ABSTRACT

Objective:To evaluate the efficacy of Baduanjin combined with mecobalamin tablets in the treatment of type 2 diabetic peripheral neuropathy. Methods:Eighty patients with type 2 diabetic peripheral neuropathy who met the inclusion criteria were randomly divided into two groups with 40 patients in each group ( n=40). The control group took oral mecobalamin tablets and self-management, and the treatment group were treated with Baduanjin on the basis of the control group. All patients were treated for 12 weeks. The clinical symptoms were evaluated by Toronto Clinical Scoring System (TCSS), and the motor nerve conduction velocity (MCV) of tibial nerve and the sensory conduction velocity (SCV) of superficial peroneal nerve were measured by EMG evoked potential instrument. The hemorheological indexes (whole blood low shear viscosity, whole blood high shear viscosity and plasma viscosity) were measured before and after treatment. The clinical efficacy was evaluated and adverse events were recorded. Results:In the course of treatment, 39 patients in each group completed the study with each one dropout. The total effective rate was 87.2% (34/39) in the treatment group and 64.1% (25/39) in the control group. There was significant difference between the two groups ( χ2=5.64, P=0.018). After treatment, the TCSS score of the treatment group was significantly lower than that of the control group ( t=-6.23, P<0.01), the tibial nerve MCV (43.06 ± 4.19 m/s vs. 39.55 ± 4.30 m/s, t=3.65), the superficial peroneal nerve SCV (43.23 ± 4.31 m/s vs. 39.92 ± 3.74 m/s, t=3.62) in the treatment group were significantly higher than the control group ( P<0.001), while the whole blood high shear viscosity, the whole blood low shear viscosity, and the plasma viscosity in the treatment group were significantly lower than the control group ( t value were -10.36, -14.21, -13.88, all Ps<0.001). During the treatment, no serious adverse events occurred in both groups. Conclusion:Baduanjin combined with mecobalamin tablets in the treatment of type 2 diabetic peripheral neuropathy can reduce blood viscosity, promote blood circulation, increase nerve conduction velocity, improve clinical symptoms and signs, and improve clinical efficacy.

ABSTRACT

Objective:To explore the efficacy of mecobalamin combined with insulin in the treatment of patients with gestational diabetes (GDM) and its effect on Toll-like receptor 4 (TLR4)/myeloid differentiation factor 88 (MyD88) signaling pathway and maternal and infant outcomes.Methods:A total of 132 patients with GDM in the Fourth Central Hospital of Baoding City were selected and they were randomly grouped according to the principles of stratified random design and controlled design trials, with 66 cases in each group. The control group was treated with insulin, and the observation group was treated with mecobalamin combined with insulin. The curative effect, fasting blood glucose (FBG) and 2 h postprandial blood glucose (2 h PBG), TLR4, MyD88, tumor necrosis factor-α (TNF-α), white blood cells interleukin 1β (IL-1β) before and after treatment and infant outcomes were compared between the two groups.Results:The total effective rate in the observation group was higher than that in the control group: 95.45%(63/66) vs. 84.85%(56/66), and the difference was statistically significant ( χ2 = 4.181, P<0.05). After 1, 2, 3 weeks of treatment, the levels of FBG and 2 h PBG in the observation group were lower than those in the control group: after 1week of treatment, the differences were statistically significant ( P<0.05). After 1, 2, 3 weeks of treatment, the levels of serum TLR4, MyD88, TNF-α, IL-1β in the observation group were lower than those in the control group: after 1 week of treatment, the differences were statistically significant ( P<0.05). The incidence of cesarean section, polyhydramnios, macrosomia and premature birth in the observation group were lower than those in the control group: 7.58%(5/66) vs. 21.21%(14/66), 4.55%(3/66) vs. 16.67%(11/66), 1.52%(1/66) vs. 13.64%(9/66), 3.03%(2/66) vs. 7.58%(5/66), the differences were statistically significant ( P<0.05). Conclusions:Mecobalamin combined with insulin can regulate the TLR4/MyD88 signaling pathway, which can help to control blood sugar and improve the maternal and infant outcomes.

ABSTRACT

OBJECTIVE: To evaluate the clinical efficacy and economics of α-lipoic acid injection alone or combined with Mecobalamin injection versus Mecobalamin injection in the adjunctive treatment of diabetic peripheral neuropathy (DPN). METHODS: Retrieved from PubMed, Cochrane Library, Embase, CNKI, VIP, CBM and Wanfang database, using “mecobalamin” “α-lipoic acid” and“diabetic peripheral neuropathy”as Chinese retrieval words, “Thioctic acid” “α-lipoic acid” “Methylcobal” “Mecobalamin” “Diabetic peripheral neuropathy” as English retrieval words, relevant randomized controlled trials (RCTs) were collected during the date of database establishment to Aug. 30th, 2018. Meta-analysis was conducted for total response rate. From the perspective of health care providers, cost-effectiveness analysis was used for economic evaluation and sensitivity analysis was conducted by a 15% fluctuation of cost and total response rate. RESULTS: Totally 13 RCTs were included, involving 1 131 patients. The results of Meta-analysis showed that the total response rate of two-drug combination therapy in the treatment of DPN was higher than that of mecobalamin alone [RR=1.41, 95%CI(1.28, 1.55), P<0.000 01]; that of α-lipoic acid injection alone in the treatment of DPN was higher than that of mecobalamin injection alone [RR=1.35, 95%CI(1.25,1.47), P<0.000 01], with statistical significance. Results of cost-effectiveness analysis showed that the cost-effectiveness ratio (CER) of Mecobalamin injection was 211.38 yuan, and CER of two-drug combination and α-lipoic acid injection alone were 1 484.42 and    1 383.49 yuan, respectively. The incremental cost-effectiveness ratio (ICER) were 4 589.52 and 4 638.82 yuan, which were all lower than per capita GDP in 2017. Sensitivity analysis showed that the cost-effectiveness analysis results kept stable. CONCLUSIONS: Compared with Mecobalamin injection, α-lipoic acid injection combined with Mecobalamin injection in the adjunctive treatment of DPN show high total response rate and economics.

ABSTRACT

Objective: To investigate the changes of serum proneurotensin (PNT) and insulin-like growth factor-1 (IGF-1) levels in patients with diabetic peripheral neuropathy (DPN) and their relationship with nerve conduction velocity (NCV) and Toronto Clinical Scoring System (TCSS), so as to observe the clinical efficacy of modified Buyang Huanwu Tang on DPN patients and its effect on the levels of PNT and IGF-1. Method: Totally 68 patients with DPN were randomly divided into treatment group and control group, with 34 cases in each group. Both groups were given mecobalamin capsules orally in addition to routine therapy of diabetes, while the treatment group was given modified Buyang Huanwu Tang orally for 4 weeks. The sensory nerve conduction velocity (SNCV) of median nerve, motor nerve conduction velocity (MNCV) of median nerve, SNCV of common peroneal nerve, MNCV of common peroneal nerve, TCSS score and serum PNT, IGF-1 levels were observed before and after treatment. Result: Before treatment, there was no significant difference in SNCV of median nerve, MNCV of median nerve, SNCV of common peroneal nerve, MNCV of common peroneal nerve, TCSS score and serum levels of PNT and IGF-1 between two groups. Serum levels of PNT and IGF-1 were positively correlated with SNCV of median nerve, MNCV of median nerve, SNCV of common peroneal nerve and MNCV of common peroneal nerve (PPPPPPPConclusion: Serum PNT and IGF-1 may be involved in the occurrence and progress of DPN in patients with diabetes mellitus. Modified Buyang Huanwu Tang can increase the levels of serum PNT, IGF-1, NCV but reduce TCSS score in patients with DPN. It has a definite curative effect on DPN.

ABSTRACT

Objective: To study the therapeutic effect of point injection of Salvia Miltiorrhiza and Ligustrazine Hydrochloride Injection (SMLHI) on diabetic peripheral neuropathy (DPN) patients and its effect on oxidative stress response. Methods: A total of 100 DPN patients were selected from the Neurology Department of the Fifth Affiliated Hospital of Zhengzhou University from March 2016 to February 2018. According to the random number table method, the patients were divided into observation group and control group with 50 cases in each group. Each group was treated with routine hypoglycemic drugs, diet control and exercise control, while the control group was treated with Mecobalamin Tablets. The observation group was treated with Mecobalamin Tablets combined with SMLHI at acupoints for 4 weeks as a course of treatment for two consecutive courses of treatment. The clinical symptom score, serum superoxide dismutase (SOD), gamma-glutamyltransferase (GGT), ferritin (SF), total anti-oxidant capacity (T-AOC), and malondialdehyde (MDA) levels, median nerve and peroneal nerve conduction velocity (MNCV), median nerve, peroneal nerve conduction velocity (SNCV), and curative effect were observed and compared before and after treatment. Results: After treatment, the clinical symptom score of the observation group was significantly lower, and the total effective rate of the observation group was 84.00%, which was higher than that of the control group 64.00% (P < 0.05). The SOD level in the observation group was higher, while the GGT, SF, T-AOC, and MDA levels were lower (P < 0.05). The MNCV and SNCV in the observation group were higher (P < 0.05). Conclusion: Acupoint injection of SMLHI for DPN patients can significantly reduce oxidative stress reaction in vivo, promote the recovery of nerve function, improve clinical symptoms of patients with high safety, which is worthy of clinical promotion.

ABSTRACT

OBJECTIVE@#To compare the effect difference between acupuncture combined with western medication and simple western medication for Meniere's disease.@*METHODS@#Ninety-six patients with Meniere's disease were randomly assigned into a combination group (48 cases, 12 cases dropping) and a medication group (48 cases, 9 cases dropping). Betastatin mesylate tablets (once 12 mg, 3 times a day) and mecobalamin tablets (once 0.5 mg, 3 times a day) were prescribed orally in the two groups. Acupuncture was used in the combination group, twice a week, 20 min a time, and the acupoints were Baihui (GV 20), Fengchi (GB 20), Tinggong (SI 19), Hegu (LI 4), Quchi (LI 11), Zusanli (ST 36), Fenglong (ST 40), Taichong (LR 3), Taixi (KI 3) and Tianshu (ST 25). All the treatment was given for continuous 12 weeks. The symptoms were observed by dizziness handicap inventory (DHI), tinnitus handicap inventory (THI), pure tone audiometry score and stuffy ear visual analogue scale (VAS). The effects were compared in the aspects of dizziness, hearing and activity.@*RESULTS@#The scores of DHI, THI, pure tone audiometry and VAS scores after treatment were lower than those before treatment in the two groups (all <0.05), with lower scores of the above 4 indexes in the combination group after treatment (all <0.05). The difference values before and after treatment of the 4 indexes in the combination group were higher than those in the medication group (all <0.05). The effective rates of dizziness, hearing and activity in the combination group were 97.2% (35/36), 91.7% (33/36), 88.9% (32/36), which were better than 71.8% (28/39), 74.4% (29/39) and 69.2% (27/39) in the medication group (all <0.05).@*CONCLUSION@#Acupuncture combined with conventional medication achieve better effect than simple conventional medication.

ABSTRACT

OBJECTIVE:To observe clinical efficacy and safety of Compound xiongshao capsules in the treatment of diabetic peripheral neuropathy (DPN).METHODS:A total of 97 DPN patients selected from our hospital during Jun.2015-Apr.2016 were divided into group A (compound xiongshao treatment group,46 cases) and control group (51 cases) according to random number table.The latter was divided into group B (epalrestat+beraprost sodium group,12 cases),group C (fursultiamine+mecobalamin group,12 cases) and group D (epalrestat group,27 cases) according to clinical symptoms and economic situation of patients.Four groups were given antidiabetic drugs for blood glucose control.Based on it,group A was additionally given Compound xiongshao capsules 0.9 g,tid;group B was additionally given Epalrestat tablets 50 mg,tid+Beraprost sodium tablets 40 μg,tid;group C was additionally given Fursultiamine tablets 50 mg,tid+Mecobalamin tablets 0.5 mg,rid;group D was additionally given Epalrestat tablets 50 mg,tid.All groups were treated for 6 months.Clinical efficacies were observed.TCSS scores,motor nerve conduction velocity (MCV),sensory nerve conduction velocity (SCV),incubation period and amplitude of median nerve and common peroneal nerve,the levels of hemorheology indexes,blood glucose,glycosylated hemoglobin,blood lipid,serum creatinine were compared before and after treatment.The occurrence of ADR was recorded.RESULTS:Total response rates of group A and B (82.61%,83.33%)were significantly higher than those of group C and D (33.33%,66.67%),total response rate of group D was significantly higher than that of group C,with statistical significance (P<0.05).Before treatment,there was no statistical significance in TCSS scores,MCV,SCV,incubation period and amplitude of median nerve,MCV and amplitude of common peroneal never,SCV,incubation period and amplitude of common peroneal never or whole blood high-shear viscosity among 4 groups (P>0.05).After treatment,TCSS scores of group A,B and D were decreased significantly compared to before treatment,and those of group A and B were lower than those of group C and D,with statistical significance (P<0.05).MCV,incubation period and amplitude of median nerve in group A and B,amplitude of median nerve in group C,MCV and amplitude of median nerve in group D were significantly better than before treatment;MCV,incubation period and amplitude of median nerve in group A and B were significantly better than group C and D,with statistical significance (P<0.05).MCV,incubation period and amplitude of common peroneal never in group A,B,C were significantly better than before treatment,MCV and amplitude of common peroneal never in group A,B were significantly better than group C,D;the improvement of incubation period of common peroneal never in group A,B,D were significantly better than group C,with statistical significance (P<0.05).SCV,incubation period and amplitude of median nerve,SCV and amplitude of common peroneal nerve in group A,B and D were significantly better than before treatment;SCV,incubation period and amplitude of median nerve,SCV and amplitude of common peroneal nerve in group A,SCV,incubation period and amplitude of median nerve and amplitude of common peroneal nerve in group B were significantly better than group C and D;SCV of median nerve in group D was significantly better than group C,with statistical significance (P<0.05).Whole blood high-shear viscosity of group A was decreased significantly compared to before treatment,and significantly lower than those of group B,C and D,with statistical significance (P<0.05).There was no statistical significance in total response rate and TCSS score between group A and B,and in the levels of blood glucose,glycosylated hemoglobin,blood lipid or serum creatinine among 4 groups (P>0.05).No obvious ADR was found in 4 groups.CONCLUSIONS:Compound xiongshao capsules shows significant therapeutic efficacy for DPN,and improves nerve conduction velocity,incubation period and amplitude of median nerve and common peroneal nerve,whole blood high-shear viscosity.Its effect is similar to that of epalrestat combined with beraprost sodium,and better than those of fursultiamine combined with mecobalamin,epalrestat alone.It does not affect the blood glucose,blood lipid and serum creatinine levels with good safety.

ABSTRACT

Objective To investigate the clinical effect of folic acid combined with methylcobalamin in the treatment of cerebral small vessel disease with hyperhomocysteinemia.Methods From January 2015 to December 2016,78 patients who were diagnosed cerebral small vessel disease with high homocysteinemia(Hcy)and also associ-ated with vascular mild cognitive impairment in the Second People's Hospital of Liaocheng were selected as the research objects,and they were randomly divided into control group and observation group,with 39 cases in each group.The control group was given routine treatment,and the observation group was treated with folic acid combined with mecobalamin on the bassis treatment of the control group.The levels of The Hcy,vitamin B12,folic acid and the Alzheimer's disease assessment scale-cognitive subscale(ADAS-cog)scores before and after treatment for 3 and 6 months were compared between the two groups.Results Before treatment,there was no statistically significant differ-ence in the level of plasma Hcy between the two groups(t=0.34,P=0.74).There was no statistically significant difference in the level of plasma Hcy of the control group between before and after 3 and 6 months treatment(t=0.36,1.30,all P>0.05).The plasma Hcy level in the observation group after 3 and 6 months treatment[(19.98 ± 3.65)mol/L,(14.96 ±3.54)mol/L]were significantly lower than that before treatment[(26.38 ±2.95)mol/L] (t=8.52,15.48,all P<0.05),which were also significantly lower than that in the control group(t=7.11,12.35, all P<0.05).Before treatment,there were no statistically significant differences in vitamin B12 and folic acid levels between the two groups(t=0.27,0.86,all P>0.05).There was no statistically significant difference in the levels of vitamin B12 and folic acid of the control group between before and after 3 and 6 months treatment(all P>0.05).The vitamin B12 and folic acid levels in the observation group after 3 and 6 month treatment were significantly higher than those before treatment(all P<0.05 ),which were also significantly higher than those in the control group(all P<0.05).Before treatment,there were no statistically significant differences in the ADAS-cog scale score between the two groups(t=0.14,P>0.05).There was no statistically significant difference in the ADAS-cog scale score of the control group between before and after 3 and 6 months treatment(all P>0.05 ).The scores of ADAS-cog scale in the observation group after 3 and 6 months treatment[(12.05 ±2.13)points,(10.24 ±2.84)points]were signifi-cantly lower than that before treatment[(15.07 ±3.56)points](t =4.55,6.62,all P<0.05),which were also significantly lower than that in the control group(all P<0.05).Conclusion Folic acid combined with methylcobal-amin in the treatment of cerebral small vessel disease with hyperhomocysteinemia can significantly reduce the Hcy, vitamin B12 and folic acid levels,which can help to relieve the vascular cognitive impairment,so it is worthy of promo-tion and application.

ABSTRACT

Objective To observe the clinical efficacy of prevention and treatment of chemotherapy induced peripheral neurpathy (CIPN) by vincristine via using TCM prescription of peripheral neuropathy hand-food soaking combined with mecobalamin. Methods Totally 120 patients with non-Hodgkin's lymphoma who all received chemotherapy drugs including vincristine were randomly divided into treatment group (36 cases), control group (36 cases) and combined treatment group (36 cases). All groups received routine treatment (all used CHOP, R-CHOP and H-CAVD plans for the first time), for 6 cycles of chemotherapy. The control group was given mecobalamin at the same time, 0.5 mg each time, oral administration, three times a day. The treatment group was given TCM prescription of peripheral neuropathy hand-foot soaking at the same time, hands first and feet later, 20 minutes each time, twice a day. The combined treatment group was given TCM prescription of peripheral neuropathy and mecobalamin at the same time, the same as the former method. 7 days were set as one treatment course, and all the treatment lasted for 6 cycles of chemotherapy. The incidence and degree of CIPN were evaluated in 2, 4, and 6 cycles of chemotherapy, and the satisfaction was compared. 50 patients of CIPN degree ≥Ⅱ were screened, paused chemotherapy, and participated in consolidation treatment research. The 50 patients were assigned into treatment group (25 cases) and control group (25 cases). The control group took oral mecobalamin, and the treatment group used TCM prescription of peripheral neuropathy hand-foot soaking; the methods were the same as former methods, for 20 d. Clinical efficacy and satisfaction of both groups were evaluated. Results After the second cycle of chemotherapy, the incidence of CIPN in the treatment group and the combined treatment group were obviously lower than the control group (P<0.05). After the fourth cycle of chemotherapy, the incidence of CIPN in the combined treatment group were obviously lower than the control group (P<0.01); the incidence of CIPN degree ≥Ⅱ in the treatment group and combined treatment group were obviously lower than the control group (P<0.05). After the sixth cycle of chemotherapy, the incidence of CIPN degree ≥Ⅱ in the treatment group and combined treatment group were obviously lower than the control group (P<0.01). The satisfaction rate of the treatment group was higher than the control group (P<0.01). For the consolidation therapy to patients of CIPN degree ≥Ⅱ, there was no statistical significance in clinical efficacy between the treatment group and the control group (P>0.05), but the satisfaction rate of the treatment group was higher than the control group (P<0.01). Conclusion TCM prescription of peripheral neuropathy hand-food soaking combined with mecobalamin can obviously relieve CIPN degree induced by vincristine, and improve the life quality of patients.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effect differences among the comprehensive therapy, acupuncture and oral mecobalamine for senile sensorineural hearing loss.</p><p><b>METHODS</b>A total of 126 senile sensorineural hearing loss patients were randomized into a comprehensive group, an acupuncture group and a western medication group, 42 cases in each group. After removing and discontinuing cases, 40 cases in each group were included in the analysis. Acupuncture, acupoint injection and auricular point pressing were used in the comprehensive group. Acupuncture for 2 courses was at the affected Tinggong (SI 19), Tinghui (GB 2), Yifeng (TE 17), etc, once every other day, 10 times as a course with one week rest between 2 courses, 3 times a week. Acupuncture as the comprehensive group was applied in the acupuncture group. Mecobalamin tablet was prescribed orally for 2 courses in the western medication group, 3 times a day, 0.5 mg a time, 3 weeks as a course, one week at the interval. Pure tone hearing threshold value and tinnitus grading were observed before and after intervention in the three groups. The clinical effects were compared.</p><p><b>RESULTS</b>After treatment, pure tone hearing threshold value and tinnitus grading were better than those before treatment in the three groups (all <0.05), with better results in the comprehensive group than those in the other two groups (all <0.05). The total effective rate of pure tone hearing threshold value was 82.5% (33/40) in the comprehensive group, which was superior to 67.0% (27/40) in the acupuncture group and 62.5% (25/40) in the western medication group (both <0.05). The total effective rate of tinnitus was 83.3% (30/36) in the comprehensive group, which was superior to 69.7% (23/33) in the acupuncture group and 57.1% (20/35) in the western medication group (both <0.05).</p><p><b>CONCLUSION</b>The clinical effects of comprehensive therapy on senile sensorineural hearing loss are superior to those of simple acupuncture and oral administration of mecobalamin in terms of pure tone hearing threshold value and tinnitus grading.</p>

ABSTRACT

Objective To study the correlation analysis of mecobalamin in treatment of diabetic peripheral neuropathy and its plasma homocysteine level. Methods 100 patients were selected from January 2013 to August 2016 treatment of diabetic peripheral neuropathy, according to admission ID, were randomly divided into control group and observation group, each group of 50 people. At the same time, 50 healthy persons were collected as normal control group and two groups were given routine treatment of Western medicine. The observation group was given mecobalamin. The plasma levels of plasma homocysteine were detected in the three groups, and the improvement degree of neuropathy was assessed by clinical and electrophysiological methods. Results The observation group and the control group of patients before treatment, plasma homocysteine levels were significantly higher than normal control group; the observation group with Mecobalamin after treatment of plasma homocysteine level in patients were significantly better than the control group (P<0.05); through clinical and electrophysiological evaluation method of observation group of patients in the neuropathic symptoms and nerve conduction velocity is the control group improved significantly (P<0.05). Conclusion The levels of plasma homocysteine and diabetic peripheral neuropathy have a strong correlation, and the use of mecobalamin in treating diabetic peripheral neuropathy can decrease homocysteine levels in patients with, and is effective for the treatment of diabetic peripheral neuropathy, and can be widely applied in clinical treatment.

Objective To study the correlation analysis of mecobalamin in treatment of diabetic peripheral neuropathy and its plasma homocysteine level. Methods 100 patients were selected from January 2013 to August 2016 treatment of diabetic peripheral neuropathy, according to admission ID, were randomly divided into control group and observation group, each group of 50 people. At the same time, 50 healthy persons were collected as normal control group and two groups were given routine treatment of Western medicine. The observation group was given mecobalamin. The plasma levels of plasma homocysteine were detected in the three groups, and the improvement degree of neuropathy was assessed by clinical and electrophysiological methods. Results The observation group and the control group of patients before treatment, plasma homocysteine levels were significantly higher than normal control group; the observation group with Mecobalamin after treatment of plasma homocysteine level in patients were significantly better than the control group (P<0.05); through clinical and electrophysiological evaluation method of observation group of patients in the neuropathic symptoms and nerve conduction velocity is the control group improved significantly (P<0.05). Conclusion The levels of plasma homocysteine and diabetic peripheral neuropathy have a strong correlation, and the use of mecobalamin in treating diabetic peripheral neuropathy can decrease homocysteine levels in patients with, and is effective for the treatment of diabetic peripheral neuropathy, and can be widely applied in clinical treatment.

ABSTRACT

Objective To investigate the calcium dobesilate tablets combined with mecobalamin injection in treatment of diabetic peripheral neuropathy and effects on oxidative stress and neurotrophic factor.Methods 82 cases of diabetic peripheral neuropathy(DPN) patients from February 2015 to October 2016 in our hospital were selected and randomly divided into observation group and control group with 41 cases in each group.The control group was treated with mecobalamin injection, the observation group were treated with calcium dobesilate tablets.The course of treatment was two weeks in two groups.Results The total efficiency of the observation group (90.24%) was higher than the control group (68.29%) , the difference was statistically significant ( P<0.05 ) , patients in the observation group after the treatment of common peroneal nerve and median nerve SCV and MCV higher than the control group, the difference was statistically significant (P <0.05), the level of serum malondialdehyde (MDA) in the observation group was lower than that in the control group and the level of superoxide dismutase ( SOD) in the observation group was higher than that in the control group, the difference was statistically significant (P<0.05), the levels of serum NGF and BDNF in the observation group were higher than those in the control group, the difference was statistically significant (P<0.05), the two groups were no obvious adverse reactions.Conclusion Calcium dobesilate tablets and mecobalamin injection curative effect in the treatment of diabetic peripheral neuropathy , and patients could improve oxidative stress and neurotrophic factors, and the safety is good, has the important research significance.

ABSTRACT

Objective To observe the curative effects of Mailuoning oral liquid combined with Mecobalamin tablets in treating aged diabetic peripheral neuropathy (DPN).Methods Forty-nine patients with DPN were randomly divided into treatment group (n=25) and control group (n=24).The patients in two groups were given Mecobalamin tablets,and the treatment group was added treated with Mailuoning oral liquid.The period of treatment was 3 months.Scores of Toronto clinical scoring system (TCSS) and traditional Chinese medicine (TCM) symptoms quantitative scale were compared between the two groups.The curative effects of all the patients were judged by the TCM symptoms quantitative scores.Results The TCSS levels in two groups after treatment were both significantly lower than those before treatment (all P<0.05).The TCSS level after treatment in treatment group was significantly lower than that in control group (P<0.05).The TCM symptoms quantitative score after treatment in treatment group was significantly lower than that before treatment (P<0.05).The TCM symptoms quantitative score after treatment in treatment group was significantly lower than that in control group (P<0.05).The total effective rate in treatment group (88%) was significantly higher than that in control group (50%) (χ2=6.631,P=0.01).Conclusion Mailuoning oral liquid combined with Mecobalamin tablets is safe and effective for treating DPN.

ABSTRACT

Objective To explore the clinical effect of epalrestat combined with methylcobalamin in the treatment of diabetic peripheral neuropathy.Methods 88 patients with diabetic peripheral neuropathy were chosen from September 2014 to September 2016 in our hospital.The patients were randomly divided into two groups,The control group was treated with conventional treatment,the observation group was given epalrestat combined with methylcobalamin based on the conventional treatment.The effect of the two groups was compared.Results The total effective rate of the observation group was 95.5 % (42/44),which was significantly higher than 70.5 % (31/44) of the control group,the difference was statistically significant(x2 =9.029,P =0.007).Before treatment,the median nerve and common peroneal nerve sensory conduction velocity betweem two groups had no significant differences (t =0.293,0.920,P =0.704,0.096).After treatment,the median nerve conduction velocity and peroneal nerve total sensory conduction velocity in the observation group were (46.2 ± 4) m/s,(42.4 ± 6.3) m/s,which were significantly higher than those in the control group(t =9.038,7.039,P =0.001,0.004).Conclusion Epalrestat combined with methylcobalamin in the treatment of patients with diabetic peripheral neuropathy can effectively improve the nerve conduction velocity.

ABSTRACT

Objective To evaluate the effect of epalrestat combined with methylcobalamin on quality of life of patients with diabetic peripheral neuropathy.Methods 124 patients with diabetic peripheral neuropathy were randomly divided into the control group and the observation group according to the random number table, each group had 62 cases.The two groups were treated on the basis of the effective control of blood glucose and medication to control peripheral neuropathy, the control group used mecobalamin, the observation group used epalrestat combined with methylcobalamin, the two groups were treated continuously for 60 days.The effects of two main physiological changes, clinical efficacy and quality of life were observed.Results The total effective rate of the observation group was 95.2%,which was higher than 80.6% of the control group, the difference was statistically significant(x2=3.072,P<0.05).The fasting plasma glucose(FPG),postprandial 2 h plasma glucose(2 h PPG), glycosylated hemoglobin(HbA1c) were improved compared with before treatment, the differences were statistically significant(all P<0.05),which of the observation group improved significantly better than the control group, the differences were statistically significant(all P<0.05).The evaluation indicators of body pain(BP),social function(SF), role physiological(RP) and mental health(MH) had statistically significant differences compared with the control group (t=16.964,44.583,30.204,44.114,all P<0.05).Conclusion Using epalrestat combined with methylcobalamin tablets in the treatment of patients with diabetic peripheral neuropathy can significantly improve blood glucose levels, clinical symptoms, the quality of life of patients and it has great clinical significance.

ABSTRACT

Objective To observe the curative effect of Xianlinggubao capsules combined with mecobalamin tablets,manipulation and cervical traction in treatment of cervicalspondylotic radiculopathy(CSR).Methods Forty-eight cases CSR patients who were treated in Affiliated Hospital of North China University of Science and Technology from February 2013 to May 2015 were randomly divided into treatment group and control group,each group with 24 cases.All patients were treated with manipulation,cervical traction and mecobalamin tablets(0.5 mg,3 times/d),treatment group received Xianlinggubao capsule(1.5 g,2 times/d)for 21 d.Nervous pain was valued with short-form of McGill questionnaire(SF-MPQ),the difference between the two groups before and after treatment was compared with that before treatment,the pain rating index(PRI),the fuzzy pain rating scale(VAS)and the current degree of pain(PPI).Table 20 quantitative analysis of symptoms of cervical spondylosis of nerve root type was applied to assess the comprehensive evaluation of symptoms and signs,the curative effect was calculate(before and after treatment,and the ratio of the integral before treatment)of the omnibus symptoms and signs.The curative ratio and the healing time were calculated.Results The treatment group McGill in PRI,VAS and PPI score difference were significantly higher than the control group((6.16±1.94)points vs(4.87±2.12)points,(5.23±2.22)points vs.(3.63±1.74)points,(2.76±0.87)points vs.(1.26±1.01)points),the differences were statistically significant(P=0.02,0.03,0.04).Table 20 quantitative analysis index of treatment was significant higher than control group(0.72±0.15 vs.0.37±0.07),the differences were statistically significant(t=1.31,P<0.05).The curative ratio of treatment group was much higher in control group(70.83%(17/24)vs.45.83%(11/24)),the differences were statistically significant(x2=8.15,P=0.02).The treatment group gained shorter healing time((8.31±3.11)d vs.(11.24±4.52)d,t=3.36,P=0.01).Conclusion Assured curative effect was found in Xianlinggubao capsules combined with mecobalamin tablets,manipulation and cervical traction for cervicalspondylotic radiculopathy.

ABSTRACT

Objective To investigate the clinical effect of modified Danggui-Sini decoction combined with Mecobalamin on diabetic peripheral neuropathy. Methods A total of 104 patients with diabetic peripheral neuropathy from January 2015 -2016 year in December in our hospital patients were randomly divided into study group and control group, each of 52 patients. The control group was given Mecobalamin 0.5 mg/times. The study group was given modified Danggui-Sini decoction plus the intervention of the observation group. Four weeks were a course of treatment. The clinical effect rates, the incidence of adverse events, change of common peroneal nerve conduction velocity, hemorheological indexes and advanced glycation end products (AGEs) were compared between the two groups before and after treatment. Results The total effective rate of the study group was 96.2%, which was significantly higher than 82.7% of the control group (χ2=4.981, P<0.05). During treatment, the incidence of adverse events was 5.8% in the study group, and the incidence of adverse events in the control group was 3.9%. There was no significant difference in the incidence of adverse reactions between two groups(P<0.05).After treatment,the motor nerve conduction velocity(49.1 ± 4.5 m/s vs.42.8 ± 4.3 m/s) and sensory nerve conduction velocity (40.1 ± 3.7 m/s vs. 36.5 ± 3.9 m/s) in the study group were significantly higher than those in the control group (t=2.732, 2.723, P<0.05). The whole blood viscosity [(3.72 ± 0.58)mPa?s vs.(4.55 ± 0.47)mPa?s],plasma viscosity[(1.03 ± 0.12)mPa?s vs.(1.78 ± 0.09)mPa?s],erythrocyte blood pressure(31.23% ± 3.19% vs.37.33% ± 4.12%),AGEs(58.44 ± 4.87 ng/L vs.68.03 ± 4.98 ng/L)in the observation group were significantly lower than those in the control group (t=2.704, -2.682, -2.737, -2.856, P<0.05). Conclusions The modified Danggui-Sini decoction combined with Mecobalamin on diabetic peripheral neuropathy can improve peripheral nerve conduction velocity, improve curative effect, showed fewer adverse events.

ABSTRACT

Objective To investigate the impacts of mecobalamin on plasma inflammatory factors (plasma high-sensitivity C-reactive protein (Hs-CRP) and carotid artery plaques in patients with H type hypertension.Methods Forty-eight acute ischemic stroke patients who were diagnosed with H type hypertension in the People's Hospital of Taixing were collected,and they were randomly divided them into treatment group and control group,and 24 cases of each group.Patients in both groups were given conventional therapies,including treatment of anti platelet aggregation,plaque stability and reduced plaque treatment.While patients in the treatment group were given additional oral drug of mecobalamin,500 μg each time,three times a day,6 months in all.Each case was evaluated at the second day of hospitalization,four weeks later,eight weeks later,three months and sixth months later.The examination items involved included level of plasma homocysteine(Hcy),level of hsCRP and conditions of carotid artery plaques under ultrasonography.Results Aafter four weeks,eight weeks,three months and sixth months therapy,there were significant differences between treatment group and control group in terms of Hcy (t =4.049,3.896,6.052,6.159 ; P < 0.05) and the level of hs-CRP (t =37.249,28.376,26.454,0.522P < 0.01).Afrter three months and sixth months therapy,compared to the control group,the carotid artery plaques were obviously reduced,and the differences were statistically significant (t =2.309,2.434 ; P < 0.05).Conclusion Mecobalamin can reduce the level of plasma homocysteine,then lead to reductions of levels of plasma inflammatory factors and volume of carotid artery plaques.

ABSTRACT

Objective To observe the influence of large dose of mecobalamin combining with breviscapin injection on the levels of serum tumor necrosis factor-α(TNF-α)and C-reactive protein (CRP)and the antithrombin Ⅲ(AT-Ⅲ)activity in the patients with diabetic foot(DF).Methods 240 cases of DF were divided into 4 groups according to the random number table method,60 cases in each group:the large dose of mecobalamin plus breviscapin injection group (group Ⅳ);the routine dose of mecobalamin plus brevis-capin injection group (group Ⅲ);the simple breviscapin injection group (group Ⅱ)and the routine dose of mecobalamin group (groupⅠ).4 weeks were taken as a course of treatment.The TNF-α,CRP levels and the AT-Ⅲ activity before and after the treat-ment were detected and performed the comparative analysis.Results The levels of TNF-α,CRP and AT-Ⅲ before treatment had no statistically significant differences among in the four groups (P >0.05).Compared with the results before treatment,the levels of TNF-αand CRP after treatment in 4 groups were obviously decreased and the AT-Ⅲ level was significantly increased,the differ-ences had statistical significance (P <0.05);compared with the group Ⅰ,Ⅱ and Ⅲ,the difference in the group Ⅳ had statistical significance(P <0.05).Conclusion Mecobalamin combined with breviscapin injection is able to decrease the serum inflammatory factors such as TNF-αand CRP,and increase the AT-Ⅲ activity in the patients with DF,which is more significant when increasing the mecobalamin concentration.Therefore,adopting the therapeutical scheme of increasing the mecobalamin concentration combined with breviscapin injection could obviously decrease the inflammatory reaction of DF with good safety,which is one of effective meas-ures for increasing the curative effect of DF.
