ABSTRACT

Objective:To evaluate the clinical efficacy of Qingfei-Xiaoyan Decoction combined with anti infective mechanical ventilation in the treatment of senile severe pneumonia. Methods:A total of 102 patients with severe pneumonia in our hospital from June 2018 to May 2021 who met the inclusion criteria were randomly divided into two groups by random number table method, with 51 in each group. The control group was treated with anti-infection mechanical ventilation, and the study group was treated with Qingfei-Xiaoyan Decoction on the basis of the control group. Both groups were treated for 2 weeks. TCM syndrome scores were performed before and after treatment. The severity of infection was evaluated by clinical pulmonary infection score (CPIS), FVC, FEV1 and peak expiratory flow (PEF) were detected by pulmonary function meter, and the levels of TNF-α, high mobility group box 1 (HMGB1) and CRP were detected by enzyme-linked immunosorbent assay, adverse events were recorded and clinical efficacy was evaluated. Results:The total effective rate was 94.1% (48/51) in the study group and 80.4% (41/51) in the control group, and there was significant difference between the two groups ( χ2=4.320, P=0.038). After treatment, the TCM syndrome score and CPIS score of the study group were significantly lower than those of the control group ( t values were 14.203 and 11.403, respectively, all Ps<0.01), FVC, FEV1 and PEF in the study group were significantly higher than those in the control group ( t values were 4.663, 3.451 and 13.220, respectively, all Ps<0.01), and the levels of serum CRP, TNF-α and HMGB1 in the study group were significantly lower than those in the control group ( t values were 14.672, 10.277 and 24.453, respectively, all Ps<0.01). During the treatment, no serious adverse reactions occurred in both groups. Conclusions:Qingfei-Xiaoyan Decoction combined with anti-infective mechanical ventilation has a significant effect in the treatment of senile severe pneumonia. It can alleviate clinical symptoms, improve lung function, reduce inflammatory reaction with safety.

ABSTRACT

Objective: To investigate the efficacy of Xiaoyan Decoction in the treatment of non-small cell lung cancer (NSCLC) by retrospective study. Methods: Patients with NSCLC treated in the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from January 1, 2013 to December 30, 2017 were collected. The patients were divided into two groups: Xiaoyan Decoction plus subtractive prescription combined with symptomatic treatment group (46) and symptomatic treatment group (50). Analyze the survival of the two groups of patients, compare the Karnofsky scores of the two groups, and analyze the sex, age, smoking history, drinking history, histological grade, chemotherapy regimen, pathological type, TCM syndrome type, vascular invasion, and soft tissue invasion Correlation with patient prognosis. Results: There was a significant difference in median progression-free survival (mPFS) between the two groups (P 0.05).The quality of life of patients in Xiaoyan Decoction plus subtractive prescription combined with symptomatic treatment group was significantly improved (P < 0.05), among which, lung squamous cell carcinoma, no vascular, no soft tissue invasion, and patients with phlegm heat obstruction of lung syndrome had the best efficacy (P < 0.05). Conclusion: Xiaoyan Decoction combined with symptomatic maintenance therapy can prolong median progression-free survival (mPFS) and improve the quality of life of patients with advanced NSCLC. The best beneficiaries are patients with lung squamous cell carcinoma, phlegm-heat obstructive pulmonary syndrome without vascular or soft tissue invasion.

ABSTRACT

Objective: To investigate the possible mechanism of reversal of cisplatin resistance in lung cancer by Xiaoyan Decoction. Methods: The migration ability of cancer cells was observed by the scratch test, and the MTT was used to test the mobility of cancer cells. The siRNA transfection was applied to demonstrate the possible molecular mechanisms. The mRNA and protein expression levels were detected by real-time quantitative reverse transcription PCR and Western Blot. Results: MTT showed that A549/DDP could significantly inhibit cell proliferation at later points of time. Similarly, cisplatin combined with Xiaoyan Decoction could significantly inhibit cell proliferation at 48, 72 and 96 h. Scratch test showed that the migration distance was shorter in A549/DDP/siRNA-Beclin1 cell line than NC. MTT showed the proliferation of A549/DDP/siRNA-Beclin1 cell line was decreased when compaired with NC. Western blot also showed that the protein expression of P-gp and LRP was decreased. MTT also detected that cisplatin and Xiaoyan Decoction significantly inhibited cell proliferation of A549/DDP/siRNA-Beclin1 cell at later points of time. Cisplatin combined with Xiaoyan Decoction significantly inhibited cell proliferation in the whole course. The results showed that protein expression level of YAP1, P-gp and LRP in A549/DDP/siRNA-Beclin1 cell line was significantly lower than those in NC. Conclusion: Xiaoyan Decoction may improve the sensitivity to cisplatin may via Beclin 1-YAP1 pathway.

ABSTRACT

Objective To study the effect of Xiaoyan Decoction on metastasis of non-small cell lung cancer by experimental study. Methods Through the scratch test, the effect of Xiaoyan Decoction on the migration ability of lung adenocarcinoma A549 cells was observed. The transwell migration assay was used to test the role of Xiaoyan Decoction in lung cancer stem cells migration. Results In the scratch test, the migration distance of lung adenocarcinoma A549 cells in 24, 48, and 72 h was significantly shorter than that of the control group at the time of the scratch test (P < 0.05). In the invasive experiment, the number of lung cancer stem cells passing through the transwell unit in Xiaoyan Decoction group was significantly less than that of the control group (P < 0.05). Conclusion Xiaoyan Decoction can effectively inhibit the invasion ability of lung cancer stem cells, and then affect their metastatic ability.

ABSTRACT

Objective: To investigate the specific mechanism of Xiaoyan Decoction in the metastasis of non-small cell lung cancer. Methods: The detection method of Western blotting method and quantitative PCR Xiaoyan Decoction intervention expression of many inflammatory proteins and cytokines in the lower lung cancer stem Xiaoyan Decoction explore the possible mechanism of effect of non small cell lung cancer metastasis. Results: Western blotting detection showed that the expression of IL-6, TGF-β, p-Smad3, and MMP-9 protein in both experimental group and control group decreased significantly (P < 0.05), the difference was statistically significant; RT-PCR showed that the experimental group compared with the control group IL-6, TGF-β, TNF-α, MMP-2, alpha beta MMP-9 gene expression decreased significantly (P < 0.05), the difference was statistically significant. Conclusion: Xiaoyan Decoction mainly affects the metastasis of non-small cell lung cancer by inhibiting the transmission of TGF-β/Smad3 signaling pathway in the inflammatory microenvironment of lung cancer.

ABSTRACT

Objective:To determine the effect of apatinib combined with Xiaoyan decoction for the treatment of non-squamous non-small cell lung cancer. Methods:Thirty-eight patients with non-squamous non-small cell lung cancer were randomly categorized into apatinib group (group A, 18 cases) and apatinib combined with Xiaoyan decoction group (group B, 20 cases). All patients did not under-go surgical treatment, radiotherapy, or chemotherapy during the study. Results:The median progression free survival (mPFS) of ad-vanced non-squamous non-small cell lung cancer patients reached up to 3 months. The mPFS, objective response rate, and disease control rate of the apatinib combined with Xiaoyan decoction group showed no significant difference and statistical significance (P>0.05). The apatinib combined with Xiaoyan decoction group was superior to the apatinib group with regard to alleviating clinical symp-toms and adverse reactions (P<0.05). Conclusion:Xiaoyan decoction combined with apatinib can improve the clinical symptoms of pa-tients and reduce the incidence of adverse reactions in the treatment of advanced non-squamous non-small cell lung cancer.

ABSTRACT

Objective: To analyze the elimination of Xiaoyan Decoction intervened survivin siRNA in lung adenocarcinoma A549 cells tumor-burdened mice tumor cell apoptosis. Methods: Human lung adenocarcinoma A549 entity nude mouse transplantation tumor model was studied as the research object. Xiaoyan Decoction medicated serum and siRNA cells after transfection group was injected within the tumor respectively by flow cytometry to observe apoptosis and cell cycle distribution in mice; Survivin protein expression was detected by immunocytochemistry method. Results: Transfection group's inhibition of the expression of survivin protein was obviously higher than that of Xiaoyan Decoction group, but significantly lower than Xiaoyan Decoction and siRNA group. Transfection group and Xiaoyan Decoction both could induce cell apoptosis, and induction effect of transfection group was stronger than Xiaoyan Decoction group, Xiaoyan Decoction combined with transfection group could ernhance the induction of cell apoptosis. Conclusion: Xiaoyan Decoction combined with Survivin apoptosis induced by siRNA can synergy to enhance its role.

Objective: To study the Xiaoyan Decoction serum influence the expression level of lung adenocarcinoma that cisplatin-resistent of A549/DDP cell multidrug resistance-associated protein 1 (MRP1), lung resistance-related protein (LRP), MRP mRNA, and LRP mRNA, to find out Xiaoyan Decoction's target to the chemotherapy resistance, and to lay foundation for the clinical of reversing lung cancer chemotherapy resistance. Methods: The A549 nude mice hypodermic tumor model was given an equal amount of saline, 2 mg/kg of cisplatin, low and high doses of Xiaoyan Decoction (20, 40 g/kg), and then the serum was gain. The cisplatin resistance of human lung adenocarcinoma A549/DDP cell line was chosen as acquired drug-resistance model, Xiaoyan Decoction on MDR of A549/DDP cell gene expression product of multidrug resistance-associated protein 1, lung resistance-related protein and its effect on mRNA expression level was detected by Western blotting and RT-PCR technique. Results: Compared with the control group, the expression of MRP1 and LRP protein decreased gradually (P<0.05) as the serum drug concentration of Xiaoyan Decoction increased. The level of LRP mRNA and MRP1 mRNA in low and high doses of Xiaoyan Decoction group also decreased, and much more obvious inhibition of gene expression was observed as the increasing of drug concentration. Conclusion: Different serum drug concentration of Xiaoyan Decoction can differently control the expression of MRP1, LRP protein, MRP1 mRNA, and LRP mRNA, with the increasing of serum drug concentration, the decreasing of expression level, namely positive-correlated to the dose, which indicates that Xiaoyan Decoction Reversal of the lung cancer drug resistance the might related to the MRP1 and LRP.

ABSTRACT

Objective: To study the effects and mechanism of Xiaoyan Decoction on bone marrow suppression in mice from aspects of peripheral blood cell ratio, immune function, and cytokines. Methods: A mouse model of bone marrow suppression was established. Fifty mice were randomly divided into five groups (n = 10): control group, model group, Xiaoyan Decoction group, rhG-CSF group, and Xiaoyan Decoction combined with rhG-CS group. Lewis-tumor mice in Xiaoyan Decoction group were ig given Xiaoyan Decoction 18.2 g/(kg·d) twice daily, for 7 d. Lewis-tumor mice in rhG-CS group were treated by sc injection of rhG-CSF. The routine blood test, immunological functions of peripheral blood and cytokines (IL-3, IL-6, EPO, and GM-CSF) in serum were detected. Results: The bone marrow suppression model was successfully established. Compared with control group, the levels of WBC, RBC, Hb, and PLT in peripheral blood of model group were decreased, especially in WBC (P 0.05). The concentration of IL-3 and IL-6 was dramatically reduced in model group compared with control group (P < 0.05). And it showed an obviously increasing trend in IL-3, IL-6, EPO, and GM-CSF levels in Xiaoyan Decoction group and combined group, as well as an increase in IL-3, IL-6 levels in rhG-CS group (P < 0.05). Conclusion: It shows that Xiaoyan Decoction combined with rhG-CS can improve the level of WBC and immunological functions of peripheral blood in bone marrow suppression model mice. And the effect of combination of both drugs is more significant. This might be related to the effect of promoting the production of serum hematopoietic regulator such as IL-3, IL-6, EPO, and GM-CSF, and relieving chemotherapy-induced bone marrow suppression.

ABSTRACT

Objective: To investigate the inhibition of Xiaoyan Decoction and its decomposed recipe on apoptosis of pulmonary carcinosis A549 cells. Methods: According to the effect, Xiaoyan Decoction is divided into three groups: Qingre Jiedu (clearing away heat-evil and expelling superficial evils) group, Huoxue Huayu group, and Yiqi Fuzheng group. Using CCK 8 method to detect the proliferation activity of lung adenocarcinoma cancer A549 cells. The Western blotting was adopted to observe the influence on protein expression of survivin and caspase-3 of pulmonary carcinosis A549 cells. And the RT-PCR was adopted to observe the influence on survivin and caspase-3 gene expression of pulmonary carcinosis A549 cells. The inhibition of Xiaoyan Decoction and its decomposed recipe on the apoptosis of pulmonary carcinosis A549 cells was investigated. Results: There were obvious inhibitory effects of cancer cell activity in the three groups. Xiaoyan Decoction in the three groups could induce the apoptosis of lung cancer A549 cell line. Its mechanism was the down regulation of survivin expression. And the related with the elevation of caspase-3 expression is not intimate. Conclusion: Xiaoyan Decoction has the antitumor effect by inducing tumor cell apoptosis through the down regulation of survivin expression.
