ABSTRACT

Introducción: La adaptación cultural de los instrumentos de calidad de vida y su evaluación psicométrica se deben hacer de forma rigurosa y estandarizada. Objetivo: evaluar la fiabilidad y la validez de constructo de la versión colombiana del instrumento Skindex-29©. Métodos: se incluyeron pacientes con enfermedades cutáneas inflamatorias o generalizadas; con enfermedades no inflamatorias o localizadas, e individuos sanos y se determinó la fiabilidad intraobservador e interobservador. Resultados: se encuestaron 265 individuos. El coeficiente alfa de Cronbach fue de 0,957 para el instrumento total. Los coeficientes de correlación intraclase de la consistencia intraobservador e interobservador fueron superiores al 0,9. Los pacientes con enfermedades dermatológicas generalizadas y con dermatosis inflamatorias tuvieron puntuaciones más altas que las de aquellos con lesiones cutáneas no inflamatorias o aisladas. Conclusiones: los resultados corroboran la validez de constructo del instrumento estudiado. En el futuro es necesario evaluar otras propiedades psicométricas del mismo tales como su validez de contenido y de criterio y su sensibilidad al cambio.


Background: The cultural adaptation of quality of life instruments and their psychometric evaluation should be done rigorously and according to the standards. Objective: This study assessed the reliability and construct validity of the Colombian version of the Skindex-29 instrument. Methods: Patients with inflammatory or generalized skin diseases and with non-inflammatory or localized diseases, and healthy individuals were included. We determined the intra- and inter-observer reliability. Results: We surveyed 265 individuals. Cronbach’s alpha was 0.957 for the total instrument. The intraclass correlation coefficients for intra-observer and inter-observer consistency were higher than 0.9. Patients with generalized or inflammatory diseases had higher scores than those with non-inflammatory or isolated lesions. Conclusions: The results support the construct validity of the studied instrument. In the future, it is necessary to evaluate other psychometric properties such as its content and criteria validity, and its sensitivity to change.

Conclusões: Mediante este estudo se corroboro a validade de constructo do instrumento. No futuro é necessário avaliar outras propriedades psicométricas do instrumento tais como sua validade de conteúdo, de critério e sua sensibilidade à mudança.

ABSTRACT

Se ha realizado un estudio piloto para evaluar la confiabilidad y validez de la escala Skindex-29, una auto comunicación que evalúa la calidad de vida en personas que sufren afecciones dermatológicas en la población argentina. La muestra estuvo compuesta por 150 sujetos, 100 pacientes y 50 no pacientes. Los resultados obtenidos y las comparaciones por sub-muestras resultaron prometedores, con alta confiabilidad y validez, lo que permitirá continuar trabajando con la misma escala en diversas poblaciones, para corroborar la estabilidad de la misma.


A pilot study has been performed in order to assess the validity and reliability of the Skindex-29 instrument, a self-administered report which evaluates the quality of life of argentine population with cutaneous conditions. The sample consisted of one-hundred and fifty subjects, one hundred were patients and fifty were not. Both the outcomes obtained and the subsampled comparisons turned out to be promising, with a high level of reliability and validity. This will allow to works with the same Instrument in different populations in order to confirm its stability.

BACKGROUND: Cleanroom air is extremely dry, as it is maintained within 1 % of relative humidity. Few studies have assessed the dermatologic life quality of workers in ultralow-humidity environments. Therefore, we aimed to evaluate the dermatologic life quality of cleanroom workers using the Skindex-29, compared to those of non-cleanroom workers. METHODS: Study participants were 501 cleanroom workers and 157 non-cleanroom workers from a secondary battery factory, who underwent an employee health examination at a single university hospital from September 2014 to September 2015. Results of the self-administered Skindex-29, and McMonnies questionnaire were analyzed. Other information and disease history were also collected during physician's medical examination. Descriptive and multivariate logistic regression analysis were performed. RESULTS: The Skindex-29 score was significantly higher in cleanroom workers than in non-cleanroom workers for all domains, Symptom (16.0 ± 15.9 vs. 6.3 ± 10.2, p < 0.001), Emotion (11.3 ± 17.4 vs. 2.5 ± 7.4, p < 0.001), Function (5.2 ± 11.1 vs. 1.6 ± 4.0, p < 0.001), and Overall (10.8 ± 13.4 vs. 3.5 ± 6.2, p < 0.001). The Skindex-29 score of cleanroom workers was similar to that of patients with skin diseases such as psoriasis, other dermatitis, corns, alopecia etc. Among the cleanroom workers, 37 workers had one or more skin diseases. Among the risk factors, ‘working at cleanroom’, ‘possessing skin disease’ and ‘McMonnies score’ had significant strong correlations with Skindex-29 score, meanwhile age, sex, smoking, drinking and exercise had weak correlations with it.‘Working at cleanroom’ and ‘possessing skin disease’ had highest odds ratios with overall 14.0 (C.I.: 5.9–33.1) and 13.4 (C.I.: 4.5–29.2), and the lowest odds ratios with function domain 3.5(C.I.: 1.7–7.1) and 4.5(C.I.: 2.1–9.5), respectively. The McMonnies score had the highest odds ratio with overall, 6.9(C.I.: 4.5–10.8) and lowest odd ratio with emotion domain 4.2 (C.I.: 2.7–6.4). CONCLUSIONS: Dermatologic life quality among cleanroom workers in the secondary battery factory is shown to be lower than that among non-cleanroom workers in this study. The study suggests that the Skindex-29 may provide helpful information on the dermatologic life quality of cleanroom workers. Therefore, regarding evaluation of dermatologic life quality using Skindex-29, preventive care is necessary for cleanroom workers in ultralow humidity environment.

ABSTRACT

BACKGROUND: Autoimmune blistering skin diseases such as pemphigus vulgaris, pemphigus foliaceus, bullous pemphigoid and epidermolysis bullosa acquisita substantially affect patients' daily life and psychosocial well-being. OBJECTIVE: The aim of this study was to evaluate the quality of life (QOL) in patients with autoimmune blistering diseases and to identify the factors that can influence their QOL by comparing them to healthy controls. METHODS: Forty patients with autoimmune blistering skin diseases and 40 healthy controls were interviewed using the Korean version of Skindex-29. The study assessed the clinical severity of the disease. RESULTS: The total, symptom, function, and emotion scores of Skindex-29 were significantly higher in patients with autoimmune blistering skin diseases (35.28, 40.78, 30.57, and 36.67, respectively) than in the healthy controls (6.90, 9.38, 5.47, and 6.60, respectively) (p<0.001). Higher disease severity had a negative correlation with QOL in patients with blistering skin diseases, and QOL was lower when patients had low levels of satisfaction with treatment. CONCLUSION: The results show that autoimmune blistering skin diseases can affect patients' QOL. In addition, disease severity and low satisfaction with treatment are important factors that reduce QOL. Development of new treatments should improve treatment efficacy and the QOL of patients with autoimmune blistering diseases.

ABSTRACT

PURPOSE: An observational prospective study was conducted to study the effects of hypotonic spa-water baths and narrowband ultraviolet B therapy given alone or in combination for treatment of moderate-severe psoriasis. MATERIALS AND METHODS: Two treatments were analysed: 2 weeks of balneotherapy followed by ultraviolet-B (UVB) 311-nm phototherapy (BPT) or 2 weeks of daily bath treatments of Comano water alone (BT). One hundred and eleven adult patients with moderate to severe chronic plaque psoriasis were enrolled. Quality of life (QoL) questionnaires {36-item Short Form of the Medical Outcomes Study questionnaire (SF-36) and Skindex-29} were administered at baseline and 2 months from the end of therapy. The self-administered Psoriasis Area Severity Index (SAPASI), and the General Health Questionnaire (GHQ)-12 (to assess clinical severity and psychological distress, respectively) were also recorded at the same time-periods. RESULTS: SAPASI was significantly reduced from 15.2 to 8.7 in BPT group and 11.6 to 7.8 in BT. A decrease of greater than 50% after therapy in SAPASI_50 score was reached by 42% and 37% of patients in the BPT and BT groups, respectively. At follow-up, both groups had better scores on all SF-36 scales (with statistically significant improvement in social functioning and mental health in the BPT group) and in all Skindex-29 scales. A statistically significant reduction of GHQ-12 positive cases was observed in the BPT group. CONCLUSION: Comano spa-water alone or in combination with phototherapy had beneficial therapeutic effects on patients with psoriasis. Although our observational study design prevents us from making meaningful comparisons between the 2 interventions, the combination of balneo and phototherapy seems to improve QoL and lessen clinical severity, and reduced the proportion of GHQ-12 positive cases.

BACKGROUND: Although vitiligo does not cause direct physical impairment or life-threatening conditions, it can produce significant psychosocial problems. Nevertheless, its effect on the psychosocial well-being of patients has been neglected. OBJECTIVE: This study compared quality of life (QOL) in patients with vitiligo and in healthy controls, and assessed the impact of vitiligo on the patients' QOL using Skindex-29. METHODS: This study recruited 105 vitiligo patients and 105 healthy controls, and the QOL scores of the groups were compared using Skindex-29. Statistical analyses were performed to examine the relationship between QOL and clinical characteristics. RESULTS: The total Skindex-29, emotion, and function scores, but not the symptom scores, were significantly higher in the vitiligo group than in the healthy controls. QOL, including symptom, function, and emotion scores, was significantly impaired in women, the elderly, married patients, and patients with a longer duration of disease, greater area of vitiligo, and lesions appearing on the exposed sites. But, symptom scores generally did not differ significantly. All aspects of QOL were impaired in patients with a previous treatment history, longer duration of treatment, and relatively low levels of satisfaction with treatment. CONCLUSION: Vitiligo can impair a patient's QOL. Therefore, it is important for the dermatologist to recognize and manage psychological problems to improve the QOL of vitiligo patients.

BACKGROUND: Although acne is extremely prevalent, few studies have focused on what the patient with acne vulgaris experiences. Furthermore, despite recent advances in the accurate measurement of health outcomes, few studies have discussed the influence of acne on discrete aspects of patients' experiences such as their quality of life. OBJECT: To determine the influence of acne vulgaris on the quality of life of adult dermatology outpatients, we performed cross-sectional and longitudinal questionnaire study. METHOD: Sixty patients with acne vulgaris attending appointments with their dermatologists. Findings using Skindex, a validated 29-item instrument to measure the effects of skin disease on patients' quality of life. Results are reported as 3 scale scores (functioning, emotions, and symptoms) and a composite score (average scale score). In addition, dermatologists rated the clinical severity of patients' skin disease. Higher Skindex scores indicate greater effects on quality of life. RESULTS: Patients with acne experienced functioning and emotional effects from their skin disease comparable with those of patients with psoriasis, but experienced fewer symptoms (for patients with acne and psoriasis, respectively, Skindex functioning scores of 14.9 and 22.8 [p=0.08]; emotion scores, 39.2 and 38.9 [p=0.95]; and symptoms scores, 29.5 and 42.1 [p<0.05]). Skindex scores were higher in older patients than in younger patients, and patients aged 40 years or older were less likely to report improvement in their acne after 3 months (43% vs 85%; p<0.05). Among patients reporting no improvement in their acne, older patients reported greater effects of their acne on their quality of life. Furthermore, in multivariate analyses, older adults reported more effects of acne on their quality of life than younger adults, even after controlling for sex and acne severity as judged by the dermatologist. CONCLUSIONS: Acne vulgaris significantly affects patients' quality of life. Sex, age, severity of acne and duration of acne can effect on quality of life of acne patients.

BACKGROUND: Most available dermatologic quality-of-life measures were written in English. So these must be translated for use in Korea. Our purpose was to translate and adapt culturally into Korean a skin-related quality-of-life measure Skindex-29, and to begin preliminary assessments of its reliability and validity. METHODS: Cross-cultural adaptation questionnaire studies were performed. Two hundred and sixty adult outpatients and healthy people responded to the Korean version of Skindex-29. Evaluations of the semantic equivalence of back-translated items, reliability, construct validity, and content validity of the Korean version were the main outcome measures. Comparison between the Korean version and other versions of Skindex-29 (Spanish and American) responses was also performed. RESULTS: Five problematic items required a second translation and back-translation to achieve satisfactory agreement with the original instrument. The final Korean version of Skindex-29 was internally reliable. The instrument demonstrated both construct and content validity. As hypothesized, scores for dermatologic patients were higher than those of healthy persons and scores for patients with inflammatory diseases were higher than those of persons with isolated skin lesions. Skindex scale scores of the Korean version and other versions of Skindex-29 were similar. CONCLUSIONS: We have developed a semantically equivalent translation of Skindex-29 in Korean. Our preliminary evaluation of its measurement properties suggests that it is a reliable and valid measure of the effects of skin disease on the quality of life in Korean patients.
