ABSTRACT
The Brazilian policy to combat sexual violence emphasizes the importance of protecting victims, as well as accounting for and taking care of the perpetrator. The aim of this paper is to describe one strategy that is being used to address difficult issues - such as the processes of offenders own victimization itself, the expression of sexuality and sexual desire for children - in the context of psychosocial interventions for adult sexual offenders against children and adolescents. The strategies adopted are based on psychodrama techniques, which facilitate the overcoming of emotional obstacles to these issues, creating a playful environment during the intervention, providing conditions for reflection, expression of feelings, decreased tension, and which contribute to integration and group engagement in the tasks. This option avoids a confrontational position and favours the development of empathy.
A política de enfrentamento da violência sexual ressalta a proteção das vítimas e a responsabilização e atendimento ao autor da violência. O objetivo deste texto é descrever estratégias para abordar temas difíceis - como a própria vitimização, a expressão da sexualidade e o desejo sexual por crianças -, em uma intervenção psicossocial para adultos autores de ofensa sexual contra crianças e adolescentes. As estratégias adotadas baseiam-se em técnicas do Psicodrama, pois facilitam a superação dos obstáculos emocionais diante destes conteúdos, criando-se um ambiente lúdico durante a intervenção, proporcionando condições para reflexão, expressão dos sentimentos, diminuição da tensão, além de contribuir para integração e engajamento grupal nas tarefas. Esta opção evita uma posição de confrontação e favorece a presença de empatia.

El propósito de este trabajo es describir estrategias para abordar temas difíciles - como la victimización en sí, la expresión de la sexualidad y el deseo sexual por los niños -, en una intervención psicosocial para autores adultos de delitos sexuales contra niños y adolescentes. Las estrategias adoptadas se basan en técnicas de psicodrama, ya que facilita la superación de los obstáculos emocionales frente a estos contenidos, creando un ambiente lúdico durante la intervención, proporcionando condiciones para la reflexión, expresión de sentimientos, reducción de la tensión, además de contribuir a la integración y participación grupal en las tareas. Esta opción evita una posición de confrontación y favorece la presencia de empatía.

Apresentamos o caso de um homem que cometeu estupro de menino de cinco anos de idade. A perícia psiquiátrica concluiu que o mesmo apresentava retardo mental moderado, sendo inimputável. Atualmente ele cumpre medida de segurança em Hospital de Custódia e Tratamento Psiquiátrico do Rio de Janeiro. São discutidos fatores motivadores desse comportamento sexual, bem como as questões subjetivas do paciente que contribuíram para esse comportamento.
We present the case of a man who committed rape of a five-year-old boy. The psychiatric expert investigation concluded that he has moderate mental retardation, being not guilty by reason of insanity. He is currently committed into a forensic hospital in Rio de Janeiro. Motivating factors as well as the patient's subjective issues that may have contributed to this sexual behavior are discussed.
Nous présentons le cas d'un homme qui a commis le viol d'un garçon de cinq ans. L'expertise psychiatrique a conclu qu'il présente une arriération mentale modérée, n'étant donc pas criminellement responsable dû à son handicap mental. Il est actuellement interné dans un hôpital de détention préventive et de soins psychiatriques à Rio de Janeiro. Nous discutons les facteurs motivants ainsi que les problèmes subjectifs du patient qui peuvent avoir contribué à ce comportement.
Presentamos el caso de un hombre que abusó sexualmente de un niño de 5 años. El peritaje psiquiátrico del agresor concluyó que, el mismo, presentaba retraso mental moderado, y se le consideró inimputable. Actualmente, cumple la medida de seguridad en el Hospital de Custodia y Tratamiento Psiquiátrico de Río de Janeiro. Se discuten los factores motivadores, así como las cuestiones subjetivas del paciente, cuestiones tales que contribuyeron con este comportamiento sexual.
Dieser Artikel diskutiert den Fall eines Mannes, der einen 5-jährigen Jungen vergewaltigte. Das psychiatrische Gutachten ergab, dass dieser an einer mäßigen geistigen Behinderung leidet und deshalb schuldunfähig ist. Er ist derzeit in Rio de Janeiros Anstalt für Sicherungsverwahrung und psychiatrischer Pflege interniert. Auslösende Faktoren sowie die subjektiven Probleme des Patienten, die möglicherweise zu diesem sexuellen Verhalten beigetragen haben werden analysiert.
ABSTRACT
Background: Rape is one of the silent brutal sexual offences in Bangladesh. Despite strong laws against it, the evil of rape continues to rise. Increasing trend of the silent cruel sexual offence (rape) represents a major psychopath sexual disorder and public health problem and progress of the country. Objective: The aim of the study was to determine the pattern of alleged rape victims in a rural district of Bangladesh with the ultimate aim to create public awareness about the brutal crime. Materials and method: This retrospective study was carried out on 330 sexually assailed alleged rape victims’ report forms, who reported at Faridpur Medical College, Bangladesh from 2007 to 2011 for medical examination. Results: Among the study subjects maximum number (70.0%) of alleged rape cases were under the age of 20 years. More than two-thirds (64.60%) of the assailants were known to the victims, most of the incidents (64.20%) occurred in the victims’ houses and nearby places. The study also revealed that minimum number of victims (14.20%) reported within 24 hours for medical examination. Almost one fourth of the alleged rape cases were gang rape and no positive finding in favour of sexual intercourse was found in about three fourth (72.40%) of cases. Conclusion: Public awareness about rape would be effective to report in due time with preserving the evidence of crime and modern techniques like DNA diagnosis may be of help to detect the assailant.

CONTEXTO: A violência sexual é um grave problema de saúde pública que preocupa e confronta nossa sociedade. A prevalência, a magnitude e as consequências desse problema têm merecido atenção crescente por parte de estudiosos e pesquisadores da saúde e dos direitos humanos. OBJETIVO: Realizar uma revisão bibliográfica sobre a relação entre crimes sexuais e transtornos mentais e do desenvolvimento. MÉTODOS: Foi realizada uma pesquisa bibliográfica nas bases de dados do PubMed, Scientific Eletronic Library Online (SciELO) e Lilacs, utilizando os descritores "sexual crime", "sexual offense", "mental disorder", "mental retardation", "developmental disability" e suas combinações. RESULTADOS: Os transtornos mentais e do desenvolvimento mais frequentemente relacionados à perpetração de crimes sexuais foram esquizofrenia, transtorno bipolar e retardo mental. CONCLUSÃO: São importantes a detecção e o tratamento da morbidade psiquiátrica entre ofensores sexuais nos sistemas de saúde e de justiça criminal, o que pode contribuir para menor risco de reincidência desse comportamento sexual.
BACKGROUND: Sexual violence is a serious public health problem that concerns and faces our society. The prevalence, magnitude and consequences of this problem have merited growing attention by health researchers and human rights scholars. OBJECTIVE: To conduct a review of the literature regarding the relationship between mental disorders, sexual offences and those of development. METHODS: A bibliographic research was performed in PubMed, Scientific Electronic Library Online (SciELO) and Lilacs, employing the terms "sexual crime", "sexual offence", "mental disorder", "mental retardation", "developmental disability" and its combinations. RESULTS: The mental disorders and developmental disorders more frequently related to the perpetration of sexual offences were schizophrenia, bipolar disorder and mental retardation. DISCUSSION: The detection and treatment of psychiatric morbidity among sexual offenders in health and criminal justice systems, which may contribute to a lower risk of recidivism of this sexual behaviour, is important.
