ABSTRACT

OBJECTIVE: To investigate the effective of acupoint thread-embedding therapy for ataxia children with cerebral palsy. METHODS: A total of 70 ataxia children with cerebral palsy and a Gross Motor Function Measure (GMFM, Dimension B) score of <35 were enrolled and randomly divided into thread-embedding group and control group, with 35 children in each group, and 2 children in the thread-embedding group were lost to follow-up. The children in the control group were given routine rehabilitation treatment, including physical therapy, spleen-strengthening, kidney-nourishing, and Governor Vessel-regulating massage, vibroacoustic therapy, and scalp acupuncture, and those in the thread-embedding group were given thread-embedding therapy at the acupoints of Jianyu (LI15), Jianliao (SJ14), Tianzong (SI11), Tianshu (ST25), and HuatuoJiaji points (C4, L1 and L4) in addition to the treatment in the control group, with 6-8 acupoints selected each time, once a week. Each course of treatment was 4 weeks, with an interval of one week between two courses of treatment, and the children were treated for 3 courses. Level of sitting scale (LSS), incurvation reflex, and GMFM score were recorded to evaluate the improvement in sitting ability and clinical outcome. RESULTS: Both groups had significant improvements in LSS, incurvation reflex, and GMFM (Dimension A and B) score after treatment (P<0.01), and the thread-embedding group had significantly greater improvements than the control group (P<0.01). The thread-embedding group had a significantly higher overall response rate than the control group (90.9% [30/33] vs 68.5% [24/35], P<0.01). CONCLUSION: In addition to routine rehabilitation treatment, acupoint thread-embedding therapy can effectively suppress primitive reflex in ataxia children with cerebral palsy and significantly promote their sitting ability, and therefore, it is an effective acupuncture treatment method for ataxia children with cerebral palsy.

ABSTRACT

@#The economic burden of children with cerebral palsy exceeds far beyond the needs of typical children leading to physical and mental stress to their parents. The study aims to examine the economic burden of parents of children with cerebral palsy in Malaysia. Calculation is made using a cost of illness approach due to cerebral palsy disorders that includes direct healthcare costs, direct non-healthcare costs, developmental costs and indirect costs. Using convenient sampling method, a total of 74 parents completed questionnaires through online or hard copies. Direct healthcare costs represent the highest cost of RM14, 715.49, followed by the developmental costs of RM10, 146.07, RM2, 674.00 for direct non-healthcare costs and RM2,175.20 for indirect costs. So the total cost of financing for the needs of a child with cerebral palsy is RM29, 710.76 per year. This is a huge and burdensome amount for their parents. The findings of this study may assist policy makers in their planning of effective service provision to suit the needs of parents of children with cerebral palsy in Malaysia.

ABSTRACT

Objective To observe the effect of team-based continuous nursing on improving the life function of children with cerebral palsy. Methods From March 2014 to March 2016, 35 children aged 18-36 months with cerebral palsy were selected as the experimental group;35 matched patients with the same age, same gender, same Gross Motor Function Classification System (GMFCS) and functionally similar were selected as the control group by matching design. Among them, 32 cases were followed up in the experimental group while 30 cases in the control group. In the control group, routine discharge education was adopted, but the experimental group was treated by team-based continuous nursing. The ability of daily living was evaluated by the Pediatric Evaluation of Disability Inventory (PEDI). Neuropsychological development was evaluated by the Gesell Development Scale and Body balance was evaluated by the Berg Balance Function Scale after discharge and also after 6 months follow-up. Results After the intervention for 6 months, the functional skill and assisting assistance of PEDI was (77.56 ± 23.58), (38.78 ± 13.96) points in the experimental group, and (65.12 ± 24.23), (30.35 ± 12.78) points in the control groups, there were significant differences between two groups(t=2.0485, 2.4750, P0.05) , but the experimental group was higher than the control group. The scores of Berg Balance Function Scale was(33.20 ± 4.12) points in the experimental group, and (29.67 ± 4.98) points in the control group, there were significant differences between two groups (t = 3.0488, P<0.05). Conclusions To improve the life function of children with cerebral palsy, the implementation of the team-based continuous nursing had a significant effect.

ABSTRACT

Performance of upper extremity function and movement sequence is influenced by postural control. Motor disorders lead to deficits in postural control, which subsequently may lead to postural instability of children with cerebral palsy (CWCP). This will limit their upper extremity activity performance. Management strategies help to support and enhance the CWCP’s upper extremity function so that they may engage with the activities of daily living. The purpose of this paper is to review previous literature on the influence of postural control towards upper extremity function. Literature searches were conducted in various electronic databases, including ProQuest, Science Direct, Springer Link, Sage, Wiley Online Library, and Google Scholar using specific key terms. Search terms included children with cerebral palsy; postural control; postural adjustments; upper extremity function; reaching and sitting and from references of retrieved articles. Nineteen journal articles published between 2000 and May 2015 were found. Most search results consisted of experimental studies, while others are reviews, case studies, and cross-sectional studies. Findings show that, postural control has a major influence on upper extremity function. In conclusion, it is necessary to highlight the importance of both factors to the CWCP parents or caregivers, as understanding and awareness on this matter is still inadequate in the community. Hence, a study is needed on the awareness of the postural control influence on upper extremity function among caregivers, as well as examining the implementation of management strategies in community settings.

ABSTRACT

At the time that they left the rehabilitation facility, we conducted a survey of mothers who had accompanied their children during their stay in the rehabilitation facility. We conducted a five-tiered evaluation of the extent of their satisfaction and of their answers to various questions and studied the significance of mothers accompanying their children during their stay in the rehabilitation facility. There were 334 respondents, and the response rate was 74%. According to the results, 91% were satisfied or somewhat satisfied ; 94% thought, or thought to a certain extent, "I was able to concentrate on engaging with my child more than at home"; 94% thought, or thought to a certain extent, "I gained a good understanding of how to handle childrearing and training at home"; 87% thought, or thought to a certain extent, "There was a positive change in my child, and I saw improvement in the way my child functioned"; and 87% thought, or thought to a certain extent, "It was good to be able to exchange information with other mothers and the staff." The importance of the role played in rehabilitation when mothers and children attended the rehabilitation facility together thus became clear. When a child's motor functions improve through intensive training, and when the mother's motivation regarding rehabilitation increases and her ability to handle the child improves, it is highly significant in many ways : improvement can be seen not only in motor development but also in psychological development. The child becomes easier to handle, the things that can be done in daily life, at home, increase ; and after leaving the facility, it is possible to link the advances made in the rehabilitation facility to setting and attaining new goals.
