ABSTRACT

Objective To validate the Union Physio-Psycho-Social Assessment Questionnaire(UPPSAQ-70)and test its validity and reliability.Methods From April,2013 to July,2018,patients were asked to finish the computer evaluation of UPPSAQ-70 and Symptom Checklist 90(SCL-90)in Peking Union Medical College Hospital(PUMCH).Confirmatory factor analysis(CFA)was conducted on the SPSS 17.0,and the number of fixed factors was 8 factors and 3 factors.Amos 23.0 was used to verify the original 8-factor model,8-factor revision model,3-factor model,3-factor revision model,and single-factor model.Each factor of SCL-90 was used as the calibration standard to calculate the correlation coefficient between factors.The retest reliability was tested by the outpatients in PUMCH in July,2018.Results Exploratory factor analysis indicated that the 8-factor revised model included:depression,anxiety and fatigue,sleep,physical discomfort,sexual function,happiness and satisfaction,hypochondria,and social anxiety.The 3 factors revised model included that:psychological,physiological and social dimension.Confirmatory factor analysis indicated that the 8-factor modified model was superior to the 3-factor model and the single-factor model: =10 410.4,=1862,RMSEA=0.07,CFI=0.753,and NFI=0.715.With SCL-90 as the standard criteria,except the low correlation coefficient between emotional scale and depression(=0.600)and anxiety(=0.520),the correlation coefficients of other symptoms were below 0.5.The chronbach's between each factor and total score of UPPSAQ-70 was between 0.823 and 0.904,and the Chronbach's coefficient of the whole scale was between 0.954 and 0.956 after each item was deleted.The retest reliability of the scale of 32 participants Chronbach's was 0.847.Each item of the scale measured between one week was significantly correlated(<0.05). Conclusion UPPSAQ-70 is a good scale for evaluating overall health status and is especially feasible in general hospitals.

ABSTRACT

Objectives To design an objective and structured evaluation system for the clinical competence of orthopedic postgraduates in the diagnosis and treatment of distal radius fractures, and to ana-lyze its reliability and validity. Methods 28 orthopaedic postgraduates representing six levels of surgical training were tested for competence in performing surgical approach for distal radius fracture on cadaver specimens during which four measures were used to assess competency: examination of basic theory based on network item bank, objective structured operation assessment,overall assessment and operation examina-tion results. In addition, the time for completion of the surgery was also recorded. Each assessment tool was correlated with the others as well as with the resident’s level of training. Results There was a significant correlation between the seniority of candidates and the score of theoretical examination (F=6.193, P=0.000), the score of structured operation examination (F=6.374, P=0.002), the score of overall assessment (F=2.321, P=0.030), and the passing rate of final operation examination (F=36.300, P=0.000). No significant differ- ences were found between seniority and time to completion of the surgical approach exposure (F=2.282, P<0.073). Conclusions The results of the present study suggested that both theoretical examination and cadaver testing discriminate between novice and accomplished postgraduates. However, although the theo-retical test scores could predict the operational test results, but the theoretical results can not guarantee excellent operational skills.

ABSTRACT

Objective To investigate the clinical effect of synchronized nasal intermittent positive pressure ventilation (SNIPPV) in the treatment of neonatal respiratory failure. Methods The clinical data of 52 neonates with respiratory failure treated in our hospital from January 2016 to January 2018 were retrospec-tively analyzed. According to the different treatment methods,they were divided into observation group and control group,26 cases in each group. The children in the observation group were treated with SNIPPV,while those in the control group were treated with synchronized intermittent positive pressure ventilation (SIPPV). Clinical efficacy,changes of blood gas analysis results before and after treatment,oxygen consumption time and hospital stay,and complications were compared between the two groups. Results The respiratory rate and heart rate of the two groups improved after treatment,but there were no significant differences between the two groups(P>0. 05,respectively). The total effective rate of the observation group was 88. 5%,while that of the control group was 92. 3%,there was no significant difference between the two groups(χ2=0. 221, P>0. 05). The pH,PaO2,PaCO2and oxygen saturation levels of 24 h after treatment in two groups were im-proved (P<0. 05,respectively),but there were no significant differences between the two groups (P>0. 05, respectively). Compared with the control group,the oxygen consumption time and hospitalization time of the observation group were significantly shorter, and the differences were statistically significant ( P <0. 05, respectively). And the observation group had fewer complications of ventilator associated pneumonia. Conclusion The effect of SNIPPV in the treatment of neonatal respiratory failure is significant. It could shorten the time of oxygen consumption and hospitalization and reduce the complications. It is worth populari-zing SNIPPV mode assisted ventilation.

ABSTRACT

Objective To investigate the reliability and validity of the medical outcomes by applying short form-36 (SF-36) in evaluating gout patients’ health related quality of life (HRQOL).Methods Gout patientswere enrolled between March 2016 and June 2016 in Foshan Hospital of Traditional Chinese Medicine.Patients completed the SF-36 questionnaire.Summary scores,physical component summary (PCS) and mental component summary (MCS) were calculated by summing factor-weighted scores across all 8 subscales,with factor weights derived from general population.The indicators of reliability and validity included internal consistency,test-retest reliability,structural and discriminant validity,ceiling and floor effect.Results Totally 306 patients were enrolled.The internal consistency test showed that the Cronbach α coefficients ranged from 0.782 to 0.822,and the test-retest reliability coefficients ranged from 0.720 to 0.986 (P<0.01).Structural validity analysis showed that there were two items whose eigenvalues were greater than one with the cumulative contribution rate of 66.1%.The discriminant validity analysis found that patients with more numbers of tophi,higher frequency of flare,multi-arthrosis involved and more complications had less scores of PCS and MCS (P<0.05).There was a high ceiling effect on physiological function and a higher ceiling and floor effect on role limitation and emotional function caused hy impaired physical health.Conclusion The SF-36 can be used for the assessment of HRQOL in Chinese gout patients but disease specific questionnaire are warranted.

ABSTRACT

Objective To analyze reliability, validity and the correlative factors of the Chinese Paced Auditory Serial Addition Test (PASAT).Methods Fifty healthy subjects were assessed with PASAT, MMSE, Hamilton Depression Rating Scale(HAMD) ,the Fatigue Severity Scale and the Pittsburgh Sleep Quality Index( PSQI).After(7 ±2) days of the initial assessment,20 of the subjects were enrolled in the test-retest reliability and scorer reliability test,and 40 were taken into the construct validity test which included other 10 neuropsychological tests such as the Prospective Memory Test,the Chinese Rey Auditory Verbal Learning Test,the Symbol Digit Modalities Test,the Trail Making Test and so on.Results The internal consistency reliability ( cronbach' s α = 0.900 ),test-retest reliability ( ICC = 0.837, P = 0.000), score reliability ( ICC = 0.999, P = 0.000) of the Chinese PA-SAT were generally high.Practice effect were observed in short term ( Z = -3.304, P= 0.001 ).Executive function,working memory,verbal memory, time based prospective memory and the ability of logic reasoning were involved to complete the task of PASAT.There was a significant positive correlation of the PASAT results and the education years( r=0.582, P=0.000).Conclusion The Chinese PASAT has good reliability and validity for Chinese young and middle-aged people.Years of education and the Chinese PASAT were significantly related.On the other hand, the relationship of the Chinese PASAT and other factors, such as age and gender, should be further explored in future.

ABSTRACT

Background:Personality characters will influence the future career planning and development of medical students.Therefore,it will be important in the field of medical education to survey their personalities.Study design:The personality questionnaire was developed according to the Big-Five characters.The content validity and consistent reliability were assessed.A total of 124 undergraduate medical students participated in this 44-item self-evaluation about their personalities.Results:The inter-item consistency reliabilities of the 44-items of Big-Five personality questionnaire were analyzed and discordant items were deleted(Crobach's Alpha from 0.484~0.782 before selection to 0.67~0.82 after selection).The content validity of this five dimensions ranged from 0.8 to 0.9(P
