ABSTRACT

OBJECTIVE@#To observe the clinical therapeutic effect of herb-separated moxibustion at Jinsuo (GV 8)- eight-diagram points on diarrhea-type irritable bowel syndrome (IBS-D) of liver stagnation and spleen deficiency as compared with oral administration of pinaverium bromide tablets and Chinese herbal decoction, .@*METHODS@#A total of 126 patients with IBS-D of liver stagnation and spleen deficiency were randomized into a herb-separated moxibustion group (moxibustion group), a western medication group and a Chinese herbal medication group, 42 cases in each one. In the moxibustion group, the herb-separated moxibustion was applied to Jinsuo (GV 8)-eight-diagram points. The herbs in (fried , fried , and ) were ground into herbal paste and the paste was put on Jinsuo (GV 8)-eight-diagram points. The suspending moxibustion was exerted over the points for 40 min, once daily. In the western medication group, pinaverium bromide tablets were taken orally, 50 mg each time, three times a day. In the Chinese herbal medication group, the decoction of was taken orally, one dose a day, taking separately in two times. The duration of treatment was 8 weeks in each group. Before and after treatment, the symptom score of traditional Chinese medicine (TCM), gastrointestinal (GI) symptom score, the score of IBS symptom severity scale (IBS-SSS) and the score of IBS quality of life (IBS-QOL) scale were observed in patients of each group separately. The clinical therapeutic effect was evaluated.@*RESULTS@#After treatment, the scores of TCM symptoms, GI symptom scores and IBS-SSS scores were all obviously reduced in each group (<0.05). Each of the scores in the moxibustion group was lower than the western medication group and the Chinese herbal medication group respectively (<0.05). After treatment, the scores of each of eight subscale structures of IBS-QOL scale, named dysphoria, interference with activity, body image, health worry, food avoidance, social reaction, sexual intercourse and relationship, were all increased obviously in each group (<0.05). The scores of each of eight subscale structures in the moxibustion group were higher than the western medication group and the Chinese herbal medication group respectively (<0.05). The total effective rate was 92.9% (39/42) in the moxibustion group, higher than 71.4% (30/42) in the western medication group and 73.8% (31/42) in the Chinese herbal medication group respectively (<0.05).@*CONCLUSION@#Herb-separated moxibustion at Jinsuo (GV 8)-eight-diagram points remarkably relieves gastrointestinal symptoms and improves the quality of life in patients of diarrhea-type irritable bowel syndrome of liver stagnation and spleen deficiency, and its clinical therapeutic effect is superior to oral administration of either pinaverium bromide tablets or .

ABSTRACT

OBJECTIVE@#To verify the clinical effect of acupuncture at "experienced ten acupoints" for functional dyspepsia (FD) of liver stagnation and spleen deficiency.@*METHODS@#A total of 96 patients with FD of liver stagnation and spleen deficiency were randomly divided into an observation group and a control group, 48 cases in each group. In the observation group, "experienced ten acupoint" prescription was applied,acupoints included Shangwan (CV 13), Zhongwan (CV 12), Xiawan (CV 10), Qihai (CV 6), Tianshu (ST 25), Zusanli (ST 36) and Neiguan (PC 6). In the control group, irrelevant acupoint prescription was applied, acupoints included Feiyang (BL 58), Heding (EX@*RESULTS@#Compared before treatment, the symptom score after treatment was decreased in the observation group (@*CONCLUSION@#Acupuncture at "experienced ten acupoints" can improve symptoms in patients with FD of liver stagnation and spleen deficiency, has the better clinical effect.

ABSTRACT

To evaluate the clinical efficacy and safety of Shugan Jieyu Capsules,Jieyu Pills and Xiaoyao Pills in the treatment of liver-stagnation and spleen-deficiency depression by the network Meta-analysis( NMA),so as to provide evidence-based clinical practice. Chinese databases,including CNKI,VIP,Wan Fang Data,CBM as well as English databases including the Cochrane Library,PubMed,EMbase and Web of Science were retrieved from inception to October 30,2018,to collect randomized controlled trials( RCTs) about clinical study of the three kinds of Chinese patent medicines. The quality and bias risk of the included studies were assessed by 5. 1 standard in Cochrane Handbook,ADDIS software was applied in the statistical analysis,and the result were compared by NMA. A total of 37 studies involving 3 105 patients were included. The results of NMA showed that the adjuvant therapy with the three kinds of traditional Chinese patent medicines could improve the clinical efficacy. Jieyu Pills had the most significant effect( OR = 4. 59,95%CI[1. 94,12. 71],P<0. 05). In HAMD,Shugan Jieyu Capsules( MD = 3. 22,95%CI[2. 07,4. 39],P< 0. 05) and Xiaoyao Pills( MD= 2. 23,95%CI[0. 41,4. 03],P<0. 05) can effectively reduce the depression scale indicators. In the incidence of adverse reactions,the three kinds of Chinese patent medicines can reduce the incidence of adverse reactions. The three kinds of Chinese patent medicines can be combined with auxiliary Western medicine to treat liver-stagnation and spleen-deficiency depression,with complementary advantages in action mechanisms. In the clinical efficacy and safety,the Chinese patent medicines had good clinical manifestations. Although this study showed that Jieyu Pills may be the referred medicine for depression,this conclusion is still immature and needs to be further verified by high-quality RCTs studies,and the application shall be selective based on specific characteristics in clinic.

ABSTRACT

Objective: To observe the clinical efficacy of Hegan granule in the treatment of non-erosive reflux disease with liver stagnation and spleen deficiency syndrome and its effect on quality of life, 5-hydroxytryptamine (5-HT) and vasoactive intestinal peptide (VIP) level. Method: Patients with non-erosive reflux disease of liver stagnation and spleen deficiency were randomly divided into treatment group (32 cases) and control group (33 cases). The treatment group was orally given Hegan granules after meal, 10 g/bag, 1 bag/time, 2 times/day. The control group was orally given with omeprazole enteric-coated tablets twice daily, 20 mg/grain. Both groups were treated for 8 weeks. The total scores of traditional Chinese medicine (TCM) symptoms, SF-36 health scale scores, 5-HT and VIP levels were observed before and after treatment. Result: The medical efficacy of the two groups was 87.5%and 81.8%in the treatment group and the control group, respectively. The treatment group was superior to control group (PPPPConclusion: Hegan granule has a good clinical efficacy in treating patients with non-erosive reflux disease and liver stagnation and spleen deficiency. It can significantly alleviate the symptoms of TCM, improve the quality of life of patients, and reduce the sensitivity of esophageal viscera.

ABSTRACT

Depression belongs to the category of "stagnation syndrome" in traditional Chinese medicine. There are many clinical types of depression in which liver stagnation and spleen deficiency syndrome is one of the main types. As a classic prescription for liver-soothing and spleen-strengthening, Xiaoyaosan has a definite curative effect in the treatment of depression due to liver stagnation and spleen deficiency. With fewer adverse reactions, a high safety and good patient compliance, it can be combined with western medicine to reduce the recurrence rate and adverse reactions of western medicine, and has become the focus and hotspot of research in recent years. Although many experts and scholars have conducted a lot of research on the pharmacological mechanism of Xiaoyaosan in the treatment of depression due to liver stagnation and spleen deficiency, its mechanism is still unclear. In addition, there are many types of animal models for depression due to liver stagnation and spleen deficiency but only a few systematic reports on Xiaoyaosan in treating depression of liver stagnation and spleen deficiency. Therefore, based on two classic models of chronic mild unpredictable stress depression model (CUMS) and chronic restraint stress depression model (CIS), a review was conducted on the pharmacological mechanisms of Xiaoyaosan in the treatment of depression due to liver stagnation and spleen deficiency by regulating neurotransmitter concentration, affecting the expression of neurotrophic factor, regulating hytothalamus-pitutary-adrenal(HPA)axis imbalance, improving intestinal micro-ecology and gastrointestinal function, regulating endogenous metabolite levels and immune inflammatory factor levels and improving synaptic structure and plasticity, so as to provide potential guidelines for basis study and clinical application of Xiaoyaosan in the treatment of depression due to liver stagnation and spleen deficiency.

ABSTRACT

Objective@#To discuss the clinical efficacy of Sini powder and scattered seven Atractylodes in the treatment of functional dyspepsia with hepatic stagnation and spleen deficiency syndrome.@*Methods@#A total of 100 cases of functional dyspepsia with hepatic stagnation and spleen deficiency syndrome in our hospital were selected and randomly divided into the western medicine group and Chinese medicine group by envelope grouping method.The patients in western medicine group were treated with mosapride tablets, 5mg each time, and the patients in Chinese medicine group were treated with Sini powder and scattered seven Atractylodes, 150mL each time, two times a day for 4 weeks.The efficacy, abdominal distension scores and adverse drug reactions were compared between the two groups.@*Results@#In the Chinese medicine group, 40 cases were obviously improved, 8 cases were improved, 2 cases were invalid, and the total clinical effective rate was 96.0%.In the western medicine group, 30 cases were obviously improved, 12 cases were improved and 8 cases were invalid.The total clinical effective rate was 80.4%.The total effective rate of the Chinese medicine group was higher than that of the western medicine group, the difference was statistically significant(χ2=8.239, P=0.041). The abdominal distension scores of the two groups after treatment was lower than those before treatment, but the abdominal distension score of the Chinese medicine group[(2.4±2.7)points]was significantly lower than that of the western medicine group[(2.9±2.2)points], the clinical efficacy of the traditional Chinese medicine group was better than that of the Western medicine group(t=3.794, P=0.006). No serious adverse reactions were found in the two groups.@*Conclusion@#Sini powder and scattered seven Atractylodes can effectively improve the clinical symptoms of liver stagnation and spleen deficiency in patients with functional dyspepsia, and has no obvious adverse reactions.

ABSTRACT

At present, TCM treatment of diarrhea-predominant irritable bowel syndrome (IBS-D) is based on the combination of disease differentiation and syndrome differentiation. Therefore, the establishment of IBS-D of liver stagnation and spleen deficiency syndrome combined with animal model as a combination of traditional Chinese and Western medicine innovation theory has become increasingly concerned about, and gradually become a new direction for the development of TCM experimental animal model. This article reviewed the research progress in IBS-D liver and spleen deficiency syndrome in recent years, discussed the establishment of IBS-D liver stagnation and spleen deficiency animal model and research ideas for the treatment of IBS-D, and provided references for mechanism research of TCM treatment for IBS-D and research and development of new medicine.

ABSTRACT

To evaluate the effect of Tongxie Yaofang on cardiac endogenous metabolism in irritable bowel syndrome(IBS) rats by using metabolomics method, find its potential biomarkers, analyze the metabolic pathways, and explore the pharmacological effects, mechanisms of action and syndrome essence of syndrome model. Forty Wistar rats were used to establish IBS models, and then randomly divided into four groups: model control group and Tongxie Yaofang treatment groups (high, medium, low dose). Another 10 rats were used as normal group. The rats in Tongxie Yaofang-treated(low, medium and high dose) groups were orally administrated with Tongxie Yaofang extracts once a day for 2 weeks, respondingly with the doses of 0.203,0.406,0.812 g•mL⁻¹. The rats in normal group and model control group were given with equal volume of saline once a day for 2 weeks. On the 0 and 15th days, serum was collected and each sample extract was analyzed by UPLC-Q-TOF-MS. Eight potential biomarkers were identified and 8 major metabolic pathways were found to be related with IBS diseases neurotransmitter metabolism, inflammatory immunity, brain function and energy metabolism, etc. Tongxie Yaofang had certain pharmacological effects on IBS, and its mechanism may be related to serotonergic synapse, tryptophan metabolism, cysteine and methionine metabolism, glycerophospholipid metabolism, nicotinate and nicotinamide metabolism and so on, which might be the biological basis of IBS liver-spleen deficiency syndrome.

Objective: To observe the effect of Weichang'an Pill on regulating the gastrointestinal motility and plasma motilin (MTL) level in liver stagnation and spleen deficiency type of functional dyspepsia (FD) rat model. Methods: Sixty rats were randomly divided into six groups (n = 10): control group, model group, Weichang'an pill high (50.4 mg/kg), medium(25.2 mg/kg) and low (12.6 mg/kg) dose (middle dose for clinical equivalent dose) groups, Domperidone Tablets group as positive control group, all groups were intragastric administered for 7 d. The gastric remnant rate and plasma MTL level in rats were measured after administration. Results: The gastric residual rate in Weichang'an pill high dose group and Domperidone Tablets group decreased significantly compared with the model group (P < 0.05); The plasma MTL levels in Weichang'an pill high dose group were significantly increased compared with the control group and model group (P < 0.05). The plasma MTL in Weichang'an pill dose group was significantly higher than that of model group (P < 0.05). Conclusion: Weichang'an Pill can regulate the MTL level so that improve gastric motility and furthermore play a therapeutic effect on FD.

ABSTRACT

Objective To explore the clinical effect of the application of modified Chaihuguizhi dried ginger soup in the treatment of liver stagnation and spleen deficiency migraine.Methods 102 cases of liver stagnation and spleen deficiency migraine patients treated in our hospital from June 2014 to February 2016 were selected and randomly divided into two groups.The control group were given conventional western medicine; the observation group were given modified Chaihuguizhi dried ginger soup.The clinical curative effect, accompanied symptoms before and after the treatment, duration, degree of pain, the symptom scores of episode, and plasma specific viscosity, whole blood viscosity, hematocrit of red blood cell (HCT), serum 5-hydroxytryptamine(5-HT) level, medication safety during the treatment of the groups were recorded and compared.Results The total effective rate of the observation group (92.16%) was significantly higher than the control group (76.47%) (P<0.05); accompanied symptoms, duration, degree of pain and the number of attacks of the two groups after the treatment, compared with those before the treatment, improved significantly( P<0.01), and the decrease degree of the observation group was more significant(P<0.01);compared with those before the treatment, the plasma specific viscosity, blood viscosity, HCT and 5-HT levels improved significantly in the two groups after the treatment(P<0.01), and the observation group was better(P<0.01).Conclusion The application of Chaihuguizhi dried ginger soup in the treatment of migraine of liver stagnation and spleen deficiency is more conducive to ease the clinical symptoms, with more remarkable curative effect, safely and reliably, which has high clinical reference value.

ABSTRACT

Objective To investigate the clinical effect of traditional Chinese medicine(TCM)for treating HBeAg positive chron-ic hepatitis B virus(HBV)carriers with liver stagnation and spleen deficiency syndrome.Methods The patients with HBeAg and HBV DNA positive,normal serum ALT and AST for successive 3 times within 1-year follow up,complicating different degrees of symptoms were included.Among them,the patients with liver stagnation and spleen deficiency syndrome according to the TCM dif-ferentiation were selected as the treatment obj ects and treated by the prescription of soothing the liver,tonifying spleen and detoxifi-cation.The changes of clinical symptoms and HBV markers after 6-month therapy were observed and followed up for 3 months for evaluating the effect persistance.348 cases were recruited,161 cases in the control group and the other 187 cases in the treatment group.Treatment group underwent traditional Chinese medicine treatment.Results 168 cases finished the therapy with the total effective rate of 80.3%(135/168),6 cases were HBeAg negative conversion and 14 cases were HBV DNA decrease greater than or equal to 2 logarithmic grades;157 patients finished 3-month follow-up and the effective rate was 80.9%(127/157),4 cases were HBeAg negative conversion and 1 1 cases were HBV DNA decrease greater than or equal to 2 logarithmic grades.The curative effect of treatment group was higher than that of control group.Conclusion The TCM prescription of soothing the liver,tonifying spleen and detoxification has definite effect for treating HBeAg positive chronic HBV carriers with liver stagnation and spleen deficiency syndrome,the symptoms can be significantly improved.
