ABSTRACT

The effect of acupuncture cooperated with low-frequency repetitive transcranial magnetic stimulation (rTMS) on chronic insomnia was explored.Seventy-eight patients with chronic insomnia were randomly allocated into two groups:treatment group and control group.In the treatment group,the patients received acupuncture combined with rTMS treatment,and those in the control group were given acupuncture cooperated with sham rTMS treatment,3 days per week for 4 weeks.Before and after treatment,the primary outcomes including the scores on Insomnia Severity Index (ISI) and Pittsburgh Sleep Quality Index (PSQI) and the secondary outcomes including total sleep time (TST),sleep onset latency (SOL),wake after sleep onset (WASO),sleep efficiency (SE%) recorded by sleeping diary and actigraphy were observed in both groups.Seventy-five participants finished the study (38 in treatment group and 37 in control group respectively).After treatment,the scores in the two groups were improved significantly,more significantly in the treatment group than in the control group.It can be inferred that acupuncture cooperated with rTMS can effectively improve sleep quality,enhance the quality of life of patients and has less side effects.

ABSTRACT

Objective To investigate the efficacy of low-frequency repetitive transcranial magnetic stimulation (rTMS) treatment of patients with alcohol dependence,anxiety and depression,and sleep problems.Methods 80 patients with alcohol-dependent patients were randomly divided into experimental group (40 cases) and control group (40 cases).All patients received routine treatment and care,the experimental group received additional rTMS to stimulate parts of the bilateral dorsolateral prefrontal region (DLPFC) 2 brain regions,each side 15 minutes at frequency of 1Hz,patient motion stimulus,intensity threshold (MT) of 80%,five times a week rTMS treatment,a total of 8 weeks,the control group received stimulation coil.Subjects were interviewed,as the time for the baseline,2 weeks,4 weeks and 8 weeks,assessment tools for the hamilton anxiety scale (HAMA),Hamilton depression rating scale (HAMD),Pittsburgh sleep quality index (PSQI).Results Compared with the pre-treatment,HAMA scores (19.5 ± 6.4,13.8± 5.3,7.3± 5.4 respectively) were significantly lower in 2,4,8 weeks after treatment (t 2 =11.29,P<0.01;t 4=15.26,P<0.01;t s =9.40,P<0.01).Compared with the control group,HAMA scores were significantly lower in the experimental group after treatment (P<0.01).Compared with the pretreatment group HAMD scores(17.5±6.6,11.2±4.9,6.1±5.2 respectively)were significantly lower in 2,4,8 weeks after treatment (t 2 =15.54,P<0.01;t 4 =16.68,P<0.01;t s =18.77,P<0.01).Compared with the control group,HAMD scores were significantly lower in the experimental group after treatment (P<0.01).Compared with the control group,PSQI scores were significantly lower in the experimental group in 2,4,8 weeks after treatment (t 2 =26.39,P<0.01;t4=15.21,P<0.01;ts=46.35,P<0.01).Condusion Low-frequency rTMS treatment for alcohol dependence withdrawal symptoms can improve symptoms,such as anxiety and depression,sleep status.

ABSTRACT

@# Objective To explore the effect of low frequency repetitive transranial magnetic stimulation (rTMS) on unilateral spatial neglect (USN). Methods 40 stroke patients with USN were divided into treatment group (n=20) and control group (n=20). Patients in the treatment group were treated with low frequency rTMS for 2 weeks. The USN degree of these groups were evaluated before and after the treatment. Results There was no significant difference of USN degree between these groups before the treatment (P>0.05); Compared with the control group, the treatment group improved significantly after the treatment (P<0.05). The USN degree of patients in the treatment group decreased significantly after the treatment (P<0.05), while patients in the controlled group had no difference (P>0.05). Conclusion USN induced by stroke could be improved obviously through low frequency rTMS.

ABSTRACT

Objective To investigate the effects of low frequency repetitive transcranial magnetic stimulation (rTMS) on motor function and excitability of motor cortex in Parkinson's disease (PD) patients and to study the mechanism of PD from the electrophysiology. Methods Twenty-eight patients with PD received 1 Hz rTMS therapy for 15 d. Thirty normal volunteers were enrolled as controls. Unified Parkinson's Disease Rating Scale (UPDRS) and motor evoked potential (MEP) were adopted as assessment indicators. The excitability of motor cortex was assessed by rest motor threshold (RMT), central motor conduction time (CMCT) and the amplitude of MEP. Results The initial RMTs and CMCTs of PD patients were significantly lower than those of the controls, but MEP amplitudes were not significantly different. After rTMS treatment, motor function of PD patients improved, RMTs increased and CMCTs prolonged. Conclusion In PD patients, motor function disorder and increased motor cortical excitability were observed. Low frequency rTMS may inhibit these changes to some extent.

ABSTRACT

Objective : To describe the effect of low-frequency (1 Hz) repetitive transcranial magnetic stimulation (rTMS) on unilateral spatial neglect (USN) in 22 right-handed stroke patients with right hemispheric damage and the corresponding change of regional cerebral blood flow (rCBF) before and after rTMS, speculating on the neural basis for recovery from USN. Methods : The time of onset to the initiation of rTMS was 70 to 220 days (mean : 128.3 days), on chronic stage in all patients. The therapeutic schedule consisted of 7 sessions of 1 Hz-rTMS delivered every other day for two weeks, with 14 sessions in two patients. Each session consisted of one train of 500 pulses applied over P5 (10/20 EEG system) at 90% motor threshold using a figure-eight-shaped coil. Assessment was made using the Japanese edition of the Behavioural Inattention Test (BIT) and a computed visual search task at four times as well as a cold-xenon CT (Xe-CT) at two times. Results : rTMS induced a significant improvement of USN patient performance in cancellation tasks and the BIT figure copy task as well as in visual search reaction time, an improvement which lasted 2 weeks after rTMS. Xe-CT showed a significant increase in rCBF in the right cerebellum after rTMS. Conclusions : It is conceivable that low-frequency rTMS may be useful for improving USN and it is a promising therapeutic tool. Also, the cerebellum, by forming a neural network with the parietal and frontal lobe, may play an important role in patient recovery from spatial neglect.
