ABSTRACT

【Objective】 To identify low-risk donor population and optimize blood screening, recruitment and consultation strategies via retrospectively analyzing the unqualified results of Hb, ALT, HBsAg, TP before whole blood donation from 2015 to 2018. 【Methods】 Pre-donation examinations of Hb, ALT, HBsAg and TP were conducted by copper sulfate method, dry chemical method, and TPPA etc. 【Results】 A total of 70 146 out of 685 469 blood donors in Zhengzhou city from 2015 to 2018 were deferred due to unqualified pre-donation. The unqualified rates of Hb, ALT, HBsAg and TP were 1.75%(11 996/685 469), 7.78%(53 329/685 469), 0.60%(4 113/685 469) and 0.10%(685/685 469), respectively. For Hb deferral, 2.5%(17 137/685 469) were male and 97.5%(668 332/685 469)female; for ALT deferral, 85.9%(588 818/685 469) male and 14.1%(96 651/685 469) female. 【Conclusion】 The causes of pre-donation deferral in whole blood donors were mainly ALT, then Hb. Hb deferral showed an increasing trend and dominated by female donors, while ALT deferral was dominated by male donors. The overall unqualified rate of ALT, HBsAg and TP, however, are decreasing year by year through taking targeted measures, strengthening the publicity and education of blood donation, standardizing the blood collection and supply process.

ABSTRACT

Objective:To evaluate the diagnostic efficiency of hypersensitivity quantitative fecal immunochemical test (hs-qFIT) in colorectal cancer (CRC) and advanced adenoma.Methods:From July to December 2020, consecutive patients aged 50 to 75 years who underwent colonoscopy in Qilu Hospital of Shandong University, and had the Asia-Pacific colorectal screening score of medium or high risk were enrolled. All patients were requested to complete two hs-qFIT before colonoscopy. The diagnostic efficacy of hs-qFIT for CRC and advanced adenoma were assessed. Receiver operating characteristic curve of hs-qFIT in CRC diagnosis was drawn and the area under the curve (AUC) was calculated.Results:A total of 811 patients including 20 (2.5%) cases of CRC, 47 (5.8%) cases of advanced adenoma, 206 (25.4%) cases of non-advanced adenoma, 219 (27.0%) cases of non-adenomatous polyp, 76 (9.4%) cases of other colorectal lesions and 243 (30.0%) cases of non-colorectal lesions were involved. When the fecal hemoglobin cut-off values were 10, 30, 50, 75 and 100 ng/mL, the positive rates of hs-qFIT detection were 17.9% (145/811), 10.9% (88/811), 8.3% (67/811), 7.4% (60/811) and 5.8% (47/811), respectively. When the cut-off value of fecal hemoglobin decreased from 100 ng/mL to 10 ng/mL, the sensitivity of hs-qFIT for CRC diagnosis increased from 90.0% to 100.0%, and the specificity decreased from 96.3% to 84.2%; and the sensitivity of hs-qFIT for the diagnosis of advanced adenoma increased from 19.1% to 66.0%, and the specificity decreased from 95.0% to 85.1%. The AUC of hs-qFIT for the diagnosis of CRC and advanced adenoma were 0.981 (95% confidence interval ( CI) 0.970 to 0.992) and 0.846 (95% CI 0.807 to 0.886), respectively. When the optimal cut-off values were taken, the sensitivity and specificity were 100.0% and 91.2% for the diagnosis of CRC, and 66.0% and 85.3% for the diagnosis of advanced adenoma, respectively. Conclusion:Hs-qFIT can help the early screening of CRC and advanced adenoma.

ABSTRACT

BACKGROUND: Although a common shoulder disease, there are no accepted classification criteria for frozen shoulder (FS). This study therefore aimed to evaluate the accuracy of the conventionally used FS classification system. METHODS: Primary FS patients (n=168) who visited our clinic from January 2010 to July 2015 were included in the study. After confirming restrictions of the glenohumeral joint motion and absence of history of systemic disease, trauma, shoulder surgery, shoulder muscle weakness, or specific x-ray abnormalities, the Zuckerman and Rokito's classification was employed for diagnosing primary FS. Following clinical diagnosis, each patient underwent a shoulder magnetic resonance imaging (MRI) and blood tests (lipid profile, glucose, hemoglobin A1c, and thyroid function). Based on the results of the blood tests and MRIs, the patients were reclassified, using the criteria proposed by Zuckerman and Rokito. RESULTS: New diagnoses were ascertained including blood test results (16 patients with diabetes, 43 with thyroid abnormalities, and 149 with dyslipidemia), and MRI revealed intra-articular lesions in 81 patients (48.2%). After re-categorization based on the above findings, only 5 patients (3.0%) were classified having primary FS. The remaining 163 patients (97.0%) had either undiagnosed systemic or intrinsic abnormalities (89 patients), whereas 74 patients had both. CONCLUSIONS: These findings demonstrate that most patients clinically diagnosed with primary FS had undiagnosed systemic abnormalities and/or intra-articular pathologies. Therefore, a modification of the Zuckerman and Rokito's classification system for FS may be required to include the frequent combinations, rather than having a separate representation of systemic abnormalities and intrinsic causes.

Abstract Today, blood tests are an indispensable tool in avian medicine. This study aimed to describe and compare hematological and morphometric data of erythrocytes, leukocytes and thrombocytes of rhea, Rhea americana. To do so, 58 rheas of both sexes on two farms, one in Cachoeiro do Itapemirim, Espírito Santo (ES), and the other in São Carlos, São Paulo (SP), were selected. Blood samples were taken and their RBC counts, PCV and Hb levels were determined and used in hematimetric index calculations. The total and differentiated leukocyte counts and the TPP and fibrinogen serum levels were also obtained. The results from the two farms analyzed were compared by means of the t test. There were differences in the parameters of the erythrocyte series between the two flocks because of a process of hypochromic macrocytic anemia observed among the rheas in ES. The values for the erythrocyte series and indexes were: RBC, 2.81 ± 0.15 x106/μl; PCV, 44.20 ± 2.86%; Hb, 12.12 ± 0.74 g/dL; MCV, 15.75 ± 0.89 fL; MCH, 43.18 ± 1.82 pg; and MCHC, 27.44 ± 0.80 g/dL. The values for the leukocyte series were: WBC, 12,072 ± 4116 /μL; heterophils, 64.10 ± 9.90%; eosinophils, 2.05 ± 2.06%; monocytes, 6.40 ± 2.99%; lymphocytes, 26.93 ± 9.62%; and basophils, 0.52 ± 1.27%. These can all be suggested as references for rheas reared in Brazil. Statistical differences were observed in erythrocyte length measurements, which were higher in the rheas in SP than in those in ES. Regarding width, the heterophils, lymphocytes and monocytes of the birds in ES were statistically greater than those of the birds in SP. It was concluded that rheas may exhibit morphometric alterations to blood cells and differences in blood elements according to the type of management to which they are subjected.


Resumo Atualmente exames de sangue são ferramentas indispensáveis na medicina aviária. Este estudo teve como objetivo descrever e comparar valores hematológicos e morfométricos de eritrócitos, leucócitos e trombócitos de emas, Rhea americana. Para isso, foram selecionadas 58 emas de ambos os sexos de dois criatórios, um em Cachoeiro do Itapemirim, Espírito Santo (ES) e o outro em São Carlos, São Paulo (SP). Amostras de sangue foram coletadas e realizadas a hematimetria, Hto e dosagem de Hb que foram utilizados nos cálculos dos índices hematimétricos. Também foram efetuadas as contagens total e diferencial de leucócitos, além da dosagem de proteínas total e do fibrinogênio séricos. Os resultados referentes às duas propriedades analisadas foram comparados pelo Teste t. Ocorreram diferenças nos parâmetros da série vermelha entre as aves dos dois criatórios devido a um processo de anemia macrocítica hipocrômica observado nas emas do ES. Os valores para a série vermelha e índices hematimétricos como: hematimetria, 2,81±0,15 x106/μl; Hto, 44,20±2,86%; Hb, 12,12±0,74 g/dL; VCM, 15,75±0,89 fL; HCM, 43,18±1,82 pg; CHCM, 27,44±0,80, além dos valores da série branca como: leucometria total, 12.072±4.116/μL, heterofilo, 64,10±9,90%, eosinófilo, 2,05±2,06%, monócito, 6,40±2,99%, linfócito, 26,93±9,62%, basófilo, 0,52±1,27% foram sugeridos como os de referência para as emas criadas no Brasil. Diferenças estatísticas foram observadas no comprimento de eritrócitos, o qual estava maior nas emas de SP do que aquelas do ES. Em relação à largura, os heterófilos, linfócitos e monócitos das aves do ES foram estatisticamente maior do que aquelas das aves de SP. Concluiu-se que emas podem apresentar alterações morfométricas de células sanguíneas e diferenças nos elementos do sangue de acordo com o tipo de manejo a que são submetidos.

ABSTRACT: The purpose of this study was to assess the levels of available phosphorus (AP) in the diet of growing pigs selected for lean deposition on the hematological, biochemical, and bone performance parameters. A total of 64 commercial hybrid pigs (32 castrated males and 32 females; mean initial weight: 23.31±2.4kg; age: 55-90 days) were used. Animals were distributed into a randomized block design with four treatments, eight replicates, and two animals (one male and one female) per experimental unit. The treatments consisted of a basal diet and another three diets, which were obtained by supplementing the basal diet with dicalcium phosphate replacing the inert phosphate, resulting in four diets with 0.100, 0.180, 0.260, and 0.340% of available phosphorus. The levels of available phosphorus quadratically influenced the daily weight gain, feed consumption, and feed conversion, which increased up to the estimated levels 0.331; 0.330, and 0.302% of available phosphorus. Levels of phosphorus showed no influence on the hematological parameters, which remained within the reference values. Levels of available phosphorus increased linearly the ash and phosphorus contents in the metacarpal bone and decreased linearly the alkaline phosphatase activity. Levels of available phosphorus (0.331 and 0.302%, corresponding to a daily intake of 5.73 and 5.18g d-1) provided respectively the best results for weight gain and feed conversion in pigs with high genetic potential, without change in hematological parameters.


RESUMO: O objetivo deste estudo foi avaliar níveis de fósforo disponível em rações para suínos em crescimento, selecionados para deposição de carne, sobre os parâmetros de desempenho, hematológico, bioquímico e ósseo. Foram usados 64 suínos híbridos comerciais (32 machos castrados e 32 fêmeas; peso médio inicial: 23,31±2,4 kg; idade: 55-90 dias). Os animais foram distribuídos em blocos casualizados com quatro tratamentos, oito repetições e dois animais (um macho e uma fêmea) por unidade experimental. Os tratamentos consistiram em uma ração basal e em outras três rações, obtidas pela suplementação da ração basal com fosfato bicálcico em substituição ao inerte, resultando em quatro rações com 0,100, 0,180, 0,260, 0,340% de fósforo disponível. Os níveis de fósforo disponível influenciaram de forma quadrática o ganho de peso diário, o consumo de ração e a conversão alimentar, que aumentaram até o nível estimado de 0,331, 0,330 e 0,302% de fósforo disponível, respectivamente. Não houve influência dos níveis de fósforo sobre os parâmetros hematológicos que permaneceram dentro dos valores de referência. Os níveis de fósforo disponível influenciaram de forma linear crescente os teores de cinzas e fósforo no metacarpo dos animais e, de forma decrescente, a atividade da fosfatase alcalina. Os níveis de fósforo disponível, de 0,331 e 0,302%, correspondentes ao consumo diário de 5,73 e 5,18g d-1, proporcionaram, respectivamente, os melhores resultados de ganho de peso e conversão alimentar em suínos de alto potencial genético, sem alterações nos parâmetros hematológicos.

Objective To evaluate the significance of detecting plasma methylated Septin9 (SEPT9) alone and combined with fecal immunochemical test (FIT) in screening colorectal cancer (CRC) and adenoma.Methods From October 2014 to April 2015,outpatients received CRC and adenoma screening were enrolled.Colonscopy examination and pathological diagnosis were taken as gold standard.The sensitivity and specificity of plasma methylated SEPT9 and FIT in CRC and adenoma were calculated.The diagnostic value of combined examination was evaluated.Receiver operating characteristic (ROC) curve of SEPT9 in CRC diagnosis was drawn.Results A total of 300 outpatients were enrolled.Among them,CRC was detected in 45 patients (15.0%) and adenoma was detected in 68(22.7%) patients,including 37(12.3%) cases of advanced adenomas.The sensitivity of SEPT9 and FIT for CRC diagnosis was 80.0% and 88.9%,and the specificity was 95.3 % and 54.1%,respectively.The area under concentration curve (AUC) of methylated SEPT9 in CRC diagnosis was 0.923.The sensitivity of SEPT9 and FIT in advanced adenoma diagnosis were 10.8% and 62.2%,and the specificity were 83.3% and 49.0%,respectively.The sensitivity of combined examination in CRC diagnosis was 97.8% and the specificity was 52.9%;the sensitivity in advanced adenoma diagnosis was 67.6% and the specificity was 47.4%.Conclusions Plasma methylated SEPT9 is helpful in CRC screening,moreover,when combined with FIT,it can increase detection rate of colorectal adenoma.

Objetivo: Avaliar a assistência pré-natal de mulheres assistidas ao parto e verificar a relevância dos exames laboratoriais na qualidade do pré-natal. Métodos: Estudo de corte transversal desenvolvido em um hospital universitário de referência na cidade de Santa Cruz- RN, no período de junho a julho de 2014, com 50 parturientes assistidas ao parto. As mulheres tinham entre 18-40 anos e possuíam baixo risco obstétrico. Os dados foram coletados a partir do cartão do pré-natal e aplicação de questionário estruturado. A caracterização da adequação do pré-natal foi elaborada com base nas diretrizes do Programa de Humanização do Pré-natal e Puerpério (PHPN) e no Manual Técnico de Pré-natal e Puerpério (MTPP).Realizou-se análise descritiva dos dados e o teste Qui-quadrado para a verificação de diferenças entre as proporções. Resultados: Observou-se que 86% (n=43) iniciaram o pré-natal precocemente e obtiveram uma média de 7,3 consultas. Os procedimentos clínicos obstétricos tiveram cinco ou mais registros em 58% (n=29) da amostra. Quando avaliado o registro de exames laboratoriais, observou-se um baixo percentual de adequação na 2ª rotina de exame preconizada (32%, n=16). De acordo com os parâmetros (PHPN e MTPP), o pré-natal esteve adequado apenas em 24% (n=12) dos casos. Quando avaliada a classificação da assistência do pré-natal sem o uso dos exames laboratoriais, a adequação subiu para 48% (n=24), com diferença estatisticamente significantes (p<0,001). Conclusão: O acesso ao pré-natal foi satisfatório, entretanto, a qualidade do pré-natal foi deficiente e a maior fragilidade parece estar relacionada aos registros dos exames laboratoriais.


Objective: To evaluate the prenatal care assistance provided to women assisted at delivery and examine the relevance of laboratory tests on the quality of prenatal care. Methods: A cross-sectional cohort study carried out in a university referral hospital, in the city of Santa Cruz-RN, from June to July 2014, including 50 pregnant women assisted at delivery. The women were between 18-40 years old and presented low obstetric risk. Data was collected from the prenatal medical chart and through a structured questionnaire. The characterization of the prenatal assistance adequacy was developed based on guidelines of the Prenatal and Puerperium Humanization Program (PPHP) and the Prenatal and Postpartum Technical Manual (PPTM). A descriptive analysis of the data was performed and the chi-square test was used for verification of differences between proportions. Results: It was observed that 86% (n=43) of the women initiated the prenatal care assistance early and had an average of 7.3 appointments. The clinical obstetric procedures had five or more records in 58% (n=29) of the sample. When assessed the laboratory tests records, a low percentage of adequacy was found in the 2nd recommended routine testing (32%, n=16). According to the parameters (PPHP and PPTM), prenatal care was suitable only in 24% (n=12) of the cases. When evaluated the classification of prenatal care assistance without the use of laboratory tests, the adequacy rose to 48% (n=24), presenting a statistically significant difference (p<0.001). Conclusion: Access to prenatal care assistance was satisfactory; however, its quality was deficient, and the major weakness seems to be related to the records of laboratory tests.


Objetivo: Evaluar la asistencia prenatal de mujeres asistidas en el parto y verificar la relevancia de las pruebas de laboratorio para la calidad del prenatal. Métodos: Estudio transversal desarrollado em un hospital universitario de referencia de la ciudad de Santa Cruz-RN en el período entre junio y julio de 2014 con 50 parturientas asistidas en el parto. Las mujeres tenían entre 18-40 años y riesgo obstétrico bajo. Fueron recogidos los datos de la tarjeta del prenatal y aplicación de un cuestionario estructurado. La caracterización de la adecuación del prenatal fue elaborada basado en las directrices del Programa de Humanización del Prenatal y Puerperio (PHPN) y em el Manual Técnico del Prenatal y Puerperio (MTPP). Se realizó um análisis descriptivo de los datos y el test Chi-cuadrado para verificar las diferencias de las proporciones. Resultados: Se observó que el 86% (n=43) iniciaron el prenatal precoz y tuvieron una media de 7,3 consultas. Los procedimientos clínicos de la obstetrícia tuvieron cinco o más registros en el 58% (n=29) de la muestra. En la evaluación del registro de las pruebas de laboratorio se observó un porcentual de adecuación bajo en la 2ª rutina de prueba establecida (32%, n=16). Según los parámetros (PHPN y MTPP), el prenatal fue adecuado solamente para el 24% (n=12) de los casos. En la evaluación de la clasificación de la asistencia del prenatal sin el uso de pruebas de laboratorio, la adecuación subió al 48% (n=24) con diferencia estadística significativa (p<0,001). Conclusión: El acceso al prenatal fue satisfactorio, sin embargo, la calidad del prenatal fue deficiente y la mayor fragilidad parece relacionarse com los registros de las pruebas de laboratorio.

ABSTRACT

Objective To study the blood routine tests results change after therapy of chlorpromazine ,clozapine ,perphenazine changed to risperidone in schizophrenic patients .Methods 100 schizophrenic patients in the hospital from March 2014 to March 2015 for treatment were enrolled in the study ,who were treated with chlorpromazine ,clozapine or perphenazine respectively for one course ,and then the therapies were replace by risperdal .Results After treatment ,mean corpuscular volume was significantly grea‐ter ,the number of lymphocytes was significantly higher and intermediate cell number was significantly lower than that before treat‐ment ,the difference was statistically significant(P<0 .05) .Conclusion Leukocyte differentiation change and mean corpuscular vol‐ume increased obviously after therapy of chlorpromazine ,clozapine or perphenazine changed to risperidone in schizophrenic patients .

Objective To explore colloidal gold method used to detect fecal occult blood tests(FOB)detection capability and establish the laboratory standard operation of detecting FOB limit of blank(LOB),limit of detection (LOD)and quantifica-tion limit (LOQ)according to the CLSI document《Evaluation of Detection Capability for Clinical Laboratory Measurement Procedures;Approved Guideline-Second Edition》(EP17-A2),in order to reduce the false negative rate of the weakly positive samples,and to provide a way of quantitative detection for qualitative detection of colloidal gold method.Methods Detected series of solution of hemoglobin made of dissolved fresh whole blood with the ELISA kit of human free hemoglobin,and es-tablished the standard curve of detection of FOB with colloidal gold method.Detected the blank samples and a series of low concentration samples with the colloidal gold test strip of FOB and measured the color bands by the Nato Checker710.The quantitative results obtained were statistically analysised by SPSS 1 9.0 and calculated blank limit,detection limit and quanti-fication limit.Results The LOB,LOD and LOD were 99.01,340.48 and 354.9 ng/ml according to the methods in CLSI EP1 7-A2 ducument.Conclusion The detection limits established by CLSI EP1 7-A2 document was more scientific in j udge-ment positive or negative to FOB than which used naked eye and can meet the clinical laboratory and clinical doctor require-ment better.Clinical laboratories should be strictly in detection limits of reagents in order to ensure their effectiveness,and should be generaly to other tests based on colloidal gold method.

ABSTRACT

Objetivou-se com este trabalho estudar o perfil hematológico de caprinos (Capra hircus) da raça Canindé, criados no Estado do Rio Grande do Norte, como também a busca de valores de referência que se adequem à nossa região. Foram coletadas amostras sanguíneas de 58 animais clinicamente sadios, distribuídos em 4 grupos (machos acima de 5 meses, fêmeas gestantes, fêmeas não gestantes e filhotes até 4 meses de idade). A partir de única amostra de sangue de cada animal foram realizados o eritrograma (contagem de hemácias, hematócrito, hemoglobina, volume corpuscular médio e concentração de hemoglobina corpuscular média) e o leucograma (contagem total e diferencial de leucócitos). Os dados obtidos foram avaliados por meio do teste de Tukey para variáveis paramétricas e Kruskal-Wallis seguido pelo teste de Dunn para não paramétricos em nível de significância de (p<0.05). Os resultados apontam para existência de maiores quantidades de He nos machos adultos e fêmeas não gestantes, diferente do que ocorre com o VCM; já com relação ao leucograma temos que os eosinófilos mostram-se mais elevados nas fêmeas adultas e os monócitos se elevam quando na presença da gestação. Os valores deste trabalho podem servir de referência para raça Canindé, tornando possível futuras interpretações para os parâmetros verificados, além de subsidiar novos estudos em animais hígidos ou doentes. Mostra a necessidade de pesquisas que evidenciem as condições semiáridas de manejo e alimentação, bem como a avaliação dos fatores de variação, sobre constituintes do sangue, que levem em conta o sexo, a idade e estado fisiológico dos animais.(AU)


The objective of this paper was to study the hematological profile of Canindé breed goats (Capra hircus) raised in the state of Rio Grande do Norte, as well as the acquirement of reference values appropriate to our region. Blood samples were collected from 58 clinically healthy animals, distributed in 4 groups (males, over 5-month-old, pregnant and not pregnant females and young goats, up to 4 months of age). From a single blood sample from each animal, it was possible to perform the red blood cell (RBC) count, packed cell volume (PCV), hemoglobin, mean cell volume (MCV), mean corpuscular hemoglobin concentration (MCHC) and the white blood cell (WBC) count and differential). The obtained data was statically evaluated by the Tukey test for parametric variables and Kruskal-Wallis test followed by Dunn's test for nonparametric-level significance (p<0.05). The results of this study can be used as a reference for this breed of goats, making future interpretations possible from the evaluated parameters, and can support further studies in healthy or diseased animals. It shows the need for further research that demonstrates the semiarid conditions of handling and feeding, as well as assessment of variation factors on the constituents of blood taking into account sex, age and the physiological state of animals.(AU)

ABSTRACT

OBJETIVOS: Foi avaliado o padrão de acometimento cervical em pacientes com diagnóstico de artrite reumatoide, correlacionando-se os achados de imagem com quadro neurológico, medicação utilizada, tempo de evolução da doença, idade de início da doença, exames laboratoriais e quadro clínico. MÉTODOS: Esse mesmo grupo de pacientes tinha sido estudado há 6 anos, e esses dados foram comparados para avaliar se houve piora do padrão de acometimento ao longo do tempo. Foram feitas radiografias simples da coluna cervical, nas incidências ântero-posterior, perfil neutro e dinâmico. A classificação de Ranawat foi utilizada para avaliar a sintomatologia dolorosa e o envolvimento neurológico. Foram utilizados os testes de regressão logística univariada (p < 0,05). RESULTADOS: Foram avaliados 24 dos 35 pacientes que compuseram a amostra anterior, havendo perda de seguimento de 7. Encontramos algumas correlações estatisticamente significativas: reflexo bicciptal (p = 0,049) e método de Ranawat (p = 0,023, no raio-x em extensão e p = 0,034, no raio-x neutro) com coluna estável, erosão do dente do áxis (p = 0,048) e subluxação lateral (p = 0,022) com instabilidade atlanto-axial e sinal de Lhermithé (p = 0,025) com invaginação basilar. Instabilidade cervical foi encontrada em 66,6 por cento (16/24), havendo um aumento estatisticamente significante da instabilidade subaxial nos últimos 6 anos (p = 0,032). CONCLUSÕES: Em ordem decrescente, as instabilidades foram: subluxação atlanto-axial com 50 por cento, subluxação subaxial com 41,6 por cento e invaginação basilar com 16,6 por cento. Houve um aumento no número de pacientes com instabilidade subaxial, não se observando correlação com a faixa etária de acometimento. Também não se observou correlação do reflexo biciptal com qualquer tipo de instabilidade e sim com a coluna cervical estável, resultado diferente do encontrado anteriormente.


OBJECTIVE: The long-term pattern of cervical involvement in patients with a diagnosis of rheumatoid arthritis was investigated, correlating the imaging findings with neurological symptoms, medication used, duration of the disease, age of onset, laboratory tests and clinical picture. METHODS: This same group of patients had already been studied for some time, and data were compared to verify if there was a worsening of the involvement pattern throughout time. Plain X-rays were obtained from the cervical column, in anteroposterior and neutral and dynamic lateral views. The Ranawat classification was used to evaluate pain symptoms and neurological involvement. For the analysis, the authors used the univariate logistic regression tests (p<0.05). RESULTS: Twenty-four of the 35 patients from the previous sample were evaluated and seven were lost to follow up. The authors found some statistically significant correlations: bicipital reflex (p=0.049) and the Ranawat method (p=0.023 with extension X-ray and p=0.034 with a neutral X-ray) with a stable column, erosion of the dens axis (p=0.048) and lateral subluxation (p=0.22) with atlanto-axial instability and the Lhermitte signal (p=0.025) with basilar invagination. Cervical instability was found in 66.6 percent (16/24) of the patients, and there was a significant statistical increase of the subaxial instability for the past 6 years (p=0.032). CONCLUSIONS: In a decreasing order of occurrence, the authors were able to verify the following types of instability: atlanto-axial subluxation, 50 percent; subaxial subluxation, 41.6 percent; and basilar invagination, 16.6 percent. An increase in the number of patients with subaxial instability was verified, and no correlation was observed regarding the age of onset. Additionally, no correlation was found between the bicipital reflex and any type of instability but rather with a stable column, and this result is different from what the authors obtained in the previous series.


OBJETIVOS: Fue evaluado el cuadrode afectación cervical en pacientes con diagnóstico de artritis reumatoidea, correlacionando los hallazgos de imagen con cuadro neurológico, medicación utilizada, tiempo de evolución de la enfermedad, edad del paciente alinicio de la enfermedad, exámenes de laboratorio y cuadro clínico. MÉTODOS: Ese mismo grupo de pacientes había sido estudiado 6 años antes, y esos datos recolectados fueron comparados para evaluar si hubo un empeoramiento del cuadrode afectacióna lo largo del tiempo. Se hicieron radiografías simples de la columna cervical, en las incidencias antero posterior, perfil neutro y dinámico. La clasificación de Ranawat fue utilizada para evaluar la sintomatología dolorosa y la involucración neurológica. Se utilizaron las pruebas de regresión logística univariada (p<0,05). RESULTADOS: Evaluados 24 de los 35 pacientes que formaron parte de la muestra anterior, habiendo pérdida de seguimiento de 7. Encontramos algunas correlaciones estadísticamente significativas: reflejo bicipital (p=0,049) y método de Ranawat (p=0,023, en el Rayo X en extensión a p=0,034, en el Rayo X neutro) con columna estable, erosión del odontoide (p=0,048) y subluxación lateral (p=0,022) con inestabilidad atlanto-axial y señal de Lhermithé (p=0,025) con invaginación basilar. Se ha encontrado inestabilidad cervical en un 66,6 por ciento (16/24), habiendo un aumento significativamente estadístico de la inestabilidad subaxial en los últimos 6 años p=0,032). CONCLUSIONES: (..) Hubo un aumento en el número de pacientes con inestabilidad subaxial, aunque no se ha observado correlación de ese tipo de inestabilidad con el nivel de edad de afectación. Tampoco se ha observado correlación del reflejo bicipital con cualquier tipo de inestabilidad, sino con la columna cervical estable, resultado distinto del que había sido encontrado en la serie anterior.
