ABSTRACT

Objective To observe the effect of integrated therapy of Chinese medicine and chemical drugs on adverse reaction and curative effect of initial treatment of secondary pulmonary tuberculosis. Methods Totally 1404 patients with secondary pulmonary tuberculosis and TCM lung consumption diagnostic criteria (syndrome of lung yin deficiency, qi-yin deficiency, yin-deficiency caused excessive fire) were chosen for single blind, randomized, controlled, multicenter clinical trials. Trial group was given 2HRZE/4HR, 1 time/day with Chinese medicine 2 or 3 times/day, and control group was given 2HRZE/4HR only for six months. The adverse reactions and clinical symptoms were observed to evaluate clinical efficacy and safety. Results In terms of reducing liver damage and other adverse reactions, the ratio of trial group had statistical difference with that of control group (P<0.001). In symptom scores of lung yin deficiency syndrome treated for 2, 4, 6 months, yin-deficiency caused excessive fire syndrome treated for 6 months, qi-yin deficiency syndrome treated for 4, 6 months, the differences between the two groups were significant (P<0.001). TCM syndrome curative effect between the two groups was statistical different (P<0.001). Safety evaluation result between the two groups was statistical different by tratified analysis (P<0.001). Conclusion Integrated therapy of Chinese medicine and chemical drugs can improve the symptoms and reduce adverse reactions caused by chemical drugs. It can enhance the curative effect and safety.

ABSTRACT

Objective To observe the therapeutic effect of integrated traditional Chinese and western medicine treatment on systemic inflammatory response syndrome after trauma. Methods Totally 86 cases of severe trauma patients with systemic inflammatory response syndrome in ICU were randomly divided into control group and treatment group. The control group was given routine treatment of ulinastatin, and the treatment group was treated with Xuebijing injection 50 mL, intravenously injected twice per day and rhubarb powder 10 g through stomach tube twice per day for 5 days. The clinical effect and levels of inflammatory factors before and after treatment of the two groups were observed. Results During 5 days of treatment, the levels of TNF-α, IL-6 and IL-8 of the treatment group decreased continuously (P0.05), but which of the treatment group was lower than the control group from the second day of treatment (P<0.05). The average hospital stay, ICU stay, 28 d mortality and APACHEⅡ score of the treatment group were better than that of the control group (P<0.05). Conclusion Xuebijing injection and rhubarb powder in addition to conventional treatment can relieve inflammatory response, effectively inhibit the generation and release of inflammatory mediators, improve the survival rate of systemic inflammatory response syndrome.

ABSTRACT

OBJECTS: The purpose of this study was to present a day hospital model(hereinafter called as "this model") for integrated therapy, to evaluate the result of performing this model, and thus to develop day hospital model for helping psychotic patients effectively and promoting social readaptation. METHODS: The authors performed this day hospital model from 1996, May to December, A total of 25 psychotic Patients participated in this study. BPRS, Life Satisfaction Self-Rating Scale, Program Helpfulness Scale, 13 Therapeutic Factors, Scale, and Most Important Event Questionnaire were used at the start of day hospital treatment and at the end. RESULTS: The results were as follows: 1) In this model, it was possible to offer the wide treatment to heterogeneous patients who varied in age, number of admission, duration of illness, and functional level. 2) In the aspect of therapeutic effect, this model contributed for improving negative symptoms and increasing subjective life satisfaction. 3) When the patients were divided into the higher functional level group and the lower functional level group, the higher functional level group set a high value on group psychotherapy and psychodrama that were unstructured and could make patients explore their inner side deeply, But the lower functional level group did not so. Therefore dividing into two groups by functional level may be more effective for day hospital treatment. 4) Therapeutic factors were different between the periods of day hospital treatment, And in the higher functional level group, 'interpersonal learning' was important at the last period, but in the lower functional level group, 'socializing technique' was important at the last period. So long-term group psychotherapy may be more suitable for the after-care of the higher functional level group, and day care or other psychosocial rehabilitation programs may be more suitable for the after-care of the lower functional level group. 5) This model was easy to apply to Korean day hospital setting, and made staffs treat patients efficiently CONCLUSION: This day hospital model would be used effectively in Korean day hospital setting.
