ABSTRACT

ObjectiveTo preliminarily establish and verify the quantitative diagnosis method of dampness syndrome of colorectal adenoma, so as to provide evidence for the diagnosis of colorectal adenoma syndrome. MethodsThis study included 334 patients with colorectal adenoma, who were grouped into 200 in the training group and 134 in the validation group by clinical visits chronologically. According to the data from the four examinations of traditional Chinese medicine, patients in training group were subgrouped into dampness syndrome subgroup and non-dampness syndrome subgroup. After eliminating items with response rate less than 3%, the factors showed statistical difference in frequency between the subgroups were screened as diagnostic items. The diagnostic items were assigned scores using the conditional probability formula conversion method, and the diagnostic thresholds and grading criteria were determined by the maximum likelihood discriminant method, so as to establish the quantitative criteria preliminarily. Retrospective and prospective tests were conducted respectively on patients in training group and validation group, including the sensitivity, specificity, accuracy, positive likelihood ratio and other indexes, to evaluate the quantitative criteria. ResultsThe training group included 176 participants as dampness syndrome subgroup and 24 participants as non-dampness syndrome subgroup, who applied 40 diagnostic items, and 19 related factors were identified as significant differences. After assigning the scores in turn, the quantitative diagnostic threshold was determined as 45, and the quantitative diagnostic criteria for colorectal adenoma with dampness syndrome were as follows: greasy coating (7 scores), thick coating (8 scores), heaviness of head (9 scores), heaviness of whole body (6 scores), heaviness of limbs (6 scores), sticky and greasy stool (6 scores), sticky and greasy mouth (10 scores), obesity (6 scores), sleepiness (12 scores), laziness (13 scores), epigastric fullness (8 scores), abdominal distension and pain (11 scores), lumbar and knee aches and heaviness (8 scores), joint and muscle aches and pains (9 scores), loose stools (12 scores), fetid mouth odor (15 scores), slippery pulse (8 scores), overabundance of eye secretion (7 scores), and large touge (10 scores). Grading criteria: 45 ≤ points < 61 as mild, 61 ≤ points ≤ 104 as moderate, points > 104 as severe. In the test retrospective of the training group involving 200 patients, the sensitivity, specificity, accuracy, and positive likelihood ratio were 86.36%, 95.83%, 87.50%, and 20.73 respectively; In the test prospective of the verification group involving 134 patients, the sensitivity, specificity, accuracy, and positive likelihood ratio were 76.64%%, 96.30%, 80.60%, and 20.69, respectively. ConclusionIt is effective to diagnose and identify the dampness syndrome of colorectal adenoma by preliminarily establish a quantitative diagnostic method with a combined model of disease and evidence, and the method may provide support for future related studies.

ABSTRACT

The alterations of serum biological endogenous chemicals in rats with phlegm dampness accumulation syndrome of prehypertension (PHT) were interfered by Banxia Baizhu Tianma decoction (BBT), and the metabolic regulatory pathway of BBT was clarified using serum metabonomics analysis. To replicate the rat model of prehypertension phlegm dampness syndrome, blood pressure, behavioral markers, and serum biochemical markers of rats were collected. BBT's effectiveness in controlling blood pressure and blood lipids was assessed, and changes in endogenous small molecules in rat serum were determined using UPLC-Q-Orbitrap MS metabolic analysis. The results showed that BBT could regulate 9 metabolites, including arachidonic acid, cholic acid, glycodeoxycholic acid, N-adenosyltyrosine, arginine, lysophosphatidylethanolamine (20:0/0:00), lysophospholipid (P-18:0), lysophospholipid (18:0), lysophospholipid (22:5(7Z,10Z,13Z,16Z,19Z)). MetaboAnalyst was used to analyze the metabolic pathway. There were 7 metabolic pathways closely related to the change of blood pressure in rats, among which arachidonic acid metabolic pathway was the most critical. The metabolism difference foreign body in the model rats tends to return to the normal level, which provides a research basis for the mechanism of BBT from the perspective of metabonomics. ABSTRACT

This study aims to identify the novel biomarkers of cold-dampness syndrome(RA-Cold) of rheumatoid arthritis(RA) by gene set enrichment analysis(GSEA), weighted gene correlation network analysis(WGCNA), and clinical validation. Firstly, transcriptome sequencing was carried out for the whole blood samples from RA-Cold patients, RA patients with other traditional Chinese medicine(TCM) syndromes, and healthy volunteers. The differentially expressed gene(DEG) sets of RA-Cold were screened by comparison with the RA patients with other TCM syndromes and healthy volunteers. Then, GSEA and WGCNA were carried out to screen the key DEGs as candidate biomarkers for RA-Cold. Experimentally, the expression levels of the candidate biomarkers were determined by RT-qPCR for an independent clinical cohort(not less than 10 cases/group), and the clinical efficacy of the candidates was assessed using the receiver operating characteristic(ROC) curve. The results showed that 3 601 DEGs associated with RA-Cold were obtained, including 106 up-regulated genes and 3 495 down-regulated genes. The DEGs of RA-Cold were mainly enriched in the pathways associated with inflammation-immunity regulation, hormone regulation, substance and energy metabolism, cell function regulation, and synovial pannus formation. GSEA and WGCNA showed that recombinant proteasome 26S subunit, ATPase 2(PSMC2), which ranked in the top 50% in terms of coefficient of variation, representativeness of pathway, and biological modules, was a candidate biomarker of RA-Cold. Furthermore, the validation results based on the clinical independent sample set showed that the F1 value, specificity, accuracy, and precision of PSMC2 for RA-Cold were 70.3%, 61.9%, 64.5%, and 81.3%, respectively, and the area under the curve(AUC) value was 0.96. In summary, this study employed the "GSEA-WGCNA-validation" integrated strategy to identify novel biomarkers of RA-Cold, which helped to improve the TCM clinical diagnosis and treatment of core syndromes in RA and provided an experimental basis for TCM syndrome differentiation.

ABSTRACT

Objective:To explore the effect of modified Chushi Weiling Decoction combined with fire acupuncture in the treatment of chronic eczema with spleen deficiency and dampness syndrome.Methods:A total of 106 patients with chronic eczema who met the inclusion criteria from January 2018 to January 2020 in Shandong Taishan hospital were randomly divided into two groups by random number table method, with 53 patients in each group. The control group was treated with loratadine tablets and compound glycyrrhizin tablets, and the observation group was treated with modified Chushi Weiling Decoction combined with fire acupuncture. Both groups were treated for 8 weeks and followed up for 12 months. TCM syndrome scores were performed before and after treatment, Eczema area and severity index (EASI) was used to assess the severity of eczema, and pruritus was assessed by pruritus VAS scale. The serum IgE level was detected by nephelometry, and eosinophils (EOS) count was detected by automatic hematology analyzer. The adverse events and recurrence were recorded and the clinical response rate was evaluated.Results:The response rate was 94.34% (50/53) in the observation group and 77.36% (41/53) in the control group. There was significant difference between the two groups ( χ2=6.29, P=0.012). After treatment, the score of TCM syndrome, EASI and VAS of pruritus in the observation group were significantly lower than those in the control group ( t values were 11.97, 6.31 and 10.61 respectively, all Ps<0.001). After treatment, the serum IgE level [(57.19±7.54) IU/ml vs. (81.55±12.08) IU/ml, t=12.45], EOS count [(310.54± 52.84) ×10 6/L vs. (465.51±58.04) ×10 6/L, t=14.37] in the observation group were significantly lower than those in the control group ( P<0.01). The incidence of adverse events was 15.09% (8/53) in the control group and 3.77% (2/53) in the observation group, and there was significant difference between the two groups ( χ2=3.98, P=0.046). The recurrence rate was 6.00% (3/50) in the observation group and 21.95% (9/41) in the control group, and there was significant difference between the two groups ( χ2=5.01, P=0.025). Conclusion:Modified Chushi Weiling Decoction combined with fire acupuncture can improve the skin lesions and itching symptoms, improve the clinical efficacy, reduce the long-term recurrence rate of patients with chronic eczema with spleen deficiency and dampness syndrome safely.

ABSTRACT

ObjectiveTo observe the therapeutic effect of Wang's Yijing decoction combined with conventional therapy on the treatment of polycystic ovary syndrome (PCOS) with kidney deficiency and phlegm-dampness syndrome and its regulating effect on the intestine-brain axis. MethodThe 132 patients from the gynecological clinic of Shanxi Bethune Hospital were randomly assigned into a control group and an observation group, with 66 patients in each group. The patients in the control group were treated with drospirenone and ethinylestradiol tablets and metformin tablets, and those in the observation group were administrated with Wang's Yijing decoction on the basis of the therapy in the control group. The treatment in both groups lasted for 3 menstrual cycles. The changes in menstrual blood volume, color, and texture, as well as that in the menstrual cycle before and after treatment were scored. Further, the Rosenfield, hirsutism, and kidney deficiency and phlegm-dampness syndrome were scored before and after treatment. Pelvic ultrasound was performed before and after treatment to compare the bilateral ovarian volume. The levels of anti-mullerian hormone (AMH), estradiol (E2), luteinizing hormone (LH), follicle-stimulating hormone (FSH), testosterone (T), dihydrotestosterone (DHT), and sex hormone-binding globulin (SHBG) were measured before and after treatment, and the free androgen index (FAI) was calculated. After the fasting insulin and fasting blood glucose were examined before and after treatment, the homeostatic model assessment of insulin resistance (HOMA-IR) was calculated. The levels of ghrelin, glucagon-like peptide-1 (GLP-1), peptide tyrosine-tyrosine (PYY) in serum and short-chain fatty acids (SCFAs) in fresh stool were measured before and after treatment. ResultThe treatments in both groups lowered the scores of irregular menstruation, Rosenfield, hirsutism, and kidney deficiency and phlegm-dampness syndrome (P<0.01), and the scores in the observation group were lower than those in the control group (P<0.01). After treatment, the T, DHT, FAI, AMH, E2, LH, and FSH levels lowered (P<0.01), while the SHBG level elevated (P<0.01). In addition, the AMH, E2, LH, T, DHT, and FAI in the observation group were lower than those in control group, while the SHBG level presented an opposite trend (P<0.01). After treatment, the bilateral ovarian volume and HOMA-IR of the two groups decreased (P<0.01), and were lower in the observation group than in the control group (P<0.05, P<0.01). The treatments in both groups elevated the levels of ghrelin, GLP-1, PYY, and SCFAs (P<0.01), which were higher in the observation group than in the control group (P<0.01). After treatment, the recovery rate in the observation group was 65.57% (40/61), which was higher than 44.83% (26/58) in the control group (χ2=5.180, P<0.05). ConclusionWang's Yijing decoction combined with conventional western medicine treatment can regulate the intestine-brain axis, endocrine and metabolic disorders, and sex hormones, adjust the menstrual cycle, and alleviate clinical symptoms, demonstrating good clinical effect for the PCOS patients with kidney deficiency and phlegm dampness syndrome.

ABSTRACT

Objective:To evaluate the clinical efficacy of Buyang-Huanwu Decoction combined with modified Tingli-Dazao-Xiefei Decoction on the basis of conventional western medicine therapy in the treatment of chronic heart failure (CHF) with syndrome of qi deficiency and blood stasis. Methods:Seventy patients who met the inclusion criteria from November 2017 to November 2019 in Shijingshan District Hospital of Traditional Chinese Medicine were randomly divided into two groups, 35 in each group. The control group was treated with western medicine for chronic heart failure, and the treatment group was treated with Buyang-Huanwu Decoction combined with Tingli-Dazao-Xiefei Decoction on the basis of the control group. Both groups were treated for 2 weeks. The TCM syndrome scores were observed and compared before and after treatment. Minnesota Living with Heart Failure Questionnaire (MLHFQ) was used to evaluate the quality of life of patients. Lee’s Heart Failure Score was used to evaluate the severity of symptoms. The N-terminal pro-B type natriuretic peptide(NT-proBNP) was detected by ELISA. The adverse reactions during treatment were observed and the clinical efficacy was evaluated. Results:The total effective rate was 91.4% (32/35) in the treatment group and 77.1 % (27/35) in the control group, and the difference was statistically significant ( χ2=7.050, P=0.014). After treatment, the serum NT-proBNP in the treatment group [(1 725.3 ± 1 473.8) ng/L vs. (2 485.7 ± 2 164.4) ng/L; U=341.200, P=0.031] was significantly lower than that of the control group. The NT-proBNP [(54.3 ± 26.7) % vs. (35.5 ± 19.8)%; U=4.310, P=0.003] was significantly higher than that of the control group. After treatment, the TCM syndrome scores and MLHFQ scores in the treatment group were significantly lower than those in the control group ( t=3.785, 9.925, P=0.031, 0.001). During the treatment, no obvious adverse reactions were observed in both groups. Conclusion:On the basis of standardized treatment of Western medicine, Buyang-Huanwu Decoction and Tingli-Dazao-Xiefei Decoction can improve the clinical efficacy of CHF patients with qi deficiency and blood stasis syndrome, promote the repair of damaged myocardium (reduce NT-proBNP), and improve the quality of life of patients.

ABSTRACT

Objective:To evaluate the efficacy of modified Xiaoji Baozhong granules combined with scraping in the treatment of abdominal obesity with gastric heat and dampness syndrome and its effect on adipocytokines. Method:One hundred and forty-four patients were randomly divided into the control group and the observation group, 72 cases in each group. The lifestyle adjustments were conducted in both groups. The observation group was given a comprehensive traditional Chinese medicine regimen of modified Xiaoji Baozhong granules combined with scraping. The control group was given modified Xiaoji Baozhong granules combined with scraping. The course of treatment was three months. The body mass index (BMI), body fat percentage (FP), waist circumference (WC), waist to hip ratio (WHR), obesity and abdominal fat thickness of the two groups were compared before and after treatment. The levels of fasting blood glucose (FBG), insulin resistance index (HOMA-IR), high density lipoprotein (HDL), low density lipoprotein (LDL), triglyceride (TG), total cholesterol (TC), adiponectin, leptin, resistin, endolipid and tumor necrosis factor-α (TNF-α) were compared between the two groups before and after treatment. The clinical efficacy and safety were evaluated. Result:After treatment, the BMI, FP, obesity, WC and WHR of the observation group were lower than those of the control group (<italic>P</italic><0.01). After treatment, the abdominal wall fat thickness and intra-abdominal fat thickness of patients in the observation group were lower than those in the control group (<italic>P</italic><0.01). The FBG, HOMA-IR, TG, TC and LDL of the observation group were lower than those of the control group (<italic>P</italic><0.01), while the HDL level was higher than the control group (<italic>P</italic><0.01). Adiponectin in the observation group was higher than the control group after treatment (<italic>P</italic><0.01). The leptin, resistin, visfatin and TNF-α were lower than those in the control group (<italic>P</italic><0.01). The total clinical efficacy rate of patients in the observation group was 88.24% (60/68), which was better than 73.53% (50/68) in the control group (<italic>χ</italic><sup>2</sup>=4.755, <italic>P</italic><0.05). Conclusion:In addition to lifestyle adjustments, modified Xiaoji Baozhong granules combined with scraping treatment can reduce the degree of obesity and fat thickness in patients with abdominal obesity, and regulate lipid, carbohydrate metabolism and adipocytokines, with a better clinical efficacy than simple scraping therapy and the safety in practice.

ABSTRACT

Objective:To observe the effect of modified Cangfu Daotantang on metabolism and pregnancy in patients with spleen deficiency and phlegm-dampness type polycystic ovary syndrome (PCOS). Method:One hundred and twelve patients were randomly divided into control group and observation group according to the random number table. Both groups took non-pharmacological interventions, oral metformin hydrochloride, 500mg/time, 3 times/day; oral ethinyl estradiol and cyproterone tablets, 1 tablet/time, 1 time/day, starting from the third to fifth day of menstruation and lasting for twenty-one days, for a total of 3 menstrual cycles. Patients in control group additionally took Erchen pills orally, 10 g/time, 2 times/day, while patients in observation group additionally took modified Cangfu Daotantang orally, 1 dose/day. The course of treatment was six menstrual cycles in both groups (or termination after conception). The waist-to-hip ratio (WHR), body mass index (BMI), insulin resistance index (HOMA-IR), pancreatic <italic>β</italic>-cell function (HOMA-<italic>β</italic>), triglycerides (TG), low-density lipoprotein (LDL) and non-high-density lipoprotein (nHDL) elevation after treatment were compared. The number of ovulation cycles monitored by B-ultrasound (6 menstrual cycles), ovulation rate, human chorionic gonadotropin (HCG) day endometrial thickness, follicle diameter, cervical mucus score>8 points and endometrial morphology type A rate were measured and recorded. The recovery of menstruation, pregnancy and early miscarriage were recorded. Luteinizing hormone (LH), estradiol (E<sub>2</sub>), follicle stimulating hormone (FSH), dehydroepiandrosterone sulfate (DHEAS), testosterone (T), anti-Müllerian hormone (AMH) levels, and insulin before and after treatment -Like growth factor-1 (IGF-1), leptin (LP), adiponectin (APN), growth differentiation factor-9 (GDF-9) and tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>) levels were detected. Result:WHR, BMI and HOMA-IR levels of the observation group were lower than those of the control group (<italic>P</italic><0.05, <italic>P</italic><0.01). HOMA-<italic>β</italic> level was higher than that in the control group (<italic>P</italic><0.01). The increase rates of LDL, TG, and nHDL in the observation group were 19.61%(10/51),25.49%(13/51),23.53%(12/51), respectively, lower than 41.18%(21/51),47.06%(24/51),45.10%(23/51)respectively in the control group (<italic>χ</italic><sup>2</sup>=5.607, <italic>χ</italic><sup>2</sup>=5.131, <italic>χ</italic><sup>2</sup>=5.263, <italic>P</italic><0.05). The menstrual recovery rate in the observation group was 90.20% (46/51), higher than 72.55% (37/51) in the control group (<italic>χ</italic><sup>2</sup>=5.239,<italic>P</italic><0.05). The observation group had more ovulation cycles than the control group (<italic>P</italic><0.01). The pregnancy rate in the observation group was 50.98% (26/51), higher than 31.37% (16/51) in the control group (<italic>χ</italic><sup>2</sup>=4.047,<italic>P</italic><0.05). On HCG day after treatment, the endometrial thickness and follicle diameter in the observation group were better than those in the control group (<italic>P</italic><0.01). The proportion of patients with cervical mucus score> 8 points was 78.43% (40/51) in the observation group, higher than 56.86% (29/51) in the control group (<italic>χ</italic><sup>2</sup>=5.420,<italic>P</italic><0.05). The intimal morphology type A rate in the observation group was 52.94% (27/51), higher than 31.37% (16/51) in the control group (<italic>χ</italic><sup>2</sup>=4.864,<italic>P</italic><0.05). The levels of AMH, E<sub>2</sub>, DHEAS, LH, T , IGF-1, LP and TNF-<italic>α</italic> in the observation group were lower than those in the control group (<italic>P</italic><0.01), while the APN and GDF-9 levels were superior to those in the control group (<italic>P</italic><0.01). Conclusion:On the basis of conventional western medicine intervention, modified Cangfu Daotantang can regulate abnormal metabolism and reproductive endocrine in patients with PCOS, improve conception, and regulate the expression of IGF-1, GDF-9, adipocytokines and inflammatory factors, improve ovulation and improve pregnancy rate.

ABSTRACT

Objective:To study the clinical efficacy of orthopedics No.1 prescription combined with celecoxib in the treatment of knee osteoarthritis (KOA) with middle stage of cold-dampness syndrome and investigate its effect on serum cytokines levels. Method:The 72 patients were randomly divided into control group and observation group, with 36 cases each. Patients in both groups were given basic treatment with oral celecoxib capsules (0.2 g/ time, 1 time/day). On the basis of western medicine treatment, patients in observation group were treated with orthopedics No.1 prescription decoction-free granules by fumigation, 1 bag/time, 1 time/day, 5 times/week. Both groups received treatment for 4 weeks. The visual analog pain score (VAS), American knee society knee score (KSS), serum interleukin-1<italic>β </italic>(IL-1<italic>β</italic>), tumor necrosis factor-<italic>α </italic>(TNF-<italic>α</italic>), and transforming growth factor-<italic>β</italic><sub>1 </sub>(TGF-<italic>β</italic><sub>1</sub>) levels were observed before and after treatment, and their clinical efficacy was evaluated. Result:After treatment, VAS score significantly decreased in both groups (<italic>P</italic><0.01), and KSS score significantly increased (<italic>P</italic><0.01), with better clinical effect in observation group. After treatment, serum IL-1<italic>β</italic> and TNF-<italic>α</italic> levels decreased significantly in both groups (<italic>P</italic><0.01), and the levels in observation group were lower than those in control group after treatment (<italic>P</italic><0.05). TGF<italic>-β</italic><sub>1 </sub>content was significantly higher than that before treatment in two groups (<italic>P</italic><0.01). Conclusion:Orthopedics No.1 prescription combined with celecoxib for the treatment of KOA with middle stage of cold-dampness syndrome can effectively relieve the clinical symptoms of patients with KOA, improve joint function, improve quality of life, reduce the contents of inflammatory factors IL-1<italic>β</italic> and TNF-<italic>α</italic> in serum, and increase the expression of TGF-<italic>β</italic><sub>1</sub> level.

ABSTRACT

Objective:To observe the regulatory effect of modified Erzhu Erchentang on metabolization of polycystic ovary syndrome (PCOS) with spleen deficiency and phlegm dampness syndrome. Method:Patients 140 cases were divided into control group and observation group. Both groups were given metformin hydrochloride tablets, 500 mg/time, 3 times/day. Control group was given Yuejun Erchen pills, 0.5 g/time, 3 times/day, while observation group was given modified Erzhu Erchentang, 1 dose/day. The course of treatment lasted for 24 weeks. Before and after treatment, levels of fasting blood glucose (FBG), fasting insulin (FINS), glycosylated hemoglobin Alc (HbA1c), 2-hour postprandial blood glucose (2 h PG), blood lipid, waist circumference (WC), body mass index (BMI), waist hip ratio (WHR), luteinizing hormone (LH), follicle stimulating hormone (FSH), serum testosterone (T), estradiol (E<sub>2</sub>), dehydroepiandrosterone sulfate (DHEAS), sex hormone binding globulin (SHBG), leptin (LP), adiponectin (APN), resistin, visfatin and tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>) were detected. Homeostasis model assessment insulin resistance (HOMA-IR) was calculated, modified Erzhu Erchentang was scored, and recovery of menstruation and ovulation and ovarian volume were recorded. Result:Levels of FBG, 2 h PG, HbA1c, FINS, HOMA-IR, triglyceride (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), LH, FSH, T, E<sub>2</sub>, DHEAS, LP, resistin, visfatin and TNF-<italic>α</italic> in observation group were lower than those in control group (<italic>P</italic><0.01), and levels of BMI, WC and WHR were lower than those of control group (<italic>P</italic><0.05). And levels of high-density lipoprotein cholesterol (HDL-C), SHBG and APN were higher than those in control group (<italic>P</italic><0.01). Score of modified Erzhu Erchentang was lower than that in control group (<italic>P</italic><0.01), and ovarian volume was smaller than that in control group (<italic>P</italic><0.01). The normal rate of BMI was 49.23% (32/65), which was higher than 30.30% (20/66) in control group (<italic>χ</italic><sup>2</sup>=5.151, <italic>P</italic><0.05). The normal rate of blood lipid was 93.85% (61/65), which was higher than 81.82 % (54/66) in control group (<italic>χ</italic><sup>2</sup>=4.418, <italic>P</italic><0.05). The normal rate of blood glucose was 96.92% (63/65), which was higher than 86.36% (57/66) in control group (<italic>χ</italic><sup>2</sup>=4.474, <italic>P</italic><0.05). Conclusion:In addition to adipocytokines, modified Erzhu Erchentang could regulate adipokines of patients of PCOS with spleen deficiency and phlegm dampness, improve glucose, lipid metabolism and overweight, adjust endocrine hormone, reduce clinical symptoms and improve ovarian structure, so as to create conditions for conception.

ABSTRACT

OBJECTIVE@#To evaluate the effect of electro-acupuncture (EA) in infertile patients with phlegm-dampness polycystic ovary syndrome-insulin resistance (PCOS-IR).@*METHODS@#Seventy-six PCOS-IR patients who underwnet in vitro fertilization and embryo transfer (IVF-ET) were equally assigned to two groups according to a random digital table: the EA group and the control group, with 38 cases in each group. Before undergoing IVF, the two groups were treated with EA or pseudo-acupuncture, respectively, for 3 menstrual cycles. The intervention was 25 min twice a week until the day of oocyte collection. The selected acupoints were Zhongwan (RN 12), Tianshu (ST 25), Daheng (SP 15), Daimai (GB 26), Qihai (CV 6), Guanyuan (CV 4), and bilateral points including Xuehai (SP 10), Fenglong (ST 40), Zusanli (ST 36), and Yinlingquan (SP 9). Evaluation of phlegm-dampness syndrome score and IR score were carried out before and after treatment. Additionally, the number of oocytes retrieved, transplantable embryo rate, high-quality embryo rate, clinical pregnancy rate and live birth rate were compared between the two groups. Real-time polymerase chain reaction analysis was used to monitor the mRNA expression of the insulin receptor substrate (IRS-1), phosphatidylinositiol 3-kinase (PI3K) and glucose transport factor 4 (GLUT4) in ovarian granulosa cells.@*RESULTS@#EA treatment reduced the phlegm-dampness syndrome score as well as the IR scores compared with the control group (P0.05). Moreover, the transplantable embryo rate [49.0% (284/580) vs. 41.9% (273/652)], high-quality embryo rate [36.6% (104/284) vs. 27.8% (76/273)], and live birth rate [50% (19/38) vs. 26.3% (10/38)] in the EA group were significantly higher than in the control group (P<0.05). Gene expression analyses revealed significantly elevated IRS-1, PI3K and GLUT4 mRNA in ovarian granulosa cells of the EA group compared with the control group (P<0.05).@*CONCLUSIONS@#EA may ameliorate the effects of phlegm-dampness syndrome and ovarian IR in PCOS-IR patients. Mechanistically, this effect might be through an upregulation of the IRS-1/PI3K/GLUT4 signaling pathway, which may result in improved oocyte quality and embryonic development potential. ABSTRACT

Objective@#To observe the temperature changes of skin on patients with cold and dampness syndrome by using the non-contact infrared human body thermometer and infrared thermal imager to treat the patients with cold and dampness syndrome by Jin-Bohua national famous Chinese medicine practitioners using the "Mountain Burning Manipulation by Jin-Shi" method to clarify the clinical thermal effects of the "Heat-producing Needling" method.@*Methods@#This study included sixty patients with cold and dampness syndrome who met the enrollment conditionsfrom November 2015 to October 2017. In a room with fixed temperature, the patients were treated by "Heat-producing Needling" method. By using a non-contact infrared human body thermometer and infrared thermal imaging technology, this study observed the skin temperature changes in Quchi and Hegu, Zusanli and Chongyang before needling, immediately afterneedling, 15 minutes after needling, and 30 minutes after needling.@*Results@#After the "Mountain Burning Manipulation by Jin-Shi" treatment, the temperature of local skin surface at acupoints of upper and lower limbs increased with different degrees and the skin temperature reached the highest level when the needle was kept for 15 minutes. The upper limbs' result indicated that skin temperature increased with significant differences in acupuncture of Quchi point for immediately, 15 minutes and 30 minutes as compared with that before acupuncture. It was found that the skin temperature at Hegu point for 15 minutes have a significant increase than before and immediately after acupuncture. The skin temperature of Hegu point lasted 30 minutes, which was significantly higher than before and immediately after acupuncture. Compared with Hegu point, Quchi point has higher skin temperature immediately after acupuncture (t=3.793, P=0.034). The lower limbss' results indicated that skin temperature at Zusanli point increased with significant differences in Zusanli point for immediately needling, 15 minutes and 30 minutes as compared respectively with that beforeneedling. It was found that the skin temperature at Chongyang point for 15 minutes had a significant increase than before and immediately after acupuncture. The skin temperature at Chongyang point for 15 mins had a significant increase than before and immediately afterneedling. The skin temperature of Zusanli point was higher than that of Chongyang point (t=3.794, P=0.034) immediately afterneedling.@*Conclusions@#This study demonstrates that the "Mountain Burning Manipulation by Jin-Shi" method has a clear clinical thermal effect on both local and peripheral longitude of patients with cold dampness syndrome.

ABSTRACT

Objective Through the collection and analysis of clinical features and biochemical indices data got from 436 women who were diagnosed as phlegm-dampness syndrome of PCOS,in order to provide the reference for the personalized treatment for disease and prevention of long-term complication.Methods We collected 436 cases of patients diagnosed with phlegm-dampness syndrome of PCOS and 62 cases of control group from December 2014 to August 2017.We analysied and compared of three groups of clinical and biochemical indicators.Results The incidence of hirsutism,acanthosis nigricans and excessive fat of phlegm-dampness syndrome of PCOS + HA subgroup were higher than phlegm-dampness syndrome of PCOS + non-HA subgroup(P < 0.05).The LH and LH/FSH of of phlegm-dampness syndrome of PCOS + HA subgroup were higher.The level of FSH and SHBG was lower than phlegm-dampness syndrome of PCOS + non-HA subgroup and the control group(P < 0.05).The TC,TG and LDL of phlegm-dampness syndrome of PCOS + HA subgroup were higher than phlegm-dampness syndrome of PCOS + non-HA subgroup and the control group(P < 0.05).The FPG,FINS and HOMA-IR of phlegm-dampness syndrome of PCOS + HA subgroup is higher than phlegm-dampness syndrome of PCOS +non-HA subgroup and the control group(P < 0.05).Conclusion The patients with phlegm-dampness syndrome of PCOS + HA have higher incidence of hirsutism,acanthosis nigricans and excessive fat.The patients with phlegm-dampness syndrome of PCOS + HA have serious sex hormone metabolic disorders.The patients with phlegm-dampness syndrome of PCOS + HA have serious glucolipid metabolic disorders.

ABSTRACT

[Objective] To observe the clinical effects of filiform-fire needle combined with abdominal acupuncture in treating ankylosing spondylitis(cold-dampness syndrome). [Methods] Sixty patients who met the criteria of AS were included in the study. They were randomly divided into treatment group and control group for 30 patients in each group. The patients of control group were treated with Diclofenac sodium sustained -release tablet and Salazosul-fpyridine intestinal solution, while treatment group were treated with filiform-fire needle combined with abdominal acupuncture on the basis of western medicine mentioned above.All patients were treated and observed for three months, and they were evaluated by score of TCM symptoms, thoracic activity, spinal activity, finger-floor distance, pillow-wall distance and blood parameters, such as C-reactive Protein(CRP), Erythrocyte Sedimentation Rate (ESR)and Immune Globulin A(IgA). [Results] The total effective rate in treatment group was higher than that in control group(P<0.05).After treatment, the two groups all got lower scores of TCM symptoms than before, and the scores of the treatment group were significantly lower than the control group(P<0.05).The thoracic activity, spinal activity, finger-floor distance and pillow-wall distance of all patients improved in varying degrees, and the blood parameters of CRP, ESR and IgA decreased compared with that before treatment, and significantly the treatment group was superior to control group(P<0.05).[Conclusion] The filiform-fire needle combined with abdominal acupuncture can effectively relieve pain, improve functional activity of the spine and peripheral joints, reduce inflammation, slow disease progression and raise the quality of life of the patients.

ABSTRACT

Objective To investigate the clinical efficacy of combined acupuncture and medicine in treating kidney deficiency and phlegm dampness syndrome in obese polycystic ovary syndrome. Method Sixty-four patients were randomly allocated to two groups. The treatment group received combined acupuncture and medicine and the control group, oral administration of metformin. The course of treatment was three months. Pre-treatment and post-treatment indicators were statistically analyzed in the two groups of patients. Result BMI, waist to hip ratio, fasting insulin, fasting blood glucose and HOMA-IR improved significantly in the treatment group of patients after treatment; there were statistically significant pre-/post-treatment differences (P<0.05). Post-treatment BMI and waist to hip ratio improved more markedly in the treatment group than in the control group (P<0.05). Conclusion Combined acupuncture and medicine has a better therapeutic effect on kidney deficiency and phlegm dampness syndrome in obese polycystic ovary syndrome.
