ABSTRACT

Objective To study the impact of laparoscopic surgical intervention for severe acute pancreatitis on TNF alpha, IL - 6 and sIL-2R. Methods Take 92 patients with SAP accepted laparoscopic surgical intervention as the research object, they were divided into laparotomy group (46 cases): received traditional open surgery;laparoscopic group (46 cases): received laparoscopic surgery intervention. Then compare the levels of TNF alpha, IL-6 and sIL-2R between the two groups. Results The time of laparoscopy group was (82.21 ± 14.56) min, intraoperative blood loss was (172.23 ± 23.31) mL, total cost was (21512.46 ± 121.35) yuan, which was less than laparotomy group; the cure rate of laparoscopic group was 93.48% (43/46). It was obviously higher than that in laparotomy group 86.96% (40/46); The complication rate was 23.91% (11/46), the death rate 6.52% (3/46) of laparoscopic group were significantly lower than that in laparotomy group. After laparoscopic surgery, the level of TNF alpha (24.70 ± 6.90) ng/L, IL-6 (18.32 ± 8.91) ng/L and sIL-2R (98.60 ± 8.91) pmol/L was significantly lower than laparotomy group. All the differences compared between the two groups was statistical significant (P < 0.05). Conclusion The laparoscopic intervention for SAP has distinct curative effect. It can effectively reduce the rate of complications and mortality, and improve the cure rate, is deserving popularization and application.

ABSTRACT

Objective To investigate the effect of alprostadil injection on serum tumor necrosis factor ( TNF-α) ,β2-microglobulin (β2-MG) and soluble interleukin 2 receptor (sIL-2R) levels in adjunctive treatment of patients of severe hepatitis with liver and kidney syndrome.Methods A total of 60 patients of severe hepatitis with liver and kidney syndrome were collected and randomly divided into experimental group and control group with 30 cases in each group.The two groups were treated by conventional therapy, patients in the control group were treated by continuous renal replacement therapy, patients in the experimental group were treated on the basis of the control group with alprostadil injection in adjuvant therapy for 3 weeks.The serum biochemistry and liver and kidney function and clinical curative effect were detected.Results Compared with the control group, the serum TNF-α, β2-MG and sIL-2R levels of the experimental group were lower (P<0.05); alanine aminotransferase (ALT), aspartate aminotransferase (AST) and total bilirnbin (TBiL) levels were lower (P<0.05), albumin (Alb) level and prothrombin time activity (PTA) were higher (P<0.05), serum creatinine (Scr), urea nitrogen (BUN), CysC and 24h urine protein were lower (P<0.05), 24h urine volume was higher (P<0.05); efficacy in experimental group was better than control group (Z=-2.321,P=0.020).Conclusion Alprostadil injection in adjunctive treatment of severe hepatitis with liver and kidney syndrome patients has significant effect and high safety, could significantly reduce the serum TNF-α, β2-MG and sIL-2R levels.

ABSTRACT

Objective:To examine the plasma soluble interleukin 2 receptor (sIL-2R)and the expression of IFN-γR in helper T (Th)cells in peripheral blood of patients with recurrent oral ulcer(ROU).Methods:The peripheral blood was obtained from 21 pa-tients in active stage of ROU and 21 healthy controls.Flow cytometry was used to detect the quantity of IFN-γR in CD +4 T cells,ELISA was used to detect the level of sIL-2R.Results:The expression of IFN-γR in CD +4 T cells and sIL-2R content in plasma of the patients with ROU were significantly higher than those in the healthy controls(P <0.05).Conclusion:Over expression of IFN-γR in CD +4 T cells and sIL-2R in peripheral blood may be related to the genesis of ROU.

ABSTRACT

Objective To investigate folic acid combined with Qizhi Weitong granule on serum gastrin, sIL-2R and immune function in chronic atrophy gastritis patients of Hp positive.Methods 46 cases with Hp positive chronic atrophy gastritis were selected and randomly divided into two groups according to different treatment, 23 cases in each group.The control group were given folic acid tablet on the basis of conventional treatment, and the experimental group were treated on the basis of control group combined with Qizhi Weitong granule.The patients in two groups were all treated for three months.Serum gastrin, sIL-2R and T cell subsets levels were detected after treatment.ResuIts Compared with control group after treatment, the serum gastrin level of experimental group was higher (P<0.05), sIL-2R level was lower (P<0.05), CD4 +T, CD4 +T/CD8 +T were higher (P<0.05), and CD8 +T level was lower ( P<0.05 ) .ConcIusion Folic acid combined with Qizhi Weitong granule could significantly increase serum gastrin and CD4 +T level and reduce sIL-2R and CD8 +T level in chronic atrophy gastritis patients of Hp positive, as a guidance for clinical.

ABSTRACT

Objective To analyze the effect of TSH inhibition on soluble interleukin 2 receptor ( sIL-2R), interleukin 44 variant 6 (CD44V6), tumor specific growth factor (TSGF) and T lymphocyte subsets in elderly patients with thyroid cancer.Methods 50 elderly patients with thyroid cancer in our hospital were collected.The patients were randomly assigned to experimental and control groups, 25 cases in each group, patients in the experimental group were given TSH suppression therapy after surgery , patients in control group were given thyroxine replacement therapy after operation, treatment for 1 month, serum SIL-2R, CD44v6, TSGF and peripheral blood T lymphocyte subsets situation were detected in all patients with .ResuIts After treatment, compared with the control group, ①in the experimental group, the level of serum sIL-2R was significantly lower, the difference was statistically significant(P<0.05);②the serum CD44V6 level was lower in the experimental group (P<0.05);③the serum TSGF level was lower in the experimental group (P<0.05);③in the experimental group,the levels of CD3 + and CD4 +were higher, levels of CD8 + were lower ( P<0.05 ) .ConcIusions TSH suppression therapy can reduce senile thyroid cancer patients serum SIL-2R, CD44v6, TSGF and CD8 +lymphocyte level, peripheral blood T lymphocyte CD3 +, CD4 +levels were elevated,and improve the immune function of patients , the clinical has guiding significance .

ABSTRACT

Objective To study the influence of serum containing Fuzheng Toudu Qudu Recipe, a Chinese formula with the actions of supporting healthy qi to expel and remove toxicity, on serum levels of interleukin 2 (IL-2) and soluble interleukin 2 receptor (sIL-2R) at different stages of CD34+derived dendritic cells (DC) of patients with minimal residual disease of myelogenous leukemia ( MRD-L) , and to explore the biological mechanism of Fuzheng Toudu Qudu Recipe in promoting CD34+ to transform into DC in MRD-L patients. Methods Bone marrow mononuclear cells ( BMMC) were separated from the bone marrow of acute myeloid leukemia patients at complete remission stage by using Ficoll centrifugation. CD34+ cells were isolated by using immuno-magnetic mircobeads method, and then were cultured with various concentrations of Chinese medicine medicated serum and cytokines in vitro for the induction of DC. The morphologic characteristics of DC were observed with the inverted phase contrast microscope, and the expression levels of DC surface molecules such as CD83, CD80, CD86, CD1a and HLA-DR were detected by using flow cytometry. On culturing day 0, 6 and 9, serum levels of IL-2 and sIL-2R of each group were measured by enzyme-linked immunosorbent assay ( ELISA). Results ( 1) Chinese medicine medicated serum combined with cytokines was effective on promoting CD34+ to differentiate into DC with typical morphology, and inducing DC to have high expression of CD80, CD83, CD86 and HLA-DR, which differed from those in fetal calf serum (FCS) group and blank rabbit serum group (P0.05) . At the same time point, combination groups had lower IL-2 content than the blank rabbit serum group (P<0.05 or P<0.01). Conclusion Fuzheng Toudu Qudu Recipe is effective on increasing serum content of IL-2 and reducing sIL-2R content, and the changes of cytokine contents are more obvious along with the maturity of DC, which indicates that the recipe plays positive effect in the process of promoting CD34+cells to differentiate into DC.

Objective To investigate the effects of sIL-2R in plasma on regulatory T cells (Treg) in children with Kawasaki disease ( KD ) .Methods Thirty-three children with KD and fourteen age-matched healthy children were enrolled in this study .The proportions of CD4+CD25+Foxp3+Treg cells in pe-ripheral blood and mean fluorescence intensity (MFI) of phosphorylated-STAT5 (pSTAT5) protein in CD4+CD25+T cells were analyzed by flow cytometry .The concentrations of sIL-2R, IL-2, IL-7 and IL-15 in plas-ma were measured by cytometric bead array ( CBA ) .Real-time PCR was performed to detect the gene ex-pressions of Foxp3, GITR, CTLA4, IL-2Rα, IL-2Rβand IL-2Rγat mRNA level as well as the expressions of IL-17A and ROR-γt at mRNA level in CD4+CD25-T cells.Results (1) Compared with the control group, the proportions of CD4+CD25+Foxp3+Treg cells in peripheral blood from patients with KD and the expressions of associated factors including Foxp 3, GITR and CTLA-4 at mRNA level were significantly down-regulated (P0.05).(4)The expressions of IL-2Rαand IL-2Rβat mRNA level in CD4+CD25+T cells from patients with acute KD were lower than those of the healthy subjects (P0.05).The concentrations of sIL-2R in plasma were negatively correlated with the expressions of IL-2Rβand Foxp3 at mRNA level and pSTAT5 at protein level (P<0.05).The expression of pSTAT5 protein had positive correlation with the ex-pression of Foxp3 at mRNA level (P<0.05).Conclusion Aberrant IL-2/STAT5 signaling pathway media-ted by significantly increased concentration of sIL-2R in plasma might be one of the factors leading to down-regulation of Treg cells in patients with acute KD .

ABSTRACT

Levels of soluble interleukin-2 receptor alpha (sIL-2Ralpha) are known to increase in the sera of patients with certain malignancies, including malignant lymphoma. This study aimed to assess the clinical significance of the sIL-2Ralpha level in non-Hodgkin's lymphoma (NHL). We used ELISA to measure the sIL-2Ralpha levels in 48 newly diagnosed and untreated patients with NHL and evaluated the correlation between the sIL-2Ralpha levels and clinical characteristics and the International Prognostic Index (IPI). We monitored serum sIL-2Ralpha in 7 patients to compare the changes in their clinical progress with these levels. High levels of serum sIL-2Ralpha (> or =2,000 U/mL) correlated well with parameters defining the high risk group according to the IPI, i.e., high tumor burden at diagnosis (stage III+IV) and lactate dehydrogenase > or =472 U/L. The levels were also associated with B symptoms, bone marrow involvement, and poor response to therapy. The sIL-2Ralpha level decreased during complete remission and was elevated during disease progression or relapse. A high level of sIL-2Ralpha was significantly associated with a low survival rate. These results suggest that serum sIL-2Ralpha might be useful as a biomarker for evaluating the prognosis of patients with NHL at the time of diagnosis and during therapy. A well-controlled, large-scale study is needed to clarify the clinical significance of sIL-2Ralpha in specific groups of NHL.

ABSTRACT

Objective To study the expression of interleukin-2 (IL-2), soluble interleukin-2 receptor (sIL-2R),determine the alteration of erythrocytic immunity and T cell subgroup in the blood of outer circulation in patients with hypertensive cerebral hemorrhage so and to probe into the relationship between them, and to explore the clinical significance. Methods Enzyme linked immnunosorbent assay (ELISA) was used to determine the content of IL-2 and sIL-2R. The immunoadsorption was employed to examine the erythrocytic immune activity and its regulating function.Streptavidin-peroxidase(S-P) was used to determine the cell number of CD3 (cluster of differentiation3), CD4 and CD8. Results The content of IL-2 in the group with hypertensive cerebral hemorrhage was significantly lower than that in the control group (P<0.01), and the content of sIL-2R increased. Red blood cell C3b receptor (RBC. C3b R)and RBC immune adherence enhancing factor (RFEB) dropped greatly (P<0.01), while RBC immune complex rosette (RBC. ICR) and RBC immune adherence inhibiting factor (RFIR) increased greatly. The cell number of CD3 and CD4decreased (P<0.01) and there was no obvious change in CD8 (P<0.05). Conclusion The decrease of immune function was observed in patients with hypertensive cerebral hemorrhage. The determination of the content of IL-2, sIL-2R, erythrocytic immunity and the activity of T subgroup has an important clinical significance in the occurrence,development, treatment, and prognosis of hypertensive cerebral hemorrhage.

ABSTRACT

Objective To study the effects of Xiaofeng Powder on IL-4,IFN-? and sIL-2R in delayed hypersensitivity and to explore the mechanism of treatment. Methods The mice model of allergic contact dermatitis was formed by DNCB. The effect of Xiaofeng Powder on ear swelling and leukocyte counts of the model mice were observed. The serum levels of interleukin-4 (IL-4),interferon-gamma (IFN-?) and soluble interleukin-2 receptor (sIL-2R) of the mice were detected by enzyme-linked immunosorbent assay (ELISA). Results Xiaofeng Powder could inhibit ear swellings and obviously reduced the total amount of leucocytes. Xiaofeng Powder could heighten IL-4 level and reduce the level of IFN-?,sIL-2R. Conclusion Xiaofeng Powder has the function of inhibiting delayed hypersensitivity. The mechanism of treatment may be related to inhibiting the total amount of leucocytes,regulating cell factor and receptor.

@#ObjectiveTo investigate the immune tolerance function and significance of allogene bone marrow injection to the small intestines transplantation of rats.MethodsInbreeding line rat F344/N and Wistar/A were selected to perform heterotopic graft of the whole small intestine. 7 days before allogene transplantation, donator bone marrow cells (BMC) were injected into thymus of acceptor (the testing group). According to the isogene and allogene rat transplant model, it was comprehended whether injecting allogene donator marrow into acceptor thymus could decrease the acute rejection after transplantation.Results3, 5 or 7 days after allogeneic rat dystopia whole small intestine transplantation, typical reject reaction appeared, but there was no reject reaction in isogenome and testing group. 3 days after allotransplantation, serum soluble interleukin-2 receptor (sIL-2R) and tumor necrotic factor-α (TNF-α) levels were significantly higher than the other groups (P<0.01). The level of serum sIL-2R and TNF-α in the allogene marrow injecting group were only slight higher on the 3rd or 5th day, and getting downtrend, and there was no significant difference compared with isogenic transplantation group.ConclusionAllogenic donator bone marrow intrathymic injecting into acceptor 7 days before small intestina transplantation, can reduce the reject reaction after the grafting. The levels of serum sIL-2R and TNF-α can be selected as a sensitive early diagnosis index of acute rejection after small intestine transplantation.

ABSTRACT

BACKGROUND: The cell-mediated immune response plays an important role in tuberculosis. After being activated by mycobacterial antigens, T lymphocytes express a high affinity receptor (IL-2R) for interleukin-2 (IL-2) on their own surface and release a soluble fraction of the IL-2 receptor (sIL-2R) from the cell membrane into the circulation. Neopterin is a metabolite of guanosine-triphosphate, which is produced by stimulated macrophages under the influence of IFN-gamma with a T lymphocyte origin. Therefore, the utility of sIL-2R, IFN-gamma and the neopterin levels as immunologic indices of the cell-mediated immune response and severity of disease in patients with pulmonary tuberculosis was assessed. METHOD: The serum sIL-2R, IFN-gamma and neopterin levels were measured in 39 patients with pulmonary tuberculosis, 6 patients with tuberculous lymphadenitis prior to treatment and 10 healthy subjects. The serum and pleural sIL-2R, neopterin and ADA levels were measured in 22 patients with tuberculous pleurisy. The patients with pulmonary tuberculosis were divided into a mild, moderate and severe group according to the severity by ATS guidelines. To compare the results from these patients with those of the pretreatment levels, the sIL-2R, IFN-gamma and neopterin levels were measured in 36 of the 39 patients(1 patient, expired; 2 patients were referred to a sanitarium) with pulmonary tuberculosis after 2 months of treatment. RESULTS: 1) The serum sIL-2R and IFN-gamma levels were elevated in patients with tuberculosis when compared to those of healthy subjects (0.05). The neopterin concentration in the serum was significantly lower in patients with pulmonary tuberculosis(2967+/-2132.8 pg/ml) than in healthy controls(4949+/-1242.1 pg/ml)(p0.05), 41 52.8 pg/ml to 22+/-23.9 pg/ml(p<0.05), respectively, after 2 month of treatment. The mean serum neopterin levels increased from 3158+/-2272.6 pg/ml to 3737+/-2307.5 pg/ml(0.05) after a 2 month of treatment. These findings were remarkable in the severe group of pulmonary tuberculosis with a clinical correlation. 4) In the patients with tuberculous pleurisy, the serum sIL-2R and ADA were significantly higher than those in the pleural fluid, However, the neopterin levels in the sera and pleural effusion were similar. CONCLUSION: On the basis of this study, sIL-2R, IFN-gamma and neopterin measurements may not only provide an insight into the present state of the cell-mediated immune response, but also serve as parameters monitoring of the prognosis of the disease, particularly in patients with severe pulmonary tuberculosis. In addition, an assay of the pleural sIL-2R levels might signal a stimulated local immunity including T cell activation in the tuberculous pleural effusion.

Objective:Effect of serum IL 2R on immunofunction were studied.Methods:Serum IL 2R and lymphocytic competence to PHA,IL 2,NK activity,LAK activity,subsets of T lymphocytes and delayed skin supersensitization were tested in four groups,including 30 normal controls,16 athletes,50 patients with lung cancer and 50 malignant lymphomas.The correlation between the tested immunofunction in each group were also studied.Results:In normal population,no correlations were found.Malignancies with high level of sIL 2R were negtively correlated with various immunofunctions,such ascytologic competence to IL 2,NK activity,LAK aitivity and number of CD 3.No significant correlation was found in cytologic competence to PHA and ratio of CD 4 and CD 8.Conclusion:sIL 2R is an important blocking factor for immunofunction,but under the interaction between various cytokines in the immunoregulatory network,its blockege is different with strong or week competence.

Object To study the change of serum soluble interleukin 2 receptor(SIL-2R) in syphilitic patients and its clinical significance.Methods The serum SIL-2R level in syphilitic patients of different stages and after therapy were detected using the ELISA technique.Results The serum SIL-2R level in syphiliti patients was higher than that in control group(P0 05).Conclusions SIL-2R level and its variation have some reference value for evaluating the therapeutic effect and judgement of the disease course in syphilitic patients.

Objective:To study the levels of soluble interleukin-2 receptor(sIL-2R) in patients with head and neck cancer.Methods:Using the method of sandwich ELISA, the sIL-2R in 60 cases of patients with head and neck cancer, 60 with benign tumor and 50 of healthy subjects were measured.Results:The concentration(U/ml) of serum sIL-2R in patients with head and neck cancer was significantly higher than that in those with benign tumor(571.9?294.2 vs 305.9?104.8, P0.05).Conclusion:sIL-2R may be related to head and neck cancer.

BACKGROUND: A soluble form of interleukin-2 receptor (sIL-2R) is released from activated T cells. Serum sIL-2R levels are elevated in some hematological malignancies and could be used to assess disease activity and prognosis. MATERIAL AND METHODS: To define clinical usefulness and significance as a marker predicting disease progress in chronic myeloproliferative disorders, the serum levels of sIL-2R were measured in 40 cases of chronic myelogenous leukemia (CML; 25 chronic phase, 7 accelerating phase, 8 blastic phase), 3 cases of polycythemia vera (PV), 5 cases of essential thrombocythemia (ET) and 4 cases of idiopathic myelofibrosis (MF) and in 37 cases of healthy subjects using sandwich enzyme immunoassay. RESULTS: Serum sIL-2R levels in the patients of CML, PV, ET, and MF were higher compared with the normal healthy controls. In CML, serum sIL-2R levels in the patients of blastic and accelerating phases were significantly higher than those of chronic phase. In CML of chronic phase, serum sIL-2R levels at diagnosis were related to WBC count but not to other clinical and hematologic paramaters. The leukemic cells of one patient with lymphoblastic phase of CML expressed IL-2R (CD25). Among 4 patients of CML with sIL-2R levels above 2,000U/mL at diagnosis, transformation to blastic crisis was noted in 3 patients and 2 patients died within 1 year after diagnosis. But among 11 patients of CML with sIL-2R levels below 2,000U/mL at diagnosis, only 2 patients experienced blastic crisis and died within 1 year after diagnosis. CONCLUSION: This study indicated that serum sIL-2R levels were high in chronic myeloproliferative disorders, and that increasing levels of serum sIL-2R might be useful to predict disease progress. Further studies including more patients and longer follow-up may substantiate serum sIL-2R as a prognostic indicator in CML.

ABSTRACT

Interleukin-5 (IL-5), which is known to be an activator of human eosinophil, increases in IgA nephropathy. In order to find out the relationship between activated eosinophil function and the pathogenesis of Henoch-Sch nlein purpura (HSP) and IgA nephropathy, serum esosinophil cationic protein (ECP) was analyzed using a monoclonal antibody Besides, the soluble IL-2 receptor (sIL-2R) was analyzed to clarify if there was a positive correlation between T cells and activated eosinophils. As anticipated, the levels of ECP, in detail, were significantly higher among HSP patients with a mean of 9.7+/-1.8microgram/L than in a control group with a mean of 4.6+/-0.7microgram/L. The HSP patients were also classified as one group with normal urine and another group with abnormal urine. The latter showed higher levels of ECP than the former. On the other hand, the levels of ECP were higher in IgA nephropathy patients than in the control group; however, there was no significance in statistics. The sIL-2R levels were higher in HSP patients than those in serums of IgA nephropathy patients and the control group. Thus, this study came to a conclusion that the activated eosinophil might be one of the pathogeneses in HSP but not in IgA nephropathy.

ABSTRACT

BACKGROUND: It was recently shown that human atherosclerotic plaque contains large numbers of T lymphocytes : this indicates that immune and inflammatory mechanism may be important factors in the pathogenesis of atherosclerosis. By measuring the soluble interleukin 2 receptor(sIL-2R) level we can evaluate the activation of T lymphocyte. The purpose of this study is to evaluate relationship between T cell activation and ischemic heart disease by measuring the soluble interleukin 2 receptor (sIL-2R) level in patient with ischemic heart disease. METHOD: Seventy-two patients(40 males and 32 female, mean age : 56.5+/-9.9 years) who were taken coronary angiography were included in this study. Among them, 49 patients showed abnormal coronary angiographic findings and 23 patients showed normal coronary angiographic findings. Ten mililiters of arterial blood was drawn at the time of coronary angiography. The blood was allowed to coagulate and then the serum was removed and tested in duplicate for soluble interleukin 2 receptor (sIL-2R) level by ELISA. RESULTS: 1) The soluble interleukin 2 receptor (sIL-2R) level was significantly different between abnormal coronary angiographic findings and normal coronary angiographic findings (P 0.05), but showed increasing tendency with clinical severity. 3) According to numbers of involved coronary vessels, soluble interleukin 2 receptor (sIL-2R) level was not significantly different between single vessel disease group and multivessels disease groups (p > 0.05). CONCLUSION: T lymphocyte activation, as reflected in elevated soluble interlekin 2 receptor (sIL-2R) level, is frequent in patient with ischemic heart disease. In the further we will investigate relationship between clinical diagnosis of ischemic heart disease of the numbers of involved coronary vessels and T cell activation.
