ABSTRACT

【Objective】 To investigate the effect of multiple apheresis platelet donation on platelet indexes and aggregation function in platelet donors, and to analyze the relationship between platelet indexes and their relationship with platelet aggregation rate. 【Methods】 A total of 83 platelet donors were randomly selected from Foshan Central Blood Station from September 2021 to October 2022, and were divided into control group (n= 9, first-time platelet donors or donors with the time interval from the last platelet donation >1 year) and study group (n= 74, repeat platelet donors or donors with the time interval from the last platelet donation ≤ 1 year) according to the times of blood donation. The study group was divided into 4 subgroups: 2-5 times group, 6-10 times group, 11-15 times group and 16 times group. The platelet count(Plt), platelet distribution width (PDW), mean platelet volume (MPV), large platelet ratio (P-LCR), and maximum platelet aggregation rate (MAR) before donation in each group were detected and analyzed. 【Results】 There were no significant differences in Plt, MPV, PDW, P-LCR and MAR between the subgroups and the control group (P>0.05), and there were no significant differences in Plt, MPV, PDW, P-LCR and MAR between each subgroup (P>0.05) .There was a positive correlation between Plt and MAR in blood donors (P<0.05), and the correlation coefficient was 0.445. MPV, PDW and P-LCR were negatively correlated with MAR (P<0.05), and the correlation coefficients were -0.282, -0.233 and -0.217, respectively. Plt was negatively correlated with MPV, PDW and P-LCR (P < 0. 05), and the correlation coefficients were -0.399, -0.307 and -0.339, respectively. MPV, PDW and P-LCR are positively correlated with each other. The correlation coefficient between MPV and PDW was 0.792, that between MPV and P-LCR was 0.863, and tthat between PDW and P-LCR was 0.817. 【Conclusion】 There was no significant effect of multiple platelet donations on Plt, PDW, MPV, P-LCR and MAR in blood donors. Plt has the most significant impact on MAR among platelet indexes of platelet donors.

ABSTRACT

Objective To compare the therapeutic effect of aspirin and clopidogrel on coronary heart disease.Methods From June 2016 to July 2017,186 patients with coronary heart disease in the Third People ˊs Hospital of Yuyao were divided into control group and observation group according to different treatment methods ,with 93 cases in each group.Both two groups were treated with routine symptomatic therapy ,the control group was given aspirin and the observation group was given clopidogrel.Biochemical indicators ,cardiac function and adverse events were observed.Results There were no statistically significant differences between the two groups before treatment (all P>0.05 ).The platelet aggregation rate and activation time of partial thrombin of the observation group were (38.11 ±2.15)%and (48.21 ±3.50)s,respectively,which were better than those of the control group (t=48.819, 18.752,all P<0.05).There were no statistically significant differences in prothrombin activity and prothrombin time between the two groups(all P>0.05).The probability of grade Ⅳand gradeⅠin the observation group was 8.60%and 38.71%,respectively,which was significantly different from those in the control group (χ2 =5.243,6.541,all P<0.05).The incidence rate of adverse events in the observation group (1.08%) was lower than that in the control group(7.53%),and the difference was statistically significant (χ2 =4.072,P<0.05).Conclusion Clopidogrel is more effective than aspirin in the treatment of coronary heart disease.It can significantly improve the coagulation function,cardiac function,and it is safe and with less adverse reactions.

ABSTRACT

OBJECTIVE:To investigate the effects of Yiqi huayu granules on gastrointestinal motility of gastrointestinal motili-ty disorder model mice and blood rheology of acute blood stasis model rats,and to provide experimental evidence for clinical use of it. METHODS:50 ICR mice were randomly divided into normal group,model group,mosapride group(positive control chemi-cal drug,0.003 g/kg),Muxiang shunqi pills group(positive control TCM,0.36 g/kg)and Yiqi huayu granules group(2.4 g/kg), with 10 mice in each group. Treatment groups were given relevant medicine solution intragastrically,once a day,for a week;nor-mal group and model group were given normal saline intragastrically. Those groups fasted for 24 h after last medication,and then were given above medicine intragastrically;1 h later,they were given atropine sulfate intragastrically to induce gastrointestinal mo-tility disorder model except for normal control group. The stomach residue rate and intestinal propulsive rate of mice were mea-sured. 60 SD rats were randomly divided into normal group,model group,Compound danshen tablet group(positive control,1.0 g/kg)and Yiqi huayu granules high-dose,medium-dose and low-dose groups(2.4,1.2,0.6 g/kg),with 10 rats in each group. Nor-mal group and model group were given normal saline intragastrically,and treatment groups were given relevant medicine,once a day,for a week. 1 h after last medication,those groups were given adrenaline intraperitoneally+aqua astricta. to induce acute blood stasis model except for normal group. The whole blood viscosity(high-shearing,middle-shearing,low-shearing),plasma viscosity, 1,3,5 min platelet aggregation rate ,hematocrit and maximal platelet aggregation rate were all detected in rats. RESULTS:Com-pared with normal group,stomach residue rate of mice in model group was increased significantly(P<0.01),while intestinal pro-pulsive rate was decreased significantly(P<0.01). The whole blood viscosity(high-shearing,middle-shearing,low-shearing),plas-ma viscosity,hematocrit,1,3,5 min platelet aggregation rate and maximal platelet aggregation rate of rats were all increased sig-nificantly in model group (P<0.01). Compared with model group,above indexes of mice/rats were all improved significantly in treatment groups(P<0.05 or P<0.01). CONCLUSIONS:Yiqi huayu granules can significantly promote the function of gastrointes-tinal peristalsis and anticoagulation.

ABSTRACT

Objective To explore the correlation between CD62p and clopidogrel resistance in patients undergoing percutaneous coronary intervention with coronary heart disease combined with diabetes mellitus.Methods 150 patients ndergoing percutaneous coronary intervention with coronary heart disease combined with diabetes mellitus were enrolled in our hospital from August 2013 to December 2016,of all patients accepted clopidogrel treatment,the platelet aggregation rate were analyzed before and after treatment,which all patients divided into two groups as the range of decreasing platelet aggregation rate whether ≥ 10%,clopidogrel effective group (C2) and clopidogrel resistance group (C 1) were detected CD62p and platelet aggregation maximum (PAGM),the relevance of which range with platelet aggregation rate was analyzed.Results After treatment,the platelet aggregation rate of all patient decreased signifieantly (P < 0.05).Of all patients divided into clopidogrel effective group (C2) and clopidogrel resistance group (C 1) as the range of decreasing platelet aggregation rate,and the CD62p of Group C1 decreased compared with before treatment (P < 0.05),the level of CD62p and PAGM in Group C2 were decreasing compared with pre-treatment,and the range of which was larger than those Group C1 (P < 0.05).The reference of CD62p and PAGM in Group C2 (r =0.424,P =0.001) was better than that Group C1 (r =0.387,P =0.020).Conclusions Clopidogrel for patients undergoing percutaneous coronary intervention with coronary heart disease combined with diabetes mellitus possessed inhibition for platelet activity,and clopidogrel resistance might be associated with platelet activity.

ABSTRACT

Objective: To evaluate the clinical efficacy and safety of ticagrelor in the patients with acute coronary syndrome ( ACS) . Methods:A retrospective study was applied to investigate the ACS patients treated with ticagrelor in our hospital from July to December in 2015. The basic information of patients, drug administration, platelet aggregation induced by ADP, major adverse cardio-vascular events ( cardiac death,nonfatal myocardial infarction,target vessel revascularization and stent thrombosis) , and adverse drug reactions ( ADR) were recorded. The incidence of end point events was calculated and the change of platelet aggregation induced by ADP before and after the drug administration was analyzed by SPSS statistical software. Results:A total of 161 patients were collected. The incidence of cardiovascular adverse events was 1. 2%, while the incidence of adverse drug reactions was 30. 4% including bleeding (15. 5%) without severe bleeding events and dyspnea (10. 6%) with 3 severe ones. The platelet aggregation rate before and after the ticagrelor treatment respectively was (54. 96 ± 14. 654)%and(24. 37 ± 13. 183)% in 122 patients with low reaction to clopidogrel( P<0. 01). Conclusion:Ticagrelor at the recommended dose can further reduce the platelet aggregation induced by ADP. In spite of high incidence of ADR, ticagrelor has slight ADR with good tolerance.

ABSTRACT

Objective To investigate curative efficacy of ginkgo biloba extract dripping pills in treatment of acute coronary syndrome(ACS) after percutaneous coronary intervention(PCI) and its effects on platelet aggregation rate(PAR), activated clotting time(ACT) and antithrombin(AT)Ⅲ. Methods 90 patients of ACS treated with PCI who received therapy from January 2014 to October 2016 in Zhejiang green town cardiovascular hospital were selected and randomly divided into the observation group and the control group , 45 cases in each group.The control group was treated with routine treatment after PCI, while the observation group was combined with ginkgo biloba extract dripping pills.After treatment of seven days, the changed of PAR, ACT, ATⅢ and adverse cardiovascular events were compared, after treatment three months, the seattle angina scale were compared. Results After treatment, the levels of PAR in the observation group were significantly lower than that of the control group, and the levels of ACT and ATⅢ were significantly higher than that of the control group, the difference was statistically significant (P<0.05), the total incidence of adverse cardiovascular events in the observation group was significantly lower than that of the control group , the difference was statistically significant ( P<0.05), in the seattle angina scale, the scores of stable state of angina pectoris, the attack of angina pectoris, physical activity limitation, treatment satisfaction in the observation group were significantly better than that of the control group, the difference was statistically significant (P<0.05). Conclusion Ginkgo biloba extract dripping pill is well for ACS after PCI, which can effectively relieve clinical symptoms, to improve the expression of PAR, ACT and ATⅢ, helps to reduce the incidence of adverse cardiovascular events.

ABSTRACT

Objective:To investigate the relationship of CYP2C19 gene polymorphisms and clopidogrel resistance in the patients with coronary atherosclerotic heart disease ( CAHD) accepted percutaneous coronary intervention ( PCI) in Chinese Han population from north Sichuan. Methods:The patients with CAHD undergoing PCI were recruited. The fluorescence in situ hybridization ( FISH) technology was used to detect the CYP2C19 gene polymorphisms in all the patients. According to the different genotypes, the patients were divided into different groups. The platelet maximum aggregation rate ( MAP) induced by ADP was detected before and after the administration of clopidogrel. Results:The 110 patients were divided into three groups:52(47. 3%) patients were divided into the fast metabolism group, 42(38. 1%) patients were in the moderate metabolism group and 16(14. 6%) patients were in the slow metab-olism group. Before the administration, there was no significant difference in the basic values of the platelet aggregation rate among the groups with various metabolism type (P=0. 873). In 24 hours after the administration of 300mg clopidogrel, there was notable differ-ence in the platelet aggregation rate (17. 74% ± 5. 87% vs 21. 44% ± 8. 71% vs 27. 05% ± 8. 83%, P=0. 044) and the decrease de-gree (20. 08% ± 5. 94% vs 18. 87% ± 4. 72% vs 11. 54% ± 2. 94%, P=0. 01) among the three groups. The moderate metabolism group and slow metabolism group had higher MAP with lower decrease degree than the fast metabolism group. Totally 21 patients (19. 1%)were with CR, and among them, 2 patients (3. 8%) were in the fast metabolism group,12 patients (28. 6%) in the moder-ate metabolism group and 7 ones (43. 8%) in the slow metabolism group, and the difference of CR distribution among the three groups was significant (P<0. 001). The moderate metabolism group and slow metabolism group were more likely to have clopidogrel resist-ance, especially the slow metabolism group. Conclusion: There is a certain relationship of CYP2C19 gene polymorphisms and clopi-dogrel resistance in the patients with CAHD accepted PCI in Chinese Han population from north Sichuan, and those with slow metabo-lism genotype are more likely to have clopidogrel resistance with reduced efficacy.

ABSTRACT

Objective To evaluate the changes of platelet aggregation rate and short-term prognosis after the application of colopidgrel with booster doses or replaced by ticagrelor for ACS patients with CYP2C19 medium-metabolized genotype). Method A total of 302 patients with medium-metabolized genotype were randomly divided into colopidgrel group (75 mg/bid) and ticagrelor group (90 mg/bid). Patients in both groups accepted other conventional treatments of coronary heart disease and accompanied diseases. The platelet aggregation rates and platelet inhibition rates were observed before and after the treatment. The incidence of adverse events was followed up within 1 m. Results After one-week treatment, the platelet aggregation rates of the inducement with 5 μmol/L ADP had statistical significance between colopidgrel group and ticagrelor group ( P = 0 . 019 ) and the platelet inhibition rates had statistical significance difference between the two groups (P = 0.000). No severe adverse events occurred within one-month follow-up and 10 dyspnea patients were all in ticagrelor group. Conclusions Compared with booster doses of clopidogrel, ticagrelor presents obvious inhibitory effect on platelet of patients with medium-metabolized of ACS but it increases the incidence of dyspnea.

Objective To explore the clinical values of the serum homocysteine (Hcy) and platelet aggregation rate in type 2 dia‐betes patient with cerebral infarction .Methods Totally 65 cases of type 2 diabetes patients with cerebral infarction ,55 simple type 2 diabetes patients and 50 cases who took health examination were enrolled in the study .The serum homocysteine and platelet ag‐gregation rate were detected ,the blood pressure ,weight and height of the patients were measured .The multi‐factor Logistic regres‐sion analysis was used to detecting the risk factors in type 2 diabetes patients with cerebral infarction .Results The serum homo‐cysteine level and platelet aggregation rate were significantly higher in diabetes patients with cerebral infarction than those simple diabetes patients or healthy group (P< 0 .05) .The levels of serum homocysteine and platelet aggregation rate were positively corre‐lated with the severity of diabetes patients with cerebral infarction .Conclusion Serum homocysteine and platelet aggregation rate are much higher in type 2 diabetes patients with cerebral infarction ,which indicated that we can predict the process of the cerebro‐vascular disease through detecting the serum homocysteine and platelet aggregation rate .

Objective To explore the effects of different dose of clopidogrel on clopidogrel resistance ,platelet aggregation rate and inflammatory factor level of patients with acute coronary syndrome .Methods Totally 98 cases of acute coronary syndrome patients were randomly divided into the observation group and the control group two groups according to the order of treatment ,49 cases in each group .The two groups were treated with conventional percutaneous coronary artery intervention (PCI) ,and the con‐trol group was treated with low dose clopidogrel ,while the observation group was given high loading dose of clopidogrel .The deter‐mination of platelet aggregation rate (PA ) and inflammatory factor hypersensitive C‐reactive protein (hs‐CRP) level respectively before treatment and after treatment 2 ,6 ,24 and 48 h were observed ,and the clopidogrel resistance (CR) occurrence of two groups were assessed .Results The CR of observation group was significantly lower than the control group ,the difference statistically sig‐nificant (P 0 .05) .Conclusion High dose of clopidogrel has better curative effect in pa‐tients with acute coronary syndrome ,and can more effectively inhibit platelet aggregation ,reduce the incidence of clopidogrel resist‐ance and the levels of inflammatory factors ,fewer adverse reactions ,and worth the clinical promotion and application .

ABSTRACT

Objective To explore the clotting mechanism in beagles irradiated by 4.5 Gy γ-rays after treatment with supportive care,or supportive care and combined cytokines.Methods Sixteen beagles were divided into irradiation control group,Supportive care group and combined cytokines treatment group.Platelet aggregation test,thrombelagtography (TEG) and the time measurement were analyzed in vitro.Results In irradiation group and supportive care group,the platelet aggregation rates in beagles were decreased markedly and the k value of TEG was increased 7 d post-irradiation,while those indexes in combined cytokines treatment group changed little.At 14 d post-irradiation,each parameter of TEG in irradiated group changed obviously.The values of r,k,r+k and M were elevated significantly,clotting time and the maximum coagulation time of thrombus delayed,the Ma value was decreased markedly,and the maximum elasticity amplitude of thrombus was diminished.All parameters in combined cytokines treatment group were better than those in supportive care group.The thrombin time was prolonged obviously in irradiated group 14 d post-irradiation,while the thrombin time was the longest at 2-3 weeks post irradiation in supportive care group and combined cytokines treatment group(P>0.05).Conclusions Cytokines could improve the platelet aggregation and the blood clotting functions of beagles suffering from acute radiation sickness.

ABSTRACT

Objective To compare the efficiency and safety of clopidogrel of different loading-dose in acute coronary syndromes(ACS) patients with percutaneous coronary intervention(PCI) therapy, and, give an assessment of security about higher loading dose of clopidogrel. Methods 120 patients from January 2008 to January 2009 in our hospital were included.They were randomly divided into 3 groups(na=nb=nc=40)and were given 600 mg,450 mg and 300 mg clopidogrel on the 6 hour before PCI. There was no significant difference between the three groups' basic clinical information(p>0.05),such as age, sex and so on. Four points were selected to observe: the platelet aggregation induced by ADP with 5umol/L before and two hours、four hours, six hours after taking medicine; Primary endpoint events, which include shock during hospitalization, death, target revascularization, myocardial infarction, recurrent angina, stroke in 30 days,and Postoperative Bleeding events and adverse Reactions after Surgery in 30 days. Results compared to the standard dose of 300 mg clopidogrel, 600 mg doses clopidogrel can give a greater degree of inhibition on platelet activating than 450 mg doses in the pre-6 h after administration. Increase Clopidogrel loading dose can reduce the occurrence of major cardiovascular events rate. Bleeding events and the occurrence of adverse events was no significant difference among three groups. Conclusions compared to the standard dose of 300 mg clopidogrel, a higher loading dose of clopidogrel can produce much more faster, greater platelet inhibition, and similar security.
