ABSTRACT

OBJECTIVE:To investigate influential factors of standardization of clinical research on therapeutic efficacy of Yao-tongning capsules, and to provide direction for reasonable and standard development of clinical research. METHODS:Retrieved from CJFD,VIP and Wanfang database,RCTs about Yaotongning capsules in the treatment of related disease were collected. After data extraction and quality evaluation,Meta-analysis was conducted by using Rev Man 5.3 statistical software. The effects of the di-alectical situation, treatment course and route of administration on the evaluation of therapeutic efficacy of Yaotongning capsules were compared;based on this, the concepts of index evidence-based value which reflected clinical standardization were put for-ward,and its effects on Yaotongning capsules were investigated. RESULTS:A total of 71 RCTs were included, involving 11009 patients. Results of Meta-analysis showed therapeutic efficacy of Yaotongning capsules was better than that of other drugs in dialecti-cal case(P=0.02),while therapeutic efficacy of Yaotongning capsules was inferior to that of other drugs in non-dialectical case(P<0.01);therapeutic efficacy of Yaotongning capsules was inferior to that of other drugs when treatment course was shorter than 30 days(P<0.05),while treatment course was longer than 30 days, there was no difference in therapeutic efficacy between Yaotongn-ing capsules and other drugs(P=0.99). When did not take medicine according to the instruction, therapeutic efficacy of Yaotongn-ing capsule was inferior to that of other drugs(P<0.05);when taking medicine according to the instruction, there was no differ-ence in therapeutic efficacy between Yaotongning capsules and other drugs(P=0.94). When evidence-based value was ≥5,thera-peutic efficacy of Yaotongning capsules was better than that of other drugs(P=0.03);when evidence-based value was equal to 4, there was no difference in therapeutic efficacy between Yaotongning capsules and other drugs(P=0.56);when evidence-based val-ue was ≤3,therapeutic efficacy of Yaotongning capsules was inferior to that of other drugs(P<0.05),with statistical significance. CONCLUSIONS:It is an important guarantee of playing therapeutic efficacy of Yaotongning capsules to regularly use drugs strictly according to the principle of dialectical treatment, and take drugs according to treatment course and instructions. Clinical trials should be standardized strictly in accordance with the instructions,so as to provide objective and fair clinical findings.

ABSTRACT

Objective: To evaluate the efficacy and safety of Yaotongning Capsules in the treatment of lumbar disc herniation (cold dampness and blood stasis syndrome). Methods: This research was carried out using stratified random method, blind, positive drug controlled, and multi-center clinical. And the random number was generated by SAS9.1.3 into two experimental groups (4 grains, 120 cases and 6 grains, 120 cases), and control group (120 cases). The experimental group was treated with Yaotongning Capsules for 4 weeks, and the control group was treated with Yaobitong Capsules for 4 weeks. At 0 day, 2, and 4 weeks, the VAS score, Oswestry dysfunction index, and onset time of pain were evaluated, and the efficacy and safety of the treatment were evaluated. Results: After 4 weeks of treatment, the efficacy of ODI grade showed that the excellent rates of the experimental group (4 grains), control group, and experimental group (6 grains) were 85.83%, 73.95%, and 85%, the efficacy of the experimental group (4 grains) was better than other groups, and the adverse reaction was the least. However, in terms of pain relief and improvement in the ODI scale, the experimental group (6 grains) in 2 and 4 weeks after treatment of In VAS and ODI improvement experiment, the decreased value of experimental group (6 grains) > experimental group (4 grains) > control group. Conclusion: Yaotongning Capsules in the treatment of lumbar disc herniation (cold dampness and blood stasis syndrome) have obviously curative effect and good safety, and have the advantage of the rapid relief of pain and could improve the function of lumbar spine.

ABSTRACT

Objective: On the basis of formulas derived from Yaotongning (YTN) Capsules to compare the different cellular pharmacological activity of six kinds of Chinese materia medica (CMM) polysaccharides, thus to supply the evidence for the screening of candidate medicines for rheumatoid bone diseases. Methods: Six samples were designed by respectively adding the polysaccharides of Glycyrrhizae Radix, Atractylodis Rhizoma, Cyathulae Radix, Ephedrae Herba, Epimedii Herba, and Taxilli Herba, at 50% into the six mixtures based on the derived formula of YTN. Murine macrophage cells and chondrocytes were exposed to the new recipes, and then the half-inhibitive concentration (IC50) and half-effective concentration (EC50) were applied to evaluating their activity for inhibiting secretion of PGE2 in Ana-1 cells induced by LPS and activities for stimulating the production of IL-6, IL-1β, and TNF-α in Ana-1 cells as well as activity for promoting IL-1β-induced proliferation of chondrocytes by enzyme linked immunosorbent assay. The interactions among the active fractions were evaluated by comparing the experimental EC50/IC50 values to their corresponding additive EC50/IC50 values, which were calculated by the least square method. Results: Among the six formulas derived from YTN, except for one of the two formulas mainly consisted by polysaccharides of Taxilli Herba or Epimedii Herba, whose activities for inhibiting the production of PGE2 in Ana-1 cells induced by LPS, promoting the secretion of IL-6, IL-1β, and TNF-α in Ana-1 cells, and stimulating the secretion of chondrocytes induced by IL-1β were significantly weaker than those of YTN. The activities of other four formulas which were respectively consisted by 50% one of polysaccharides from YTN material medicines were better than those of YTN or statistically equivalent to YTN. The sample mainly composed of polysaccharides of Cyathulae Radix worked well on inhibiting the production of PGE2 in Ana-1 cells induced by LPS, promoting the secretion of IL-6, IL-1β in macrophages, and stimulating the secretion of chondrocytes. Conclusion: The polysaccharides of Cyathulae Radix have the good effect on cellular anti-inflammation, immune regulation, and chondrocytes proliferation. It can be a candidate for the active fraction of polysaccharides in the prescription of rheumatoid disease.
