ABSTRACT

Objective:To investigate the effect of chimeric flap pedicled with superficial branch of superficial iliac circumflex artery in repair of soft tissue defect of dorsal hand combined with metacarpal bone defect.Methods:From May 2015 to January 2022, 34 patients(28 males and 6 females) of soft tissue defects of dorsal hand with metacarpal bone defects were treated in the Department of Orthopedics of Yibin Third People's Hospital. The age of patients ranged from 22 to 51 years old, with an average age of 37 years old. The areas of soft tissue defects after debridement were 2.5 cm×5.0 cm-4.5 cm×9.0 cm, and the defects were all in dorsal hand and dorsal wrist. The lengths of metacarpal bone defect were 1.8-4.1 cm. All the patients had only single metacarpal bone defect, among which: 14 patients had defects in first metacarpal bone, 7 in second metacarpal bone, 4 in third metacarpal bone, 8 in fourth metacarpal bone and 1 in fifth metacarpal bone. All the patients were repaired by chimeric flap pedicled with superficial branch of superficial iliac circumflex artery. The size of flaps were 3.6 cm×5.4 cm-5.2 cm×9.5 cm. Anticoagulation, thermal preservation and plaster fixation were applied for 4-6 weeks after surgery. Postoperative follow-ups included regularly outpatient clinic visit, telephone or Wechat reviews. Follow-up items covered: the feeling and appearance of flaps in recipient sites, healing of the donor sites and recovery of hand functions.Results:All the 34 chimeric flaps survived. Regular follow-up lasted for 3 to 15(average, 10) months. All incisions in the donor sites of hip healed in stage I. TPD of the flaps was 5.1-7.3(mean, 6.4) mm. Appearance of flaps in the receiving area were satisfactory without swelling. Movement of wrists and metacarpophalangeal joints met the basic requirement of movement. The healing time of metacarpal defect was 2-3 months with an average of 2.8 months. Hand functions were evaluated at excellent in 6 patients and good in 28, according to the Evaluation Standard of Upper Limb Partial Functional of Hand Surgery of Chinese Medical Association.Conclusion:The chimeric flap pedicled with superficial branch of superficial iliac circumflex artery is an ideal flap to repair the soft tissue defect in dorsal hand combined with metacarpal bone defect. It has advantages of less donor site damage, good blood supply of flap, simple surgical procedure, and one-stage repair of a combined soft tissue and metacarpal bone defects.

ABSTRACT

Objective To discuss the morphological features of the fifth metacarpal bone and clinical significance by measuring the three-dimensional (3-D) reconstruction model, and to improve the clinical technique and surgical instruments. Methods A total of 114 3-D reconstruction models based on computed tomography data of fifth metacarpal bone were created. The length, the radius of head, the radius of arc, the 1/ 2 central angle of arc, the distance of head offset, the ratio of head offset, the inner diameter of the isthmus of medullary space were measured based on 3-D models. Results The fifth metacarpal bone length was(51. 55±3. 01) mm. The radius of the fifth metacarpal head was (6. 59± 0. 49)mm. The radius of the fifth metacarpal bone arc was(99. 58±26. 83)mm. The 1/ 2 central angle of fifth metacarpal bone arc was(15. 90±3. 36)°. The distance of the fifth metacarpal head offset was(0. 49±0. 26) mm. The ratio of head offset was(7. 4±3. 9)%. The inner diameter of the isthmus of medullary space was (2. 28±0. 77)mm. Conclusion The length, the radius of head, the distance of head offset, the inner diameter of the isthmus of medullary space are different between male and female, and the other parameters are not different. There is no significant difference between left and right hand.

ABSTRACT

PURPOSE: Pseudoepiphysis originates from the secondary ossification center of the non-ossification end during the normal pediatric growth process. It is not uncommonly found in the course of metacarpal and metatarsal ossification. We investigated the radiologic prevalence and features of pseudoepiphysis in normal Korean children. MATERIALS AND METHODS: Sex and age distribution following radiologic prevalence as well as the features of metacarpal pseudoepiphysis of 2,320 Korean children aged below 15 years of age and younger who underwent hand radiography between January 2009 and February 2016 were analyzed. RESULTS: A total of 304 out of 2,320 patients had pseudoepiphysis on metacarpal bone, which is a prevalence of 13.1%. Male showed higher prevalence (16.6% for male and 10.5% for female). The peak age was 11 years for boys and 5 years for girls. The first metacarpal bone was most prevalent, with 9.6% of the total population, followed by the second metacarpal bone (5.2%) and fifth metacarpal bone (2.5%). The prevalence of single pseudoepiphysis was 9.4%, and that of multiple pseudoepiphysis was 3.7%. The prevalence of incomplete pseudoepiphysis was 8.9% and was higher than complete pseudoepiphysis (5.6%). CONCLUSION: The prevalence of metacarpal pseudoepiphysis in normal Korean children was 13.1%. It is necessary to be aware of the radiologic features and distributions of pseudoepiphysis to avoid misinterpretation as a bone disease or traumatic fracture in pediatric patients.

ABSTRACT

Objective To investigate the clinical efficacy of double locking mini-plates for fixation of the comminuted fracture of the first metacarpal base.Methods Twenty-four patients with the comminuted fracture of the first metacarpal base were treated by double locking mini-plates in our hospital from January 2013 to January 2016.They were 17 males and 7 females,from 27 to 65 years of age (average,33.5 years).By the Green classification,6 cases were type Ⅰ,13 cases type Ⅱ and 5 cases type Ⅲ.All the fractures were closed.The average time from injury to surgery was 2.3 days (from 8 hours to 7 days).After open reduction via the palmar-radial incision,the fracture was fixated with 2 mini-plates,one locking T-plate on the radial side and one straight locking plate on the dorsal side.Fracture healing time,pain and finger function were followed up postoperatively.Visual analogue scale (VAS) was used to evaluate the pain at 12 months and scoring for digital total range of motion to assess the function of the affected finger at the final follow-up.Results The 24 patients obtained follow-up for 8 to 28 months (average,18 months).All the Fractures healed after 9 to 12 weeks (average,10.5 weeks).The VAS scores at 12 months ranged from 0 to 3 (average,1.5).The function of the affected finger at the final follow-up was excellent in 20 cases and good in 4,giving an excellent to good rate of 100%.Two patients complained of pain after frequent motion of the finger.No complications like skin problems,dislocation of the first metacarpal base,implant failure,necrosis or irritation of the soft tissue were observed during follow-up.No angulation or rotational deformity occurred after fracture union.Conclusion Fixation with double locking mini-plates can effectively treat comminuted fractures of the first metacarpal base,because it can provide rigid stabilization which promotes early functional exercise of the finger,and prevents joint stiffness as well.

ABSTRACT

Objective To investigate the epidemiological features and trends of adult fractures at the base of the first metacarpal bone in The Third Mfiliated Hospital to Hebei Medical University from 2003 through 2012.Methods The data of human fractures treated between January 2003 to December 2012 at our hospital were collected through the PACS system and case reports checking system.Adult fractures at the base of the first metacarpal bone were included in the present study and assigned into 2 groups,group A containing the data between 2003 and 2007 and group B the data between 2008 and 2012.Comparison and analysis was done with analytic items of gender,age and fracture type.Results A total of 378 adult fractures at the base of the first metacarpal bone were included,accounting for 1.92% of hand fractures and 0.35% of the total adult fractures at the same period.The fractures involved 311 males (82.28%) and 67 females (17.72%).The youth group had 254 fractures with the highest constituent ratio (67.20%).The right side was involved in 286 cases (75.66%) and the left side in 92 (24.34%).There were 198 cases of extra-articular fracture with the highest constituent ratio (52.38%).Group A had 200 fractures,a male to female ratio of 6.14∶ 1,a median age of 34 years,the highest constituent ratio in youth (73.50%) and in type A1 fractures (53.50%);group B had 178 fractures,a male to female ratio of 3.56∶ 1,a median age of 40 years,the highest constituent ratio in youth (60.11%) and in type A1 fractures (51.12%).Compared with group A,group B had a higher constituent ratio of females,older ages,a higher constituent ratio of elderly patients,a lower constituent ratio of youth,and a lower constituent ratio of type C1 fractures.All these differences between the 2 groups were statistically significant (P < 0.05).Conclusions During the 10 year,the adult fractures at the base of the first metacarpal bone accounted for 1.92% of hand fractures and 0.35% of the total adult fractures at the same period.The fractures occurred mostly in men and in the age range of 16 to 44 years.About 3/4 of them occurred on the right side.Compared with the first 5 years,the latter 5 years witnessed increasing trends of female and elderly patients and decreasing trends of young patients and type C1 fractures.

ABSTRACT

Objective To compare and analyze the epidemiological features of fractures at the base of the first metacarpal bone between East and West China.Methods We retrospectively analyzed the clinical data of the patients with fracture at the base of the first metacarpal bone who had been treated from January 2010 to December 2011 in 63 hospitals in the east and west areas of China.The data from 35 hospitals in East China were classified as group A while the data from 28 hospitals in West China as group B.The analytic items included gender,age,age distribution and fracture classification.Results A total of 890 cases were collected.The total male/female ratio was 4.56∶ 1.The youth accounted for the highest proportion and the high-risk type of fracture was type Ⅲ (44.49%,396/890).The male/female ratio was 4.59∶1 in group A of 621 patients and 4.49∶1 in group B of 269 patients,showing no statistic between-group difference (P > 0.05).The median age in group A was 39 years,significantly older than that in group B (35 years) (Z =-3.687,P < 0.001).In both groups,the youth accounted for the highest proportion and there were more right-side fractures than left-side ones.Group A had a significantly lower proportion of the youth and a significantly higher proportion of the middle-aged than group B (P < 0.05),but there was no statistic difference in proportions of the children and the aged (P > 0.05).Type Ⅲ was the high-risk type in both groups;group A had significantly more fractures of type Ⅰ and significantly fewer fractures of type Ⅱ than group B (P < 0.05),but there was no statistic difference in proportions of type Ⅲ and type Ⅳ between the 2 groups (P > 0.05).Conclusions Fractures at the base of the first metacarpal bone mostly occurred in the young males.There were more fractures at the right side than at the left side.The most frequent type was type Ⅲ.The proportion of type Ⅰ fractures in East China was higher than in West China while the proportion of type Ⅱ fractures in East China lower than in West China.

ABSTRACT

Objective To compare the clinical effects of mini phalangeal external fixation and mini plate in the treatment of intraarticular fractures of the metacarpal bone head,and to provide a theoretical basis for clinical selection of surgical plan. Methods Seventy?four cases of metacarpal bone head intraarticular fractures treated in Huizhou People′s Central Hospital from March 2014 to March 2016 were selected as the research object. All cases were closed fractures and were randomly divided into the external fixation group and the mini plate group,37 cases in each group and treated respectively with the mini phalangeal external fixator and mini AO plate. The patients were followed up for 6 months to 1 year,the function of the metacarpophalangeal joint,the union of the fracture and the incidence of side effects were compared between the two groups. Results The excellent rates of the external fixation group at 6 months and I year after operation were 91. 9% ( 34/37) and 97. 3%( 36/37) respectively,which were significantly higher than those of the mini plate group ( 73. 0%( 27/37) ,81. 2%( 30/37 ) ) , the difference was statistically significant (χ2 = 7. 120, 7. 062, P<0. 05 ) . The complication rate of the external fixation group was 2. 7%( 1/37) ,which was significantly lower than that of the mini plate group ( 24. 3% ( 9/37 ) ) , the difference was statistically significant ( P=0. 014 ) . The average fracture healing time in the external fixator group was (2. 6±0. 4) months,which was significantly lower than that in the mini plate group (3. 1±0. 7) months,the difference was statistically significant (t=2. 40,P<0. 001) . Conclusion In the treatment of metacarpal bone head fracture,the external fixation has small trauma,can do cross joint fixation,and maintain a good fixation strength,and promote the healing of fracture and the recovery of metacarpophalangeal joint function. It is worthy of clinical application.

ABSTRACT

Ewing sarcoma is a primary bone malignancy that rarely occurs in the hand. Resection and reconstruction will usually result in reduced or loss of thumb function. We describe a case of successful transplantation of nonvascularized osteoarticular second metatarsal autograft following wide resection of Ewing sarcoma of first metacarpal in an 11-year-old girl. The capsule of the graft’s metatarso-phalangeal joint was repaired to achieve mobility of the joint. This enabled preservation of thumb function by a relatively simple surgical technique. Detailed surgical procedures and excellence function after 2 years following surgery are described.

Solitary plasmacytoma of the bone (SPB) is a plasma cell neoplasm that usually presents as a lytic lesion mainly localized within the axial skeleton. The occurrence of SPB in young individuals is exceedingly rare, but has been sporadically reported before. We report a case of SPB involving metacarpal bone in a 16-year-old male with a prior history of trauma at the same site.

ABSTRACT

Objective To compare the therapeutic effects of closed reduction and transverse fixation with Kirschner wires and open reduction and fixation with mini-plates or Kirschner wires for the fifth metacarpal base fracture.Methods Data of 50 patients,who had been treated in our hospital for the fifth metacarpal base fracture from August 2005 to May 2012,were enrolled in this study.Twenty-six patients underwent closed closed reduction and transverse fixation with Kirschner wires,including 22 males and 4 females,aged from 17 to 41 years; according to the OTA classification,there were 8 of type A1,11 of type B1,and 7 of type C1.Twenty-four patients underwent open reduction and fixation with mini-plates or Kirschner wires,including 21 males and 3 females,aged from 19 to 46 years (average,26 years); according to the OTA classification,there were 11 of type A1,8 of type B1 and 5 of type C1.The total active motion (TAM) score of Chinese Medical Associaition was used to evaluate the range of motion of the joint.Results In closed reduction group,the operation time ranged from 14 to 33 min; the treatment costs ranged from 2018 to 2995 yuan; 23 patients were followed up for 6 to 26 months; X-rays showed the anatomic reduction was achieved in all patients,and the fracture healing time ranged from 4 to 7 weeks; according to the TAM score,the result was excellent in 15,good in 6 and fair in 2,and the excellent and good rate was 91.3%.In open reduction group,the operation time ranged from 45 to 105 min; the treatment costs ranged from 3874 to 4793 yuan; 22 patients were followed up for 6 to 21 months; X-rays showed the anatomic reduction was achieved in all patients,and the fracture healing time ranged from 4 to 8 weeks; according to the TAM score,the result was excellent in 13,good in 7 and fair in 2,and the excellent and good rate was 90.9%.Conclusion Closed reduction and transverse fixation with Kirschner wires is an effective method for the fifth metacarpal base fracture,which has several advantages,such as simple operation,stable fixation,satisfactory results,short operation time and low treatment costs.

ABSTRACT

PURPOSE: To evaluate the radiologic and clinical results of percutaneous transverse fixation with K-wires for 5th metacarpal neck fracture. MATERIALS AND METHODS: Between January 2007 and September 2010, 18 patients with a 5th metacarpal neck fracture, who underwent operative treatment, were included in this study. The surgical method was percutaneous transverse fixation using K-wires. We evaluated fracture angulation in oblique radiographs preoperatively, postoperatively, and at final follow-up, and used SPSS to perform statistical analysis. We also performed clinical evaluation using the Disabilities of the Arm, Shoulder and Hand (DASH) score. RESULTS: All of the 18 cases were completely united, and in the oblique radiographs, the angulation was corrected from 50.69degrees to 11.68degrees. The average difference between postoperative and final follow-up angulations was 0.14degrees, which was statistically insignificant. Clinically, the DASH score was 1.030 and no complications were observed. CONCLUSION: Percutaneous transverse fixation using K-wires could be one of the best ways to treat a 5th metacarpal neck fracture because of its simple method and low rate of complications.

Objective To study the sex difference and discriminating formulas by measuring both metacarpal bones on X-ray and body height of Kazakh nationality adults,providing anatomical data for physical anthropology and forensic medicine. Methods The metacarpal bones length on X-ray films and body height from 200 adults of Kazakh nationality,including 100 male and 100 females were measured. The obtained date were statistically dealt by SPSS15.0 software. Results The length of the 4th metacarpal bone in male and the 5th in female were statistical significance beside, 4 discriminating formulas were obtained as follows: Y=2.824r_1+2.563r_4-0.654r_5+0.614l7-0.039l_9+2.452l_(10)-212.186,Y=2.350r_1+2.377r_4-0.995r_5+0.445l_7+0.046l_9+2.966l_(10)-191.622,Y=0.393h_1+3.152r_1+0.435r_5+1.250r_(10)-463.734,Y=0.362h_1+2.785r_1+0.028r_5+1.834 r_(10)-404.748, the accuracy being 89.67%、86.55%、90.00% and 87.50%. Conclusion The data presented in this paper indicates that the body height of male and female kazakh minority is closely related with the enlargement of their metacarpal bones.

ABSTRACT

Objective To compare the clinical outcome of external fixation device with Kirschner wire or fragment fixation pin with mini plate and screw fixation in treatment of metacarpal and phalangeal articular fracture dislocation. Methods From October 2002 to March 2008, 106 patients with metacarpal and phalangeal articular fracture dislocation were randomly divided into A and B group. The 53 patients in A group were treated with external fixation device with Kirschner wire or fragment fixation pin. There were thumb injury in 24 cases, fracture-dislocation of proximal interphalangeal joint(PIP) in 36 cases, fracture-dislocation of metacarpophalangeal joint (MP) in 17 cases. The 53 patients in B group were treated with mini plate and screw fixation. There were thumb injury in 22 cases, fracture-dislocation of PIP in 30 cases, fracture-dislocation of MP in 23 cases. Duncan rating criteria were used to compare finger range of motion. Results The mean follow-up of 16.8 and 17.5 months in A group and B group. According to Duncan rating criteria, there were excellent in 33 cases, good in 16, fair in 3, and poor in 1 case. The excellent and good rate was 92.5% in A group. There were excellent in 30 cases, good in 17, fair in 5, and poor in 1 case. The excellent and good rate was 88.7% in B group. In A group, 1 case of wound infection was found. The average arc of motion of thumb joint was 134°±21° while the average arc of motion of other fingers was 248°±19°. In B group, no wound infection occurred. The average arc of motion of thumb joint was 122°±18° while the average arc of motion of other fingers was 225°±17°. Conclusion External fixation device with Kirschner wire or fragment fixation pin was better than the application of mini plate and screw fixation in treatment of metacarpal and phalangeal articular fracture-dislocation.

ABSTRACT

A avaliação clínica e radiológica do terceiro metacarpeano (McIII) de 42 potros Puro Sangue de Corrida de dois anos de idade, em treinamento, realizou-se a cada 15 dias. Vinte e cinco potros foram acompanhados durante dois meses e 17 durante os quatro meses anteriores a sua participação na primeira corrida. Nas radiografias (projeção lateromedial), foram avaliadas o córtex dorsal (CD) do McIII e a espessura do CD, do córtex palmar e da zona medular para determinação do índice radiológico (IR). Não foram observadas alterações radiológicas no CD do McIII nos potros que manifestaram periostite metacarpeana dorsal (PM) aguda. O aumento tanto do CD, como do IR, entre as avaliações foi significativo (P<0,05). Porém, apenas a diferença de espessura do CD entre os McIII sem alterações e os que manifestaram PM foi significativa (P<0,05). O aumento mais acentuado do CD foi observado sempre na avaliação anterior àquela em que foram detectados os sinais clínicos de PM. Portanto, especialmente a mensuração do CD pode ser utilizada como um método diagnóstico precoce de PM, permitindo a sua prevenção.


The third metacarpal bones (McIII) of 42 2-year-old thoroughbreds in training were evaluated clinically and radiologically every 15 days. The evaluation was performed during 2 and 4 month, prior to the first race of 25 and17 horses, respectively. X-rays were taken using latero-medial projection. Dorsal cortex (DC) was evaluated on the X-rays and DC, palmar cortex and medular zone were measured in order to determine the radiologic index (RI). No radiographic changes were observed on the DC of the McIII of horses clinically affected by dorsal metacarpal disease (DMD). There was a significant increase (P<0.05) in DC thickness and RI between evaluations of both, healthy and DMD affected McIII. DC thickness also showed a significant increase between affected and not affected McIII. A larger increase in DC thickness was observed at evaluation before the one in which DMD was diagnosed. In conclusion, the increase in DC thickness could be used as a diagnostic tool for early recognition of DMD allowing adoption of preventive measures.

ABSTRACT

PURPOSE: To evaluate radiologic and clinical results of bouquet pin intramedullary nail technique for the 5th metacarpal neck fracture. MATERIALS AND METHODS: Between April, 2005 and February, 2006, 17 patients treated by bouquet pin intramedullary nail technique for the 5th metacarpal neck fracture were evaluated. All patients were reviewed clinically and radiologically after operation. RESULTS: All of 17 cases of fractures were completely united. In the anteroposterior radiographs, the average of preoperative angulation was corrected from 34.4° to 5.2°. Also, in the oblique radiographs, radiographic results of angulation correction were satisfactory which was corrected from 44.2° to 11.7°. Although, the averages of difference between postoperative and final follow-up angulations were 1.5° in the anteroposterior radiographs and 0.9° in the oblique radiographs, they were not statistically different. All patients were excellent clinically except 1 patient who has moderate joint stiffness after operation. CONCLUSION: Selecting of appropriate patients who is indicated, bouquet pin intramedullary nail technique for the 5th metacarpal neck fracture could be a good treatment method without complications.

ABSTRACT

PURPOSE: The purpose of this study was to describe the patterns and results of treatment of fractures of the hand and wrist after punching injury. MATERIALS AND METHODS: The authors retrospectively reviewed 91cases of fractures of the hand and wrist after punching injury in 67 patients from January 1998 to September 2002. The mean follow-up was 18 months. The mean age was 25.7 years old, ranging from 11 years old to 41 years old. The results were evaluated by modified criteria of Bruce and Maudsley. RESULTS: The most common fractures were metacarpal base fractures (42.9%), and the second most common fractures were distal metacarpal fractures (19.8%). Metacarpal base fractures were usually combined with dislocation of carpometacarpal joint, especially in the fourth and fifth metacarpal bone and single fifth metacarpal bone (29.7%). There were a few complications of limitation of motion (4 cases), pain (1 case), angular deformity of distal metacarpal bone (1 case), malunion of metacarpal shaft (2 cases) and nonunion of metacarpal shaft (1 case). There were excellent results in 84 cases, good in 4 cases and fair in 3 cases. CONCLUSION: The most common fractures after punching injury were metacarpal base fractures, especially the forth and fifth metacarpal bone. Distal metacarpal and metacarpal shaft fractures were mostly single fractures. There were diverse patterns of fracture-dislocation of the hand and wrist after punching. We must be careful to evaluate combined injury when a patient visits after punching injury.

ABSTRACT

Objective To analyze the curative effects of different managements of different types of fracture of the first metacarpal basal body. Methods From October 1984 to October 2003, 142 patients with fracture of the first metacarpal basal body were treated with 5 different methods: manipulative reduction and fixation with abduction tooth arch, manipulative reduction and suspension traction, manipulative reduction and fixation with abduction frame, manipulative reduction and percutaneous internal fixation with Kirschner wire, as well as open reduction and internal fixation with Kirschner wire or screw. Results 80 patients were followed up. The therapeutic efficacy was excellent in 65 cases , good in 13 cases, poor in 2 cases. Conclusion Different types of fracture of the first metacarpal basal body can be treated satisfactorily if a suitable management is applied accordingly.

While the findings in the right hand (first metacarpal bone) and first phalanx of the index finger are characteristic, there are changes characterized by massive periostitis of the right humerus and left ulna which as far as we know have not yet bee described in the literature. ABSTRACT

86 cases of defect of thumb with defect of metacarpal bone were operated by the second toe transfer to reconstruction of the thumb, of which 82 cases were successful occuping 95. 6% of total cases. The metacarpal-phalangal arthral reconstruction of the thumb was completed by the hemi-arthral and full-arthral transplantation of metatarsal-phalangee joint of second toe. The results of follow-ups after operation revealed that the degree of movement of the joint was 26 in the former, 36% in the latter. The reconstruction of the first web by means of the local flap of the dasalis manual; dasalis pealical flap and groin flap. Fair results were obtainded with 63.2 %; 87. 1% and 100%. respectively. The disadvantages of groinflap method are timeconsuming, and incomfortable in position. The operative methods vary with the degree of the defect of the matacarpal bone and the condition of the web space
