ABSTRACT

RESUMEN Cronobacter sakazakii es una bacteria Gram negativa que pertenece a un grupo emergente de patógenos oportunistas de la familia de los Enterococos, que causa infecciones nosocomiales. Afecta típicamente a los recién nacidos de bajo peso; puede causar graves infecciones como meningitis, sepsis o enterocolitis necrotizante, potencialmente mortales, aunque la gran mayoría de las infecciones se producen en pacientes ancianos, en los que son mucho más leves. Se reporta el primer caso confirmado de infección de herida quirúrgica en España causada por C. sakazakii en un adulto inmunocompetente(AU)


ABSTRACT Cronobacter sakazakii is a Gram negative bacterium that belongs to an emerging group of opportunistic pathogens of the Enterococci family, which causes nosocomial infections. It typically affects low birth weight newborns. It can cause serious infections such as meningitis, sepsis, or life-threatening necrotizing enterocolitis, although the vast majority of infections occur in elderly patients, where they are much milder. We report the first confirmed case of surgical wound infection in Spain, caused by C. sakazakii in an immunocompetent adult(AU)

ABSTRACT

Enterococci are intrinsically resistant to several antimicrobial classes and show a great ability to acquire new mechanisms of resistance. Resistance to β-lactam antibiotics is a major concern because these drugs either alone or in combination are commonly used for the treatment of enterococcal infections. Ampicillin resistance, which is rare in Enterococcus faecium occurs in most of the hospital-associated Enterococcus faecium isolates. High-level resistance to ampicillin in E. faecium is mainly due to the enhanced production of PBP5 and/or by polymorphisms in the beta subunit of this protein. The dissemination of high-level ampicillin resistance can be the result of both clonal spread of strains with mutated pbp5 genes and resistance horizontal gene transfer.


Los enterococos son intrínsecamente resistentes a varias clases de antimicrobianos y presentan una gran capacidad para adquirir mecanismos de resistencia. La resistencia a los antibióticos p-lactámicos es preocupante porque estos fármacos solos o combinados se usan comúnmente para el tratamiento de las infecciones enterocócicas. La mayoría de los aislamientos hospitalarios de Enterococcus faecium presentan resistencia a la ampicilina, la cual es rara en Enterococcus faecalis. El alto nivel de resistencia a la ampicilina en E. faecium se debe principalmente a la hiperproducción de PBP5 y/o a polimorfismos en la subunidad beta de esta proteína. La propagación de esta resistencia puede deberse tanto a la diseminación clonal de cepas con genes pbp5 mutados como a la transferencia horizontal de genes.

ABSTRACT

Enterococos resist entes à vancomicina são frequentemente multirresistentes e tornaram-se uma das principais causas de infecções nosocomiais. Estão presentes causando mastites em rebanhos leiteiros e contaminando o leite de tanques de refrigeração. Porém, os dados são escassos em relação à sensibilidade das linhagens isoladas na produção leiteira. O objetivo do presente estudo foi determinar o perfil de resistência “in vitro” de Enterococcus spp. isolados de leite de tanques de refrigeração. Foram analisadas 194 amostras de 10 fazendas de Minas Gerais e São Paulo. Foi analisado o perfil de sensibilidade microbiana dessas linhagens frente a 14 antimicrobianos. Mais de 70% das amostras apresentaram resistência à cefalexina, cefoxitina, oxacilina e neomicina, 21,19% foram resistentes intermediárias a vancomicina.

La vancomicina (VAN) es un glucopéptido que inhibe la síntesis de la pared celular y con el tiempo se había transformado en la droga de elección para el tratamiento de infecciones graves por gram positivos. La resistencia observada en enterococos y la sensibilidad disminuida registrada en estafilococos hasta cierto punto han limitado su uso. La aparición de estreptococos del grupo viridans y del grupo B con marcadores de resistencia a vancomicina enciende un alerta en función de su posible pasaje a neumococos y Streptococcus pyogenes (AU)


Vancomycin (VAN) is a glycopeptide inhibiting cell-wall synthesis and has become the drug of choice for the treatment of severe gram-positive infections. Resistance observed in enterococci and decreased sensitivity in staphylococci have to a certain point limited its use. The appearance of the viridans group and group B streptococci showing markers for resistance to vancomycin have caused an alert regarding the possible passage to pneumococci and Streptococcus pyogenes (AU)

ABSTRACT

Background: Bloodstream infections (BSIs) due to Enterococcus faecium (E. faecium), particularly those due to vancomycin-resistant enterococcus (VRE), are still a therapeutic challenge. Aim: To evaluate mortality from BSI due to E. faecium and VRE in central Taiwan. Materials and Methods: We retrospectively analyzed cases of significant E. faecium BSI in the Changhua Christian Hospital System between January 1, 2010 and December 31, 2013. Results: Of the 76 cases, 28 patients (36.8%) were admitted to intensive care units (ICUs) at the onset of BSI, 10 (13.2%) cases were associated with polymicrobial bacteremia, and 29 (38.2%) cases were associated with entry via the biliary tract. VRE was observed in 18 (23.7%) cases. The 30-day mortality rate was 13.1% (10/76). Multivariate logistic regression analysis showed that bacteremia of non-biliary tract origin (OR = 8.43, 95% confidence interval (95% CI) = 1.32-54.00, p = 0.002) and ICU admission (OR = 4.2, 95% CI = 1.7-10.0, p = 0.002) were significant risk factors for 30-day mortality, whereas appropriate antimicrobial therapy was a protective factor for 30-day mortality (OR = 0.33, 95% CI = 0.14-0.79, p = 0.013). Conclusions: Our results underscore the need to assist patients admitted to ICUs with E. faecium BSIs with a non-biliary tract origin. We emphasize the use of appropriate antimicrobial therapy for E. faecium BSI with the aim to rescue more patients with these infections.


Antecedentes: Las infecciones del torrente sanguíneo por Enterococcus faecium, particularmente aquellas causadas por enterococos resistentes a vancomicina (ERV), representan aún un desafío para los tratamientos. Este estudio está orientado a la evaluación de la mortalidad debido a la infección del torrente sanguíneo (ITS) por E. faecium y por enterococos resistentes a vancomicina (ERV) en Taiwán central. Materiales y Métodos: Analizamos de forma retrospectiva casos de ITS causadas por E. faecium genuinas en el Sistema del Hospital Changhua Christian, entre los días 1 de enero de 2010 y 31 de diciembre de 2013. Resultados: De los 76 casos analizados, 28 pacientes fueron ingresados a las Unidades de Cuidados Intensivos (UCI) al comienzo de una ITS (36,8%), 10 casos fueron asociados a bacteriemia polimicrobiana (13,2%), y 29 casos tuvieron como puerta de entrada la vía biliar. En 18 casos se pudieron observar ERV (23,7%). La mortalidad a 30 días fue de 13,1% (10/76). El análisis multivariado mediante regresión logística mostró que la bacteriemia de origen no biliar (OR = 8,43, 95% intervalo de confianza (95% CI) = 1,32-54,00; p = 0,002), y el ingreso a la UCI (OR = 4,2; 95% CI = 1,7-10,0; p = 0,002), fueron factores de riesgo significativos para el rango de mortalidad de 30 días, así como un tratamiento de antimicrobiano apropiado constituye un factor protector en contra la mortalidad (OR = 0,33; 95% CI = 0,14-0,79; p = 0,013). Conclusiones: Nuestros resultados destacan la necesidad de asistir a los pacientes ingresados a la UCI con ITS por E. faecium con origen no biliar. Hacemos énfasis a la aplicación de una antibioterapia adecuada para sacar adelante a un mayor número de pacientes con este tipo de infecciones.

ABSTRACT

Lactic acid bacteria species were molecularly identified in milk from Lacaune, Santa Inês and crossbred sheep breeds and their in vitro probiotic potential was evaluated. The species identified were Enterococcus faecium (56.25%), E. durans (31.25%) and E. casseliflavus (12.5%). No other lactic acid bacteria species, such as lactobacilli, was identified. Most of the isolated enterococci were resistant to gastric pH (2.0) and to 0.3% oxgall. All tested enterococci were resistant to ceftazidime, oxacillin and streptomycin and sensible to clindamycin, erythromycin and penicillin. The resistance to ciprofloxacin, gentamicin, tetracycline and vancomycin varied among tested species. All tested enterococci strongly inhibited (P<0.05) Escherichia coli and Listeria monocytogenes, moderately inhibited E. faecalis and Staphylococcus aureus and did not inhibit Pseudomonas aeruginosa, Salmonella enterica var. Typhimurium and also one E. durans sample isolated from sheep milk. Four samples of E. faecium, one of E. durans and one of E. casseliflavus presented the best probiotic potential...


Espécies de bactérias ácido-lácticas foram identificadas em nível molecular em leite das raças ovinas Lacaune, Santa Inês e suas mestiças, e o seu potencial probiótico in vitro foi avaliado. As espécies identificadas foram Enterococcus faecium (56,25%), E. durans (31,25%) e E. casseliflavus (12,5%). Nenhuma outra espécie de bactéria ácido-láctica, como Lactobacillus sp., foi identificada. A maioria dos enterococos isolados foi resistente ao pH gástrico (2.0) e a 0,3% de oxgall. Todos os enterococos testados foram resistentes à ceftazidima, oxacilina e estreptomicina e sensíveis à clindamicina, eritromicina e penicilina. A resistência à ciprofloxacina, gentamicina, tetraciclina e vancomicina variou entre as amostras. Todos os enterococos testados inibiram fortemente (P<0,05) Escherichia coli e Listeria monocytogenes, inibiram moderadamente E. faecalis e Staphylococcus aureus e não inibiram Pseudomonas aeruginosa, Salmonella enterica var. INTRODUCCIÓN: el control de la calidad de las aguas es vital para la protección de la salud humana y la biodiversidad en el ambiente acuático. Varios microorganismos son utilizados como indicadores de contaminación fecal para este propósito, dentro de ellos los enterococos son considerados buenos indicadores, porque sobreviven más tiempo ante condiciones adversas en la naturaleza. OBJETIVOS: evaluar la capacidad de un nuevo método cromogénico alternativo para la detección y enumeración de enterococos en aguas por la técnica de filtración por membrana, en comparación con el método estándar ISO 7899:2. MÉTODOS: se recolectaron muestras de aguas de tres orígenes diferentes para determinar su calidad. Las muestras se ensayaron en paralelo por las dos metodologías (alternativa y referencia), empleando la técnica de filtración por membrana. Para evaluar el desempeño de los métodos se determinaron varios parámetros: sensibilidad, especificidad, exactitud, porcentaje de falsos positivos, porcentaje de falsos negativos, índice kappa. Los tres primeros parámetros se calcularon para ambos métodos antes y después de la confirmación por pruebas bioquímicas. Los resultados se analizaron desde el punto de vista estadístico, teniendo en cuenta los principales criterios definidos en las normas ISO 16140 e ISO 177994. RESULTADOS: con el método alternativo los resultados se obtuvieron a las 24 h, estos mostraron valores de sensibilidad (99 %) y exactitud (98 %) más elevados que con el de referencia (97 % y 97 %, respectivamente), el cual demoró más de 48 h. La eficiencia y exactitud de ambos métodos fue similar y se obtuvo una concordancia entre ellos casi perfecta (kappa = 0,96) con el total de las muestras de aguas ensayadas. CONCLUSIONES: los resultados alcanzados indican que el método alternativo es eficiente para el recuento de enterococos provenientes de diferentes tipos de muestras de agua. Resulta además un método simple, más rápido y económicamente factible.


INTRODUCTION: water quality control is essential for the protection of human health and biodiversity in the aquatic environment. For this purpose, several microorganisms are used as indicators of fecal contamination. Among them are enterococci, which are considered to be good indicators, since they survive for a longer time in adverse natural conditions. OBJECTIVES: evaluate the suitability of a new alternative chromogenic method for the detection and enumeration of enterococci in water by membrane filtration technique, in comparison with the ISO 7899:2 standard method. METHODS: water samples were collected from three different sources to determine their quality. The samples were assayed in parallel with the two methodologies (alternative and reference) using the membrane filtration technique. The following parameters were determined to evaluate the performance of the methods: sensitivity, specificity, accuracy, percentage of false positives, percentage of false negatives, kappa index. The first three parameters were estimated for both methods before and after confirmation by biochemical testing. Results were analyzed statistically based on the main criteria defined by ISO standards 16140 and 177994. RESULTS: with the alternative method, results were obtained at 24 h, whereas the reference method required more than 48 h. Sensitivity and accuracy were higher with the alternative method (99 % and 98 %, respectively) than with the reference method (97% and 97%, respectively). Both methods had similar efficiency and accuracy, with almost perfect concordance between them (kappa = 0.96) in all the water samples tested. CONCLUSIONS: results show that the alternative method is efficient for the enumeration of enterococci from different types of water samples. It is also a simple, faster and economically feasible method.

ABSTRACT

Debido a su uso por humanos y para preservar la salud pública, los ambientes marinos recreacionales, deberían cumplir requisitos de calidad fisico-química y microbiológica. Sin embargo, actividades antropogénicas, tales como agricultura, ganadería, industriales, turísticas y domésticas, pueden descargar aguas servidas en playas marinas. La metodología vigente para evaluar la carga bacteriana de aguas recreacionales, no pareciera satisfactoria, particularmente en la detección de microorganismos patógenos debido a la presencia, en aguas marinas, de bacterias viables pero no cultivables (VBNC). Este estudio evaluó la carga bacteriana de aerobios mesófilos, coliformes totales/fecales y enterococos, en playas de Chichiriviche, Falcón, Venezuela y el efecto de concentraciones de agua marina sobre las densidades bacterianas. Las determinaciones se efectuaron en agar nutritivo, Mac Conkey y KF, preparados con agua destilada (MAD) o con agua de mar 10% v/v, (MAM). La carga bacteriana se incrementó entre 20 y 47 veces cuando los medios de cultivo se suplementaron con agua de mar. Las diferencias en las cargas bacterianas entre MAD y MAM, para aerobios mesófilos, coliformes totales y fecales fueron estadísticamente significativas (p≤0,05). El oxígeno disuelto y pH, en ambos medios presentaron valores permisibles.


Due to their use by humans and to preserve public health, recreational marine environments should fulfill physico-chemical and microbiological quality prerequisites. Nevertheless, anthropogenic activities such as agriculture, cattle raising, industrial, touristic and domestic, can discharge contaminated water at marine beaches. The present methods for evaluating the bacterial load of recreational waters does not appear satisfactory, especially concerning the detection of pathogenic microorganisms due to the presence, in marine waters, of viable but not cultivable bacteria (VNCB). This study evaluated the bacterial load of mesophilic aerobials, total and fecal coliforms, and enterococci at the Chichiriviche beaches, Falcon State, Venezuela, and the effect of marine water concentrations over the bacterial densities. The determinations were done in nutrient, Mac Conkey and KF agar prepared with distilled water (MAD) or with sea water (MSW). The bacterial load increased between 27 and 47 times when the culture media were supplement with sea water. The differences of the bacterial loads between MAD and MSW for mesophilic aerobials, total and fecal coliforms were statistically significant (p≤0.05). Dissolved oxygen and pH presented permissible values in both media.

En Enterococcus spp. aislados de quesos de oveja se investigó la resistencia in vitro a antimicrobianos de utilización clínica. Se determinó la concentración inhibitoria mínima (CIM) para gentamicina, estreptomicina, vancomicina, teicoplanina, ampicilina, imipenem, linezolid y tigeciclina. Se detectó la producción de β-lactamasa y se realizaron experimentos de conjugación para transferir resistencia a gentamicina. En un aislamiento de E. faecalis y en 10 de E. faecium se observó resistencia a ampicilina; se demostró la producción de β-lactamasa en un E. faecalis. En E. faecium, 15 aislamientos fueron resistentes a imipenem; 4 a linezolid y 5 a glucopéptidos. En 4 aislamientos de E. faecalis se demostró alto nivel de resistencia plasmídica a gentamicina. No se observaron aislamientos resistentes a tigeciclina. El queso de oveja es un reservorio de enterococos resistentes con potencial diseminación al hombre a través de la cadena alimentaria.


In vitro resistance to clinically used antimicrobials was investigated in Enterococcus spp. isolated from goat cheese. The gentamycin, streptomycin, vancomycin, teicoplanin, ampicillin, imipenem, linezolid and tigecyclin minimum inhibitory concentration (MIC) was determined. Β-lactamase production was detected and conjugation experiments were carried out for transferring gentamycin resistance. In one E. faecalis and in 10 E. faecium isolates there was ampicillin resistance and β-lactamase production was demonstrated in one E. faecalis. Fifteen E. faecalis were imipenenem resistant; 4 linezolid resistant and 5 glucopeptide resistant. There was a high level of plasmidic resistance to tigecyclin in 4 E. faecalis isolates. There were no tigecyclin resistant isolates. Goat cheese is a reservoir of resistant enterococci with potencial human dissemination through the food chain.

Este trabalho objetivou verificar a presença de Staphylococcus e de Enterococcus sp nos manipuladores de alimentos de 10 entidades sociais de São José do Rio Preto – SP. Coletaram-se amostras das mãos e cavidades nasais utilizando-se swab estéril, que foi introduzido nas cavidades nasais e outro swab, umedecido com solução salina-estéril (NaCl 0,85%) passado na palma e nos dedos das mãos. A presença de Staphylococcus foi investigada através da semeadura por esgotamento na superfície do meio ágar manitol salgado, incubando-se a 35±2º por 48 horas e as colônias fermentadoras do manitol foram submetidas ao teste da coagulase. Os resultados mostraram que 53% dos manipuladores apresentaram estafilococos coagulase-positiva em suas mãos, 37% nas cavidades nasais e 37% apresentaram enterococos em suas mãos, sendo um indicativo de deficiência sanitária. Esses resultados indicam a necessidade de controle mais rígido da higiene pessoal para evitar casos de infecções e intoxicações alimentares.

La Bahía de Tumaco es un territorio donde desembocan ocho ríos en 27 bocanas, y los principales son el Mira y el Patía. De igual forma, es una bahía que no cuenta con un sistema de saneamiento básico adecuado, y allí se vierten directamente los desechos sin ningún tipo de tratamiento. Por esto se realizó un estudio en las aguas marinas de la Bahía de Tumaco en seis estaciones seleccionadas que presentan diferentes características, en el período comprendido entre marzo, junio y septiembre del año 2010. El número total de muestras analizadas fue de 36. Se determinó la distribución espacio temporal del indicador microbiológico de contaminación fecal (Enterococos), y adicionalmente se realizaron mediciones de los parámetros fisicoquímicos (salinidad y nutrientes) en cada sitio de muestreo. Las mayores concentraciones se encontraron en las estaciones (E23 y E26) de mayor influencia de aguas continentales y en la época húmeda y de transición (húmeda a seca). De igual forma, estas estaciones en los meses de marzo y junio se encontraron por fuera de los valores permitidos por la Organización Mundial de la Salud (40 UFC/100 mL). Se observó relación directa decreciente entre la abundancia de Enterococos y la salinidad, pero no con nutrientes (NH4+, NO2- y NO3-). Dado lo observado, es importante vigilar la calidad del agua de la bahía, la cual constituye una fuente potencial de riesgo para el desarrollo de las activadas económicas de la población tumaqueña.


Eight rivers empty into 27 estuaries in the area of Tumaco Bay. The most significant are Mira and Patía. The bay does not have an appropriate basic sanitation system, and untreated wastes are discharged directly into it. For this reason, a study was conducted of Tumaco Bay marine waters at six stations exhibiting different features in the months of March, June and September 2010. The total number of samples analyzed was 36. Determination was made of the spatio-temporal distribution of the fecal contamination microbiological indicator (Enterococci), and physical-chemical parameters (salinity and nutrients) were measured at each sampling site. The highest concentrations were found at Stations E23 and E26, where the influence of continental waters is greater, in the wet and transition (dry to wet) seasons. In the months of March and June, these stations lay outside the limits established by the World Health Organization (40 UFC/100 mL). A direct decreasing relationship was found between the abundance of Enterococci and salinity, but not with respect to nutrients (NH4+, NO2- y NO3-). Given the above findings, it is important to keep watch on the quality of the water in the bay, for it may pose a risk to the economic activities carried out by the Tumaco population.

INTRODUCTION: Laboratory-based surveillance is an important component in the control of vancomycin resistant enterococci (VRE). METHODS: The study aimed to evaluate real-time polymerase chain reaction (RT-PCR) (genes vanA-vanB) for VRE detection on 115 swabs from patients included in a surveillance program. RESULTS: Sensitivity of RT-PCR was similar to primary culture (75 percent and 79.5 percent, respectively) when compared to broth enriched culture, whereas specificity was 83.1 percent. CONCLUSIONS: RT-PCR provides same day results, however it showed low sensitivity for VRE detection.


INTRODUÇÃO: Vigilância com base em detecção laboratorial é um componente importante no controle de enterococos resistentes a vancomicina (ERV). MÉTODOS: Avaliamos procedimento da reação em cadeia da polimerase real time (PCR-RT) (genes vanA-vanB) para detecção de ERV em 115 swabs de pacientes incluídos em um programa de vigilância. RESULTADOS: A sensibilidade do RT-PCR foi semelhante a da cultura primária (75 por cento e 79,5 por cento, respectivamente) quando comparada com a cultura em caldo enriquecido, enquanto a especificidade foi de 83,1 por cento. CONCLUSÕES: O RT-PCR fornece resultados no mesmo dia, contudo mostra baixa sensibilidade para a detecção de VRE.

INTRODUCTION: In the past two decades members of the genus Enterococcus have emerged as important nosocomial pathogens worldwide. This study prospectively analyzed the distribution of species and trends in antimicrobial resistance among clinical isolates of enterococci in a Brazilian tertiary hospital from 2006-2009. METHODS: Enterococcal species were identified by conventional biochemical tests. The antimicrobial susceptibility profile was performed by disk diffusion in accordance with the Clinical and Laboratory Standards Institute (CLSI). A screening test for vancomycin was also performed. Minimal inhibitory concentration (MIC) for vancomycin was determined using the broth dilution method. Molecular assays were used to confirm speciation and genotype of vancomycin-resistant enterococci (VRE). RESULTS: A total of 324 non-repetitive enterococcal isolates were recovered, of which 87 percent were E. faecalis and 10.8 percent E. faecium. The incidence of E. faecium per 1,000 admissions increased significantly (p < 0.001) from 0.3 in 2006 to 2.3 in 2009. The VRE rate also increased over time from 2.5 percent to 15.5 percent (p < 0.001). All VRE expressed high-level resistance to vancomycin (MIC >256µg/ mL) and harbored vanA genes. The majority (89.5 percent) of VRE belonged to E. faecium species, which were characteristically resistant to ampicillin and quinolones. Overall, ampicillin resistance rate increased significantly from 2.5 percent to 21.4 percent from 2006-2009. Resistance rates for gentamicin, chloramphenicol, tetracycline, and erythromycin significantly decreased over time, although they remained high. Quinolones resistance rates were high and did not change significantly over time. CONCLUSIONS: The data obtained show a significant increasing trend in the incidence of E. faecium resistant to ampicillin and vancomycin.


INTRODUÇÃO: Nas últimas duas décadas, os enterococos emergiram como importantes patógenos nosocomiais no mundo inteiro. Neste estudo, foi analisada a distribuição das espécies e a evolução da resistência aos antimicrobianos entre isolados clínicos de enterococos obtidos em um hospital terciário, no período de 2006 a 2009. MÉTODOS: As espécies foram identificadas por testes bioquímicos convencionais e o perfil de sensibilidade foi determinado pelo método de disco difusão. A sensibilidade à vancomicina foi também determinada pela triagem em agar e pela concentração inibitória mínima (CIM). Testes moleculares foram utilizados para confirmar as espécies e determinar os genótipos dos enterococos resistentes à vancomicina (VRE). RESULTADOS: Foram analisadas 324 amostras de enterococos, sendo 87 por cento E. faecalis e 10,8 por cento E. faecium. A incidência de E. faecium por 1.000 pacientes internados aumentou significativamente (p < 0,001) de 0,3 em 2006 para 2,3 em 2009. A taxa de VRE também aumentou significativamente de 2,5 por cento para 15,5 por cento (p < 0,001). Todos os VRE apresentaram genótipo VanA e CIM >256µg/mL para vancomicina. A maioria (89,5 por cento) dos VRE pertencia à espécie E. faecium e foram resistentes à ampicilina e quinolonas. Foi observado um aumento significativo na taxa de resistência à ampicilina, de 2,5 por cento (2006) para 21,4 por cento (2009). As taxas de resistência para gentamicina, cloranfenicol, tetraciclina e eritromicina diminuíram significativamente no período do estudo. Para as quinolonas, as taxas de resistência foram elevadas não alteraram significativamente, no período do estudo. CONCLUSÕES: Os resultados do presente estudo mostram um aumento significativo na incidência de E. faecium resistentes à ampicilina e vancomicina.

ABSTRACT

Fifty-six Enterococcus spp. strains were isolated from foods in Southern Brazil, confirmed by PCR and classified as Enterococcus faecalis (27), Enterococcus faecium (23) and Enterococcus spp(6). Antimicrobial susceptibility tests showed resistance phenotypes to a range of antibiotics widely administrated in humans such as gentamycin, streptomycin, ampicillin and vancomycin.


Cinqüenta e seis cepas de Enterococcus spp. foram isoladas de alimentos no Sul do Brasil, confirmados por PCR e classificadas como Enterococcus faecalis (27), Enterococcus faecium (23) e Enterococcus spp. (6). Testes de susceptibilidade aos antimicrobianos demonstraram fenótipos de resistência a uma gama de antibióticos administrados em humanos, como gentamicina, estreptomicina, ampicilina e vancomicina.

ABSTRACT

The purpose of this study was to determine the prevalence of enteric bacteria and yeasts in biofilm of 80 HIV-positive patients with plaque-associated gingivitis or necrotizing periodontitis. Patients were subjected to extra, intra oral and radiographic examinations. The oral hygiene, bleeding on probing, gingival conditions, and attachment loss were evaluated. Clinical specimens were collected from gingival crevices or periodontal pockets, transferred to VMGA III, diluted and transferred to Sabouraud Dextrose agar with 100 µg/ml of chloramphenicol, peptone water, EVA broth, EMB agar, SS agar, Bile esculin agar and Brilliant green agar. Isolation of yeasts was carried out at room temperature, for 3-7 days; and for the isolation of enteric microorganisms plates were incubated at 37ºC, for 24-48 h. The yeasts identification was performed according to the carbon and nitrogen assimilation, fermentation of carbohydrates and germ tube formation. Bacteria were identified according to their colonial and cellular morphologies and biochemical tests. Yeasts were identified as Candida albicans and its occurrence was more common in patients with CD4+ below 200/mm³ and was affected by the extension of periodontal involvement (P = 0.0345). Enteric bacteria recovered from clinical specimens were identified as Enterobacter sakazakii, Enterobacter cloacae, Serratia liquefaciens, Klebsiella oxytoca and Enterococcus sp. Enterobacteriaceae and enterococci were detected in 32.5 percent of clinical samples from patients with necrotizing periodontitis. In conclusion, non-oral pathogenic bacteria and C. albicans were more prevalent in periodontal sites of HIV-positive patients with necrotizing periodontitis and chronic gingivitis.


O objetivo desse estudo foi avaliar a ocorrência de bactérias entéricas e leveduras no biofilme subgengival de pacientes HIV-positivos com gengivite crônica ou periodontite necrosante. Os pacientes foram submetidos a exame clínico e radiográfico e de higiene bucal, sangramento à sondagem, condições gengivais e a perda de inserção. Os espécimes clínicos de sulcos gengivais ou bolsas periodontais foram inoculados em ágar Sabouraud dextrose com 100 mg/ml de cloranfenicol, água peptonada, caldo EVA, ágar EMB, ágar SS, ágar Bile esculina e ágar verde brilhante. O cultivo de leveduras foi realizado à temperatura ambiente, de 3-7 dias; das enterobactérias a 37ºC de 24-48 h. A identificação das leveduras foi realizada pela assimilação de carbono e nitrogênio, fermentação de açucares e formação de tubo germinativo. As bactérias de acordo com a morfologia celular e colonial e testes bioquímicos. Foram identificadas Candida albicans e sua prevalência foi maior em pacientes com contagens de CD4+ < 200/mm³, e sua ocorrência foi afetada pela extensão da destruição periodontal (P = 0,0345). Enterobacteriaceae e enterococos foram detectados em 32,5 por cento dos pacientes com periodontite necrosante. As enterobactérias foram Enterobacter sakazakii, E. cloacae, Serratia liquefaciens, Klebsiella oxytoca e Enterococcus sp. Concluiu-se que bactérias patogênicas exógenas à cavidade bucal e C. albicans podem ser detectadas no biofilme subgengival de pacientes HIV-positivos com periodontite necrosante e gengivite.

ABSTRACT

This work evaluates the chemical and bacteriological qualities of the recreational waters of the Sauce Grande lagoon (Argentina). Samples were taken between March 2002 and December 2003. Ninety-six samples from three sampling stations were analyzed in order to determine the density of aerobic heterotrophic microorganisms, the presence of sulphite-reducing clostridia, and the most probable number of total coliforms, E. coli, fecal enterococci and P. aeruginosa. The water pH, temperature and chemical composition (N-NO3-, PO4³-, Na+, Ca++ +Mg++, EC and SAR) were also determined. Statistical analysis shows an increase in the microbial parameters of fecal pollution and in the population of heterotrophic microorganisms during the warmest months, influenced by higher temperatures and the more intensive recreational use. Bacterial count indicated that fecal pollution was statistically lower at the recreational area monitoring station; however, P. aeruginosa, an opportunistic pathogen, was present in higher than permitted densities in all determinations. These results show that, from the physico-chemical point of view, anthropogenic activities do not significantly affect the quality of the resource.


En el presente trabajo se evaluó la calidad bacteriológica y química en aguas de la laguna Sauce Grande (Argentina). Los muestreos fueron realizados entre marzo de 2002 y diciembre de 2003. Se analizaron un total de 96 muestras provenientes de tres estaciones de monitoreo, determinando: densidad de microorganismos heterótrofos mesófilos, presencia de clostridios sulfito-reductores y número más probable de coliformes totales, Escherichia coli, enterococos fecales y Pseudomonas aeruginosa. También se efectuaron determinaciones de pH, temperatura del agua y composición química (N-NO3-, PO4(3-), Na+, Ca++ + Mg++, CE y RAS). Se observó que en los meses más cálidos se produjo un aumento en los parámetros microbianos indicadores de contaminación fecal y en la población de microorganismos heterotrófos; dicho comportamiento estaría influenciado por el aumento de la temperatura y el mayor uso recreativo del recurso. El recuento de bacterias indicadoras de contaminación fecal fue menor en la estación de monitoreo donde se encuentra ubicado el balneario; no obstante, P. aeruginosa, patógeno oportunista, estuvo presente en todas las determinaciones con densidades mayores a los valores permitidos. Desde el punto de vista fisicoquímico, no hay un aporte antropogénico significativo de contaminantes que afecten la calidad del recurso.

ABSTRACT

Enterococcus spp. pertencem ao grupo das bactérias láticas e estão presentes em solos, águas, plantas, microbiota autóctone de vários alimentos e como membros da microbiota intestinal de humanos e animais. Esses microrganismos foram considerados por muito tempo como comensais, mas o aumento da severidade das infecções nosocomiais causadas por enterococos mutirresistentes a antimicrobianos e, a falta de conhecimento sobre seus fatores de virulência geram insegurança na utilização de cepas deste gênero na produção de alimentos como culturas fermentadoras e/ou probióticas. A diferença entre uma cepa de enterococos com potencial patogênico e outra aparentemente segura para uso em processamento de alimentos não é clara, e a probabilidade de que esta última adquira fatores de virulência merece investigação. O objetivo do presente projeto foi determinar características fenotípicas e genotípicas de Enterococcus spp. isolados de amostras de alimentos e águas correlacionando sua presença com indicadores clássicos de higiene e contaminação fecal. De 812 colônias indicativas do gênero enterococos obtidas a partir de 120 amostras de alimentos, 299 isolados (37%) foram presuntivamente caracterizados como Enterococcus spp. Após identificação por PCR, 139 (46,5%) E. faecium, 80 (26,8%) E. faecalis, 36 (12%) E. casseliflavus e 8 (2,7%) E. gallinarum. Produção de gelatinase foi detectada apenas em isolados de E. faecalis (60%). Um isolado de E. faecium (0,7%) e 31 isolados de E. faecalis (38,7%) apresentaram perfil ß-hemolítico. Produção de bacteriocina contra Lactobacillus sakei e/ou Listeria monocytogenes foi observada para 10% dos isolados de E. faecalis e 23% dos isolados de E. faecium. Hidrólise de sais biliares foi observada para 100% dos isolados de E. gallinarum, 86% E. casseliflavus, 65% E. faecalis e 62,6% de E. faecium. Alguns isolados de E. faecium apresentaram resistência à vancomicina, eritromicina e tetraciclina. Entre os isolados de E. faecalis não houve resistência à vancomicina, mas foi observada resistência à tetraciclina, eritromicina e alta concentração de gentamicina. Houve uma maior prevalência dos genes de virulência (esp, gel, ace, as, efaA e cylA) entre os isolados de E. faecalis quando comparado a E. faecium. Além disso, os isolados de E. faecalis, resistentes a antibióticos, mostraram forte adesão a células Caco-2 e capacidade de formação de biofilme em superfície abiótica. RAPD-PCR individualizou 14 cepas de E. faecium e 17 cepas de E. faecalis dentre os 52 isolados Enterococcus spp. resistentes a antibióticos. A variabilidade dos resultados impediu o estabelecimento de uma correlação entre a presença ou contagem de coliformes, E. coli e enterococos nas amostras analisadas. Os dados deste trabalho sobre marcadores fenotípicos e genotípicos de virulência, e a presença de cepas resistentes a antibióticos evidenciam a necessidade da avaliação cuidadosa de linhagens de enterococos para aplicações em alimentos


Enterococcus spp. belong to the group of lactic acid bacteria widely distributed in soil, plants, foods, animals and humans. In the past, these microorganisms were considered commensals but the increase of antibiotic-resistant enterococci and the lack of knowledge about their virulence markers, had raised concerns regarding the safety of using strains of this genus in the food production as fermentative or probiotic cultures. Besides this, literature data suggests the use of enterococci as sanitary indicator for foods. Differences between enterococci strains with pathogenic potential and an apparently safe ones is unclear and there is a concern about virulence markers transfer. The aim of this work was to determine phenotypic and genotypic characteristics of Enterococcus spp. isolated from foods and water and also to correlate their presence with classical indicators of sanitary quality. Out of 812 presumptive enterococci colonies obtained from 120 food samples, 299 isolates (37%) were presuntively characterized as Enterococcus spp. Isolates were identified by PCR: 139 (46.5%) E. faecium, 80 (26.8%) E. faecalis, 36 (12.0%) E. casseliflavus and 8 (2.7%) E. gallinarum. Only E. faecalis isolates (60%) produced gelatinase. One E. faecium (0.7%) and 31 E. faecalis (38.7%) were ß-haemolytic. Bacteriocin activity against Lactobacillus sakei and/or Listeria monocytogenes was observed for 10% of the E. faecalis and for 23% of the E. faecium isolates. All E. gallinarum isolates, 86% of the E. casseliflavus, 65% of the E. faecalis and 62.6% of the E. faecium isolates showed bile salt hydrolysis activity. Some E. faecium isolates were resistant to vancomycin, erythromycin and tetracycline. Vancomycin resistance was absent among the E. faecalis but, resistance to tetracycline, erythromycin and highlevel gentamicin was observed. There was a higher prevalence of virulence genes (esp, gel, ace, as, efaA e cylA) among the E. faecalis isolates when compared to the E. faecium. Antibiotic resistant E. faecalis isolates strongly adhered to Caco-2 cells and formed biofilm on abiotic surface. Using RAPDPCR 14 E. faecium and 17 E. faecalis strains could be individualized from the 52 antibiotic resistant enterococci. It was not possible to correlate the presence of total coliforms, E. coli and Enterococcus spp. in the samples of food and water analysed due to results variability. Data obtained regarding phenotypic and genotypic virulence markers and the presence of antibiotic resistant enterococci raise the needs of a carefully evaluation of the Enterococcus spp. strains before future applications in foods

En un período de cinco meses y 25 días se investigó la portación intestinal de enterococos resistentes a vancomicina (EVR). Se estudiaron 124 pacientes (73%) de 171 admitidos en la unidad de terapia intensiva (UTI), 35 de los cuales (28%) resultaron ser portadores. Los aislamientos de EVR (n=35) fueron identificados como Enterococcus faecium (n=18), Enterococcus gallinarum (n=16) y Enterococcus raffinosus (n=1). Todos los aislamientos estudiados fueron resistentes a vancomicina (VAN) (CIM90= 512 µg/ml) y teicoplanina (CIM90= 32 µg/ml) y portaban el gen vanA. Los estudios de tipificación molecular mostraron un alto grado de homología entre los aislamientos de E. faecium (un clon dominante) y E. gallinarum (dos tipos clonales), sugiriendo su diseminación a modo de brote. No se encontraron diferencias significativas con la edad y el sexo de los pacientes no portadores (p>0,05), pero si con el tiempo de hospitalización y el uso de esquemas antibióticos de amplio espectro (p<0,05), estando estos dos factores asociados al estado de portación. Se deduce de este estudio, la importancia de maximizar las medidas de prevención y control de las infecciones nosocomiales, analizar los esquemas empíricos empleados, tratar de disminuir el tiempo de hospitalización y continuar con los estudios de vigilancia para evaluar la eficacia de las acciones implementadas.


Intestinal tract colonization with vancomycin resistant enterococci (VRE) was studied during five months and 25 days. Out of 171 patients hospitalized in the intensive care unit, 124 (73%) were included in this study. Thirty five of them (28%) were recognized as colonized with VRE. VRE isolates (n = 35) were identified as Enterococcus faecium (n=18), Enterococcus gallinarum (n=16), and Enterococcus raffinosus (n=1). All of them were resistant to vancomycin (MIC90= 512 µg/ml) and to teicoplanin (MIC90= 32 µg/ml), having the vanA gene. By means of molecular methods a high homology was found among E. faecium and E. gallinarum isolates, respectively, suggesting their spread as a kind of outbreak. No significant differences in age or sex were found among colonized and non-colonized patients (p>0.05). On the other hand, the hospitalization time and the use of broad-spectrum antibiotics were associated with colonization. From this study we highlight the importance of enhancing all measures of control and prevention of hospital infections, carefully analyzing the empiric antimicrobial schemes, trying to reduce the hospital stage, and following the surveillance to evaluate the efficacy of such procedures.

This study was aimed at evaluating the microbiological quality of mangrove oysters (Crassostrea rhizophorae), collected at a natural oyster bed in the estuary of Cocó river (Fortaleza, Ceará, Brazil). MPN values were used for estimating the total (TC) and fecal (FC) coliforms and Enterococcus spp. TC and FC MPN values in the whole muscle and intervalve liquid ranged from 1.8 to >1,600/g and from 1.8 to 920/g, respectively. The MPN estimates for Enterococcus spp. were between 3.0 and >1,100/g. No correlation was found between the physico-chemical parameters (temperature, salinity and pH) of the surrounding water and the bacteriological contamination levels found in the tested oysters. The only correlation found was between TC and FC values. Enterococcus spp. strains were isolated and subjected to biochemical tests for species identification. The capacity of those strains for production of a bacteriocin-like inhibitory substance was tested using the Escherichia coli strain ATCC 25922 as a testing organism. Only one, E. faecalis, out of 121 Enterococcus strains tested, presented the inhibitory activity.


O objetivo deste trabalho foi avaliar a qualidade microbiológica da ostra de mangue (Crassostrea rhizophorae) originária de um criadouro natural no estuário do Rio Cocó, Fortaleza, Ceará, Brasil. Para isso, foram realizadas as estimativas do Número Mais Provável (NMP) de Coliformes Totais (CT) e de Fecais (CF) e de Enterococcus spp. Os valores encontrados para CT e CF no músculo (com líquido intervalvar) variaram de 1,8 a >1.600 e 1,8 a 920 por grama, respectivamente. O valor do NMP de Enterococcus spp. variou de 3,0 a >1.100/g. Não houve correlação entre os parâmetros físico-químicos (temperatura, salinidade e pH) da água na área do criadouro e os níveis de contaminação encontrados nas ostras. Somente houve correlação entre os valores de CT e CF. Cepas de Enterococcus spp. foram isoladas e submetidas a testes bioquímicos para identificação das espécies e, posteriormente, foram testadas para verificar a produção de substância inibitória semelhante à bacteriocina utilizando a cepa-teste Escherichia coli ATCC 25922. De um total de 121 cepas de Enterococcus spp. testadas apenas uma, E. faecalis, apresentou atividade inibitória.

Se evaluaron en forma prospectiva 108 cepas de enterococos (83 E. faecalis, 16 E. faecium, 5 E. avium y 4 E. gallinarum) aisladas de diversas muestras clínicas en la sección de Bacteriología del Laboratorio Metropolitano en el período comprendido entre enero y julio de 1999 con la finalidad de comparar cuatro de los métodos descritos para la detección de resistencia a vancomicina in vitro en este grupo de gérmenes: difusión en disco (DD), E test (AB Biodisk, Solina, Sweden), el sistema automatizado Vitek a través de la tarjeta GPS-102 (bioMérieux, Inc., Hazelwood, Mo.), tomando como referencia el agar cerebro-corazón con 6 µ/ml de vancomicina (VSA), observándose una correlación del 93,56 por ciento, 100 por ciento y 97,52 por ciento respectivamente. Se recomienda el uso en paralelo de cualquiera de los métodos en conjunto de la VSA para la segura detección de cepas de enterococcos con sensibilidad disminuida a la vancomicina.
