ABSTRACT

Objective: To analyze the serum carbohydrate antigen 125 (CA125) level and its influencing factors in male silicosis patients with pulmonary heart disease. Methods: In October 2021, data of 38 male patients with simple silicosis (silicosis group), 28 cases of silicosis with pulmonary heart disease (pulmonary heart disease group), and 27 healthy controls (control group) in the same age group were collected in inpatient and outpatient of Nanjing Occupational Disease Prevention and Control Hospital from January 2017 to December 2020. The serum CA125 levels of the three groups were compared, and the correlation between disease-related indexes and serum CA125 in silicosis patients with pulmonary heart disease was analyzed, as well as the influencing factors of pulmonary heart disease and serum CA125 levels in silicosis patients. Results: The serum CA125 level[ (19.95±7.52) IU/ml] in pulmonary heart disease group was higher than that in silicosis group[ (12.98±6.35) IU/ml] and control group[ (9.17±5.32) IU/ml] (P<0.05). There was no significant difference in serum CA125 level between the silicosis group and the control group (P>0.05). Serum CA125 levels were positively correlated with blood uric acid and fasting blood glucose in silicosis patients with pulmonary heart disease (r=0.39, 0.46, P<0.05). Serum CA125 level was a risk factor for silicosis patients with pulmonary heart disease (OR=1.13, 95%CI: 1.02-1.24, P<0.05). Dust exposure time, lactate dehydrogenase and smoking history were positively correlated with serum CA125 level in silicosis patients (P<0.05) . Conclusion: The serum CA125 level of male silicosis patients with pulmonary heart disease is significantly increased, and the level of CA125 is correlated with the level of fasting blood glucose and blood uric acid.

ABSTRACT

Objective: To explore the prognostic factors for the breast cancer patients with spinal metastasis, and establish a prognostic scoring model. Methods: A total of 160 breast cancer patients with spinal metastasis in Ruijin Hospital, Shanghai Jiao Tong University School of Medicine from January 2008 to January 2016 were retrospectively identified. The clinical characteristics and prognosis were analyzed by univariate and multivariate survival analysis to explore the prognostic factors. And then a prognostic scoring model was developed according to the regression coefficient for each independent prognostic factor. Results: The 160 breast cancer patients with spinal metastasis whose average age was 56.8 years (range 22-82 years) were identified, and the median follow-up was 40 (24, 55) months. The multivariate Cox analysis showed that the patients' general condition, hormone receptor expression, visceral metastasis, and serum carbohydrate antigen 125 (CA125) level significantly influenced survival (P<0.05). According to the regression coefficients, a survival prediction scoring model comprising these factors was established, which ranged from 0 to 6 points. Three risk groups with different prognoses were identified : low risk group (0-1 point), intermediate risk group (2-4 points), and high risk group (5-6 points). Conclusion: The general condition, hormone receptor expression, visceral metastasis, and serum CA125 level were independent prognostic factors for the breast cancer patients with spinal metastasis. And the prognostic scoring model comprising these four clinical factors can effectively predict the patients' prognoses.

ABSTRACT

Objective: To analyze the clinical features of ovarian fibrothecoma complicated with ascites and high level of CA125. and to explore the causes of elevated CA125 level and its significance in identifying benign and malignant ovarian tumors. Methods: The clinical data of one patient with varian fibrothecoma complicated with ascites and high level of senim CA125; combined with relevant literature review, the clinical characteristics, diagnosis and treatment process, and the significance of CA125 in the differential diagnosis of benign and malignant ovarian tumors were analyzed. Results: The patient was admitted to hospital due to vaginal bleeding for 4 months and pelvic mass for 1 month. The diagnosis result was pelvic and abdominal mass arising from ovarian according to the preoperative color Doppler ultrasound, enhanced CT» serum CA125 level and physical examination, and was highly suspected as ovarian malignant tumor. Surgery treatment was performed; based on the intraoperative rapid pathological results and postoperative slow pathology, the patient was diagnosed as ovarian fibrothecoma; the operation was successful. The patient recovered well and discharged from hospital. The serum CA125 level was decreased to the normal range 1 month after operation. Conclusion; Identification and quantitative analysis of the glycosylation modification and specific glycan structure of CA125 from different sources can significantly improve the diagnostic specificity of CA125 for benign and malignant ovarian tumors.

ABSTRACT

Objective: To investigate the value of risk ovarian malignancy algorithm (ROMA) in evaluation of the risk of epithelial ovarian cancer and its association with the clinical pathological stages, and to provide a basis for the clinical diagnosis and treatment and prognosis analysis of epithelial ovarian cancer.Methods: The clinical materials of 135 patients with ovarian tumor confirmed by paraffin section pathology after operation were retrospectively analyzed. The patients were divided into benign ovarian tumor group(n=66), epithelial ovarian cancer group(n=58) and non-epithelial ovarian cancer group(n=11).According to the cutoff values of HE4,CA125,and ROMA of the patients in various groups,the positive rates of HE4,CA125,and ROMA of the patients in various groups were calculated.Based on the serum levels of human epididymis protein 4(HE4) and carbohydrate antigen 125(CA125) detected before operation of the patients in various groups, the relationships between the positive rates of serum HE4, CA125 and ROMA and the clinical stages of the patients with ovarian cancer;their diagnosis efficacies in the patients with ovarian cancer were evaluated. Results: The serum CA125, HE4 levels and ROMA value of the patients in epithelial ovarian cancer group were significantly higher than those in benign ovarian tumor group and non-epithelial ovarian cancer group (P0.05). Among the three indexes, ROMA had the highest diagnostic efficacy index and CA125 had the lowest.Conclusion: The diagnostic value of ROMA for evaluating the risk of epithelial ovarian cancer was higher than those of CA125 and HE4, which has a better diagnostic value for the postmenopausal patients and can improve the early diagnosis efficiency of ovarian cancer.

Objective@#To explore the diagnostic value of combined detection of serum human epididymis protein 4 (HE4) and carbohydrate antigen 125 (CA125) for ovarian epithelial cancer.@*Methods@#Patients who underwent surgery for the adnexal tumor at Shanxi Provincial Cancer Hospital from January 2012 to December 2017 were enrolled. According to the postoperative pathological results, these patients were divided into the ovarian epithelial cancer group (494 cases) and benign ovarian disease group (462 cases). The serum expressions of HE4 and CA125 in the two groups were detected by enzyme-linked immunosorbent assay (ELISA) and chemiluminescence immunoassay. The diagnostic value of detection of HE4 and CA125 alone or in combination for ovarian epithelial cancer was analyzed.@*Results@#The median levels (P 25 - P 75) of serum CA125 and HE4 in ovarian epithelial cancer group were 273.34 U/ml (39.34 U/ml, 709.74 U/ml) and 199.08 pmol/L (75.81 pmol/L, 449.20 pmol/L), which were higher than those in ovarian benign disease group [16.30 U/ml (6.30 U/ml, 53.60 U/ml) and 39.54 pmol/L (29.57 pmol/L, 53.80 pmol/L)] (both P < 0.05). There was a positive correlation between serum CA125 and HE4 levels in ovarian epithelial cancer group (r = 0.481, P < 0.01). Serum CA125 and HE4 levels in patients with stage Ⅲ and Ⅳ were higher than those in patients with stage Ⅰ and Ⅱ (both P < 0.05), and serum CA125 and HE4 levels in patients with poor differentiation were higher than those in patients with moderate differentiation (P < 0.05). Compared with CA125, the specificity and positive predictive value of serum HE4 for the diagnosis of ovarian epithelial cancer were higher (both P < 0.01). Compared with HE4 alone, the sensitivity of CA125 combined with HE4 increased (P = 0.004) and the specificity decreased (P = 0.044). When both CA125 and HE4 were positive for positive results, compared with HE4 alone, the specificity and positive predictive value increased (both P < 0.01), but the sensitivity decreased (both P < 0.01). In patients with CA125+ HE4-, the sensitivity and specificity decreased, while in CA125- HE4+ patients, the specificity was elevated, but the difference was not statistically significant (P = 0.892). When one of CA125 and HE4 was positive for positive results, the sensitivity and negative predictive value increased (both P < 0.01), but the specificity and positive predictive value decreased (both P < 0.01). The area under the curve of serum CA125 and HE4 combined detection was 0.911, indicating its clinical diagnostic value was better than that of the two alone.@*Conclusions@#In the diagnosis of epithelial ovarian cancer, the specificity of serum HE4 is higher than that of CA125, but the sensitivity is lower than that of CA125. The combined detection of HE4 and CA125 is more conducive to improve the diagnostic accuracy of ovarian epithelial cancer.

ABSTRACT

Epithelial ovarian cancer (EOC) has the highest mortality rate among the gynecological malignancies. Because of its early symptoms are not obvious and lack of effective screening methods, more than 70% patients have been diagnosed at late stage (stage Ⅲ-Ⅳ). Although the 5-year progression-free survival (PFS) of EOC was prolonged with the improvement of treatment methods, its prognosis was still poor, and 50%-95% patients relapsed within 2 years after treatment. Human epididymis protein 4 (HE4) and carbohydrate antigen 125 (CA125) are widely used in the evaluation of diagnosis, treatment efficacy and prognosis of EOC, but the evaluation effect of both HE4 and CA125 is rarely reported. Therefore, it is necessary to summarize the research progress of HE4 and CA125 in evaluating the diagnosis, treatment efficacy and prognosis of EOC.

ABSTRACT

Objective To discuss the clinical value of IL-6 combined with CA125 and HE4 as a new association marker in the diagnosis of early ovarian cancer.Methods The expression of IL-6, CA125 and HE4 in 21 cases of early ovarian cancer (stageⅠ-Ⅱ), 36 cases of advanced ovarian cancer (stageⅢ-Ⅳ), 40 cases of benign ovarian tumor and 40 healthy women were measured by the Roche automatic chemiluminescence analyzer (electrochemiluminescence).The sensitⅣity, specificity, positive predictive value, negative predictive value and ROC curves were used to evaluate the diagnostic value.Results The serum levels of IL-6, CA125 and HE4 in patients with ovarian cancer were significantly higher than those in the benign ovarian tumor and the healthy control (P<0.05).The sensitⅣity, specificity, positive predictive value and negative predictive value of IL-6 combined with CA125 and HE4 were respectively 85.7%, 90%, 81.8%, 92.3%in the diagnosis of early ovarian cancer (stageⅠ-Ⅱ) patients and respectively 94.4%, 97.5%, 97.1%and 95.1%for advanced ovarian cancer (stageⅢ-Ⅳ) patients.For IL-6 combined with CA125 and HE4, the ROC AUC was respectively 0.955 4 and 0.974 0 for early ovarian cancer (stageⅠ-Ⅱ) patients and advanced ovarian cancer (stageⅢ-Ⅳ) patients.It performed significantly better than any single test of IL-6, CA125 and HE4 and the 2-marker combination of CA125+HE4.Conclusion The marker panel, IL-6, CA125 and HE4, shows higher sensitⅣity, positive predictive value and ROC AUC.It is an ideal serum marker combination for the diagnosis of early ovarian cancer (stageⅠ-Ⅱ) patients, which can improve the diagnostic efficiency of early ovarian cancer.

ABSTRACT

Objective To investigate the expression of human epididymal protein 4 ( HE4) and its effect on prognosis in patients with epithelial ovarian cancer (EOC). Methods From 2009 to 2014,ninety-eight cases of epithelial ovarian cancer (EOC group) and 29 cases of benign ovarian tumor (benign group) and 100 healthy subjects (control group) were selected as research subjects. The serum HE4 was detected by double antibody sandwich enzyme-linked immunosorbent assay (ELISA),and the positive expression was determined by HE4>150 pmol/L in serum. The concentration and positive rate of serum HE4 were compared among the three groups. The median serum HE4 level of EOC patients before treatment was divided into high level HE4 group and low level HE4 group. The median survival time of the two groups of EOC patients was compared. Results The median value of serum HE4 concentration (four percentile) in EOC group was 421. 8 (68. 7 ~ 2173. 9) pmol/L,47. 8 (35. 24 ~ 68. 5) pmol/L in the benign group and 43. 8 ( 32. 91 ~ 70. 4) pmol/ L in the control group. The serum concentration of HE4 in the EOC group was significantly higher than that in the benign group and the control group ( P=0. 000) . The serum HE4 concentration in group EOC was significantly higher than that in benign group and control group (P<0. 01),but there was no significant difference between benign group and control group (P>0. 05). The positive rate of serum HE4 in the EOC group was 74. 5 (73/98),the positive rate of the benign group and the control group were 0,the difference between the three groups was statistically significant (P=0. 000). The positive rate of serum HE4 in the EOC group was significantly higher than that in the benign group and the control group ( P<0. 01), while the benign group and the control group had no statistical significance (P>0. 05). The median survival time of EOC patients in low level HE4 group was 66. 2 months,while that in high level HE4 group was 36. 9. The difference between two groups was statistically significant (P=0. 001). Conclusion The serum HE4 level of EOC patients was significantly higher than that of patients with benign ovarian tumors and healthy controls. The higher serum HE4 level,the worse the prognosis, and the shorter the survival time of patients. Therefore,the expression level of serum HE4 can be used for the diagnosis and prognosis of ovarian cancer.

ABSTRACT

Objective To investigate the clinical value of serum ferritin (SF) combined with 4 tumor mark-ers in the early diagnosis of lung cancer in elderly patients .Methods 185 cases of elderly patients with early-stage lung cancer as lung cancer group ,60 cases of elderly patients with benign lung disease as benign lung disease group ,50 healthy people were enrolled as healthy control group ,compared three groups of subjects of serum SF ,carbohydrate antigen 125 (CA125 ) ,carcinoembryonic antigen (CEA ) ,neuron-specific enolase (NSE) ,cytokeratin 19 fragment antigen 21-1 (CYFRA21-1) expression and sensitivity ,specificity ,ROC curve analysis of the diagnostic index value .Results CA125 ,SF ,CEA ,NSE and CYFRA21-1 in lung cancer group , were higher than those of benign lung disease group and healthy control group ,the difference was statistically significant (P<0 .05) ;SF ,CA125 ,CEA ,NSE and CYFRA21-1 of the benign lung disease group and healthy control group ,which showed no difference (P>0 .05) .SF and NSE were the highest in patients with small cell lung cancer ,the difference was statistically significant (P<0 .05) ,CA125 and CEA were the highest in adeno-carcinoma patients ,the difference was statistically significant (P<0 .05) ,and CYFRA21-1 was the highest in squamous cell carcinoma patients ,the difference was statistically significant (P<0 .05) .Compared with the single diagnosis of each index ,the sensitivity of combined detection of the five indicators increased ,the differ-ence was statistically significant (P<0 .05) ,while the specificity did not change ,the difference was not statis-tically significant(P>0 .05) .The area under the ROC curve of SF ,CA125 ,CEA ,NSE and CYFRA21-1 was 0 .808 ,0 .762 ,0 .761 ,0 .712 and 0 .781 ,respectively .The area under the ROC curve of the 5 indexes was 0 .915 . Conclusion Serum ferritin combined with 4 tumor markers can improve the diagnostic sensitivity of early eld-erly patients with lung cancer ,and effectively improve the diagnostic value of patients ,thereby contributing to the improvement of the clinical prognosis of patients .

ABSTRACT

Objective To investigate the clinical value of combined detection of serum carcinoembryonic an-tigen(CEA) ,carbohydrate antigen-125 (CA125) ,cytokeratin 19 fragment antigen (CYFRA21-1) ,squamous cell carcinoma(SCC) antigen ,neuron-specific enolase(NSE) and plasma progastrin-releasing peptid(ProGRP) in the diagnosis ,pathological typing and clinical staging in lung cancer .Methods The serum CEA ,CA125 , CYFRA21-1 ,SCC ,NSE levels and plasma ProGRP level were detected by adopting the chemiluminescent mi-croparticle immunoassay method in 378 cases of lung cancer ,200 cases of benign lung diseases and 200 people undergoing healthy physical examination .Results The levels and positive rates of CEA ,CA125 ,CYFRA21-1 , SCC ,NSE and ProGRP in the patients with lung cancer were significantly higher than those in the patients with benign lung diseases and healthy control group ,the difference was statistically significant (P<0 .05);in the single index detection ,the biomarkers of highest positive rate in adenocarcinoma ,squamous cell carcinoma and small cell lung carcinoma were CEA ,CYFRA21-1 and SCC ,NSE and ProGRP respectively .The sensitivi-ty ,specificity ,accuracy ,negative predictive value and positive predictive value of combined detection of these 6 indexes were 92 .86% ,85 .00% ,88 .17% ,92 .64% and 85 .40% respectively ,except the specificity and positive predictive value were slightly decreased ,the levels of other indexes were higher than those of single index de-tection .The aresa under the receiver operating characteristic (ROC) curves of CEA ,CA125 ,CYFRA21-1 ,SCC ,NSE and ProGRP were 0 .775 ,0 .778 ,0 .891 ,0 .585 ,0 .710 and 0 .620 respectively .The area under ROC curves of the combined detection was 0 .950 .The positive rates of CA125 ,CYFRA21-1 ,NSE and the combined detection in the patients with stage Ⅲ ,Ⅳof lung cancer were obviously higher those in the patients with stageⅠand Ⅱof lung cancer ,the difference was statistically significant (P<0 .05);the combined detection obviously improved the positive rates for the diagnosis in the patients with different stages of lung cancer .Conclusion The combined detection of CEA ,CA125 ,CYFRA21-1 ,SCC ,NSE and ProGRP is conducive to improve the di-agnosis performance and early detection rate for lung cancer ,differentially diagnosing different pathological types of lung cancer and judge the clinical stage of lung cancer .The combined detection of 6 tumor biomarkers is an ideal diagnosis index of lung cancer .

Objective To observe the expression of human epididymis protein 4 (HE4) and carbohydrate antigen 125 (CA125) in endometrial carcinoma(EC) tissues,and to explore the relationship between the expression of HE4 and CA125 and the occurrence and development of EC.Methods The EC and paracancerous tissue specimens of 35 EC patients were selected from December 2015 to December 2016 in the First People's Hospital of Yibin City.The expression of HE4 and CA125 in EC tissues and paracancerous tissues was detected by immunohistochemistry.The relationship between the expression of HE4,CA125 and the clinicopathological features of EC was analyzed;and the correlation between HE4 and CA125 was analyzed too.Results The positive expression rates of HE4 and CA125 in EC tissues were 82.9% (29/35) and 74.3% (26/35);they were 11.4% (4/35) and 14.3% (5/35) in paracancerous tissues respectively;the positive expression rates of HE4and CA125 in EC tissues were significantly higher than those in paracancerous tissues(x2 =35.831,25.533;P <0.05).The positive expression rates of HE4 and CA125 in stage Ⅲ and Ⅳ EC tissues were 100.0% (20/20) and 90.0% (18/20);they were 60.0% (9/15) and 53.3% (8/15) in stage Ⅰ and Ⅱ EC tissues respectively;the positive expression rates of HE4 and CA125 in stage Ⅲ and Ⅳ EC tissues were significantly higher than those in stage Ⅰ and Ⅱ EC tissues(x2 =9.655,4.266;P < 0.05).The positive expression rates of HE4 and CA125 in medium and low differentiated EC tissues were 100.0% (18/18) and 83.3% (15/18);they were 64.7% (11/17) and 64.7% (11/17) in highly differentiated EC tissues respectively;the positive expression rates of HE4 and CA125 in medium and low differentiated EC tissues were significantly higher than those in highly differentiated EC tissues(x2 =7.667,4.137;P <0.05).The positive expression rates of HE4 and CA125 in EC tissues with myometrial invasion depth≥ 1/2 were 100.0% (18/18) and 94.4% (17/18);they were 64.7% (11/17) and 52.9%(9/17) in EC tissues with myometrial invasion depth < 1/2 respectively;the positive expression rates of HE4 and CA125 in EC tissues with myometrial invasion depth ≥ 1/2 were significandy higher than those in EC tissues with myometrial invasion depth < 1/2(x2 =7.667,7.884;P < 0.05).The positive expression rates of HE4 and CA125 in EC tissues with lymph node metastasis were 100.0% (19/19) and 78.9% (15/19);they were 62.5% (10/16) and 68.8% (11/16) in EC tissues without lymph node metastasis respectively;the positive expression rate of HE4 in EC tissues with lymph node metastasis was significantly higher than that in EC tissues without lymph node metastasis (x2 =8.599,P < 0.05),but there was no significant difference in the positive expression rate of CA125 in EC tissues with lymph node metastasis and without lymph node metastasis (x2 =0.473,P <0.05).The expression of HE4 and CA125 in EC tissues was not related to age,tumor size and pathological type(P < 0.05).The expression of HE4 in EC tissues was positively correlated with the expression of CA125 (r =0.608,P <0.05).Conclusion HE4 and CA125 may be involved in the development and development of EC.The expression of HE4 and CA125 is high in EC tissues,the expression level of them is closely related to clinical stage,histological differentiation and myometrial invasion depth of EC.The high expression of HE4 was also related to lymph node metastasis.

ABSTRACT

Objective To investigate the clinical value of serum human epididymis protein 4 (HE4), carbohydrate antigen 125 (CA125) combined with risk of ovarian malignancy algorithm (ROMA) index in the diagnosis of ovarian cancer. Methods A total of 541 patients in the Department of Gynaecology in Baoji Central Hospital from August 2016 to February 2018 were collected. The serum HE4 and CA125 levels were measured by using electrochemiluminescence immunoassay in 226 cases of ovarian cancer (ovarian cancer group), 315 cases of ovarian benign disease (ovarian benign disease group) and 100 female healthy people (the control group). ROMA index was calculated according to ROMA model and the results were analyzed statistically. Results The levels of serum HE4, CA125 and ROMA index were significantly different in premenopausal and postmenopausal ovarian cancer group, ovarian benign disease group and the control group (all P<0.05). The sensitivity and Yonden's index of the three combined diagnosis of serum HE4, CA125 and ROMA for premenopausal ovarian cancer were 97.01 % and 77.01 % respectively, which were higher than those of a single detection (HE4:79.37%, 69.11%;CA125:76.06%, 66.38%;ROMA index:89.55%, 72.41%;χ2sensitivity=12.35, P=0.000;χ2Yonden's index=6.460, P=0.013. The sensitivity and Yonden's index ofthe three combined diagnosis of serum HE4, CA125 and ROMA for postmenopausal ovarian cancer were 98.99 % and 82.99 % respectively, which were higher than those of a single detection (HE4: 86.90 %, 79.40 %; CA125: 82.98 %, 76.31 %; ROMA index: 93.54 %, 80.64 %; χ2sensitivity = 14.25, P = 0.000;χ2Yonden's index= 4.822, P= 0.031). The area under the curve of the combined detection of three indicators was higher than that of a single detection (premenopausal group: 0.871 vs. 0.682, 0.626, 0.708; postmenopausal group: 0.981 vs. 0.724, 0.705, 0.833), and there were significant differences (premenopausal group: χ2 =11.24, P= 0.000; postmenopausal group: χ2= 16.38, P= 0.000). Conclusion The combined detection of serum HE4, CA125 and ROMA index can increase the sensitivity and accuracy for diagnosing ovarian cancer, which can provide a more reliable basis for diagnosis and screening of ovarian cancer.

Results The positive rates of HE4 ,CA125 ,CA199 ,CA153 and AFP in the ovarian cancer group were 59 .42% ,68 .12% ,33 .33% , 46 .38% and 39 .13% ,respectively ,which were significantly higher than those in the benign ovarian lesion group and the control group (P<0 .05) .The sensitivity of CA125 in the diagnosis of ovarian cancer was 68 .11% ,the specificity was 88 .98% ,the nega-tive predictive value was 78 .33% ,the positive predictive value was 82 .68% ,Youden index was 0 .571 .The diagnostic efficiency was better than that of other tumor markers .Pathological examination revealed 34 cases of serous adenocarcinoma ,18 cases of mucinous adenocarcinoma and 17 cases of endometrioid carcinoma in 69 cases of ovarian cancer .The positive rate of serous adenocarcinoma CA125 was 85 .29% ,significantly higher than mucinous carcinoma and endometrioid carcinoma (χ2 =9 .398 ,P<0 .05) .Conclusion CA125 has a good application value in the early diagnosis of ovarian cancer ,the positive rate is higher in serous adenocarcinoma .

ABSTRACT

Objective To investigate the clinical value of combined detection of serum human epididymis protein 4(HE4),carbo-hydrate antigen 125(CA125)and tumor marker(CEA)in the diagnosis and staging of ovarian cancer.Methods 90 cases of ovarian cancer(ovarian cancer group),120 cases of benign ovarian disease(ovarian benign group)and 60 cases of healthy women(healthy control group)were selected from March 2014 to 2016 March in our hospital.The levels of HE4 and CA125 and CEA were detected by immunochemiluminescence assays,the results of the three groups were compared and analyzed.Results The positive rate of 3 indicators in the ovarian cancer group was significantly higher than that of the ovarian benign group and the healthy control group (P <0.05).The positive rates of combined detection of Ⅰ-Ⅳ in ovarian cancer were 81.2%,92.1%,97.4% and 100.0%,and CEA,CA125 and HE4 joint inspection in different stages of ovarian cancer positive rate is significantly higher than that of single in-dex positive detection rate.Conclusion The clinical value of HE4,CA125,CEA combined test for the diagnosis and staging of ovar-ian cancer is significant,it is worth to be further promoted.

ABSTRACT

Objective To investigate the separate and combined detection value of serum carbohydrate antigen 125 (CA125),human epididymis protein 4 (HE-4) and D dimer (D-dimer) in the diagnosis of ovarian cancer.Methods One hundred and twenty ovarian cancer patients were selected as the observation group,one hundred and twenty patients with ovarian benign tumor as the benign group,eighty women with healthy physical examination results as the control group,then the chemiluminescence immunoassay was used to detect the expression of serum CA125 and HE-4 of the three groups,and the immune turbidimetric method was applied to examine the expression of D-dimer.At last,the diagnosis efficiency of CA125,HE-4 and D-dimer in separate and combined detection was calculated.Results CA125 in the observation group was (623.07±274.18) U/ml,HE-4 was (594.22±329.068) ng/ml and D-dimer was (418.57±276.75) ng/L,CA125 in the benign group was (45.09±32.58) U/ml,HE-4 was (97.92±57.52) ng/ml and D-dimer was (204.52±80.07) ng/L;CA125 in the control group was (40.23±28.16) U/ ml,HE-4 was (85.65±37.27) ng/ml and D-dimer was (187.57±65.74) ng/L,the differences between the three groups were statistically significant (F=122.82,89.91,54.46;P<0.05).The level of CA125 in the serum of patients with advanced stage (stage III-IV) was (586.10±278.33) U/ml,HE-4 was (437.49±238.06) ng/ml,D-dimer was (493.78±274.45) ng/L,in the early stage (stage I-II),the level of CA125 in the serum of patients with ovarian cancer was (372.12±265.31) U/ml,HE-4 was (673.64±301.68) ng/ml,D-dimer was (364.84±267.54) ng/L,and the difference was statistically significant (t=4.244,4.78,2.560,P<0.05).The detection sensitivity and specificity of CA-125 were 71.67% and 62.50% respectively.The sensitivity of HE-4 was 74.17% and the specificity was 75.00%.The sensitivity of D-dimer was 62.5% and the specificity was 60.00%.The combined detection sensitivity of the three was 80.00% and the specificity was 77.5%.Conclusion The sensitivity and specificity of the three tumor markers in combined detection were higher than those of separate detections,so the combined detection of CA125,HE-4 and D-dimer can further improve the diagnosis level of ovarian cancer.

ABSTRACT

Objective To evaluate the clinical value of serum carcinoembryonic antigen(CEA),neuron specific enolization enzyme(NSE),21-1 non-small cell lung cancer associated antigen(CYFRA21-1),carbohydrate antigen 125(CA125) and ferritin(Fer) in the diagnosis of lung cancer.Methods The levels of CEA,NSE,CYFRA21-1,CA125 and Fer were measured by electrochemlium-inescence in 103 patients with lung cancer,32 patients with benign lung diseases and 40 healthy people.Results The serum levels of CEA,NSE,CYFRA21-1,CA125 and Fer in patients with lung cancer[(110.2±95.5)ng/mL,(50.6±43.4)ng/mL,(32.8±29.5)ng/L,(122.7±110.4)U/L,(854.6±497.2)ng/mL] were significantly higher than those in patients with benign lung diseases and those in healthy people(t=6.21,5.71,6.75,6.62,7.74,P<0.05;t=5.26,4.86,5.81,5.20,6.26,P<0.05).The sensitivity values of CEA,NSE,CYFRA21-1,CA125,Fer and the combined determination containing five tumor markers were 39.81%,24.27%,71.84%,68.93%,77.66% respectively.The sensitivity and specificity of the combined determination containing five tumormarkers were 96.12%,95.00%.Conclusion The joint detection in the diagnosis of lung cancer could improve the sensitivity significantly,to help for early diagnosis of lung cancer,which is value to widely applied in clinic.
