ABSTRACT
ObjectiveTo evaluate the efficacy and safety of Rongjin Tongbi decoction on sciatica caused by lumbar intervertebral disc herniation with liver and kidney deficiency type based on a randomized controlled study. MethodFrom January 2019 to July 2022， 90 patients in the department of Traditional Chinese medicine（TCM） of Beijing Jishuitan Hospital who met the inclusion criteria were selected and divided into two groups according to the random number table， with 45 patients in each group. During the study， 19 cases dropped out， with 41 cases included in the final observation group and 30 cases in the control group. The observation group was given Rongjin Tongbi decoction orally， and the control group was given Loxoprofen Sodium Tablets orally for 28 days. The differences in the visual analog scale （VAS） scores， the Japanese Orthopaedic Association （JOA） scores， activities of daily living （ADL） assessments， TCM clinical symptoms， subjective symptoms， and clinical signs scores between two groups before and after treatment were observed. Liver and kidney functions and gastrointestinal adverse reactions were detected for safety evaluation. ResultBefore treatment， there was no statistically significant difference in scores between the observation group and the control group. After treatment， the absolute values of the differences in VAS and ADL scores in the observation group were higher than those in the control group （P<0.05）. There was no statistically significant difference in the absolute value of the difference in JOA scores between two groups. The absolute value of the difference in TCM clinical symptom scores in the observation group was higher than that in the control group （P<0.01）. There was no statistically significant difference in the absolute values of the differences in subjective symptom and clinical sign scores between two groups. The levels of liver and kidney function indicators in both groups before and after the experiment were normal， and there was no significant difference in gastrointestinal reactions. ConclusionRongjin Tongbi decoction can significantly improve the symptoms of sciatica patients caused by lumbar intervertebral disc herniation with liver and kidney deficiency type. After treatment， the patients exhibited significant improvements in pain， activity and other aspects， and it is proven to be safe and reliable， which is conducive to the recovery of physical function.
ABSTRACT
Objective To evaluate the effect of Huayu-Tongbi decoction combined with sarpogrelate in the treatment of senile diabetes mellitus complicated with arteriosclerosis obliterans (ASO) of lower extremities. Methods A total of 105 ASO patients who met the inclusion criteria were divided into control group (52 cases) and observation group (53 cases) by random number table method. The control group was treated with oral sarpogrelate. The observation group was treated with Huayu-Tongbi decoction on the basis of the control group. Both groups were treated for 2 months. The level of FPG, HbA1c, TC, HDL-C and LDL-C were detected by automatic biochemical analyzer. The arteriosclerosis index (AI), ankle brachial index (ABI) and intima-media thickness (IMT) were calculated. The internal diameter of posterior tibial artery and dorsal pedal artery, peak velocity and blood flow were measured by ultrasound detector before and after treatment in two groups. The clinical effect was evaluated. Results The total effective rate was 94.3% (50/53) in the observation group and 76.9% (40/52) in the control group. There was a significant difference between the two groups (χ2=6.502, P=0.011). After treatment, the level of FPG, HbA1c, TC and LDL-C in the observation group was significantly lower than those in the control group (t were 5.201, 6.716, 5.878 and 10.293, respectively, all Ps<0.01). internal diameter of posterior tibial artery and dorsal pedal artery, peak velocity and blood flow in the observation group were significantly larger than those in the control group (t were 4.295, 6.639, 8.310, 5.045, 5.393 and 9.672, respectively, all Ps<0.01). After treatment, the AI (4.23 ± 0.65 vs. 4.81 ± 0.83, t=3.991), IMT (0.95 ± 0.10 mm vs. 1.11 ± 0.13 mm, t=4.829) in the observation group were significantly lower than those in the control group ( P<0.01), while the ABI (1.10 ± 0.19 vs. 0.93 ± 0.17, t=7.077) was significantly higher than those in the control group (P<0.01). Conclusions The Huayu-Tongbi decoction combined with sarpogrelate can regulate the disorder of glucose and lipid metabolism in elderly diabetic patients with ASO, improve the blood circulation of posterior tibial artery and dorsal pedal artery, in order to improve the clinical effect.

Objective To observe the effect of Tongbi decoctionon for the changes of the endothelial function and hemorheology in patients with coronary heart disease. Methods A total of 90 patients with angina pectoris were divided into the observation group and the control group according to the random number table method, with 45 in each group. The control group was given routine clinical treatment, and the observation group was treated with Tongbi decoction on the basis of the control group. After treated for 4 weeks, the pain severity of angina pectoris was observed before and after treatment, and the frequency and duration of angina pectoris were recorded. The levels of serum vascular endothelin (ET) and endothelin (VEGF) were measured by ELISA. The serum NO was measured by nitrate reductase method, and whole blood viscosity and plasma viscosity were measured by blood viscometer. The hematocrit was measured by hematocrit analyzer and optical density method. The erythrocyte aggregation index was measured, and the fibrinogen level was measured by heating precipitation method. And the clinical efficacy was evaluated. Results After treatment, the total effective rate was 93.3％ (42/45) in the observation group and 77.8％ (35/45) in the control group, and there was significant difference between the two groups (Z=-3.164, P=0.002). After treatment, the score of angina pain (0.62 ± 0.05 vs. 1.17 ± 0.14, t=5.733) , the frequency of angina attack (3.62 ± 0.43 times/week vs. 5.02 ± 0.64 times/week, t=5.042), the duration of angina attack (3.17 ± 0.42 minutes/time vs. 4.06 ± 0.51 minutes/time, t=4.781) in the observation group were significantly lower than those in the control group (P<0.05). After treatment, the levels of VEGF and NO in the observation group were significantly higher than those in the control group (t value was 5.892, 5.347, all Ps<0.05), and ET was significantly lower than that in the control group (t=4.926, P=0.037). After treatment, the whole blood viscosity, plasma viscosity, hematocrit, aggregation index and fibrinogen levels in the observation group were significantly lower than those in the control group (t values were 4.832, 4.233, 5.031, 4.136, 4.531, all Ps<0.05). Conclusions The application of Tongbi decoction can improve the endothelial function of patients with angina pectoris, adjust the hemorheological indexes and relieve the symptoms related to angina pectoris.

Objective@#To investigate the therapeutic effect of Bushen-Tongbi decoction combined with acupuncture Beishu for the treatment of lumbar disc herniation.@*Methods@#According to the random table method, 80 patients who met the inclusion criteria were divided into two groups, 40 in each group. The patients in the control group were treated with diclofenac sodium, while the patients in the treatment group were treated with Bushen-Tongbi decoction combined with acupuncture Beishu on the basis of control group. Two groups were treated for 1 month. The VAS scale was used to evaluate pain degree, Modified Japanese Orthope-dic Association (M-JOA) was used to evaluate spine function, and serum levels of TNF-α, CRP and IL-1β were detected by ELISA. Adverse reactions during treatment were recorded and clinical efficacy was evaluated.@*Results@#The total effective rate of treatment group was 95.0% (38/40) and the control group was 77.5% (31/40). The difference between two groups was statistically significant (χ2=5.165, P=0.023). After treatment, the VAS scores and M-JOA scores in the treatment group were significantly lower than those in the control group (t=6.226, 10.120, P<0.01). After treatment, the serum levels of TNF-α, CRP and IL-1β in the treatment group were significantly lower than the control group (t=8.840, 5.939, 11.397, P<0.01).@*Conclusions@#The Bushen-Tongbi decoction combined with acupuncture Beishu for the treatment of lumbar disc herniation can reduce the pain and inflammation, improve spinal function.
ABSTRACT
Objective To Qingre ChuBi decoction combined with psychological intervention efficacy and safety in the treatment of patients with acute gout were analyzed. Methods In our hospital from March 2014 to November 2016 received 68 cases of acute gout patients as the research object, according to patients were randomly divided into control group and treatment group, 34 cases in the control group were treated with Etoricoxib Tablets and psychological intervention treatment, 34 cases of patients in the treatment group in the control group plus Chubi decoction for clearing. Results Two groups of patients before treatment VAS score were compared, no significant difference; The VAS score after treatment compared with before treatment were significantly decreased (P<0.05), comparison between groups had significant differences; compared with control group, VAS score improved more obviously (P<0.05), comparison between groups had significant differences. Conclusion Qingre ChuBi decoction combined with psychological intervention has a good effect on the treatment of acute gout, high safety, worthy of clinical application.

Objective Attempting to create a chemical diversity" combinatorial library consisting of active components of Chinese materia medica compounds. Methods This paper was based on the theory of "thinking and method of 'combinatorial screening' of Chinese material medica compound", using the Qingluo Tongbi Decoction (QTD) which has exact effect against rheumatoid arthritis as system. Re-combination group of the formulation was used and various industrial separation techniques, such as resin and membrane, were used to replace the diversity of combinatorial chemistry synthetic route. Different products were used to establish combinatorial library using "Building Block". Results Different contents of active components in each "Building Block" were found during the establishment of combinatorial libraries. AB-8 resin had good adsorption and desorption functions on sinomenine, and good refined effects on saponin as well. The contents of kirenol were different in each "Building Block" of different combinations. Each "Building Block" had a relative high content of total flavonoids. Conclusion Different combinations and separation techniques lead the differences in types and contents of chemical constituents. So certain pharmacodynamic experiments are needed to evaluate the preparation and to filter out the relevant active "Building Blocks".

[Objective] To evaluate the therapeutic effect of Chinese herbal medicine of Yishen Zhuanggu Tongbi Decoction (YZTD), a formula mainly composed of Radix Aconiti Lateralis Preparata, Hema Epimedii, Fructus Psoraleae,Rhizoma Chuanxiong, Radix Angelicae Sinensis, Radix Angelicae Pubescentis and Radix Dipsaci, for rheumatoid arthritis (RA). [Methods] Forty-four cases of RA were randomized into two groups: group A ( n = 24) was treated with YZTD for oral use and with the residue of YZTD for external application; group B ( n = 20) was treated with diclofenac sodium. After 8-week treatment, function of joint, adverse reactions, erythrocyte sedimentation rate (ESR), rheumatoid factor (RF), C-reactive protein (CRP) and immunoglobulins levels were observed. Continuous treatment was given for the effective cases and a follow-up was conducted for 1 - 3 years to observe its long-term effect. Objective To study the fouling mechanism during the filtration of traditional Chinese medicine decoction by inorganic ceramic membrane so as to make a valuable exploration both on membrane fouling prevention and the industrialization of membrane separation technology in the field of Chinese materia medica. Methods Micro-filtrating Qingluo Tongbi Decoction using alumina ceramic membrane, researching the applicability of the membrane with different pore sizes, observing the fouling on membrane surface by scanning electron microscopy, studying the resistance distribution of membrane in filtration process, analyzing the fouling existence shape, position, and forming rule. Results In this system, alumina membrane with the pore size of 0.2 ?m was most suitable, which had the biggest permeating flux and reservation rate of effective component with less fouling. Among the alumina membranes with three different pore sizes, auto resistance and pore block resistance made up inferior percentage in total resistance, fouling resistance distribution mainly consisted of the reversible resistance, such as concentration polarization and gel layer resistance. Conclusion Optimizing parameters of the techniques in membrane process, using suitable pretreatment method and cleaning method can reduce membrance fouling and make the memberane property recover.
