ABSTRACT

Intra-abdominal infection is often secondary to the injury or lesion of various organs in the abdominal cavity, or after abdominal surgery. With the continuous development of its concept and technology, nutritional support has gradually become one of the important means for the treatment of patients with intra-abdominal infection. The comprehensive treatment of abdo-minal infection includes controlling of infection source, reasonable antimicrobial therapy, supporting of organ function, nutritional treatment, regulating of immune function, etc. Combined with their clinical experiences, the authors review the relevant researches at home and abroad and analyze and expound the nutritional support strategies for patients with intra-abdominal infection.

ABSTRACT

Succinyl-CoA:3-ketoacid CoA transferase (SCOT) deficiency is a rare inborn error of ketone body utilization, characterized by episodic or permanent ketosis. SCOT deficiency is caused by mutations in the OXCT1 gene, which is mapped to 5p13 and consists of 17 exons. A 12-month-old girl presented with severe ketoacidosis and was treated with continuous renal replacement therapy. She had two previously unrecognized mild-form episodes of ketoacidosis followed by febrile illness. While high levels of ketone bodies were found in her blood and urine, other laboratory investigations, including serum glucose, were unremarkable. We identified novel compound heterozygous mutations in OXCT1:c.1118T>G (p.Ile373Ser) and a large deletion ranging from exon 8 to 16 through targeted exome sequencing and microarray analysis. This is the first Korean case of SCOT deficiency caused by novel mutations in OXCT1, resulting in life-threatening ketoacidosis. In patients with unexplained episodic ketosis, or high anion gap metabolic acidosis in infancy, an inherited disorder in ketone body metabolism should be suspected.

ABSTRACT

Objective To study the influence of highly efficient antiretrovirus combined with anti-immune therapy on the im-mune function and metabolism in the patients with AIDS complicating pulmonary tuberculosis (TB) .Methods 170 patients with AIDS complicating pulmonary TB from May 2013 to May 2013 were selected and divided into the observation group and control group according to the random number table method ,85 cases in each group .The control group was given highly efficient antiretro-virus and conventional anti-TB medication therapy ,while on this basis the observation group was combined with anti-immune thera-py .Then the indicators of immune function ,lipid metabolism ,toxic and side reactions were compared between two groups .Results The CD4+ and CD4/CD8+ levels after 6-month treatment in the observation group were significantly higher than those in the con-trol group(P<0 .05) ,the CD8+ level was significantly lower than that in the control group(P<0 .01);Serum total cholesterol ,tria-cylglycerol and low density lipoprotein cholesterol levels in the observation group were significantly lower than those in the control group(P<0 .05) ,the high density lipoprotein cholesterol level was significantly higher than that in the control group (P<0 .01);the occurrence rate of toxic reactions and side effects such as nausea and vomiting in the observation group was 14 .12% ,which was significantly lower than 27 .06% in the control group(χ2 =4 .353 ,P<0 .05) .Conclusion Highly efficient antiretrovirus combined with anti-immune therapy is conducive to improve the immune function in the patients with AIDS complicating pulmonary TB ,de-creases its influence on the body metabolism ,and reduces the digestive tract reactions such as nausea and vomiting .

ABSTRACT

The pharmacokineties of intravitreally injected tetrandrine was monitered by high-performance liquid chromatography (HPLC). After intravitreal injection of tetrandrine in rabbits' eyes, aqueous humor and vitreous were drawn at different times for determination. The drug concentration in the aqueus humor and vitreous was measured by HPLC. The results showed that the concentration-time curves of tetr-andrinc in the vitreous fitted in with a two-compar-tment open model with half-life periods of 1.02 hour(?) and 16.62 hour (?) respectively. The drug concentration in aqueous humor was low, with a peak value of 6.73?g/ml at two hour.
