ABSTRACT

This study summarizes the clinical thinking of acupuncture for snoring based on "disharmony qi leads to restlessness". According to the pathological characteristics of qi stagnation and blood stasis, phlegm dampness and internal obstruction in snoring patients, combined with the etiology, pathogenesis and location of the disease, the innovative viewpoint of "disharmony qi leads to restlessness" is proposed. It is believed that the key to snoring treatment lies in "regulating qi ". In clinical practice, acupuncture can directly regulate the qi of the disease's location, regulate the qi of the organs and viscera, and regulate the qi of the meridians to achieve overall regulation of the body's internal and external qi, smooth circulation of qi and blood, and ultimately achieve the therapeutic goal of harmonizing qi, stopping snoring, and improving sleep quality.

ABSTRACT

Objective:To explore the effect of "ten needles in the stomach" on sleep perception of patients with spleen-gastric damp-heat insomnia based on the theory of "stomach discord, then sleep restlessness", to provide references for clinical diagnosis and treatment.Methods:The study subjects were 300 patients with insomnia from the sleep department and outpatient department of Shijiazhuang Traditional Chinese Medicine Hospital. According to the random number method, they were divided into the stomach ten needle group, the diazepam group, and the stomach ten needle plus diazepam group, with 100 in each group. Among them, the stomach ten needles group received "stomach ten needles" acupuncture treatment, the diazepam group was given diazepam therapy, and the stomach ten needles plus diazepam group was given diazepam combined with "stomach ten needles" acupuncture treatment. The Pittsburgh Sleep Quality Index (PSQI) scores, the self-rating disease symptom scale (SCL-90) scores, nighttime polysomnography (PSG) monitoring indicators, and the mean sleep latency (MSL) values were compared before and after treatment in the three groups. The difference in clinical efficacy between the two groups were observed.Results:The total effective rate was 94.0% (94/100) in the combined group, 77.0% (77/100) in the diazepam group, and 85.0% (85/100) in the stomach ten needle group. There were significant differences among the three groups ( χ2=11.56, P<0.01). After treatment, the daytime dysfunction, hypnotic drugs, sleep disturbance, sleep efficiency, sleep time, sleep time, sleep quality score and total score of the combined group were significantly lower than those in the diazepam group and the stomach ten needle group ( F=5.70, 3.65, 5.23, 6.37, 3.66, 6.19, 7.54, 6.40, all Ps<0.01); The scores of SCL-90 and MSL were significantly lower than those in the diazepam group and stomach ten needle group ( F=8.97, 7.53, all Ps<0.01). In the combined group, NWAK [(1.36 ± 0.87) times vs. (2.69 ± 1.15) times, (2.11 ± 1.05) times, F=5.88], SOL [(13.62 ± 5.85) min vs. (25.06 ± 9.42) min, (19.78 ± 7.63) min, F=6.49], N1/TST [(5.69 ± 1.09)% vs. (12.65 ± 2.58)%, (8.49 ± 2.45)%, F=5.13], N2/TST [(51.07 ± 10 2.06)% vs. (58.36 ± 2.18)%, (55.61 ± 2.04)%, F=9.86] were significantly lower than those in the diazepam group and stomach ten needle group ( P<0.01), SE [(90.33 ± 2.89)% vs. (85.39 ± 2.75)%, (87.53 ± 2.69)%, F=8.36], TST [(449.32 ± 23.65) min vs. (421.66 ± 25.33) min, (431.26 ± 25.98) min, F=8.26], REM/TST [(23.87 ± 4.52)% vs. (14.52 ± 3.87)%, (18.36 ± 3.25)%, F=5.34], N3/TST [(18.69 ± 2.02)% vs. (10.31 ± 1.64)%, (14.89 ± 1.74)%, F=7.69] were significantly higher than those in the diazepam group and stomach ten needle group ( P<0.01). Conclusion:Based on the "stomach discord, sleep restlessness" theory, the ten needles on the stomach can treat insomnia and improve psychological state and sleep quality, with the functions of clearing the damp and heat of the middle joker, regulating the lifting and lowering of qi in the middle.

ABSTRACT

Attention deficit hyperactivity disorder (ADHD) is the most common neurodevelopmental disorder among children. The theory of traditional Chinese medicine (TCM) does not have a special term of this disease, and classifies ADHD to the category of " forgetfulness" " dysphoria" " injudiciousness" according to its clinical manifestations. TCM theoretic and clinical systems for treating ADHD have been developed, but current pharmacodynamics studies of TCM on ADHD have mostly focused on neurotransmitter metabolic system and its receptor signaling pathway to reflect the advantages of multi-component and multi-target effect of TCM, but failed to demonstrate the scientific connotations of TCM theory, such as " coordination between kidney and brain" and " steady Yin and vexed Yang" , and ignored the delay in cortical maturation as the core neuropathology of ADHD. The symptoms relating to " restlessness of Yang" due to Yin deficiency that cannot inhibit hyperactivity of Yang, kidney-Yin deficiency and liver-Yang excess syndrome is the most common syndrome of ADHD, and TCMs with effect of invigorating kidney and filling sea of marrow are mostly used by TCM practitioners. According to anatomy basis of neuronal development disorder, the new theory of " energy metabolism dysfunction" , the pathogenesis hypothesis of sea of marrow development disorder and its relevant clinical practices and experimental studies, substantial basis studies of the therapy for " invigorating kidney and filling sea of marrow" , as well as pharmacodynamics study of <italic>Rehmannia glutinosa</italic> (Shudihuang) for treating ADHD, we put forward ADHD pathogenesis that " restlessness of Yang due to Yin deficiency" is related to the disorder of executive function due to the delay in cortical maturation, energy metabolism dysfunction and neurodevelopmental disorder, and regarded that TCMs with effect of invigorating kidney and filling sea of marrow are the most commonly used to treat ADHD, and improve delay in cortical maturation corresponding to sea of marrow insufficiency by regulating dysfunction of neurodevelopment and energy metabolism, so as to relieve the core symptoms of ADHD. The new treatment model of TCM with effect of invigorating kidney and filling sea of marrow for treating ADHD will provide a theoretical basis for improving the clinical efficacy of ADHD.

RESUMEN El objetivo del artículo es hacer un aporte al proceso de fundamentación del concepto foucaultiano de estética de la existencia desde una perspectiva estético-antropológica, a partir de dos referentes de la tradición filosófica: l) el pensamiento de Nietzsche, específicamente su crítica de la metafísica y su ontologia estética, y 2) la filosofía greco-romana antigua del arte de vivir de las escuelas helenísticas estoica y epicúrea; en particular, su concepción de la filosofía como forma de vida y las nociones de gozo (estoica) y placer (epicúrea). Como valor agregado se derivan algunos aspectos del potencial antropológico de este concepto foucaultiano. Por último, se señala la importancia que tuvo el cinismo para el último Foucault, la relación entre los cínicos y el concepto de estética de la existencia, y se explica por qué no son incluidos en el ejercicio de fundamentación teórica del concepto.


ABSTRACT The aim of this paper and its underlying research project is to be a contribution to the grounding of the Foucaldian concept of aesthetics of existence from an aesthetic and anthropological standpoint using two referents of the philosophical tradition: l) the thoughts of Nietzsche, particularly his criticism of metaphysics and his aesthetic ontology and 2) the ancient Greek-Roman philosophy on the art of living, namely the Stoic and Epicurean Hellenistic schools, particularly their conception of philosophy as a way of life and the notions of joy (Stoics) and pleasure (Epicureans). As an added value, we derive some aspects of the anthropological potential of this Foucauldian concept. Finally we highlight the importance of cynicism during the final stages of Foucault's works, the relationship between Cynics and the concept of aesthetics of existence. We also explain why these are not included in the concept's theoretical grounds.


RESUMO O objetivo do artigo é contribuir para o processo de fundamentação do conceito foucaultiano de estética da existência a partir de uma perspectiva estético-antropológica, baseada em dois referentes da tradição filosófica: l) o pensamento de Nietzsche, especificamente sua critica da metafísica e sua ontologia estética; e 2) a antiga filosofia greco-romana da arte de viver das escolas helénicas estoicas e epicuristas, em particular, sua concepção de filosofia como um modo de vida e as noções de alegria (estóico) e prazer (epicurista). Como valor agregado, derivam-se alguns aspectos do potencial antropológico desse conceito foucaultiano. Por fim, destaca-se a importância que o cinismo teve para o último Foucault, a relação entre os cínicos e o conceito de estética da existência, e explica-se por que eles não estão incluídos no exercício do fundamento teórico do conceito.

ABSTRACT

Objective To explore the effect and safety of dexmedetomidine and parecoxib sodium alone or in double combinations in improving restlessness in recovery period in patients undergoing thoracic surgery.Methods One hundred and twenty eight patients who underwent thoracic surgery in the Central Hospital of Xinyang from September 2014 to September 2016 were selected and randomly divided into control group,dexmedetomidine group,parecoxib sodium group and combined group,with 32 cases in each group.Patients in the dexmedetomidine group were given 10 mL of saline at 0.5 h before surgery,and were given a slow injection of 0.5 μg · kg-1 of dexmedetomidine by intravenous injection at 10 minutes before the end of the operation.Patients in the parecoxib group were given 40 mg (10 mL) of parecoxib sodium 0.5 h by intravenous injection before surgery.Patients in the combined group were given 40 mg (10 mL) of parecoxib sodium 0.5 h by intravenous injection before surgery,and were given a slow injection of 0.5 μg · kg-1 of dexmedetomidine intravenously at 10 minutes before the end of the operation.Patients in the control group received intravenous injections of equal saline at 0.5 h before surgery and 10 minutes before the end of the operation.The operation time,peroperative bleeding,peroperative infusion volume,and anesthesia time were observed.Plasma tumor necrosis factor-α (TNF-t),C reactive protein (CRP) and interleukin-10 (IL-10) levels and TNF-α/IL-10 of all patients at the time of 10 min before induction of anesthesia (T0),15 min before extubation (T1),tracheal extubation time (T2),15 min after extubation (T3) were detected.At the same time,restlessness in stage of analepsia and sedation of all patients were evaluated.The adverse reactions of the four groups in the recovery stage were statistically analyzed.Results There was no significant difference in the operation time,peroperative bleeding,peroperative infusion volume and anesthesia time among the four groups(P > 0.05).The levels of TNF-α,CRP and IL-10 at T1,T2 and T3 in the four groups were significantly higher than those at T0 (P < 0.05).The levels of TNF-α,CRP,IL-10 at T2 and T3 were higher than those at T1 (P <0.05),but the levels of them at T3 were lower than those at T2 in the four groups(P <0.05).The TNF-α/IL-10 at T1,T2 and T3 in the control group was significantly higher than that at T0 (P < 0.05),and TNF-α/IL-10 at T1,T2 and T3 in the other three groups was significantly lower than that at T0 (P < 0.05).The level of TNF-α/IL-10 at T2 and T3 was higher than that at T1,at T3 it was higher than that at T2 in the control group(P < 0.05) . But there was no significant difference among the time point of T1,T2 and T3 in the other three groups(P > 0.05).Compared with the control group,TNF-α,CRP and TNF-α/IL-10 levels at T1,T2 and T3 in the other three groups were significantly lower(P < 0.05),and IL-10 levels were significantly higher(P <0.05).The levels of TNF-α,CRP and TNF-α/IL-10 at T1,T2 and T3 in the combined group were significantly lower than those in dexmedetomidine group and parecoxib sodium group(P < 0.05),while the IL-10 levels were significantly higher(P < 0.05).The restlessness rate in the dexmedetomidine group,parecoxib sodium group and combined group were significantly lower than that in the control group (P < 0.05),while the ramsay sedation was significantly higher (P < 0.05).The restlessness rate in combined group was significantly lower than that in dexmedetomidine group and parecoxib sodium group (P < 0.05).All the patients had no tachycardia,nausea,vomiting,respiratory depression and other adverse reactions.Conclusion Dexmedetomidine combined with parecoxib can reduce the restlessness rate significantly,and can produce some inhibition to the inflammatory reaction.The clinical effect of dexmedetomidine combined with parecoxib is better than dexmedetomidine and parecoxib sodium alone.

ABSTRACT

Objective To compare the effects of two kinds of lidocaine gel indwelling catheter on male patients with general anesthesia during recovery period. Methods A total of 90 cases of male patients with general anesthesia were selected and divided into experimental group and control group by random digits table method,45 cases in each group. The experimental group was given lidocaine gel injection indwelling catheter urethral surface anesthesia, the control group will lidocaine gel evenly to the catheter indwelling catheter. The incidence of catheter related bladder irritation and restlessness were observed during the recovery period of general anesthesia. Results The incidence of catheter related bladder irritation in grade 0, I, II, III during the recovery period of general anesthesia were 28, 14, 3, 0 cases in the experimental group and 7, 23, 12, 3 cases in the control group, there was significant difference between 2 groups (Z=1500, P<0.01). The incidence of restlessness in grade 0, I, II and III during the recovery period of general anesthesia were 27, 18, 0 and 0 cases, respectively in the experimental group and 7, 20, 9, and 9 cases in the control group, respectively. The difference between the 2 groups was statistically significant (Z=1435.5, P<0.01). Conclusions Lidocaine gel injection into the urethra surface anesthesia can effectively improve the comfort of male patients during the recovery period of general anesthesia.

Objective:To observe the dexmedetomidine by nasal drip on perioperative spinal surgery patients sedation and the effect of hemodynamic changes.Methods:90 ASA grade Ⅰ ~ Ⅲ,years 18 to 65,and in general anesthesia spinal surgery,and postoperative patients have to extube the endotracheal,were randomly divided into three groups:control group (group C),the dexmedetomidine 0.1 μg/kggroup (D1) and dexmedetomidine 1.5 μg/kggroup (D2),(n=30).Record into the OR (T0),before the induction (T1),1 min before intubation (T2),1 min after intubation (T3),start surgy (T4),began to stop general anesthetics (T5),1 min before extubation (T6),3 min after extubation (T7),and into the PACU (T8).The patient's heart rate (HR),mean arterial pressure (MAP),oxygen saturation (SPO2),finish operation from the general anesthetics toextube the endotracheal tube,stay time,patients in the postoperative recovery room Ramsay sedation scores.Results:Three groups of patients when T0 HR,MAP,SPO2,Ramsay sedation scores had no statistical difference (P≥ 0.05);Compared with group C,D1 and D2 group significantly lower each time point of HR,MAP,Ramsay sedation scores increase (P≤ 0.05),SPO2 no significant change (P≥ 0.05);Each time point compared with group D1,D2 group HR,MAP,significantly decreased,Ramsay sedation scores increase (P≤ 0.05),SPO2 no significant change (P≥ 0.05);D1 group HR,MAP,SPO2 at each time point,Ramsay sedation scores have no obvious difference (P≥ 0.05);D2 group HR,MAP,SPO2,Ramsay sedation scores significantly difference (P≥ 0.05).Group C T3,T4,T5,T6,T7 has each time point,T8 HR,MAP,up from T1,T2,Ramsay sedation scores were significantly improved (P≤ 0.05),SPO2 no significant change (P≥ 0.05).Conclusions:40 min before anesthesia induction dexmedetomidine by nasal drip can effectively inhibit intubation and extubation reaction,make the hemodynamic changes is more stable,and significantly reduce the incidence of patients with postoperative agitation.

ABSTRACT

Objective To investigate the effect of dexmedetomidine on restlessness in recovery stage in patients with limbs fracture undergoing combined intravenous inhalation anesthesia and operation.Methods Eighty patients with limbs fracture undergoing combined intravenous inhalation anesthesia and operation in our hospital were randomly divided into the control group and the dexmedetomidine group,40 cases in each group.Before operation, patients in the dexmedetomidine group were given dexmedetomidine 0.5 μg/kg intravenously in 15 min;patients in the control group were given the same volume of 0.9% saline solution.The recovery time, extubation time, the incidence of restlessness, agitation score and sedation score were compared between the two groups.Results The recovery time and extubation time had no significant difference between the two groups.The incidence of restlessness and agitation score in the dexmedetomidine group were significantly lower than those in the control group(P<0.05),while the sedation score in the dexmedetomidine group was significantly higher than that in the control group(P<0.05).Conclusion Dexmedetomidine can reduce the incidence of restlessness and the degree of restlessness in patients limbs fracture undergoing combined intravenous inhalation anesthesia and operation.

ABSTRACT

Objective To explore the effect of oropharyngeal airway on the fixation of oral trachea cannula in restless patients. Methods Toally 82 restless patents with oral trachea cannulation hospitalized in the two sections of ICU during January 2014 and August 2015 were involved in the study: those in the first section were assigned into the control group and those in the second into the experiment group. The former was treated with ordinary dental pad and the latter with oropharyngeal airway. The groups were compared in terms of effectiveness in fixation of the cannula and damage to oral mucosa. Result The rates of cannula displacement, oral mucosa damage in the experiment group were significantly smaller than those in the control group (P<0.05). Conclusion Oropharyngeal airway used in restless patients is effective in the fixation of oral trachea cannula and decrease the rate of oral mucosa damage and improve the comfort degree.

ABSTRACT

Objective To analyze the clinical effects of dexmedetomidine on postoperative agitation in children patients . Method Forty-four children undergoing surgeries in our hospital between March 2011 and March 2014 were treated with intravenous injection of 10 mL dexmedetomidine with a dose of 0.5μg/kg in 15 minutes after general anesthesia. Result The occurrence rate of postoperative restlessness was only 6.82%(3/44). Conclusion The use of dexmedetomidine combined with specific clinical nursing intervention for children undergoing surgery can effectively reduce the occurrence of postoperative restlessness , which ensures the clinical therapeutic effect and life safety.

Objective:To analyze the effect of dezocine on the agitations after remifentanil-sevoflurane anaesthesia.Methods: Choose patients received remifentanil sevoflurane anesthesia in our hospital as study object, randomly divided into observation group received Dezocine intervention and control group received saline, compared with agitation score, VAS and BCS score, CRP and inflammatory cytokines level.Results:1)observation group patients’ average score of agitation restlessness and probability after intervention were significantly lower than control group; 2)observation group patients’ BCS and VAS scores after intervention were significantly lower than control group patients; 3)CRP and inflammatory cytokines recovery period of two groups of patients have risen, but observation group were significantly better than control group patients(t=3.950,t=4.739,t=6.493;P<0.05).Conclusion: Dezocine help mitigate wake of remifentanil sevoflurane agitation in patients, reduce pain experience while optimizing factor levels in vivo, with a positive clinical significance.

Objective To investigate the different doses of dexmedetomidine on reducing the effects of restlessness after anesthesia.Methods Forty-eight patients who occurred restlessness after general anesthesia were collected and randomly divided into A,B,C group (16 cases in each group).Dexmedetomidine were given at dose of 0.3 μg/kg,0.5 μg/kg,1.0 μg/kg for treatment as A,B,C group.Blood pressure,heart rate,Riker sedation-restlessness (SAS) score and other changes of patients were recorded at different time points after treatment.Results SAS scores were significantly lower in B,C groups than that in A group (P < 0.05) at immediately after administration and 5 min and 10 min after administration.Systolic pressure and heart rate were significantly lower in C group than in B,A group (P < 0.05) at immediately after administration and 5 min after administration.Only the heart rate in B group at immediately after administration was lower than that in A group.Time in the recovery room in B group was (37.5 ± 6.4) min,significantly lower than A group and C group ((43.2 ± 8.9) min,(47.5 ± 9.8) min ; F =14.362 ; P < 0.001).Conclusion Dexmedetomidine is a more ideal sedation drug and 0.5 μg/kg dexmedetomidine can reduce restlessness and cardiovascular reactions in patients with restlessness after general anesthesia,which is the best recommending therapeutic dose.

ABSTRACT

Objective To observe the effect of treating generalized anxiety disorder with clearing heart and relieving rstlessness acupuncture combined with western medicine. Methods 87 patients with generalized anxiety disorder were randomly recruited into a control group and a treatment group. Both groups received conventional western medical treatment. On this basis, the control group was given conventional acupuncture therapy, while the treatment was given clearing heart and relieving restlessness acupuncture. The efficacies were evaluated with Hamailton anxiety (HAMA)scale and clinical global impression scale (CGI). The adverse reaction was evaluated with treatment emergent symptoms scale (TESS). Results One week after treatment, the efficacies and adverse reaction in the treatment group markedly decreased comparing with those before the treatment(P<0.05). Two weeks after treatment, the efficacies and adverse reaction of both groups decreased comparing with those before the treatment(P<0.01). Besides, the decrease continues with time passing by. One week after treatment, the HAMA score in the control group markedly decreased comparing with that of the control group (P<0.05), and the CGI score in the treatment group had no significant difference comparing with that of the control group(P<0.05). Conclusion The treatment of clearing heart and relieving restlessness acupuncture on generalized anxiety disorder has rapid effect and good compliance.

ABSTRACT

We report a case of atopic dermatitis that was successfully treated with daiseiryuto. The patient was a 31-year-old Japanese female, who had atopic dermatitis and bronchial asthma since childhood. Recently, her itching, as well as her bronchial asthma, had simultaneously worsened. We diagnosed the patient with “exterior excess” (hyoujitsu), because she complained of anhidrosis. She also complained of being thirsty, suggestive of “interior heat” (rinetsu). Furthermore, her severe itching reminded us of “restlessness” (hansou). According to these Kampo observations, we prescribed daiseiryuto. After several weeks of taking the medication, the patient's asthma attacks had disappeared completely. The severe itching had also improved within a couple of months, and the skin eruptions disappeared in about 6 months. We think that daiseiryuto is a useful formulation for itching of the skin, and that it could be applied to many skin disorders besides atopic dermatitis, because severe itching of the skin can be interpreted as “restlessness” (hansou) in many cases. Daiseiryuto is safe for long-term use by comparatively young, strong patients, but a precaution must be taken in prescribing it to patients with a past history of ischemic heart disease, because daiseiryuto has a high Ephedrae herba content.

Objective To investigate the risk factors and effective nursing for patients with benign prostatic hyperplasia(BPH)within 3 days after operation.Methods The risk factors for restlessness of patients with BPH in 2006 were analyzed and the nursing points were summarized.Results Age of patients,surgical style and sleeping time before the operation were evidently related to the incidence of restlessness.Conclusion Strengthening of psychological nursing,appropriate surgical style and insurance of abundant sleeping time before operation could reduce the incidence of restlessness of patients after operation.

ABSTRACT

Shishi-hakuhi-to was prescribed for four patients with panic disorder and sleep disturbance who requested the extract. As indications for the usage of this formulation, discomfort and restlessness of the chest for which Gardeniae Fructus has proved efficacious against, were used. Symptoms such as palpitation, chest discomfort, hot flash, anxiety and insomnia improved promptly with Shishi-hakuhi-to administration. The four patients had several common points, that is, menopause, hot flash and dysesthesia, but no agoraphobia. So far, Shishi-hakuhi-to had been used for liver dysfunction and dermatological diseases. Our results suggest that Shishi-hakuhi-to might be useful for cases with psychiatric diseases such as panic disorder.

ABSTRACT

This paper, however, deals with a set of symptoms observed in four of the 52 patients-symptoms which we feel are apart from those in the thyroxine release syndrome. The uniformity of the signs and symptoms presented by the four patients, the consistency in the time of appearance and the correspondence of the biochemical observations in all the patients excited our curiosity. It is possible that the four cases are those of a delayed, severe or atypical thyroxine release syndrome. They seem more likely to be what many would call cases of thyroid storm or thyroid crisis a vague and ill defined syndrome which has defied understanding. (Author)
