ABSTRACT

Introdução: O trauma de mão constitui uma epidemia, ocorrendo por diversos fatores, como acidentes de trabalho e violência urbana. Isso gera um impacto significativo por ser uma unidade anatômica vulnerável e importante na realização de atividades cotidianas e para o trabalho. Desse modo, faz-se necessária uma avaliação epidemiológica mais aprofundada no que tange, sobretudo, às fraturas de mãos, entendendo sua elevada morbidade. Método: Estudo descritivo e retrospectivo, feito por análise de prontuários de pacientes conduzidos em um hospital terciário no período de janeiro de 2018 a dezembro de 2020. Resultados: Participaram do estudo 290 pacientes, sendo 85,7% do sexo masculino e 14,3% do sexo feminino. 138 indivíduos tinham entre 20 e 39 anos, representando um total de 47,6%. 51,6% eram do interior do estado do Ceará e 48,4% eram da capital. O principal mecanismo de trauma associado às fraturas foram os acidentes de trânsito (36,4%), seguidos por acidentes de trabalho (26,9%) e ferimentos por arma de fogo (14%). Em relação aos ossos fraturados, houve uma acentuada prevalência do acometimento das falangeanas, em todas as faixas etárias, representando 210 pacientes (72,4%). Conclusão: O presente estudo corrobora com os dados presentes na literatura. Desse modo, é evidenciada a repetição de prevalência de faixa etária, principais mecanismos de trauma envolvidos, bem como ossos acometidos nas fraturas de mão.


Introduction: Hand trauma is an epidemic occurring due to several factors, such as work accidents and urban violence. This significantly impacts it as it is a vulnerable and important anatomical unit for daily activities and work. Therefore, a more in-depth epidemiological assessment is necessary regarding hand fractures and understanding their high morbidity. Method: A descriptive and retrospective study was conducted by analyzing the medical records of patients treated in a tertiary hospital from January 2018 to December 2020. Results: 290 patients participated in the study, 85.7% male and 14.3 % female. One hundred thirty-eight individuals were between 20 and 39 years old, representing 47.6%. 51.6% were from the interior of the state of Ceará, and 48.4% were from the capital. The main trauma mechanism associated with fractures was traffic accidents (36.4%), followed by work accidents (26.9%) and firearm injuries (14%). Concerning fractured bones, there was a marked prevalence of phalangeal involvement in all age groups, representing 210 patients (72.4%). Conclusion: The present study corroborates the data present in the literature. In this way, the repetition of the prevalence of age group, main trauma mechanisms involved, as well as bones affected by hand fractures is evidenced.

ABSTRACT

O fenômeno da judicialização da saúde carece de dados organizados e comparáveis entre estudos sobre o tema. Diversas fontes, recortes prévios e intermediários geram resultados conflitantes e de difícil repro-dução. Esta nota argumenta a necessidade de definir um padrão/elemento comum nos processos judiciais em saúde, propondo o sistema JUDJe, que utiliza o Diário de Justiça Eletrônico para extrair, organizar e classificar esses dados. O JUDJe gerou um banco de dados aberto com 100 mil movimentações processuais sobre casos de câncer. Defende mais qualidade e conexão dos dados, e mais acesso a esses últimos, pro-movendo equidade e visão multidimensional. Propõe a "judicialização 2.0" com dados em rede conectando saúde e direito.


The phenomenon of health judicialisation lacks organised and comparable data between studies on the subject. Different sources, previous and intermediate pieces of information generate conflicting results that are difficult to reproduce. This note argues the need to define a common standard/element in health lawsuits and proposes the JUDJe system, using the online Official Gazette to extract, organize and classify such data. JUDJe generated an open geo-referenced database with 100 thousand legal proceedings on cancer cases. It advocates more quality and connection of data, and more access to them, promoting equity and a multidimensional vision. It proposes a "judicialization 2.0" connecting the health and law domains.


El fenómeno de la judicialización de la salud carece de datos organizables y comparables entre los estudios sobre el tema. Diferentes fuentes, cortes previos y intermedios generan resultados contradictorios y dificiles de reproduzir. Esta nota argumenta la necesidad de definir un elemento común/estándar en los procesos judiciales de salud, proponiendo el sistema JUDJe, que utiliza el Diario Oficial Electrónico de Justicia para extraer, organizar y clasificar esos datos. El JUDJe generó una base de datos abiertos georreferenciada con 100 mil actuaciones judiciales sobre casos de cáncer. Defiende más calidad y conexión de datos, y más acceso a esos últimos, promoviendo la equidad y una visión multidimensional. Propone la "judicialización 2.0" con datos en red que conecten salud y derecho.

ABSTRACT

Este estudo tem como objetivo identificar, na literatura científica, produtos e serviços desenvolvidos por bibliotecários vislumbrando as práticas de Ciência Aberta. A questão principal é identificar: qual o papel dos bibliotecários frente aos desafios da Ciência Aberta? Predominantemente qualitativa, esta pesquisa pode ser caracterizada como bibliográfica, exploratória e descritiva. Para atingir seu objetivo, utilizou-se a técnica de revisão rápida de literatura. Foi realizado um levantamento de publicações indexadas na Brapci, na Scopus e na Web of Science, sendo recuperadas três publicações em cada. Ao excluir um título que se repetiu, o corpus da pesquisa configurou-se com seis artigos e dois resumos apresentados em evento. Conclui-se que debates sobre o novo modus operandi de fazer ciência vêm aumentando e os bibliotecários parecem intimamente relacionados às ações de Ciência Aberta nas diversas etapas da pesquisa científica. Devido às suas habilidades e aos seus serviços, entende-se que exercem um dos papéis centrais na concretização da abertura da ciência.


This study aims to identify, in the scientific literature, products and services developed by librarians with a view to Open Science practices. The main question is to identify: what role is played by librarians facing the challenges of Open Science? Predominantly qualitative, this research can be characterized as bibliographic, exploratory, and descriptive. To achieve its objective, a rapid literature review technique was used. A survey of publications indexed in Brapci, Scopus and Web of Science was carried out, and three publications from each were retrieved. After excluding one title that was repeated, the research corpus consisted of six articles and two abstracts presented at an event. We conclude that debates about the new modus operandi of doing science have been increasing and librarians seem closely related to Open Science actions in the various stages of scientific research. Because of their skills and services, they play one of the central roles to achieve the opening of science.


Este studio tiene como objetivo identificaren la literature científica los productos y servicios desarrollados por los bibliotecarios com vistas a las prácticas de la Ciencia Abierta. La cuestión principal es identificar: ¿ cuál es el papel de los bibliotecarios ante los desafíos de la Ciencia Abierta? Predominantemente cualita-tiva, esta investigación puede caracterizar se como bibliográfica, exploratoria y descriptiva. Para lograr su objetivo, se utilizó la técnica de revision rápida de la literatura. Se realizó un estudio de las publicaciones indexadas en Brapci, Scopus y Web of Science, recuperándo se tres publicaciones en cada una de ellas. Al excluir un título repetido, el corpus de la investigación quedó configurado con seis artículos y dos resúmenes presentados en un evento. Concluimos que los debates sobre el nuevo modus operandi de hacer ciencia han aumentado y los bibliotecarios parecen estar estrechamente relacionados con las acciones de la Ciencia Abierta en las distintas etapas de la investigación científica. Por sus habilidades y servicios, se entiende que ejercen uno de los papeles centrales en la realización de la Ciencia Abierta.

ABSTRACT Purpose: To evaluate the saccadic movements of patients with visual field loss due to primary open-angle glaucoma. Methods: Thirteen patients with good visual acuity (0.2 logMAR or better) (seven patients with primary open-angle glaucoma 65 ± 13 years) and six controls (51 ± 6 years) yielded a comprehensive ophthalmological examination, including Humphrey Visual Field tests (SITA-Standard 24-2), and performed a monocular, exploratory digital visual search task that quantifies the duration for finding the number "4" on a random array of digits distributed on the screen. After individual adjustments of the angle and distance positioning, the screen was spatially matched with the 24-2 visual field, and divided into five areas for analysis. During the task, saccades were simultaneously recorded in the same eye with a video-based eye tracker. Results: The patients with primary open-angle glaucoma showed a significantly higher number of saccades/screen (median ± interquartile range, 59.00 ± 29.00 vs. 32.50 ± 19.75 saccades (p=0.027) and visual search time per screen (38.50 ± 60.14 vs. 23.75 ± 8.90 seconds (p=0.035) than the controls did. Although the univariate analysis indicated a significant correlation with visual field mean deviation (coefficient=26.19 (p=0.02), only the visual search time/screen was significantly associated with the number of saccades/screen in the multivariate regression model (coefficient=0.55 (p<0.001). Overall, no significant correlation was observed between the sectorial number of saccades and the sensitivity of the five visual field areas. Conclusions: The patients with primary open-angle glaucoma show impaired search performance and showed a higher number of saccades needed to find stimuli when performing the exploratory visual task.

ABSTRACT

Objective@#This study aimed to explore the root length of maxillary and mandibular anterior teeth and central incisor crown-root morphology in patients with high-angle skeletal Class Ⅱ open bite, aiming to provide a reference for clinical treatment.@*. Methods@#This study was reviewed and approved by the Ethics Committee, and informed consent was obtained from the patients. CBCT images of eighty-one untreated patients (40 anterior open bite patients and 41 normal overbite patients) with high-angle skeletal Class Ⅱ malocclusion were selected before treatment. Dolphin software was used to study the root length of maxillary and mandibular anterior teeth and central incisor crown-root morphology, and the differences between the two groups were analyzed.@*Results@#There was no statistical significance in the root length of maxillary lateral incisor and canine between the open bite group and the normal overbite group, significant differences were found in the root length of maxillary central incisor (11.12 ± 1.37) mm、mandibular central incisor(10.15 ± 1.09)mm, mandibular lateral incisor(11.27 ± 1.15)mm and mandibular canine(12.81 ± 1.48)mm between the open bite group and the normal overbite group(P<0.05). On the other hand, the two groups were significantly different in crown-root morphology of the maxillary central incisor (1.10° ± 3.62° vs. 4.53° ± 2.30°, P<0.01) but not in the mandibular central incisor.@*Conclusion@#The root length of the maxillary central incisor, mandibular central incisor, mandibular lateral incisor, mandibular canine in high-angle Class Ⅱ open bite patients is shorter than that in high-angle Class Ⅱ normal overbite patients, and the long axis of the crown of the maxillary central incisor in high-angle Class Ⅱ open bite patients obviously deviates toward the labial side relative to the long axis of the root. The crown-root angle is smaller, which is beneficial to torque control or adduction movement of the anterior teeth in high-angle Class Ⅱ open bite patients.

ABSTRACT

AIM: To compare the clinical outcomes of trabeculectomy combined with phacoemulsification intraocular lens implantation with suture-guided penetrating Schlemm canaloplasty combined with phacoemulsification intraocular lens implantation for the treatment of primary open angle glaucoma(POAG).METHODS: A retrospective case-control study was conducted on 53 cases(53 eyes)with advanced-stage POAG and cataract that received surgery at our hospital from June 2021 to June 2022, among which 26 eyes underwent trabeculectomy combined with phacoemulsification and intraocular lens implantation(group A), and 27 eyes underwent suture-guided penetrating Schlemm canaloplasty combined with phacoemulsification and intraocular lens implantation(group B). The best-corrected visual acuity(BCVA), intraocular pressure, and the number of intraocular pressure lowering drugs were compared between the two groups. Furthermore, the intraoperative and postoperative complications and success rate of surgery were recorded.RESULTS: The intraocular pressure of two groups of patients at 1 wk, 1, 3 and 6 mo after surgery were lower than that before preoperatively(all P&#x003C;0.017). There was a notable disparity in the frequency of anterior chamber hemorrhage between the two groups(P&#x003C;0.05), but there was no significant variance in the occurrence of superficial anterior chamber and choroid detachment(all P&#x003E;0.05). There was no significant difference in BCVA(LogMAR)between the two groups at 6 mo after surgery(group A: 1.29±1.19, group B: 0.78±1.01, P=0.098), and there was no significant difference in the number of anti-glaucoma drugs [group A: 0(0, 2.75), group B: 0(0, 1), P=0.209]. Additionally, there was no significant difference in the success rate of surgery between the two groups at 6 mo postoperatively(Z=0.448, P=0.654).CONCLUSIONS: Suture-guided penetrating Schlemmcanaloplasty is a safe and effective treatment for advanced-stage POAG, with a success rate comparable to that of trabeculectomy.

AIM: To understand the publication status, research trends, and cutting-edge and hot topics in this field by conducting a bibliometrics analysis of relevant literatures on the pathogenesis of primary open angle glaucoma(POAG)in the past 30 a.METHODS:A total of 986 relevant literatures on the pathogenesis of POAG published on the core databases of China National Knowledge Infrastructure(CNKI)and Web of Science(WOS)from 1 September 1993 to 1 September 2023 were retrieved. CiteSpace(6.2.R.4)and VOSviewer(1.6.18)software were used to conduct knowledge graph analysis on the retrieved literature, including publication volume, author, research institution, country/region, and keywords.RESULTS:The United States(243 articles)has the highest number of publications, followed by China(121 articles). The foreign institution with the highest number of publications is Harvard University(37 articles), while domestic institutions such as Zhongshan Ophthalmic Center, Sun Yat-sen University, ophthalmology department of Xuanwu Hospital of Capital Medical University, and Peking University First Hospital tied for the highest number of publications. Louis R. Pasquale(21 articles)is the most prolific English author. Wang Ningli is the most active Chinese researcher in this field. Keywords include trabecular meshwork, intraocular pressure, aqueous humor, glucocorticoid, hemorheology, etc.CONCLUSION: The research on the pathogenesis of POAG is in a period of vigorous development. The United States has the largest number of publications in this field, and Harvard University is a leading institution in this field. The research focus in the field of POAG has shifted from the structural aspect to the genetic level, and gene research and traditional Chinese medicine treatment have broad application prospects in this field.

ABSTRACT

AIM:To observe the clinical efficacy of vitrectomy at different times for open ocular trauma and explore the timing of stage Ⅱ vitrectomy.METHODS: Retrospective case series study. A total of 60 cases(60 eyes)with open ocular trauma who visited our ophthalmology department from June 2022 to February 2023 were included. They were divided into treatment group A(interval ≤14 d)and treatment group B(interval &#x003E;14 d)based on the interval between the stage Ⅰ emergency treatment surgery and the stage Ⅱ vitreoretinal surgery. Among the 32 cases(32 eyes)in the treatment group A, 16 eyes(50%)had eyeball rupture, 13 eyes(41%)had penetrating injury, and 3 eyes(9%)had perforating injury. Among the 28 cases(28 eyes)in the treatment group B, 15 eyes(54%)had eyeball rupture, 12 eyes(43%)had penetrating injury, and one eye(4%)had perforating injury. The two groups of patients were followed-up for 6 mo after surgery, and the treatment effects were compared.RESULTS:There was no statistically significant difference in visual acuity between the two groups of patients before vitrectomy(P&#x003E;0.05). In the treatment group A, 10 eyes(31%)had significantly improved visual acuity, 21 eyes(66%)had effectively enhanced visual acuity, and 1 eye(3%)had no improvement in visual acuity at 6 mo after surgery. Among the 28 eyes in the treatment group B, 5 eyes(18%)had significantly improved vision, 16 eyes(57%)had effectively enhanced vision, and 7 eyes(25%)had no change in vision, with statistically significant difference between the two groups(U=322.5, P=0.032). There was no significant difference between the treatment group A and the treatment group B in complications such as secondary glaucoma, silicone oil dependence, vitreous hemorrhage, and eyeball atrophy(P&#x003E;0.05). There was no evidence of traumatic proliferative vitreoretinopathy(TPVR)in the treatment group A during postoperative follow-up, which was significantly lower than that of the treatment group B(P&#x003C;0.05).CONCLUSION:The prognosis of the stage Ⅱ vitrectomy for open ocular injury is relatively good after completing the stage Ⅰ surgery within 2 wk.

OBJECTIVE@#To explore the effectiveness of Nice knot technique for wound closure in Gustilo type ⅢA and ⅢB open tibial fractures.@*METHODS@#A retrospective study was performed on 22 patients with Gustilo type ⅢA and ⅢB open tibial fractures, who underwent wound closure using the Nice knot technique and were admitted between June 2021 and June 2022. There were 15 males and 7 females. The age ranged from 18 to 67 years, with an average of 41.9 years. The causes of injury included traffic accident in 11 cases, falling from height in 7 cases, and heavy object injuries in 4 cases. Fractures were located on the left side in 9 cases and on the right side in 13 cases. And 9 cases were type ⅢA fractures and 13 were type ⅢB fractures according to Gustilo classification. All patients had extensive soft tissue injuries, and no vascular or neurological damage was observed. The time from injury to debridement was 3-8 hours (mean, 6.5 hours). The sizes of wounds before operation and at 2 weeks after operation were measured and wound healing rate at 2 weeks after operation were calculated. The wound healing time and wound healing grading were recorded. The Vancouver Scar Scale (VSS) score was used to assess the wound scar after wound healed and the excellent and good rate was calculated.@*RESULTS@#The wound area was 21.0-180.0 cm 2 (mean, 57.82 cm 2) before operation, and it was 1.2-27.0 cm 2 (mean, 6.57 cm 2) at 2 weeks after operation. The wound healing rate at 2 weeks after operation was 76%-98% (mean, 88.6%). After operation, 2 cases needed to adjust Nice knot due to skin cutting and 1 case occurred soft tissue infection on the wound. The other patient's wounds healed. The average wound healing time was 27.8 days (range, 18-44 days). And the wound healing were grade A in 13 cases and grade B in 9 cases. VSS score was 2-9, with an average of 4.1; 10 cases were rated as excellent, 10 as good, and 2 as poor, with an excellent and good rate of 90.9%. All patients were followed up 9-24 months (mean, 14.6 months). During follow-up, no deep infection or osteomyelitis occurred. Two cases experienced fracture non-union, and were treated with compression fixation and bone grafting. The fractures of the other patients all healed, with a healing time of 85-190 days (mean, 148.2 days).@*CONCLUSION@#Nice knot technique can be used in wound closure of Gustilo type ⅢA and ⅢB open tibial fractures effectively, which is easy to operate.

ABSTRACT Purpose: To evaluate the effect of tobacco smoking on trabeculectomy outcomes. Methods: Charts of patients with glaucoma who underwent trabeculectomy performed by a single surgeon between 2007 and 2016 were retrospectively reviewed. Charts were screened for a documented history of smoking status before surgery. Demographic and clinical preoperative variables were recorded. Based on smoking history, subjects were divided into two groups: smokers and nonsmokers. Any bleb-related interventions (e.g., 5-flourouracil injections ± laser suture lysis) or bleb revision performed during the postoperative period were noted. Success was defined as an intraocular pressure >5 mmHg and <21 mm Hg without (complete success) or with (qualified success) the use of ocular hypotensive medications. Failure was identified as a violation of the criteria mentioned above. Results: A total of 98 eyes from 83 subjects were included. The mean age of the subjects was 70.7 ± 11.09 years, and 53% (44/83) were female. The most common diagnosis was primary open-angle glaucoma in 47 cases (47.9%). The smokers Group included 30 eyes from 30 subjects. When compared with nonsmokers, smokers had a significantly worse preoperative best-corrected visual acuity (p=0.038), greater central corneal thickness (p=0.047), and higher preoperative intraocular pressure (p=0.011). The success rate of trabeculectomy surgery at 1 year was 56.7% in the smokers Group compared with 79.4% in the Group nonsmokers (p=0.020). Smoking presented an odds ratio for failure of 2.95 (95% confidence interval, 1.6-7.84). Conclusion: Smokers demonstrated a significantly lower success rate 1 year after trabeculectomy compared with nonsmokers and a higher requirement for bleb-related interventions.


RESUMO Objetivo: Avaliar o efeito do tabagismo nos desfechos da trabeculectomia. Métodos: Uma revisão retrospectiva do gráfico de pacientes com glaucoma submetidos à trabeculectomia foi realizada por um único cirurgião entre 2007 e 2016. Os gráficos foram examinados para uma história documentada de condição de fumante antes da cirurgia. Variáveis pré-operatórias clínicas e demográficas e clínicas foram registradas. Os pacientes foram divididos em dois grupos de acordo com sua história de tabagismo em fumantes e não fumantes. Quaisquer Intervenções relacionadas à bolha, por exemplo, injeções de 5-fluorouracil + lise de sutura com laser, ou revisão da bolha realizada durante o período pós-operatório foram observadas. O sucesso foi definido como pressão intraocular > 5 mmHg e < 21 mm Hg sem (sucesso completo) ou com (sucesso qualificado) medicamentos hipotensores oculares. A falha foi identificada como violação dos critérios mencionados acima. Resultados: O estudo incluiu 98 olhos de 83 pacientes com idade média de 70,7 ± 11,09 anos, sendo 53% (44/83) dos pacientes do sexo feminino. O diagnóstico mais comum foi o glaucoma de ângulo aberto primário com 47 casos (47,9%). O Grupo de fumantes incluiu 30 olhos de 30 pacientes. Os fumantes, quando comparados aos não fumantes, apresentaram uma melhor acuidade visual pré-operatória significativamente pior (p=0,038), maior espessura central da córnea (p=0,047) e maior pressão intraocular pré-operatória (p=0,011). A taxa de sucesso de um ano para a cirurgia de trabeculectomia foi de 56,7% no Grupo de fumantes contra 79,4% no Grupo de não fumantes (p=0,020). O tabagismo apresentou razão de chances para falha de 2,95 95% de IC (1,6-7,84). Conclusão: Os fumantes demonstraram uma taxa de sucesso significativamente menor em um ano após a trabeculectomia em comparação com os não fumantes e uma maior necessidade de intervenções relacionadas à bolha.

Introducción: Las fracturas supracondíleas del húmero en el niño, por lo general, son tratadas de manera quirúrgica, durante esa intervención se pueden presentar situaciones específicas en este tipo de enfermos. Objetivo: Actualizar y brindar información sobre algunas de las situaciones transoperatorias en pacientes con fractura supracondílea del húmero. Métodos: La búsqueda y análisis de la información se realizó en un periodo de 61 días (primero de septiembre al 31 de octubre de 2022) y se emplearon palabras de búsqueda relacionadas con la investigación. A partir de la información obtenida, se realizó una revisión bibliográfica de un total de 245 artículos publicados en las bases de datos: PubMed, Hinari, SciELO y Medline, mediante el gestor de búsqueda y administrador de referencias EndNote, de ellos, se utilizaron 29 citas seleccionadas para realizar la revisión, 28 de los últimos cinco años. Resultados: Se hace referencia a cuatro de las situaciones transoperatorias más frecuentes en este tipo de fractura. Se mencionan la conminución de la pared medial, como identificar esta situación y su conducta. En relación a las lesiones asociadas, se recomienda primero estabilizar el antebrazo y luego la fractura supracondílea. Para las fracturas inestables en flexión se recomienda la técnica a emplear. Por su parte, la conversión de la reducción cerrada a abierta está justificada en ciertas circunstancias que de forma detallada se describen en el trabajo. Conclusiones: Las fracturas supracondíleas del húmero en el niño son tratadas en su mayoría mediante tratamiento quirúrgico. Durante el transoperatorio se pueden presentar situaciones para las cuales el médico tratante debe estar preparado.


Introduction: Supracondylar fractures of the humerus in children are generally treated surgically, during surgery intervention may occur specific situations in this type of patient. Objective: To update and provide information on some of the intraoperative situations in patients with supracondylar fracture of the humerus. Methods: The search and analysis of the information was carried out in a period of 61 days (September 1st to October 31st, 2022) and search words related to the investigation were used. Based on the information obtained, a bibliographic review of a total of 245 articles published in the PubMed, Hinari, SciELO and Medline databases was carried out using the EndNote search manager and reference administrator, of which 29 selected citations were used to carry out the review, 28 of the last five years. Results: Reference is made to four of the most frequent intraoperative situations in this type of fracture. Comminution of the medial wall, how to identify this situation and its behavior are mentioned. In relation to associated injuries, it is recommended to first stabilize the forearm and then the supracondylar fracture. For unstable fractures in flexion, the technique to be used is recommended. For its part, the conversion from closed to open reduction is justified in certain circumstances that are described in the article. Conclusions: Supracondylar fractures of the humerus in children are mostly treated by surgical treatment. During the trans-operative period situations may arise for which the treating physician must be prepared.

ABSTRACT Objective: Describe the development, implementation, and utilization of dashboards for epidemiological analysis through open data research during the COVID-19 pandemic. Methods: The dashboards were designed to analyze COVID-19 related public data from various sources, including official government data and social media, at world level. Data processing and cleaning techniques were used to join datasets. We calculated Spearman correlation coefficient between the COVID-like symptoms data of the University of Maryland and Facebook Health research, called COVID Trends and Impacts Survey (CTIS) and the official data of notified COVID-19 cases by the Brazilian Health Ministry. Results: The dashboards were successful in predicting the onset of new waves of COVID-19 in Brazil. The data analysis revealed a correlation between the CTIS and the official number of cases the country. This article shows the potential of interactive dashboards as a decision-making tool in the context of public health emergencies, as it was used by the official communication of the Rio Grande do Sul state government. Conclusion: The use of dashboards for predicting the spread of COVID-19 in Brazil was a useful tool for decision-making. To anticipate waves of the disease gives time so that these decisions can be potentially more assertive. This drafts the need of more interdisciplinary actions of this nature, with visualization tools on epidemiologic research.


RESUMO Objetivo: Descrever o desenvolvimento, a implementação e o uso de painéis para a análise epidemiológica de dados abertos durante a pandemia de COVID-19. Métodos: Os painéis foram criados para analisar dados públicos relacionados á COVID-19 de várias fontes, incluindo dados oficiais dos governos e de redes sociais, a nível global. Técnicas de processamento e limpeza foram utilizadas para aglutinar os bancos de dados. Calculamos o coeficiente de correlação de Spearman entre as curvas de sintomas gripais da pesquisa da Universidade de Maryland em conjunto com o Facebook, chamada COVID Trends and Impacts Survey (CTIS), e a curva de casos notificados pelo Ministério da Saúde no Brasil. Resultados: Os painéis obtiveram sucesso em antecipar a chegada de novas ondas de COVID-19 no Brasil. A análise do dado revelou a correlação entre a pesquisa CTIS e o número oficial de casos no país. O artigo destaca o potencial de painéis interativos como uma ferramenta de tomada de decisão no contexto de emergências de saúde pública, como, por exemplo, no uso destes para a comunicação oficial do governo do Rio Grande do Sul. Conclusão: O uso de painéis para prever o avanço da COVID-19 no Brasil foi uma ferramenta útil para a tomada de decisão. A antecipação de ondas da doença possibilita tempo oportuno para que essas decisões sejam potencialmente mais assertivas. Isso esboça a necessidade de mais ações interdisciplinares dessa natureza, com ferramentas de visualização nas pesquisas epidemiológicas.

El campo de la radiología ha experimentado avances notables en las últimas décadas, con desarrollos que van desde la mejora de la calidad y digitalización de las imágenes hasta la detección asistida por computadora. Particularmente, la aparición de técnicas de Inteligencia Artificial basadas en Deep Learning y Visión Computacional han promovido soluciones innovadoras en el diagnóstico y el análisis radiológico. Se explora la relevancia de los desarrollos y modelos open source en el progreso de estas técnicas, resaltando el impacto que la colaboración y el acceso abierto han tenido en el avance científico del campo. La investigación tiene un enfoque cualitativo, con alcance descriptivo y retrospectivo, de corte longitudinal. Se realizó un análisis documental de la evolución y el impacto del open source en la Radiología, poniendo de relieve la colaboración multidisciplinar. Se examinaron casos de uso, ventajas, desafíos y consideraciones éticas en relación con la implementación de soluciones basadas en Inteligencia Artificial en Radiología. El enfoque open source ha mostrado ser una influencia positiva en la Radiología, con potencial para influir en la atención médica, ofreciendo soluciones más precisas y accesibles. No obstante, se presentan desafíos éticos y técnicos que requieren atención.


The field of radiology has seen notable advances in recent decades, with developments ranging from image quality improvement and digitization to computer-aided detection. Particularly, the emergence of Artificial Intelligence techniques based on Deep Learning and Computer Vision have promoted innovative solutions in diagnosis and radiological analysis. This article explores the relevance of open source developments and models in the progress of these techniques, highlighting the impact that collaboration and open access have had on the scientific advancement in this field. This research has a qualitative approach, with a descriptive, retrospective, longitudinal scope. A documentary analysis of the evolution and impact of open source in Radiology was carried out, highlighting multidisciplinary collaboration. Use cases, advantages, challenges and ethical considerations were also examined in relation to the implementation of AI-based solutions in Radiology. The Open Source approach has been shown to be a positive influence in Radiology, with the potential to influence medical care, offering more precise and accessible solutions. However, there are ethical and technical challenges that require attention.

ABSTRACT

Objetivo: discorrer sobre um caso clínico de mordida aberta anterior com uma abordagem de tratamento orto-cirúrgica. Relato de caso: Paciente, leucoderma, sexo feminino, 21 anos, ASA 1, buscou tratamento cirúrgico, após a recomendação do ortodontista. Esta já havia realizado tratamento ortodôntico no período de dois anos, mas sem alcançar resultados satisfatórios. Ao exame clínico, se concluiu uma ausência de assimetria facial significativa e uma paciente face longa. Além de um padrão esquelético classe II, com uma bi-protusão dos incisivos caracterizando uma mordida aberta anterior, no qual, posteriormente foi solicitada a cirurgia para a correção da deformidade dentofacial e maloclusão. Conclusão: Em síntese, após todo o tratamento orto-cirúrgico houve uma melhora significativa da oclusão da paciente, com o posterior ganho da sua autoconfiança e estética facial e, consequentemente, o reestabelecimento da sua função mastigatória, respiratória e fonação.


Objective: discuss a clinical case of anterior open bite with an ortho surgical treatment approach. Case report: Patient, Caucasian, female, 21 years old, ASA 1, sought surgical treatment after orthodontist recommendation. The patient already went through orthodontic treatment for two years, not achieving satisfactory results. On clinical examination, there was an absence of significant facial asymmetry and a long-face. In addition to a class II skeletal pattern, with a bi-protrusion of the incisors characterizing an anterior open bite, in which surgery was later requested to correct the dentofacial deformity and malocclusion. Conclusion: In summary, after ortho-surgical treatment, there was a significant improvement in the patient's occlusion, with substantial increase of her self-confidence and facial aesthetics and consequently, the reestablishment of patient's masticatory, respiratory and phonation function.


Objetivo: discutir un caso clínico de mordida abierta anterior con abordaje de tratamiento ortoquirúrgico. Reporte de caso: Paciente, caucásica, sexo femenino, 21 años, ASA 1, buscó tratamiento quirúrgico, por recomendación del ortodoncista. Ya se había sometido a un tratamiento de ortodoncia durante dos años, pero sin lograr resultados satisfactorios. En el examen clínico, había ausencia de asimetría facial significativa y una cara alargada. Además de un patrón esquelético clase II, con biprotrusión de los incisivos caracterizando una mordida abierta anterior, en la que, posteriormente, se solicitó cirugía para corregir la deformidad dentofacial y maloclusión. Conclusión: En resumen, después de todo el tratamiento ortoquirúrgico, hubo una mejoría signifi cativa en la oclusión de la paciente, con la consiguiente ganancia en su confi anza en sí misma, en su estética facial y en consecuencia, el restablecimiento de su función masticatoria, respiratoria y fonatoria.

Introducción: El quiste pilonidal puede ser abordado según técnicas quirúrgicas abiertas y cerradas, con distintos resultados estéticos. La cicatriz es un factor importante y representa un indicador de satisfacción. Una herramienta para valorar cicatrices es el cuestionario POSAS 2.0, adaptado transculturalmente para Chile. Objetivo: Conocer la valoración subjetiva de los pacientes operados de quiste pilonidal respecto a su cicatriz, según la técnica quirúrgica. Material y Método: Estudio es de carácter retrospectivo, observacional, transversal, analítico y de cohorte. Se aplica la escala de evaluación por el paciente del cuestionario POSAS 2.0, con una calificación desde 1 a 10, con puntaje total entre 7 y 70. Se recolectaron variables clínico-demográficas. Se aplicó estadística analítica, se compararon los resultados obtenidos entre técnicas abiertas y cerradas. Resultados. Se encuestaron 101 pacientes operados de quiste pilonidal entre 2013 y 2019, de los cuales 59 (59,4%) fueron de sexo masculino. La edad promedio fue de 23 años (DS 7,2). Del total, 22 (21,8%) de los pacientes fueron manejados con técnicas cerradas y 79 con técnicas abiertas (78,2%). El promedio del puntaje total de la escala fue 20 (DS 11,4), 22 en técnicas cerradas y 19,6 en técnicas abiertas. No se encontraron diferencias estadísticamente significativas. Conclusión: La cicatriz postoperatoria es valorada positivamente, con un puntaje que denota características similares a la piel normal. No existen diferencias significativas de la percepción de los pacientes respecto a su cicatriz. Esta es la primera comunicación chilena que evalúa la valoración subjetiva de pacientes sobre la cicatriz postoperatoria de quiste pilonidal.


Introduction: The pilonidal cyst can be approached according to open and closed surgical techniques, with different aesthetic results. The scar is an important factor and represents an indicator of satisfaction. A tool to assess scars is the POSAS 2.0 questionnaire, adapted cross-culturally for Chile. Objective: To know the subjective assessment of patients operated on for pilonidal cyst with respect to their scar, according to the surgical technique. Material and Method: This is a retrospective, observational, cross-sectional, analytical, and cohort study. The patient evaluation scale of the POSAS 2.0 questionnaire is applied, with a score from 1 to 10, with a total score between 7 and 70. Clinical-demographic variables were collected. Analytical statistics were applied, the results obtained between open and closed techniques were compared. Results: We surveyed 101 patients operated on for pilonidal cyst between 2013 and 2019, of which 59 (59.4%) were male. The mean age was 23 years (SD 7.2). Of the total, 22 (21.8%) of the patients were managed with closed techniques and 79 with open techniques (78.2%). The mean total score of the scale was 20 (SD 11.4), 22 in closed techniques and 19.6 in open techniques. No statistically significant differences were found. Conclusion: The postoperative scar is positively valued, with a score that denotes similar characteristics to the normal skin. There are no significant differences in patient's perception of their scar. This is the first chilean communication that evaluates the subjective assessment of patients on the postoperative scar of pilonidal cyst.

RESUMEN El aneurisma de aorta abdominal (AAA) sintomático no roto es una patología que involucra a aquellos pacientes con AAA intacto, pero que presentan dolor abdominal y/o lumbar atribuido al aneurisma. Esta forma de presentación clínica es po tencialmente mortal dado que su etiopatogenia comprende cambios agudos en la pared aórtica, incluyendo inflamación, lo que incrementa la probabilidad de ruptura inminente. Está claro que estos pacientes deben ser derivados a reparación del AAA. Sin embargo, el momento de la intervención es controvertido. Por lo tanto, el objetivo del presente trabajo fue revisar la información actualizada sobre el abordaje diagnóstico-terapéutico del AAA sintomático no roto.


ABSTRACT Symptomatic unruptured abdominal aortic aneurysm (AAA) refers to a group of patients with intact AAA but who present abdominal and/or lumbar pain attributed to the aneurysm. This form of clinical presentation is potentially fatal since its etiopathogenesis, involving acute changes in the aortic wall, including inflammation, increases the probability of impending rupture. It is clear that these patients should be referred to AAA repair. However, the timing of the intervention is contro versial. Therefore, the aim of the present work was to review updated information on the diagnostic-therapeutic approach of symptomatic unruptured AAA.

Purpose: To determine the factors affecting the visual outcome after surgical repair of limbal corneal lacerations at a tertiary eye care center in South India. Methods: A retrospective analysis of patients diagnosed with limbal tears between 2011 and 2021 was conducted. Demographic information such as age, gender, cause of injury, and size of the laceration was recorded. Comprehensive ocular examination was performed, including gentle B scan evaluation whenever not contraindicated for detailed posterior segment evaluation. Only those cases with a minimum follow?up of one year were included. Postoperative best?corrected visual acuity, intraocular pressure (IOP), cornea clarity, and integrity of the wound at last follow?up were noted. Results: Out of the 20 patients, 15 (75%) were males and 5 (25%) were females. The mean age was 42.6 � 22.4 years. All 20 patients had a penetrating injury, with four (20%) injured by a stick, two (10%) by an iron rod, three (15%) due to road traffic accident (RTA), three (15%) by glass, and eight (40%) with other nonspecific objects [two (10%) with needle, two (10%) with elastic rope, two (10%) with bangle, and two (10%) with metal]. The average time between the injury and the surgery was 48 hours (2 days). Four (20%) patients underwent a second surgery within a week of repair. After limbal tear repair, at final follow?up at 3 years, 7 (35%) had VA worse than 20/800, 3 (15%) had VA between 20/100 and 20/800, and 10 (50%) achieved VA better than 20/80. Conclusion: Preoperative visual acuity (VA), mode of injury, and size of wound affect the final visual outcome after surgical repair of limbal corneal laceration. Preoperative VA and mode of injury were statistically significant even in the multivariate analysis.

Resumen Introducción: Las lesiones de la mano son uno de los padecimientos más frecuentemente atendidos en el Servicio de Cirugía Plástica y Reconstructiva del Hospital Central Militar, dentro de la amplia variedad de estas lesiones se encuentran las lesiones del sistema flexor. Objetivo: Estimar la incidencia, funcionalidad y tiempo de reintegración laboral en pacientes con lesiones de sistema flexor de la mano tratados en el Hospital Central Militar en el periodo del 1º de marzo del 2020 al 1º de marzo del 2022. Metodología: Estudio observacional, retrospectivo, longitudinal descriptivo en pacientes adultos con lesión de sistema flexor de la mano, en los cuales la causa de la lesión no haya condicionado un trauma complejo de la mano. Se utilizó como referencia los criterios de recuperación funcional de Strickland y Goglovac. Resultados: A pesar de que las lesiones de la zona II flexora en la literatura presentan un peor pronóstico para la recuperación de la función normal de la mano, los resultados demuestran que la mayor parte de los pacientes se encuentra en una escala funcional buena. Limitaciones del estudio: El estudio fue llevado a cabo durante el periodo comprendido por la pandemia COVID, por lo que las consultas subsecuentes con tiempos más alargados y el inicio tardío de las consultas de rehabilitación pudieron haber contribuido hacia una recuperación más deficiente de la recuperación funcional de los pacientes.


Abstract Introduction: Hand injuries are one of the conditions most frequently treated in the Plastic and Reconstructive Surgery Service of the Hospital Central Militar, within the wide variety of these injuries are injuries to the flexor system. Objective: To estimate the incidence, functionality and labor reintegration time in patients with hand flexor system injuries treated at the Hospital Central Militar in the period from March 1, 2020 to March 1, 2022. Methodology: Observational, retrospective, longitudinal, descriptive study in adult patients with injury to the flexor system of the hand, in whom the cause of the injury did not result in complex trauma to the hand. The Strickland and Goglovac functional recovery criteria were used as reference. Results: Despite the fact that flexor zone II lesions in the literature present a worse prognosis for the recovery of normal hand function, the results show that most of the patients are in a good functional scale. Limitations of the study: The study was carried out during the period covered by the COVID pandemic, so that subsequent consultations with longer times and the late start of rehabilitation consultations could have contributed to a poorer recovery of functional recovery. from the patients.

Introducción. La colecistectomía laparoscópica es el estándar de oro para el manejo de la patología de la vesícula biliar con indicación quirúrgica. Durante su ejecución existe un grupo de pacientes que podrían requerir conversión a técnica abierta. Este estudio evaluó factores perioperatorios asociados a la conversión en la Clínica Central OHL en Montería, Colombia. Métodos. Estudio observacional analítico de casos y controles anidado a una cohorte retrospectiva entre 2018 y 2021, en una relación de 1:3 casos/controles, nivel de confianza 95 % y una potencia del 90 %. Se caracterizó la población de estudio y se evaluaron las asociaciones según la naturaleza de las variables, luego por análisis bivariado y multivariado se estimaron los OR, con sus IC95%, considerando significativo un valor de p<0,05, controlando variables de confusión. Resultados. El estudio incluyó 332 pacientes, 83 casos y 249 controles, mostrando en el modelo multivariado que las variables más fuertemente asociadas con la conversión fueron: la experiencia del cirujano (p=0,001), la obesidad (p=0,036), engrosamiento de la pared de la vesícula biliar en la ecografía (p=0,011) y un mayor puntaje en la clasificación de Parkland (p<0,001). Conclusión. La identificación temprana y análisis individual de los factores perioperatorios de riesgo a conversión en la planeación de la colecistectomía laparoscópica podría definir qué pacientes se encuentran expuestos y cuáles podrían beneficiarse de un abordaje mínimamente invasivo, en búsqueda de toma de decisiones adecuadas, seguras y costo-efectivas


Introduction. Laparoscopic cholecystectomy is the gold standard for the management of gallbladder pathology with surgical indication. During its execution, there is a group of patients who may require conversion to the open technique. This study evaluated perioperative factors associated with conversion at the OHL Central Clinic in Montería, Colombia. Methods. Observational analytical case-control study nested in a retrospective cohort between 2018 and 2021, in a 1:3 case/control ratio, 95% confidence level and 90% power. The study population was characterized and the associations were evaluated according to the nature of the variables, then the OR were estimated by bivariate and multivariate analysis, with their 95% CI, considering a value of p<0.05 significant, controlling for confounding variables. Results. The study included 332 patients, 83 cases and 249 controls, showing in the multivariate model that the variables most strongly associated with conversion were: the surgeon's experience (p=0.001), obesity (p=0.036), gallbladder wall thickening on ultrasonography (p=0.011), and a higher score in the Parkland classification (p<0.001). Conclusions. Early identification and individual analysis of the perioperative risk factors for conversion in the planning of laparoscopic cholecystectomy could define which patients are exposed, and which could benefit from a minimally invasive approach, in search of making safe, cost-effective, and appropriate decisions
