ABSTRACT

Acute mastoiditis with osteitis is secondary to bacteria involving mastoid cavities. Describe the epidemiologic aspects. Between 1990 and 2003, we listed 44 files of children hospitalized for acute mastoiditis. The treatment was in any case medical associated to a surgical treatment in 30 cases. The age of our patients was consisted between 2 months and 14 years with an average is 2 years. The sex ratio of patient was 1.1. 29 patients didn't have any antecedents particular O.R.L, 8 patients were followed for otitis middle chronic and 7 had an cholesteatomateuse. The auricular old-fashioned swelling was the most frequent functional sign. A scanner has been achieved in 13 cases. The treatment was medico-surgical in 30 cases is 68.8% and an only antibiotherapie in 14 cases is 31.8%

ABSTRACT

A 14 year old girl, with a past history of chronic otitis media was presented with otogenic cerebellar abscess. CAT scan finding, and initial management of the patient are described. Radical Mastoidectomy was performed for the drainage of the abscess which was complemented with antibiotics and metronidazol therapy. A second operation for covering cerebellar hernia was performed. The outcome of the whole procedure was good the patient was-discharged after a hospital stay of 48 days

ABSTRACT

Se presenta la experiencia de nuestro hospital en el manejo de la otomastoiditis crónica colesteatomatosa en el periodo de 1985 a 1995 mediante el estudio retrospectivo de 160 pacientes. Setenta y tres pacientes (84 oídos) cumplieron los criterios de inclusión, a quienes se les practicaron 107 cirugías. Se realizó mastoidectomía radical en el 77.5 por ciento de los casos, cirugía conservadora en el 18.7 por ciento y a un pequeño porcentaje 3.73 por ciento, cirugía de revisión o funcional. El control de la enfermedad fue mejor en el grupo de cirugía radical, con una recidiva de 13 por ciento, mientras que en procedimientos conservadores fue el 61 por ciento. Dentro de las causas de fracaso de mastoidectomía radical se encontró un meato insuficiente en el 57 por ciento, muro alto en el 43 por ciento y celdillas residuales en el 43 por ciento. Consideramos que la mastoidectomía radical (clásica o modificada) ofrece mejor control de la enfermedad, sin embargo, cada caso debe de individualizarse.

ABSTRACT

Thirty six cases of acute mastoiditis were treated to Federal Government Services Hospital, Islamabad over a period of 11 years [from 1986- up to 1997]. The salient features in these cases was presence of extensive Cholesteatoma in the middle ear cleft. The affected children belonged to poor communities, they were malnourished and attended hospital in a state of emergency. The presentation and course of the disease confirms the aggressive and potentially serious nature of the pathology, requiring early attention, in order to avert impending complications. Significantly all patients had a history of ear discharge prior to abscess formation. It appears that in our environment, most mastoiditis with abscess formation develop from chronic otitis media with Cholesteatoma

Se presentan los resultados del análisis de sexo, edad, lado infectado, diagnóstico definitivo, técnica quirúrgica utilizada, cirugías previas ipsi y contralaterales y complicaciones. Se discute el tema y se revisa la literatura respectiva
