ABSTRACT

Background: The objective of this study was evaluation of symptomatic women with cervical erosion. Methods: Prospective clinical study done in the department of obstetrics and gynecology, VIMS, Ballari for one-year period from June 2016 to May 2017. All patients with symptoms of white discharge, post coital bleeding and menstrual irregularities were examined by per speculum examination and all those who had cervical erosion (112 Patients) were included in the study. Pap smear and colposcopy were done in all subjects. Colposcopic guided biopsy was done in women with abnormal findings on colposcopy. Histological prediction of colposcopic findings was done according to the Reid’s modified colposcopic index. The result of the biopsy was correlated with the predicted histology of the Reid’s modified colposcopic index and statistical analysis done to calculate diagnostic efficacy of colposcopy in the evaluation of symptomatic women with cervical erosion. Results: Out of the 112 patients, 5.35% (6/112) had abnormal Pap smear. All patients underwent colposcopy and colposcopic guided biopsy, 11.6% (13/112) had abnormal colposcopic findings. Histopathology confirmed CIN I in 6.25% (7/112) and CIN II in 1.78% (2/112) patients. The sensitivity, specificity, PPV, NPV and accuracy of PAP smear was found to be 22.22%, 96.11%, 33.33%, 93.39% and 90.17%. The above values for colposcopy were found to be 88.89%, 95.15%, 61.54%, 98.99% and 94.64%.Conclusions: Colposcopy should be done in all symptomatic patients with cervical erosion as it is a good diagnostic tool for premalignant conditions of the cervix and correlates well with histopathological findings.

ABSTRACT

CONTEXT AND PURPOSE: Uterine cervical ectopy (cervical erosion) is today considered to be a physiological condition, but there still seems to be a strong tendency towards treating it. The purpose of this study was to review the medical literature for evidence regarding benefits from treating cervical ectopy. METHODS: The following databases were reviewed: Medical Literature Analysis and Retrieval System Online (Medline), Excerpta Medica Database (Embase), Literatura Latino-Americana e do Caribe em Ciências da Saúde (Lilacs) and Cochrane Library databases. In addition, six medical textbooks were consulted. RESULTS: The review showed that: 1) there is probably an association between ectopy and higher risk of Chlamydia trachomatis, human papillomavirus and human immunodeficiency virus infection; 4) there is probably an association between ectopy and cervical intraepithelial neoplasia; 5) there is an association between ectopy and mucous discharge and nocturia; and 6) there is no evidence of an association between ectopy and cervical cancer, or of protection against cervical cancer associated with ectopy treatment. CONCLUSIONS: 1) No data were found in the medical literature to support routine treatment for ectopy; 2) Treatment could be recommended for symptom relief, but more symptoms are attributed to ectopy than could be demonstrated in a controlled study; 3) Further studies to test the hypothesis of protection against cervical cancer associated with treatment are necessary.

ABSTRACT

The significance of cervical erosion with benign cellular change on Papanicolaou smear has not been evaluated. A retrospective review of 430 coloscopically directed punch biopsies performed over 15 months revealed 206 cases who had a benign cellular change on cytologic smear. Of these 206 smears, 48(23%) had clear cervix without erosion, 99(48%) had mild erosion on cervix, 41(20%) had moderate cervical erosion and 18(9%) had severe cervical erosion. The false negative rate that revealed more than mild dysplasia on histologic diagnosis was 2% in patients without cervical erosion, 3% in patients with mild cervical erosion, 5 % in patients with moderate cervical erosion, and 11% in patients with severe cervical er- osion. Koilocytotic change that suggested human papilloma virus infection was 17% in each group regardless cervical erosion. In conclusion, benign cellular change on Pap smears in patients with severe cervical erosion may be associated with significant cervical cancer pathology.
