Introducción: La vigilancia de la morbilidad materna extrema es complemento para la evaluación y mejoramiento de la calidad de los servicios de salud. Objetivo: Evaluar la adherencia a las buenas prácticas clínicas en el manejo de la morbilidad materna extremadamente grave. Métodos: Se realizó un estudio descriptivo transversal en el Hospital Universitario Ginecobstétrico Mariana Grajales, Villa Clara, Cuba, desde el 2012 al 2015 en mujeres con morbilidad materna extremadamente grave. De las 577 pacientes atendidas se escogió una muestra intencional de 93. Además, fueron revisados los documentos de archivo del hospital y las historias clínicas individuales y hospitalarias. Resultados: En la consulta de atención primaria fueron registrados 80 casos como riesgo obstétrico, en consultas de re-evaluación realizadas a las 14,1 semanas como promedio, solo 88,2 por ciento habían realizado los exámenes complementarios iniciales y solo 94,6 por ciento, tenían reflejado en su carné obstétrico el control de las curvas de tensión y peso. La altura uterina solo apareció reflejada en 96,8 por ciento de los casos. En la atención secundaria se aplicó el código de colores a 92 pacientes y se identificó el riesgo obstétrico en 91. Fueron diagnosticadas al ingreso 85 mujeres. Se aplicaron los protocolos de atención al puerperio y de seguimiento por la comisión de la institución en todos los casos. Conclusiones: Se detectan deficiencias en el proceso de atención a la morbilidad materna extremadamente grave. En este período fueron evaluados como aceptables la captación precoz, la atención al puerperio (inmediato y mediato) y el seguimiento por la comisión de morbilidad materna extremadamente grave de la institución(AU)


Introduction: Surveillance of extreme maternal morbidity is a complement to the assessment and improvement of the quality of health services. Objective: To evaluate adherence to good clinical practices in Managing Extremely Severe Maternal Morbidity. Methods: A descriptive cross-sectional study was carried out in women with extremely severe maternal morbidity at Mariana Grajales Gyneco-Obstetric University Hospital, Villa Clara, Cuba, from 2012 to 2015. An intentional sample of 93 was chosen from 577 patients treated. In addition, the hospital records and individual and hospital medical records were reviewed. Results: In the primary care clinic 80 cases were registered as obstetric risk. Only 88.2 percent had performed the initial complementary exams in re-evaluation consultations conducted at 14.1 weeks on average. Moreover, 94.6 percent had registered tension and weight curves in their obstetric control card. Uterine height only appeared registered in 96.8 percent of cases. In secondary care, color code was applied in 92 patients and obstetric risk was identified in 91. Upon admission, 85 women were diagnosed. Puerperium care and follow-up protocols were applied by the institution's commission in all cases. Conclusions: Deficiencies are detected in the process of providing care to extremely severe maternal morbidity. During this period, early pregnancy uptake, care for the immediate and mediate puerperium and follow-up by the institution's extremely serious maternal morbidity commission were assessed as acceptable(AU)

Resumen OBJETIVO: Estimar la morbilidad obstétrica grave y mortalidad materna en el Hospital Juárez de México. MATERIALES Y MÉTODOS: Estudio retrospectivo y transversal efectuado para evaluar los registros de pacientes con morbilidad obstétrica grave y la mortalidad materna, de acuerdo con los criterios de la OMS, atendidas en el Hospital Juárez de México de 2010-2016. Se analizaron las variables relacionadas con el diagnóstico, insuficiencia o disfunción orgánica y las intervenciones asociadas con el tratamiento. Se calculó la tasa de morbilidad materna extrema y obstétrica grave (Near Miss/total de embarazadas). Se utilizaron medidas de tendencia central y de dispersión. RESULTADOS: Se registraron 137 casos de morbilidad obstétrica grave y 26 de muerte materna. La mortalidad promedio fue de 5.78% y el índice de mortalidad de 0.415. Las principales causas de morbilidad materna grave y mortalidad fueron: hemorragia obstétrica, hipertensión asociada con el embarazo y sepsis. Se observó un incremento en los casos de morbilidad materna grave y disminución de la mortalidad materna en el periodo estudiado. CONCLUSIÓN: Se sugiere establecer un comité de evaluación y análisis de pacientes con morbilidad obstétrica grave para mejorar la calidad de atención y tratamiento, y disminuir la tasa de mortalidad materna.


Abstract OBJECTIVE: To determine how many cases of severe obstetric morbidity and maternal mortality were presented in 2010-2016, at the Hospital Juárez de México. MATERIALS AND METHODS: A retrospective, cross-sectional, observational study was carried out in cases of severe obstetric morbidity and maternal death were included according, to WHO criteria, attended in the Hospital Juárez de México from 2010-2016,. We included variables related to the diagnosis, with the organic failure or dysfunction and the interventions related to the management. The extreme maternal morbidity rate was calculated; obstetric (Near miss / Total pregnant). Measures of central tendency and dispersion were used. RESULTS: A total of 137 cases of severe obstetric morbidity and 26 of maternal deaths were registered. The mortality rate on average was 5.78% and death rate .415. The main causes of severe maternal morbidity and mortality were obstetric haemorrhage, hypertension associated with pregnancy and sepsis. There was an increase in cases of severe maternal morbidity and decrease in maternal death in the period studied. CONCLUSION: We suggest the establishment of a committee to evaluate and analyze cases of severe obstetric morbidity to improve the quality of care and treatment for this group of pregnant women, to reduce maternal death.

En la atención secundaria se aplicó el código de colores a 92 pacientes, se identificó el riesgo obstétrico en 91 y fueron diagnosticadas al ingreso 85 mujeres. Se aplicaron los protocolos de atención al puerperio y al seguimiento por la comisión de la institución en todos los casos. Conclusiones: se detectan deficiencias en el proceso de atención a la morbilidad materna extremadamente grave. Introduction: Surveillance of extreme maternal morbidity supplements the evaluation and improvement of the quality of health services. Objective: To evaluate the good clinical practices as a fundamental element in the management of the extremely severe maternal morbidity. Methods: A descriptive cross-sectional study conducted in women with extremely severe maternal morbidity in Mariana Grajales university gynecological and obstetric hospital in Villa Clara province from 2012 to 2015. A final sample of 93 women was chosen from 577 patients by using a non-probabilistic method. Filed documents and individual and hospital clinical records were all checked. Results: Eighty patients were classified as obstetric risk in the primary health care level; re-assessment appointments with the physician occurred at 14.1 weeks as average; the initial supplementary tests were performed in 82 women, the control of blood pressure and weight, and of the uterus height occurred in 88 and 90 women, respectively. At the secondary care, the color code was applied to 92 patients; the obstetric risk was detected in 91 whereas 85 women were diagnosed on admission to the hospital. The puerperium care and the follow-up protocols were used by the institutional commission in all cases. Conclusions: Deficiencies were detected in the process of care to extremely severe maternal morbidity. In this period, early detection, care to puerperium (immediate and mediate) and follow up by the institutional extremely severe maternal morbidity commission(AU)

ABSTRACT

Obstetric near-miss (ONM) describes a situation of lethal complication during pregnancy, labor or puerperium in which the woman survives either because of medical care or just by chance. In a cross-sectional observational study, fi ve factor scoring system was used to identify the near-miss cases from all the cases of severe obstetric morbidity. Assessment of the causes of maternal mortality and near-miss obstetric cases was done. The ONM rate in this study was 4.18/1000 live births. Totally 54 maternal deaths occurred during this period, resulting in a ratio of 202 maternal deaths per 100,000 live births. Hemorrhage, hypertension and sepsis were major causes of near-miss maternal morbidity and mortality, respectively in descending order.

Background & objectives: The ambiguity and variability in existing literature on the magnitude of socio-economic inequality in self-reported morbidities makes it difficult to set priorities in health policy. This study examined three critical research questions: first, how far self-reporting affects measuring socio-economic inequalities in case of obstetric morbidities. Second, does using simple bivariate variations mislead in estimating socio-economic differentials in prevalence of obstetric morbidities? Finally, whether use of sophisticated regression based decomposition results can overcome such problems. Methods: The data from National Family Health Survey (NFHS-3; 2005-06) were used, and analyzed by statistical tools such as bivariate estimates and regression based decomposition analysis. Results: Bivariate results revealed that self-reported obstetric morbidity data were misleading in measurement of socio-economic differentials, as these failed to show existing socio-economic variations in obstetric morbidities by socio-economic standing of women. However, decomposition analysis showed that the prevalence of obstetric complications was greater among socioeconomically disadvantaged groups. Interpretation & conclusions: Based on our findings on measurement of socio-economic inequality in self-reported obstetric morbidity, we conclude that the use of regression based inequality decomposition estimates not only overcomes the problems of measuring socio-economic inequality based on self-reported morbidities, but also increases the validity of such measures.
