ABSTRACT

@#AIM: To investigate the efficacy of selective laser trabeculoplasty(SLT)for lowering intraocular pressure(IOP)in patients with emulsified- silicone oil related secondary open angle glaucoma(SO-OAG).<p>METHODS: By a retrospective case study, consecutive case series of 120 eyes with sustained elevation of IOP after SO-OAG removal were treated by 360° SLT treatment. The mean IOP, mean number of glaucoma medications, and success rates at baseline 1wk, 1mo, 3mo were evaluated.<p>RESULTS: After the SLT treatment, the mean baseline IOP was significantly decreased from 25.02±5.93mmHg to 17.64±4.52mmHg at 1wk(<i>P</i>=0.002), 18.96±3.33mmHg at 1mo(<i>P</i>=0.04), and 18.03±5.69mmHg at 3mo(<i>P</i>=0.03). The mean number of glaucoma medications significantly decreased from 3.11±0.62 to 1.78±0.14 at 1wk(<i>P</i>=0.006), 1.95±0.56 at 1mo(<i>P</i>=0.04), and 1.87±0.53 at 3mo(<i>P</i>=0.02). The complete and qualified success rate were 68.3%(82 eyes)and(20.0%)(24 eyes)at 1wk, 61.7%(74 eyes)and 17.5%(21 eyes)at 1mo, and 64.2%(77 eyes)and 12.5%(15 eyes)at 3mo.<p>CONCLUSION: SLT may be effective for lowering IOP in patients with SO-OAG which was not controlled with maximum antiglaucomatous medical treatment.

ABSTRACT

We report a case of dilated episcleral vein with secondary open angle glaucoma. A 65-year-old female presented with redness of both the eyes without any prior systemic history. Her ocular examination revealed dilated episcleral veins and a high intraocular pressure (IOP) of 38 mm Hg in the right eye. Systemic examination was negative for carotid cavernous fistula, low-grade dural arteriovenous fistula, dysthyroid ophthalmopathy, and primary pulmonary hypertension. During follow-up, her IOP remained in high thirties despite maximum medications. She underwent right eye trabeculectomy with partial thickness sclerectomy with sclerotomy. In the beginning, the sclerotomy incision was not deepened into the suprachoroidal space. She developed choroidal effusion during surgery and the sclerotomy was deepened into suprachoroidal space and straw color fluid was drained. Postoperatively, she developed choroidal effusion, which resolved with conservative management. This case highlights the importance of sclerotomy in such cases of high episcleral venous pressure.

ABSTRACT

PURPOSE: Transforming growth factor-beta2 is known to be present at elevated levels in the aqueous humor of patients with primary open angle glaucoma (POAG) and diabetes but not in uveitis-related secondary glaucoma. We investigated total TGF-beta2 levels and levels of the active form of TGF-beta2 in the aqueous humor of eyes with different types of glaucoma. METHODS: The concentration of the total and active form of TGF-beta2 was measured in 63 patients with primary open angle glaucoma, neovascular glaucoma complicated with diabetes (NVG), and secondary open angle glaucoma complicated with uveitis (SOAG) using a double antibody 'sandwich-indirect' ELISA method. RESULTS: The levels of total TGF-beta2 in the aqueous samples of POAG, NVG, and SOAG were elevated. The levels of active TGF-beta2 in the aqueous samples of POAG, and NVG were also elevated, whereas the level of active TGF-beta2 was within the normal range in the aqueous samples of SOAG. CONCLUSIONS: These results suggest that the level of TGF-beta2 may play a role in the pathology of various types of glaucoma.
