ABSTRACT

As the overweight and obese population is growing, the incidence of obstructive sleep apnea is rising, and most of the cases are complicated with coronary heart disease and other cardiovascular diseases. The two diseases affect each other and seriously endanger the patients' health, becoming a major public health problem of global concern. It is of great clinical importance to explore the combination of Chinese and Western medicine in the prevention and treatment of coronary heart disease complicated with obstructive sleep apnea syndrome. Researchers have explored the relationship between the two based on traditional Chinese medicine(TCM) theory and found that the two diseases belong to the TCM disease categories of chest impediment and snoring, respectively, and their co-morbidity is associated with the abnormal physiological functions of the heart and lungs. The failure of the heart to govern blood leads to the generation of blood stasis, and that of the lung to govern Qi movement leads to the generation of phlegm. The accumulation of phlegm and blood stasis in the chest causes chest impediment and snoring due to obstruction of the airway. This paper discusses the internal linkage between the pathogenesis of coronary heart disease and obstructive sleep apnea syndrome in Chinese and Western medicine from the TCM theory of heart-lung correlation. Furthermore, this paper proposes the treatment principles of simultaneously treating the heart and lung and activating blood and resolving phlegm, aiming to provide a theoretical basis for the clinical prevention and treatment of coronary heart disease complicated with obstructive sleep apnea.

ABSTRACT

OBJECTIVE@#To investigate the efficacy of integrated Chinese and Western medicine extending the progression-free survival (PFS) and overall survival (OS) of limited-stage small cell lung cancer (LS-SCLC) patients after the first-line chemoradiotherapy.@*METHODS@#The data of 67 LS-SCLC patients who received combined treatment of CM and Western medicine (WM) between January 2013 and May 2020 at the outpatient clinic of Guang'anmen Hospital were retrospectively analyzed. Thirty-six LS-SCLC patients who received only WM treatment was used as the WM control group. The medical data of the two groups were statistically analyzed. Survival analysis was performed using the product-limit method (Kaplan-Meier analysis). The median OS and PFS were calculated, and survival curves were compared by the Log rank test. The cumulative survival rates at 1, 2, and 5 years were estimated by the life table analysis. Stratified survival analysis was performed between patients with different CM administration time.@*RESULTS@#The median PFS in the CM and WM combination treatment group and the WM group were 19 months (95% CI: 12.357-25.643) vs. 9 months (95% CI: 5.957-12.043), HR=0.43 (95% CI: 0.27-0.69, P<0.001), respectively. The median OS in the CM and WM combination group and the WM group were 34 months (95% CI could not be calculated) vs. 18.63 months (95% CI: 16.425-20.835), HR=0.40 (95% CI: 0.24-0.66, P<0.001), respectively. Similar results were obtained in the further stratified analysis of whether the duration of CM administration exceeded 18 and 24 months (P<0.001).@*CONCLUSION@#The combination treatment of CM and WM with continuing oral administration of CM treatment after the first-line chemoradiotherapy for LS-SCLC patients produced better prognosis, lower risks of progression, and longer survival than the WM treatment alone. ABSTRACT

With the acceleration of social rhythm, the progress of science and technology, and the increase of the number of phubbers, the incidence of cervical degenerative diseases is also increasing year by year. Cervical spondylotic radiculopathy(CSR), as one of the diseases induced by cervical degeneration, has seriously affected people's quality of life and physical and mental health. Traditional Chinese medicine(TCM) is effective in the treatment of CSR, but the theoretical and basic experimental evidence is not sufficient, and the current formulation standard of animal model is not unified. Therefore, based on the clinical guidelines of Chinese and Western medicine for CSR and the characteristics of the disease and syndrome, the author systematically summarized and analyzed the existing animal models, and found that the existing models of microvascular clamp nerve root compression method had a poor agreement with the the Chinese and Western medical guidelines, while the modeling methods of spinal canal insertion, autologous bone insertion compression, stainless steel column compression, and fixed frame cervical degeneration reflected a high degree of agreement in the Western medical guidelines. However, the Chinese medical diagnostic criteria were poorly matched. This indicates that the existing animal models of this disease show few TCM syndrome elements, and lack information collection and evaluation in animal behavioral evaluation similar to the four diagnoses of TCM. In conclusion, this paper aims to systematically evaluate the current status of animal model establishment of CSR based on the concept of combination of disease and syndrome, so as to provide a theoretical basis for the model establishment of CSR that is more consistent with clinical characteristics and symptoms of Chinese and Western medicine.

ABSTRACT

Objective To investigate the clinical effect of ambroxol hydrochloride combined with cough syrupintegrated traditional Chinese and Western Medicine on infantile cough.Methods Selected 200 cases of patients with cough who were treated in our hospital from January 2012 to December 2015,divided into two groups randomly,100 cases in observation group,100 cases in control group.The observation group was treated with ambroxol hydrochloride combined with cough syrup,the control group were treated with ambroxol hydrochloride.To observe the clinical symptoms and clinical manifestations of the patients after treatment,recorded the time of cough and the disappearance of cough,and to determine the effect of treatment.The lung function indexes of the two groups were detected:peak expiratory flow,forced vital capacity,forced expiratory volume in one second,maximal expiratory flow rate of 50% vital capacity and maximal expiratory flow rate of 25% vital capacity.All the patients were followed up for 6 months by telephone or clinic,the recurrence rate of the two groups was observed,and the number of children with asthma were recorded.Results The time of cough and the disappearance time of cough in observation group were significantly shorter than that of the control group (P < 0.05);the total effective rate of observation group was 93.00% (93/100),significantly higher than that of the control group 74.00% (74/100) (P < 0.05).After treatment,expiratory flow peak,forced vital capacity,one second forced expiratory volume,vital capacity 50% of maximum expiratory flow and 25% of vital capacity maximal expiratory flow of two groups were significantly increased (P < 0.05) and the observation group increased more significantly than the control group (P <0.05).After 6 months of follow-up,the control group had 22 cases of recurrence,the recurrence rate was 22.00%;the observation group had 6 cases of recurrence,the recurrence rate was 6.00%,the recurrence rate of the observation group was significantly lower than that of the control group (P < 0.05);the control group had 14 cases of children with asthma,the change rate was 14%;the observation group had 2 cases of children with asthma,the change rate was 2%,the change rate of the observation group was significantly lower than that of the control group (P < 0.05).Conclusion Ambroxol hydrochloride combined with cough syrup has better curative effect on infantile cough,can effectively improve children's lung function,reduce the rate of recurrence and change of asthma,which has high clinical application value.

ABSTRACT

It is very common to use Chinese medicine (CM) combined with Western medicine (WM) in clinical practice. The appropriate combination of CM with WM can reduce toxicity and enhance effects in order to make the best use of advantages and bypass the disadvantages. However, an inappropriate combination can not only affect the curative effect but even cause death. Therefore, strengthening the complementary advantages of the CM and WM to improve the therapeutic efficacy and reduce side effects has become an important research topic of clinical medicine and pharmacy. Many researchers try to clarify the effects of combining CM with WM on therapeutic efficacy and absorption, distribution, metabolism and excretion by pharmacodynamics and pharmacokinetics studies, providing evidence for clinical application. This review focuses on the new developments in the pharmacodynamics and pharmacokinetics of the combination of CM with WM in order to give references for clinical treatment.

ABSTRACT

Objective To assess the effectiveness of western routine treatments combined with Tongxinluo capsule for angina pectoris .Methods The randomized controlled trials(RCTs ) of western routine treatments combined with tongxinluo capsule for angina pectoris were collected through searching on computer from the following databases ,CNKI (2000 -2011 ) ,WanFang Data (2000 -2011) ,WeiPu Data(2000-2011) ,PubMed(2000-2011) and manual search relevant journals ,conference papers .Results A total of 11 studies were included and improvement rate of abnormal ECG were estimated in 9 studies and improvement rate of symptoms of angina were estimated in 11 studies .ECG clinical evaluation of the treatment group relative to the control group ,relative risk (RR) value was 1 .40 (95% CI:1 .28‐1 .54) ,P<0 .01 ,symptoms of angina clinical evaluation ,treatment group relative to the control group RR value of 1.25 (95% CI:1 .18‐1 .32) ,P<0 .01 .Only slight gastrointestinal discomfort were found in treatment group .Conclusion Tongxinluo capsule in combination on the basis of conventional western treatment could effectively improve the clinical efficacy and drug safety .

ABSTRACT

This article was aimed to study the therapeutic effects and anti-inflammatory effects of Chinese and west-ern medicine ointment on fungal infection animals. Guinea pig models which were infected with Trichophyton rubrum or Microsporum lanosum were applied to observe the effects of antifungal infections on She-Zhi Qin-Xian (SZQX) ointment by transdermal administration. The ointment is the combination of miconazole nitrate, snake oil and Sophora flavescens which is traditional Chinese medicine (TCM). Simultaneously, the models of croton oil-induced mouse ear swelling and carrageenan-induced rat paw swelling were established to observe the curative effect of SZQX ointment. The results showed that the SZQX ointment had a protective role on both guinea pig models. The microscopic scale of localized lesions in infected guinea pigs was significantly decreased (P< 0.01). The effect of SZQX ointment was better than the single using of Chinese or western medicine. The SZQX ointment was also able to significantly reduce the croton oil-induced mouse ear swelling and carrageenan-induced rat paw swelling (P< 0.05, or P< 0.01) with distinct anti-inflammatory effect. And the effect of SZQX ointment was superior to the single using of either Chinese or western medicine. It was concluded that the SZQX ointment had significant antifungal infection and anti-inflam-matory effect.

ABSTRACT

Objective To investigate the clinical efficacy of Tongluohuayu decoction treating ankylosing spondylitis (AS). Methods 100 cases of AS were randomly recruited into a treatment group ( 50 cases) and a control group ( 50 cases). In control group, aceclofenac enteric-coated tablets and methotrexatum were administered. On the basis of control group, the treatment group was taken with Tongluohuayu decoction in addition. Results After the treatment, abnormal indexes was improved markedly in the treatment group (P<0.01 or P<0.05 ) . The total effective rate (96%) in treatment group was significantly higher than the control group (70%) , while the adverse reactive rate ( 21.6%) was significantly lower than the control group (34%) . Conclusion Tongluohuayu decoction has good therapeutic effects in treating ankylosing spondylitis clinically.

ABSTRACT

[Objective]To investigate the clinical effect of treatment of complex Pilon's fractures with limited internal fixations and external fixators.[Method]Since 2000,78 cases of complex Pilon's fractures with limited internal fixations and external fixators were treated,of which male 51cases,female 27cases,aged 20~79 years old,average 36 yrs.According to Ovadia and Beals classification,type Ⅲ 27 cases,type Ⅳ 35 cases,type Ⅴ 16 cases,fresh fracture 53cases,old fracture 25 cases,close fracture 57 cases,open fracture 21 cases.They were all treated by Kirsschner wires and screws together with external fixators.External fixator were removed at 1.5~2 months after operation and splints were used and began ankle movement.[Result]All cases were followed up after operation.The following observation time was 1~3 years,average 20 months.All fractures were ostcal concrescence.Two cases were evaginate about 5 degree.Three cases were open injury and needed flap grafting rehabilitation because skin and soft tissue were compressed severely and necrotized.Traumatic artharitis occurred in 4 cases,nitric oxide-releasing anti-inflammatory drug was needed.One case was treated with arthrodesis.The functional evaluation was completed according to standard of American Orthopedic Association of Foot and Ankle.The excellent rate was 87.2%.[Conclusion]Limited internal fixation and external fixator is one of the effective way of treatment of Pilon's fracture because it can immobilize the sections of fracture and avoid infection of soft tissue and nonunion of fracture.
