ABSTRACT

Objective:To analyze the application effect of health failure mode and effect analysis (HFMEA) model in patients with artificial airways in the cardiovascular surgery intensive care unit (CSICU) by establishing a HFMEA project team, and to develop targeted improvement measures and processes.Methods:The patients undergoing cardiovascular surgeries and with established artificial airways in the Shandong Provincial Hospital Affiliated to Shandong First Medical University were recruited from October 2021 to March 2022. The enrolled patients were divided into the conventional management group and the HFMEA model management group according to random number table method. The conventional management group applied the conventional procedures for monitoring the air bag pressure. The HFMEA model management group used the HFMEA model to implement and improve the airbag pressure monitoring process. The efficacy of HFMEA was assessed by comparing the incidence of ventilator-associated pneumonia (VAP), the pass rate of airbag pressure monitoring, the duration of endotracheal intubation and the length of CSICU stay between two groups. The practicability of HFMEA model was evaluated by analyzing the theoretical assessment scores and practical skill scores of nurses and their satisfaction scores with HFMEA.Results:Compared with the conventional management group, the patients in the HFMEA mode management group had a significantly higher rate of passing airbag pressure monitoring [94.99% (2 994/3 152) vs. 69.97% (1 626/2 324), P < 0.01], shorter duration of endotracheal intubation and length of CSICU stay [duration of endotracheal intubation (hours): 6 (7, 12) vs. 6 (8, 13), length of CSICU stay (hours): 40 (45, 65) vs. 41 (46, 85), both P < 0.05], but the incidences of VAP between the two groups were similar. The theoretical assessment scores and practical skill scores of nurses were significantly higher (theoretical assessment score: 44.47±2.72 vs. 37.59±6.56, practical skill score: 44.56±2.66 vs. 40.03±4.32, total score: 89.03±3.07 vs. 77.63±9.56, all P < 0.05) in the HFMEA mode management group. And the satisfaction scores with airbag pressure management were also significantly higher in the HFMEA mode management group (7.72±1.11 vs. 6.44±1.32, P < 0.05). Conclusions:The application of the HFMEA can improve the airbag pressure measures and standardize the monitoring procedures in patients with artificial airways, and reduce the risk of clinical nursing. It is safe and effective for patients with invasive mechanical ventilation in the CSICU.

ABSTRACT

Objective To propose an airbag-type helmet cushioning lining structure and analyze its protective effect on head injury of two-wheeled bicycle riders. Methods The airbag lining was applied to two typical two-wheeled bicycle helmets for bicycles (half helmets) and motorcycles (full helmets). Then kinematic and biomechanical responses of the human head model were predicted from impact simulations under test conditions of the standard GB 24429-2009 and the regulations ECE R22.05, and conventional expanded polystyrene (EPS) helmets were compared from the perspective of skull fracture and brain injury risk, so as to make comprehensive evaluation on protective performance of the airbag helmet. Results When the airbag pressure was 0.06 MPa, the relevant amount of human skull fracture under protection of airbag helmet (half/full helmet) was smaller than 120 g and 150 g, respectively; the risk of skull fracture was basically lower than 40%; the maximum principal strains of the brain were both smaller than 0.3, which indicated that the risk of mild brain injury was lower than 25%. Generally, the risk of human skull fracture and head injury under protection of airbag helmets was lower than that under protection of EPS helmets. Conclusions The airbag helmet designed in this study has a good protective effect, which can give attention to the protection of both skull fracture and head injury, providing a basic example for the design of novel helmet. Injury risk analysis can also provide the preliminary reference for emergency diagnosis on head injury of cyclists.

ABSTRACT

In order to solve the problem of continuous monitoring and automatic regulation of patient airbag pressure in intensive care unit, the study designed an intelligent management system of artificial airway airbag pressure. It can realize real-time monitoring and automatic control of airbag pressure. Its pressure data was sent to the PC in real time by the serial port. It can realize the display, store, review and analysis of pressure data. Its clinical application effect was discussed. Experiments showed that the system can monitor airbag pressure in real time and control the pressure to stabilize at 25~30 cmH

ABSTRACT

Objective To study the effect of neck restrain on traumatic brain injury during airbag inflation in traffic accidents. Methods Based on the previously validated 3-year-old child head finite element (FE) model, the impact on out-of-position (OOP) child occupant during airbag inflation was simulated by FE method, so as to investigate the effects of neck restraint on intracranial response and injury mechanism in traffic accidents. Results The head kinematics with neck restrain was different from that without neck restrain under the impact of airbag inflation. The neck restraint would obviously decrease the maximum Von Mises stress of pediatric brain. When airbag-head distance was 20 cm or 25 cm, the neck restraint would obviously decrease the maximum intracranial pressure. Conclusions Neck restraint had a relatively large influence on pediatric intracranial response. When the FE method is used to predict pediatric craniocerebral injury, consideration of neck restrain on child brain response is necessary.

ABSTRACT

Objective To quantitatively study the pressure of residual lateral displacement in distal radius AO C3.1 fracture after manual reduction corrected by dynamic airbag pad using finite element analysis and to verify its effectiveness for correcting the residual displacement of fractures. Methods Imageware 13.0, Mimics 15.0 and ANSYS Workbench were used to simulate 1 cm residual lateral displacement after manual reduction of distal radius fracture corrected by dynamic airbag pad. Then the correlation between the distance of residual lateral displacement and the adjustment of dynamic airbag pad pressure were quantitatively analyzed. Results In the case of constant load restrained by airbag ribbon, during the process of pressure adjustment by splint pad, the stress was mainly distributed in the fracture end where the airbag pad was located. About 2.4 kPa pressure was needed to correct 1 mm displacement on radial side, while about 1.3 kPa pressure was needed to correct 1 mm displacement on dorsal side. The dynamic airbag pad was depressurized after the restoration of residual shift. At this time, displacement could be effectively prevented due to the constant load of airbag ribbon and the frictional load at the fracture end. Conclusions In the case of constant load constrained by airbag ribbon, intelligent airbag splint can effectively correct the residual lateral displacement after the manual reduction of the distal radius AO C3.1 fracture and prevent it from being displaced by adjusting pressure of the dynamic airbag pad.

ABSTRACT

Patient positioning is a vital in ensuring patient comfort, dignity and good access to the operative site. Accurate positioning helps in minimising the risk of injury to the patient. An in-house airbag made from recyclable medical supplies can be used to replace the conventional sandbag for head and neck surgeries. It involves the use of an empty irrigation bottle, a valve and suction tubing. Its shape and volume is readily adjustable by altering the volume of inflated air. Moreover, compressible air moulds the bag to the curvature of the patient’s external surface reducing the risk of pressure sores in prolonged surgery. Ultimately this serves as an innovative technique which is financially savvy with the use of recyclable items.

ABSTRACT

Objective Observe the clinical effect of the controllable airbag elastic chest strap after the breast cancer operation,studying the clinical value of the controllable airbag elastic chest strap.Methods Between May.2009 and Oct.2012,71 women having surgery for breast cancer at The Affiliated Second Hospital of Nan Hua University Oncology Center.They were to receive either a common thoracic girdle compressed binding(n =35),or the controllable airbag elastic chest strap compressed binding (n =36) at the postoperation.The following complications were evaluated:the incidence of seroma formation and duration,the incidence of skin flap swelling,and the dressing time.Results Thirty-five patients with common thoracic girdle is 14 cases of subcutaneous effusion,fluid rate was 40.0%,the effusion duration 7 to 31 days,an average of 17.6 ±7.3 days,15 cases with skin redness (disc),swelling rate is 42.9%,the average treatment time 18.7 ±3.5 minutes; With the controllable airbag elastic chest strap group of six cases of subcutaneous effusion,fluid rate was 16.7%,the duration of effusion 5-14days,an average of 10.5 ± 3.9 days,skin redness (disc),swelling rate is 13.9%,the average treatment time 12.2 ± 2.4 minutes.Conclusions The controllable airbag elastic chest strap can reduce subcutaneous effusion,reduce skin flapswelling,saving the treatment time,is worth promoting in the clinical medical supplies.

ABSTRACT

Objective To get the message of master, attitudes and clinical implementation of ICU nurses about clearing the secretion on airbag in patients with mechanical ventilation. Methods Using the method of questionnaires and simple sampling, 85 questionnaires were issued to ICU nurses of four general hospitals in our city.and survey results were analyzed. Results The score had significant difference among ICU nurses with different ages,titles and departments,but no difference among nursing staff with different educational background,There were not enough knowledge and not positive attitudes and atmosphere in nurses about clearing the secretion on airbag for patients with mechanical ventilation. Conclusions Knowledge of management of clearing the secretion on airbag should be popularized, standardized training to nurses should be strengthened,these are the important methods to increase the cognitive degree on secretion on airbag. The accreditation of ICU nurses should be practicedjiurses should work with certificates and quality of care for artificial airway should be ensured. A unified,scientific nursing practice standard and process to clear the secretion on airbag should be established.

ABSTRACT

Air-bags have received widespread support as an effective means of enhancing automotive safety, and they are becoming more common as standard automobile equipment on many cars. Although air-bag induced ocular injuries are rare, they present a serious concern because of the possibility of permanent damage or visual impairment. To date, most reports have investigated ocular injury from high velocity motor vehicle accidents and reports of ocular injury from low speed motor vehicle accidents have been rare. We describe a patient who sustained severe ocular injury, including periorbital fracture, hyphema, vitreous hemorrhage, and choroidal rupture of the macular area, due to an inflated air-bag in a low speed motor vehicle accident.
