ABSTRACT

Objective:To explore the value of early pulmonary function tests in children with community acquired pneumonia(CAP)by analyzing the clinical and imaging manifestations and pulmonary function tests of children with CAP.Methods:The clinical data of children diagnosed with CAP and undergoing routine pulmonary ventilation tests within 24 hours of admission were retrospectively collected from January 2016 to June 2018 in the Department of Pediatric Respiratory Medicine of Shengjing Hospital Affiliated to China Medical University.The children′s pulmonary ventilation function test data were statistically analyzed with clinical manifestations, laboratory tests and imaging data.Results:A total of 154 children were included in the study, of which 54(35.06%)had normal pulmonary ventilation function, and the remaining 100 children(64.94%)had different degrees of pulmonary ventilation impairment, which were manifested as restrictive ventilation disorder in 54 cases(35.06%), obstructive ventilation disorder in 8 cases(5.19%), mixed ventilation disorder in 30 cases(19.48%), and small airway ventilation dysfunction in 8 cases(5.19%). According to the results of pulmonary ventilation function, the mixed ventilation dysfunction group had a greater probability of pulmonary imaging changes to consolidation than the normal group( χ2=7.83, P=0.007). Mycoplasma pneumoniae infection was negatively correlated with forced expiratory volume in the first second, 75% forced expiratory flow, one second rate and maximal mid-expiratory flow level( r=-0.23, r=-0.22, r=-0.25, r=-0.23, all P<0.05), and there was no significant correlation with the level of forced vital capacity. Conclusion:Early pulmonary ventilation test has important clinical significance in the diagnosis and treatment of CAP in children.

ABSTRACT

The data of 10 patients with pituitary metastases were retrospectively analyzed, including tumor origin, clinical features, imaging characteristics, diagnosis and differential diagnosis, treatment and prognosis. The results showed that the average age of 10 patients at the time of consultation was 62.0 years. Nine metastases were originated from lung cancer and one from breast cancer. All patients started with central diabetes insipidus, and some of them accompanied with hypopituitarism, as well as occupancy manifestations such as headache, blurred vision, etc. MRI showed abnormalities in the pituitary stalk and posterior pituitary, four of which showed characteristic " dumbbell-shaped" changes. Three patients with epidermal growth factor receptor(EGFR)-mutated lung adenocarcinoma revealed improvement in both primary lesion and pituitary metastases after targeted therapy.

ABSTRACT

The medial temporal lobe epilepsy with hippocampal sclerosis is one of the most common drug resistant epilepsy,which involves complex neural networks and often requires surgical treatment.Studies have shown that it is related to the history of febrile convulsions,however,the further research is needed.Therefore,it is particularly important to accurately identify hippocampal sclerosis and guide clinical treatment.In this paper,the classification of hippocampal sclerosis was summarized,and the possible mechanism of hippocampal sclerosis was explored in terms of cationic-chloride co-transporters,glutamate and its receptors,and changes in expression of mRNA.The characteristics of magnetic resonance,magnetic resonance spectroscopy and PET of hippocampal sclerosis were listed to provide evidence for early recognition of hippocampal sclerosis.In theory,it provides ideas for further prevention of hippocampal sclerosis and improvement of quality of life in patients with medial temporal lobe epilepsy and hippocampal sclerosis.

ABSTRACT

[Objective] To observe imaging changes of Lumbar Intervertebral Disc Protrusion(LIDP)treated by Leverage Location manipulation.[Methods] 33 patients with lumbar intervertebral disc protrusion were observed(male=19,female=14),4 cases of expand type,25 cases of the outstanding type,4 cases of the free type.Using lever to observe the CT imaging of the cases.[Results]The 24 cases had no change in degree of Intervertebral Disc Protrusion while 9 cases had the changes.29 cases changed the compression relation between the nerve root and the lumbar intervertebral protrusion.Another 4 cases had nothing changed.[Conclusion] Leverage location manipulation has a little effect on changing the degree of the lumbar intervertebral protrusion,but can change compression relation between the nerve root and the lumbar intervertebral protrusion which proves that leverage location manipulation has effect on curing the LIDP.
