ABSTRACT

ABSTRACT Objectives: to evaluate the knowledge of Community Health Agents about dementia before and after the training workshop for detecting signs of the disease. Methods: a quasi-experimental study with 33 community agents, in which sociodemographic information and knowledge about dementia were collected and assessed using the Alzheimer's Disease Knowledge Scale before and after the workshop. There were 10 weekly, online, synchronous meetings. The Student's t-test for related samples was used, and the effect size was calculated. Results: while the average score on the initial assessment, using the measurement instrument, was 16.3, it was 21.24 in the final assessment. An increase in the scale score was observed after participating in the workshop, with a value of 4.94. Conclusions: it is urgent to invest in the ongoing education of these professionals for greater awareness in the timely detection of dementia cases in primary care and awareness of potentially modifiable factors.


RESUMEN Objetivos: evaluar el conocimiento de los Agentes Comunitarios de Salud sobre la demencia antes y después de un taller de capacitación para la detección de signos de la enfermedad. Métodos: estudio cuasi experimental con 33 agentes comunitarios, en el que se recopilaron datos sociodemográficos y conocimiento sobre la demencia, evaluados mediante la Escala de Conocimiento de la Enfermedad de Alzheimer, antes y después del taller. Hubo 10 sesiones semanales en línea y sincrónicas. Se utilizó la prueba t de Student para muestras relacionadas y se calculó el tamaño del efecto. Resultados: mientras que la puntuación promedio en la evaluación inicial a través del instrumento de medición fue de 16,3, en la evaluación final fue de 21,24. Se observó un aumento en la puntuación de instrumento después de participar en el taller, con un valor de 4,94. Conclusiones: es urgente invertir en la educación continua de estos profesionales para aumentar la conciencia en la detección oportuna de casos de demencia en atención primaria y la concienciación sobre factores potencialmente modificables.


RESUMO Objetivos: avaliar o conhecimento dos Agentes Comunitários de Saúde sobre demência antes e após a oficina de capacitação para detecção de sinais da doença. Métodos: estudo quase-experimental com 33 agentes comunitários, no qual foram coletadas informações sociodemográficas e de conhecimento sobre demência, avaliadas pela Alzheimer's Disease Knowledge Scale, no pré e pós-oficina. Houve 10 encontros semanais, online e síncronos. Utilizou-se o teste t de Student para amostras relacionadas e calculou-se o tamanho do efeito. Resultados: enquanto a média de pontos na avaliação inicial, por meio do instrumento de medida, foi de 16,3, na avaliação final foi de 21,24. Observou-se aumento na pontuação na escala após a participação na oficina, com um valor de 4,94. Conclusões: é urgente investir na educação permanente desses profissionais para maior conscientização na detecção oportuna de casos de demência ainda na atenção básica e conscientização de fatores potencialmente modificáveis.

Abstract Introduction Community health workers, like the physiotherapist, perform essential functions in primary health care, being an important element in the transformation of public policies. There are no reported studies investigating the knowledge of community health workers about health conditions amenable to physiotherapy intervention in primary health care. Objective To construct a questionnaire to investigate the perception of community health workers about health conditions that could be remedied by physiotherapy intervention in primary health care. Methods This was a methodological study in which it was initially an analysis matrix with the aim of encompassing the ideas contemplated in the questionnaire. To construct the instrument, a literature review was carried out, and health conditions treatable with physiotherapy in primary health care were chosen. To validate the content and appearance of the items, twelve physiotherapists specialized in primary health care judged the suitability of the items contained. The content validity index was used to determine the degree of agreement during the response analysis process. Subsequently, a semantic analysis was carried out through the understanding of the items by 15 community health workers. In the validation stage, two rounds of evaluation were carried out. Adjustments were made to 17 questions. Results The study investigated a questionnaire with 20 questions containing hypothetical situations of home visits, in which the resident's situation could or could not constitute a health risk amenable to physiotherapeutic intervention. Conclusion The community health workers perception instrument on health conditions amenable to physiotherapy intervention in primary health care proved to be valid for use in this context. The use of the instrument may contribute to the development of community health worker training programs, with the aim of facilitating team communication.


Resumo Introdução O Agente Comunitário de Saúde (ACS), assim como o fisioterapeuta, desencadeia funções fundamentais na Atenção Primária à Saúde (APS), sendo ele-mento importante na transformação de políticas públicas. Inexistem estudos que investiguem o conhecimento dos ACS sobre as condições de saúde sensíveis à inter-venção da fisioterapia na APS. Objetivo Construir um questionário de investigação da percepção dos ACS sobre as condições de saúde sensíveis à intervenção da fisioterapia na APS. Métodos Trata-se de um estudo metodológico no qual, inicialmente, construiu-se uma matriz de análise com o intuito de englobar as ideias contempladas no questionário. Para a construção do instrumento, realizou-se uma revisão da literatura, sendo eleitas condições de saúde sensíveis à intervenção da fisioterapia na APS. Para a validação de conteúdo e aparente dos itens, 12 fisioterapeutas especialistas em APS julgaram a adequação dos itens contidos. Utilizou-se o índice de validade de conteúdo para verificar o grau de concordância durante o processo de análise das respos-tas. Posteriormente, realizou-se análise semântica por meio da compreensão dos itens por 15 ACS. Na etapa de validação, foram realizadas duas rodadas de avaliação. Foram feitos ajustes em 17 questões. Resultados O estudo resultou em um questionário com 20 questões contendo situações hipotéticas de visitas domiciliares, cuja situação do morador poderia ou não configurar um risco à saúde sensível à intervenção fisioterapêutica. Conclusão O instrumento de percepção dos ACS sobre as condições de saúde sensíveis à intervenção da fisioterapia na APS mostrou-se válido para ser utilizado no contexto da APS. A utilização do instrumento poderá contribuir na elaboração de programas de capacitação dos ACS, com o intuito de facilitar a comunicação da equipe.

Background: Despite measures put in place to combat teenage pregnancy, the rate remains high. Community health workers (CHWs) are a cadre of health workers that can help put measures in place to reduce teenage pregnancy in the communities in which they live and work. Aim: This article aims to gain a deeper understanding of CHWs' perceptions regarding teenage pregnancy in the rural districts of Limpopo province. Methods: An exploratory qualitative study approach was employed to collect data from CHWs in two rural districts of Limpopo. A non-probability purposive sampling approach was used to choose 81 CHWs. Eight focus group discussions (FGDs) were organised, and audio recorded to collect data from participants. The discussions were 2­3 h long and conducted in English, and data saturation was attained by the fifth FGDs. Results: An eight-step tech's content analysis approach was employed to deductively code, analyse and summarise data into themes. Three themes emerged: the prevalence of teenage pregnancy in rural villages, factors contributing to teenage pregnancy and challenges faced by CHWs when dealing with teenage pregnancy. Conclusion: The study's findings revealed that CHWs face challenges in their communities when offering appropriate teen pregnancy services and CHWs believe that teen pregnancy numbers remain high. There is a significant barrier in combating teenage pregnancy; if contraceptives are not acceptable to the community, the only solution and option for combating teenage pregnancy is abstinence. Contribution: The CHWs presented their insights of teenage pregnancy in rural communities. The outcomes of this study could help clinical practise, schools, communities, youth-friendly services, policymakers and other non-governmental organisations reduce teenage pregnancy.

Abstract Objective: to identify the vaccination and serological status against hepatitis B among community health workers; to vaccinate against hepatitis B virus and to evaluate the immune response of susceptible workers. Method: phase I, cross-sectional and descriptive study, among community health workers in a capital city of the Midwest region, through a self-administered questionnaire, checking of vaccination cards, and blood collection for testing of serological markers for hepatitis B. Phase II, cohort study carried out in vaccinated non-immune workers identified in phase I. They received one dose of vaccine (challenge dose) and serological testing. Results: a total of 109 workers participated in the study. Most had vaccination record (97; 89.0%) and vaccination completeness (75; 77.3%), while the isolated anti-HBs (Antibodies against hepatitis B virus) marker was detected in 78 (71.6%) workers. The prevalence of hepatitis B virus exposure was 8.2%. Of the ten non-immune vaccinated workers, after challenge dose, one remained susceptible. Conclusion: although most workers are vaccinated and show immunological response to hepatitis B, susceptibility after challenge dose was identified. Therefore, it is necessary to have a surveillance program of the vaccination situation and serological status for this virus, to promote these workers' safety.


Resumo Objetivo: identificar a situação vacinal e sorológica contra hepatite B entre agentes comunitários de saúde; vacinar contra o vírus da hepatite B e avaliar a resposta imunológica dos agentes susceptíveis. Método: fase I, estudo transversal e descritivo, entre agentes comunitários de saúde de uma capital da região Centro-oeste, por meio de questionário autoaplicável, conferência do cartão vacinal e coleta de sangue para testagem dos marcadores sorológicos para hepatite B. Fase II, estudo de coorte realizado em trabalhadores vacinados não imunes e identificados na fase I. Estes receberam uma dose da vacina (dose desafio) e teste sorológico. Resultados: participaram do estudo 109 agentes. A maioria tinha registro de vacinação (97; 89,0%) e completude vacinal (75; 77,3%), já o marcador anti-HBs (anticorpos contra o vírus da hepatite B) isolado foi detectado em 78 (71,6%) agentes. A prevalência de exposição ao vírus da hepatite B foi de 8,2%. Dos dez agentes vacinados não imunes, após a dose desafio, um permaneceu susceptível. Conclusão: apesar da maioria dos trabalhadores estarem vacinados e apresentarem resposta imunológica para hepatite B, a suscetibilidade após a dose desafio foi identificada. Portanto, é necessário que haja um programa de vigilância da situação vacinal e estado sorológico para este vírus, para promover a segurança destes trabalhadores.


Resumen Objetivo: identificar la situación de la vacunación y serología contra la hepatitis B entre agentes comunitarios de la salud, vacunar contra el virus de la hepatitis B y evaluar la respuesta inmunológica de los agentes susceptibles. Método: fase I, estudio transversal y descriptivo, entre agentes comunitarios de la salud de una capital de la región centro oeste, por medio de cuestionario autoadministrado, verificación del carné de vacunación y extracción de sangre para comprobar los marcadores serológicos para la hepatitis B. Fase II, estudio de cohorte realizado en trabajadores vacunados no inmunes e identificados en la Fase I; estos recibieron una dosis de la vacuna (dosis de desafío) y realizaron el test serológico. Resultados: participaron del estudio 109 agentes. La mayoría tenía registro de vacunación (97; 89,0%) y de cobertura de vacunación (75; 77,3%); el marcador anti-HBs (Anticuerpos contra el virus de la hepatitis B) aislado fue detectado en 78 (71,6%) de los agentes. La prevalencia de exposición al virus de la hepatitis B fue de 8,2%. De los diez agentes vacunados no inmunes, después de la dosis desafío, uno permaneció susceptible. Conclusión: a pesar de que la mayoría de los trabajadores estaban vacunados y presentaron respuesta inmunológica para la hepatitis B, la susceptibilidad, después de la dosis desafío, fue identificada. Por tanto, es necesario que exista un programa de vigilancia de la situación de vacunación y estado serológico para este virus, para promover la seguridad de estos trabajadores.

ABSTRACT

Objetivo: descrever conhecimento, uso, forma de preparação, indicação para problemas de saúde de plantas medicinais entre Agentes Comunitários de Saúde ribeirinhos. Método: estudo descritivo, transversal, entre 117 Agentes Comunitários de Saúde Ribeirinhos, realizado entre agosto a novembro de 2018, sobre os hábitos de consumo de plantas medicinais, uso associado a medicamentos industrializados, aprendizado, parte utilizada, forma de preparação, indicação e problemas de saúde. O estudo foi aprovado pelo Comitê de Ética em Pesquisa. Resultados: as plantas medicinais mais utilizadas foram o Capim Santo (Cymbopogon citratus) (11,1%), Erva Cidreira (Melissa officinalis) (9,3%) e o Boldo (Vernonia condensata) (6,5%). Houve associação significativa entre consumo de plantas e faixa etária, escolaridade, problemas de saúde e plantas no quintal. Conclusão: o desenvolvimento de estratégias de promoção do conhecimento sobre plantas medicinais e seu uso adequado entre os ACS ribeirinhos, pode promover o autocuidado com a saúde nas áreas remotas da Amazônia brasileira.


Objective: to describe knowledge, use, form of preparation and indication for health problems of medicinal plants among community health agents from the riverside region. Method: a descriptive, cross-sectional study among 117 community health agents from the riverside, carried out between August and November 2018, on the habits of consumption of medicinal plants, use associated with industrialized medicines, learning, part used, form of preparation, indication and health problems. The study was approved by the Research Ethics Committee. Results: the most commonly used medicinal plants were Capim Santo (Cymbopogon citratus) (11.1%), Erva Cidreira (Melissa officinalis) (9.3%) and Boldo (Vernonia condensata) (6.5%). There was a significant association between plant consumption and age, schooling, health problems and plants in the yard. Conclusion: the development of strategies to promote knowledge about medicinal plants and their appropriate use among riverine CHAs can promote self-care in remote areas of the Brazilian Amazon.


Objetivo: Describir el conocimiento, el uso, la forma de preparación e indicación para problemas de salud de plantas medicinales entre agentes comunitarios de salud de la región ribereña. Método: estudio descriptivo, transversal, entre 117 agentes comunitarios de salud ribereños, realizado entre agosto y noviembre de 2018, sobre los hábitos de consumo de plantas medicinales, uso asociado a medicamentos industrializados, aprendizaje, parte utilizada, forma de preparación, indicación y problemas de salud. El Comité de Ética de Investigación aprobó el estudio. Resultados: las plantas medicinales más utilizadas fueron Zacate limón (Cymbopogon citratus) (11,1%), Melisa (Melissa officinalis) (9,3%) y Boldo de Bahía (Vernonia condensata) (6,5%). Hubo una asociación significativa entre el consumo de plantas y la edad, la escolaridad, los problemas de salud y las plantas que tenían en su patio. Conclusión: El desarrollo de estrategias para promover el conocimiento sobre plantas medicinales y su uso apropiado entre los ACS ribereños puede promover el autocuidado respecto a la salud en áreas remotas de la Amazonia brasileña.

ABSTRACT

Objetivo: analisar o conhecimento sobre aleitamento materno e os fatores associados a esse entendimento entre agentes comunitários de saúde. Método: estudo quantitativo, descritivo-analítico, transversal, realizado com 153 ACS de Senhor do Bonfim, Bahia. Para a verificação das variáveis associadas ao conhecimento sobre aleitamento, realizou-se análise bivariada (Odds Ratio, Intervalo de Confiança 95% e Testes Qui-quadrado/Fisher) e análise ajustada através da Regressão Logística Múltipla (Stepwise). Resultados: o estudo mostrou que os agentes comunitários de saúde apresentam elevado nível de conhecimento sobre aleitamento materno. Houve associação entre o conhecimento sobre aleitamento materno e ter filhos, trabalhar na Estratégia de Saúde da Família, ter participado de capacitação e alto conhecimento sobre as atribuições gerais da profissão. Conclusão: Os profissionais possuem adequado entendimento sobre a temática. Todavia, importantes equívocos foram observados e caso repassados para a comunidade podem prejudicar a instalação e manutenção do aleitamento materno.


Objective: to analyze knowledge about breastfeeding and the factors associated with this understanding among community health agents. Method: quantitative, descriptive-analytical, cross-sectional study, carried out with 153 CHAs from Senhor do Bonfim, Bahia. To verify the variables associated with knowledge about breastfeeding, bivariate analysis was carried out (Odds Ratio, 95% Confidence Interval and Chi-square/Fisher Tests) and adjusted analysis using Multiple Logistic Regression (Stepwise). Results: the study showed that community health agents have a high level of knowledge about breastfeeding. There was an association between knowledge about breastfeeding and having children, working in the Family Health Strategy, having participated in training and high knowledge about the general responsibilities of the profession. Conclusion: Professionals have an adequate understanding of the topic. However, important mistakes were observed and, if passed on to the community, they could harm the installation and maintenance of breastfeeding.

Método: estudio cuantitativo, descriptivo-analítico, transversal, realizado con 153 ACS del Senhor do Bonfim, Bahía. Para verificar las variables asociadas al conocimiento sobre lactancia materna, se realizó análisis bivariado (Odds Ratio, Intervalo de Confianza del 95% y Pruebas Chi-cuadrado/Fisher) y análisis ajustado mediante Regresión Logística Múltiple (Stepwise). Resultados: el estudio demostró que los agentes comunitarios de salud tienen un alto nivel de conocimiento sobre la lactancia materna. Hubo asociación entre conocimiento sobre lactancia materna y tener hijos, trabajar en la Estrategia Salud de la Familia, haber participado en capacitaciones y alto conocimiento sobre las responsabilidades generales de la profesión. Conclusión: Los profesionales tienen un conocimiento adecuado del tema. Sin embargo, se observaron errores importantes que, de transmitirse a la comunidad, podrían perjudicar la instalación y el mantenimiento de la lactancia materna.

ABSTRACT

En el presente artículo compartimos los avances de la investigación que estamos impulsando en la Universidad Nacional de Quilmes, desde el proyecto de Emprendimientos sociales y salud comunitaria. En el objetivo general, nos propusimos estudiar en profundidad las transformaciones subjetivas que ocurren en el marco de las experiencias de cooperación social, con el fin de identificar y comprender los elementos que condicionan su expansión, así como aquellos tendientes al fortalecimiento del campo de la Economía Social y Solidaria (ESS). En ese sentido, abordamos la temática de la cooperación social entendiendo que la misma a través de experiencias socio-económicas favorece el acceso a los derechos - trabajo, educación y salud- de las personas en situación de vulnerabilidad. Del amplio campo de la cooperación social nos focalizamos más puntualmente en el estudio de las empresas sociales, es decir, esas organizaciones asociativas que realizan una actividad económica regular de producción de bienes o prestación de servicios con una definida finalidad social para la comunidad y la integración social de las personas, particularmente de los grupos socialmente vulnerables y vulnerados.Entre las reflexiones finales que venimos construyendo destacamos que entre los testimonios de les emprendedores, lo colectivo surge como estructurante de la organización, en tal sentido es acto participativo, es nosotres, horizontalidad y confianza. Ese nosotres es inclusivo y diverso. Se considera saludable ya que da marco a lo que se construye y a su vez oficia como un espacio simbólico productor de procesos identificatorios de les emprendedores con la organización.


Neste artigo queremos compartilhar o andamento da pesquisa que estamos realizando na Universidade Nacional de Quilmes, a partir do projeto Empreendedorismo Social e Saúde Comunitária. No objetivo geral deste projeto, nos propusemos a estudar em profundidade as transformações subjetivas que ocorrem no âmbito das experiências de cooperação social, a fim de identificar e compreender os elementos que condicionam sua expansão, bem como aqueles que tendem a fortalecer o campo da Economia Social e Solidária (ESS). Nesse sentido, abordamos a questão da cooperação social, entendendo que ela, por meio de experiências socioeconômicas, favorece o acesso a direitos - trabalho, educação e saúde - de pessoas em situação de vulnerabilidade. Do amplo campo da cooperação social, focamos mais especificamente no estudo do que se convencionou chamar de empresas sociais, ou seja, aquelas organizações associativas que desenvolvem uma atividade econômica regular de produção de bens ou prestação de serviços com finalidade social definida comunidade e a integração social das pessoas, em particular dos grupos socialmente vulneráveis e vulneráveis. Dentre as reflexões finais que fomos construindo, nos interessa destacar que dentre os depoimentos dos empreendedores, o coletivo surge como estruturante da organização, nesse sentido é um ato participativo, somos nós, horizontalidade e confiança. Que somos inclusivos e diversos. É considerado salutar por fornecer um arcabouço para o que é construído e, por sua vez, atuar como um espaço simbólico que produz processos de identificação dos empreendedores com a organização.


In this article we want to share the progress of the research that we are carrying out at the National University of Quilmes, from the Social Entrepreneurship and Community Health project. In the general objective of this project, we proposed to study in depth the subjective transformations that occur within the framework of social cooperation experiences, in order to identify and understand the elements that condition its expansion, as well as those tending to strengthen the field of the Social and Solidarity Economy (ESS). In this sense, we address the issue of social cooperation, understanding that it through socio-economic experiences favors access to rights - work, education and health - of people in vulnerable situations. From the broad field of social cooperation, we focus more specifically on the study of social enterprises, that is, those associative organizations that carry out a regular economic activity of producing goods or providing services with a defined social purpose, for the community and the social integration of people, particularly of socially vulnerable and vulnerable groups. Among the final reflections that we have been building, we are interested in highlighting that among the testimonies of the entrepreneurs, the collective emerges as a structuring of the organization, in this sense it is a participatory act, it is us, horizontality and trust. We are inclusive and diverse. It is considered healthy since it provides a framework for what is built and, in turn, acts as a symbolic space that produces identification processes of the entrepreneurs with the organization.

ABSTRACT

Fundamento: El panorama económico y epidemiológico ha repercutido en la salud bucal de las comunidades rurales. Objetivo: Describir el estado de salud bucal, así como identificar factores de riesgo en 16 campesinos de la comunidad rural La Picadora. Metodología: En 2022 se realizó un estudio transversal descriptivo en 16 campesinos de la comunidad rural La Picadora, Yaguajay, Sancti Spíritus. Se hicieron entrevistas y examen bucal. Se calcularon indicadores epidemiológicos estomatológicos en el SPSS 16 y se resumieron los factores de riesgo mediante estadística descriptiva. Resultados: El índice de COP-D fue de 10.7 con predominio de dientes perdidos, el índice gingival fue de 1.4 y el de placa bacteriana de 1.0. Fue frecuente el desgaste de incipiente a moderado en menores de 60 años y de moderado a severo en mayores de 60 años. El 75 % de la muestra presentó oclusión normal o maloclusión mínima y el 94 % necesitó rehabilitación protésica. El 81.2 % consumió bebidas alcohólicas en los últimos 30 días, el 62.5 % ha fumado o fuma actualmente, el 87.5 % refirió el cepillado diario menor de 3 veces y el 43.8 % no asistió al estomatólogo en los últimos 10 años. Conclusiones: Se identificó la necesidad de atención estomatológica, así como la urgencia de acciones de prevención y promoción en esta comunidad.


Background: The economic and epidemiological panorama has impacted on oral health in rural communities. Objective: To describe the oral health status, as well as to identify risk factors in 16 farmers from the La Picadora rural community. Methodology: In 2022, a descriptive cross-sectional study was conducted in 16 farmers in the La Picadora rural community, Yaguajay, Sancti Spíritus. Interviews and oral examination were performed. Stomatological epidemiological indicators in SPSS 16 were calculated and risk factors were summarized using descriptive statistics. Results: The COP-D index was 10.7 with a predominance of missing teeth, The COP-D index was 10.7 with a predominance of missing teeth, the gingival index was 1.4 and the bacterial plaque index was 1.0. Incipient to moderate wear was frequent in people under 60 years of age and moderate to severe in over 60 years of age. 75% of the sample presented normal occlusion or minimal malocclusion and 94% required prosthetic rehabilitation. 81.2% consumed alcoholic beverages in the last 30 days, 62.5% have smoked or currently smokes, 87.5% referred the daily brushing less than 3 times a day and 43.8% did not attend a stomatologist in the last 10 years. Conclusions: The need for stomatological attention, was identified, as well as the urgency for prevention and promotion actions in this community.

ABSTRACT

Resumo O objetivo deste estudo foi analisar a prevalência de ansiedade e depressão associada às condições de trabalho dos agentes comunitários de saúde (ACS) no contexto da pandemia de COVID-19. Estudo transversal, realizado no norte de Minas Gerais. Foi aplicado um questionário que abordava condições sociodemográficas, domínio de tecnologias e condições de trabalho durante a pandemia, além do Inventário de Ansiedade Traço-Estado e do Patient Health Questionnare-9 (PHQ-9). Foram realizadas análises descritivas e de regressão múltipla de Poisson com variância robusta, considerando um nível de significância de 5% (p < 0,05) para o modelo final. Participaram do estudo 1.220 ACS de 36 municípios. A prevalência foi de 41,8% e 31,6% para ansiedade e depressão, respectivamente, e 22,5% para ambas as condições. O sexo feminino, idade até 40 anos, ter mais de 500 usuários cadastrados para acompanhamento e oferta inadequada de equipamentos de proteção individual estiveram associados à ansiedade e à depressão. Registrouse uma elevada prevalência de ansiedade e depressão entre ACS no período da pandemia, com destaque para as condições de trabalho entre os fatores associados.


Abstract This study examined the prevalence of anxiety and depression associated with Community Health Workers' (CHWs') working conditions in the context of the COVID-19 pandemic. This cross-sectional study was carried out in the north of Minas Gerais, Brazil by applying a questionnaire addressing sociodemographic conditions, command of technologies and working conditions during the pandemic, as well as the State-Trait Anxiety Inventory and the Patient Health Questionnare-9 (PHQ-9). Descriptive and multiple Poisson regression analyses were performed with robust variance, to a 5% level of significance (p < 0.05) for the final model. A total of 1,220 CHWs from 36 municipalities participated in the study. Prevalences were 41.8% and 31.6% for anxiety and depression, respectively, while 22.5% of the group displayed symptoms of both conditions. Being female, up to 40 years old, having more than 500 users registered for monitoring, and inadequate supply of personal protective equipment were associated with anxiety and depression among the CHWs. Prevalence of anxiety and depression was high among CHWs during the pandemic and working conditions figured prominently among the associated factors.

ABSTRACT

Purpose: During the coronavirus disease 2019 (COVID?19) pandemic, private practice, inpatient consult services, and academic residency programs in ophthalmology saw a decrease in patient encounters. This study elucidates how community hospital ophthalmology consult (OC) services were affected during the pandemic. We aim to determine whether there was a change in resident OC volume in a community?based ophthalmology program consult service during the COVID?19 pandemic. Secondary objectives included analyzing the change in the types of diagnoses and the number of patients seen for diabetic retinopathy over the same time. Methods: A retrospective cross?sectional study was conducted reviewing the electronic health record (EHR) charts from OCs for the period 2017–2021. Records were categorized by referral source and the nature of OCs (trauma, acute, or chronic); OCs were further grouped by year and weak of referral. An intermonth analysis of weekly OC counts in each category was performed for the average number of consults in February–April 2017–2019 and for February–April 2020. A one?tailed t?test was performed. All t?tests assumed equal variances. Results: Weekly OCs in 2020 revealed no statistically significant differences in overall cases or in acute or chronic cases when the volume before the COVID?19 pandemic was compared to the volume after the onset of the pandemic. However, a statistically significant increase in the average weekly trauma cases was noted when 2020 (an average of 2.7 cases per week) was compared to the weekly average for the same weeks of years 2017– 2019 (0.4; P = 0.016). This statistically significant increase in trauma in 2020 disappeared when comparing weeks 11–17 in 2020 (2.2 cases per week) and the average of 2017–2019 (1.1).Conclusion: This report outlines no significant change in OCs before and after the onset of the pandemic compared to three previous years. There was, however, an increase in trauma consults during the pandemic and an increase in the number (though not the proportion) of diabetic retinopathy (DR+) patients seen by residents. This report uniquely describes no significant changes in the resident volume of patients seen during the COVID?19 global pandemic

Resumo Este estudo objetivou descrever a estrutura representacional de agentes comunitários de saúde sobre violência doméstica contra a mulher. Pesquisa quanti-qualitativa fundamentada na Teoria das Representações Sociais na abordagem estrutural. Participaram 107 agentes comunitárias de saúde vinculadas a 18 Unidades de Saúde da Família de um município do interior da Bahia, Brasil, durante os meses de maio a agosto de 2019. A coleta de dados ocorreu por meio das técnicas de evocações livres e de centralidade: choix-par-bloc, constituição de pares de palavras e mise-en-cause. Para a análise dos dados, utilizou-se o software EVOC (Ensemble de Programmes Permettant I'analyse des Evocations), análise de similitude e análise do mise-en-cause. A estrutura representacional das profissionais se organiza a partir dos elementos centrais desrespeito e tristeza, que atribuem à representação sentidos negativos relativos a seus posicionamentos e suas repercussões. Conclui-se que a organização do pensamento social das agentes comunitárias sobre o fenômeno apresenta uma dimensão atitudinal e afetiva que fortalece a elaboração de estratégias de enfrentamento a situação de violência contra mulheres, agressores e comunidade.


Abstract This study aimed at describing community health agents' representational structure on domestic violence against women. A quantitative-qualitative research study based on the Theory of Social Representations in its structural approach and carried out in a municipality from inland Bahia, Brazil. The community health agents participated through free evocation and centrality techniques: choix-par-bloc, constitution of word pairs and mise-en-cause, from May to August 2019. Data analysis was carried out by means of the EVOC software (Ensemble of Programs Permettant I'analyse des Evocations), similarity analysis and mise-en-cause analysis. These professionals' representational structure is organized from the central elements of disrespect and sadness, which attribute negative meanings to the representation regarding their positions and repercussions; the other elements integrate specific information to the structure of the representations, justifying them. It is concluded that the understanding regarding organization of the community agents' social thinking about the phenomenon allows its problematization, as well as the elaboration of prevention and coping strategies for women in situations of violence, the aggressors and the community.

Background: Mental health is important at every stage of life, from childhood and adolescence through adulthood. Worldwide interest in geriatric depression has increased but studies to assess the depression among the elderly population in urban slums has hardly been done. So this study would shed light on the risk factors of depression among geriatrics in urban slums. Aim & Objective: To study the epidemiological determinants such as age, marital status, education, financial dependency, economic status and chronic illness in relation to of depression among the geriatric population (60–80 years). Settings and Design: Community-based, cross-sectional study for 24 months with a samplesize of 209. Methods & Material: Urban slum area represented by one health post was selectedas the study area which caters to around 96,630 slum residents. Pre-tested semi-structuredinterviews were conducted after selecting households by systematic random sampling. Statistical analysis: Data was analyzed using SPSS version 21. A chi-square test was applied. Result: The present study shows overall prevalence of depression among the elderly above 60 years of age to be 48.6%. Depression is found to been significantly associated with an increase in age, living alone (72.7%), lower class economic status (100%), financial dependency (54%), history of chronic family illness (54.7%) and with no formal education (62.1%) among elderly dwelling in slums. Conclusions: There is a high prevalence of depression in the community and hence, a need to sensitize Primary level Health Care Services to institute screening tests considering the association of these determinants with depression and link them to specialized services for further management.

RESUMO Objetivo Analisar os indicadores de prazer e sofrimento no trabalho associados a depressão e ansiedade entre agentes comunitários de saúde (ACSs). Métodos Estudo transversal, no qual variáveis dependentes foram a presença de sintomas de ansiedade e depressão, avaliadas com o Patient Health Questionnaire (PHQ-9) e com o Inventário de Ansiedade de Traço-Estado (IDATE). As variáveis independentes foram avaliadas com um questionário sociodemográfico e a Escala de Indicadores de Prazer e Sofrimento no Trabalho (EIPST) do Inventário do Trabalho e Riscos de Adoecimento (ITRA). As associações entre as variáveis foram testadas com o uso de regressão logística multinominal. Resultados Participaram do estudo 675 ACSs, sendo a maioria mulheres (83,7%), com até 40 anos (51,3%). As chances de apresentar sintomas de ansiedade foram maiores entre ACSs efetivos (1,61 [1,10-2,36]), e avaliações críticas ou graves nos fatores realização (Crítica 1,87 [1,30-2,68]; Grave 4,16 [2,06-8,40]) e esgotamento profissional (Crítica 2,60 [1,78-3,80]; Grave 3,97 [2,53-6,21]). Sexo feminino (2,12 [1,03-4,40]), idade de até 40 anos (1,741 [1,05-2,89]), tempo de serviço superior a cinco anos (1,88 [1,18-2,99]), avaliações crítica ou grave nos fatores realização (Crítica 2,53 [1,55-4,10]; Grave 6,07 [2,76-13,38]), esgotamento profissional (Crítica 5,21 [2,30-11,80]; Grave 15,64 [6,53-37,44]) e falta de reconhecimento (Crítica 1,93 [1,13-3,28]) estiveram associados a maiores chances de sintomas depressivos. Conclusões Apesar de se tratar de estudo transversal, que não permite inferir causalidade, os achados sugerem importante associação entre aspectos laborais dos ACS e os sintomas de ansiedade e depressão. Sexo feminino e possuir 40 anos ou menos também mostraram relação com o aumento dos sintomas de depressão.


ABSTRACT Objective To analyze indicators of pleasure and suffering at work associated with depression and anxiety among community health workers (CHW). Methods Crosssectional study in which the dependent variables were the presence of anxiety and depression symptoms, assessed using the Patient Health Questionnaire (PHQ-9) and State-Trait Anxiety Inventory (STAI). The independent variables were evaluated using the Scale of Indicators of Pleasure and Suffering at Work (EIPST) of the Inventory of Work and Risks of Illness (ITRA). Associations between dependent and independent variables were tested using multinomial logistic regression. Results 675 CHW participated in the study, the majority being women (83.7%), aged up to 40 years (51.3%). The chances of presenting anxiety symptoms were higher among effective CHW (OR=1.61; 95%CI: 1.10-2.36), and critical or severe assessments in the achievement factors (Critical OR=1.87; 95%CI: 1.30-2.68; Severe OR=4.16; 95%CI: 2.06-8.40) and professional exhaustion (Critical OR=2.60; 95%CI: 1.78-3.80; Severe OR=3.97; 95% CI: 2.53-6.21). Female gender (OR=2.12; 95%CI: 1.03-4.40), age up to 40 years (OR=1.74; 95%CI: 1.05-2.89), length of service greater than five years (OR=1.88; 95%CI: 1.18-2.99), critical or severe ratings on achievement factors (Critical OR=2.53; 95%CI: 1.55-4.10; Severe OR= 6.07; 95%CI: 2.76-13.38), professional exhaustion (Critical OR=5.21; 95%CI: 2.30-11.80; Severe OR=15.64; 95%CI: 6.53 -37.44) and lack of recognition (Critical OR=1.93; 95%CI: 1.13-3.28) were associated with greater chances of depressive symptoms. Conclusions Despite being a cross-sectional study, which does not allow inferring causality, the findings suggest an important association between the work aspects of the CHW and the symptoms of anxiety and depression. Female gender and being 40 years old or younger also showed a relationship with increased symptoms of depression.

ABSTRACT

Resumo Os agentes comunitários de saúde (ACS) compuseram uma força de trabalho na linha de frente dos sistemas de saúde no combate a COVID-19. O trabalho identificou as condições estruturais para organização e caracterização do trabalho dos ACS em três municípios do Nordeste brasileiro no período pandêmico. Realizou-se um estudo qualitativo de casos múltiplos. Foram entrevistados 28 sujeitos entre agentes comunitários e gestores municipais. A produção dos dados cotejou as entrevistas com a análise documental. As categorias operacionais que emergiram da análise dos dados foram: condições estruturais e características das atividades. Os resultados deste estudo revelaram escassez nas condições estruturais das unidades de saúde, que durante a pandemia passou por adaptações improvisadas em seus espaços internos. Quanto às características do trabalho, evidenciaram-se ações permeadas por aspectos burocráticos de cunho administrativo no interior das unidades de saúde, acarretando o esvaziamento de sua função vincular de articulação territorial e mobilização comunitária. Assim, as alterações em seu trabalho podem ser vistas como sinalizadores da fragilização do sistema de saúde e, especialmente, da atenção primária à saúde.


Abstract The community health agents (CHAs) comprised the workforce at the forefront of health systems in the fight against COVID-19. The study identified the structural conditions for organizing and characterizing the work of CHAs in three municipalities of northeastern Brazil during the pandemic period. A qualitative study of multiple cases was carried out. Twenty-eight subjects were interviewed, including community agents and municipal managers. Data production assessed the interviews with document analysis. The operational categories that emerged from the data analysis were: structural conditions and characteristics of the activities. The results of this study disclosed the scarcity of the structural conditions in the health units, which during the pandemic made improvised adaptations of the internal spaces. As for the work characteristics, actions permeated by bureaucratic aspects of an administrative nature were evidenced in the health units, resulting in the elimination of their binding function of territorial articulation and community mobilization. Thus, changes in their work can be seen as signs of the fragility of the health system and, especially, of primary health care.

Introducción: el estudio se desarrolló en el contexto de un programa de salud familiar y comunitaria de una universidad de Bogotá (Colombia), del cual participan los estudiantes de Medicina. El objetivo fue comprender los significados y sentidos que los estudiantes otorgan a la práctica de salud familiar y comunitaria en el barrio El Codito, ubicado en la localidad de Usaquén al nororiente de Bogotá. Materiales y métodos: la investigación es un estudio cualitativo de tipo fenomenológico. Se realizaron entrevistas semiestructuradas a ocho estudiantes de medicina pertenecientes a un programa universitario de salud familiar y comunitaria en Bogotá. Resultados: del proceso de análisis emergieron cuatro categorías: a) realidad y territorio; b) institucionalidad y salud familiar y comunitaria; c) agencia y proyección profesional, y d) comunicación, escucha y reconocimiento del otro. Se evidenciaron narrativas embebidas de experiencias significativas, dificulta- des y retos. Se destaca el rol participativo del estudiante en la interacción y comunicación con la familia. Conclusiones: la experiencia da cuenta de un proceso dinámico de donde emergen vivencias que confrontan a los estudiantes. Esta experiencia refleja aprendizajes y desafíos de carácter multidimensional, en especial por la vulnerabilidad del sector. De dicho proceso surge el reconocimiento de otras realidades, el autorreconocimiento y la proyección del estudiante como agente social y futuro profesional en medicina.


Introduction: This study was developed in accordance to the family and community health program at Bogotá University. The objective of this study was to understand the experiences of students in family and community health practice in the El Codito neighborhood of northeast Bogotá (Colombia) in the town of Usaquen. Materials and methods: Medical students of the university participated in this qualitative phenomenological study. Eight semi-structured interviews were conducted with the students who participated in the family and community health program at Bogotá University (Colombia). Results: Four categories emerged from the analysis process: reality and territory, institutionality and family and community health, agency and professional projection, and communication; listening; and recognition of other. Embedded narratives of significant experiences, difficulties, and multidimensional challenges were evident. The student's role in the interaction and communication with families was highlighted. Self-recognition, recognition of other realities, and the student's projection as a future professional emerged from this process. Conclusions: The participation of students was a dynamic process from which experiences that confront students emerged. This experience reflected multidimensional learning and challenges due to the vulnerability of this sector. This process resulted in the recognition of other realities, self-recognition, and the student's projection as a social agent and future professional in medicine.


Introdução: este estudo foi desenvolvido no contexto de um programa de saúde familiar e comunitária de uma universidade de Bogotá, do qual participam estudantes de medicina. O objetivo do estudo foi compreender os significados e sentidos que os alunos atribuem à prática de saúde familiar e comunitária no bairro El Codito, localizado no nordeste de Bogotá (Colômbia). Materiais e métodos: para a pesquisa, foi realizado um estudo qualitativo de tipo fenomenológico. Foram realizadas oito entrevistas semiestruturadas com estudantes de medicina pertencentes a um programa universitário de saúde familiar e comunitária em Bogotá (Colômbia). Deste processo decorre o reconhecimento de outras realidades, o autorreconhecimento e a projeção do aluno como agente social e futuro profissional da medicina.

ABSTRACT

Background: A healthy community is often seen to be the outcome of an effective and accessible health service delivery model, but factors like informed community, equal and adequate access to services as well as social and economic context, significantly contribute to this goal. An explorative research study is conducted across 15 locations in Assam, to capture the need-based narratives of the communities on the subject of health and wellness. Objective: The objective of the study is to understand community experiences and expectations within the context of the social determinants of health. Methodology: The study is qualitative in nature and uses Focused Group Discussions and Key Informant Interviews on a sample size of 294 respondents. The holistic viewing of health and wellness, encompassing social, cultural, economic, and environmental factors through active community engagement, not only helps to contextualize the issues of health as faced by the communities but also to bridge the gap of access and outreach, thereby fostering health equity. Discussion: Issues stemming from gender, livelihood, and economic disparity largely shape the health-seeking behaviours of individuals and can drive or constrict them from attaining desired levels of well-being. Conclusion: With the idea of universal health coverage as promoted by Govt. of India’s Ayushman Bharat Initiative; an opportunity thus opens up to incorporate these voices from the ground in developing targeted policy interventions.

Resumo Trata-se de uma revisão narrativa cujo objetivo é compreender o estado da arte da literatura sobre programas de Agentes Comunitárias de Saúde (ACS) no mundo, identificando suas nomenclaturas, práticas, formação e condições trabalhistas. A grande concentração de programas de ACS ainda ocorre em países de baixa e média renda da África (18), Ásia (12) e América Latina (05), com algumas poucas experiências em países de alta renda na América do Norte (02) e Oceania (01). No total foram catalogadas 38 experiências, tendo sido descritas as práticas de cuidado, vigilância, educação, comunicação em saúde, práticas administrativas, de articulação intersetorial e mobilização social. Caracterizou-se os níveis e duração das formações das ACS, assim como as diversas condições de trabalho em cada país. Em grande parte, o trabalho é precarizado, muitas vezes voluntário e realizado por mulheres. A revisão proporcionou um panorama comparativo que pode contribuir para enriquecer o olhar de gestores e tomadores de decisão em contextos de implantação, ampliação e reconfiguração de tais programas.


Abstract This is a narrative review whose objective is to understand the state of the art of the literature on Community Health Worker (CHW) programs worldwide, identifying their nomenclatures, practices, training, and working conditions. The major concentration of CHW programs can still be found in low- and middle-income countries in Africa (18), Asia (12), and Latin America (05), with a few experiences in high-income countries in North America (02) and Oceania (01). In total, 38 experiences were cataloged, and the practices of care, surveillance, education, health communication, administrative practices, intersectoral articulation, and social mobilization were described. The levels and duration of CHW training were characterized, as were the different working conditions in each country. Much of the work is precarious, often voluntary and carried out by women. This review provided a comparative overview that can contribute to enrich the view of managers and decision-makers in contexts of the implementation, expansion, and reconfiguration of such programs.

Community health services can be classified into Preventive Health Services, Promotive Health Services, Curative Health Services, and Rehabilitative Health Services. Similarly, promotive heath care services include Rasayan, Vaajikaran, Ashtaaharvidhi vishesh ayatan, Sadvrit, Tryoupstham. Curative health services include single-drug therapy for the different diseases called Ekal dravya and treatment of Kaas, Swash, Atisaar, Amlapita which are very common. Rehabilitative health services such as physical therapy, counseling, and other mental health services can be provided by Ashwashan chikitsa, Medhya rasaayan, Yoga, Pathya palan, Padansikkarm palan.

INTRODUÇÃO: O Agente Comunitário em Saúde, em seu ambiente de trabalho, lida com vários desafios, tais como: as inúmeras tarefas, a exigência de produtividade, a falta de recursos, os conflitos interpessoais e as condições salariais inadequadas, implicando assim em sobrecarga física e mental, aspectos que podem repercutir na qualidade de vida e no aparecimento de distúrbios osteomusculares. OBJETIVO: Avaliar a Qualidade de Vida no Trabalho (QVT) e a presença dos sintomas osteomusculares dos Agente Comunitário de Saúde (ACS). MÉTODOS: Trata-se de um estudo transversal, com amostragem por conveniência, cujos participantes foram os ACS atuantes nas Unidades de Saúde da Família do Segundo Distrito Sanitário de Saúde em Maceió - Alagoas. Coletou-se os dados por meio dos seguintes instrumentos: questionário sociodemográficos; questionário de avaliação da qualidade de vida no trabalho (QWLQ-bref); e o questionário de sintomas osteomusculares (QNSO). RESULTADOS: Participaram 38 ACS, sendo a maioria do sexo feminino (86,8%). Obtiveram um escore médio de QVT de 59,84, o que representa uma posição de satisfação. Já na análise por domínios, o pessoal, psicológico e físico também se encontram na faixa de satisfação, exceto o domínio profissional com escore de 53,44, classificado como neutro. Em relação aos sintomas osteomusculares, o pescoço (55,3%) e a coluna lombar (47,4%) foram as regiões mais acometidas nos últimos 12 meses. CONCLUSÃO: Os resultados deste estudo, sinalizam a necessidade de adoção de melhorias nas condições de trabalho, tanto no sentido de repensar as questões que repercutem na QVT quanto no manejo dos problemas musculoesqueléticos.


INTRODUCTION: The Community Health Agent, in their work environment, deals with several challenges, such as the numerous tasks, the demand for productivity, the lack of resources, interpersonal conflicts and inadequate salary conditions, thus implying physical and mental overload; aspects that can affect the quality of life and the onset of musculoskeletal disorders. OBJECTIVE: Evaluate the Quality of Life at Work (QWL) and the presence of musculoskeletal symptoms in Community Health Agents (CHA). METHODS: This is a cross-sectional study, with convenience sampling, in which the participants were the CHA servers working in the Family Health Units of the Second Health Health District in Maceió - Alagoas. Data were collected using the following instruments: a sociodemographic questionnaire; a quality of work life assessment questionnaire (QWLQ-bref); and the musculoskeletal symptoms questionnaire (QNSO). RESULTS: Thirty-eight CHAs participated, most of them female (86.8%). It was obtained an average QWL score of 59.84, which represents a satisfactory position. In the analysis by domains, the personal, psychological and physical are also in the range of satisfaction, except for the professional domain with a score of 53.44, classified as neutral. Regarding musculoskeletal symptoms, the neck (55.3%) and lumbar spine (47.4%) were the most affected regions in the last 12 months. CONCLUSIONS: The results of this study indicate the need to adopt improvements in working conditions, both in the sense of rethinking the issues that affect QWL, as well as in the management of musculoskeletal problems.
