ABSTRACT

Objective To compare effects of warm water infusion and air insufflation on patient tolerance and satisfaction during the insertion phase of unsedated colonoscopy. Methods In a single-center,prospective,single blind randomized trial, patients were randomly assigned to three groups (water infusion group, air insufflation group,or hybrid method group) during the insertion phase of colonoscopy.Main outcome measurements included insertion time, cecal intubation rate, adenoma detection rate, the rate of position change, the rate of abdominal compression, intraoperative pain and distension VAS scores, postoperative pain and distension VAS scores. Results A total of 180 cases were enrolled,60 in each group.Patients in water infusion group and hybrid method group required less abdominal compression or position change, longer insertion time, reported lower intraoperative pain and distension VAS scores than air insufflation group during the insertion phase of colonoscopy. There were no significant differences in cecal intubation rates, adenoma detection rates, postoperative pain and distension or VAS scores among three groups. Conclusion Water infusion significantly decreases patient abdominal pain and distension during the insertion phase of colonoscopy without affecting cecal intubation rate and adenoma detection rate.

ABSTRACT

Background:Colonoscopy is difficult in elderly patients and is accompanied by more frequent abdominal pain and tissue injury. Aims:To investigate the impact of water infusion colonoscopy on abdominal pain and inflammatory response in elderly patients. Methods:Forty elderly patients requiring colonoscopy were enrolled from Jan. 2014 to Jun. 2016 at the Nanyang Second People’s Hospital and randomly allocated into two groups:colonoscopy with traditional air insufflation and colonoscopy with water infusion instead of air insufflation for cecal intubation. Times of cecal intubation and withdrawal were recorded;visual analogue scale( VAS)was used to assess the severity of abdominal pain;and serum levels of three inflammatory indices, tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and C-reactive protein(CRP)were determined by ELISA before and after the procedure. Results:VAS score at colonoscopy in water infusion group was significantly lower than that in air insufflation group(P﹤0. 05),however,times of cecal intubation and withdrawal were not significantly different between the two groups(P﹥0. 05). Before the procedure, serum levels of inflammatory indices were similar between air insufflation group and water infusion group(P﹥0. 05);0,6 and 24 hours after the procedure, serum levels of TNF-α,IL-6 and CRP in air insufflation group were significantly increased(P﹤0. 05)and higher than those in water infusion group at the same time points(P﹤0. 05). These inflammatory indices were recovered more rapidly in water infusion group than in air insufflation group. Conclusions:For elderly patients, colonoscopy with water infusion might ameliorate abdominal pain and inhibit systemic inflammatory response effectively.

ABSTRACT

Objective To explore the effects of warm water insufflation applied in difficult colonoscopy on its success rate, insertion time, operation difficulty level and tolerance of patients. Method 200 cases of the difficult defecation of non-narcotic colonoscopy, IBS and always has a history of pelvic/abdominal surgery patients, were randomly divided into warm water infusion group (experimental group) and traditional method of air insufflation group (control group), then compare the differences of the success rate of the two groups of patients, the insertion time and degree of tolerance in patients. Results All the patients completed the investigation in this study, neither bowel perforation nor other complications occurred. Experimental group success rate of the whole colon examination, the insertion time, the operator satisfaction and check again wishes were significantly higher than that of control group (P 0.05) in Polyp detection rate. Conclusion Warm water insufflation colonoscopy can effectively improve the tolerance and accelerating the colonoscopy in patients with difficult insertion process, improve work efficiency, and is an ideal method, which is worth of promoting.

ABSTRACT

Objective To compare the effect of water-infusion colonoscopy and conventional air-infusion colonoscopy on the bowel cleanliness during withdrawing. Methods A single blind randomized controlled trial was conducted. 222 patients were randomly divided into water-infusion group (group A, 112 cases) and air-infusion group (group B, 110 cases). The cleanliness scores, scores improvement, pain scores, time of reaching cecum, depth of colonoscopy, rate of whole colon examination, adenoma detection rate (ADR), operator’s dififculty and complications were compared between the two groups. Results The cleanliness score during withdrawing was higher in group A [M(8)/IQR(1) vs M(8)/IQR(2),P = 0.000], the improvement was higher in group A [(0.53 ± 0.74) vs (0.23 ± 0.55), P = 0.000], the abdominal pain score was signiifcantly lower in group A [M(2)/IQR(1) vs M(4)/IQR(2),P = 0.000] and the ADR was higher in group A (36.61 % vs 23.64 %,P = 0.041). The operator’s dififculty evaluation score was signiifcantly lower in group A [M(1)/IQR(1) vs M(2)/IQR(1),P = 0.005]. There were no signiifcant differences at the time of reaching cecum, the depth of colonoscopy and the whole colon examination rate between the two groups. Conclusion Water-infusion colonoscopy could signiifcantly improve the bowel cleanliness during withdrawing scope, improve the ADR and reduce abdominal pain of patients, without increasing the time to reach cecum.

ABSTRACT

Objective To explore the impact on rabbit intestinal mucosa injury and the influence on endoscopic operation with different temperatures of water infusion. Methods 30 common adult male rabbit (weight 1.6~2.5 kg), were randomly divided into three groups with different temperatures of water infusion as follows:A group (35~36℃), B group (37~38 ℃), C group (39~41 ℃), then, perform water infusion colonoscopy, evaluating the operators satisfaction, then we observe the pathological changes of postoperative intestinal mucosal tissue. Results There was no statistically significant difference among the three groups of intestinal mucosa injury ( > 0.05). The vision exposure score of Group C was lowest, compared with the other two groups, which has a statistically significant differences ( < 0.05). Conclusions Warm water infusion colonoscopy is a safe and effective method, and the optimum temperature was about 39~41℃.

ABSTRACT

ABSTRACT Camellia sinensis is widely known for its beneficial effects to human health. Among the various forms of processing this herb, the green and the black tea are the most popular ones. The objective of this work was to evaluate the contents of total polyphenol and flavonoids in samples of 25 different brands available for sale in pharmacies and supermarkets of Salvador-Ba. The results showed great variation in the concentration of phenolic compounds among the brands of green tea which were tested. In the study it was also proven that in the green tea preparation the samples obtained by mechanical agitation with aqueous infusion showed higher levels of phenolics and flavonoids contents than those which were not subjected to stirring.
