados con las mejoras tienen alrededor de un 27 por ciento de cumplimiento. Conclusiones: se evidenció que la provincia posee una fortaleza organizativa y de recursos, que le permitirá implementar el Sistema de Gestión de la Calidad en la medida que se complementen los requisitos y recursos pendientes.

Background: the necessity to improve public health practice forces to implement the Quality Management Systems in the laboratories. Objective: to describe the actions carried out in Camagüey province to begin the implementation in effective and sustained way of a in clinical laboratories. Method: a descriptive and documental study of the actions carried out in Camagüey province to implement a Quality Management System according to the Plan of National Action was conducted. Results: twenty-one professionals of different laboratories of the province were qualified to give performance to a plan of national action elaborated on the matter. Emphasis in exhaustive revision and diffusion of the project Good Practices of Clinical Laboratory was made, according to the regulation 3-2008 and in the topic of Ethics with the support of a specialist in Medical Bioethics, to conclude with the diagnosis of the current situation of the participating units. The organizational requirements, of resources and related with the product are fulfilled in the 60 percent of the units in the province, however, the requirements related with the improvements have around 27 percent of performance. Conclusions: it is evidenced that the province possesses an organizational strength and resources that will allow implementing the Quality Management System to the extent that requirements and pending resources were supplemented.

Objective:to detect the effect of working qulity in operating room before and after apply ISO9000 series standard.Methods:the questionnaire of the satisfaction comes from the patients to the nurses of the operating room,the examine score chart to the instrumental nurses in the operating room,the examine score chart to the circulating nurse of operating room as the 3 measuring scale to be investigated as the comparation about the effect of the application of the ISO9000 standard.Results:①The degree of the patients satisfation is improved to such an extent after the ISO9000 standards applied.②The checking result of the instrumental nurses and the circulating nurses is also of the obvious difference;what is more,once the nursing matron published the specific checking standards and the checking result,it is liable to the nurses to realize their own faults.③Through this period of the investigation and the research,the application of the ISO9000 quality managing architecture better formalize the nurses work than before,everything is equipped with foundation and standard,thus the working quality is improved.

Though implementing the quality management system of ISO9000 series standard in the hospital,it enables to make the nursing quality in a controlled state and make every service and operation of nursing staff have regulations to abide by.According to ISO9000 series standard,implementing the management of“procedureization”makes the nursing work closed to the need of patients and satisfies them by stipulating each workflow clearly.
