ABSTRACT

Abstract Introduction Ménière disease (MD) affects the inner ear, comprising the cochlea and semicircular canals. Symptoms include severe incapacitating vertigo, nausea, vomit, aural fullness, and sensorineural hearing loss - in which speech discrimination and intelligibility are impaired and can be quantified with speech audiometry. Objective To investigate the influence of the stimuli presentation level in speech audiometry and the quality of life in adults with and without a diagnosis of MD. Method Two groups were formed with nine individuals each - one with and the other without MD. The Speech Recognition Percentage Index was researched with stimuli presented above the self-reported comfort level or 5 dB below the discomfort level. Dizziness Handicap and Tinnitus Handicap Inventories were administered to individuals with tinnitus and vertigo complaints. Results Speech recognition was better in the study group with higher presentation levels, as 75% of the sample improved their performance. The presence of vertigo significantly impacted the quality of life of individuals in the study group. Conclusion Speech recognition improves with higher presentation levels. Also, MD impacts the quality of life, especially regarding limitations caused by vertigo.

RESUMO Objetivo Adaptar listas de sentenças para avaliar o reconhecimento de fala em adultos. Método Foram atualizadas 200 sentenças balanceadas foneticamente que passaram por duas etapas de revisão. Na primeira etapa, foi enviado um questionário on-line para 60 juízes analisarem as sentenças em relação aos critérios de familiaridade, significado e previsibilidade. Para análise da consistência interna do questionário foi aplicado coeficiente Alfa de Cronbach. Na segunda etapa, três juízes especialistas analisaram se as mesmas estavam de acordo com os parâmetros indicados pela literatura para a construção de sentenças e organizaram em 10 listas de 20 sentenças cada, a fim de facilitar a avaliação clínica do reconhecimento de fala. Foi realizado um estudo piloto com três indivíduos jovens e normo-ouvintes. Resultados Na primeira etapa foram analisadas as respostas de 15 juízes que preencheram todo o questionário. Verificou-se que a concordância entre os juízes foi alta para todos os critérios. Foram indicadas 71 sentenças para serem modificadas na primeira etapa, sendo a previsibilidade o critério que teve maior ocorrência de modificação. Na segunda etapa foram identificadas mais 28 sentenças passíveis de ajustes, sendo a presença de nome próprio o critério mais frequente. No estudo piloto os jovens apresentaram alto índice de reconhecimento de fala. Conclusão Concluiu-se que a maioria das modificações realizadas nas sentenças deste estudo possibilitou a criação de um material fidedigno para a prática clínica fonoaudiológica que contribuirá na padronização da avaliação da percepção da fala de indivíduos normo-ouvintes e com perda auditiva.


ABSTRACT Purpose Adapt a list of sentences for a speech intelligibility test. Methods A speech material data base consisting of 200 phonetically balanced sentences was analyzed and partially updated. In the first stage, 60 reviewers, specialists in linguistics and speech and hearing science, analyzed the sentences in relation to the parameters of familiarity, meaning and predictability using an on-line questionnaire. Cronbach's Alpha coefficient was used to analyze the internal consistency of the questionnaire. In the second stage, the reviewers analyzed whether they were in accordance with the criteria indicated by the literature for the construction of sentences. Results In the first stage, the responses of 15 reviewers who completed the entire questionnaire were analyzed. Agreement between reviewers was high for all criteria. 71 sentences were recommended for modification in the first stage, with predictability being the most indicated parameter as requiring change. In the second stage, 28 more sentences were selected for adjustment, with the presence of a proper name in the sentence being the most frequently cited criterion. Conclusion It was possible to adapt a list of sentences in order to provide speech language therapists with a free of charge speech perception protocol. It is hoped that this new test can assist in standardizing assessment for normal hearing adults and individuals with hearing loss in Brazilian Portuguese.

ABSTRACT Purpose: to investigate the auditory recognition of intermittent speech in relation to different modulation rates and ages. Methods: 20 young people, 20 middle-aged adults, and 16 older adults, all of them with auditory thresholds equal to or lower than 25 dB HL up to the frequency of 4000 Hz. The participants were submitted to intermittent speech recognition tests presented in three modulation conditions: 4 Hz, 10 Hz, and 64 Hz. The percentages of correct answers were compared between age groups and modulation rates. ANOVA and post hoc tests were conducted to investigate the modulation rate effect, and the mixed linear regression model (p < 0.001). Results: regarding the age effect, the data showed a significant difference between young people and older adults, and between middle-aged and older adults. As for the modulation rate effect, the indexes of correct answers were significantly lower at the slower rate (4 Hz) in the three age groups. Conclusion: an age effect was verified on intermittent speech recognition: older adults have greater difficulty. A modulation rate effect was also noticed in the three age groups: the higher the rate, the better the performance.


RESUMO Objetivo: investigar o reconhecimento auditivo da fala intermitente em função de diferentes taxas de modulação e em função da idade. Métodos: participaram do estudo vinte jovens, vinte adultos de meia idade, e dezesseis idosos, todos com limiares auditivos iguais ou menores que 25 dB NA até a frequência de 4000 Hz. Os participantes foram submetidos a testes de reconhecimento da fala intermitente apresentada em três condições de modulação: 4 Hz, 10 Hz e 64 Hz. Percentuais de acerto foram comparados entre grupos etários e taxas de modulação. Para a investigação do efeito da taxa de modulação foi realizada ANOVA e testes Post Hoc, enquanto para a investigação do efeito da idade, utilizou-se o modelo de regressão linear misto (p<0,001). Resultados: sobre o efeito da idade, os dados mostram diferença significante entre jovens e idosos, e entre adultos de meia idade e idosos. Sobre o efeito da taxa de modulação, os índices de acerto da taxa mais lenta (4Hz) foram significantemente menores nos três grupos etários. Conclusão: foi verificado um efeito da idade no reconhecimento da fala intermitente: idosos apresentaram maior dificuldade. Percebeu-se também um efeito de taxa de modulação nos três grupos etários: quanto maior a taxa, melhor o desempenho.

Introdução: A audiometria convencional não é suficientemente confiável para prever a compreensão de uma pessoa num ambiente ruidoso e, desta forma, inserir testes de fala no ruído na rotina clínica audiológica pode ser uma ferramenta útil para detectar possíveis problemas da função auditiva central. Objetivo: Realizar uma revisão integrativa da literatura sobre testes de fala no ruído disponíveis para uso na clínica audiológica. Método: Busca de publicações sem delimitação temporal nos bancos de dados Lilacs, PubMed, Medline, IBCS e SciELO, utilizando-se como descritores: teste de fala no ruído, percepção auditiva, testes de discriminação auditiva, distúrbios auditivos, padronização, desenvolvimento, validação, testes do limiar de recepção da fala, percepção auditiva e perda auditiva. Não houve exclusão por período de publicação. Os artigos foram pesquisados nos meses de junho e julho de 2017. Resultados: Foram localizados um total de 1200 artigos e 39 foram inseridos nesta revisão integrativa por satisfazerem os critérios de inclusão. Nos artigos selecionados, foram localizados 25 materiais diferentes que utilizaram para avaliação de fala no ruído: sílabas, palavras, sentenças, dígitos e associação de palavras e tonalidade e palavras e sentenças. Os tipos de ruídos empregados foram: espectro de fala, ruído tipo babble, ruído branco e ruído estacionário e tais materiais foram desenvolvidos para uso em adultos e/ou crianças e sujeitos com e/ou sem perda auditiva. Conclusão: Todos os autores relataram a importância de inserir testes de fala no ruído na rotina clínica, já que só a audiometria convencional não prevê a compreensão de fala em ambiente ruidoso.


Introduction: Conventional audiometry is not reliable enough to predict a person's understanding in a noisy environment, so inserting speech noise tests into the audiological clinical routine can be a useful tool for detecting possible central auditory function problems. Objective: To conduct an integrative literature review on noise speech tests available for use in the audiological clinic. Method: Search for publications without temporal delimitation in the Lilacs, PubMed, Medline, IBCS and SciELO databases, using the following keywords: Noise speech test, auditory perception, auditory discrimination tests, hearing disorders, standardization, development, validation, speech reception threshold tests, hearing perception and hearing loss. There was no exclusion by publication period. The articles were searched in June and July 2017. Results: A total of 1200 articles were found and 39 were included in this integrative review because they met the inclusion criteria. In the selected articles, 25 different materials were used to evaluate speech in noise: syllables, words, sentences, digits and association of words and tone and words and sentences. The types of noise used were: speech spectrum, babble noise, white noise and stationary noise and these materials were developed for use in adults and / or children and subjects with and / or without hearing loss. Conclusion: All authors reported the importance of inserting speech tests in noise into the clinical routine, since only conventional audiometry does not predict speech comprehension in noisy environment.


Introducción: La audiometría convencional no es confiable para predecir la comprensión de una persona en entorno ruidoso, por lo que insertar pruebas de ruido del habla en la rutina audiológica puede ser una herramienta útil para detectar posibles problemas de la función auditiva central. Objetivo: realizar una revisión integral de literatura sobre las pruebas de ruido en el habla disponibles para su uso en la clínica audiológica. Método: Búsqueda de publicaciones sin delimitación temporal en las bases de datos Lilacs, PubMed, Medline, IBCS y SciELO, utilizando las palabras clave: prueba de ruido, percepción auditiva, pruebas de discriminación auditiva, trastornos auditivos, estandarización, desarrollo, validación , pruebas de umbral de recepción del habla, percepción auditiva y pérdida auditiva. No hubo exclusión por período de publicación. Se buscaron los artículos en junio y julio de 2017. Resultados: se encontraron un total de 1200 artículos y se incluyeron 39 en esta revisión porque cumplían con los criterios de inclusión. En los artículos seleccionados, se utilizaron 25 materiales diferentes para evaluar el habla en ruido: sílabas, palabras, oraciones, dígitos y asociación de palabras y tonos y oraciones. Tipos de ruido utilizados: espectro del habla, ruido de balbuceo, ruido blanco y ruido estacionario, y estos materiales se desarrollaron para su uso en adultos y/o niños y sujetos con y/o sin pérdida auditiva. Conclusión: Todos los autores informaron la importancia de insertar pruebas de habla en ruido en la rutina clínica, ya que solo la audiometría convencional no predice la comprensión del habla en entornos ruidosos.

RESUMO Objetivo Avaliar o ganho de percepção de fala na ausência e na presença de ruído competitivo; avaliar, após três meses de uso do implante coclear, se houve correlação entre a percepção de fala nas duas condições e correlacionar o tempo de privação auditiva e o lado implantado com a percepção de fala na ausência e na presença do ruído. Métodos Participaram deste ensaio clínico 12 indivíduos com perda auditiva de severa a profunda, submetidos à cirurgia de implante coclear. A percepção de fala foi avaliada através do teste Listas de Sentenças em Português, utilizando a direcionalidade Opti Omni, em situações de ausência e presença de ruído competitivo. Resultados A percepção de fala na ausência e na presença de ruído competitivo aumentou, em comparação com a percepção de fala pré-operatória com a utilização da direcionalidade Opti Omni e estes achados não tiveram associação com o tempo de privação auditiva ou o lado implantado. Conclusão Houve ganho na percepção de fala, tanto na ausência, como na presença do ruído competitivo, com forte correlação entre essas duas situações. Além disso, não houve associação entre tempo de privação auditiva sem o uso do aparelho de amplificação sonora individual ou o lado implantado com o desempenho na percepção de fala.


ABSTRACT Purpose To evaluate speech perception improvement in the absence and presence of competing noise; to evaluate, after three months using a cochlear implant, whether there was a correlation between speech perception in these two conditions, and to correlate the time of hearing deprivation and the implanted side with speech perception in the absence and presence of noise. Methods Twelve individuals with severe-to-profound hearing loss who underwent cochlear implant surgery participated in this clinical trial. Speech perception was assessed using the Lists of Phrases in Portuguese test, using the Opti Omni directionality, in situations of absence and presence of competing noise. Results Speech perception with the use of Opti Omni directionality in the absence and presence of competing noise increased when compared with preoperative speech perception; these findings were not associated with hearing deprivation time or implanted side. Conclusion There was an improvement in speech perception both in the absence and presence of competing noise, with a strong correlation between these two situations. Besides, there was no association between time of hearing deprivation without the use of hearing aid, or the implanted side, with performance in speech perception.

ABSTRACT

Abstract Introduction Speech tests such as logoaudiometry measure the ability to perceive and recognize oral sounds. The Speech Recognition Index (SRI) is one of the speech tests adopted in clinical routine; it uses standardized live voice and recorded speeches. The live voice speech method can be influenced by intra and interspeaker variability, as well as by regionalism variability, whereas recorded tests show consistency in their presentation. Objective Analyzing results of the SRI test applied to live voiceand recorded speeches from Paraná State, in different Brazilian counties. Method The sample comprised 125 individuals, 25 fromeach county (Rio de Janeiro, Florianópolis, Porto Alegre, Salvador and Curitiba), from both sexes, in the age group 20 to 70 years; the SRI was applied in both techniques. Results The recorded speech method showed prevalence of hit improvement in Rio de Janeiro (40%), Salvador, Porto Alegre and Florianópolis (28%). Individuals from Salvador and Florianópolis subjected to the recorded speech method showed better results in the left ear. Individuals from Rio de Janeiro and Porto Alegre showed satisfactory results in both ears, whereas those from Curitiba did not show statistically significant difference between the left and the right ear. Conclusion The recorded CD application method showed prevalence of hit improvement (%) in the SRI responses in comparison to the live voice speech technique in most of the studied counties. According to the hit rate measured in the herein investigated counties, Rio de Janeiro showed the best results in the recorded speech method.

ABSTRACT

Objective@#To investigate the hearing loss and speech disorders in the elderly, to analyze the risk factors of the elderly deafness, as well as to provide reference for the clinical research of the elderly deafness.@*Methods@#From March 2016 to March 2018, 913 elderly people, who were tested for hearing and speech disorders, were examined by a unified questionnaire to investigate the demographic data of the subjects and the related factors of deafness, and the hearing and speech recognition tests were carried out. According to the hearing loss, the hearing impaired group was divided into the hearing impaired group (500, 1 000, 2 000 and 4 000 Hz, the average hearing threshold>25 dBHL) and the non hearing impaired group (the average hearing threshold of the four frequencies ≤25 dBHL), and then the single factor analysis and the unconditional Logistic regression analysis were used. Finally, the risk factors of senile deafness were analyzed.@*Results@#Of the 913 elderly subjects in the survey, 389 (42.61%, 389/913) had no hearing impaired, 345 (37.79%, 345/913) were mild hearing impaired, and 149 (16.32%, 149/913) had moderate hearing loss. Twenty-six patients were severe hearing loss (2.85%, 26/913); 4 patients had severe hearing loss (0.44%, 4/913). Among the 524 hearing-impaired elderly, there were 244 speech-recognition disorders (46.56%, 244/524), of whom 106 were mild hearing-impaired, accounting for 30.72% (106/345), 108 were moderate hearing loss, accounting for 72.48% (108/149), 26 were severe hearing loss, accounting for 100% (26/26), and 4 were the profound hearing loss, accounting for 100% (4/4). Statistical analysis showed that the age, job status, history of hypertension, history of hyperglycemia, and smoking history were independent risk factors for senile hearing loss (P<0.05).@*Conclusions@#High incidences of hearing and speech recognition obstacle are found in health examination for the elderly patients. Noise exposure, age, history of hypertension, high blood sugar, and smoking history are high-risk factors for senile deafness, therefore, prevention and rehabilitation programs are urgent to be developed.

RESUMOObjetivo: Comparar a habilidade de reconhecimento de sentenças no silêncio e no ruído em indivíduos normo-ouvintes monolíngues, falantes do português brasileiro e, bilíngues, falantes do português brasileiro e do alemão, e do português brasileiro e italiano, bem como analisar a influência do período de aquisição da segunda língua no desempenho dos bilíngues. Método Participaram da pesquisa 87 indivíduos entre a faixa etária de 18 e 55 anos de idade, normo-ouvintes, os quais foram distribuídos em: grupo controle, composto de 30 monolíngues falantes do português brasileiro; grupo estudo alemão, 31 bilíngues simultâneos falantes do português e do alemão como segunda língua; grupo estudo italiano, formado por 26 bilíngues sucessivos, falantes do português e do italiano como segunda língua. Por meio do teste Listas de Sentenças em Português Brasileiro, foram obtidos seus Limiares de Reconhecimento de Sentenças no Silêncio e no Ruído. Resultados No silêncio, não houve diferenças estatisticamente significantes de desempenho tanto quando comparados os indivíduos bilíngues e monolíngues como quando comparados os grupos bilíngues. Por sua vez, no ruído, verificou-se diferença significante entre os grupos bilíngues, em relação ao monolíngue. Entretanto, quando comparado o desempenho dos grupos bilíngues, não se observou diferença significante entre eles. Conclusão O bilinguismo influenciou positivamente o desenvolvimento de habilidades auditivas, que repercutiram em desempenhos superiores dos bilíngues no reconhecimento de fala na presença de ruído, e o período de aquisição da segunda língua não influenciou o desempenho dos bilíngues.


ABSTRACT Purpose To compare the ability to recognize sentences in silence and in noise in monolingual normal-hearing Brazilian Portuguese speakers, and bilingual speakers of Brazilian Portuguese and German, and bilingual speakers of Brazilian Portuguese and Italian, as well as to analyze the influence of age of second language acquisition on the performance of bilinguals. Methods 87 normal-hearing individuals aged between 18 and 55 years participated of this research. They were categorized into: Control Group, composed by 30 monolingual Brazilian Portuguese speakers; German Research Group, 31 simultaneous bilingual native speakers of Portuguese and speakers of German as a second language and; Italian Research Group, consisting of 26 successive bilinguals, native speakers of Portuguese and speakers of Italian as a second language. The Sentence List Test in Brazilian Portuguese was used to measure their Sentence Recognition Thresholds in Silence and Noise. Results In silence, there were no statistically significant differences in performance when comparing the bilingual to the monolingual individuals, and when comparing the bilingual speakers among themselves. On the other hand, in noise, there was a significant difference between the bilingual groups and the monolingual one. However, there were no significant differences between the bilingual groups when their performance was compared. Conclusion Bilingualism positively influenced the development of language and listening skills, which led the bilinguals to outperform in speech recognition in the presence of noise. Also, the period of a second language acquisition did not influence bilingual performance.

Abstract Introduction The P3 cognitive evoked potential is recorded when a subject correctly identifies, evaluates and processes two different auditory stimuli. Objective to evaluate the latency and amplitude of the P3 evoked potential in 26 cochlear implant users with post-lingual deafness with good or poor speech recognition scores as compared with normal hearing subjects matched for age and educational level. Methods In this prospective cohort study, auditory cortical responses were recorded from 26 post-lingual deaf adult cochlear implant users (19 with good and 7 with poor speech recognition scores) and 26 control subjects. Results There was a significant difference in the P3 latency between cochlear implant users with poor speech recognition scores (G-) and their control group (CG) (p= 0.04), and between G- and cochlear implant users with good speech discrimination (G+) (p= 0.01). We found no significant difference in the P3 latency between the CG and G+. In this study, all G- patients had deafness due to meningitis, which suggests that higher auditory function was impaired too. Conclusion Post-lingual deaf adult cochlear implant users in the G- group had prolonged P3 latencies as compared with the CG and the cochlear implant users in the G+ group. The amplitudes were similar between patients and controls. All G- subjects were deaf due to meningitis. These findings suggest that meningitis may have deleterious effects not only on the peripheral auditory system but on the central auditory processing as well.

Objectives@#To study an effects on hearing ability and speech performance of the patients with auditory neuropathy (AN) after cochlear implantation (CI).@*Methods@#Thirty-five AN patients (26 males and 9 females) after CI in our center since 2007 were chosen, including 5 postlingual patients (implanted age from 14.3 to 38.6 years old) and 30 prelingual patients (implanted age from 1.1 to 13.7 years old). Hearing sensitivity and speech performance were estimated via following methods: (1) implanted hearing thresholds by sound field audiometry; (2) speech audiometry, including monosyllable, disyllable and sentences recognition test by computer-controlled software and sentence recognition test under noise condition (signal to noise ratio=+ 10dB); (3) Mandarin Early Speech Perception test (MESP), Mandarin Pediatric Speech Intelligibility test (MPSI), MAIS, or IT-MAIS for prelingual AN implantees.@*Results@#The average implanted hearing threshold (250-4 000 Hz) was (41.1±11.7) dBHL in 22 AN implantees, while those prelingual implantees was (39.1±10.9) dBHL, and (44.9±12.3) dBHL in postlingual implantees. Speech audiometry were implemented in four of 5 postlingual AN implantees, the results showed increasing tendency in monosyllable recognition scores, dramatic individual variation in computer-controlled disyllable and sentences recognition test, and poor scores (<30%) in speech recognition test in noise condition. Four of 30 prelingual implantees reconstruct their speech recognition ability within 1-3 years after switch-on, characterized as recognition scores (>60%) in monosyllable, disyllable and sentences. The one of prelingual implantee gained recognition scores of speech in noise within 1 year after switch-on. MAIS or IT-MAIS were implemented in twenty-five prelingual implantees, the average score in these patients was 28.6±11.7.@*Conclusion@#Cochlear implant can improve the hearing thresholds of AN patients, however, the improvement in speech performance presented significant variations among the implanted individuals.

Objective@#To investigate an effect on speech recognition after bilateral cochlear implants(CI) simultaneously.@*Methods@#Nine subjects who underwent bilateral CI operation simultaneously in Peking Union Hospital in 2007 were assigned as bilateral group, another 9 subjects with unilateral CI were chosen as unilateral group according to the age, gender, duration of deafness. Hearing threshold, speech recognition of phrases, disyllabic words, single word in quiet and noise environment were calculated, respectively. Three different sound source positions were set up to simulate the three effects of binaural hearing (head shadow, binaural redundancy and binaural squelch) in noise environment. The speech signal intensity was 70 dBSPL, the signal to noise ratio (SNR) was 0 dB, + 4 dB, + 8 dB, and the speech recognition of phrases were examined in bilateral CI group. All the data was analyzed by SPSS 19.0 software.@*Results@#In quiet environment, the average aid-hearing threshold was significantly reduced as (7.2±3.0)dB in bilateral CI group compared to unilateral CI group(P<0.05). However, the speech recognition of phrases, disyllabic words, and single word in quiet and noise environment did not show significant differences between bilateral CI and unilateral CI groups (P>0.05). In the testing under noise environment, the speech recognition of bilateral CI group was significantly higher than that of unilateral CI group (P<0.05). In the estimation of noise environment (SNR=+ 8), the phrase test and speech recognition of head shadow model in bilateral groups were significantly better than that of unilateral group (P<0.01). Further, bilateral CI group showed statistical significances in binaural redundancy and binaural squelch models(P<0.01), there was statistically significant were also found difference between two groups (P<0.01); Bilateral CI group did not reveal significant difference compared with unbilateral CI group in binaural squelch model(P>0.05). Under different signal-to-noise ratio (SNR=+ 8, + 4) noise environment, all the three given models in bilateral CI group show higher signal-to-noise ratio, higher speech recognition rate (Independent-Samples t test, P<0.01). In addition, head shadow model presented negative influences on speech recognition rate compared with binaural redundancy and binaural squelch model by SNR(P<0.05). SNR influences on binaural redundancy and binaural squelch model were almost identical, F=0.371, P=0.549.@*Conclusion@#The subjects who underwent simultaneous bilateral cochlear implantation could significantly reduce aid-hearing threshold, and greatly improve the speech recognition of phrases, disyllabic words, and single word under noise environment.

RESUMO Objetivo Compor um banco de palavras dissilábicas para elaborar, realizar a validação de conteúdo, obter evidências de fidedignidade e gravar digitalmente listas de dissílabos equivalentes, para a realização do Índice Percentual de Reconhecimento de Fala (IPRF), a fim de complementar a bateria de materiais de fala disponíveis para essa avaliação. Métodos Foram selecionados vocábulos dissilábicos, paroxítonos, substantivos, os quais foram submetidos ao processo de validação de conteúdo, que abrangeu o julgamento quanto à familiaridade, adequação e reconhecimento auditivo, por juízes especialistas e não especialistas. Foram elaboradas listas de dissílabos, com 25 palavras em cada uma, a partir dos vocábulos resultantes da validação de conteúdo, e realizada a pesquisa de equivalência dessas listas, a fim de obter evidências de fidedignidade para o novo instrumento de teste proposto. Resultados A primeira versão do banco de palavras foi composta por 442 dissílabos. Destes, 198 foram considerados familiares pela maioria dos juízes, sendo que 176 foram julgados como adequados. Após o reconhecimento auditivo, foram mantidos, no banco de palavras, 172 vocábulos, distribuídos em seis listas, com 25 palavras em cada uma. Dentre estas listas, apenas uma diferiu das demais e cinco foram consideradas equivalentes, denominadas LD-A, LD-B, LD-C, LD-D e LD-E, gravadas em formato digital em Compact Disc. Conclusão Cinco listas de dissílabos elaboradas foram consideradas equivalentes, nomeadas de listas LD-A, LD-B, LD-C, LD-D e LD-E, disponibilizadas em gravação digital, com evidências satisfatórias de validade e confiabilidade, para complementar a bateria de materiais de fala disponíveis para a realização do IPRF.


ABSTRACT Purpose To compose a bank of dissyllabic words to develop equivalent disyllabic lists, perform content validation, obtain evidence of reliability and digitally record these lists to determine the Speech Recognition Percentage Index (SRPI) in order to complement the set of materials available for this evaluation. Methods We used disyllabic, paroxytone nouns, which were submitted to content validation, which included assessment of familiarity, appropriateness and auditory recognition by expert and non-expert raters. Lists of disyllabic words (with 25 words each) were developed from the words selected after content validation, and the equivalence search of these lists was carried out to collect evidence of reliability for the proposed new test instrument. Results The first version of the word bank was composed of 442 disyllables; 198 of them were considered to be familiar by most raters, and 176 were deemed as appropriate; after auditory recognition, 172 words were kept in the word bank, distributed into six lists, with 25 words in each one. Among these lists, only one differed from the others while the other five were considered to be equivalent, and were named LD-A, LD-B, LD-C, LD-D and LD-E, and recorded in digital format onto a Compact Disc. Conclusion Five lists of disyllabic words were considered as equivalent, named LD-A, LD-B, LD-C, LD-D and LD-E lists. They were digitally recorded and made available with satisfactory evidence of validity and reliability, to complement the set of available speech materials for SRPI assessment.

RESUMO Objetivo Avaliar a influência das variáveis idade, escolaridade e gênero na ocorrência de pausas após o tempo padrão no teste de identificação de sentenças dicóticas. Método A amostra foi constituída por 200 indivíduos destros distribuídos em quatro grupos segundo a faixa etária: grupo I - 13 a 19 anos de idade, grupo II - 20 a 29 anos de idade, grupo III - 30 a 39 anos de idade e o grupo IV - 40 a 49 anos de idade. Cada grupo continha 50 sujeitos (25 homens e 25 mulheres) pareados por escolaridade. Foram adotados os seguintes critérios de elegibilidade: língua materna português brasileiro, normouvinte, leitura fluente independentemente do grau de escolaridade. O teste de identificação de sentenças dicóticas foi aplicado nas etapas de integração binaural e escuta direcionada e a necessidade de pausas no teste após o tempo padrão foi anotada. Foram realizadas estatísticas descritivas e inferenciais. Resultados Para as etapas de integração binaural, houve associação positiva entre idade e ocorrência de pausas. A variável escolaridade apresentou associação negativa com a ocorrência de pausas em todas as etapas do teste. A variável gênero não apresentou nenhuma associação com a ocorrência de pausa em nenhuma das etapas do teste. Conclusão Com o aumento da idade, há um aumento na incidência de pausas na etapa de integração binaural do teste. Quanto mais anos de estudo menor a chance de o indivíduo necessitar de pausas para realizar o teste em todas as etapas de apresentação. A variável gênero não influenciou na ocorrência de pausas.


ABSTRACT Purpose To evaluate the influence of variables age, educational status and sex in the occurrence of pauses after the standard time in the dichotic sentence identification test. Methods This investigation included 200 right-handed subjects divided into four groups according to age: group I - from 13 to 19 years old, group II - from 20 to 29 years old, group III - from 30 to 39 years old and group IV - from 40 to 49 years old. Each group contained 50 subjects (25 men and 25 women) matched by educational level. The following eligibility criteria was adopted: Brazilian Portuguese mother language, listeners, and fluent readers independent of the educational level. It was applied the dichotic sentence identification test in the steps of binaural integration, directed listening, and it was noted the need for pauses in the test after the standard time. The descriptive and inferential statistics were performed. Results For the binaural integration stages there was a positive association between age and pause occurrence. The educational variable presented a negative association with the occurrence of pauses in all stages of the DSI test. The gender variable showed no association with the occurrence of pause in any of the test steps. Conclusion With increasing age, there is an increase in the incidence of pauses in the binaural integration stages of the test. The more years of study the less chance that the individual will need breaks to perform the test at all stages of presentation. The variable gender did not influence the occurrence of pauses.

ABSTRACT Purposes: to design a sentence identification test with pictures for adults (Teste de Identificação de Sentenças com Figuras (TIS-F)) as an alternative for evaluation of auditory figure-background ability for verbal sounds, based on the Pediatric Speech Intelligibility Test and also for assessment of unskillful individuals who cannot accomplish other tests with higher levels of difficulty and greater demands. Methods: the Adobe Illustrator software was used and the image vectorization technique applied for figures creation. The sentences and the competitive message were audio-recorded in a sound treated room by a female announcer, using the software - REAPER - FM Digital Audio Workstation. Results: the TIS-F consisted of a 32 x 45 cm card, containing 10 figures, each one measuring 12 x 12 cm; one compact disc containing a track with the test calibration tone and seven test tracks, each one containing ten sentences and a competitive message and a specific protocol. Conclusion: the TIS-F is composed of a compact disc with dual-channel recording, with seven tracks containing ten sentences and the competitive story, one card containing ten pictures and a labeling protocol for all presentations and S/N in use, as well as the established normality values.


RESUMO Objetivos: elaborar, um teste para adultos, Teste de Identificação de Sentenças com figuras (TIS-F), o qual poderá ser uma alternativa para avaliar a habilidade figura-fundo, para som verbal, tendo como base o Teste de Logoaudiometria Pediátrica. E também como alternativa para avaliação de sujeitos inábeis para realizar testes com grau de dificuldade e exigência maiores. Métodos: para criação das figuras, utilizou-se o software Adobe Illustrator e realizou-se a técnica de vetorização de imagens. A gravação das sentenças e da mensagem competitiva foram realizadas em sala tratada acusticamente, por uma locutora do gênero feminino, utilizando-se o programa - REAPER - FM Digital áudio Workstation. Resultados: o TIS-F constou de: uma cartela de 32 x 45 cm, com dez figuras, cada uma medindo 12 x 12 cm; um compact disc com o tom de calibração e sete faixas teste, cada uma contendo dez sentenças e uma mensagem competitiva; e um protocolo específico do mesmo. Conclusão: o TIS-F é composto de um compact disc, gravado em dois canais, que apresenta sete faixas contendo dez sentenças e a história competitiva, uma cartela contendo dez figuras e um protocolo de marcação para todas as apresentações e S/R utilizados, bem como os valores de normalidades estabelecidos.

Objetivo: Verificar o desempenho dos idosos nos testes de fala considerando os aspectos: ajustes das próteses auditivas e/ou grau e configuração da perda auditiva, desempenho na triagem cognitiva e habilidades auditivas. Métodos: Participaram 36 idosos, com idade entre 60 e 87 anos, com perda auditiva neurossensorial de grau leve a moderadamente severo. Obtidos os Índices Percentuais de Reconhecimento de Sentenças no Ruído (IPRSR), através das Listas de Sentenças em Português Brasileiro, com as próteses auditivas, utilizando os seguintes ajustes: microfone omnidirecional; redutor de ruído e microfone omnidirecional; microfone direcional; redutor de ruído e microfone direcional. Consideradas: médias tritonais de 500, 1000, 2000 Hz e 3000, 4000 e 6000 Hz; resultados do Mini Exame de Estado Mental, teste dicótico de dígitos e padrão de duração. Resultados: Quando fala e ruído vieram da mesma direção (0/0º azimute), não houve predominância dos ajustes no melhor desempenho no IPRSR. Já na condição em que o ruído incidiu atrás do sujeito (0/180° azimute), o microfone direcional associado ao redutor de ruído foram imprescindíveis para o melhor desempenho. Houve correlação entre o IPRSR a 0/0º e o teste dicótico de dígitos. Conclusão: A habilidade de figura-fundo para sons verbais mostrou influenciar o desempenho comunicativo do idoso usuário de próteses auditivas, quando fala e ruído vieram da mesma.


Purpose: To verify the performance of the elderly in speech tests considering the aspects: adjustments of hearing aids and / or degree and configuration of hearing loss, performance on cognitive screening and hearing skills. Methods: Participated 36 elderly aged 60 to 87 years , with sensorineural hearing loss of mild to moderately severe. Obtained the Sentence Recognition Indexes in Noise (SRIN) through Brazilian Portuguese Sentence List, with hearing aids, using the following settings: omnidirectional microphone; noise reduction and omnidirectional microphone; directional microphone; the noise reduction and directional microphone. Considered: tritonal average 500,1000, 2000 Hz and 3000, 4000 and 6000 Hz; Mini mental state examination results, dichotic digits test and standard duration. Results: When speech and noise came from the same direction (0/0° azimuth), there was no predominance of the settings in the best performance in SRIN. In the condition that the noise focused behind the subject (0/180° azimuth), the directional microphone associated noise reduction were essential for the best performance. There was a correlation between the SRPRN 0/0° and the dichotic digits test. Conclusion: The ability of figure-ground for verbal sounds showed influence in the communicative performance of the elderly user of hearing aids, when speech and noise came from the same direction. But when speech and noise are spatially separated, the directional microphone and the associated noise reduction helped the individual to respond more efficiently to the stimuli presented.


Objetivo: Identificar el desempeño de adultos mayores en pruebas de habla llevando a cabo los aspectos: ajustes de las prótesis auditivas y/o grado y configuración de la pérdida de audición, desempeño en la tría cognitiva y habilidades auditivas. Métodos: Participaron 36 adultos mayores, con edades entre los 60 y 87 años, con pérdida auditiva neurosensorial de grado leve a moderadamente severo. Obtenidos los Índices de Porcentajes de Reconocimento de Frases con Ruido (IPRSR), por las Listas de Frases en Portugués Brasileño, con prótesis auditivas, utilizando los siguientes ajustes: micrófono omnidireccional; reductor de ruido y micrófono omnidireccional; micrófono direccional; reductor de ruido y microfone direcional. Consideradas: medias tritonales de 500, 1000, 2000 Hz y 3000, 4000 y 6000 Hz; resultados del Mini Examen de Estado Mental, test dicótico de dígitos y padrón de duración. Resultados: Cuando habla y ruido vinieron en misma dirección (0/0º acimut), no hubo predominancia de los ajustes en el mejor desempeño en el IPRSR. En la condición que el ruido incidió atrás del sujeto (0/180° acimut), el micrófono direccional asociado al reductor de ruido fue imprescindible para el mejor desempeño. Hubo correlación entre IPRSR a 0/0º y el test dicótico de dígitos. Conclusión: La habilidad de figura-fundo para sonidos verbales mostró influenciar el desempeño comunicativo de adultos mayores, usuarios de prótesis auditivas, cuando habla y ruido vinieron en misma dirección. Cuando habla y ruido están separados espacialmente, el micrófono direccional asociado al reductor de ruído auxiliaron el individuo a responder a los estímulos presentados de manera eficiente.

Objective To study the distribution of cochlear dead regions in the cochlea with sensorineural hearing loss (SNHL),and to investigate the effects of cochlear dead regions on speech recognition.Methods A total of 41 SNHL patients (81 ears) were divided into the cochlear dead region group (35 ears) and the group without cochlear dead regions (46 ears) by using threshold equalizing noise test (TEN test).Then we used speech recognition threshold (SRT) and speech discrimination score (SDS) tests to study the distribution of cochlear dead regions and to investigate the effects of cochlear dead regions on speech recognition.Results There were 41 cases (81 ears) sensorineural hearing loss patients and 43.21% (35/81) were found to have the cochlear dead regions.The cochlear dead region detection rate for patients with mild SNHL was 0(0/11);in patients with moderate SNHL,the cochlear dead region detection rate was 24.1% (7/29);in patients with severe SNHL the cochlear dead region detection rate was 66.7% (24/36);the cochlear dead regions of profound SNHL patients were 80.0% (4/5) respestively.The existence of the cochlear dead regions was significantly correlated with the degree of hearing loss (P<0.05).The proportion of high frequency cochlear dead regions (16 ears)was much higher than that of the low frequency cochlear dead regions(8 ears).There was no significant reduction of SRT and SDS between high and low cochlear dead regions groups(P>0.05).The SRT and SDS of the patients with cochlear dead regions were 61.63± 16.76 dB,86.35%±12.03%.The SRT and SDS of the patient with no cochlear dead regions were 75.54 ± 9.56 dB and 64.97%±20.84%.Theresults showed a significant (P<0.05) reduction of SRT and SDS between the patient with cochlear dead regions and the patient with no cochlear dead regions.Conclusion The greater the degree of hearing loss is,the higher possibility of the existence of cochlear dead regions there is.Cochlear dead regions are common in high frequencies than in low frequencies.The speech recognition ability can be affected.

Objective To investigate the clinical features,etiology,diagnosis and treatment of acute auditory agnosia.Methods We studied the clinical manifestation,diagnosis and treatment of acute auditory agnosia in a patient in our hospital.Results A 28 year oldyoung woman visited our department because she suffered from the tinnitus for 7 days and she could not distinguish the semantics for 1 day.There were no other abnormal symptoms in the central and peripheral nervous system on admission.Audiological testing showed normal,language testing showed that the speech discrimination score was zero.MRI showed extensive damage to temporal lope.MR spectroscopy revealed increased lactate and reduced N-acetyl aspartate.Acute auditory agnosia resulted from mitochondrial myopathy was considered.After symptomatic treatment,the symptoms were significantly improved.Molecular genetics examination showed the A3243G mtDNA mutation,further confirmed the diagnosis of mitochondrial encephalomyopathy with lactic acidosis and stroke like episodes (MELAS) syndrome.Conclusion Acute auditory agnosia and acute tinnitus can be the first symptoms in MELAS,thus,MELAS should be suspected in patients with acute auditory agnosia,acute tinnitus,sudden hearing loss in children and youth.Imaging examination plays an important role in the etiological diagnosis of acute auditory agnosia.

Objective To study the relationship between the cognitive function and speech recognition ability in young patients with OSAHS.Methods We selected 60 young male patients,according to the apnea-hypopnea index(AHI)and the severity of hypoxemia.They were divided into three subgroups on the basis of their syndrome severities:mild group (n= 19;AHI 5~15/h,85%≤minimum SaO2≤90%),moderate group (n= 20;AHI>15~30/h,80%≤minimum SaO230/h,minimum SaO2<80%).First,we used the MoCA scale for cognitive function tests and recorded the scores.Then 15 lists of sentence Mandarin Speech Test Materials(MSTMs)were utilized to test each group.A data analysis was performed using SPSS 17.0 software. Results The total MoCA scores(mild group:27.32±1.16;moderate group:25.85±1.23;severe group:24.52± 1.69;control group:28.52 ±1.16)decreased progressively as the disease severity increased,showing significant differences between the control group and the mild,moderate and severe groups of OSAHS patients (allP<0.05). When sound stimuli were presented at 22,24,and 26 dB SPL,the speech recognition rates in the patients with se-vere(35.4±22.6,56.3±23.9,75.2±16.5)lower than the other groups (mild group:38.4±23.5,58.3±25.5,79.2 ±18.5;moderate group:38.8±21.6,58.7±22.7,78.5±16.7;control group:39.4±23.5,60.3±24.3,80.2±16.4, respectively,allP<0.05).The differences in intensity of 50% recognition rate between the severe group(4.15± 0.80)and the control(3.62±0.41),mild (3.66±0.50)and moderate groups(3.72±0.55)of OSAHS patients were statistically significant(allP<0.05).Conclusion With hypoxia and disease severity increased,speech recogni-tion abilities in OSAHS patients decreased.This may be an important factor associated with cognitive assessment scale score.

RESUMO Introdução A capacidade em perceber os sons da fala possibilita ao ser humano comunicar-se de forma eficiente. Esse aspecto deve ser considerado na avaliação dos distúrbios da comunicação humana. O Hearing in Noise Test (HINT) foi adaptado para o português brasileiro e faz uso de sentenças para avaliar o reconhecimento de fala em silêncio e na presença de ruído competitivo. O teste pode ser uma ferramenta clínica importante, embora não se observe a utilização do HINT na prática clínica audiológica no Brasil. Um dos motivos dessa ausência pode estar relacionado com a falta de padronização em alguns aspectos do teste, incluindo o julgamento de respostas apresentadas durante sua aplicação. Objetivo Analisar os diferentes critérios de julgamento de respostas obtidas de indivíduos submetidos à pesquisa de limiares de reconhecimento das sentenças do HINT em português brasileiro. Método A pesquisa foi realizada com 30 adultos jovens (três grupos de 10 pessoas), entre 18 e 25 anos, de ambos os gêneros, com audição normal. Os indivíduos foram submetidos ao teste de reconhecimento de sentenças em presença de ruído competitivo com uso de sentenças do HINT, adaptado para o português brasileiro. Foram determinados limiares de reconhecimento de sentenças na presença de ruído, através da utilização de três critérios de julgamento publicados na literatura brasileira. Foi realizada uma análise de variação única para comparar as médias dos limiares entre os três grupos. A probabilidade máxima de erro para rejeitar a hipótese nula foi de 5%. Resultados As médias e os desvios padrão dos limiares de reconhecimento de sentenças, respectivamente, foram: 59,90 dB NPS ± 1,43; 59,60 dB NPS ± 0,53 e 59,95 dB NPS ± 0,6. Não houve diferença, estatisticamente significativa, entre as médias (F=0,398; p>0,05). Conclusão Independentemente do critério de julgamento utilizado, as respostas obtidas de indivíduos submetidos à pesquisa de limiares de reconhecimento de sentenças na presença de ruído, foram semelhantes.


ABSTRACT Introduction The ability to recognize the sounds of speech enables an efficient communication. This ability must always be considered when communication disorders are evaluated. In this study, sentences of the Hearing in Noise Test (HINT), originally developed in English and adapted to Brazilian Portuguese, were used to evaluate speech recognition in silence and in the presence of noise. Although this test can be an important clinical tool, it is noticed that it has not been used in audiological clinical practice in Brazil. One possible reason is the lack of standardization of some aspects of the test, including the methods adopted to analyze the patient’s answers. Purpose The aim of this study was to analyze different judgment criteria of individuals’ answers during sentence recognition thresholds measurement using the HINT in Brazilian Portuguese. Methods The study was conducted with 30 young adults (three groups of 10 people), between 18 and 25 years old, of both genders, with normal hearing. HINT sentences were adapted to Brazilian Portuguese and speech recognition thresholds were determined in the presence of noise by using three judgment criteria published in Brazilian literature. A single variation analysis was performed to compare the average threshold between the three groups. The maximum error probability to reject the null hypothesis was 5%. Results The mean and standard deviations of thresholds, respectively, were: 59.90 ± 1.43 dB SPL; 59.60 ± 0.53 dB SPL and 59.95 ± 0.6 dB SPL. There was no statistically significant difference between the means (F = 0.398; p> 0.05). Conclusion Regardless the judging criteria used, results obtained in all groups were equivalent.

ABSTRACT

RESUMO Objetivo Analisar o desempenho das crianças sem alteração do processamento auditivo central no teste de fala comprimida. Método Trata-se de um estudo do tipo descritivo, observacional, transversal. Participaram do estudo 22 crianças com idade entre 7 e 11 anos, normo-ouvintes, sem transtorno do processamento auditivo central. Para descartar esse transtorno, foram aplicados o questionário Scale of Auditory Behaviors, a avaliação simplificada do processamento auditivo e o teste dicótico de dígitos na etapa de integração binaural. Nas crianças sem alteração, foi aplicado o teste de fala comprimida. Resultados Os indivíduos apresentaram melhor desempenho na lista de monossílabos do que na de dissílabos, apesar de não haver diferença significativa. Não houve influência da ordem de apresentação das listas, das variáveis gênero e orelha no desempenho do teste. Em relação à idade, apenas na lista de dissílabos houve diferença no desempenho. Conclusão Foi possível concluir que a média de acertos das crianças no Teste de Fala Comprimida foi inferior aos achados na literatura nacional em adultos. Houve diferença no desempenho do Teste de Fala Comprimida com a idade apenas na lista de dissílabos. Não houve diferença quanto à ordem de apresentação das listas de aplicação nem em relação ao tipo de estímulo.


ABSTRACT Purpose To analyze the performance of children without alteration of central auditory processing in the Time-compressed Speech Test. Methods This is a descriptive, observational, cross-sectional study. Study participants were 22 children aged 7-11 years without central auditory processing disorders. The following instruments were used to assess whether these children presented central auditory processing disorders: Scale of Auditory Behaviors, simplified evaluation of central auditory processing, and Dichotic Test of Digits (binaural integration stage). The Time-compressed Speech Test was applied to the children without auditory changes. Results The participants presented better performance in the list of monosyllabic words than in the list of disyllabic words, but with no statistically significant difference. No influence on test performance was observed with respect to order of presentation of the lists and the variables gender and ear. Regarding age, difference in performance was observed only in the list of disyllabic words. Conclusion The mean score of children in the Time-compressed Speech Test was lower than that of adults reported in the national literature. Difference in test performance was observed only with respect to the age variable for the list of disyllabic words. No difference was observed in the order of presentation of the lists or in the type of stimulus.
