ABSTRACT

Le syndrome de Kartagener est une maladie génétique qui se caractérise par une association de situs inversus, de polypose naso-sinusienne et dilatation des bronches. Ce syndrome est découvert le plus souvent dans l'enfance, mais aussi à l'âge adulte. Nous rapportons une observation médicale d'une patiente atteinte de ce syndrome révélé à l'âge adulte suite à la recherche étiologique des dilatations des bronches. A travers cette observation les auteurs soulignent la rareté de ce syndrome comme cause de dilatation des bronches dans leurs pratiques quotidiennes

ABSTRACT

Primary ciliary dyskinesia [PCD] is an autosomal disorder that includes Kartagner's syndrome, immotile cilia syndrome, ciliary dysmotility and primary orientation defects. It is characterized clinically by sinusitis, otitis media, bronchiectasis, male infertility and often dextrocardia. The present work represents a .comparative quantitative study of the respiratory ciliary mucosa in PCD [6 cases], secondary ciliary dyskinesia [SCD] [6 cases] and 12 cases of normal non-inflamed mucosa as a control group. The light microscopy, the ultrastructure constituents and the deviation angle of the cilia were studied. There was no difference by light microscopy while the differences on the ciliary constituents were statistically insignificant [p>0.005]. It was found that the mean ciliary deviation angle in both the control group and the SCD were less than 30 while in PCD it was 43. The pattern of the histogram can aid in the diagnosis. The interpretation of the different ultrastructural findings should be associated with the deviation angle, along with the history of sinusitis, otitis media and respiratory tract infection since birth

ABSTRACT

Situs Inversus, a rare congenital anatomical abnormality although is not directly related to Sudden Death. However, 3-5% of people with Situs Inversus have associated functional heart defect & 25% have an underlying condition i.e. Primary Ciliary Dyskinesis presenting as Kartagener's Syndrome; are prone to Sudden Death. Two cases of Situs Inversus on which medico-legal autopsy was conducted are reported. In first case, death was attributed to cardiac component (massive cardiomegaly, narrowing of coronaries and an old, healed apical scar) and in the second, due to respiratory complications (infection of lungs with areas of pus formation) with Blood Alcohol Concentration as 241.50 mg % ; being another significant finding. Apart from discussing the various aspects of the condition, vital medico-legal issues varying from 'Negligence' to 'Assessment for Insurance purpose' are being discussed.

Kartagener descreveu uma associação entre dextrocardia, sinusite e bronquiectasia, que leva o seu nome. Sabe-se que esta síndrome completa é a forma extrema em gravidade da doença dos cílios imóveis, causada por defeitos ciliares em diferentes partes do organismo. Os autores relatam um caso desta síndrome e discutem os aspectos clínicos, diagnósticos e terapêuticos, bem como a necessidade de uma ampla avaliação de pacientes que se apresentem com qualquer sintoma isolado, para possibilitar um diagnóstico precoce

ABSTRACT

O objetivo do trabalho foi estudar o comportamento da depuraçäo mucociliar nasal, utilizando o teste da sacarina, em um grupo de pacientes com síndrome de discinesia ciliar (SDC), e verificar a utilidade do teste no seu diagnóstico. Foram estudados 54 indivíduos, 24 do sexo masculino e 30 do feminino, com idade compreendida entre 9 e 56 anos, divididos da seguinte forma: 18 pacientes com SDC, 9 normais (N), 9 com rinite alérgica (RA), 9 com sinusite aguda (SA) e 9 com bronquiectasias e sinusite mas sem SDC (BS). O teste foi realizado colocando-se um fragmento de comprimido de sacarina, com cerca de 1mm de diâmetro, sobre os cornetos inferiores de ambas as fossas nasais e mediando-se o tempo necessário para que o indivíduo percebesse seu sabor, até um máximo de 60 minutos. Dezesseis pacientes com SDC näo referiram o sabor da sacarina ao cabo de 60 minutos e dois o sentiram aos 18 e 20 minutos, respectivamente. Os seguintes tempos foram registrados nos outros grupos: N = 5,0 mais ou menos 2,1 min., RA = 7,8 mais ou menos 3,1 min., SA = 17,3 mais ou menos 6,5 min., e BS = 19,7 mais ou menos 6,9 min. Houve diferença significativa entre os pacientes com SDC e os demais grupos (p<0,01 para SA e BS e p<0,001 para N e RA), com sensibilidade de 89 por cento, especificidade de 100 por cento e valor preditivo positivo de cem por cento para o teste diagnosticar pacientes com SDC. Em conclusäo, o teste de sacarina, como proposto por Andersen e cols. (1), pode ser mais um elemento no diagnóstico de pacientes com a síndrome; entretanto, devemos estar alerta para o fato de que alguns desses pacientes podem ter tempo inferior a 60 minutos
