ABSTRACT

Objective: To find a proper method to assess colistin resistance in multidrug resistant Gram negative bacteria (MDR-GNB) on a routine basis in resource limited settings. Methods: Clinical samples were processed. MDR-GNB were identified and were examined for colistin resistance by colistin broth elution method, colistin agar method, and colistin disk elution screening method. Broth microdilution method was used the gold standard. Results: A total of 10 235 clinical samples were processed, in which 857 (8.4%) MDR-GNB were identified. The very significant errors, categorical agreement, major errors, positive predictive values, negative predictive values, specificity and sensitivity of all the phenotypic methods were 5.5%, 0%, 94.4%, 100%, 99.6%, 100% and 94.4%, respectively for the detection of colistin resistance. The colistin elution screening method was cheap and easy to perform with similar results to broth microdilution method. Conclusions: All the evaluation methods for colistin resistance showed similar results. So the laboratories can choose any method for detection of colistin resistance. However, we recommend colistin disk elution screening method because, it is easy and cheap and can be performed in limited resources.

ABSTRACT

Objective: To evaluate the essential oils (EO) composition, antimicrobial and antioxidant power of a local plant, Daucus gracilis (D. gracilis). Methods: The aerial parts of D. gracilis were subjected to hydro distillation by a Clevenger apparatus type to obtain the EO which had been analyzed by gas chromatography and gas chromatography coupled with mass spectrometry, and screened for antimicrobial activity against five bacteria and three fungi by agar diffusion method. The mechanism of action of the EO was determined on the susceptible strains by both of time kill assay and lysis experience. The minimal inhibitory concentrations were determined by agar macro-dilution and micro-dilution methods. Anti-oxidative properties of the EO were also studied by free diphenyl-2-picrylhydrazyl radical scavenging and reducing power techniques. Results: The EO yielded 0.68 (v/w). The chemical analysis presented two dominant constituents which were the elemicin (35.3%) and the geranyl acetate (26.8%). D. gracilis EO inhibited the growth of Bacillus cereus and Proteus mirabilis significantly with minimal inhibitory concentrations of 17.15 μg/mL by the agar dilution method and 57.05 μg/mL and 114.1 μg/mL, respectively by liquid micro-dilution. A remarkable decrease in a survival rate as well as in the absorbance in 260 nm was recorded, which suggested that the cytoplasm membrane was one of the targets of the EO. ABSTRACT

Objective To investigate the distribution and antibiotics resistance in blood culture isolates.Methods The clinical isolates of blood specimens from patients in the First Affiliated Hospital of Anhui Medical University during 2004 to 2006 were identified anti the drug resistance to antimierobial agents was tested.The results were analyzed and compared with those during 1986 to 1998.Results A total of 576 strains were isolated from 6203 blood specimens,among which gram positive cocci and gram negative bacilli account for 57.8%(333/576)and 31.4%(181/576),respectively.The frequent isolates were coagulase negative Staphylococcus,Escherichia coli,Staphylococcus aureus,hemolytic Streptococcus viridans,Klebsiella pneumoniae and fungi.The methicillin-resistant coagulase negative Staphylococci(MRCNS)accounted for 78.3%(155/198).Gram negative bacilli were highly susceptible to imipenem,piperacillin/tazobactam and amikacin.Conclusions The incidence of bloodstream infections caused by MRCNS,gram negative bacilli producing ESBLs and fungi are increasing.The clinical isolates from blood have hish resistance to the first line antibiotics.
