ABSTRACT

Introducción: la inmunosenescencia está asociada con un mayor riesgo de desarrollo de cáncer. Dentro de las hemopatías malignas que afectan a este grupo de edad, está la leucemia linfoide crónica (LLC), caracterizada por trastornos en la inmunidad adaptativa que incluye las subpoblaciones de linfocitos T. Objetivo: Determinar la frecuencia de las subpoblaciones de linfocitos T en los pacientes adultos mayores con leucemia linfoide crónica evaluados en el Instituto de Hematología e Inmunología de Cuba. Métodos: Se realizó un estudio transversal en 30 adultos mayores con leucemia linfoide crónica. Se cuantificaron los linfocitos TCD3+CD4+ y TCD3+CD8+ en sangre periférica por citometría de flujo. Para la lectura y el análisis de los datos se empleó un citómetro de flujo Beckman Coulter Gallios. Se utilizaron los valores porcentuales, la media y la desviación estándar. Se consideró estadísticamente significativo si p≤0.05. Resultados: Hubo un predominio de hombres que representaron el 56,7 por ciento y del grupo de 70-79 años de edad. No se reportó ningún adulto mayor con LLC con valores altos ni normales de linfocitos TCD3+CD4+. Predominaron los hombres con valores bajos porcentuales de linfocitos TCD3+CD4+, TCD3+CD8+ e inversión del índice CD4/CD8 en relación con las mujeres. Conclusiones: Los adultos mayores con LLC presentan alteraciones en el número de las subpoblaciones de linfocitos T. La acción de estas células en relación al crecimiento de células B malignas aún es desconocido y resulta importante determinar si esto puede reflejar un intento de evasión de las células tumorales al control inmunológico(AU)


Introduction: Immunosenescence is associated with an increased risk of cancer development. Among the malignant hemopathies that affect this age group, it is chronic lymphoid leukemia (CLL), characterized by disorders in adaptive immunity, which include subpopulations of T lymphocytes. Objective: To determine frequency of T lymphocyte subpopulations in older adult patients with chronic lymphoid leukemia evaluated at the Institute of Hematology and Immunology of Cuba. Methods: A cross-sectional study was conducted in 30 older adults with chronic lymphoid leukemia. TCD3+CD4+ and TCD3+CD8+ lymphocytes were quantified in peripheral blood by flow cytometry. A Beckman Coulter Gallios flow cytometer was used to read and analyze the data. The percentage values, the mean and the standard deviation were used. It was considered statistically significant if p≤0.05. Results: There was a predominance of men who represented 56.7 percent and the age group of 70-79 years. No older adults with CLL with high or normal values of TCD3+CD4+ lymphocytes were reported. Men predominated with low percentage values of TCD3+CD4+, TCD3+CD8+ lymphocytes and inversion of the CD4/CD8 ratio in relation to women. Conclusions: Older adult with CLL present alterations in the number of T lymphocyte subpopulations. The role of these cells in relation to the growth of malignant B cells it is unknown and it turns out important to determine if this may reflect an attempt to evade tumor cells from immune control(AU)

Resumo Introdução O câncer tem impacto na vida das crianças e seus familiares. As Histórias em Quadrinhos podem ser uma estratégia de fortalecer o vínculo e a comunicação entre profissional/paciente/família. Objetivo Desenvolver e validar um material instrucional/educativo, no formato de Histórias em Quadrinhos, voltada para crianças hospitalizadas com leucemia linfóide aguda. Metodologia Estudo metodológico desenvolvido em nove etapas: elaboração do projeto de pesquisa; definição e seleção do conteúdo; adaptação da linguagem; inclusão de ilustrações; construção de um material piloto; validação do material; layout; impressão final e disponibilização. A validação ocorreu com 10 especialistas entre março e maio de 2022, utilizando-se o Instrumento de Validação de Conteúdo Educativo em Saúde. Resultados Foram elaboradas 5 Histórias em Quadrinhos, com 6 personagens principais, sendo necessárias 63 horas de trabalho. Elas foram divididas por temáticas (distúrbios gastrointestinais; cistite hemorrágica; problemas relacionados a autoestima e autoimagem; risco de infecção e dor óssea) que obtiveram Índice de Validade de Conteúdo global satisfatório entre 0,78 e 0,87. Conclusões e implicações para a prática As histórias em quadrinhos podem ser utilizadas como fonte atrativa e confiável de informações sobre a doença, servindo como apoio às informações durante a internação hospitalar e o preparo para alta.

Los cómics pueden ser una estrategia para fortalecer el vínculo y la comunicación entre profesional/paciente/familia. Objetivo Desarrollar y validar un material didáctico/educativo, en formato de Historietas, dirigido a niños hospitalizados con leucemia linfocítica aguda. Metodología Estudio metodológico desarrollado en nueve etapas: elaboración del proyecto de investigación; definición y selección de contenidos; adaptación lingüística; inclusión de ilustraciones; construcción de un material piloto; validación del material; disposición; impresión final y disponibilidad. La validación se realizó con 10 especialistas entre marzo y mayo de 2022, utilizando el Instrumento de Validación de Contenido de Educación en Salud. Abstract Introduction Cancer has an impact on the lives of children and their families. Comics can be a strategy to strengthen the bond and communication between professional/patient/family. Objective To develop and validate an instructional/educational material, in the format of Comics, aimed at children hospitalized with acute lymphocytic leukemia. Methodology Methodological study developed in nine stages: preparation of the research project; content definition and selection; language adaptation; inclusion of illustrations; construction of a pilot material; validation of the material; layout; final printing and availability. Validation took place with 10 specialists between March and May 2022, using the Health Education Content Validation Instrument. Results 5 Comics were created, with 6 main characters, requiring 63 hours of work. They were divided by themes (gastrointestinal disorders; hemorrhagic cystitis; problems related to self-esteem and self-image; risk of infection and bone pain) that obtained a satisfactory global Content Validity Index between 0.78 and 0.87. Conclusions and implications for practice Comics can be used as an attractive and reliable source of information about the disease, supporting information during hospitalization and preparation for discharge.

ABSTRACT Introduction: Infection is a major complication in patients with chronic lymphocytic leukemia (CLL). Newly diagnosed patients are at high risk of developing infection caused by encapsulated bacteria, such as Streptococcus pneumoniae and Haemophylus influenzae. Method and Results: However, once treatment is initiated, the spectrum of pathogens causing infection broadens, depending on the treatment regimens. With disease progression, cumulative immunosuppression occurs as a consequence of multiple treatment lines and the risk of infection further increases. On the other hand, the use of targeted therapies in the treatment of CLL have brought new risks of infection, with an increased incidence of invasive fungal diseases, particularly aspergillosis, in patients receiving Bruton kinase inhibitors. Conclusion: In this article, we review the epidemiology of infection in patients with CLL, taking into account the treatment regimen, and briefly discuss the management of infection.

ABSTRACT

La leucemia linfoblástica aguda constituye la neoplasia infantil más frecuente. Los tratamientos actuales posibilitan más del 80% de supervivencia libre de enfermedad por cinco años. En el 2000, se probó un protocolo de quimioterapia llamado leucemia linfoblástica intercontinental Berlín-Frankfurt-Münster (ALLIC BFM). El proceso investigativo se realizó mediante la metodología PRISMA, con el propósito de sistematizar la información acerca de la supervivencia de los pacientes pediátricos con leucemia linfoblástica aguda tratados con el uso del protocolo de quimioterapia ALLIC BFM en sus versiones de 2002 o 2009. La supervivencia global en pacientes donde se utilizó el protocolo de 2002 fue del 52% al 91,7% y la libre de enfermedad fue del 45% a 83,3%; mientras que, con el uso del protocolo 2009 se reportó una supervivencia global del 71,1% al 90% y la libre de enfermedad fue del 69,4% al 90,3%. Los principales factores que afectaron la supervivencia fueron las complicaciones relacionadas con el tratamiento, los pacientes de alto riesgo y la medicación insuficiente.


Acute lymphoblastic leukemia is the most common childhood neoplasia. Current treatments allow more than 80% disease-free survival for five years. In 2000, a chemotherapy protocol called Berlin-Frankfurt-Münster intercontinental lymphoblastic leukemia (ALLIC BFM) was tested. The investigative process was carried out using the PRISMA methodology. This study aimed to systematize the information about the survival of pediatric patients with acute lympho-blastic leukemia treated with the ALLIC BFM chemotherapy protocol in its 2002 or 2009 versions. 52% to 91.7% of patients showed an overall survival in patients where the 2002 proto-col was used, and disease-free was from 45% to 83.3%; while, with the use of the 2009 protocol, an overall survival of 71.1% to 90% was reported, and disease-free survival was 69.4% to 90.3%. The main factors affecting survival were treatment-related complications, high-risk patients, and insufficient medication.

ABSTRACT

Helminth infections may trigger host innate and adaptive immune responses. Group 2 innate lymphoid cells (ILC2) are an important factor involved in type 2 immune responses, and produce a large number of T helper 2 cell (Th2) cytokines following stimulation by interleukin (IL)-25, IL-33 and thymic stromal lymphopoietin (TSLP), which play a critical role in parasite clearance and tissue repair. Following helminth infections, autocrine factors, mast cells, enteric nervous system and Th2 cells have been recently found to be involved in regulation of ILC2. Unraveling the role of ILC2 in immune response against helminth infections is of great value for basic research and drug development. This review summarizes the research progress on ILC2 and its role in helminth infections.

ABSTRACT

OBJECTIVE To study the effects of Wubao capsule on airway inflammation in asthmatic model mice by regulating upstream and downstream cytokines of type Ⅱ innate lymphoid cells (ILC2s). METHODS Totally 40 female BABL/c mice were randomly divided into normal group, model group, positive control group (dexamethasone 1 mg/kg), Wubao capsule low-dose and high-dose groups (0.5, 1 g/kg), with 8 mice in each group. Asthma models were induced by ovalbumin (OVA) sensitization and nebulization. Each group was given normal saline or drug intragastrically for 7 consecutive days. The contents of IgE and OVA-IgE in serum, the contents of interleukin 5 (IL-5), IL-9, IL-13, IL-25, IL-33, thymic stromal lymphopoietin (TSLP) and mucin 5AC (MUC5AC) in bronchoalveolar lavage fluid (BALF) were determined by ELISA. HE staining was used to observe the pathological changes of lung tissues in mice. PAS staining was used to observe the changes of goblet cell proliferation in each group. The number of ILC2s in lung tissue was determined by flow cytometry (except for Wubao capsule low-dose group). RESULTS Compared with model group, the contents of IgE and OVA-IgE in serum and the contents of IL-5, IL-9, IL-13, IL-25, IL-33, TSLP and MUC5AC in BALF were significantly reduced in Wubao capsule high-dose and low-dose groups (P<0.01). The infiltration of inflammatory cells and the thickening of basement membrane in lung tissue was alleviated to varying degrees, and the proliferation of goblet cells was inhibited; the number of ILC2s in lung tissues of mice in Wubao capsule high-dose group was significantly reduced (P<0.01). CONCLUSIONS Wubao capsule could effectively reduce the number of ILC2s in lung tissue, the contents of upstream and downstream cytokines of ILC2s in BALF of asthmatic model mice, so as to inhibit the airway inflammation and improve asthma.

Tertiary lymphoid structures (TLSs) is important channel for tumor immune cell infiltration. The existence of tumor TLSs is not only related to the prognosis of patients, but also to the efficacy of a variety of anti-tumor therapies. To explore the function and immunomodulatory mechanism of TLSs and its potential value as a tumor prognostic biomarker in comprehensive anti-tumor therapy will provide new ideas for follow-up research.

ABSTRACT

Patients with lymphoid hematologic malignancies have a poor prognosis after developing SARS-CoV-2 infection, and their seropositivity rate after SARS-CoV-2 vaccination is lower than that of the healthy population. Since most clinical trials of SARS-CoV-2 vaccines do not include immunodeficient populations, the safety and efficacy of various types of SARS-CoV-2 vaccines for patients with lymphoid hematologic malignancies are unclear. Therefore, physicians should decide whether patients with lymphoid hematologic malignancies receive SARS-CoV-2 vaccination and the timing, type and dose of vaccine after taking into full consideration the patient's immune status, type of treatment and the risk of SARS-CoV-2 infection.

ABSTRACT

Objective To investigate the impact of PD1-TCF1+CD8+ stem-like memory T cells on immunotherapy prognosis and tertiary lymphoid structure in tumors. Methods Pathological tissue sections were collected from 33 patients treated with immunotherapy, 18 cases of NSCLC and 15 cases of ESCC. The expression of PD1-TCF1+CD8+T cells was detected through quantitative analysis by multiplex immunofluorescence. Survival curves were described by the Kaplan-Meier method. Pearson's correlation test was used for correlation analysis. Results The high levels of PD1-TCF1+CD8+T cells had a better PFS in NSCLC and ESCC cohorts. In the NSCLC cohort, high levels of PD1-TCF1+CD8+ T cells were significantly and positively correlated with the number (P=0.0151) and size (P=0.0007) of TLSs. Conclusion In patients with NSCLC and ESCCs, high PD1-TCF1+CD8+ stem-like memory T cell expression indicates improved prognosis and immune response and is associated with the formation of TLSs in the tumor microenvironment of NSCLC.

OBJECTIVES@#To investigate the changes and significance of type 2 innate lymphoid cells (ILC2), interleukin-33 (IL-33), interleukin-25 (IL-25), thymic stromal lymphopoietin (TSLP), interleukin-5 (IL-5), and interleukin-13 (IL-13) in peripheral blood of preterm infants with bronchopulmonary dysplasia (BPD).@*METHODS@#A total of 76 preterm infants with a gestational age of <32 weeks and a length of hospital stay of ≥14 days who were admitted to the Department of Pediatrics of the Affiliated Hospital of Jiangsu University from September 2020 to December 2021 were enrolled. According to the diagnostic criteria for BPD, they were divided into a BPD group with 30 infants and a non-BPD group with 46 infants. The two groups were compared in terms of the percentage of ILC2 and the levels of IL-33, IL-25, TSLP, IL-5, and IL-13 in peripheral blood on days 1, 7, and 14 after birth.@*RESULTS@#The BPD group had significantly lower birth weight and gestational age than the non-BPD group (P<0.05). On days 7 and 14 after birth, the BPD group had significantly higher levels of ILC2, IL-33, TSLP, and IL-5 than the non-BPD group (P<0.05), and these indices had an area under the curve of >0.7 in predicting the devolpment of BPD (P<0.05). Multivariate logistic regression analysis showed that after adjusting for gestational age and birth weight, peripheral blood IL-33, TSLP and IL-5 on days 7 and 14 after birth were closely related to the devolpment of BPD (P<0.05).@*CONCLUSIONS@#Early innate immune activation and upregulated expression of related factors may be observed in preterm infants with BPD. ILC2, IL-33, TSLP, and IL-5 may be used as biological indicators for early diagnosis of BPD.

OBJECTIVE@#To investigate the effects of composite Sophora colon-soluble Capsule (CSCC) on gut microbiota-mediated short-chain fatty acids (SCFAs) production and downstream group 3 innate lymphoid cells (ILC3s) of dextran sulfate sodium (DSS)-induced ulcerative colitis (UC) mice model.@*METHODS@#The main components of CSCC were analyzed by hybrid ultra-high-performance liquid chromatography ion mobility spectromety quadrupole time-of-flight mass spectrometry (UHPLC-IM-QTOF/MS). Twenty-four male BALB/c mice were randomly divided into 4 groups (n=6) by using a computer algorithm-generated random digital, including control, DSS model, mesalazine, and CSCC groups. A DSS-induced colitis mice model was established to determine the effects of CSCC by recording colonic weight, colonic length, index of colonic weight, and histological colonic score. The variations in ILC3s were assessed by immunofluorescence and flow cytometry. The results of gut microbiota and SCFAs were acquired by 16s rDNA and gas chromatography-mass spectrometry (GC-MS) analysis. The expression levels of NCR+ ILC3-, CCR6+ Nkp46- (Lti) ILC3-, and ILCreg-specific markers were detected by enzyme-linked immunosorbent assay, and real-time quantitative polymerase chain reaction and Western blot, respectively.@*RESULTS@#The main components of CSCC were matrine, ammothamnine, Sophora flavescens neoalcohol J, and Sophora oxytol U. After 7 days of treatment, CSCC significantly alleviated colitis by promoting the reproduction of intestinal probiotics manifested as upregulation of the abundance of Bacteroidetes species and specifically the Bacteroidales_S24-7 genus (P<0.05). Among the SCFAs, the content of butyric acid increased the most after CSCC treatment. Meanwhile, compared with the model group, Lti ILC3s and its biomarkers were significantly downregulated and NCR+ ILC3s were significantly elevated in the CSCC group (P<0.01). Further experiments revealed that ILC3s were differentiated from Lti ILC3s to NCR+ ILC3s, resulting in interleukin-22 production which regulates gut epithelial barrier function.@*CONCLUSION@#CSCC may exert a therapeutic effect on UC by improving the gut microbiota, promoting metabolite butyric acid production, and managing the ratio between NCR+ ILC3s and Lti ILC3s.

OBJECTIVE@#To explore the relationship between occurrence of acute graft-versus-host disease (aGVHD) and various immune cell composition in patients with acute myeloid leukemia (AML) after allogeneic hematopoietic stem cell transplantation (allo-HSCT).@*METHODS@#The clinical data of 104 patients with AML undergoing allo-HSCT in our hospital were retrospectively analyzed, and the hematopoietic reconstitution and occurrence of GVHD were analyzed. Flow cytometry was used to detect the proportion of various types of immune cells in the grafts, the number of graft composition in patients with different degrees of aGVHD was calculated and compared, and to analyze the correlation between the severity of aGVHD in AML patients after allo-HSCT and the immune cell components in the graft.@*RESULTS@#There was no significant difference in the time of hematopoietic reconstitution between the high number group of total number of nucleated cells (TNC) and the low number group, while the time of neutrophil and platelet reconstruction in the high number of CD34 group was significantly faster than that in the low number of CD34 group (P<0.05), and the total hospital stay also tends to be shorten. Compared with patients in 0-Ι aGVHD group, both HLA-matched and HLA-haploidentical transplantation, the infusion amounts of CD3+ cells, CD3+CD4+ cells, CD3+CD8+ cells, NK cells and CD14+ monocytes were higher in patients of Ⅱ-Ⅳ aGVHD group, but the difference was not statistically significant (P>0.05); In addition, in patients with HLA-haploidentical transplantation, the number of CD4+CD25+ cells in Ⅱ-Ⅳ aGVHD group was significantly lower than that in 0-Ι aGVHD group (P<0.05), and the same trend was also observed in HLA-matched transplanted patients, but the difference was not significant (P=0.078).@*CONCLUSION@#High number of CD34+ cells in the graft is beneficial to hematopoietic reconstitution in AML patients. To a certain degree, high number of CD3+ cells, CD3+CD4+ cells, CD3+CD8+ cells, NK cells and CD14+ cells tend to increase the occurrence of aGVHD, but high number of CD4+CD25+ regulatory T cells is beneficial to reduce the incidence of aGVHD in AML patients.

Introducción: La leucemia linfoide aguda es una proliferación y transformación maligna de las células progenitoras linfoides en la médula ósea, la sangre y los sitios extramedulares. Es la neoplasia más frecuente en la infancia. Constituye el 80 % de todas las leucemias agudas de la edad pediátrica y la más frecuente de las que se originan a partir del linaje de células B. Desde el punto de vista genético se presentan múltiples alteraciones moleculares y cromosómicas que son utilizadas para la estratificación pronóstica. Objetivo: Describir los biomarcadores genéticos de la leucemia linfoide aguda de linaje B y su implicación pronóstica. Métodos: Se realizó una revisión de la literatura en los idiomas inglés y español, a través del sitio web PubMed y el motor de búsqueda Google académico, de artículos publicados en los últimos cinco años. Se efectuaron un análisis y resumen de la bibliografía revisada. Conclusiones: En la leucemia linfoide aguda de linaje B se detectan múltiples alteraciones citogenéticas como las traslocaciones t(9;22) y la t(12;21), rearreglos en 11q23 que generan genes de fusión, así como otras aberraciones cromosómicas y mutaciones génicas. Este espectro genético involucra genes que participan en el desarrollo de las células linfoides y en la regulación del ciclo celular. El conocimiento de su biología, a partir del estudio de las alteraciones genéticas como biomarcadores predictivos, permite la estratificación de la leucemia linfoide aguda y la aplicación de tratamientos más personalizados para evitar recaídas.


Introduction: Acute lymphoid leukemia is a proliferation and malignant transformation of lymphoid progenitor cells in the bone marrow, blood and extramedullary sites. It is the most common neoplasm in childhood; it constitutes 80% of all acute leukemias in children; and the most frequent of those that originate from the B cell lineage. From the genetic point of view, there are multiple molecular and chromosomal alterations. Objective: To describe the genetic biomarkers of the disease and its prognostic implication. Methods: A review of the literature in English and in Spanish was carried out, in the PubMed website and using the search engine of Google Scholar, for articles published in the last five years. We performed analysis and summary of the reviewed bibliography. Conclusions: In acute lymphoid leukemia, multiple cytogenetic alterations are detected, such as translocation t(9;22), t(12;21), rearrangements in 11q23 that generate fusions genes as well as other chromosomal aberrations and gene mutation. This genetic spectrum involves genes that participate in the development of lymphoid cells and in the regulation of the cell cycle. Knowledge of its biology, based on the study of genetic alterations as predictive biomarkers, allows the stratification of Acute lymphoid leukemia and the application of more personalized treatments to avoid relapses.

Introducción: Los pacientes que ingresan a la unidad de cuidados intensivos pediátricos son aquellos con alto riesgo de mortalidad que pueden presentar síndrome de disfunción orgánica múltiple. Los pacientes que padecen leucemia linfoide aguda forman parte de este grupo. Objetivos: Validar la escala pediátrica de evaluación del fallo multiorgánico secuencial (pSOFA) en pacientes cubanos graves con diagnóstico de leucemia linfoide aguda. Métodos: Se realizó un estudio observacional, prospectivo, multicéntrico, en unidades de cuidados intensivos de hospitales cubanos con 92 pacientes y 184 ingresos. Se calcularon las puntuaciones de las escalas de disfunción multiorgánica secuencial, riesgo de mortalidad e índice de mortalidad pediátrica, y se evaluó la presencia de disfunción orgánica en las primeras 24 h y a las 48 h. Resultados: La puntuación pSOFA fue mayor en los no supervivientes (p < 0,001) y la mortalidad se incrementó de modo progresivo en los subgrupos con las puntuaciones pSOFA más altas. El análisis de las curvas de las características operativas del receptor (ROC) mostró que el área bajo la curva (AUC) para la predicción de la mortalidad con la puntuación pSOFA fue de 0,89, comparado con 0,84 y 0,79 con las escalas PRISM-3 y PIM-2, respectivamente. Conclusiones: La escala pSOFA mostró ser útil para establecer los criterios disfunción orgánica y su especificidad en el riesgo de mortalidad en los pacientes pediátricos cubanos críticos con diagnóstico de leucemia linfoide aguda(AU)


Introduction: Patients admitted to the pediatric intensive care unit (PICU) are those with a high risk of mortality who may present multiple organ dysfunction syndrome. Patients with acute lymphoid leukemia are part of this group. Objectives: To validate the pediatric sequential multi-organ failure assessment scale (pSOFA) in severe Cuban patients diagnosed with acute lymphoid leukemia. Methods: An observational, prospective, multicenter study was carried out in intensive care units of Cuban hospitals with 92 patients and 184 admissions. The scores of the sequential multiple organ dysfunction, mortality risk and pediatric mortality index scales were calculated, and the presence of organ dysfunction was evaluated in the first 24 hours and at 48 hours. Results: The pSOFA score was higher in non-survivors (p <0.001) and mortality progressively increased in the subgroups with the highest pSOFA scores. The analysis of the receiver operating characteristics (ROC) curves showed that the area under the curve (AUC) for the prediction of mortality with the pSOFA score was 0.89, compared to 0.84 and 0.79 with the PRISM-3 and PIM-2 scales, respectively. Conclusions: The pSOFA scale proved useful to establish the criteria for organ dysfunction and its specificity in the risk of mortality in critical Cuban pediatric patients diagnosed with acute lymphoid leukemia(AU)

Introducción: Los cambios en el inmunofenotipo de los linfocitos en los pacientes con linfoma no Hodgkin están asociados con el pronóstico y las respuestas terapéuticas. Sin embargo, no se ha establecido sistemáticamente la asociación con la enfermedad y por tanto su contribución al diagnóstico. Objetivo: Evaluar la asociación del inmunofenotipo linfocitario en sangre periférica con la presencia del linfoma no Hodgkin. Métodos: Se analizaron 31 muestras de sangre periférica de pacientes con diagnóstico confirmado de linfoma no Hodgkin y de 68 individuos sanos como controles, durante el período de 2018 a 2020. Se empleó la citometría de flujo multiparamétrica para el inmunofenotipado. Se calculó el área bajo la curva y el índice de Youden para establecer puntos de corte en los porcentajes linfocitarios. La asociación de los cambios inmunofenotípicos con el linfoma no Hodgkin, se realizó mediante cálculos de Odd ratio. Resultados: El aumento de linfocitos TCD8+ y NKCD56opaco se asoció significativamente con la presencia de linfoma no Hodgkin (OR= 3,4 y 2,9; respectivamente). Por el contrario, la disminución de linfocitos TCD4+, T doble positivo, T doble negativo y NKCD56brillante también se asoció con la existencia de linfoma no Hodgkin (OR= 23,0; 10,7; 6,9 y 15,8; respectivamente). Además, la disminución del índice CD4/CD8 también fue asociada con la enfermedad. Conclusiones: Los cambios encontrados en los inmunofenotipos linfocitarios se asociaron de forma significativa con la presencia del linfoma no Hodgkin, lo cual representa una expresión sistémica de la enfermedad y sugiere su valor diagnóstico(AU)


Introduction: Lymphocyte immunophenotype changes in non-Hodgkin lymphoma patients are associated with prognosis and therapeutic responses. However, its association with the disease has not been systematically established. Therefor its contribution to the diagnosis process. Objective: To assess the association of lymphocyte immunophenotype in peripheral blood with the presence of non-Hodgkin lymphoma. Methods: 31 peripheral blood samples were analyzed from patients with a confirmed diagnosis of non-Hodgkin lymphoma and from 68 healthy individuals as controls, during the period 2018 to 2020. Multiparametric flow cytometry was used for immunophenotyping. The area under the curve and the Youden index were calculated to establish cut-off points in lymphocyte percentages. The association of immunophenotypic changes with non-Hodgkin's lymphoma was made using Odd ratio calculations. Results: The increase in TCD8+ and NKCD56dim lymphocytes from peripheral blood was significantly associated with the presence of non-Hodgkin lymphoma (OR= 3.4 and 2.9, respectively). Oppositely, the decrease in TCD4+, double positive T, double negative T and NKCD56bright lymphocytes was associated with the existence of non-Hodgkin lymphoma (OR= 23.0, 10.7, 6.9 and 15.8, respectively). Therefore, the decrease in the CD4/CD8 rate was also associated with the disease. Conclusion: The changes found in these lymphocytic immunophenotypes were significantly associated with the presence of non-Hodgkin lymphoma, which represents a systemic expression of the disease and suggests its diagnostic value(AU)

RESUMEN Introducción: en la hepatitis autoinmune los mecanismos inmunopatogénicos no están totalmente esclarecidos, múltiples son las investigaciones en este campo, con vistas a enriquecer los conocimientos y ampliar las opciones terapéuticas. Objetivo: sintetizar los conocimientos más recientes acerca de la inmunopatogenia de esta enfermedad. Material y Método: se efectúa una búsqueda exhaustiva de la bibliografía disponible en SciELO, ScienceDirect, Google Académico y PubMed, incluyendo artículos de revisión, estudios experimentales, clínicos, de cohorte y metaanálisis. Desarrollo: se explican los principales mecanismos de tolerancia central y periférica, así como el papel de las subpoblaciones linfoides, las citocinas y el microambiente en la patogenia de la enfermedad. Conclusiones: los avances en el conocimiento de la inmunopatogenia de la hepatitis autoinmune permiten una mejor comprensión de esta enfermedad y son el referente para el diseño de estrategias futuras de tratamiento.


ABSTRACT Introduction: Immunopathogenic mechanisms are not fully clarified in autoimmune hepatitis; there are many investigations in this field with a view to enriching knowledge and expanding therapeutic options. Objective: To synthesize the most recent knowledge about the immunopathogenesis of this disease. Material and Methods: An exhaustive search of the bibliography available in SciELO, ScienceDirect, Google Scholar and PubMed was carried out, including review articles, experimental, clinical, cohort and meta-analysis studies. Development: The main mechanisms of central and peripheral tolerance are explained, as well as the role of lymphoid subpopulations, cytokines and the microenvironment in the pathogenesis of the disease. Conclusions: Advances in the knowledge of the immunopathogenesis of autoimmune hepatitis allow a better understanding of this disease and are the referents in the design of future treatment strategies.

ABSTRACT

La leucemia es una patología neoplásica maligna que constituye un problema de salud que afecta fundamentalmente a la población infantil. Así, se realizó un proceso investigativo con el objetivo de describir la calidad de vida en pacientes pediátricos de LLA con edades entre 2 y 18 años, atendidos en 2019, en el Hospital Pediátrico Baca Ortiz y en el Hospital de Solca - Núcleo Quito, Ecuador; para lo cual se hizo un estudio observacional, transversal, descriptivo, con enfoque cuantitativo. Los datos fueron recopilados mediante la revisión de las historias clínicas de los 60 pacientes en el contexto de investigación. El 66,7% correspondió al sexo masculino, el 43,3% tenía edades entre 2 y 4 años, el 38,3% tuvo fiebre como síntoma inicial. El síndrome de Down resultó la comorbilidad más frecuente (6,7%). En 54 pacientes se diagnosticó LLA tipo B. El 66,7% recibía terapia psicológica, 22 de los enfermos estaban en la fase de inducción y mantenimiento. El 65% abandonó la escuela mientras se le administraba quimioterapia. Predomina-ron los que consideraron su calidad de vida como buena, seguido de los que tuvieron severa afectación. Las mayores afectaciones en los participantes fueron: dificultades con la alimentación, presencia de dolor, falta de comunicación, existencia de ansiedad y presencia de estrés por la preocupación debido a la posible infectividad del tratamiento.


Leukemia is a malignant neoplastic disease that constitutes a health problem that mainly affects children. Thus, this research aimed to describe the quality of life in pediatric ALL patients between 2 and 18 years of age, treated in 2019, at the Baca Ortiz Pediatric Hospital and at the Solca Hospital - Núcleo Quito, Ecuador. A cross-sectional, descriptive, and observational study with a quantitative approach. Data were collected by reviewing the medical records of the 60 patients in the research context. 66.7% were male, 43.3% were between 2 and 4 years old, 38.3% had fever as the initial symptom. Down syndrome was the most frequent comorbidity (6.7%). Type B ALL was diagnosed in 54 patients. 66.7% received psychological therapy. 22 of the patients were in the induction and maintenance phase. 65% dropped out of school while recei-ving chemotherapy. Those ones who considered their quality of life as good predominated, followed by those ones who were severely affected. The greatest effects on the participants were: difficulties with feeding, presence of pain, lack of communication, existence of anxiety and presence of stress due to worry due to the possible infectivity of the treatment.

Comparar a capacidade dos analisadores hematológicos BC-6800 (Mindray) e URIT 5500 em sinalizar a presença de blastos em pacientes portadores de leucemia aguda. Foram analisadas 13 amostras de sangue periférico contendo blastos mielóide ou linfóide, provenientes de um hospital oncológico de Belém ­ Pará, previamente imunofenotipados por citometria de fluxo para verificar a capacidade dos equipamentos Mindray BC-6800 e URIT 5500 em sinalizar a presença dessas células no scatter leucocitário ou por emissão de ÀDJV. Para avaliação da existência de diferença estatística entre os resultados de hemácias, hemoglobina, leucócitos e plaquetas obtidos pelos equipamentos BC 6800 (Mindray) e URIT 5500 foi aplicado o teste não paramétrico ANOVA para análise de variância das amostras, o qual mostrou que não havia diferença estatística entre esses analitos. Não foi aplicado método estatístico para as contagens da diferencial leucocitária, pois o equipamento URIT 5500 não gerou dados numéricos para as amostras patológicas. Os dois equipamentos foram capazes de gerar ÀDJV e mudanças espacial do scatter leucocitário para amostras patológicas, contudo, o analisador BC 6800 (Mindray) foi o único a mudar a cor da população de blastos no scatter leucocitário. Os analisadores BC-6800 (Mindray) e URIT 5500 mostraram boa capacidade em sinalizar, através ÀDJV e do scatter leucocitário, para a presença de blastos mielóides ou linfóides em amostras patológicas. Compare the ability of the BC-600 (Mindray) and URIT 5500 hematological analyzers to signal the presence of blasts in patients with acute leukemia. Thirteen samples of peripheral blood containing myeloid or lymphoid blasts, from a cancer hospital in Belém - Pará, previously immunophenotyped by flow cytometry were analyzed to determine the capacity of the Mindray BC-6800 and URIT 5500 equipment in signaling the presence of these cells in the leukocyte scatter or by emitting flags. To assess the existence of statistical difference between the results of red blood cells, hemoglobin, leucocytes and platelets obtained by the BC 6800 (Mindray) and URIT 5500 equipments, the non-parametric ANOVA test was applied for analysis of variance of the samples, which showed that there was no statistical difference between these analytes. Statistical method was not applied for leukocyte differential counts, as the URIT 5500 equipment did not generate numerical data for the pathological samples. Both were able to generate flags and spatial changes from the leukocyte scatter to pathological samples, however the BC 6800 (Mindray) analyzer was the only one to change the color of the blast population in the leukocyte scatter BC-6800 (Mindray) and URIT 5500 analyzers showed good ability to signal, through flags and leukocyte scatter, for the presence of myeloid or lymphoid blasts in patholical samples. ABSTRACT

Group 2 innate lymphoid cells (ILC2) are a newly identified subset of innate immune cells which are closely associated with the development of bronchial asthma(asthma). ILC2 develops from lymphoid progenitor cells in the bone marrow under the regulation of retinoid-related nuclear orphan receptor α (RORα) and GA-TA-binding protein-3 (GATA-3). ILC2 can be activated by IL-25, IL-33, thymic stromal lymphopoietin (TSLP) and lipid mediator and produce Th2 cytokines, such as IL-4, IL-5, IL-9 and IL-13, which activate innate immune cells and adaptive immune cells to participate in asthma inflammation.ILC2 may be a therapeutic target for asthma, providing new possibilities for the prevention and treatment.

ABSTRACT

Objective:To investigate the influencing factors of delayed methotrexate (MTX) elimination after high-dose methotrexate (HD-MTX) treatment in children with acute lymphoblastic leukemia (ALL) and the effects of delayed MTX elimination and HD-MTX reduction on the prognosis of children with ALL.Methods:The clinical data of 242 children with ALL diagnosed and treated in Tongji Hospital of Tongji Medical College of Huazhong University of Science and Technology from January 2015 to June 2020 in accordance with the Chinese Children's Cancer Group study ALL 2015 (CCCG-ALL 2015) were retrospectively analyzed. Low risk and intermediate/high risk children respectively received 3 g/m 2 and 5 g/m 2 HD-MTX for 4 times, and the serum MTX concentration was monitored. The serum MTX concentration > 1 μmol/L at 44 h of administration was considered as the delayed elimination, which was divided into mild (> 1 μmol/L and ≤ 5 μmol/L), moderate (> 5 μmol/L and ≤ 10 μmol/L) and severe (> 10 μmol/L) delayed elimination. Univariate and multivariate logistic regression analysis were used to analyze the influencing factors of delayed MTX elimination, and univariate Cox proportional hazards model was used to analyze the related factors of ALL relapse. Results:The 242 children with ALL completed 962 times of HD-MTX chemotherapy. The median serum MTX concentration [ M ( Q1, Q3)] at 44 h of administration was 0.45 μmol/L (0.33 μmol/L, 0.72 μmol/L). The total incidence of delayed MTX elimination was 17.7% (170/962). The incidence of mild, moderate and severe delayed elimination was 13.8% (133/962), 2.6% (25/962) and 1.2% (12/962), respectively. Logistic regression analysis showed that age ≥ 7 years old ( OR = 1.68, 95% CI 1.17-2.41, P = 0.005), MTX dose >3 g/m 2 at each course ( OR = 2.14, 95% CI 1.52-3.03, P < 0.001) and the first course of HD-MTX chemotherapy ( OR = 2.05, 95% CI 1.43-2.93, P < 0.001) were independent risk factors for delayed MTX elimination. The median follow-up time was 50 months (34 months, 68 months), 12.8% (31/242) of the children relapsed, and the median relapse time was 30 months (30 months, 39 months). Univariate Cox regression analysis showed that there were no significant differences in the relapse rates among children with different gender, immunophenotype, risk, the number of delayed MTX elimination, and the completion of HD-MTX chemotherapy (the ratio of MTX average dose to initial planned dose) (all P > 0.05). Conclusions:The independent risk factors of delayed elimination of MTX in children with ALL are age ≥ 7 years old, MTX dose > 3 g/m 2 at each course and the first course of HD-MTX chemotherapy. Delayed elimination of MTX and reduction of HD-MTX have no significant effect on ALL relapse.
