ABSTRACT

Objective To investigate the effect of high sodium diet on the prevalence and mortality of gastric cancer in Xi'an , and to provide reference for its clinical treatment and prevention. Methods A questionnaire survey was conducted on 604 inpatients in tertiary hospitals in Xi'an from 2018 to 2020, and data including medical history, medication use, daily sodium intake and gastric cancer incidence were collected. A one-year follow-up was completed, and all-cause mortality was recorded during the follow-up period. Results A total of 604 cases , there were 81 cases of gastric cancer and 523 cases of non-gastric cancer, with a prevalence rate of 13.41%. Among patients with gastric cancer, 49 (8.11%) were male and 32 (5.30%) were female. There were 9 cases (1.49%) aged 30-40 years, 14 cases (2.32%) aged 41-50 years, 27 cases (4.47%) aged 51-60 years and 31 cases (5.13%) aged 60 years and above. The gastric cancer and non-gastric cancer patients yielded no statistical difference in terms of age, gender and medical history (P>0.05), while the proportion of sodium intake was significantly higher in gastric cancer patients than in non-gastric cancer patients (P<0.05). Logistics analysis showed that high sodium diet was a risk factor for the development of gastric cancer (P<0.05). The proportion of high sodium diets was 40.89% (247/604) in all selected individuals, 72.84% (59/81) in gastric cancer patients and 35.95% (188/523) in non-gastric cancer patients. The proportion of patients on a high sodium diet demonstrated statistical difference between gastric cancer and non-gastric cancer patients (P<0.05). One-year follow-up showed that 58 of 81 gastric cancer patients survived (71.60%) and 23 died (28.40%), including 20 survivors (34.48%) and 17 deaths (73.91%) on a high-sodium diet. Significant difference was found in the proportion of patients on high sodium diet between dead and surviving gastric cancer patients (P<0.05). Conclusion The prevalence and mortality rate of gastric cancer caused by high sodium diet is high among residents in Xi'an , clinical guidance on sodium intake is of great importance to patients.

@#Introduction: Hypertension is a public health concern that is associated with heart disease. This study aims to investigate the prevalence of undiagnosed hypertension and its associated factors among the staff in a private university in Selangor, Malaysia. Methods: 100 respondents participated in this cross-sectional study. Blood pressure, Body Mass Index (BMI), percentage body fat (%BF), visceral fat rating (VFR), and waist circumference (WC) were measured. Questionnaires regarding knowledge, attitude, and practice (KAP) of salt diet validated from WHO/PAHO and Food Frequency Questionnaires regarding sodium intake validated from IPH Malaysia were administered. Statistical data was analysed using SPSS. Results: The prevalence of undiagnosed hypertension was 24%. About 66% of the participants had excessive sodium intake. The mean sodium intake was 2869.43±930.75mg/d. Simple linear regression showed that BMI. %BF, VFR, and WC were significantly correlated with systolic blood pressure (SBP). Age and sodium intake were not correlated with SBP. After controlling all the variables in multiple linear regression, VFR remained as a significant contributor to SBP (adjusted R2=0.419, F=18.833, p=<0.001). Soy sauce, omelette, fried rice, and nasi lemak were the main contributors of sodium intake. A lack of knowledge on the negative impacts of high salt diet was significantly associated with high dietary sodium intake (>2400mg/d). Conclusion: Our study found a high prevalence of undiagnosed hypertension among the private university staff. The association between VFR and SBP is an important finding for community study. Increased awareness on the excessive salt consumed and its association with health is needed to reduce the sodium intake.

ABSTRACT

Objectives: To investigate the impact of high-salt diet on plasma level of vascular endothelial growth factor C (VEGF-C) in healthy subjects and hypertension patients; to explore the relationship between VEGF-C level and blood pressure (BP). Methods: Our research included in 2 groups: Essential hypertension (EH) group, 75 patients treated in our hospital from 2013 to 2014 and Control group, 98 healthy subjects at the same period. Using salt-intake 6 g/day as the borderline, both groups were respectively divided into High-sodium diet (HS) subgroups and Low-sodium diet (LS) subgroups. The age, gender, creatinine clearance (CCr), UA, fast blood glucose (FBG), body mass index (BMI), blood lipids (TC, TG, HDL-C, LDL-C), systolic blood pressure (SBP), diastolic blood pressure (DBP), pulse pressure (PP), mean arterial pressure (MAP), heart rate (HR) and CF-PWV were compared between EH group and Control group, HS subgroups and LS subgroups. Results: Compared with Control group, EH group had increased plasma level of VEGF-C, (3 940.8±1 141.1) pg/ml vs (2 938.0±987.0) pg/ml, P<0.001; the age, BMI, SPB, DBP, PP, MAP and CF-PWV were different between 2 group, all P<0.005. In ES group, compared with LS subgroup, HS subgroup showed the higher VEGF-C (4 208.8±113.1) pg/ml vs (3 515.8±1 070.1) pg/ml, P=0.009; the age, SBP, DBP and PP were different between 2 group, all P<0.005. In Control group, compared with LS subgroup, HS subgroup showed the higher VEGF-C (3 158.7±917.2) pg/ml vs (2 655.7±1 011.3) pg/ml, P=0.012; the age BMI and CCr were different between 2 group, all P<0.005, while BP was similar between 2 subgroups. Spearman correlation study presented that with adjusted confounding factors, no matter in all participates and in EH group or Control group, MAP were positively related to plasma levels of VEGF-C (r=0.536, P<0.001 and r=0.546, P=0.002 or r=0.291, P=0.006) respectively. Conclusions: High-sodium diet could increase plasma VEGF-C level in either healthy subjects or hypertension patients, VEGF-C level was positively related to BP.

ABSTRACT

OBJECTIVES: This study was conducted to examine blood pressure and other characteristics of a high sodium intake group assessed with 24-hr urine analysis and the dietary factors related to the risk of high sodium intake among Korean adults. METHODS: A cross-sectional study was conducted with adults aged 20-59 years. Subjects who completed 24-hr urine collection (N = 205) were divided into 3 groups (tertile) according to the sodium intake estimated with 24-hour urine analysis. We compared the blood pressure, BMI and dietary related factors of the 3 groups (low, medium, high sodium intake group) with General Linear Model (GLM) and Duncan's multiple range test (p or = 25) (OR = 2.619; 95% CI: 1.368-5.015), current alcohol consumption (OR = 1.943; 95%CI: 1.060-3.564), and having salty soybean paste with salt percentage > 14% (OR = 3.99; 95% CI: 1.404-6.841). The dietary attitude related to increased risk of high sodium intake included 'enjoy dried fish and salted mackerel' (p < 0.001) and 'eat all broth of soup, stew or noodle' (p < 0.001). CONCLUSIONS: Because high sodium intake was associated with higher blood pressure, nutrition education should focus on alcohol consumption, emphasis on related dietary factors such as using low salt soybean paste, improvements in the habit of eating dried fish and salted mackerel or eating all broth of soup, stew or noodle.

ABSTRACT

This study investigated the preference and intake frequency of a list of 15 commonly available high sodium Malaysian foods/dishes, discretionary salt use, and their possible association with demographics, blood pressures and anthropometric measurements among 300 Malaysian university students (114 males, 186 females; 259 ethnic Chinese, 41 Indians; 220 lean, 80 overweight). French fries and instant soup noodle were found to be the most preferred and most frequently consumed salty food, respectively, while salted fish was least preferred and least frequently consumed. Males had a significantly higher intake frequency of at least 6 of the salty foods, but the preference of most salty foods was not significantly different between genders. Ethnic Chinese significantly preferred more and took more frequently traditional and conventional Malaysian foods like asam laksa (a Malaysian salty-sour-spicy noodle in fish stock), salted biscuits and salted vegetable, while Indians have more affinity and frequency towards eating salty Western foods. Body Mass Index was significantly negatively correlated with the intake frequency of canned/packet soup and salted fish while waist circumference was significantly positively correlated with the preference of instant noodle. Also, an increased preference of potato chips and intake frequency of salted biscuits seemed to lead to a decreased WHR. Other than these, all the other overweight/obesity indicators did not seem to fully correlate with the salty food preference and intake frequency. Nevertheless, the preference and intake frequency of asam laksa seemed to be significant negative predictors for blood pressures. Finally, increased preference and intake frequency of high sodium shrimp paste (belacan)-based foods like asam laksa and belacan fried rice seemed to discourage discretionary salt use. In conclusion, the preference and intake frequency of the high sodium belacan-based dish asam laksa seems to be a good predictor for ethnic difference, discretionary salt use and blood pressures.

ABSTRACT

OBJECTIVE: The objective of this study was to determine whether complications and nursing interventions during hemodialysis could be reduced by using gradient ultrafiltration(GUF) and high sodium dialysate(HSD). METHODS: Thirty outpatients who had been on hemodialysis for more than 3 months were enrolled. Patients received Conventional HD(Na+ 137 mEqX4 hrs, constant UF), GUF(UF 62.5%X2 hrs, UF 37.5 %X2 hrs), HSD(Na+ 145 mEqX2 hrs, Na+ 140 mEqX2 hrs) and HSUF(GUF and HSD). Each methods were prescribed for 2 weeks. The clinical features such as thirst, interdialytic weight gain, postdialytic weight loss and MAP(mean arterial pressure) and the frequency of intradialytic complications and nursing interventions were evaluated. RESULTS: The results were as followings. Interdialytic thirst, weight gain, postdialytic weight loss and MAP(pre/post HD) were not significantly different in each method. On the other hand, there was significant difference between 4 methods in serum sodium, osmolality and Hct. There were significantly fewer episodes of hypotention, muscle cramp and headache when using GUF, HSD and HSUF than CHD. The number of nursing interventions was significantly reduced in GUF, HSD and HSUF than CHD. CONCLUSION: Hemodialysis with gradient ultrafiltration and high sodium dialysate leads fewer complications and nursing interventions, more patient's well being.

ABSTRACT

Hypotensive episode is one of the serious complication during hemodialysis. HSD has used for relief of this problem, but sometimes increased interdialytic weight gain or exacerbation of hypotension are happened. To evaluate the effect of HSD, 7 non- diabetic normotensive uremic patients were hemodialyzed with different sodium level of dialysate (stage A : Na 138mEq/L for 4hrs, stage B : Na 148mEq/L for 4hrs, stage C : initially Na 148mEq/L for 3hrs and Na 138mEq/L for 1hrs), and each stages were continued for 2 weeks. Ultrafiltration was performed to maintain the patient's estimated dry weight constantly. Interdialytic weight gain(stage A : 2.9+/-1.2kg, stage B : 3.2+/-1.1kg, stage C : 3.1+/-0.8kg) and presystolic systolic and diastolic blood pressure were not different in each stage. The incidence of hypotension (systolic BP<90mmHg) during hemodialysis was significantly lower in stage B and stage C(24%, 21%) than stage A(54%)(P<0.01). During hemodialysis the incidence of thirst was higher in stage B(40%) than stage A and stage C(11%, 12%) (P<0.05) significantly but there are no difference in headache and itching during hemodialysis between the each stage. Pre-dialysis and post-dialysis serum sodium and osmolality were not different in each stage but at 2 hrs after initiation of hemodialysis serum sodium and osmolality were higher in stage B(145.6+/-2.1mEq/L, 306+/-6.7mOsm/kg) and stage C (146.1+/-2.1mEq/L, 306+/-13.1mOsm/kg) than stage A (140.1+/-2.5mEq/L, 292+/-8.7mOsm/kg)(P<0.05). The ANP levels of pre-dialysis and post-dialysis were not different in each stage. In conclusion, HSD improved hypotenive episode. In spite of sodium load, increased interdialytic weight gain and elevation of pre-dialysis blood pressure were not developed and sodium modeling during hemodialysis attenuate some adverse effect of HSD.
