ABSTRACT

Objective To evaluate the association between bradycardia and neurological sequel in patients with restoration of spontaneous circulation (ROSC) after cardiopulmonary resuscitation (CPR) during targeted body temperature management (TTM).Methods Forty-three unconscious patients with ROSC after CPR were treated with TTP.The patients were cooled with therapeutic hypothermia to body temperature target range (32-34°C) after cardiac arrest and divided into bradycardia and control groups depending on the lowest heart rate less than 50 beats/min and more than or equal to 50 beats/min respcetively at that time.The bispectral index (BIS) and the neuron-specific enolase (NSE) values were respectively recorded at the following intervals,0h (h0)、24h (h24)、48h (h48)、72h (h72) after ICU admission.Neurological outcome was defined according to the Pittsburgh cerebral performance category (CPC) at 3 months after ICU discharge.Results Compared with the control group,during TTM the bispectral index levels were significantly higher in the bradycardia group at h0,h48,h72 after admission,(h0 bradycardia group 73.0 ± 12.3;control group 58.0 ± 18.6,P <0.01)、(h48 bradycardia group 71.4 ± 21.2;control group 46.3 ± 18.9,P < 0.01)、(h72 bradycardia group 78.6 ± 24.6;control group 51.8 ± 24.1,P =0.01).The neuron-specific enolase level in bradycardia group was significantly lower than that in control group on day3 (118.8 ± 118.8 ng/mL vs.248.3 ± 191.9 ng/mL,P =0.02).The level of CPC in the bradycardia group was significantly higher than that in the control group (P =0.046).Conclusions Patients with bradycardia during TTM had favorable neurological outcome,which could provide evidence for clinical treatment and prognostic evaluation of the patients.

ABSTRACT

Objective To evaluate the clinical value of serum carcinoembryonic antigen(CEA),neuron specific enolization enzyme(NSE),21-1 non-small cell lung cancer associated antigen(CYFRA21-1),carbohydrate antigen 125(CA125) and ferritin(Fer) in the diagnosis of lung cancer.Methods The levels of CEA,NSE,CYFRA21-1,CA125 and Fer were measured by electrochemlium-inescence in 103 patients with lung cancer,32 patients with benign lung diseases and 40 healthy people.Results The serum levels of CEA,NSE,CYFRA21-1,CA125 and Fer in patients with lung cancer[(110.2±95.5)ng/mL,(50.6±43.4)ng/mL,(32.8±29.5)ng/L,(122.7±110.4)U/L,(854.6±497.2)ng/mL] were significantly higher than those in patients with benign lung diseases and those in healthy people(t=6.21,5.71,6.75,6.62,7.74,P<0.05;t=5.26,4.86,5.81,5.20,6.26,P<0.05).The sensitivity values of CEA,NSE,CYFRA21-1,CA125,Fer and the combined determination containing five tumor markers were 39.81%,24.27%,71.84%,68.93%,77.66% respectively.The sensitivity and specificity of the combined determination containing five tumormarkers were 96.12%,95.00%.Conclusion The joint detection in the diagnosis of lung cancer could improve the sensitivity significantly,to help for early diagnosis of lung cancer,which is value to widely applied in clinic.

ABSTRACT

Objective To analyze the value of carbohydrate antigen(CA)153,CA125,neuron specific enolization enzyme(NSE) and carcinoembryonic antigen(CEA),cytokeratin(CYFRA)21-1 in the diagnosis of lung cancer.Methods From October 2014 to April 2016,60 patients were selected into lung cancer group,60 patients were selected into lung cancer group,60 healthy persons recruited into control group,blood specimens from all subjects received chemiluminescence immunoassay detection and analyzed.Results Five tumor markers for lung cancer detection had certain difference in sensitivity and specificity,the specific of NSE and CA153,the sensitivity of CEA and CYFRA21-1 in 5 tumor markers were the highest.The combination of 5 tumor markers could make the diagnosis of lung cancer more accurate,the differences of the specific and sensitivity between the combined detection and single detection were significant (P<0.05).Conclusion The combination detection of these five tumor markers could improve the diagnosis ability in patients with lung cancer.

ABSTRACT

Objective To investigate the diagnostic value of combined detection of serum gastrin-releasing peptide precursor (ProGRP),neuron specific enolization enzyme(NSE)and carcinoembryonicantigen(CEA)in small cell lung cancer(SCLC). Methods 471 patients with lung tumor from department of respiratory medicine and thoracic surgery and 162 healthy people from medical examination center were studied.Serum levels of ProGRP,NSE and CEA were detected by using electrochemi-cal luminescence method.ROC curves were drawn and the area under the curve (AUC)was calculated.Results The levels of ProGRP and NSE were significantly higher in patients with SCLC than those in NSCLC,lung benign disease group and normal control group (P <0.01).The levels of CEA were significantly higher in SCLC than those in patients with lung be-nign disease group and normal control group (P <0.05).The AUC of ProGRP,NSE and CEA in the diagnosis of SCLC were 0.933,0.777 and 0.554,respectively.The sensitivity of ProGRP,NSE and CEA in the diagnosis of SCLC were 82.6%,60.4%,41.6% and the specificity were 95.2%,83.3% and 71.7% respectively.The sensitivity of combined detec-tion of ProGRP,NSE and CEA was 91.3% and the specificity was 65.3%.Conclusion The serum ProGRP detection has a higher diagnostic value for SCLC.The combined detection of ProGRP,NSE and CEA is useful in the early diagnosis of SCLC.

ABSTRACT

Objective To understand the clinical significance of detection of tumor necrosis factor alpha (TNF -α),neuron specific enolization enzyme (NSE)and S100B levels in children with hand -foot -mouth disease (HFMD)complicated with encephalitis,and to provide research data for children with HFMD complicated with encephalitis.Methods 157 children with HFMD were divided into 79 cases of HFMD complicated encephalitis group and 78 cases of HFMD group according to whether there was encephalitis complications.Another 80 healthy children were chosen as control group.TNF -α,NSE and S100B protein in blood and cerebrospinal fluid of the three groups were detected by enzyme -linked immunosorbent assay.Results TNF -αlevels in blood and cerebrospinal fluid of HFMD complicated encephalitis group,HFMD group were significantly higher than those of the control group (t =8.315,6.443,4.132,6.443,all P <0.05);TNF -αlevels in blood and cerebrospinal fluid of HFMD complicated encephalitis group were significantly higher than those of HFMD group (t =4.378,3.220,all P <0.05).NSE in blood and cerebrospinal fluid of HFMD complicated encephalitis group were significantly higher than those of the control group (t =8.706,7.840,all P <0.05);NSE in blood and cerebrospinal fluid of HFMD complicated encepha-litis group were significantly higher than those of HFMD group (t =6.945,7.134,all P <0.05).S100B in blood and cerebrospinal fluid of HFMD complicated encephalitis group were significantly higher than those of the control group (t =9.443,10.031,all P <0.05);S100B in blood and cerebrospinal fluid of HFMD complicated encephalitis group were significantly higher than those of HFMD group (t =8.552,9.206,all P <0.05).TNF -α,NSE and S100B levels in blood and cerebrospinal fluid of HFMD complicated encephalitis group were positively correlated (r =0.803, 0.794,0.856,all P <0.05);TNF -αlevels in blood and cerebrospinal fluid of HFMD group was positively correla-ted (r =0.743,P <0.05).Conclusion Detection of TNF -α,NSE and S100B levels in blood and cerebrospinal fluid can assist to diagnose children with HFMD,and NSE and S100B can be used as specific indicators of HFMD complicated encephalitis.

ABSTRACT

OBJECTIVE:To observe therapeutic efficacy and safety of Yinxing damo injection for neural function recovery af-ter hypertensive intracerebral hemorrhage (HICH) minimally invasive surgery. METHODS:84 HICH patients were randomly di-vided into control group and observation group with 42 patients in each group. Both groups received CT guiding minimally inva-sive aspiration. Control group was given western medicine baseline therapy,such as dehydration and intracranial decompression, controlling blood pressure,preventing infection,alimenting never,symptomatic treatment. Observation group was additionally giv-en Yinxing damo injection 20 ml,ivgtt,bid. Treatment course lasted for 14 d. NIHSS score,GCS score and Fugl-Meyer motor function assessment scale score were comducted before and after treatment. The serum levels of NSE,serum C3,C4 and hs-CRP were determined in 2 groups before and after treatment. RESULTS:After treatment,the effective rate of observation group (85.71%)was better than that of control group(66.67%),with statistical significance(P<0.05);after treatment,NIHSS score, Fugl-Meyer score and GCS score of 2 groups were all better than before,the observation group was better than the control group, the levels of C3 and C4 in observation group were lower than in control group,with statistical significance(P<0.01). There was one case of allergic reaction that the patient can tolerate. Magnesium sulfate for external use was given,which did not affect the treatment. CONCLUSIONS:Yinxing damo injection could improve neurologic impairment,promote the recovery of patients and have good safety.
