ABSTRACT

Objective To detect the sealing ability of iRoot as root end retrograde filling material in vitro experiment,and to compare it with mineral trioxide aggregate(MTA) in order to provide reference basis for clinical use of iRoot.Methods The dental crown was removed in 67 single root canal anterior teeth.The routine root canal preparation and filling were performed.The root end was resected by 3 mm for conducting root end retrograde preparation.Sixty-three teeth in the samples were randomly divided into 4 experimental groups.Then the different materials were used for root end filling:iRoot BP (group A,n=17),iRoot BP plus (group B,n=17),MTA (group C,n =17),and group D(iRoot BP + gutta percha,n =12),the residual 4 samples served as the control group.The apical microleakage of 10 teeth in each experimental group was detected by the fluid dynamics.The microleakage situation at the time points of 1,7,14,28 d was observed.In addition,2 teeth selected from each group were performed the filling material ultramicrostructure observation by SEM.Each 5 teeth were remained in the group A,B and C and performed the Micro CT scanning.The porosity rate of filling material was detected.Results No obvious edge gap was found between the filling materials and dentin surface in the group A,B and D by SEM,but obvious gap could be seen in various samples of group C.Micro CT scanning found that the porosity rates in the group A,B and C were 0.020 ±0.023,0.045 ±0.035 and 0.031 ± 0.011 respectively,but showing no statistical difference(P>0.05).The fluid dynamic experiment showed that the microleakage detection value in each group had no statistical difference(P>0.05).Conclusion The sealing ability of iRoot and MTA used in root end retrograde sealing has no obvious difference,But iRoot operability is superior to MTA.

ABSTRACT

ABSTRACT Periapical granulomas are chronic inflammatory processes appearing as a lesion around the apex of the tooth, which increase in size due to a proliferation of granulomatous tissue formed by lymphocytes, plasmocytes, some polymorphonuclears, macrophages, eosinophils, multinucleated giant cells, fibroblasts, and neoformation of capillaries. Etiologically, they occur due to deep caries affecting the pulp, infected root canals or endodontic failures. The objective of the present article is to report the finding of a periapical granuloma in a previously treated tooth, by means of excisional biopsy, curettage, and apical surgery with retrograde filling using mineral trioxide aggregate (MTA) as filling material on tooth # 11 in a 65-year old male patient, with subsequent histopathological study of the lesion. The clinical and radiographic follow-up showed the effectiveness of MTA as a retrograde filling material and apicoectomy as surgical procedure for the treatment of periapical granuloma.


RESUMEN. Los granulomas periapicales son procesos inflamatorios crónicos que se presentan como una lesión rodeando el ápice del diente, la cual aumenta de tamaño debido a una proliferación de tejido granulomatoso conformado por linfocitos, plasmocitos, algunos polimorfonucleares, macrófagos, eosinófilos, células gigantes multinucleadas, fibroblastos y capilares de neoformación. Etiológicamente, su aparición obedece a caries profundas que afectan la pulpa, conductos radiculares infectados o fracasos endodónticos. El objetivo del presente artículo es informar el hallazgo de un granuloma periapical en un diente previamente tratado. Se realizó biopsia excisional, curetaje y cirugía apical con obturación retrógrada usando compuesto de minerales trióxido (MTA) como material de obturación a nivel del órgano dentario 11 en un paciente masculino de 65 años de edad, con posterior estudio histopatológico de la lesión. El seguimiento clínico y radiográfico demostró la efectividad del uso de MTA como material de obturación retrógrada y la apicectomía como procedimiento quirúrgico para el tratamiento del granuloma periapical.

ABSTRACT

Objetivo: evaluar la capacidad de BiodentineTM para resistir la microfiltración cuando es usado como material de retroobturación medianteun sistema de transporte de fluidos computarizado. Metodología: En este estudio se utilizaron 60 raíces de dientes unirradiculares, se instrumentaron y obturaron con el sistema de cono único estandarizadodel sistema ProTaper (Dentsply, Maillefer, Suiza) con ultrasonido,utilizando el sellador de conductos radiculares Silco (Silva-Collyan SLP,México). Se hizo la preparación de la cavidad retrógrada con ultrasonidoutilizando la punta E31D del sistema Retro Kit (NSK Company, Japón),con un diámetro de 2 mm. Se llevó a cabo la retroobturación con cada uno de los cementos a evaluar; grupo 1: bturados con Biodentine™ (Septodont, Francia), grupo 2: obturados con MTA Gris (Angelus™, Londrina PR, Brasil), grupo 3: control. Se utilizó un software creado específi camente para evaluar microfi ltración; éste, por medio de sensores infrarrojos, detecta la variación de voltaje de una burbuja de aire dentro de un capilar. El programa expresa el tiempo que tarda la burbuja en pasar por el segundo sensor (mm/min), los cuales se convierten a μL/min, para después expresarlos en unidades de fi ltración, realizándosemediciones en un solo lapso. Resultados: La evaluación en este estudio de la microfi ltración apical de Biodentine™ y MTA Gris Angelus™, arrojó resultados favorables para ambos; se comprobó que no existe diferencia signifi cativa en cuanto a la microfiltración entre ellos; sin embargo, ambos materiales mostraron diferentes características en cuanto a su manipulación, tiempo de fraguado y porosidad. No se presentódiferencia estadísticamente significativa entre los grupos (p = 0.256) U Mann-Whitney y Kruskal Wallis. Conclusión: El Biodentine™ y TA Gris AngelusTM presentan un comportamiento óptimo para la obturación retrógrada. Se requiere realizar otros estudios en cuanto a la interfase con la estructura dentinaria.


Objective: To evaluate, using computational fl uid dynamics, the ability of BiodentineTM to resist microleakage when used as a retro-sealing material. Methodology: In this study, 60 single-rooted teeth were used, which were biomechanically prepared and fi lled using the standardized ProTaper single-cone technique (Dentsply, Maillefer, Switzerland) and ultrasound, using Silco root-canal sealer (Silva-Collyan, SLP, Mexico). The retrograde cavity was prepared using ultrasound and a Retro Kit (NSK Company, Japan) E31D 2 mm-diameter tip. Retrograde fi lling was performed using each of the sealers to be evaluated, as follows: group 1: sealed with BiodentineTM (Septodont, France); group 2: sealed with Grey MTA (AngelusTM, Londrina PR, Brazil); group 3: control. A specially designed software was used, which measures microleakage by using infrared sensors to detect changes in voltage in an air bubble within a capillary tube. The software expresses the time it takes (mm/min) for the bubble to pass through the second sensor, then converts these measurements into μL/min to express the rate of leakage. All of the measurements are taken considering a single interval. Results: The results of the evaluation of the apical microleakage of BiodentineTM and MTA Grey AngelusTM obtained in this study were, in both cases, favorable, proving that there is no signifi cant difference between the two in this respect. However, the characteristics of each in terms of their handling, setting time, and porosity differed. No statistically signifi cant difference between the Mann-Whitney U group and the Kruskal-Wallis group was found (p = 0.256). Conclusion: The performance of Bio-dentineTM and Gray MTA AngelusTM is ideal for retrograde fi lling pur-poses. Further studies are needed, including SEM analysis, in order to determine the quality of the seal, with respect to the interface formed with the dentin structure.

Resumen La fenestración apical sucede cuando el ápice de un diente está totalmente expuesto al medio ambiente oral; se considera una variación del rango normal periodontal por múltiples factores. Aunque sin síntomas, la presencia de fenestraciones puede complicar el tratamiento de endodoncia o poner en peligro un resultado funcional y estético de la cirugía periodontal. Esta condición no patológica debe tratarse con precaución ya que representa un desafío para el clínico; pues a menudo es mal diagnosticada y maltratada, por lo tanto, es necesario un enfoque multidisciplinar. Varios procedimientos quirúrgicos y no quirúrgicos se documentan para el tratamiento; incluso, cuando se diagnostica correctamente, su manejo con una sencilla intervención quirúrgica es suficiente. Se presenta el caso de una mujer de 42 años de edad que consulta al odontólogo por presentar raíz dental expuesta en encía del sector anterosuperior generando inconformidad estética. Se diagnosticó fenestración apical en la raíz del órgano dentario 23 debido a placas corticales delgadas y biotipo periodontal fino, la cual se trató de manera exitosa con tratamiento endodóntico convencional, en conjunto con cirugía apical. El control posoperatorio reveló la cobertura completa de la fenestración. Se describe el protocolo quirúrgico de la fenestración apical con resultados estéticos y funcionales satisfactorios, mostrando el procedimiento como una opción de tratamiento seguro y eficaz en estos casos inusuales.


Apical fenestration describes the situation where the apex of a tooth is exposed to the oral environment following breakdown of the overlying bone and alveolar mucosa; it is considered a variation within the range of periodontal normality. Although symptom free, the presence of fenestrations may complicate endodontic treatment or jeopardize the functional and aesthetic outcome of periodontal surgery. This non-pathological condition should be treated with caution because it represents a challenge to the clinician, because it is often misdiagnosed and mistreated and therefore, requires a multidisciplinary approach. Several surgical and nonsurgical procedures were documented for treatment, even when diagnosed properly, the management with a simple surgical procedure is sufficient. We report an unusual case of a 42 year old woman who consulted the dentist and referred an exposed tooth root in anterior gingival area with an aesthetic compromise. A diagnosis was made of an apical fenestration of the maxillary canine root at the apex with very thin buccal cortical plate and thin periodontal biotype, which was treated successfully by a combination of nonsurgical root canal treatment and apical surgery. Postoperative control revealed complete coverage of the fenestration. We describe The surgical protocol to correct the apical fenestration with satisfactory aesthetic and functional results is described, showing the procedure as a safe treatment option which is effective in these unusual cases.

Introduction: The Mineral Trioxide Aggregate (MTA) has excellent biological property. However, its consistency makes it difficult to be inserted into retrograde cavities. Objective:To evaluate the ability of different methods to fill retrograde cavities with MTA. Material and methods: Root canals of thirty single-rooted resin teeth were prepared and filled. After the cut of 3 mm short of apical third, retrograde cavities with 3 mm deep were prepared using an ultrasound device and retrotips (CVD, São José dos Campos, SP, Brazil). The retrograde preparation was evaluate by using an operative microscope (D.F. Vasconcellos, São Paulo, SP, Brazil). The teeth were randomly divided into three groups (n = 10), according to the method: 1) condenser (Trinity, São Paulo, SP, Brazil); 2) MTA applicator (Angelus, Londrina, Brazil) + condenser; 3) condenser associated with ultrasound (CVD, São José dos Campos, SP, Brazil). After the filling of retrograde cavities with white MTA (Angelus, Londrina, Brazil), teeth were radiographed using a digital system (Kodak RVG 6000, Rochester, NY, USA). The images were analyzed by UTHSCSA Image Tool 3.0 software. The percentage of filling was calculated by the proportion between the total area of retrograde cavity and the filled area. The radiographic density mean of each third of retrograde cavity filled with MTA was measured by using the histogram tool of the software. The results were submitted to ANOVA and Tukey tests, with 5% of significance. Results: There was no difference in percentage of filling among the groups (p > 0.05) (approximately 85%). By comparing the thirds, the condenser and MTA applicator groups showed higher density for apical and middle third than cervical third (p < 0.05). The ultrasound group presented similar density among the thirds. Conclusion: The filling ability was similar for the studied methods. Ultrasound promoted better distribution of MTA in retrograde cavity, but did not increase the density of material.

Introdução: A cirurgia paraendodôntica é um procedimento que tem como finalidade resolver problemas que não puderam ser solucionados pelo tratamento endodôntico convencional, ou quando este não é possível. Relato do caso e conclusão: Neste trabalho há o relato de um caso clínico de dentes portadores de coroas protéticas com pinos intra-radiculares, em que se optou pela realização do procedimento cirúrgico. Foram efetuadas apicectomia e retroobturação com MTA no dente 11, apicectomia no dente 21 e apicectomia, retroinstrumentação e retrobturação com MTA no dente 22.


Introduction: Paraendodontic surgery is a procedure that aims problems resolution that couldn’t be solved by the conventional endodontic treatment, or when the accomplishment conventional treatment is not possible. Case report and conclusion: The aim of this study was to report a clinical case where was made apicectomy on the teeth 11, 21 and 22. The tooth 22 was sealing of root-end cavity MTA retrofilling.

Objective:To discuss the effect of radiation of Nd:YAG laser in apical retrograde filling. Methods: 50 similar roots were randomly divided into 5 groups.The anatomical crowns were removed at the cementoenamel junction and the remaining roots were treated endodontically with conventional methods before the apical 2 mm were cut and a dish-like hole made in the cutting surface by high speed round bur. Except the first group,each hole were radiated by the Nd:YAG laser for different time or with different patterns and after that,the glass ionomer cement were retrofilled into the hole. A microleakage measurements were carried out and the result were measured and recorded. The hole surface was also observed under electron microscopy.
